Objective

To investigate the awareness rate and influencing factor of weight management knowledge, and the prevalence of abnormal body mass index (BMI) among junior high school students in Yinchuan, analyze the correlation between weight management knowledge awareness and abnormal BMI, and provide a theoretical basis for the implementation of weight-related health education among children and adolescents.

Methods

Between March 12 and April 26, 2025, a multi-stage stratified cluster random sampling method was employed to recruit 2 485 students from Grade 7 to Grade 9 across 9 junior high schools in Yinchuan as study participants. A self-developed structured questionnaire was administered to assess participants’awareness of comprehensive weight management knowledge (including nutrition, physical activity, etc.) and its associated influencing factors. Abnormal BMI was screened using a bioelectrical impedance analysis (BIA) body composition analyzer. In accordance with the national standards Screening for Malnutrition in School-age Children and Adolescents (WS/T456-2014)and Screening for Overweight and Obesity in School-age Children and Adolescents (WS/T586-2018) issued by the National Health and Family Planning Commission of China, students with emaciation, overweight, or obesity were classified by gender and age.

Results

The overall awareness rate of weight management knowledge among junior high school students in Yinchuan was 57.34%. The highest awareness rate was observed in the dimension of “prevention cognition” (51.75%), while the lowest was in “scientific weight control” (35.57%). No statistically significant difference was detected across different grades (P>0.05),whereas the awareness rate among female students was significantly higher than that among male students (P<0.05).Multivariable logistic regression analysis revealed that female gender (OR=0.53), Grade 9 enrollment (OR=1.21), urban residency (OR=0.78), being an only child (OR=1.31), parental education level of college or above (OR=1.18), cohabitation with parents (OR=2.21), normal guardian weight (OR=0.84), school-based health education implementation (OR=1.33), and teacher-delivered weight management knowledge (OR=2.61) were significant influencing factors of weight management knowledge awareness among junior high school students (all P<0.05). The prevalence of abnormal BMI among the study population was 29.86%, with emaciation, overweight, and obesity accounting for 8.09%, 14.29%, and 7.48% of the total sample, respectively.The awareness rate of weight management knowledge was significantly negatively correlated with emaciation (r=-0.712),overweight (r=-0.723), and obesity (r=-0.756), with all P<0.05.

Conclusion

The overall awareness rate of weight management knowledge among junior high school students in Yinchuan is relatively low, while the prevalence of abnormal BMI is notably high. A significant negative correlation exists between weight management knowledge awareness and abnormal BMI, which warrants serious attention.

目的

调查银川市初中生体重管理知识知晓率、影响因素及身体质量指数（body mass index, BMI）异常现状，分析体重管理知识知晓率与BMI异常相关性，为儿童青少年体重健康教育开展提供帮助。

方法

2025年3月12日—4月26日，采用多阶段分层整群随机抽样法，抽取银川市9所初中的2 485名初一至初三学生为研究对象，使用自行设计问卷调查保持健康体重相关的营养、运动等综合知识的知晓情况及影响因素；采用电动人体成分分析仪筛查BMI异常。参照国家卫生和计划生育委员会发布的《学龄儿童青少年营养不良筛查》（WS/T456—2014）、《学龄儿童青少年超重与肥胖筛查》（WS/T586—2018）标准，确定不同性别和年龄的消瘦、超重、肥胖学生。

结果

银川市初中生体重管理知识总体知晓率为57.34%，“预防认知”方面知晓率最高，为51.75%，“科学控重”方面知晓率最低，为35.57%；不同年级间无统计学意义（P>0.05），不同性别间表现出女生高于男生，且具有统计学意义（P<0.05）。多因素logistic回归分析显示，女生（OR=0.53）、初三年级（OR=1.21）、城市学生（OR=0.78）、独生子女（OR=1.31）、父母学历大专及以上（OR=1.18）、与父母同住（OR=2.21）、监护人体重正常（OR=0.84）、学校开展健康教育（OR=1.33）、教师讲授体重管理知识（OR=2.61）是影响初中生体重管理知识知晓率的相关因素（P<0.05）。银川市初中生BMI异常人数占总调查人数的29.86%，消瘦、超重、肥胖人数分别占总人数的8.09%、14.29%、7.48%。初中生体重管理知识知晓率与BMI消瘦、超重、肥胖组均存在负相关性（r=-0.712、-0.723、-0.756，P值均<0.05）。

