Objective

To understand the level of postpartum depression literacy among women in Guiyang within 0-6 months postpartum, and to analyze the latent profile categories of postpartum depression literacy, in order to effectively carry out mental health services.

Methods

The communities were selected through multi-stage stratified random sampling, and 1 088 subjects were continuously included in the study. A questionnaire survey was conducted using general information, the Edinburgh Postnatal Depression Scale (EPDS), and the Postpartum Depression Literacy Scale (PoDLis). A latent profile analysis was performed on postpartum depression literacy, and logistic regression was used to analyze the influencing factors.

Results

The positive rate of postpartum depression symptoms was 14.5%. The total score of postpartum depression literacy was 3.29 (3.00, 3.58) points. The postpartum depression literacy of the mothers was divided into three latent classes: the low literacy group (5.97%), the medium literacy group (47.88%), and the high literacy group (46.14%). Younger maternal age (OR=3.282, 95% CI: 1.130-9.534), high school education (OR=2.296, 95% CI: 1.080-4.880), and low monthly per capita family income (OR=2.385, 95% CI: 1.050-5.420) were risk factors for the low literacy group, while participating in relevant courses (OR=0.673, 95% CI: 0.472-0.959) was a protective factor for the high literacy group (P<0.05).

Conclusion

There is heterogeneity in postpartum depression literacy among women. Special attention should be paid to women with younger maternal age, low educational level, low family economic level, and no participation in maternity school courses. Interventions can be implemented based on the characteristics and influencing factors of different latent classes to improve postpartum depression literacy and prevent the occurrence of postpartum depression.

目的

了解贵阳市产后0—6月产妇产后抑郁素养水平，分析产后抑郁素养的潜在剖面类别，以便有效开展心理健康服务。

方法

按多阶段分层随机抽样抽取社区，连续纳入调查对象1 088名，采用一般情况资料、爱丁堡产后抑郁量表（Edinburgh postnatal depression scale，EPDS）、产后抑郁素养量表（postpartum depression literacy scale，PoDLis）进行问卷调查，对产后抑郁素养进行潜在剖面分析，采用logistic回归进行影响因素分析。

结果

产后抑郁症状阳性率为14.5%，产后抑郁素养总分为3.29（3.00，3.58）分，产妇产后抑郁素养分为三个潜在类别：低素养组（5.97%）、中素养组（47.88%）、高素养组（46.14%），低生育年龄（OR=3.282，95%CI：1.130～9.534）、高中文化程度（OR=2.296，95%CI：1.080～4.880）、家庭人均月收入低（OR=2.385，95%CI：1.050～5.420）是低素养组的危险因素，参加相关课程学习（OR=0.673，95%CI：0.472～0.959）是高素养组的保护因素（P<0.05）。

结论

产妇产后抑郁素养存在异质性，重点关注低生育年龄、低文化水平、家庭经济水平不高且未参与过孕产妇学校的产妇，可根据不同潜在类别特征及影响因素实施针对性干预，以提高产后抑郁素养水平，预防产后抑郁发生。

目的

了解贵阳市产后0—6月产妇产后抑郁素养水平，分析产后抑郁素养的潜在剖面类别，以便有效开展心理健康服务。

方法

按多阶段分层随机抽样抽取社区，连续纳入调查对象1 088名，采用一般情况资料、爱丁堡产后抑郁量表（Edinburgh postnatal depression scale，EPDS）、产后抑郁素养量表（postpartum depression literacy scale，PoDLis）进行问卷调查，对产后抑郁素养进行潜在剖面分析，采用logistic回归进行影响因素分析。

结果

产后抑郁症状阳性率为14.5%，产后抑郁素养总分为3.29（3.00，3.58）分，产妇产后抑郁素养分为三个潜在类别：低素养组（5.97%）、中素养组（47.88%）、高素养组（46.14%），低生育年龄（OR=3.282，95%CI：1.130～9.534）、高中文化程度（OR=2.296，95%CI：1.080～4.880）、家庭人均月收入低（OR=2.385，95%CI：1.050～5.420）是低素养组的危险因素，参加相关课程学习（OR=0.673，95%CI：0.472～0.959）是高素养组的保护因素（P<0.05）。

