Objective

To explore the mediating role of activities of daily living (ADL) and depression between chronic disease comorbidity and self-rated health in empty-nest elderly, and to provide a reference for improving the self-rated health and quality of life of empty-nest elderly.

Methods

A total of 3 542 empty-nest elderly aged 60 years and older from the 2020 China Health and Retirement Longitudinal Study were selected as study subjects. Spearman’s correlation analysis was employed to explore the associations between chronic disease comorbidity, ADL, depression, and self-rated health among empty-nest elderly. The Process 4.1 plugin model 6 in SPSS was used to analyse the mediating effects of ADL and depression on the relationship between chronic disease comorbidity and self-rated health among empty-nest elderly.

Results

The direct effect of chronic disease comorbidity on self-rated health among empty-nest elderly individuals was statistically significant (β=-1.2397, 95% CI: -1.4139~-1.0655), the separate mediating effect of ADL was statistically significant (β=-0.159, 95% CI: -0.278~-0.079), the separate mediating effect of depression was statistically significant (β=-0.160, 95% CI: -0.214~-0.115), and the chain mediating effect of ADL and depression was statistically significant (β=-0.073, 95% CI:-0.094~-0.056).

Conclusion

Chronic disease comorbidity can directly negatively impact the self-rated health status of empty-nest elderly individuals. It may also influence their self-rated health through the independent or chain mediating effects of activities of ADL and depression. Consequently, it is recommended that policy measures prioritise empty-nest elderly individuals within health interventions to reduce their risk of chronic disease comorbidity. This should involve establishing a comprehensive support system centred on functional assessment and psychological intervention, thereby improving the self-rated health status of this demographic.

目的

探讨日常生活活动能力（activities of daily living，ADL）和抑郁在慢性病共病和空巢老人自评健康间的中介作用。为改善空巢老人自评健康状况，提高其生活质量提供参考。

方法

选取2020年中国健康与养老追踪调查中3 542例≥60岁的空巢老人作为研究对象，运用Spearman相关分析法探究慢性病共病、ADL、抑郁和空巢老人自评健康的相关性，使用SPSS中Process 4.1插件模型6分析ADL、抑郁在慢性病共病和空巢老人自评健康间的中介作用。

结果

慢性病共病对空巢老人自评健康的直接效应具有统计学意义（β=-1.240，95% CI：-1.414～-1.066），ADL的单独中介效应具有统计学意义（β=-0.159，95% CI：-0.278～-0.079），抑郁的单独中介效应具有统计学意义（β=-0.160，95% CI：-0.214～-0.115），ADL和抑郁的链式中介效应具有统计学意义（β=-0.073，95% CI：-0.094～-0.056）。

结论

慢性病共病可以直接负向影响空巢老人自评健康状况，还可以通过ADL和抑郁的单独或链式中介作用影响空巢老人自评健康情况。因此，建议政策层面将空巢老人纳入健康干预的优先序列，降低患慢性病共病风险，构建以功能评估与心理干预为核心的综合支持体系，进而改善空巢老人自评健康状况。

目的

探讨日常生活活动能力（activities of daily living，ADL）和抑郁在慢性病共病和空巢老人自评健康间的中介作用。为改善空巢老人自评健康状况，提高其生活质量提供参考。

方法

选取2020年中国健康与养老追踪调查中3 542例≥60岁的空巢老人作为研究对象，运用Spearman相关分析法探究慢性病共病、ADL、抑郁和空巢老人自评健康的相关性，使用SPSS中Process 4.1插件模型6分析ADL、抑郁在慢性病共病和空巢老人自评健康间的中介作用。

结果

慢性病共病对空巢老人自评健康的直接效应具有统计学意义（β=-1.240，95% CI：-1.414～-1.066），ADL的单独中介效应具有统计学意义（β=-0.159，95% CI：-0.278～-0.079），抑郁的单独中介效应具有统计学意义（β=-0.160，95% CI：-0.214～-0.115），ADL和抑郁的链式中介效应具有统计学意义（β=-0.073，95% CI：-0.094～-0.056）。

结论

慢性病共病可以直接负向影响空巢老人自评健康状况，还可以通过ADL和抑郁的单独或链式中介作用影响空巢老人自评健康情况。因此，建议政策层面将空巢老人纳入健康干预的优先序列，降低患慢性病共病风险，构建以功能评估与心理干预为核心的综合支持体系，进而改善空巢老人自评健康状况。

Objective

To explore the mediating role of activities of daily living (ADL) and depression between chronic disease comorbidity and self-rated health in empty-nest elderly, and to provide a reference for improving the self-rated health and quality of life of empty-nest elderly.

