Objective

To explore the effects of community-based self-management intervention on depression, anxiety, and quality of life in high-risk populations with chronic obstructive pulmonary disease(COPD), providing scientific evidence for the formulation of community-based COPD prevention and intervention policies.

Methods

Data on sociodemographic characteristics, lifestyle, health status, and lung function were collected from 464 participants through 8 community-based self-management sessions for COPD conducted in Suzhou. Anxiety and depression were assessed using the Generalized Anxiety Disorder-7(GAD-7) scale and the Patient Health Questionnaire-9 (PHQ-9). Quality of life was assessed using the EQ-5D-3L scale (including the EQ-VAS).

Results

The results from the PHQ-9 and GAD-7 scales indicated that, after the intervention,the proportion of participants in the mild depression (anxiety) group and moderate or severe depression (anxiety) group significantly decreased, while the proportion of participants in the normal group increased significantly (P<0.05). A significant change was also observed in the PHQ-9 scale for the group with FEV1/FVC≥70% (P=0.018). In terms of quality of life,significant differences were found in the distribution of self-care, pain/discomfort, and anxiety/depression dimensions between pre- and post-intervention in both the overall population and the FEV1/FVC≥70% group (P<0.05). Except for the FEV1/FVC<70% group in EQ-VAS analysis, significant improvements were observed in the EQ-5D-3L utility index and EQ-VAS scores in all groups (P<0.001).

Conclusion

Community self-management interventions can improve the psychological health and quality of life of high-risk COPD populations, with more pronounced effects in the FEV1/FVC≥70% group. This study provides support for community-based COPD intervention strategies in Suzhou.

目的

探讨社区慢性阻塞性肺疾病（chronic obstructive pulmonary disease，COPD)自我管理小组对高危人群抑郁、焦虑及生活质量的影响，为COPD社区防控与干预政策的制定提供科学依据。

方法

通过在苏州市开展的8次社区COPD自我管理干预，收集干预前后464名研究对象的社会人口学特征、生活方式和健康状况等数据。焦虑和抑郁通过广泛性焦虑障碍量表（generalized anxiety disorder-7，GAD-7）和病人健康问卷（patient health questionnaire-9，PHQ-9）进行评估，生活质量采用EQ-5D-3L量表（包括EQ-VAS）进行评估。

结果

PHQ-9和GAD-7结果显示，干预后，总体研究对象在轻度抑郁（焦虑）组和中度抑郁（焦虑）及以上组的比例显著下降，正常组比例显著增加（P均<0.05）。在PHQ-9量表的第1秒用力呼气容积(forced expiratory volume in one second，FEV1)/用力肺活量（forced vital capacity,FVC）≥70%组中也观察到有意义变化（P=0.018）。生活质量方面，干预前后总体研究对象和FEV1/FVC≥70%组在自理能力、疼痛/不舒适和焦虑/沮丧三个维度分布存在显著差异（P均<0.05），除EQ-VAS分析中FEV1/FVC<70%组外，其余EQ-5D-3L效用指数和EQ-VAS分值在各组中均有改善（P均<0.001）。

结论

社区自我管理干预可改善慢阻肺病高危人群的心理健康和生活质量，在FEV1/FVC≥70%的组中干预效果更加明显。本研究为苏州市社区慢阻肺病干预策略提供了有力支持。

目的

探讨社区慢性阻塞性肺疾病（chronic obstructive pulmonary disease，COPD)自我管理小组对高危人群抑郁、焦虑及生活质量的影响，为COPD社区防控与干预政策的制定提供科学依据。

方法

通过在苏州市开展的8次社区COPD自我管理干预，收集干预前后464名研究对象的社会人口学特征、生活方式和健康状况等数据。焦虑和抑郁通过广泛性焦虑障碍量表（generalized anxiety disorder-7，GAD-7）和病人健康问卷（patient health questionnaire-9，PHQ-9）进行评估，生活质量采用EQ-5D-3L量表（包括EQ-VAS）进行评估。

结果

PHQ-9和GAD-7结果显示，干预后，总体研究对象在轻度抑郁（焦虑）组和中度抑郁（焦虑）及以上组的比例显著下降，正常组比例显著增加（P均<0.05）。在PHQ-9量表的第1秒用力呼气容积(forced expiratory volume in one second，FEV1)/用力肺活量（forced vital capacity,FVC）≥70%组中也观察到有意义变化（P=0.018）。生活质量方面，干预前后总体研究对象和FEV1/FVC≥70%组在自理能力、疼痛/不舒适和焦虑/沮丧三个维度分布存在显著差异（P均<0.05），除EQ-VAS分析中FEV1/FVC<70%组外，其余EQ-5D-3L效用指数和EQ-VAS分值在各组中均有改善（P均<0.001）。

结论

社区自我管理干预可改善慢阻肺病高危人群的心理健康和生活质量，在FEV1/FVC≥70%的组中干预效果更加明显。本研究为苏州市社区慢阻肺病干预策略提供了有力支持。

Objective

To explore the effects of community-based self-management intervention on depression, anxiety, and quality of life in high-risk populations with chronic obstructive pulmonary disease(COPD), providing scientific evidence for the formulation of community-based COPD prevention and intervention policies.

