Objective

To analyze the current status of perceived risk of recurrence among patients with first-time upper gastrointestinal bleeding due to liver cirrhosis and to explore the impact of risk perception on bleeding recurrence within one year.

Methods

Patients hospitalized at a tertiary hospital from December 2023 to April 2024 with first-time upper gastrointestinal bleeding due to liver cirrhosis were enrolled. Baseline data and scores from the Perceived Risk of Recurrence Scale were collected. Logistic regression analyzed the effect of perceived risk levels on 1-year recurrence. ROC curves assessed the predictive value of risk perception for 1-year recurrence, and decision curve analysis evaluated its practical utility.

Results

A total of 221 patients was enrolled, with 47 experiencing rebleeding during one-year follow-up. The risk perception score was 72.0 (65.0, 90.5). Logistic regression indicated that, after adjusting for confounding factors, risk perception was an independent protective factor against rebleeding. The AUC for predicting one-year rebleeding using risk perception was 0.847 (95% CI:0.779-0.915). The net benefit ratio for predicting 1-year bleeding recurrence risk was 0.03-0.80.

Conclusion

Patients with cirrhosis experiencing their first upper gastrointestinal bleeding event exhibit a low to moderate perception of recurrence risk, and this risk perception is significantly associated with bleeding recurrence.

目的

分析肝硬化首次上消化道出血患者出血复发风险感知现状，探讨风险感知对1年内出血复发的影响。

方法

选取2023年12月—2024年4月某三甲医院住院的肝硬化首次上消化道出血患者为研究对象，收集基线资料和复发风险感知量表得分，采用logistic回归分析复发风险感知水平对1年内出血复发的影响，采用ROC曲线分析风险感知对1年内出血复发的评估价值，以决策曲线分析法分析预测1年内出血复发的实用价值。

结果

共纳入221名患者，随访一年内有47名患者出现再出血。风险感知得分为72.0（65.0，90.5），logistic回归显示，校正混杂因素后，风险感知是出血复发的独立保护因素。风险感知预测1年内出血复发的AUC为0.847（95% CI:0.779～0.915）。风险感知预测1年内出血复发风险的净获益率为0.03～0.80。

结论

肝硬化首次上消化道出血患者出血复发风险感知处于中低水平，风险感知与出血复发存在显著关联。

目的

分析肝硬化首次上消化道出血患者出血复发风险感知现状，探讨风险感知对1年内出血复发的影响。

方法

选取2023年12月—2024年4月某三甲医院住院的肝硬化首次上消化道出血患者为研究对象，收集基线资料和复发风险感知量表得分，采用logistic回归分析复发风险感知水平对1年内出血复发的影响，采用ROC曲线分析风险感知对1年内出血复发的评估价值，以决策曲线分析法分析预测1年内出血复发的实用价值。

结果

共纳入221名患者，随访一年内有47名患者出现再出血。风险感知得分为72.0（65.0，90.5），logistic回归显示，校正混杂因素后，风险感知是出血复发的独立保护因素。风险感知预测1年内出血复发的AUC为0.847（95% CI:0.779～0.915）。风险感知预测1年内出血复发风险的净获益率为0.03～0.80。

结论

肝硬化首次上消化道出血患者出血复发风险感知处于中低水平，风险感知与出血复发存在显著关联。

Objective

To analyze the current status of perceived risk of recurrence among patients with first-time upper gastrointestinal bleeding due to liver cirrhosis and to explore the impact of risk perception on bleeding recurrence within one year.

Methods

Patients hospitalized at a tertiary hospital from December 2023 to April 2024 with first-time upper gastrointestinal bleeding due to liver cirrhosis were enrolled. Baseline data and scores from the Perceived Risk of Recurrence Scale were collected. Logistic regression analyzed the effect of perceived risk levels on 1-year recurrence. ROC curves assessed the predictive value of risk perception for 1-year recurrence, and decision curve analysis evaluated its practical utility.

