目的

系统评价中国HIV感染者认知障碍患病率及影响因素。

方法

检索中、英文数据库有关中国HIV感染者认知障碍的研究，检索时间为建库至2025年8月。采用Stata 18.0进行Meta分析。

结果

共纳入27篇文献，累计11 076例HIV感染者。Meta分析显示，中国HIV感染者认知障碍患病率（含筛查阳性）为42.3%；纳入研究间存在极高异质性（I²=99.4%），提示研究间存在巨大差异，该合并结果仅可视作患病估计范围，不宜视为精确数值。亚组分析显示，患病率在不同评估工具、性别、年龄与文化程度之间存在显著差异（P<0.05），且亚组分层后异质性仍偏高，提示仍存在未测量的潜在混杂因素。大专及以上文化程度为认知障碍的保护因素，合并抑郁及治疗方案中含有依非韦伦为危险因素。

结论

尽管本研究存在极高异质性、评估工具不统一等方法学局限，但现有证据初步提示我国HIV感染者认知障碍患病率（含筛查阳性）较高，建议对高危人群加强筛查与综合管理，以降低其对预后的不良影响。

Objective

To systematically evaluate the prevalence and influencing factors of cognitive impairment among people living with HIV (PLWH) in China.

Methods

We searched Chinese and English databases for studies on cognitive impairment among Chinese PLWH, from inception to August 2025. Meta-analysis was performed using Stata 18.0.

Results

A total of 27 studies involving 11 076 PLWH were included. The meta-analysis showed that the pooled prevalence of cognitive impairment (including screening-positive cases) in Chinese PLWH was 42.3%, with extremely high heterogeneity (I²=99.4%). The pooled estimate should be interpreted as an approximate range rather than an exact value. Subgroup analysis revealed significant differences across assessment tools, gender, age and education level (P<0.05), and residual heterogeneity remained high, suggesting unmeasured confounding factors. College or above education was a protective factor, while comorbid depression and efavirenz-containing regimens were risk factors.

Conclusion

Given the limitations of high heterogeneity and inconsistent assessment tools, cognitive impairment remains prevalent among Chinese PLWH. Strengthened screening and comprehensive management for high-risk groups are needed to reduce adverse prognostic impacts.

全球现存HIV感染者（people living with HIV, PLWH）约4 080万^［1］，中国报告HIV感染者/AIDS患者达135.5万例，累计死亡49.14万例^［2］。HIV感染主要损伤免疫系统，引发机会性感染及恶性肿瘤，同时可直接侵犯中枢神经系统，显著增加认知障碍发生风险^［3］。随着抗逆转录病毒治疗（antiretroviral therapy，ART）的推广应用，PLWH的死亡率明显降低、预期寿命显著延长，HIV感染已成为可控的慢性疾病^［4］。即便病毒得到有效抑制，认知障碍仍高发于PLWH人群，全球患病率达39.6%^［3,5］，易出现日常生活能力下降、药物滥用、治疗依从性差等问题，严重降低患者生活质量，加剧公共卫生负担^［6］。目前国际上PLWH认知障碍的常用评估工具分为两类：诊断类工具以神经心理学测试组合为临床确诊金标准，筛查类工具（如MoCA、IHDS）简便快捷，适用于大规模人群筛查^［5-6］。国内已陆续开展面向中国PLWH人群的评估工具本土化验证工作^［7-8］。现阶段我国HIV感染者认知障碍患病率报道区间波动较大（2.3～87.2%）^［9-10］，关于认知障碍与性别、病程、CD4细胞计数、合并抑郁等因素的关联，不同研究结论仍存在较大分歧。因此，本研究采用Meta分析方法，系统评价中国PLWH人群中认知障碍患病率及影响因素，为临床实践与卫生政策制定提供循证依据。

