Objective

To analyze the trends in incidence of acutemyocardialinfarction(AMI)in Wuhan from 2016 to 2023, and to provide scientific basisfor prevention and control of AMI in Wuhan.

Methods

The incidence data during 2016—2023 from the Surveillance System of cardiovascular diseases in Wuhan were collected to compute the incidence, age-standardized rate by China population 2010 (ASR-C) and by Segi’s world standard population(ASR-W).Theannual percent change (APC) and average annual percent change (AAPC) of incidence of AMI were evaluated using Joinpoint regression model,to analyzethe incidence trends of AMI.

Results

The incidence of AMI in Wuhan increased from 56.67/105 in 2016 to 115.29/105 in 2023(APC=10.24%,P<0.05);andASR-C of AMI increased from 40.45/105 in 2016 to 79.57/105 in 2023(APC=9.75%,P<0.05).The AMI incidence of males(83.65/105) was significantly higherthanthat of females(34.99/105) from 2016 to 2023 (P<0.05).The AMI incidenceofurban area(62.59/105) was significantly higher than that of rural area(55.35/105)(P<0.05).The regression model showed that the incidence and ASR-C of AMI in male and female all showed a significant upward trend (all P<0.05);the incidence and ASR-C of AMI of urban residents all showed a significant upward trend (all P<0.05), while the trends of rural residents from 2016 to 2020 were not statistically significant, but they all showed a significantupward trend from 2020 to 2023 (all P<0.05).The incidence of AMI in age groups<30,30-<40,40-<50, 50-<60, 60-<70 years and≥70 years in Wuhan increased at different rates from 2016 to 2023(P<0.05).Incidence in agegroups 30-<40,40-<50 years increased rapidly,with the AAPCs of 16.73% and 13.87%, respectively.

Conclusion

The incidence of AMI in Wuhan showedan upward trend from2016 to 2023,young adult males are the crucial population for prevention andcontrol.

目的

分析2016—2023年武汉市急性心肌梗死(acutemyocardial infarction,AMI)发病率及变化趋势,为武汉市开展AMI防控工作提供科学依据。

方法

收集武汉市心脑血管疾病监测系统2016—2023年AMI发病数据,计算粗发病率、中标发病率、世标发病率,采用Joinpoint回归模型来估算AMI的年度变化百分比(APC)和平均年度变化百分比(AAPC)。

结果

武汉市AMI发病率从2016年56.67/10万上升至2023年115.29/10万(APC=10.24%,P<0.05),中标发病率从2016年40.45/10万上升至2023年79.57/10万(APC=9.75%,P<0.05),2016—2023年武汉市男性AMI中标发病率(83.65/10万)高于女性(34.99/10万),城市(62.59/10万)高于农村(55.35/10万),差异均具有统计学意义(P<0.05)。Joinpoint回归模型分析结果显示,2016—2023年武汉市男女性AMI发病率、中标发病率均呈明显上升趋势(均有P<0.05);城市居民发病率、中标发病率均呈明显上升趋势(均有P<0.05),农村居民发病率、中标发病率2016—2020年的变化趋势均无统计学意义,2020—2023年均呈明显上升趋势(均有P<0.05);2016—2023年武汉市<30岁、30~<40岁、40~<50岁、50~<60岁、60~<70岁和≥70岁年龄组居民AMI发病率以不同的速度上升,趋势均有统计学意义(均有P<0.05),其中,30~<40岁、40~<50岁年龄组居民发病率上升速度较快,AAPC分别为16.73%和13.87%。

结论

2016—2023年武汉市AMI发病率呈逐年上升趋势,青壮年男性是重点防控人群。

目的

分析2016—2023年武汉市急性心肌梗死(acutemyocardial infarction,AMI)发病率及变化趋势,为武汉市开展AMI防控工作提供科学依据。

方法

收集武汉市心脑血管疾病监测系统2016—2023年AMI发病数据,计算粗发病率、中标发病率、世标发病率,采用Joinpoint回归模型来估算AMI的年度变化百分比(APC)和平均年度变化百分比(AAPC)。