Methods

A total of 3 542 empty-nest elderly aged 60 years and older from the 2020 China Health and Retirement Longitudinal Study were selected as study subjects. Spearman’s correlation analysis was employed to explore the associations between chronic disease comorbidity, ADL, depression, and self-rated health among empty-nest elderly. The Process 4.1 plugin model 6 in SPSS was used to analyse the mediating effects of ADL and depression on the relationship between chronic disease comorbidity and self-rated health among empty-nest elderly.

Results

The direct effect of chronic disease comorbidity on self-rated health among empty-nest elderly individuals was statistically significant (β=-1.2397, 95% CI: -1.4139~-1.0655), the separate mediating effect of ADL was statistically significant (β=-0.159, 95% CI: -0.278~-0.079), the separate mediating effect of depression was statistically significant (β=-0.160, 95% CI: -0.214~-0.115), and the chain mediating effect of ADL and depression was statistically significant (β=-0.073, 95% CI:-0.094~-0.056).

Conclusion

Chronic disease comorbidity can directly negatively impact the self-rated health status of empty-nest elderly individuals. It may also influence their self-rated health through the independent or chain mediating effects of activities of ADL and depression. Consequently, it is recommended that policy measures prioritise empty-nest elderly individuals within health interventions to reduce their risk of chronic disease comorbidity. This should involve establishing a comprehensive support system centred on functional assessment and psychological intervention, thereby improving the self-rated health status of this demographic.

空巢老人指的是60岁以上，没有子女照顾或子女长期不在身边的独居老年人或者仅与配偶居住的老年人[1]。随着城市化、工业化进程的加快，子女异地就业现象突出，空巢老人群体规模不断增加，使得空巢家庭成为我国家庭结构的重要形态[2]。由于家庭支持系统的相对缺乏，空巢老人更容易感到孤独，导致他们的健康受损程度明显高于普通老年人[3]。慢性病共病是指同时患有两种或两种以上慢性非传染性疾病[4]。已有研究表明，空巢老人普遍存在着较高的慢性病共病比例，患有慢性病共病的空巢老人往往拥有较差的健康状况[5]，导致其生活质量降低[6]。然而，空巢老人是老年人群中的特殊群体，长期处于孤独环境中，缺乏家庭支持，更容易产生抑郁情绪[7]。日常生活活动能力（activities of daily living，ADL）是评估个体进行日常活动自理能力的重要指标。已有研究认为，慢性病共病会降低空巢老人的ADL，进而影响空巢老人的自评健康状况[8]。抑郁是影响空巢老人健康的危险因素。慢性病共病能够引发老年人抑郁情绪，从而影响到空巢老人主观健康状况[9]。基于此，本文利用2020年中国健康与养老追踪调查（China Health and Retirement Longitudinal Study，CHARLS）数据，探讨ADL和抑郁在慢性病共病和空巢老人自评健康间的中介作用，为提高空巢老人生活质量提供对策建议。
1　对象与方法