1　资料与方法

1.1　文献检索策略

计算机检索知网、万方数据库、维普网、CBM、PubMed、Web of Science、Cochrane Library、Embase中公开发表的关于中国HIV感染者认知障碍患病率及影响因素的研究，检索时间从建库至2025年8月。检索策略采用主题词与自由词相结合的方式进行，中文检索式：（HIV OR AIDS OR HIV/AIDS OR 艾滋病 OR 艾滋病病毒感染者 OR 人类免疫缺陷病毒 OR 人类免疫缺陷综合征 OR 获得性免疫缺陷综合征）AND（认知障碍 OR 认知功能障碍 OR 认知下降 OR 认知减退 OR 认知损害 OR 认知损伤 OR 认知功能损伤 OR 认知功能减退 OR 认知功能损害 OR 认知功能下降）。英文检索式：（HIV OR Human Immunodeficiency Virus* OR AIDS Virus* OR Acquired Immune Deficiency Syndrome Virus OR Acquired Immunodeficiency Syndrome* OR AIDS）AND（Cognitive Dysfunction* OR Cognitive Deficit* OR Cognitive Decline* OR Cognitive Disorder* OR Cognitive Impairment*）AND（China OR Chinese）。

1.2　文献纳入与排除标准

纳入标准：研究对象为临床确诊的中国HIV感染者；结局指标为认知障碍患病率和（或）其影响因素，其中影响因素需提供OR值及95%CI，或可转换为该指标的原始数据；研究类型：队列研究、病例对照研究及横断面研究。排除标准为：综述、会议文献、学位论文等；质量低、未报道相关结局指标且数据不完整的文献（缺失95%CI者先联系作者补充，未回复则将删除）；非中、英文文献；重复发表文献。

1.3　文献数据提取与质量评价

文献筛选与资料提取由两名研究者采用Endnote 21.0独立完成，并交叉核对结果；如遇分歧，将通过协商达成共识，必要时由第三位研究者参与裁定。提取内容包括：作者、年份、研究地区、样本量、研究类型、认知障碍患病率、影响因素等。针对队列研究与病例对照研究采用纽卡斯尔渥太华量表（Newcastle-Ottawa Scale，NOS）进行偏倚风险评价。该量表为国际通用的观察性研究质量评价工具，从研究对象选择、组间可比性、暴露或结局判定三个维度进行评分，总分0～9分，其中0～4分为低质量，4～6分为中等质量，7～9分为高质量。对于横断面研究，采用美国卫生保健研究与质量机构（Agency for Healthcare Research and Quality，AHRQ）制定的质量评价工具。该工具为横断面研究专用评价标准，涵盖样本代表性、研究设计、统计方法等11个条目，总分0～11分，0～3分为低质量，4～7分为中等质量，8～11分为高质量。

1.4　统计学方法

采用Stata 18.0软件进行Meta分析，计算认知障碍患病率及其95%CI，影响因素采用OR值及其95%CI进行描述。采用I²检验评估异质性，若I²<50%，P≥0.1时则采用固定效应模型进行Meta分析；反之则采用随机效应模型，且进行敏感性分析，或进行亚组分析。采用Egger检验评估发表偏倚。以P<0.05表示差异有统计学意义。

2　结果

2.1　文献筛选结果

初检获文献1 442篇，经分层筛选后纳入文献27篇^［9-35］，总样本量为11 076例。文献筛选流程见图1。

2.2　文献基本特征及质量评价

纳入的27篇文献发表于2010年—2025年，共纳入11 076例中国HIV感染者，提取到8项影响因素。其中8篇文献为高质量文献，19篇为中等质量。纳入文献的基本特征见表1，纳入文献质量评价结果（NOS量表）见表2。

2.3　Meta分析结果

2.3.1　HIV感染者认知障碍患病率

纳入的27篇研究间存在较高的异质性（I²=99.4%，P<0.001），故采用随机效应模型进行Meta分析，结果显示，中国HIV感染者认知障碍患病率（含筛查阳性）为42.3%（95%CI：32.9%～51.6%，Z=8.848，P<0.001），见图2。

根据地区、评估工具、性别、年龄、婚姻、文化程度、是否进入AIDS期、CD4⁺ T淋巴细胞计数、是否ART等进行亚组分析。结果显示，评估工具为IHDS、女性、年龄≥50岁、文化程度为大专以下的HIV感染者认知障碍患病率更高（P<0.05）。华中地区合并患病率为48.0%（95%CI：7.0%～89.0%），置信区间跨度极大，结果可靠性较差，临床解释需极其谨慎。见表3。