结论

产妇产后抑郁素养存在异质性，重点关注低生育年龄、低文化水平、家庭经济水平不高且未参与过孕产妇学校的产妇，可根据不同潜在类别特征及影响因素实施针对性干预，以提高产后抑郁素养水平，预防产后抑郁发生。

Objective

To understand the level of postpartum depression literacy among women in Guiyang within 0-6 months postpartum, and to analyze the latent profile categories of postpartum depression literacy, in order to effectively carry out mental health services.

Methods

The communities were selected through multi-stage stratified random sampling, and 1 088 subjects were continuously included in the study. A questionnaire survey was conducted using general information, the Edinburgh Postnatal Depression Scale (EPDS), and the Postpartum Depression Literacy Scale (PoDLis). A latent profile analysis was performed on postpartum depression literacy, and logistic regression was used to analyze the influencing factors.

Results

The positive rate of postpartum depression symptoms was 14.5%. The total score of postpartum depression literacy was 3.29 (3.00, 3.58) points. The postpartum depression literacy of the mothers was divided into three latent classes: the low literacy group (5.97%), the medium literacy group (47.88%), and the high literacy group (46.14%). Younger maternal age (OR=3.282, 95% CI: 1.130-9.534), high school education (OR=2.296, 95% CI: 1.080-4.880), and low monthly per capita family income (OR=2.385, 95% CI: 1.050-5.420) were risk factors for the low literacy group, while participating in relevant courses (OR=0.673, 95% CI: 0.472-0.959) was a protective factor for the high literacy group (P<0.05).

Conclusion

There is heterogeneity in postpartum depression literacy among women. Special attention should be paid to women with younger maternal age, low educational level, low family economic level, and no participation in maternity school courses. Interventions can be implemented based on the characteristics and influencing factors of different latent classes to improve postpartum depression literacy and prevent the occurrence of postpartum depression.

《健康中国2023》规划纲要明确提出要促进孕产妇心理健康，提升心理健康素养。目前产妇产后抑郁症状阳性率呈逐年上升[1-2]。研究发现仅约40%的产后抑郁被早期识别，其主要原因为产妇缺乏对抑郁症状、体征及治疗的认知，产后抑郁素养水平较低，使产妇将抑郁最小化或正常化[3]。产后抑郁素养被认为是一种特殊的心理健康素养，是针对产后抑郁疾病的知识、信念和技能，有助于产后抑郁的识别、管理或预防[4]。研究表明我国产妇健康素养水平16.6%～29.0%[5-6]，并提示健康素养可能为产后抑郁的危险因素[7]。因此，准确评估及提升产妇产后抑郁素养具有重要意义，但现有研究多以量表总分高低判断素养高低，忽略了群体的异质性[8]。潜在剖面分析以个体为中心，可通过外显指标划分潜在类别，并以客观的统计指标评估分类的准确性，以实现精准干预。基于此，拟对贵阳市社区产妇进行现况调查，了解产后抑郁症状现状，基于潜在剖面分析探索产后抑郁素养特征及类别的影响因素，为提升产后抑郁素养、预防产后抑郁、完善妇幼保健工作提供科学依据。
1　对象与方法