结论

银川市初中生体重管理知识总体知晓率较低，BMI异常率较高；初中生体重管理知识知晓率与BMI异常存在负相关性，需要引起高度重视。

目的

调查银川市初中生体重管理知识知晓率、影响因素及身体质量指数（body mass index, BMI）异常现状，分析体重管理知识知晓率与BMI异常相关性，为儿童青少年体重健康教育开展提供帮助。

方法

2025年3月12日—4月26日，采用多阶段分层整群随机抽样法，抽取银川市9所初中的2 485名初一至初三学生为研究对象，使用自行设计问卷调查保持健康体重相关的营养、运动等综合知识的知晓情况及影响因素；采用电动人体成分分析仪筛查BMI异常。参照国家卫生和计划生育委员会发布的《学龄儿童青少年营养不良筛查》（WS/T456—2014）、《学龄儿童青少年超重与肥胖筛查》（WS/T586—2018）标准，确定不同性别和年龄的消瘦、超重、肥胖学生。

结果

银川市初中生体重管理知识总体知晓率为57.34%，“预防认知”方面知晓率最高，为51.75%，“科学控重”方面知晓率最低，为35.57%；不同年级间无统计学意义（P>0.05），不同性别间表现出女生高于男生，且具有统计学意义（P<0.05）。多因素logistic回归分析显示，女生（OR=0.53）、初三年级（OR=1.21）、城市学生（OR=0.78）、独生子女（OR=1.31）、父母学历大专及以上（OR=1.18）、与父母同住（OR=2.21）、监护人体重正常（OR=0.84）、学校开展健康教育（OR=1.33）、教师讲授体重管理知识（OR=2.61）是影响初中生体重管理知识知晓率的相关因素（P<0.05）。银川市初中生BMI异常人数占总调查人数的29.86%，消瘦、超重、肥胖人数分别占总人数的8.09%、14.29%、7.48%。初中生体重管理知识知晓率与BMI消瘦、超重、肥胖组均存在负相关性（r=-0.712、-0.723、-0.756，P值均<0.05）。

结论

银川市初中生体重管理知识总体知晓率较低，BMI异常率较高；初中生体重管理知识知晓率与BMI异常存在负相关性，需要引起高度重视。

Objective

To investigate the awareness rate and influencing factor of weight management knowledge, and the prevalence of abnormal body mass index (BMI) among junior high school students in Yinchuan, analyze the correlation between weight management knowledge awareness and abnormal BMI, and provide a theoretical basis for the implementation of weight-related health education among children and adolescents.

Methods

Between March 12 and April 26, 2025, a multi-stage stratified cluster random sampling method was employed to recruit 2 485 students from Grade 7 to Grade 9 across 9 junior high schools in Yinchuan as study participants. A self-developed structured questionnaire was administered to assess participants’awareness of comprehensive weight management knowledge (including nutrition, physical activity, etc.) and its associated influencing factors. Abnormal BMI was screened using a bioelectrical impedance analysis (BIA) body composition analyzer. In accordance with the national standards Screening for Malnutrition in School-age Children and Adolescents (WS/T456-2014)and Screening for Overweight and Obesity in School-age Children and Adolescents (WS/T586-2018) issued by the National Health and Family Planning Commission of China, students with emaciation, overweight, or obesity were classified by gender and age.

Results

The overall awareness rate of weight management knowledge among junior high school students in Yinchuan was 57.34%. The highest awareness rate was observed in the dimension of “prevention cognition” (51.75%), while the lowest was in “scientific weight control” (35.57%). No statistically significant difference was detected across different grades (P>0.05),whereas the awareness rate among female students was significantly higher than that among male students (P<0.05).Multivariable logistic regression analysis revealed that female gender (OR=0.53), Grade 9 enrollment (OR=1.21), urban residency (OR=0.78), being an only child (OR=1.31), parental education level of college or above (OR=1.18), cohabitation with parents (OR=2.21), normal guardian weight (OR=0.84), school-based health education implementation (OR=1.33), and teacher-delivered weight management knowledge (OR=2.61) were significant influencing factors of weight management knowledge awareness among junior high school students (all P<0.05). The prevalence of abnormal BMI among the study population was 29.86%, with emaciation, overweight, and obesity accounting for 8.09%, 14.29%, and 7.48% of the total sample, respectively.The awareness rate of weight management knowledge was significantly negatively correlated with emaciation (r=-0.712),overweight (r=-0.723), and obesity (r=-0.756), with all P<0.05.

