Objective

To investigate the efficacy of role-playing in enhancing students’ practical capabilities for responding to public health emergencies, grounded in situated learning theory.

Methods

Employing the Delphi expert consultation method (n=12), the practical course content was systematically developed. A role-playing teaching approach based on the five-act scenario play Campus Defence was implemented with 120 students from the School of Public Health at a medical university.

Results

Experts reached high consensus on core modules such as “contingency planning” and “case reporting,” providing scientific rationale for scenario development. Quantitative analysis revealed that post-intervention scores (70.94±18.83) significantly exceeded pre-intervention scores (54.43±19.11) (t=12.52, P<0.001), with a substantial effect size (Cohen’s d=1.16). Senior students, female cohorts, and those of intermediate academic standing demonstrated particularly marked improvements.

Conclusion

By establishing a “community of practice,” role-playing effectively facilitated students’ construction of practical knowledge within authentic scenarios. This approach provides theoretical support and a replicable pathway for bridging the knowledge-practice gap in public health education.

目的

基于情境学习理论，探讨角色扮演法在提升学生应对突发公共卫生事件实践能力的有效性。

方法

采用德尔菲专家咨询法（n=12）系统构建实践课程内容，并以某医科大学公共卫生学院120名学生为研究对象，实施基于五幕情景剧《校园防线》的角色扮演教学。

结果

专家在“预案制定”“病例报告”等核心模块达成高度共识，为情景剧开发提供了科学依据。量化分析表明，学生实践后测得分（70.94±18.83）显著高于前测（54.43±19.11）（t=12.52, P<0.001），且效应量较大（Cohen’s d=1.16）。高年级学生、女生群体及中等排名学生提升尤为显著。

结论

基于情境学习理论，探讨角色扮演法在提升学生应对突发公共卫生事件实践能力的有效性。

方法

采用德尔菲专家咨询法（n=12）系统构建实践课程内容，并以某医科大学公共卫生学院120名学生为研究对象，实施基于五幕情景剧《校园防线》的角色扮演教学。

结果

专家在“预案制定”“病例报告”等核心模块达成高度共识，为情景剧开发提供了科学依据。量化分析表明，学生实践后测得分（70.94±18.83）显著高于前测（54.43±19.11）（t=12.52, P<0.001），且效应量较大（Cohen’s d=1.16）。高年级学生、女生群体及中等排名学生提升尤为显著。

结论

角色扮演法通过创设“实践共同体”，有效促进了学生在真实情境中实践性知识的建构，为弥合公共卫生教育“知行鸿沟”提供了理论支持与可复制的实践路径。

Objective

To investigate the efficacy of role-playing in enhancing students’ practical capabilities for responding to public health emergencies, grounded in situated learning theory.

Methods

Employing the Delphi expert consultation method (n=12), the practical course content was systematically developed. A role-playing teaching approach based on the five-act scenario play Campus Defence was implemented with 120 students from the School of Public Health at a medical university.

Results

Experts reached high consensus on core modules such as “contingency planning” and “case reporting,” providing scientific rationale for scenario development. Quantitative analysis revealed that post-intervention scores (70.94±18.83) significantly exceeded pre-intervention scores (54.43±19.11) (t=12.52, P<0.001), with a substantial effect size (Cohen’s d=1.16). Senior students, female cohorts, and those of intermediate academic standing demonstrated particularly marked improvements.

Conclusion

By establishing a “community of practice,” role-playing effectively facilitated students’ construction of practical knowledge within authentic scenarios. This approach provides theoretical support and a replicable pathway for bridging the knowledge-practice gap in public health education.