2.3.2　HIV感染者认知障碍的影响因素

15项研究报道了中国HIV感染者认知障碍的影响因素，对其进行Meta分析。结果显示，教育水平为大专及以上、合并抑郁、治疗方案含有依非韦伦（Efavirenz，EFV）是认知障碍的影响因素（P<0.05）。其中，病程、丙肝合并感染等因素仅纳入2项研究，文献数量偏少，合并结果稳定性有限。具体见表4。

2.4　敏感性分析

逐一剔除各项研究进行敏感性分析，结果显示：分别剔除Wang等^［33］、熊文琴等^［16］、黎彦君等^［19］的研究后，性别、年龄、CD4细胞计数的异质性分别下降，合并结果转为具有统计学意义（P<0.05）；其余指标的合并结果均较为稳定，未发生明显改变。见表5。

2.5　发表偏倚

对中国HIV感染者认知障碍患病率的发表偏倚进行分析，漏斗图显示不对称分布（见图3）；Egger’s 检验提示存在显著发表偏倚（t=5.20，P<0.001）；进一步采用剪补法校正发表偏倚，共补充14项虚拟研究，校正后合并患病率降至17.1%（95%CI：7.8%～26.4%，P<0.001），效应值仍具有统计学意义，但幅度显著下降，提示原始合并结果受发表偏倚影响存在明显高估。对文献数量≥5篇的影响因素进行Egger检验，P值均大于0.05，提示无明显发表偏倚。

3　讨论

3.1　中国PLWH认知障碍患病率

本研究Meta分析结果显示，中国HIV感染者认知障碍（含筛查阳性）合并患病率为42.3%，整体合并效应量存在极高异质性（I²=99.4%），研究间差异较大，因此该数值仅可作为该人群整体患病估计范围，不宜视为严格临床确诊的精确数值。尽管已进行亚组分析，但多数亚组内异质性仍高于90%，提示异质性来源较为复杂，仍存在尚未识别或未测量的潜在混杂因素。纳入研究同时采用筛查工具与诊断工具，将筛查阳性人群与确诊的认知障碍病例合并统计，可能导致总体估值偏高。因此，本研究结果更接近认知障碍筛查阳性率，而非严格意义上的临床诊断患病率。

本研究结果略高于KENG等^［5］报告的全球水平（39.6%），该差异可能与研究对象、地域及文献来源差异有关，尤其与我国PLWH人群自身特征密切相关。一方面，我国部分地区抗逆转录病毒治疗可及性不足、监测不充分，加之HIV相关社会歧视导致的治疗中断与长期心理应激，进一步加重认知障碍^［36］；另一方面，我国PLWH人群中高血压、糖尿病、抑郁等共病比例较高，此类共病与HIV协同作用，加速神经退行性变，共同升高认知障碍发生风险^［37］。

亚组分析结果表明，评估工具中，MoCA、IHDS检出患病率均高于NP。MoCA与IHDS为高效便捷的筛查工具，高敏感性识别潜在病例时可能纳入临界个体，致估值偏高^［38］，NP作为诊断金标准，通过多维度测验综合判定，评估全面严谨、特异性高，故结果相对保守。性别中，女性PLWH患病率显著高于男性。可能与男女认知功能固有差异有关，女性更易出现学习记忆、运动功能等领域的特异性认知障碍^［39］，且认知储备偏低、对抑郁情绪更为敏感^［40］；其次，女性在HIV感染后可能出现雌激素水平下降，进而削弱其对大脑认知功能的保护作用；此外，药物代谢及药物使用模式的性别差异，也会使女性面临更高的认知障碍风险。年龄中，≥50岁PLWH患病率显著高于<50岁群体。Chen等^［8］研究证实年龄是该人群中认知障碍进展的重要危险因素。伴随衰老过程中的生理功能减退，加之HIV感染对中枢神经系统的累积损害，共同加剧了认知功能下降的风险；其中老年PLWH情景记忆与运动功能尤为突出^［41］。华中地区合并患病率置信区间较宽，结果稳定性较低，主要与仅纳入4项研究且异质性极高、各研究患病率差异较大有关，提示该地区结果需谨慎解读。抗逆转录病毒治疗可通过有效抑制病毒载量减少认知障碍发生风险^［3］，但本研究ART亚组未显示统计学差异，可能与纳入研究间治疗方案、治疗时长及病毒抑制情况存在混杂有关。

