Objective

To analyze the data of public consultation events and corresponding online public opinion on disease prevention and control in Chengdu from 2017 to 2023, so as to provide reference for understanding the public’s demand for public health information and reducing the occurrence of adverse public opinion in disease prevention and control and response to emergency public health events.

Methods

Collect information on public consultation events related to Chengdu Center for Disease Control and Prevention from 2017 to 2023, and use Baidu search tool “Baidu Index” to collect public opinion monitoring data during the same period. Statistical analysis was conducted on the distribution of public consultation events and their correlation with the Baidu Index of corresponding keywords using methods such as statistical description, chi square test, and linear correlation.

Results

From 2017 to 2023, a total of 7 253 public consultation incidents were received, with the highest number in 2022 (4 915); 12345 hotline platform calls are the main source, accounting for 58.78% (4 263 cases); The main types of consultations from 2017 to 2019 were vaccination issues; From 2020 to 2023, the consultation related to COVID-19 will account for 90.41% (5 952/6 583), and the questions related to the prevention and control of COVID-19 (χ2=1 092.147, P<0.001) and COVID-19 nucleic acid detection (χ2=45.083, P<0.001) in 2022 have increased respectively compared to 2020-2021; In 2023, the proportion of non-COVID-19 related vaccination issues and disease prevention and control issues will decrease (χ2=459.109, P<0.001) and increase (χ2=13.999, P<0.001) respectively compared with that before COVID-19; Linear correlation analysis showed that the number of questions related to vaccination between the “vaccination” search index (r=0.864, P=0.027), the number of questions related to COVID-19 prevention and control between the “COVID-19” search index (r=0.794, P=0.033) and information index (r=0.872, P=0.010), the number of questions related to COVID-19 vaccination between the “COVID-19 vaccine” search index (r=0.840, P=0.018) and information index (r=0.835, P=0.019), and the number of questions related to COVID-19 nucleic acid detection between the “nucleic acid detection” information index (r=0.972, P<0.001) were positively correlated respectively.

Conclusion

Using public health professional knowledge to answer questions for the public is a targeted form of health education. In daily work, we should pay more attention to public opinion monitoring, understand public needs through multiple channels, strengthen public opinion guidance and risk prediction, continue to carry out publicity and education, and improve public health literacy; When an emergency public health event occurs, it is necessary to promptly disclose the epidemic situation, strengthen prevention and control propaganda and guidance, inform the public of response methods, and continuously optimize prevention and control measures to provide support for disease prevention and control and response to emergency public health events.

目的

通过分析2017—2023年成都市疾病预防控制相关群众咨询事件及相应网络舆情数据,为了解群众公共卫生健康信息需求,减少疾病防控和突发公共卫生事件应对中不良舆情的发生提供参考。

方法

收集成都市疾病预防控制中心2017—2023年业务相关群众咨询事件信息,并使用百度搜索工具“百度指数”收集同时段舆情监测数据,采用统计描述、 χ2检验和直线相关等方法对群众咨询事件的分布及其与相应关键词的百度指数相关性进行统计分析。

结果

2017—2023年共接到疾病预防控制相关群众咨询事件7 253件,2022年数量最多(4 915件);市长公开电话/12345热线平台来电为咨询事件的主要来源,占58.78%(4 263件);2017—2019年咨询类别主要为疫苗/预防接种相关问题;2020—2023年与新型冠状病毒感染相关的咨询占比达90.41%(5 952/6 583),2022年新冠疫情防控相关问题(χ2=1 092.147,P<0.001)及新冠核酸检测相关问题(χ2=45.083,P<0.001)咨询占比较2020—2021年均有升高;2023年非新冠相关的疫苗/预防接种问题及疾病预防控制问题咨询占比较新冠发生前分别降低(χ2=459.109,P<0.001)和升高(χ2=13.999,P<0.001);直线相关分析显示,疫苗/预防接种相关问题咨询数量与“预防接种”搜索指数之间(r=0.864,P=0.027)、新冠疫情防控相关问题咨询数量与“新冠疫情”搜索指数(r=0.794,P=0.033)及资讯指数(r=0.872,P=0.010)之间、新冠疫苗接种相关问题咨询数量与“新冠疫苗”搜索指数(r=0.840,P=0.018)及资讯指数(r=0.835,P=0.019)之间、新冠核酸检测相关问题咨询数量与“核酸检测”资讯指数之间(r=0.972,P<0.001)均分别存在正相关。