Results

A total of 221 patients was enrolled, with 47 experiencing rebleeding during one-year follow-up. The risk perception score was 72.0 (65.0, 90.5). Logistic regression indicated that, after adjusting for confounding factors, risk perception was an independent protective factor against rebleeding. The AUC for predicting one-year rebleeding using risk perception was 0.847 (95% CI:0.779-0.915). The net benefit ratio for predicting 1-year bleeding recurrence risk was 0.03-0.80.

Conclusion

Patients with cirrhosis experiencing their first upper gastrointestinal bleeding event exhibit a low to moderate perception of recurrence risk, and this risk perception is significantly associated with bleeding recurrence.

肝硬化（Liver Cirrhosis, LC）是一种临床常见的慢性肝脏疾病，其病理特征包括肝细胞广泛坏死、弥漫性纤维组织增生和假小叶形成[1]。流行病学数据显示，全球每年约100万人死于肝硬化[2]，其相关并发症导致的死亡人数约130万[3]。上消化道出血（Upper gastrointestinal bleeding，UGIB）是最常见且严重的并发症之一，具有高复发率和死亡率的特点。研究表明，复发性出血的发生率每年高达60%，死亡率高达30%[4]。反复出血不仅会加速肝功能恶化，严重影响预后，还会给患者带来沉重的心理和经济负担。复发风险感知是指个体对疾病复发预警特征、风险因素、可能性及严重性的感知[5]。健康信念模型[6]也强调了患者自身对疾病威胁感知的重要性。目前我国疾病复发风险感知研究处于发展阶段，且大多数研究集中于脑卒中[7]和冠心病[8]群体开展。本研究拟探讨肝硬化首次UGIB患者的出血复发风险感知现状，分析其对再出血的预测价值，并据此制定针对性预防措施，以期降低再出血率，提高患者的生存周期。
1　资料与方法

1.1　研究对象
选取肝硬化首次上消化道出血患者为研究对象，于病情稳定期向患者解释说明并取得知情同意，自患者出院起随访一年，根据一年内是否发生再出血分为出血未复发组和出血复发组。纳入标准：①符合肝硬化诊治指南标准[9]，且首次发生上消化道出血的患者；②年龄>18周岁；③具有正常的理解与沟通力，能够完成简单书面书写；④知情同意并自愿参与本研究。排除标准：①合并恶性肿瘤或其他严重器质性疾病；②中途自愿退出无法完成调查者。研究参考多因素分析样本量估计方法，样本量取自变量个数的15～20倍[10]。本研究预计进行回归分析时纳入10个因素，因此样本量应为150～200例，考虑到20%的失访，拟定样本量为180～250例，最终纳入研究样本量为221例。本研究已通过本院伦理委员会审批（2023YX140）。
1.2　调查工具
1.2.1　一般资料调查表
采用自制的一般资料调查表，其中社会人口学资料包括性别、年龄、文化程度、职业状况、婚姻状况、居住地、家庭月均收入、医疗费用支付方式；饮食习惯相关资料包括饮食口味与饮食方式；疾病相关资料包括脂肪肝程度、肝功能分级、疾病诊断类型。
1.2.2　肝硬化患者上消化道出血复发风险感知量表
该量表由我国学者朱新颖编制[11]，用于评估肝硬化患者出血复发风险感知能力，包括复发可能性感知、复发后果严重性感知、复发危险因素感知和自我效能感知4个维度，共32个条目。其中，复发可能性感知维度（2个条目）、复发后果严重性感知维度（6个条目）、复发危险因素感知维度(13个条目)、自我效能感知维度(11个条目)。采用Likert 5级评分法，复发可能性感知维度从“完全不可能”到“完全可能”，复发后果严重性感知和复发危险因素感知维度从“完全不同意”到“完全同意”，自我效能感知维度从“完全不能”到“完全能”，分别赋1～5分，后3个维度得分越高表明患者对上消化道出血复发风险感知越高。量表总体Cronbach α系数为0.957。
1.3　资料收集
于患者入院病情稳定期收集基线资料。资料收集人员要进行专门的培训，避免收集时因表达方式存在差异等问题造成问卷填写不准确。所有问卷原则上由对象自主完成，对于存在书写、阅读或理解方面有困难的对象，由资料收集人员逐项朗读问卷内容，并依据对象的口述回答来代为填写问卷，最终收集的资料由双人核对录入。
1.4　统计学方法
运用 SPSS 29.0软件进行数据的统计分析，非正态分布的计量资料采用M(P25,P75)描述，组间比较采用Mann-Whitney U检验。计数资料采用频数和构成比（%）表示，组间比较采用卡方检验。使用R 4.3.2软件绘制受试者工作特征曲线（ROC曲线）并计算曲线下面积（AUC），以评估风险感知对1年内出血复发的预测效能，使用Delong检验对不同AUC进行Z检验比较。此外，绘制决策曲线（DCA曲线）评估风险感知在不同阈值概率下的临床净获益率，量化其临床适用性。检验假设取α=0.05，以P<0.05 时差异有统计学意义。
2　结果