Methods

Data on sociodemographic characteristics, lifestyle, health status, and lung function were collected from 464 participants through 8 community-based self-management sessions for COPD conducted in Suzhou. Anxiety and depression were assessed using the Generalized Anxiety Disorder-7(GAD-7) scale and the Patient Health Questionnaire-9 (PHQ-9). Quality of life was assessed using the EQ-5D-3L scale (including the EQ-VAS).

Results

The results from the PHQ-9 and GAD-7 scales indicated that, after the intervention,the proportion of participants in the mild depression (anxiety) group and moderate or severe depression (anxiety) group significantly decreased, while the proportion of participants in the normal group increased significantly (P<0.05). A significant change was also observed in the PHQ-9 scale for the group with FEV1/FVC≥70% (P=0.018). In terms of quality of life,significant differences were found in the distribution of self-care, pain/discomfort, and anxiety/depression dimensions between pre- and post-intervention in both the overall population and the FEV1/FVC≥70% group (P<0.05). Except for the FEV1/FVC<70% group in EQ-VAS analysis, significant improvements were observed in the EQ-5D-3L utility index and EQ-VAS scores in all groups (P<0.001).

Conclusion

Community self-management interventions can improve the psychological health and quality of life of high-risk COPD populations, with more pronounced effects in the FEV1/FVC≥70% group. This study provides support for community-based COPD intervention strategies in Suzhou.

慢性阻塞性肺疾病（chronic obstructive pulmonary disease，COPD）简称慢阻肺病，是一种常见的、具有较高异质性且可防可治的慢性疾病。研究显示，中国40岁以上人群患病率升高至13.7%，有近1亿居民患有COPD[1]，然而，对于尚未出现明显的不可逆气流受限的疾病早期患者，肺功能检查的敏感性是有限的。因此，许多人在疾病进展后才被诊断出来，从而错过了早期干预的机会，并面临更高的残疾和死亡风险[2]，2019年就有研究[3]预测我国城市COPD高危人群为1.053亿，占城市成年总人口16.9%。同时，研究表明焦虑和抑郁在中国的COPD患者中普遍存在[4]，焦虑、抑郁与COPD共存会严重影响生活质量[5]。《慢性阻塞性肺疾病全球倡议（2023版）》认为，病人良好的自我管理能力是其疾病得到有效控制的重要因素[6-7]。目前，自我管理干预在提高COPD患者知晓率，降低呼吸相关住院的可能性等方面取得了一定的成果[8-9]，但对数量庞大且隐匿的COPD高危人群及其心理健康与生活质量的应用与研究却相对缺乏，因此，本研究就社区自我管理小组分析干预前后COPD高危人群的心理健康和生活质量的变化情况，旨在为COPD社区防控与干预政策的制定提供科学依据。
1　对象与方法