1.1　研究对象
于2025年6—8月，采用多阶段分层随机抽样，在贵阳市六区三县一市随机抽取共30个社区，按服务顺序连续纳入社区卫生服务中心（站）儿保科符合条件的0～6月龄婴儿母亲，并进行问卷填写。纳入标准：（1）产后6个月内；（2）产妇具备正常语言沟通及理解能力者；（3）自愿参加本次调查者。排除标准：产妇有重大疾病者（重要脏器功能不全者）。
1.2　方法
1.2.1　抽样方法
据横断面调查样本量计算公式，α=0.05，既往研究贵阳市某区产后抑郁症状阳性率30.0%[9]，容许误差d为0.1p。无应答率为10%，故需要的样本量为1 050人。采用多阶段分层随机抽样，第一步将贵阳市按地域分区六区三县一市分层，每层按辖区社区数等比例抽样法随机抽取1～5个社区，共30个社区。第二步每个社区按服务顺序连续纳入约40名调查对象。
1.2.2　调查方法
调查前对社区卫生服务中心的妇幼保健医师进行注意事项告知，在现场通过发放电子调查问卷产妇自填、医师在旁协助的方式在规定时间完成调查。共发放问卷1 165份，剔除重复填写、连续10题选同一选项的问卷，有效调查问卷1 088份，有效率93.6%。
1.2.3　调查工具
1.2.3.1　一般情况资料
年龄、民族、文化程度、职业状况、家庭人均月收入、婚姻状况、生育次数、妊娠意向、产前抑郁病史、妊娠期并发症、分娩方式及孕期课程学习。
1.2.3.2　爱丁堡产后抑郁量表(Edinburgh postnatal depression scale, EDPS)
爱丁堡产后抑郁量表采用1998年Lee等翻译修订的中文版量表，共10个条目，采用0～3分计分法，总分0～30分，本研究以EPDS评分≥13分为阳性[10]。Cronbach α系数为0.812[11]。
1.2.3.3　产后抑郁素养量表
产后抑郁素养量表采用2020年Jorm等翻译修订的中文版，含6个维度：识别产后抑郁症状的能力（5个条目）、疾病风险因素和原因的知识（6个条目）、自我护理行为的知识和信念（5个条目）、可获得专业帮助的知识和信念（4个条目）、促进疾病认知及合理寻求帮助的态度（6个条目）、获取产后抑郁相关信息的知识（5个条目）。量表共31个条目，采用1～5分计分法，各维度/总分公式如下：分数=原始分数之和/条目数，总分1～5分，分数越高说明素养越高，Cronbach α系数为0.896[4]。
1.2.4　伦理声明
本研究经贵州省第三人民医院伦理委员会审批通过（审批号：2025年伦理第015号）。采用电子问卷调查，首页已告知研究内容、保密及自愿参与原则。研究对象均知情同意后自愿填写，信息严格保密，数据仅用于本次科研。
1.2.5　统计学分析
应用Excel和R 4.3.1进行数据处理和分析，计数资料以频数、百分比(%)描述，采用χ2检验和Kruskal-Wallis H检验；定量资料呈正态分布则以（）描述，进行t检验；呈非正态分布则以中位数和四分位数间距［M（P25，P75）］表达，影响因素分析采用logistic回归。采用Mplus 8.3进行潜在剖面分析，以产后抑郁素养6个维度为外显指标。模型适配指标：赤池信息准则(Akaike information criterion,AIC)、贝叶斯信息标准(Bayesian information criterion,BIC)、样本矫正的贝叶斯信息标准(adjusted Bayesian information criterion,aBIC)值越小代表模型拟合度越好；似然比检验(Vuong-Lo-Mendell-Robin likelihoood ratio test,LMR)及基于bootstrap的似然比检验(bootstrap likelihoood ratio test,BLRT)若P值显著,表示k个类别优于k-1个类别。熵值(Entropy)≈0.8时表明分类准确率超90%。检验水准α=0.05。
2　结果

