Objective

To evaluate the information quality of hypertension-related videos on the TikTok short video platform and assess their potential value for public health education.

Methods

In June 2024, we searched the TikTok short video platform using the keyword "hypertension" selecting the top 200 videos and ultimately including 177 videos for quality evaluation. Structured content integrity assessment tools and the DISCERN video quality assessment tool were used to evaluate the videos. The correlation between video quality and characteristics (duration, likes, comments, favorites, and shares) was studied.

Results

The 177 videos received 11 250 432 likes, 366 588 comments, 3 265 929 favorites, and 3 780 513 shares. The median video length was 111 seconds (74, 158), and the median upload days were 672 days (147, 878). 89.3% of the videos were posted by doctors, 8.5% by individual science communicators, and 2.3% by news agencies. 98.9% of the videos had subtitles, all featured individuals, and only 26.0% included animated demonstrations. Content integrity scores were: definition 0.19±0.40, symptoms 0.11±0.32, risk factors 0.29±0.50, assessment 0.36±0.45, management 0.81±0.47, and outcomes 0.63±0.58. 90.4% of the videos had a DISCERN score of ≤50, indicating "average" or below quality, with no significant difference in DISCERN scores among different publishers (P>0.05). DISCERN scores were significantly correlated with video duration (r=0.409, P<0.001).

Conclusion

The overall quality of hypertension-related videos on TikTok is low, with a significant positive correlation between video duration and quality. High-quality videos receive less attention, making TikTok an unsuitable platform for hypertension education due to low information quality and reliability.

目的

本研究旨在评价抖音短视频平台高血压相关视频的信息质量,并评估其对公众健康教育的潜在应用价值。

方法

2024年6月,以“高血压”为关键词在抖音短视频平台检索,筛选前200个视频,最终纳入177个视频进行质量评价。使用结构化内容完整性评估工具和DISCERN视频质量评估工具进行评价,并研究视频质量与视频特征(时长、点赞、评论、收藏和转发)之间的相关性。

结果

177个视频共获得11 250 432个点赞、366 588条评论、3 265 929次收藏和3 780 513次转发。视频时长中位数为111秒(74,158),上传天数中位数为672天(147,878)。89.3%的视频由医生发布,个人科学传播者占8.5%,新闻机构占2.3%。98.9%的视频有字幕,所有视频均有人物出现,仅有26.0%包含动画演示。内容完整性各维度评分:定义0.19±0.40、症状0.11±0.32、风险因素0.29±0.50、评估0.36±0.45、管理0.81±0.47和结局0.63±0.58。90.4%的视频DISCERN评分在≤50分,其质量为“一般”或以下;不同发布者之间的DISCERN评分无显著差异(P>0.05)。DISCERN评分与视频时长显著相关(r=0.409,P<0.001)。

结论

抖音短视频平台高血压相关视频整体质量较低,视频时长与质量之间显著正相关,而高质量视频的关注度较低。由于信息质量和可靠性较低,抖音短视频平台并不是一个适合用于高血压科普教育的知识来源。

目的

本研究旨在评价抖音短视频平台高血压相关视频的信息质量,并评估其对公众健康教育的潜在应用价值。

方法

2024年6月,以“高血压”为关键词在抖音短视频平台检索,筛选前200个视频,最终纳入177个视频进行质量评价。使用结构化内容完整性评估工具和DISCERN视频质量评估工具进行评价,并研究视频质量与视频特征(时长、点赞、评论、收藏和转发)之间的相关性。

结果

177个视频共获得11 250 432个点赞、366 588条评论、3 265 929次收藏和3 780 513次转发。视频时长中位数为111秒(74,158),上传天数中位数为672天(147,878)。89.3%的视频由医生发布,个人科学传播者占8.5%,新闻机构占2.3%。98.9%的视频有字幕,所有视频均有人物出现,仅有26.0%包含动画演示。内容完整性各维度评分:定义0.19±0.40、症状0.11±0.32、风险因素0.29±0.50、评估0.36±0.45、管理0.81±0.47和结局0.63±0.58。90.4%的视频DISCERN评分在≤50分,其质量为“一般”或以下;不同发布者之间的DISCERN评分无显著差异(P>0.05)。DISCERN评分与视频时长显著相关(r=0.409,P<0.001)。

结论

抖音短视频平台高血压相关视频整体质量较低,视频时长与质量之间显著正相关,而高质量视频的关注度较低。由于信息质量和可靠性较低,抖音短视频平台并不是一个适合用于高血压科普教育的知识来源。

Objective

To evaluate the information quality of hypertension-related videos on the TikTok short video platform and assess their potential value for public health education.