结果

武汉市AMI发病率从2016年56.67/10万上升至2023年115.29/10万(APC=10.24%,P<0.05),中标发病率从2016年40.45/10万上升至2023年79.57/10万(APC=9.75%,P<0.05),2016—2023年武汉市男性AMI中标发病率(83.65/10万)高于女性(34.99/10万),城市(62.59/10万)高于农村(55.35/10万),差异均具有统计学意义(P<0.05)。Joinpoint回归模型分析结果显示,2016—2023年武汉市男女性AMI发病率、中标发病率均呈明显上升趋势(均有P<0.05);城市居民发病率、中标发病率均呈明显上升趋势(均有P<0.05),农村居民发病率、中标发病率2016—2020年的变化趋势均无统计学意义,2020—2023年均呈明显上升趋势(均有P<0.05);2016—2023年武汉市<30岁、30~<40岁、40~<50岁、50~<60岁、60~<70岁和≥70岁年龄组居民AMI发病率以不同的速度上升,趋势均有统计学意义(均有P<0.05),其中,30~<40岁、40~<50岁年龄组居民发病率上升速度较快,AAPC分别为16.73%和13.87%。

结论

2016—2023年武汉市AMI发病率呈逐年上升趋势,青壮年男性是重点防控人群。

Objective

To analyze the trends in incidence of acutemyocardialinfarction(AMI)in Wuhan from 2016 to 2023, and to provide scientific basisfor prevention and control of AMI in Wuhan.

Methods

The incidence data during 2016—2023 from the Surveillance System of cardiovascular diseases in Wuhan were collected to compute the incidence, age-standardized rate by China population 2010 (ASR-C) and by Segi’s world standard population(ASR-W).Theannual percent change (APC) and average annual percent change (AAPC) of incidence of AMI were evaluated using Joinpoint regression model,to analyzethe incidence trends of AMI.

Results

The incidence of AMI in Wuhan increased from 56.67/105 in 2016 to 115.29/105 in 2023(APC=10.24%,P<0.05);andASR-C of AMI increased from 40.45/105 in 2016 to 79.57/105 in 2023(APC=9.75%,P<0.05).The AMI incidence of males(83.65/105) was significantly higherthanthat of females(34.99/105) from 2016 to 2023 (P<0.05).The AMI incidenceofurban area(62.59/105) was significantly higher than that of rural area(55.35/105)(P<0.05).The regression model showed that the incidence and ASR-C of AMI in male and female all showed a significant upward trend (all P<0.05);the incidence and ASR-C of AMI of urban residents all showed a significant upward trend (all P<0.05), while the trends of rural residents from 2016 to 2020 were not statistically significant, but they all showed a significantupward trend from 2020 to 2023 (all P<0.05).The incidence of AMI in age groups<30,30-<40,40-<50, 50-<60, 60-<70 years and≥70 years in Wuhan increased at different rates from 2016 to 2023(P<0.05).Incidence in agegroups 30-<40,40-<50 years increased rapidly,with the AAPCs of 16.73% and 13.87%, respectively.

Conclusion

The incidence of AMI in Wuhan showedan upward trend from2016 to 2023,young adult males are the crucial population for prevention andcontrol.