Conclusion

The overall awareness rate of weight management knowledge among junior high school students in Yinchuan is relatively low, while the prevalence of abnormal BMI is notably high. A significant negative correlation exists between weight management knowledge awareness and abnormal BMI, which warrants serious attention.

儿童青少年是祖国未来的建设者和接班人，他们的体重健康关乎自身的发展。中小学时期是身心发展的重要阶段，这一时期掌握体重健康知识、养成良好体重健康行为习惯，不仅直接影响中小学生的学习和生活，还会对他们成年后的体重健康产生影响。当前我国6～17岁儿童青少年中超重和肥胖率将近20%[1]，部分地区甚至更高[2],这为慢性疾病、身体姿态异常[3-4]等埋下了隐患。诸多研究认为，中小学生体重管理知识欠缺是影响他们体重增加的原因之一[5-6]。为了解银川市初中生身体质量指数（body mass index, BMI）及体重健康管理知识知晓现状及其影响因素，本研究抽取银川市9所中学的初中生进行调查分析，希冀为儿童青少年开展体重健康教育和预防提供理论与实践依据。
1　对象与方法

1.1　对象
2025年3月12日—4月26日，采用多阶段分层整群随机抽样法，在银川市教育局的帮助下，运用抽签法在市所辖的3个区每区随机抽取3所公办初中（初一至初三年级），共计9所初中；每区学校按年级分层，每年级采用抽签法抽取2个班，共抽取54个班级，以班级为单位共有2 524名初一至初三学生参与调查。剔除39名数据不完整学生，有效人数2 485名，有效率98.45%，其中男生1 264名，女生1 221名；初一811名，初二822名，初三852名。本研究通过了宁夏医科大学伦理委员会批准（2025-G017）,所有调查对象及监护人知情并签署了自愿参与同意书。
1.2　方法
参考《学生健康检查技术规范》（GB/T26343—2010）[7]标准，利用法国Terraillon品牌Tpro6300电动人体成分分析仪，测量BMI值，BMI=体重(kg)/身高的二次方(m)2。参照国家卫生和计划生育委员会发布的《学龄儿童青少年营养不良筛查》（WS/T456—2014）[8]、《学龄儿童青少年超重与肥胖筛查》（WS/T586—2018）标准[9]，确定不同性别和年龄的消瘦、超重、肥胖学生。初一至初三男/女生标准分别为：消瘦≤15.9/15.3、16.4/16.0和16.9/16.6 kg/m2，超重≥21.4/22.2、22.3/22.8和22.9/23.2 kg/m2，肥胖≥25.2/25.0、26.1/25.9和26.6/26.6 kg/m2。
依照国家卫生健康委印发的居民体重管理核心知识（2024版）及释义[10]，参考学生健康素养知识调查及影响因素等研究[11]，自行设计初中生体重管理知识知晓及影响因素调查问卷。问卷内容包括调查对象基本情况、体重管理知识、影响因素三部分，第一部分包括：性别、年龄、年级、生源、身高、体重等；第二部分围绕体重管理知识八个方面：预防认知、终生管理、监测评估、膳食营养、合理运动、睡眠心理、科学控重、全家行动，设置40项相关内容均为判断题，调查学生知晓情况；第三部分影响因素包括：家庭情况（父母学历（二者中最高学历）、居住情况、过去1周零花钱数量、监护人体重（1人即算，同住并日常陪伴最多者）、学校教育情况（学校开展健康教育、教师讲授体重管理知识）。采用KMO检验、Bartlett检验及探索性因子分析验证问卷结构与预设维度的一致性，采用Cronbach α系数评估各维度及整体问卷的条目同质性；通过重测法检验问卷信度，随机选取30名初中生在间隔15 d内进行二轮重复调查，计算两次调查结果相关系数。
1.3　质量控制
调查开始前，对所有调查人员进行BMI测查和问卷调查培训并考核，经考核后开展实际调查。问卷调查正式开始前，调查员说明调查目的、意义和主要内容；由预防专业在读研究生担任调查员，按照统一指导语解读，组织学生以班级为单位在教室自填匿名问卷，并当场回收。问卷回收时，对问卷缺填、错填进行核查；问卷双录入保障数据准确性。
1.4　统计分析
利用excel软件进行数据整理，包括学生体重管理知识个体知晓、不同组别知晓率、总体知晓率，学生个体体重知识参考相关研究以正确回答总题数（40项）的60%（≥24项）及以上视为知晓良好[12]，不同组别知晓率=该组别知晓学生数/总学生数×100%，总体知晓率=知晓学生总数/总学生数×100%。运用美国开发的SPSS 22.0软件进行统计分析，对不同性别、年级等学生BMI异常率、体重管理知识不同组别知晓率进行χ2检验，对体重知识管理影响因素进行单因素和多因素logistic回归分析，对BMI异常组别与体重管理知识知晓率进行相关性分析，检验水准α=0.05。
2　结果