Methods

In June 2024, we searched the TikTok short video platform using the keyword "hypertension" selecting the top 200 videos and ultimately including 177 videos for quality evaluation. Structured content integrity assessment tools and the DISCERN video quality assessment tool were used to evaluate the videos. The correlation between video quality and characteristics (duration, likes, comments, favorites, and shares) was studied.

Results

The 177 videos received 11 250 432 likes, 366 588 comments, 3 265 929 favorites, and 3 780 513 shares. The median video length was 111 seconds (74, 158), and the median upload days were 672 days (147, 878). 89.3% of the videos were posted by doctors, 8.5% by individual science communicators, and 2.3% by news agencies. 98.9% of the videos had subtitles, all featured individuals, and only 26.0% included animated demonstrations. Content integrity scores were: definition 0.19±0.40, symptoms 0.11±0.32, risk factors 0.29±0.50, assessment 0.36±0.45, management 0.81±0.47, and outcomes 0.63±0.58. 90.4% of the videos had a DISCERN score of ≤50, indicating "average" or below quality, with no significant difference in DISCERN scores among different publishers (P>0.05). DISCERN scores were significantly correlated with video duration (r=0.409, P<0.001).

Conclusion

The overall quality of hypertension-related videos on TikTok is low, with a significant positive correlation between video duration and quality. High-quality videos receive less attention, making TikTok an unsuitable platform for hypertension education due to low information quality and reliability.