结论

采用公共卫生专业知识为群众答疑解惑是一种有针对性的健康宣教形式,在日常工作中要更加重视舆情监测,多渠道了解群众信息需求,加强舆论引导和风险预判,持续做好宣传教育,提升群众健康素养;出现突发公共卫生事件时要及时公布疫情详细情况,加强防治宣传引导,告知群众应对方法并不断优化完善防控措施,为疾病防控和突发公共卫生事件应对提供支持。

目的

通过分析2017—2023年成都市疾病预防控制相关群众咨询事件及相应网络舆情数据,为了解群众公共卫生健康信息需求,减少疾病防控和突发公共卫生事件应对中不良舆情的发生提供参考。

方法

收集成都市疾病预防控制中心2017—2023年业务相关群众咨询事件信息,并使用百度搜索工具“百度指数”收集同时段舆情监测数据,采用统计描述、 χ2检验和直线相关等方法对群众咨询事件的分布及其与相应关键词的百度指数相关性进行统计分析。

结果

2017—2023年共接到疾病预防控制相关群众咨询事件7 253件,2022年数量最多(4 915件);市长公开电话/12345热线平台来电为咨询事件的主要来源,占58.78%(4 263件);2017—2019年咨询类别主要为疫苗/预防接种相关问题;2020—2023年与新型冠状病毒感染相关的咨询占比达90.41%(5 952/6 583),2022年新冠疫情防控相关问题(χ2=1 092.147,P<0.001)及新冠核酸检测相关问题(χ2=45.083,P<0.001)咨询占比较2020—2021年均有升高;2023年非新冠相关的疫苗/预防接种问题及疾病预防控制问题咨询占比较新冠发生前分别降低(χ2=459.109,P<0.001)和升高(χ2=13.999,P<0.001);直线相关分析显示,疫苗/预防接种相关问题咨询数量与“预防接种”搜索指数之间(r=0.864,P=0.027)、新冠疫情防控相关问题咨询数量与“新冠疫情”搜索指数(r=0.794,P=0.033)及资讯指数(r=0.872,P=0.010)之间、新冠疫苗接种相关问题咨询数量与“新冠疫苗”搜索指数(r=0.840,P=0.018)及资讯指数(r=0.835,P=0.019)之间、新冠核酸检测相关问题咨询数量与“核酸检测”资讯指数之间(r=0.972,P<0.001)均分别存在正相关。

结论

采用公共卫生专业知识为群众答疑解惑是一种有针对性的健康宣教形式,在日常工作中要更加重视舆情监测,多渠道了解群众信息需求,加强舆论引导和风险预判,持续做好宣传教育,提升群众健康素养;出现突发公共卫生事件时要及时公布疫情详细情况,加强防治宣传引导,告知群众应对方法并不断优化完善防控措施,为疾病防控和突发公共卫生事件应对提供支持。

Objective

To analyze the data of public consultation events and corresponding online public opinion on disease prevention and control in Chengdu from 2017 to 2023, so as to provide reference for understanding the public’s demand for public health information and reducing the occurrence of adverse public opinion in disease prevention and control and response to emergency public health events.

Methods

Collect information on public consultation events related to Chengdu Center for Disease Control and Prevention from 2017 to 2023, and use Baidu search tool “Baidu Index” to collect public opinion monitoring data during the same period. Statistical analysis was conducted on the distribution of public consultation events and their correlation with the Baidu Index of corresponding keywords using methods such as statistical description, chi square test, and linear correlation.

Results

From 2017 to 2023, a total of 7 253 public consultation incidents were received, with the highest number in 2022 (4 915); 12345 hotline platform calls are the main source, accounting for 58.78% (4 263 cases); The main types of consultations from 2017 to 2019 were vaccination issues; From 2020 to 2023, the consultation related to COVID-19 will account for 90.41% (5 952/6 583), and the questions related to the prevention and control of COVID-19 (χ2=1 092.147, P<0.001) and COVID-19 nucleic acid detection (χ2=45.083, P<0.001) in 2022 have increased respectively compared to 2020-2021; In 2023, the proportion of non-COVID-19 related vaccination issues and disease prevention and control issues will decrease (χ2=459.109, P<0.001) and increase (χ2=13.999, P<0.001) respectively compared with that before COVID-19; Linear correlation analysis showed that the number of questions related to vaccination between the “vaccination” search index (r=0.864, P=0.027), the number of questions related to COVID-19 prevention and control between the “COVID-19” search index (r=0.794, P=0.033) and information index (r=0.872, P=0.010), the number of questions related to COVID-19 vaccination between the “COVID-19 vaccine” search index (r=0.840, P=0.018) and information index (r=0.835, P=0.019), and the number of questions related to COVID-19 nucleic acid detection between the “nucleic acid detection” information index (r=0.972, P<0.001) were positively correlated respectively.