1.1　研究对象
本研究数据采用最新公布的2020年CHARLS。该调查涵盖国内28个省的150个县（区），450个社区（村）。涉及家庭情况、健康状况、生活方式及养老退休等多方面内容。本研究关注空巢老人的自评健康状况，纳入标准：（1）年龄≥60岁；（2）去年与子女同居时间<180 d[10]；（3）关键变量中未有无效数据及空缺值。最终纳入样本量3 542人。
1.2　变量选择
1.2.1　自变量：是否患有慢性病共病
慢性病患病情况通过询问受访者是否曾被医生诊断为高血压、血脂异常、糖尿病、癌症等14种慢性病来确定。若受访者同时患有两种或以上慢性病，则将其样本记为1，其他样本记为0。
1.2.2　因变量：自评健康
因变量为我国空巢老人的自评健康状况。根据“您认为您的健康状况怎样？是很好、好、一般、不好、还是很不好”这一问题的回答进行变量选择，并参考鲜国炜等人[11]的分类方法，将自评健康状况进行变量赋值（0=不健康、1=健康）。
1.2.3　中介变量：ADL和抑郁
ADL是评估和研究老年人能力的重要标准以及身体健康状况指标。本研究采用CHARLS问卷中的ADL量表，包括基础性ADL和工具性ADL二个分量表。每个条目根据完成情况分为四级，得分范围0～36，计算ADL得分，分数越高代表ADL越好。此研究中，量表Cronbach α系数为0.864。
此外，使用CHARLS问卷中的CESD-10评定抑郁，共包括10个条目，采用四级评分法，评估个体在过去一周内的情绪状态。条目包括感到沮丧、睡眠不好等，得分≥10分作为有无抑郁症状的临界，总分范围0～30，分数越高，抑郁程度越严重。在此研究中，量表的Cronbach α系数为0.792。
1.2.4　控制变量
参照以往研究[10]，将性别、年龄、婚姻状态、医疗保险、养老保险、是否上网、是否吸烟、是否饮酒列为控制变量，见表1。
1.3　统计学方法
运用SPSS 26.0软件进行数据清洗和统计分析。首先，采用χ2检验对数据进行单因素分析，并通过相关分析探讨慢性病共病、ADL和抑郁与自评健康之间的关系。此外，使用SPSS宏程序PROCESS v4.1对样本进行显著性检验，采用非参数百分位bootstrap法分析ADL和抑郁在慢性病共病与自评健康之间的链式中介效应。检验水准α=0.05。
2　结果

2.1　研究对象基本情况
3 542名≥60岁的空巢老人中，男性1 892人（53.4%）；有配偶2 958人（83.5%）；吸烟、饮酒各有1 712（48.3%）和1 244人（35.1%）。单因素分析结果表明，性别、是否上网、是否饮酒与空巢老人的自评健康之间有统计学意义（P<0.05）。见表2。
2.2　相关性分析
相关分析结果显示，慢性病共病与ADL（r=-0.229，P<0.001）、自评健康（r=-0.302，P<0.001）呈负相关，与抑郁程度呈正相关（r=0.194，P<0.001）。ADL和抑郁程度（r=-0.395，P<0.001）呈负相关，与自评健康（r=0.228，P<0.001）呈正相关。抑郁程度和自评健康（r=-0.265，P<0.001）呈负相关。见表3。
2.3　链式中介模型分析
在描述性分析和相关性分析结果的基础上，将年龄、婚姻、性别、吸烟、饮酒纳为控制变量，构建一个以ADL、抑郁为中介变量的链式中介模型，进一步探讨慢性病共病对空巢老人自评健康的影响。结果显示，慢性病共病能对ADL和自评健康产生负向影响，同时能正向影响空巢老人抑郁程度；ADL不仅能对空巢老人自评健康产生正向影响，还能负向影响空巢老人抑郁程度；此外，抑郁能对空巢老人自评健康产生负向影响。见表4、图1。
2.4　链式中介路径分析
结果显示，慢性病共病对空巢老人自评健康的直接效应有统计学意义（β=-1.240，95%CI：-1.414～-1.066），ADL和抑郁在慢性病共病与空巢老人自评健康的中介效应有统计学意义（β=-0.159，95% CI：-0.278～-0.079；β=-0.160，95% CI：-0.214～-0.115）。ADL和抑郁在慢性病共病与空巢老人自评健康的链式中介效应有统计学意义（β=-0.073，95% CI：-0.094～-0.056）。见表5。