2.1　一般情况
本次共调查1 088人，其中26～30岁人群占39.3%；本科及以上占48.7%；家庭人均月收入2 000～5 000元占48.7%；计划受孕者占61.7%；25人曾有过产前抑郁病史；仅19.7%的产妇接受过孕产妇学校课程学习。
2.2　不同产后抑郁症状组的产后抑郁素养得分
根据EPDS得分≥13分为抑郁症状阳性分组，产后抑郁症状阳性人数为158人，阳性率为14.5%，阳性组的素养得分低于非阳性组，差异具有统计学意义(P<0.05)。见表1。
2.3　产后抑郁素养潜在剖面分析及影响因素分析
2.3.1　产后抑郁素养总分及各维度得分
产妇产后抑郁素养总分为3.29(3.00,3.58)分，各维度得分由高到低分别为：自我护理行为的知识和信念为3.60(3.20,4.00)分、疾病风险因素和原因的知识为3.60(3.00,4.15)分、促进疾病认知及合理寻求帮助的态度为3.33(3.00,3.83)分、获取产后抑郁相关信息的知识为3.20(3.00,3.80)分、识别产后抑郁症状的能力为3.17(2.67,3.67)分、可获得专业帮助的知识和信念为3.00(3.00,3.50)分。
2.3.2　产后抑郁素养的潜在剖面分析
以1分类模型为基础，逐渐增加剖面数量，探索1～6分类的潜在剖面。随着模型数目增加，AIC、BIC、aBIC值逐渐减小，BLRT1～5分类模型均提示有统计学意义(P<0.05)。模型6的信息拟合指标值最小，但BLRT/P、LMR/P值均大于0.05，故排除。LMR在2～5分类模型中有统计学意义，但4、5模型中类别概率含<5%，故排除。剖面数为3时，Entropy≈0.8，且BLRT/P、LMR/P值均达到显著水平，AIC、BIC、aBIC值较低，综合考虑后将其分为3分类模型。见表2、图1。
2.3.3　产后抑郁素养潜在剖面的特点与分组
根据3种潜在类别产后抑郁素养总分由低到高的特点分别命名为：低素养组、中素养组、高素养组，占研究对象的5.97%、47.88%、46.14%。见表3。
2.3.4　产妇产后抑郁素养潜在类别的单因素分析
3组潜在类别产妇产后抑郁素养在年龄、民族、文化程度、职业、家庭人均月收入、生育次数、妊娠意向、课程学习差异有统计学意义(P<0.05)。见表4。
2.3.5　产妇产后抑郁素养潜在类别的logistic回归分析
以产妇产后抑郁素养潜在类别为因变量，单因素分析中差异有统计学意义的指标为自变量并以高赋值为参照，见表5。Logistic回归分析显示，与高素养组相比，年龄20～25岁(OR=3.282,95%CI:1.130～9.534)、初中及以下(OR=5.396,95%CI:2.402～12.120)/高中文化程度(OR=2.296,95%CI:1.080～4.880)、人均月收入<2 000元(OR=2.385,95%CI:1.050～5.420)是低素养组的危险因素(P<0.05)；初中及以下(OR= 5.102,95%CI:3.351～7.769)/高中文化程度(OR= 2.357,95%CI:1.668～3.330)、人均月收入2 000～5 000元(OR=1.610,95%CI:1.187～2.184)是中素养组的危险因素，而参与课程学习(OR=0.673,95%CI:0.472～0.959)为保护因素(P<0.05)；与中素养组相比，年龄<20岁(OR=5.978,95%CI:1.245～28.694)、职业状况(OR=2.186,95%CI:1.110～4.304)是低素养组的危险因素(P<0.05)。见表6。
3　讨论