3.2　认知障碍的影响因素

本研究敏感性分析发现，剔除部分文献后，性别、年龄及CD4⁺ T淋巴细胞计数相关因素的异质性分别下降，统计结果转为具有统计学意义。提示上述因素受个别研究影响较大，整体结果稳健性不足。

本研究结果显示，高教育水平是认知障碍的重要保护因素。教育作为认知储备的核心要素，不仅与PLWH整体认知表现呈正相关^［42］，还可延缓其认知障碍及HIV相关神经退行性改变^［43］。同时，高教育水平的PLWH通常具备较强的健康信息获取与处理能力，对疾病认知程度更深，可提升治疗依从性与风险意识，进而间接降低认知障碍风险。因此，在健康教育中，应特别为低教育水平感染者设计简明易懂、可视化的认知障碍专项科普材料，缩小不同文化层次人群的健康认知差距，进而降低其认知障碍发生风险。

本研究结果显示，抑郁是该人群发生认知障碍的重要危险因素，与Qin等^［44］研究一致。受社会环境和疾病负担的双重影响，PLWH患精神心理疾病的风险远高于一般人群，抑郁检出率可达50.1%^［45-46］。在神经生物学层面，抑郁不仅可能加重大脑结构的损害，还会干扰胆碱能系统正常功能，进而导致记忆力减退与整体认知能力下降^［47-48］。建议在HIV感染者常规诊疗流程中重视对抑郁情绪的筛查，对合并抑郁的高危人群及早开展心理疏导与情绪干预，减轻抑郁对认知功能的不良影响。

本研究结果显示，治疗方案中含EFV是PLWH认知障碍的危险因素。尽管EFV仍被纳入我国一线联合抗逆转录病毒治疗方案，但国内外指南已优先推荐整合酶抑制剂作为初治优选方案^［49-50］。EFV的潜在神经毒性备受关注，超半数使用者会出现神经精神症状^［51］，现有研究也提示长期使用EFV与患者神经认知功能下降有关^［52］。然而，目前研究结论仍存在不一致，如有研究发现血浆EFV浓度与整体神经认知功能评分呈正相关^［53］，亦有研究显示换用其他药物后未观察到神经认知功能的显著改善^［54］。这表明EFV与神经认知功能障碍之间的关联尚未完全明确，仍需更多高质量研究深入探讨。因此，在临床实践中对PLWH进行长期管理时，宜优先选择神经毒性较低的抗病毒治疗方案，尽可能减少对中枢神经系统的不利影响；对于仍使用含EFV方案的患者，临床管理中应保持审慎，加强监测与个体化评估，必要时结合患者病情与药物耐受情况及时优化治疗方案。

3.3　本研究的局限性

（1）不同研究所采用的认知障碍评估工具存在差异，降低了研究间的可比性；（2）本研究可能高估认知障碍患病率，因纳入研究主要使用快速筛查工具，方法学严谨性不足，假阳性风险较高；（3）研究间样本量差异大，部分研究样本代表性不足；（4）部分影响因素相关的研究数量较少，敏感性分析和发表偏倚评估受限，未来需更多高质量研究进行验证；（5）认知障碍患病率Egger检验提示存在显著发表偏倚，经剪补法校正后，合并效应虽仍有统计学意义，但患病率明显回落，提示原始合并结果存在高估。发表偏倚会影响结论外推与真实性，故本研究结论需谨慎解读。

4　结论

尽管本研究存在高异质性和方法学局限，但现有证据初步提示，我国HIV感染者认知障碍患病率（含筛查阳性）较高；高教育水平为认知障碍的保护因素，合并抑郁、采用含EFV抗病毒方案则为危险因素。