随着“新医科”建设的深入推进以及后疫情时代对复合型公共卫生人才需求的日益迫切，传统以理论讲授为主的教学模式在培养学生应对突发公共卫生事件的实战能力方面逐渐显现出局限性，其核心在于“去情境化”的教学过程导致了知识与实践的割裂[1]。情境学习理论认为，学习不仅是个体通过认知建构获取知识的过程，更是一种在“实践共同体”中通过“合法的边缘性参与”而社会性地建构知识的过程[2-3]。在这一过程中，学习者通过情境互动、与他人协作以及利用多样化的资源，实现知识的主动建构与迁移。角色扮演教学法作为一种互动性强、实践性突出的沉浸式教学方法，其本质是创设一个模拟的“实践共同体”，为学习者提供了从边缘观察到核心参与的合法身份与真实情境，在理论上与解决上述“知行鸿沟”高度契合[4]。
然而，现有研究表明，角色扮演法在突发公共卫生事件教学中的应用多局限于单一环节的模拟[5]或技能操练[6]，其设计往往基于经验总结，缺乏系统化的课程内容作为支撑。这导致教学活动与完整的应急处置知识体系关联不强，难以实现多学科知识点的有效串联与整合，学生获得的更多是零散的体验而非系统化的能力构建。为此，本研究创新性地将德尔菲专家咨询法与角色扮演教学深度融合：首先通过德尔菲法系统梳理并确立了应急处置的核心知识模块，继而以此为基础，原创开发了贯穿“监测－上报－处置－发布”全流程的情景剧《校园防线》。本研究旨在实证检验“系统化设计、跨学科整合”的教学模式对提升学生实践能力与应急素养的效果，为培养胜任复杂公共卫生挑战的复合型人才提供一套科学、可复制的方案[7-8]。
1　研究对象与方法

1.1　研究对象
于2025年4月20日选取某医科大学公共卫生学院预防医学和公共事业管理专业共计120位本科生作为研究对象。其中，男生47人（39.17%），女生73人（60.83%）；预防医学专业43人，公共事业管理专业77人；高年级（大三至大五）74人，低年级（大一至大二）46人。所有参与者均知情同意。
1.2　研究设计
本研究采用“基于德尔菲法的课程开发与单组前后测准实验”的混合研究设计，具体如下：
1.2.1　德尔菲专家咨询法
为科学构建实践活动内容体系，本研究设计了《突发公共卫生事件各部门职责重要性评估表》，其涵盖政府、疾控中心、医疗机构等部门的32项职责。专家需按“非常重要”“重要”“一般”“不重要”“完全不重要”五级评分进行评价。参与专家来自医科大学、疾控中心、医疗卫生机构等相关单位。在2024年12月至2025年3月期间，通过电子邮件、微信等方式发放和收回问卷，共计两轮。
1.2.2　角色扮演法
本研究采用岗位导向型角色扮演法，以德尔菲专家咨询结果为基础，将突发公共卫生事件应急处置的核心职责细化为多个实战角色，如市场监管局人员、校医院医生、疾控中心专家及学校后勤主任等角色。研究结合诺如病毒暴发真实案例，设计多幕情景剧脚本。通过“检查－上报－处置－发布”的任务链驱动教学。
1.2.3　课前准备与角色分工
为确保情景模拟教学的参与广度与深度，本研究在活动开展前两周，通过“学习通”平台向全体研究对象发布《突发公共卫生事件应对知识问卷》及演练任务书。学生自由组9队，并随机抽取疾控中心、市场监督、教育行政、学校、医疗机构、新闻媒体等角色卡。各组在老师指导下完成脚本编写与角色分工，并集中排练。
为实现全员参与，各小组内部进一步细分为表演组、资料组、观察与反馈组。其中，表演组（1~2人/组）负责相应角色在情景剧中的现场演绎；资料组（5~6人/组）负责查阅所扮演角色的职责及相关专业知识，参与脚本的撰写与修订；观察与反馈组（5~6人/组），负责在排练及正式展示中记录他组表现，并在互动环节进行交叉点评。
1.2.4　教学干预与测量工具
教学干预为学生基于德尔菲结果原创开发的五幕情景剧《校园防线》。实践活动前后，采用《突发公共卫生事件应对知识问卷》进行测评。该问卷共24题，总分100分，在本研究中其Cronbach α信度为0.79，具有较好的测量信度。
1.3　统计学方法
使用Stata 18.0软件进行数据分析。计量资料以（）描述，组内前后比较采用配对样本t检验，以P<0.05为差异有统计学意义，并计算Cohen’s d值以评估效应量（d≥0.2为小效应，d≥0.5为中等效应，d≥0.8为大效应）。
2　研究结果