既往研究报道，国内产后抑郁症状阳性率为3.5%～38.0%[1-2]，本研究结果显示贵阳市产后抑郁症状阳性率为14.5%，处于中等水平。产后抑郁是生理因素、心理因素、社会因素等综合原因所致[12-13]，不仅影响产妇产后康复，对婴幼儿生长发育影响也不可忽视[14]。本次调查中发现产后抑郁症状阳性组的抑郁素养得分低于非阳性组，产后抑郁素养是针对产后抑郁疾病的知识、信念和技能[4]。产妇作为特殊群体，良好的抑郁素养有助于产妇适应所处环境、获取心理支持，完成角色转换并减少不良情绪[15]，提高产妇产后抑郁素养水平对预防产后抑郁具有重要意义。
通过潜在剖面分析可识别不同剖面类型的特点和需求，为提供个性化支持提供指导。本研究通过此方法将产妇产后抑郁素养分为3个潜在类别：低素养组、中素养组、高素养组，3组总分及各维度得分存在差异，提示产妇产后抑郁素养存在个体差异。中素养组占比最高，素养总分为3.10(2.94,3.23)，提示多数产妇具备产后抑郁素养，但整体水平仍需提升。本次调查得分略高于陈飞等研究[16]，可能与本研究对象为城市户籍有关；但低于山东烟台市产妇抑郁素养得分3.56分[17]。贺丹等[6]在贵阳市的调查显示孕产妇总体健康素养具备率为29.0%，其中产妇得分高于孕妇。
对比发现三组产后抑郁症状阳性率无差异，但随素养升高递减。高水平组在产后抑郁的认知和自我护理行为的知识信念这两维度得分较高，说明高水平组的产妇能准确识别症状、区分正常情绪波动与病理性抑郁，且自我护理的能力、意愿和态度更优。但其专业帮助获取的知识信念维度得分偏低，与黄维健[17]的调查结果一致。调查显示，高水平组本科及以上、卫生相关专业占比较高，既往研究发现[18]文化程度较高者知识接受度更高，产后抑郁素养也更好；从事母婴相关职业的产妇因接触相关案例，对疾病表现与危害的认知更为深入[19]。中水平组的六个维度中“识别产后抑郁症状的能力”分数最低，调查发现该类人群年龄≤25岁占比较高、本科以上文化水平仅占32.1%，故对于该类人群应着重于产后抑郁症状的辨别、区分正常产后抑郁情绪波动与病理性抑郁状态。
低素养组在“识别产后抑郁症状的能力”“疾病风险因素和原因的知识”“自我护理行为的知识和信念”“获取产后抑郁相关信息的知识”这四维度得分均低于其他组，提示其产后抑郁相关认知、自我护理能力与态度较弱。结合调查结果，这与文化水平、收入偏低及生育年龄较小相关。研究表明文化程度影响产妇对知识的获取及理解能力[20]，文化水平越低，知识的知晓能力越低；低收入不仅限制产妇获取科学育儿知识和专业心理支持的途径，还会因经济压力加重对养育成本的焦虑[21]；而低生育年龄人群因社会阅历、育儿相关经验相对不足，对产后抑郁的认知有待提升[22]。但低素养组在“促进疾病认知及合理寻求帮助的态度”这个维度得分显著高于中、高素养组，提示该群体虽认知不足，但求助意愿更强，渴望获得专业帮助[23]。因此，在实际保健工作中，可将此类产妇列为产后抑郁健康教育重点人群，根据受教育程度、年龄差异调整心理健康教育方案，强化产后抑郁知识普及并提供专业支持，以降低产后抑郁发生。
本研究结果显示，年龄、文化程度、职业、人均月收入及课程学习是产妇产后抑郁素养潜在类别的影响因素。相关调查发现参与孕产妇学校可帮助产妇系统掌握相关知识、缓解焦虑等不良情绪，降低产后抑郁风险[24]。因此，医疗机构应健全孕产妇学校课程，拓宽知识获取渠道，以提升其健康素养。
本研究存在局限，横断面设计仅能揭示变量关联，无法推断因果；自陈量表收集数据或存在社会期许偏移；社区调研受实际条件限制未采用个体水平随机抽样，而是采用连续招募方式纳入对象，存在一定选择偏倚，虽保障地域代表性，但随机性不足。未来可通过纵向随机调查加以验证。
综上所述，产妇产后抑郁素养包括低、中、高三个潜在类别，针对低素养产妇，需加强产后抑郁相关心理健康宣教。妇女保健人员应重点关注低生育年龄、低文化、家庭经济水平低且未参与过相关课程学习的产妇。产妇健康素养提升应始于孕期健康教育，贯穿孕产期，提高产妇产后抑郁素养以预防产后抑郁。