2.1　调查问卷的信效度
问卷经检验KMO=0.86，Bartlett检验为χ2=3 862.45，v=780，P<0.001，探索性因子分析显示8个公因子的累积方差贡献率为68.32%，各条目在对应公因子上的载荷量均在0.62～0.89；Cronbach α系数为0.89，各维度的Cronbach α系数在0.79～0.92，表明问卷效度可靠。问卷间隔15 d内进行二轮重复调查，相关系数为0.82，证实问卷信度可靠。
2.2　银川市初中生体重管理知识知晓现状
银川市初中生体重管理知识总体知晓率为57.34%，且不同年级间无统计学意义（P>0.05），不同性别间具有统计学意义（P<0.05）。单项体重管理知识中，“预防认知”知晓率最高，为51.75%，其次为“终生管理”，为44.10%，“科学控重”知晓率最低，为35.57%；各单项体重管理知识知晓率随年级升高略有提高，但均无统计学意义（P>0.05）；各单项体重管理知识知晓率均表现出女生高于男生，且均具有统计学意义（P<0.05）。见表1。
2.3　银川市初中生体重管理知识知晓单因素影响分析
银川市初中生体重管理知识知晓率与BMI、性别、年级、学生生源、是否独生子、父母学历、居住情况、监护人体重、学校开展健康教育（体育课外）、教师讲授体重管理知识有关，差异具有统计学意义（P<0.05）；与过去一周零花钱数量无关，差异无统计学意义（P>0.05）。见表2。
2.4　银川市初中生体重管理知识知晓因素logistic回归分析
以银川市初中生体重管理知识知晓与否为因变量（0=否，1=是），将单因素分析有统计学意义的因素［BMI异常：1=消瘦，2=肥胖，3=超重；性别：1=男生，2=女生；年级：1=初一，2=初二，3=初三；学生生源：1=城市，2=乡镇；是否独生子：1=是，2=否；父母学历：1=大专及以上，2=高中及以下；居住情况：1=与父母住,2=与(外)祖父母住，3=住校；监护人体重（1人即算）：1=偏胖，2=正常，3=偏瘦；学校开展健康教育(体育课外)：1=有,2=无；教师讲授体重管理知识：1=有,2=无］作为自变量纳入logistic回归分析，从而发现影响因素。结果表明，BMI异常、女生、初三年级、城市学生、独生子女、父母学历大专及以上、与父母同住、监护人体重正常、学校开展健康教育、教师讲授体重管理知识是影响初中生体重管理知识知晓的重要因素，且差异具有统计学意义（P<0.05）。见表3。
2.5　银川市初中生BMI异常现状
银川市初一至初三学生BMI异常（消瘦、超重、肥胖）总人数为742人，占总人数的29.86%，消瘦、超重、肥胖人数分别占总人数的8.09%、14.29%、7.48%。除初一年级男、女生肥胖人数占比不具有统计学意义（P>0.05）外，各年级BMI异常男、女生占比均表现出男生<女生，且具有统计学意义（P<0.05）。初一至初三各年级BMI异常人数合计占比中，除初二超重人群外其余均表现出随年级升高占比下降趋势，且不同年级间具有统计学意义（P<0.05）。见表4。
2.6　银川市初中生BMI异常与体重管理知识知晓相关性
银川市初中生体重管理知识知晓率与BMI异常组别相关分析结果显示，体重管理知识知晓率与消瘦、超重、肥胖人群均存在负相关性（r=-0.712、-0.723、-0.756，P<0.05），即初中生体重管理知识知晓率低的人群出现BMI异常率高。见表5。
3　讨论