本研究旨在探讨ADL和抑郁在慢性病共病与空巢老人自评健康之间的链式中介作用。研究结果显示，慢性病共病不仅对空巢老人的自评健康有直接的负向影响，亦通过ADL受损、抑郁以及“ADL受损—抑郁”的链式路径对空巢老人的自评健康产生间接影响。
3.1　慢性病共病对空巢老人自评健康的直接影响
本研究结果显示，慢性病共病能直接负向影响空巢老人的自评健康状况。研究结果与李岩静等[3]学者一致。相比于多代同堂家庭、非空巢以及城乡混合的老年人，空巢老人缺乏子女照料和精神支持，对于健康和医疗的需求更加显著[12]。当自身健康状况出现变化时，缺乏相应的应对能力和疾病管理资源，进而使疾病进一步恶化或新症状的出现，导致空巢老人慢性病共病的患病率上升，对自身的健康状况的看法更为悲观[13]。
3.2　ADL、抑郁对空巢老人自评健康的间接影响
本研究的核心发现揭示了这一影响背后的两条关键中介路径。首先，研究结果显示，慢性病共病能够通过ADL影响空巢老人自评健康。与Koroukian SM[14]等学者研究结果一致。有研究表明，慢性病共病能导致空巢老人ADL受损[15]，慢性病共病患者的ADL失能风险是未患慢性病人群的5.61倍[16]。然而，ADL受损的空巢老人往往拥有较差的自评健康状况。究其原因，ADL受损使空巢老人在独立生活中遇到更多困难，从而增加了空巢老人的孤独感和无助感，降低其自评健康状况[17]。因此，ADL的中介作用通过影响生活质量和心理状态，间接影响了空巢老人对自身健康的自我感知。
其次，抑郁是连接躯体疾病与主观健康感知的另一条重要路径。慢性病共病能够通过抑郁影响空巢老人自评健康状况，研究结果与孟娣娟等[18]学者一致。已有研究发现，抑郁情绪越高的空巢老人生命质量越差[12]，进而影响空巢老人自评健康。抑郁症状直接导致空巢老人情绪低落、悲观和无助，这种负面情绪使他们对自身健康的评价变得更加消极，表现为身体不适时的过度担忧，认为自己的健康状况比实际情况更差，从而影响自我健康的评估。
3.3　ADL和抑郁的链式中介作用
尤为重要的是，以往相关研究只集中于两到三个变量，缺乏四个变量之间的研究。本研究验证了空巢老人“慢性病共病→ADL→抑郁→自评健康”这一链式中介路径。ADL和抑郁在慢性病共病对空巢老人自评健康状况的影响中发挥链式中介作用，ADL能够负向影响老年人抑郁状况。具体来说，空巢老年人日常活动能力下降，自主性减弱，会导致老年人出现负面情绪，产生“习得性无助感”,抑郁情绪进一步恶化[19]，导致老年人出现抑郁症状。这可能是因为空巢老人在执行基本生活活动时面临困难，这种功能丧失导致他们感到无助和失落，从而影响自我效能感[20]。
本研究构建的链式中介模型清晰地揭示了慢性病共病导致自评健康下降的一条重要传导路径。然而，值得注意的是，现实中的变量关系并非静态的单向线性，而是呈现出复杂的双向因果和反馈循环，可能将老年人推入健康状况不断“螺旋式下降”的困境。这提示本研究的链式路径可能处于一个动态循环中，干预应从“破环”入手，而非仅限于单点控制。本研究仍存在一些局限性，第一，本研究为横断面研究，无法准确把握变量之间的因果关系，难以进行因果推断；第二，数据调查受主观因素等影响，变量测量存在限制。
基于上述结论，建议政策层面将空巢老人纳入健康干预的优先序列，构建以功能评估与心理干预为核心的综合支持体系。具体说来：（1）应依托基层卫生服务机构建立“ADL+抑郁”联合评估机制，定期筛查功能受损和心理异常的高风险个体，并将其纳入家庭医生签约重点管理范围；（2）推动社区康复服务常态化，为空巢老人提供可及的体能训练与康复指导。同时，在老年健康体检项目中增设功能状态和心理评估模块，推动从单一疾病治疗转向功能维护与情绪支持并重的健康治理模式。（3）社会支持政策应以“赋能”为导向，鼓励发展推动老年人主动生活和社会参与的服务模式，避免“照料型”支持可能导致的依赖性；（4）资源配置上应优先保障空巢老年人心理咨询、康复训练、助行辅助器具等公共服务覆盖，推动构建可持续的老龄友好型社区环境。