急性心肌梗死(acutemyocardial infarction,AMI)是在冠状动脉狭窄、硬化的基础上,产生了一系列的生理、病理变化而导致心肌的缺血缺氧,诱发心肌坏死等表现[1]。AMI起病急,病情重且病死率高,严重损害居民健康,并且会造成巨大的社会经济负担[2]。《中国心血管健康与疾病报告2022》指出,在2004—2020年期间,中国居民AMI住院总费用以年均24.66%的速度逐年增高[3]。考虑到血清总胆固醇、血压升高,糖尿病和主动吸烟人群的增加,预测2010—2030年我国心血管疾病的发生风险每年将会增加23%,将增加大约2 130万例心血管事件以及770万例死亡[4]。随着我国人口老龄化带来的生理年龄结构变化,AMI的疾病负担将进一步加重[5]。目前武汉市尚缺乏有关AMI发病及趋势变化的研究,本文通过分析2016—2023年武汉市AMI的发病情况以及发病率变化趋势,为后续评估武汉市AMI防控效果,完善AMI防控策略,科学开展AMI防控工作提供依据。
数据来源于武汉市心脑血管监测系统,本次研究包含确诊日期在2016—2023年间的所有AMI发病数据。病例参照《国际疾病分类(第十次修订本)》ICD-10进行编码,AMI编码为I21~I22。AMI发病以28天为时限,按发病事件来计算,28天内即使多次发作也只记为1次事件,28天后若再次急性发作,则记为1次新的发病事件[6]。武汉市历年户籍人口数据来源于武汉市公安局。本研究中将武汉市辖9个区(江岸区、江汉区、汉阳区、硚口区、武昌区、洪山区、青山区、东湖风景区、东湖高新区)界定为城市,将其余6个区(东西湖区、蔡甸区、经开汉南区、黄陂区、新洲区、江夏区)界定为农村。
具有诊断能力的医疗机构接诊医生为AMI的责任报告人,上报的AMI病例由医疗机构公共卫生科、区疾病预防控制中心(以下简称CDC)、市CDC进行三级审核,报病医疗机构定期按要求针对漏报、错报和重报问题开展自查工作,市、区两级CDC每个季度开展对医疗机构AMI报告质量的联合督导考核工作。武汉市CDC将AMI监测纳入年终考核,每季度核查AMI监测数据质量,并对核查结果进行通报。
采用Excel 2017整理数据,分别采用全国第六次人口普查、1960Segi世界标准人口数据来计算中国人口标化率和世界人口标化率。
采用Joinpoint 5.1.0软件拟合对数线性模型,对发病率进行对数变换,根据曲线拟合效果选择软件推荐连接点个数,将每条曲线分割为“连接点个数+1”段。计算年度变化百分比(APC)和平均年度变化百分比(AAPC),AAPC是对各段的APC按照每段线段的长度进行几何加权得到的结果。其中,当APC>0,表示曲线呈增长趋势;当APC=0,表示曲线无明显变化;当APC<0,表示曲线呈下降趋势。如果APC=AAPC表示发病率呈单调上升/下降趋势。根据双侧t检验分布计算出APCAAPC双侧检验的P值,以α=0.05为检验水准来检验趋势是否具有统计学意义[7-8]
2016—2023年武汉市共登记户籍新发AMI事件60 006例,AMI粗发病率、中标发病率、世标发病率分别为83.21/10万、58.10/10万、43.47/10万。男性新发AMI 40 994例,女性19 012例,男性粗发病率、中标发病率、世标发病率分别为110.14/10万、80.66/10万、63.47/10万;女性粗发病率、中标发病率、世标发病率分别为55.35/10万、35.39/10万、23.93/10万。城市粗发病率、中标发病率、世标发病率分别为92.78/10万、61.73/10万、46.27/10万;农村粗发病率、中标发病率、世标发病率分别为70.55/10万、53.40/10万、39.83/10万。AMI发病率随年龄的增长而上升,30岁前发病率低于10/10万,40岁后快速上升,85岁及以上年龄组达到高峰,男女性发病率分别为862.16/10万和753.51/10万。见表1图1
2016—2023年武汉市AMI粗发病率、中标发病率、世标发病率均呈明显上升趋势(P<0.05)。男性粗发病率、中标发病率、世标发病率均呈明显上升趋势(P<0.05);女性粗发病率、中标发病率、世标发病率均呈明显上升趋势(P<0.05),男性粗发病率、中标发病率、世标发病率的增长幅度均高于女性。城市居民粗发病率、中标发病率、世标发病率均呈明显上升趋势(P<0.05);农村居民粗发病率、中标发病率、世标发病率2016-2023年整体均呈上升趋势(P<0.05),但是分段回归结果显示:2016—2020年的变化趋势均无统计学意义(P>0.05),2020—2023年均呈明显上升趋势,APC分别为16.71%(t=4.10,P<0.05)、15.36%(t=3.42,P<0.05)、15.78%(t=3.44,P<0.05)。见表2图2