本研究显示，银川市初中生体重管理知识总体知晓率为57.34%，原因在于体重管理知识更加具体、专业，学生对此了解不够深入。同时，家庭对他们体重管理知识的知晓也有影响，父母受教育水平较高的孩子体重异常的可能性较低；另外，学校开展体重管理教育也会影响学生体育管理知识的知晓[13]。学生“预防认知”的知晓率最高，说明学生对进行体重管理具有较好认知。对于“科学控重”的知晓率最低，原因在于此方面知识需根据消瘦、超重、肥胖不同个体差异区别开展，并且初中《体育与健康》教材较少涉及不同BMI的科学控重知识，故学生对此知晓率不高。其他几方面知晓率在36.02%～44.10%，反映出银川市在初中生体重管理知识的宣传、教育还有不足，需加强重视。各单项体重管理知识的知晓率均表现出女生高于男生，且均具有统计学意义，与Carson H[14]的研究结果一致，原因是初中女生进入青春期，对塑造自身良好形象的知识关注更多。
本研究显示，银川市初中生体重管理知识知晓率与学生个人、家庭、学校教育情况有关。（1）个人情况包括女生、初三、城市生源、独生子女四方面。女生高于男生在于其更重视美好体态，注重体重管理知识不足的危害[15]。初三高于另两个年级与Khanal[16]的研究结果一致，原因是他们面临体育中考，对于体重管理知识相应在意。城市生源学生高于乡镇与Caihui H[17]的研究结果一致，城市在互联网、社区宣传等方面具有优势，学生接触体重管理知识更加便捷，知晓率易高于乡镇学生。独生子女高于非独生子女与Yang[18]研究结果一致，主要因为独生子女受家庭关注更多，获得的课外体育投资可能更多，进而获得了相应知识。（2）家庭情况包括父母学历、居住情况、监护人体重三方面。父母学历为大专及以上学生知晓率更高与Liu[19]的调查结果一致，高学历父母对于体重异常的危害认知可能更全面，会传输给孩子体重控制知识，这从本研究“与父母居住”学生体重管理知识好于“与（外）祖父母居住”和“住校”的学生结果也得到了证实。监护人体重正常学生知晓率更高，说明家长以身作则对孩子体重管理知识具有影响。（3）学校教育情况包括学校开设健康教育、教师讲授体重管理知识两方面。这两种方式有助于学生掌握体重管理知识，研究显示健康教育教师对学生健康知识知晓率有正向作用，体育教师对学生健康行为达标率的正向作用高于健康教育教师[20]。
本研究显示，银川市初中生BMI异常人数占总人数的29.86%，消瘦、超重、肥胖人数分别占总人数的8.09%、14.29%、7.48%，与体重管理知识知晓存在负相关性。研究发现健康、营养等知识的知晓与中学生消瘦、超重、肥胖具有关联[21]；一项对希腊学生的调查发现，健康素养知识得分与儿童体重指数呈负相关［回归系数（95% CI）：-0.010（-0.018,-0.001）］[22]；一项对法国高中生的研究表明[23]，身体健康素养知识与脂肪质量百分比呈显著负相关（r=-0.43，P≤0.01）；还有一项对伊朗大学生的研究显示，体重健康认知与体重管理显著相关（β=0.41,P<0.001），是预测大学生BMI的显著变量[24]；本研究与这些研究结果相一致。
根据本研究结果，建议学校体育与健康课程加大体重健康管理知识的传授，尤其是BMI评价方法和标准，不同类型和强度运动控制体重的方法等，提高学生体重管理知识专业深度。家庭要提高体重管理与相关知识相关性的认识，及时掌握孩子体重变化，共同行动保持健康体重。社会做好体重管理知识宣传，提供学生更广泛接触体重管理知识的渠道。此外，针对消瘦学生要在午晚餐后2 h左右，增加一顿高营养密度餐；积极参与俯卧撑、篮球等抗阻或综合性运动，刺激肌肉生长。针对超重、肥胖中学生，“戒断”含糖饮料，每天累积60 min中高强度活动，如上下学快走/骑车20 min，课间休息、体育课认真运动、避免久坐等。本文存在一定的局限与不足：学生BMI仅分为消瘦、超重、肥胖组，每组下未能再细分年级、生源、是否独生子女等组别；体重管理知识未能涉及更多心理、营养、运动等内容；学生体重管理知识影响因素没有涉及社会宣传、家庭收入，监护人学历、体重信息未全员收集和精准确定等。