高血压(Hypertension)是全球性的公共卫生问题,WHO发布的首份高血压流行趋势综合分析报告显示,过去30年间,30~79岁成人高血压人数已从6.5亿人上升到12.8亿人[1]。我国高血压患病率为27.9%,患病人数超2.7亿,是城乡居民心脑血管疾病死亡的最重要危险因素,严重影响人民健康和经济社会发展[2]。尽管高血压这一医学术语众所周知,但我国高血压知晓率、治疗率和控制率分别为51.6%、45.8%和16.8%,仍低于全球平均水平[3]。因此,医学专业人员有必要帮助人民群众了解高血压的防治知识,指导其尽早采取科学、规范和有效的防控措施。
随着互联网技术的快速发展,短视频逐渐成为人们获取医学信息的主要方式之一[4]。“抖音”已在160多个国家/地区拥有11亿用户,用户平均每天在该平台上花费高达95分钟,表明“抖音”已成为人们日常生活的重要组成部分[5]。研究表明,社交媒体平台上的健康信息传播具有广泛的覆盖面和高效性,但内容质量参差不齐、信息不准确等问题也随之而来[6-7]。抖音短视频平台上关于高血压相关视频也尚未得到系统评估,本研究旨在评估抖音短视频平台上与高血压相关视频的特征、内容和质量,以期为高血压健康信息的制作和传播提供有价值的参考,促进更有效的健康教育和疾病管理。
2024年6月使用“高血压”为关键词在抖音短视频平台进行检索,按默认综合排序,发布时间设置为不限。评估范围限制在前200个视频,因为研究表明,用户通常浏览100到300个视频[8-9]。排除标准:(1)与高血压无关或缺乏教育信息的视频;(2)重复视频;(3)非中文视频;(4)非原创视频。
内容完整性评估,采用Goobie提出的编码方案[10],对视频内容完整性进行六个维度的评分:疾病定义、症状、风险因素、评估、管理和结果,各维度内容来自中文指南和共识[11-18]。内容完整性各维度按0-2分评分:0分(无内容)、0.5分(内容较少)、1分(部分内容)、1.5分(大部分内容)和2分(内容完整)。
信息可靠性与治疗可选择性评估,采用DISCERN工具评估[19],该工具包括16个条目,3个维度,维度1(条目1~8)评估视频的可靠性,维度2(条目9~15)关注治疗信息的质量,维度3(条目16)评估整体质量。每个条目评分为1~5分,DISCERN总分范围为16至80分,结果划分:非常差(16~26分)、差(27~38分)、一般(39~50分)、好(51~62分)和优秀(63~80分)。
由2名临床专家进行视频质量评估,均拥有超过10年的临床经验。在筛选和评分过程中,2人同时使用内容完整性评估工具和DISCERN工具对视频进行评价,评分通过讨论确定。若评分员1和评分员2之间存在评分差异,由仲裁员(评分员3)裁决并提供最终评分,最终所有评分达成一致。
基于抖音APP内的“排序依据”(默认排序、最多点赞、最新发布)和“按时长排序”(<60 s、60~300 s、>300s)功能,分析两种排序下的前30个视频,该方法确保分析集中在观看和互动最多的视频上[20]
运用SPSS 25.0进行数据描述性统计以及差异性检验,Graphpad 9.5.1进行作图。符合正态分布的数据采用表示,差异性检验采用单因素ANOVA分析,事后检验采用Bonferroni法[21];不符合正态分布的数据用MP25, P75)表示,差异性检验采用Kruskal-Wallis检验,事后检验采用Dunn’s事后检验[22];Spearman相关分析来评估定量变量之间的关系[23],检验水准为α=0.05。
收集了抖音短视频平台上前200个高血压相关视频,排除内容与主题无关的23个视频,177个视频被纳入评价,筛选过程见图1。截至数据收集日期,视频上传天数中位数为672天(147,878),视频时长中位数为111秒(74,158);共获得11 250 432个点赞、366 588条评论、3 265 929次收藏和3 780 513次转发。不同年份上传视频的详细信息及观看次数见表1
89.3%(158/177)的视频由医生发布,个人科学传播者发布占8.5%(15/177),新闻机构发布占2.3%(4/177)。98.9%(175/177)的视频有字幕,所有视频均有人物出现,仅有26.0%(46/177)的视频包含动画,24%(44/177)的视频有背景音乐。
雷达图直观显示了高血压相关视频在六个维度上的评分,其中,定义0.19±0.40、症状0.11±0.32、危险因素0.29±0.50、评估0.36±0.45、管理0.81±0.47和结局0.63±0.58。图2。不同年份症状维度评分差异统计学有统计学意义(P=0.008),其中2024年评分显著高于2021年评分(P<0.05),表2
160个视频(90.4%)的DISCERN评分≤50分,总体质量为“一般”或以下。其中,72个视频(40.7%)评分为“一般”,77个视频(43.5%)评分为“差”,11个视频(6.2%)评分为“非常差”。仅有17个视频(8.6%)的DISCERN评分达到“好”的水平。不同发布者视频的DISCERN评分之间差异无统计学意义(P>0.05)。表3
热图直观展示了不同发布者在各项DISCERN评分上的表现,医生发布的视频在“目标清晰”、“相关性”等条目评分较高,但在“治疗机制”、“治疗获益”、“治疗风险”等条目上也存在较大不足,新闻媒体和个人科学传播者发布的视频在“证据来源”和“详细审查”等标准上的评分较低,提示信息准确性和科学性不足。见图3所示。