1.1　对象
选择2024年苏州市44个社区卫生服务中心或卫生院招募的COPD高危人群为研究对象。截至2024年12月31日，自我管理小组干预前共招募488位符合纳入排除标准的研究对象并签署知情同意，剔除24位（4.92%）因生病不便、住院、半途退出者，最终纳入464位研究对象。纳入标准：（1）年龄为40～79岁；（2）符合COPD高危人群判定标准；（3）在调查地区居住时间≥6个月。排除标准：（1）未签署知情同意书、不愿意参加调查；（2）存在精神疾病或认知障碍。本研究通过苏州市疾病预防控制中心医学伦理委员会审查，研究对象均已签署知情同意书。伦理审批号：SZJKMB-002。
符合以下任一情况即可判定为COPD高危人群[10-11]：（1）第1秒用力呼气容积（forced expiratory volume in one second，FEV1）/用力肺活量（forced vital capacity，FVC）<75%；（2）每年冬天咳嗽>3个月，持续时间≥2年；（3）走路气喘；（4）爬楼到3楼感觉胸闷气喘；（5）接触吸烟、职业粉尘和化学物质、烹调时不开抽油烟机产生的油烟或燃料产生的烟尘等危险因素；（6）有COPD家族史；（7）呼吸困难进行性加重，通常在活动时加重，持续存在，主要表现为呼吸费力、胸闷、气短和喘息；（8）慢性咳嗽、间歇性咳痰或无咳痰；（9）慢性咳痰。
1.2　方法
1.2.1　问卷调查
小组活动开始前和结束时分别采用问卷调查的方法收集研究对象的社会人口学特征（性别、年龄、文化程度、婚姻状况等）、生活方式（吸烟、饮酒、运动情况）、个人健康状况（呼吸系统疾病史、心理健康、生活质量、肺功能）。其中呼吸系统疾病史为研究对象在基线调查时明确被医院诊断肺结核和哮喘等疾病；心理健康主要包括焦虑、抑郁状况。
焦虑状况采用广泛性焦虑障碍量表（generalized anxiety disorder-7，GAD-7）[12-13]评估。共7个条目，每个条目采用0～3级评分，总分21分。得分≤4分为正常，5～9分为轻度焦虑，10～14分为中度焦虑，≥15分为严重焦虑。抑郁状况采用病人健康问卷（patient health questionnaire-9，PHQ-9）[14]评估。共9个条目，每个条目采用0～3级评分，总分27分。得分≤4分为正常，5～9分为轻度抑郁，10～14分为中度抑郁，15～19分为中重度抑郁，>19分为重度抑郁。两量表具有较好的信度与效度[15-16]。
EQ-5D-3L是全球生活质量评估中使用最频繁的工具之一[17]，量表包含一个简短的描述系统问卷和一个视觉模拟标尺。前者从五个维度（行动能力、自我照顾、日常活动、疼痛或不舒服、焦虑或沮丧）来定义个人在调查当天的健康状况，每个维度都包含三个级别（没有困难、有一些困难、有极度困难），因此，系统总共定义了243（35）个运行状况状态。然后，将每个运行状况转换为效用指数，效用指数越高表示健康状况越好。视觉模拟标尺为受访者提供从0（可以想象的最差健康状况）到100（可以想象的最佳健康状况）的垂直视觉模拟，报告能描述他们自评健康状况的等级。
使用浙江亿联康医科技有限公司的PF680优呼吸便携式肺功能仪测定肺功能，读取FEV1/FVC数值。FVC和FEV1测试质量标准分为A、B、C、D、E级，达到C级以上结果被采用。
1.2.2　社区自我管理干预
每个社区按照10～15个COPD高危对象的规模成立一个自我管理小组，每一组有一位组长与一位副组长，组长与副组长均来自地区社区卫生服务中心的医生，对社区比较熟悉，具备良好的与老年人沟通能力和服务经验，拥有健康教育与传播能力。小组活动形式分为以组长为主的教学、以学员为主的分享和展示、以身体活动为主的现场实地练习三大类，副组长将重点关注参与者的学习参与状态并协助组长进行组织实施、授课、练习和课后讨论，并通过一定的激励措施以保证参与者的依从性。小组活动结束时，鼓励学员实践所学知识，制定行动计划，帮助干预对象建立相关行为习惯。小组活动内容主要包括健康教育、运动锻炼和心理管理等，总共8节，每周进行1次，每次小组活动实际完成时间在60～90 min之间，期限为2个月。
1.3　质量控制
问卷调查前，调查方案、流程及注意事项由调查人员对社区医生进行统一培训。调查中若存在问卷填写不清楚的地方，由调查人员统一现场指导。问卷填写后当场核对并回收问卷，删除重复填写及存在逻辑错误的问卷。肺功能检测由经统一培训并经验丰富的呼吸科医生进行操作，FEV1和FVC的数值均符合质控要求。在干预活动开展过程中，苏州市疾病预防控制中心慢病科随时带队抽取一个社区进行现场督导，频次≥1次/周。
1.4　统计分析
使用SPSS 26.0和R 4.5.1软件进行统计学分析。正态分布的计量资料用（均数±标准差）表示，干预前后比较采用配对t检验；计数资料以频数及构成比描述，干预前后频数分布变化采用Stuart-Maxwell检验进行分析；非正态分布计量资料采用［M（P25，P75）］表示，干预前后得分比较采用Wilcoxon符号秩和检验。本研究均采用双侧检验，检验水准α=0.05。
2　结果