2.1　调查对象基本情况
共纳入221名患者，其中女性88人，男性133人，年龄<60岁131人，≥60岁90人，随访一年患者出血未复发组174例，出血复发组47例，其余一般资料见表1。与出血未复发组相比，出血复发组在年龄、文化程度、家庭月均收入、居住地、饮食口味、饮食方式、肝功能分级以及出血复发风险感知方面展现出显著的统计学差异（P<0.05）。
2.2　肝硬化首次UGIB患者风险感知与1年内出血复发现状
本研究所纳入的研究对象风险感知均分为72.0（65.0，90.5）,处于中低水平，将风险感知以四分位数分组:Q1组<65.0,Q2组65.0～72.0,Q3组72.0～90.0,Q4组>90.0。风险感知Q1组有52人、Q2组有51人、Q3组有62人、Q4组有56人。
1年内出血总复发率为21.27%（47/221），Q1-Q4组出血复发率分别为44.23%(23/52)、29.41%(15/51)、7.69%(5/62)和7.14%(4/56),差异有统计学意义（χ2=31.523, P<0.001）。
2.3　肝硬化首次UGIB患者1年内出血复发的影响因素分析
将1年内是否发生再出血作为因变量(出血未复发=0，出血复发=1)，将年龄、文化程度、居住地、家庭月均收入、饮食口味、饮食方式、肝功能分级、复发风险感知总分差异有统计学意义的指标作为自变量构建logistic回归模型。分类变量赋值方式如下年龄：0=<60岁，1=≥60岁；文化程度：0=小学及以下，1=初中，2=高中或中专，3=大专及以上；居住地：0=农村，1=城镇；家庭月均收入（元）：0=2 000～5 000元，1=>5 000元；饮食口味：0=口味清淡，1=高盐饮食，2=喜油腻辛辣，3=喜虾蟹带壳海鲜；饮食方式：0=过热过烫，1=暴饮暴食，2=偏食挑食，3=细嚼慢咽；肝功能分级：0=A级，1=B级，2=C级，风险感知总分以实测值代入。结果显示，在校正年龄、家庭月均收入、饮食口味、饮食方式、肝功能分级等因素后，风险感知得分越高，肝硬化首次UGIB患者1年内出血复发的可能性越小（OR=0.93,95% CI:0.93~0.97,P<0.05）。
将风险感知按四分位数转化为分类变量:Q1组=1,Q2 组=2,Q3组=3,Q4组=4,以肝硬化首次UGIB患者出血复发情况为因变量(出血未复发=0，出血复发=1)进行logistic分析。结果显示,在校正年龄、家庭月均收入、饮食口味、饮食方式、肝功能分级混杂因素前后,Q3组、Q4组再出血风险显著降低(均P<0.05)。见表2。
2.4　风险感知对肝硬化首次UGIB患者1年内出血复发的预测价值
根据约登指数最大值确定最佳截断值,约登指数最大值为0.506,对应截断值为68.5。校正混杂因素后,风险感知(≤68.5)预测肝硬化首次UGIB患者1年内出血复发的AUC为0.847(95% CI:0.779～0.915),敏感度为66.0%,特异度为 91.4%,校正前后 AUC比较,Z=3.304,P=0.001，详见图1。
2.5　风险感知预测肝硬化首次UGIB患者1年内出血复发的DCA曲线
校正混杂因素后，风险感知得分预测肝硬化首次UGIB患者1年内出血复发风险的净获益率为0.03～0.80，详见图2。DCA曲线越靠近右上角，说明该模型的临床临床实用性越好。当阈值概率在20%～80%区间范围内，模型的DCA曲线更靠近图的右上方，表明在此范围内具有更优的临床实用价值，有助于指导临床决策。
3　讨论