Conclusion

Using public health professional knowledge to answer questions for the public is a targeted form of health education. In daily work, we should pay more attention to public opinion monitoring, understand public needs through multiple channels, strengthen public opinion guidance and risk prediction, continue to carry out publicity and education, and improve public health literacy; When an emergency public health event occurs, it is necessary to promptly disclose the epidemic situation, strengthen prevention and control propaganda and guidance, inform the public of response methods, and continuously optimize prevention and control measures to provide support for disease prevention and control and response to emergency public health events.

公共卫生安全是国家安全的重要体现,是最根本的民生[1]。12345热线等网络理政平台是党和政府联系群众的桥梁、倾听群众呼声的窗口。以疾控中心为代表的公共卫生机构提供的政务服务主要是回应群众对于公共卫生的各种关切,解决群众的各类公共卫生需求[2]。近年来,突发公共卫生事件频发,媒体和群众对公共卫生领域的关注度也日益提高,特别是新冠疫情的发生引起了极高的舆情关注度,群众咨询需求日益凸显。有效挖掘和分析此类信息,具有重要的现实意义[3-5]。本文通过分析2017—2023年成都市疾病预防控制中心疾病防控相关群众咨询事件及网络舆情数据,以期为及时了解群众公共卫生健康信息需求,切实提高服务群众效能,减少不良舆情事件的发生及扩散提供优化建议,也为疾病预防控制策略的制定和突发公共卫生事件的应对提供支持和参考。
收集成都市疾病预防控制中心2017—2023年业务相关群众咨询事件信息并按咨询时间、咨询件来源、咨询内容所属类别等进行整理,同时,使用百度搜索工具“百度指数”[6]的数据作为舆情分析参数,选取“预防接种”、“疾病预防”、“新冠疫情”、“新冠疫苗”、“核酸检测”等5个关键词进行同时段全国范围搜索指数和资讯指数信息的收集、整理。
根据“百度指数”[6]释义,搜索指数是以网民在百度的搜索量为数据基础,以关键词为统计对象,科学分析并计算出各个关键词在百度网页搜索中搜索频次的加权和,显示互联网用户对关键词搜索关注程度及持续变化情况。资讯指数以百度智能分发和推荐内容数据为基础,显示新闻资讯在互联网上对特定关键词的关注及报道程度及持续变化,将网民的阅读、评论、转发、点赞、不喜欢等行为的数量加权求和、指数化处理后得出,全面衡量网民对智能分发和推荐内容的被动关注程度。资讯指数连同搜索指数形成闭环,用“主动搜索+内容关注”来诠释网民对某一话题的关注程度。
应用SPSS 22.0软件对数据进行统计分析。群众咨询事件的分布情况以频数、构成比等描述,组间比较采用χ2检验;对群众咨询事件与相应百度指数进行直线相关分析。检验水准α=0.05。
2017—2023年接到并回复业务相关群众咨询事件共7 253件,具体分别为215件、253件、202件、441件、704件、4 915件和523件,2022年咨询数量最多,是2021年的47.26倍,见图1。对2017—2023年每月咨询数量分析发现,除2022年不同月份之间咨询数量波动较大外,其他年份月咨询数量均相对平稳和集中;2022年12月群众咨询数量最多,达832件;月咨询量超过600件的月份还有2022年8月(745件)和9月(609件),见图2