2016—2023年各年龄组人群AMI发病率均呈上升趋势,变化趋势均有统计学意义(P<0.05),其中30~<40岁年龄组人群AMI发病率的增长幅度最大。男性各年龄组人群AMI发病率均呈上升趋势,变化趋势均有统计学意义(P<0.05),其中30~<40岁年龄组人群AMI发病率的增长幅度最大。女性50岁以下年龄组人群AMI发病率变化趋势均无统计学意义(P>0.05),50~<60岁、60~<70岁和≥70岁年龄组人群AMI发病率均呈上升趋势(P<0.05),其中40~<50岁年龄组人群2020—2023年AMI发病率的增长幅度最大。城市<30岁人群AMI发病率变化趋势无统计学意义(P>0.05),30岁及以上年龄组人群AMI发病率均呈上升趋势(P<0.05),其中30~<40岁年龄组人群AMI发病率的上升幅度最大。农村<30年龄组、≥70岁年龄组人群AMI发病率呈上升趋势(P<0.05);30~<40岁年龄组人群2016—2018年AMI发病率变化趋势无统计学意义(P>0.05),2018—2023年呈上升趋势(APC=30.39%,P<0.05);50~<60岁、60~<70岁年龄组人群2016—2020年AMI发病率变化趋势无统计学意义(P>0.05),2020—2023年呈上升趋势(P<0.05)。见表3
本研究结果显示,2016—2023年武汉市AMI平均发病率为84.98/10万,中标发病率为59.41/10万,世标发病率为44.58/10万。中标发病率低于2013—2018年深圳市中标发病率(88.78/10万)[2],高于2014—2021年杭州市中标发病率(26.31/10万~33.26/10万)[9]、2012—2018年重庆市中标发病率(24.52/10万~47.74/10万)[10]、2014—2020年青岛市中标发病率(54.71/10万)[11]。这可能与不同地区经济发展水平、生活压力、人口年龄结构、饮食结构、生活方式、AMI相关防控措施及医疗服务等差异较大有关,也可能因为监测年份不同导致的发病率差异。
武汉市AMI中标发病率男性高于女性,与杭州市[9]、重庆市[10]、青岛市[11-12]、天津市[13]、上海市普陀区[14]、北京市延庆区[15]等地的研究结果一致。男性与女性发病率及中标发病率均呈明显的上升趋势,男性增长幅度高于女性。相关研究指出,男性暴露于吸烟、饮酒、缺乏身体活动、不合理膳食、生活压力大、高胆固醇、高血压等AMI的危险因素的比例高于女性[16-18]。武汉市AMI中标发病率城市高于农村,与杭州市[9]、重庆市[10]、密山市[19]等地区研究结果相反。可能与武汉市城市居民缺乏体育锻炼、不合理膳食、生活压力大有关,同时,也可能与城市医疗机构诊断能力、信息化程度更高导致报告率更高有关。
在本研究中,2016—2023年武汉市30岁以下年龄组AMI发病率较低,发病率随着年龄增长明显升高,85岁以上年龄组发病率最高。可能因为随着年龄增长危险因素的长期积累导致AMI发生风险升高,也可能因为随着年龄的增长,血管老化,血管壁发生动脉粥样硬化,管腔狭窄,血管壁弹性下降,舒张功能下降导致AMI发生风险升高。我国老龄化进程正在加速,需要及时采取有效措施预防AMI的发生,老年人群体在未来很长一段时间依旧是武汉市AMI重点干预和保护对象。2016—2023年间,各年龄组的发病率每年以5.70%~16.73%的增幅在增长,尤其是30~<40岁年龄组增长幅度最大,2016—2023年男性30~<40岁年龄组以17.73%的增幅增长,2020—2023年女性40~<50岁年龄组以20.68%的增幅增长,2016—2023年城市居民30~<40岁年龄组以13.32%的增幅增长,2018—2023年农村居民30~<40岁年龄组以30.39%的增幅增长,可能与中青年人群工作、生活压力大,久坐不动有关。AMI发病年龄前移,呈现年轻化趋势,提示在重点关注武汉市老年人群AMI发病状况的同时,也应该针对不同年龄组人群,采取针对性的防控措施,做好AMI的防控工作。
2016—2023年武汉市AMI中标发病率呈逐年上升趋势,从40.45/10万上升至79.57/10万,年均增长9.75%,武汉市AMI中标发病率变化趋势与深圳市[2]、杭州市[9]、重庆市[10]、青岛市一致[11],但与天津市[13]的变化趋势相反。随着武汉市急性心脑血管疾病网络直报系统以及考核体系的不断完善,医疗机构的诊断能力及报告能力不断提高,武汉市AMI的报告质量及数量也在不断提高。同时,人口老龄化的发展,高血压、糖尿病、高脂血症等患病率越来越高[20],均可能是导致武汉市AMI发病率逐年升高的原因。城市居民2016—2023年中标发病率呈明显上升趋势,农村居民2016—2020年无明显变化趋势,2020—2023年呈明显上升趋势,且增长幅度高于城市,可能与武汉市农村居民高血压知晓率、控制率低于城市有关[21],也可能与农村居民相对缺乏健康意识,吸烟、有害饮酒以及盐摄入过多等不健康的生活方式有关。
综上所述,2016—2023年武汉市AMI发病率呈逐年上升的趋势,30~<50岁人群的上升趋势更为明显,男性发病情况相比女性更为严重,城市发病情况比农村严重。需要重点关注武汉市男性、中青年人群的发病情况。在健康武汉行动背景下,加大AMI健康宣教力度,引导居民戒烟限酒、改善不健康的生活方式,倡导合理膳食、适度运动等,对30岁及以上人群进行早期筛查和综合干预,提高AMI知晓率、控制率,降低AMI发病风险及增长速度。