视频的三种排序依据的DISCERN各维度评分差异均无统计学意义(P>0.05)。不同时长的视频的DISCERN各维度评分差异均有统计学意义(P<0.05),表4。时长大于300 s的视频DISCERN各维度评分均高于300 s及以下的视频,60~300 s的视频信息可靠性评分和总体评分高于短于60 s的视频。表4图4
点赞数与评论数、收藏数和转发数显著正相关(P<0.001),评论数与收藏数和转发数显著正相关(P<0.001)。上传天数与DISCERN总分、点赞数、评论数、收藏数和转发数显著负相关(P<0.05),详见表5
本研究纳入分析的177个视频,上传天数中位数为672天,总体较为久远;2021年至2024年每年的视频数量分别约占25%,不同年份视频数量分布较为均匀;视频的总点赞数为11 250 432,评论数为366 588,收藏数为3 265 929,转发数为3 780 513,表明这些视频的观看人数众多,潜在的社会影响力巨大,可能会影响公众对高血压的认知、预防和治疗态度。发布者类型中,医生占绝大多数,但其视频质量并不具有显著优势,总体质量依然较低,这与Ming的发现具有一致性[24],提示在视频内容制作和发布过程中仍有改进空间,以进一步提高观众的满意度和接受度。需要注意的是,普通观众难以判断视频的真实质量和科学性[25],因此,医生发布者的专业背景虽然重要,但应注重提升视频内容的实际质量,确保信息准确、科学,同时应更多的运用图片、动画等形式使内容易于理解,以便更好地服务大众,提高视频的教育价值和影响力。
抖音短视频平台高血压相关视频内容完整性显著不足,大部分视频在定义、症状、评估和危险因素这四个关键维度上缺失内容的比例分别为78.1%、88.8%、56.7%和69.1%。这表明视频在提供全面、详细的高血压相关信息方面存在缺陷。雷达图直观展示了视频完整性各维度的量化评分情况,各维度评分均小于1分,这表明绝大多数视频在这些维度上的内容完整性严重不足。进一步分析不同年份视频内容完整性评分的比较发现,症状维度在不同年份之间存在统计学差异(P=0.008),且2024年的评分显著高于2021年(P<0.05)。然而,其他维度在各年份之间并无显著差异(P>0.05)。这表明尽管症状维度在2024年有所改善,但整体上视频内容的完整性仍需大幅提升。通过对视频内容完整性的深入分析,部分视频在管理和结局信息方面做得较好,但仍需在基础知识和风险因素的科普上投入更多资源,以提高视频的科普价值和社会影响力。未来的工作应更均衡地提升各维度内容的全面性,确保观众能够获得更完整、更科学的高血压知识,以促进公众健康意识的提升。
依据DISCERN评价结果,177个视频中,43.5%的视频质量为“差”,40.7%的视频质量为“一般”,仅有7.9%和1.7%的视频质量达到了“好”和“非常好”的标准,说明整体上抖音短视频平台上高血压视频质量不高。为提高视频内容的准确性和可靠性,抖音短视频平台可以制定并实施医学视频的同行评审系统。在DISCERN标准下,医生、个人科学传播者和新闻媒体发布的高血压相关视频,质量差异不显著(P=0.229),主要原因为内容同质化程度高和缺乏信息深度。不同发布者在内容选择上都集中在一些基础性的信息,内容同质化程度高。此外,创作者为了获得更多的曝光和互动,往往选择制作简短且易于理解的视频,忽略了信息深度。根据排序方式,最受点赞的视频略高于默认排序和最新发布的视频,但差异并不显著(P=0.519)。然而,不同时长的视频评分存在显著差异(P<0.05)。时长超过300 s的视频在信息可靠性、治疗可选择性、总体评价和总分上均显著高于60 s以下的视频(P<0.001)。这表明较长时长的视频往往具有更高的内容质量。
DISCERN总分与点赞数、评论数、收藏数和转发数之间的相关性弱,视频质量与这些互动指标之间没有显著的直接关系(P>0.05)。观众可能更倾向于点赞多和分享内容有趣或易于理解的视频,而不一定是质量高的视频质量,同时表明抖音的算法更倾向于优先考虑互动指标,观众可能难以识别可信信息,从而导致错误的健康决策[26]。点赞数与评论数、收藏数和转发数之间存在显著的正相关(P<0.001),收藏数与转发数之间亦显著相关(P<0.001)。这些结果表明,视频的互动指标之间存在显著的正相关,观众在互动行为上具有一致性。DISCERN总分与上传天数之间存在显著负相关(P=0.002),表明较新的视频质量更高。上传天数与点赞数、评论数、收藏数和转发数之间也存在负相关,说明较新的视频在互动指标上表现较好。视频时长与DISCERN总分之间存在显著正相关(P<0.001),表明较长的视频质量较高[27]。制作较长且内容全面的视频,以及及时更新,可能有助于提高视频质量和观众互动。这与以往的研究发现一致,较长的视频在内容完整性和质量上更有优势[28]。建议未来视频制作时注重内容全面性和时效性,同时关注如何提高视频的趣味性和易理解性,以提升观众的互动和满意度。
本研究的优势在于全面分析了抖音短视频平台上高血压相关视频的质量和互动性,揭示了视频质量与受欢迎度之间的关系。然而,本研究也存在局限性。首先,研究仅限于抖音短视频平台,结果可能不适用于其他社交媒体平台。其次,研究采用的样本量较小,可能无法完全代表抖音上的所有高血压相关视频。最后,研究主要依赖于定量分析,缺乏对视频内容质量的深入定性分析,未来研究应结合两者以获得更全面的理解。
抖音短视频平台高血压相关视频整体质量较低,表明抖音短视频平台上的高血压医学信息目前不足以指导大众做出准确的健康决策。通过平台级干预和改进内容发布标准,对于提高抖音短视频平台上医学视频的教育价值至关重要。