2.1　德尔菲专家咨询结果
参与咨询的12位专家均来自公共卫生实践与教学一线。其中，66.67%具有高级职称，66.67%拥有硕士及以上学位，对研究内容“很熟悉”者占50.00%。专家权威系数（Coefficient of authority, Cr）为0.89，高于0.70可接受标准，咨询结果可靠[9]。两轮咨询所有条目的变异系数均小于0.25。Kendall协调系数由第一轮的0.298显著提升至第二轮的0.403（P<0.05），表明专家意见趋于一致。
咨询结果显示，专家在“前端预防－预案制定的主体及内容”“常规性预防工作－传染病预警与监测”“病例报告－内容与时限”“防止院感－防止交叉感染和污染的措施”等模块形成了高度共识（重要性评分≥4.83，满分率≥83.33%）。表明这些模块是情景剧编排中必须重点呈现的核心内容。然而，在“卫生政策法规制定与执行－卫生政策法规制定与执行能力”“调查评估－基于标准的卫生监督与评估流程”“政府公关－媒体沟通策略与新闻发布会流程”模块上，专家意见存在一定分歧（满分率≤25.00%）。具体咨询结果见表1。
2.2　教学干预的开发结果：基于德尔菲法与情境学习理论的情景剧构建
本研究基于德尔菲专家咨询结果与情境学习理论，以“诺如病毒校园暴发”叙事为驱动，严格遵循突发公共卫生事件应急处置流程，成功开发了五幕情景剧《校园防线》作为核心教学干预工具，为学生提供了一个“合法边缘性参与”的实践情境。详见表2。剧情安排如下：
序幕——市场监督管理局日常监管：构建事件风险情境。以食堂后厨的常规检查为切入点，通过市场监管局周队长执行ATP荧光检测、查验操作台卫生等细节，在真实工作场景中具象化“食品安全风险防控”“现场快速检测”等监管职责，展现突发公共卫生事件前端的风险识别与常态化监督流程，为后续疫情暴发埋下伏笔。
第一幕——校医院监测：构建病例发现情境。以学生就诊为切入点，通过校医询问“呕吐物形态”“密接情况”等细节，在仿真情境中具象化“病例定义”“隔离措施”等医疗机构职责，完成疫情识别与初始上报。
第二幕——跨部门报告与响应：模拟信息传递与协同决策。通过模拟市教委、市卫健委、市应急办的联动过程，将“法定传染病分类”“网络直报时限”等突发事件应急知识转化为角色间的具体指令与对话，强化突发公共卫生事件发生时各部门联防联控机制。
第三幕——调查与控制：整合多学科操作技能。设计实验室检测、环境消毒、涉事商家查封等环节，通过角色行动，如标注消毒剂浓度、引用《中华人民共和国食品安全法》条款等，将“病原学鉴定”“法律依据”等跨学科知识转化为可视化、可操作的实践，重点回应了专家共识中的技术焦点。
第四幕——评估与终止：训练系统总结与公共沟通能力。通过新闻发布会与闭门会议场景，将抽象的“政府公关”“舆情管理”“预案制定”策略转化为具体的角色台词，如引用《突发公共卫生事件应急条例》回应记者提问，训练学生的系统思维与公共沟通能力。
2.3　教学干预的效果评价：学生实践能力的前后测比较
为检验上述教学干预方案的效果，我们对参与学生进行了前后测问卷评估。如表3所示，参与课程的所有学生在后测中得分均显著高于前测（均P<0.001），总体效应量Cohen’s d为1.16，表明干预产生了较大效应。具体而言，高年级学生（d=1.26）的提升幅度大于低年级学生（d=0.99），女生（d=1.37）的改善程度高于男生（d=0.88），且中等排名（11-20名）学生表现出最大的提升潜力（d=1.34）。
情景剧教学在知识掌握、应急能力、预防措施与感染控制、政策、法律与信息认知模块均展现出显著的提升效果。其中，突发公共卫生事件分级判定、网络直报时限和政府公关模式三个知识点提分最多，分别提升了39.32%、34.18%和30.77%。这表明情景剧在复杂知识点理解、应急流程掌握和政策法律知识强化方面具有显著优势。见表4。