7 253件咨询中,市长公开电话/12345热线平台来电是咨询事件的主要来源,占比达到58.78%(4 263件);其次是市长/市委书记/网络理政等信箱,占比为29.45%(2 136件);来自健康成都官微微博/微信、麻辣社区、问政四川等平台、论坛的网络咨询居第三位,占比为7.73%(561件);市疾控中心官网主任信箱占比为2.54%(184件);来自网上信访系统及其他渠道的咨询占比1.50%(109件),见图3
7 253件咨询中咨询问题类别主要包括:疫苗/预防接种相关问题(不含新冠)、疾病预防控制相关问题(不含新冠)、食品/环境/职业卫生相关问题、健康证办理相关问题、新冠疫情防控相关问题、新冠疫苗接种相关问题、新冠核酸检测相关问题。
在新冠疫情尚未发生的2017—2019年,疫苗/预防接种相关问题分别占当年咨询总数的80.00%(172/215)、91.30%(231/253)、88.61%(179/202),咨询主要涉及疫苗种类有HPV疫苗、狂犬疫苗、流感疫苗、脊灰疫苗、乙肝疫苗、老年人肺炎疫苗,咨询内容主要包括疫苗缺苗、预防接种政策/流程及服务、接种后不良反应、疫苗质量等方面;疾病预防控制相关问题分别占当年咨询总数的7.44%(16/215)、7.51%(19/253)、10.40%(21/202),咨询涉及疾病类别主要为传染病和性病/艾滋病;2017年底中心停止从业人员健康检查办证服务工作,当年有关健康证办理相关问题的咨询占比为10.23%(22/215)。
2020年新冠疫情发生后,群众咨询量出现较大幅度升高,2020—2023年与新冠相关的咨询占比达到90.41%(5 952/6 583),其中新冠疫情防控相关问题逐年占比分别为34.24%(151/441)、23.58%(166/704)、78.05%(3 836/4 915)、24.09%(126/523),咨询内容主要涉及新冠疫情防控相关政策和措施,包括来(返)蓉疫情防控政策、密接/次密的判定及隔离政策、入境人员隔离政策等,以及疫情相关情况咨询、疫情防控相关知识和健康码问题,2022年该类咨询占比较2020—2021年明显升高,χ2检验显示差异有统计学意义(χ2=1 092.147,P<0.001);新冠疫苗接种相关问题逐年占比分别为7.71%(34/441)、56.11%(395/704)、9.01%(443/4 915)、30.98%(162/523);新冠核酸检测相关问题逐年占比分别为4.76%(21/441)、3.41%(24/704)、10.25%(504/4 915)、1.72%(9/523),咨询内容主要涉及核酸检测政策建议、检测结果判定/查询、采样方式建议、检测操作规范性等,2022年该类咨询占比较2020—2021年亦有所升高,χ2检验显示差异有统计学意义(χ2=45.083,P<0.001)。
2023年,非新冠相关的疫苗/预防接种问题占比为25.62%(134/523),较新冠发生前(2017—2019年)有所降低,χ2检验显示差异有统计学意义(χ2=459.109,P<0.001);而非新冠相关的疾病预防控制问题咨询占比达15.30%(80/523),较新冠发生前有所升高, χ2检验显示差异有统计学意义(χ2=13.999,P<0.001),主要为流感、诺如、肺炎支原体感染等学校高发传染病以及“大运会”疫情防控咨询和建议。见图4图5
“预防接种”、“疾病预防”、“新冠疫情”、“新冠疫苗”、“核酸检测”等5个关键词2017—2023年全国范围搜索指数和资讯指数信息见图6。对2017—2023年成都市疾病预防控制相关群众咨询事件与相应百度指数数据的直线相关分析显示,疫苗/预防接种相关问题(不含新冠)咨询数量与“预防接种”搜索指数之间存在正相关(r=0.864,P=0.027);新冠疫情防控相关问题咨询数量与“新冠疫情”搜索指数(r=0.794,P=0.033)以及资讯指数(r=0.872,P=0.010)之间均存在正相关;新冠疫苗接种相关问题咨询数量与“新冠疫苗”搜索指数(r=0.840,P=0.018)以及资讯指数(r=0.835,P=0.019)之间均存在正相关;新冠核酸检测相关问题咨询数量与“核酸检测”资讯指数之间存在正相关(r=0.972,P<0.001);而疾病预防控制相关问题(不含新冠)咨询数量与“疾病预防”搜索指数(r= -0.365,P=0.421)以及资讯指数(r= -0.346,P=0.448)之间均未发现相关性。见图7