3.1　肝硬化首次UGIB患者出血复发与风险感知现状
肝硬化上消化道出血后，再出血指首次止血后再次发生具有临床意义的活动性出血，主要临床表现为呕血和黑便[12]。多项研究数据显示，肝硬化出血患者1年内再出血发生率在27.1%~41.2%之间[13-14]。本项研究对221例患者的随访发现，发现47例（21.26%）发生再出血。再出血可能的原因是，由于肝硬化患者门脉高压等基础病理变化持续存在，现有治疗手段虽能实现暂时止血，但难以彻底逆转病理进程，使得患者长期面临较高的再出血风险。
本研究发现，首次发生UGIB的肝硬化患者对出血复发的风险感知普遍处于中低水平，与多项慢性疾病风险感知研究结果一致[15-18]。个人健康经历是影响风险感知的重要因素，缺乏相关经历的患者往往容易低估潜在风险[19]。首次出血患者由于未经历过复发事件，缺乏对复发严重性和后果的直接体验，同时受到“乐观偏差”心理的影响，使得患者倾向于认为复发不会发生在自己身上，从而导致风险感知水平降低。
3.2　风险感知对肝硬化首次UGIB患者出血复发的影响
本研究结果显示，风险感知水平显著影响再出血的发生（P<0.05）。风险感知水平较低的患者由于对疾病复发后果认识不足，往往治疗依从性较差，不能规律服药和定期复查，从而导致再出血风险增加。国内相关研究[20-21]也发现，多数肝硬化患者缺乏对病情变化的准确判断能力，即使黑便症状也仍无法引起对病情复发的重视。因此，建议临床工作者应特别关注风险感知水平较低的患者，通过讲座、宣传册等形式加强患者健康教育，提高患者风险感知水平，驱动“认知-行为-结局”的正向循环。
3.3　风险感知对肝硬化首次UGIB患者1年内出血复发的预测性能
风险感知能够影响患者的行为决策和治疗态度[22]，这些因素与患者治疗依从性和再出血风险密切相关。ROC曲线分析显示，风险感知对1年内再出血具有显著的预测价值为0.847。决策曲线分析显示，校正后的预测模型始终具有正向净获益，且决策曲线始终位于原始风险感知预测曲线上方，表明优化后的风险感知评估模型具有更好的临床决策价值。建议临床医护人员在对肝硬化首次UGIB患者进行出院评估时，将风险感知纳入常规评估体系，根据评估结果开展针对性的健康教育和心理干预。
本研究初步探讨风险感知与出血复发的相关性，尚未纳入患者治疗依从性、内镜治疗方式等潜在影响因素；研究为单中心设计，再出血事件数相对有限，可能影响模型稳定性。未来将开展多中心、大样本的前瞻性研究，纳入更多维度的影响因素，增加事件数与自变量数量的比例，优化模型稳定性。