2.1　基本情况
本研究共纳入464名研究对象，平均年龄为（67.59±10.65）岁，年龄以65～79岁者居多，占69%；其中女性294（63.4%）人，男性170（36.6%）人；在婚424人，占91.4%；初中及以上340人，占73.3%；家庭年收入≥6万元212人，占45.7%；从不饮酒370人，占79.7%；从不吸烟361人，占77.8%。见表1。
2.2　干预前、后COPD高危人群心理健康状况的变化情况
PHQ-9总体结果显示，干预后轻度抑郁组（6.0%下降至3.7%）和中度抑郁及以上组（2.2%下降至0.4%）的比例均降低，而正常组的比例从91.8%增加至95.9%，干预前后分布差异具有统计学意义（P=0.006），在FEV1/FVC≥70%组中也观察到相似的变化（P<0.05）；PHQ-9得分值的［M（P25，P75）］从0（0,2）降低至0（0,1）（P<0.001），在FEV1/FVC<70%和≥70%分组中都观察到相似的变化（P<0.05）。GAD-7总体结果显示，干预后中度焦虑及以上的比例由1.1%降至0，轻度抑郁的比例由3.6%降至1.7%，而正常组的比例从95.3%增加至98.3%（P= 0.012），FEV1/FVC≥70%和<70%分组中干预前后分布不存在统计学意义（P>0.05）；GAD-7得分值在FEV1/FVC<70%组观察到［M（P25，P75）］从0（0,1）降低至0（0,0）（P<0.05）。见表2。
2.3　干预前、后COPD高危人群生活质量的变化情况
在总体研究对象中观察到干预前后自理能力、疼痛/不舒适和焦虑/沮丧EQ-5D-3L维度的分布存在显著差异（P均<0.05）,见表3。相较于干预前，FEV1/FVC≥70%和总体组中，干预后的EQ-5D-3L效用指数提高，分别从0.983增加至0.997，0.980增加至0.992（P均<0.001）；EQ-VAS总体得分的中位数从干预前的82提高至干预后的85（P<0.001），在FEV1/FVC<70%和≥70%组中都有所提升（P均<0.001），见表4。
3　讨论

本研究通过社区自我管理干预，评估了COPD高危人群心理健康和生活质量的变化情况。结果显示，干预后，总体研究对象在轻度抑郁（焦虑）组和中度抑郁（焦虑）及以上组的比例下降，正常组比例增加。生活质量方面，干预后总体研究对象和FEV1/FVC≥70%组的自理能力、疼痛/不舒适和焦虑/沮丧三个维度分布中完全没有问题的比例增加，有一些困难和极度困难的比例下降；EQ-5D-3L效用指数和EQ-VAS分值在各组均有改善；相较于FEV1/FVC<70%组，≥70%组的心理和生活质量改善更明显。这些结果表明，社区自我管理干预能一定程度改善COPD高危人群的心理健康和生活质量。
焦虑与抑郁是COPD常见并发症[18]，其发生不仅贯穿COPD患者早期、稳定期及晚期阶段，更可能于高危人群中预先显现，既往研究对COPD不同病程阶段心理干预的疗效差异进行了探索。Stephanie等人[19]发表于European Respiratory Journal结果表明，认知行为干预并不能改善晚期COPD患者的抑郁或焦虑，提示晚期患者因肺功能严重受损，心理干预的疗效可能被生理限制所削弱。然而，Abebaw等人[20]针对稳定期COPD患者实施了一项为期八周的干预计划，包括每周一小时的运动训练及一小时的健康教育，干预后抑郁量表评分从干预前18.62下降到干预后13.12（P<0.001），焦虑量表评分从14.60下降到10.99（P<0.001）。本研究进一步将干预对象扩展至高危人群，发现轻度抑郁（焦虑）组和中度抑郁（焦虑）及以上组的比例下降，正常组比例增加，提示高危人群可能存在更强的心理干预敏感性，越早对COPD人群的焦虑和抑郁进行干预，可能可以获得更大的收益。
焦虑抑郁可导致情绪紊乱，降低健康行为主动性，进而影响生活质量[21-22]。本研究采用EQ-5D-3L（包括EQ-VAS）量表评估生活质量，其涵盖的焦虑/沮丧维度使评估更为全面、细致。一项同样基于健康教育的社区干预[23]结果表明，早期干预可以改善研究对象对COPD的认识，减缓FEV1的下降速度，尤其在COPD高危人群中效果更为显著，这可能与他们对疾病认知的改变、健康行为的调整以及医疗资源的有效利用有关[24]。另有研究表明，体育锻炼能显著提高COPD高危人群和患者的运动耐量，缓解呼吸困难与疲劳，改善整体健康状况[25]。本研究综合运用健康教育讲座、排痰训练、运动锻炼及心理管理等干预措施，这些多方面的协同作用可能是生活质量得以改善的关键因素。
本研究强调预防关口前移，在改善COPD高危人群的心理健康和生活质量方面具有潜力，有一定的公共卫生学意义；同时，干预内容涵盖了疾病知识、情绪调节、运动锻炼等多个方面，具有较高的干预强度。然而，本研究亦存在若干局限。首先，纳入的样本量相对较小，可能会影响结果的外推性。其次，所实施的小组活动干预周期为八周，时间相对较短，因而可能在一定程度上低估了干预的实际效果。最后，本研究设计为单臂研究，缺乏同期对照组，难以完全排除时间效应或其他混杂因素对结果的潜在影响。
综上所述，社区自我管理干预可以一定程度上改善COPD高危人群的心理健康状况和生活质量。未来的研究应进一步探讨不同干预方式、干预强度和干预持续时间对COPD高危人群心理健康和生活质量的影响，为COPD的综合干预提供更为精准的指导，优化COPD社区干预策略。