本研究在情境学习理论框架下，验证了角色扮演法作为一种创设“实践共同体”的有效手段，能够显著提升公共卫生专业本科生的实践能力。通过让学生在模拟的应急管理体系中承担“合法”角色，使其学习过程从被动的知识接收转变为主动的、情境化的知识建构与意义协商，这正是情境学习理论的核心要义[10]。
专家咨询结果显示，专家在“前端预防”“常规性预防工作”“病例报告”等模块已形成高度共识。这些共识内容为情景剧的主干剧情与重点教学环节提供了权威依据，确保了教学内容与一线实践需求的紧密契合[11]。然而，在“卫生政策法规制定与执行”“调查评估”“政府公关”模块上，专家意见存在一定分歧。这一现象不仅反映了这些领域在实际操作中具有较高的复杂性和不确定性，也提示其在教学转化中具有特殊价值[12]。本研究并未将这些分歧视为缺陷，而是将其转化为情景剧设计的关键切入点，通过将争议性内容设置为剧情中的核心冲突与决策情景，为学生提供了在模拟实践中直面复杂问题，构建自身实践知识的宝贵机会。可见，德尔菲专家咨询的结果不仅能促使实践活动内容的科学性，更决定了情景剧编排的逻辑主线与教学针对性。当然，本调查亦存在不足，如专家构成以公共卫生领域为主，一定程度上导致实践活动在“法律条文适用”“信息工具操作”等跨学科复合能力的覆盖上存在局限，这为未来研究引入更广泛的跨学科专家视角指明了方向。
教学干预效果显示，整体效应量达1.16，表明情景模拟教学对公共卫生专业本科生的实践能力提升具有显著效果。以下从群体差异和知识点差异两个维度进行分析。
从群体差异来看，高年级学生的提升幅度显著高于低年级学生。从情境学习理论视角分析，高年级学生因具备更丰富的“前结构”知识，在实践共同体中能够进行更深层次的认知建构与知识迁移[13]。他们不仅更容易理解角色所承担的职能内涵，还能将前期课堂所学与模拟情景中的任务有效结合；女生的改善程度显著高于男生。这一差异可能与性别在沟通协调能力与角色投入上的优势有关。在情景模拟中，女生更倾向于主动参与角色互动，在现场互动环节中也表现出较高的参与度；中等排名的学生表现出更大的潜力。这一群体在传统笔试考核中处于中间名次，其实践潜能未被充分激发。而角色扮演这一新的“实践场域”为其提供了能力展示空间。这一发现揭示了教学方法与学习者特质的交互作用，为实施精准教学干预提供了依据。
从知识点差异来看，学生在“突发公共卫生事件分级判定”“网络直报时限”和“政府公关”三个知识点提分最为显著。其中，前两知识点在课前正确率均处于较低水平（33.33%和26.50%）。表明学生在传统课堂中虽曾接触过相关内容，但仅停留在应试层面的机械记忆，缺乏真实场景的应用，导致知识遗忘率较高。而经过情景模拟教学后，这两个知识点的正确率分别提升至72.65%和60.68%，提升幅度位居前列。这一现象揭示了情景模拟教学的内在机制，即通过将抽象知识点嵌入真实任务情境，使学生在角色扮演中经历知识再现。当学生以“疾控中心工作人员”身份判断疫情分级、以“责任报告人”身份完成网络直报时，原本静态的、易遗忘的条文知识被激活为动态的、可操作的实践知识。这种“做中学”的认知加工方式，实现了从“知道”到“会用”的实质性跨越；而第三个知识点“政府公关”恰好对应了德尔菲专家咨询中意见分歧较大的模块，但是学生在此模块的认知提升也较为显著（30.77%）。这一现象有力证明了情景模拟的教学价值，即将争议性内容设置为剧情的核心冲突与决策情景，使学生在模拟中直面复杂问题，在“做中学”的过程中构建自身对争议问题的理解。这一发现与黄晶晶等关于角色扮演法能有效提升应急能力的研究结论相互印证，共同支持了情境化教学在公共卫生教育中的核心价值[14]。

综上，本研究证实，基于情境学习理论构建的“实践共同体”角色扮演模式，是弥合公共卫生教育“知行鸿沟”的有效路径，显著提升了学生在复杂情境中的知识整合与应急决策能力[15]。本研究的局限性在于采用单组前后测设计，未来研究可通过设置对照组、结合质性访谈深入揭示学习者的认知建构过程。尽管存在以上局限，本研究所形成的“理论引领－专家建构－情境赋能”的教学模式，对于应急管理、社会工作等其他需要高度实践素养的教育领域，仍具有重要的迁移价值与借鉴意义。