本研究通过对2017—2023年成都市疾病预防控制相关群众咨询事件及相应网络舆情数据的分析发现,2022年以8—9月和12月群众咨询数量最多,与当时成都市新冠疫情流行严重程度相符,表明在遇到大型突发公共卫生事件时,群众对公共卫生方面的健康咨询服务需求会增高,且疾病流行情况越严重咨询需求量越大[7]。12345热线平台来电是群众咨询事件的主要来源,成都市疾病预防控制中心坚决履行成都市卫健委提出的充分发挥网络理政平台“听民声、察民情、解民忧、集民智”的重要窗口作用和联系群众的桥梁纽带作用,同时将其作为健康教育人际传播干预手段之一[8],通过答疑解惑提升人民群众的公共卫生专业素养,是一种有针对性的健康宣教形式,扩大了卫生服务半径,在疾病防控中有不可替代的作用。
不同时段,群众对咨询信息内容的需求也有变化。在新冠疫情尚未发生的2017—2019年,群众咨询诉求点主要集中在疫苗/预防接种相关方面,提示在免疫规划工作中要持续做好多渠道、针对性的宣传教育,促进群众掌握预防接种相关知识和政策,以更好发挥免疫规划对保护人群健康的积极作用[4]。新冠疫情发生后,2020—2023年与新冠相关的咨询占比达90.41%,成为群众关注的重点内容,特别是2022年新冠疫情防控及新冠核酸检测相关问题咨询占比较2020—2021年均有升高,反映了大型突发公共卫生事件给群众生活、工作各方面带来的深入影响。因此,在出现突发公共卫生事件时,及时公布疫情详细情况,加强防治宣传引导并不断因时因势优化完善防控措施至关重要[9]。2023年,非新冠相关的疫苗/预防接种问题占比较新冠发生前有所降低,而疾病预防控制问题咨询占比较新冠发生前有所升高,说明经历了大型突发公共卫生事件后,群众对各类传染病的防治问题更加重视和关注。
随着互联网技术的发展和移动终端设备的普及,信息的生产、传播和接收方式都发生了巨大变化[10],网络舆情是广大网民情感、态度、意见、观点的表达[11],近年来媒体和群众对公共卫生领域始终保持较高的关注度[12],而随着社会流动性的增强,突发公共卫生事件的发生频率比以往大大增加[13]。由于突发公共卫生事件与人民群众的生命健康密切相关[14],其舆情具有热度高、群众参与广泛、持续时间长的特点[15]。数理统计方法是一种运用浏览量、点击量等网络原始数据作为参数指标进行数据挖掘和处理的计量分析方法,在网络舆情传播特点和规律的可视化分析中发挥着重要作用[13]。本研究对各类别群众咨询事件与“预防接种”、“疾病预防”、“新冠疫情”、“新冠疫苗”、“核酸检测”等5个相应关键词百度指数的直线相关分析发现疫苗/预防接种相关问题咨询数量与“预防接种”搜索指数、新冠疫情防控相关问题咨询数量与“新冠疫情”搜索指数和资讯指数、新冠疫苗接种相关问题咨询数量与“新冠疫苗”搜索指数和资讯指数、新冠核酸检测相关问题咨询数量与“核酸检测”资讯指数之间均分别存在正相关关系,提醒我们在疾病防控工作中要更加重视舆情监测,多方面、多渠道了解群众的信息需求,加强舆论引导和风险预判,提高信息利用水平及网络舆情处置能力[10,16],为疾病预防控制和突发公共卫生事件应对提供有力的支持。
微博、微信是目前最流行、最活跃的新型网络媒体,是群众获得各类信息、进行交流互动的主要平台[10],也为疾控机构对其开展健康指导提供了渠道[16]。对关注度高和咨询多的热点问题,如最新的传染病疫情及其防控知识/政策、疫苗接种政策和缺苗投诉/咨询多等情况,应及时通过官方微博、微信、网站等多途径发布相应信息,如详细解读传染病防控热点知识和政策,告知群众缺苗原因及应对措施,并及时更新到货情况,以提升群众满意度。同时持续做好性病/艾滋病、狂犬病、寄生虫/地方病、慢性病、食品/环境/职业卫生等科普的宣传,不断完善公共卫生机构服务群众的职能[2],助力群众健康素养的提升。
综上,群众健康相关咨询信息的整合与分析是大数据时代的热点研究领域。本研究中,疾病预防控制相关问题咨询数量与“疾病预防”搜索/资讯指数之间未见相关性,推测与该类问题包含了传染病、性病/艾滋病、狂犬病、寄生虫/地方病、慢性病等多种类别疾病,而“疾病预防”关键词指向性不够明确有关,且出于对求询者个人信息的保护要求,未能系统收集到求询者的性别、年龄、职业、收入和所在地区等方面数据,无法进行人口学特征的分析,对咨询事件的记录以描述性为主,能量化分析的指标也较少,影响了后续相关研究和评价工作。今后应进一步扩大样本含量,对疾病更加精准分类探讨,并对上述不足之处持续寻求改进措施。
