Objective

To explore strategies to address the gap and uneven distribution of general practitioner(GP) resources from a provincial perspective, aiming to meet the growing medical needs of the people and advance the reform of the medical and health system.

Methods

Based on the provincial GP data from 2013-2022, Kernel density estimation was used to reveal the dynamic evolution of GP resources, while the spatial autocorrelation method was applied to explore their spatial correlation characteristics. Additionally, a spatial econometric model was used to investigate the direct impacts and spillover effects of the factors influencing general practitioner resources.

Results

There is a positive autocorrelation in China’s general practitioner resources. Among the direct effects, urbanization rate and population density have a positive impact on general practitioner resources, while the elderly dependency ratio has a negative impact. In terms of indirect effects, urbanization rate, population density, the proportion of medical and health expenditure in fiscal expenditure, and the number of medical and health institutions all have a negative impact on general practitioner resources.

Conclusion

General practitioner resources in China are not randomly distributed spatially, and the spillover effects of influencing factors in adjacent areas can also impact general practitioner resources. It is recommended to focus on collaboration of medical resources and coordination of medical policies among different regions, promoting cross-regional communication and sharing of general practitioners.

目的

从省域角度探索弥补全科医生资源缺口以及分布不均的策略,以满足人民不断增长的医疗需求,推进医疗卫生体制改革。

方法

基于2013—2022年省级全科医生数据,采用Kernel密度估计揭示全科医生资源的动态演变特征,运用空间自相关方法探索全科医生资源的空间相关特征,采用空间计量经济模型探索全科医生资源影响因素的直接影响与溢出效应。

结果

我国全科医生资源存在正向自相关性。直接效应中,城镇化率和人口密度对全科医生资源存在正向影响,老年人口抚养比存在负向影响;间接效应中,城镇化率、人口密度、医疗卫生支出占财政支出的比重、医疗卫生机构数均对全科医生资源存在负向影响。

结论

我国的全科医生资源在空间上不是随机分布的,邻接地区影响因素的溢出效应也会对全科医生资源产生影响。建议注重不同区域间的医疗资源协作和医疗政策协调,促进跨地区全科医生的交流和共享。

目的

从省域角度探索弥补全科医生资源缺口以及分布不均的策略,以满足人民不断增长的医疗需求,推进医疗卫生体制改革。

方法

基于2013—2022年省级全科医生数据,采用Kernel密度估计揭示全科医生资源的动态演变特征,运用空间自相关方法探索全科医生资源的空间相关特征,采用空间计量经济模型探索全科医生资源影响因素的直接影响与溢出效应。

结果

我国全科医生资源存在正向自相关性。直接效应中,城镇化率和人口密度对全科医生资源存在正向影响,老年人口抚养比存在负向影响;间接效应中,城镇化率、人口密度、医疗卫生支出占财政支出的比重、医疗卫生机构数均对全科医生资源存在负向影响。

结论

我国的全科医生资源在空间上不是随机分布的,邻接地区影响因素的溢出效应也会对全科医生资源产生影响。建议注重不同区域间的医疗资源协作和医疗政策协调,促进跨地区全科医生的交流和共享。

Objective

To explore strategies to address the gap and uneven distribution of general practitioner(GP) resources from a provincial perspective, aiming to meet the growing medical needs of the people and advance the reform of the medical and health system.

Methods

Based on the provincial GP data from 2013-2022, Kernel density estimation was used to reveal the dynamic evolution of GP resources, while the spatial autocorrelation method was applied to explore their spatial correlation characteristics. Additionally, a spatial econometric model was used to investigate the direct impacts and spillover effects of the factors influencing general practitioner resources.

Results

There is a positive autocorrelation in China’s general practitioner resources. Among the direct effects, urbanization rate and population density have a positive impact on general practitioner resources, while the elderly dependency ratio has a negative impact. In terms of indirect effects, urbanization rate, population density, the proportion of medical and health expenditure in fiscal expenditure, and the number of medical and health institutions all have a negative impact on general practitioner resources.

Conclusion

General practitioner resources in China are not randomly distributed spatially, and the spillover effects of influencing factors in adjacent areas can also impact general practitioner resources. It is recommended to focus on collaboration of medical resources and coordination of medical policies among different regions, promoting cross-regional communication and sharing of general practitioners.

作为居民健康的“守门人”,全科医生不仅承担着疾病预防、诊断和治疗的任务,还在患者健康管理和社区卫生服务中发挥着核心作用[1]。然而,自2011年国务院颁布《关于建立全科医生制度指导意见》以来,中国全科医生的数量虽然逐年增加,但全科医生的供给却没有跟上需求的增长,这导致了许多地区存在全科医生稀缺的现象。截至2022年底,我国全科医生总数为46.3万人,仅占卫生人员总数的3.2%。
既往对全科医生资源配置的研究主要着眼于全科医生资源的公平性研究及预测。寇儒欣等基于全国省级数据,运用基尼系数和泰尔指数分析我国全科医生资源配置公平性[2]。赵越等运用集聚度和GM(1,1)模型对我国某一区域全科医生配置的公平性进行分析和预测[3-4]。吴爽等运用卫生资源密度指数评价我国全科医生资源配置公平性,运用模糊集定性比较分析方法分析影响全科医生资源配置公平性的路径[5]。回顾以往的研究发现,对全科医生的影响因素进行探索的研究相对较少,而从空间角度对全科医生资源的分布及其影响因素进行分析的研究更是罕见。本研究首先利用Kernel密度估计方法揭示全科医生资源配置的时空演变特征。随后运用全局莫兰指数来判断全科医生资源在空间上的整体相关性。最后结合空间计量经济模型,分析影响全科医生资源配置的主要因素,并评估各因素的影响程度和空间溢出效应。以期提出弥补全科医生资源配置缺口的策略,促进全科医生资源的均衡分布,满足不断增长的医疗需求。
本研究以除港澳台外的中国31个省级行政单元为研究对象,时间窗为2012—2021年,数据来源于2013—2022年《中国统计年鉴》、《中国卫生健康统计年鉴》以及国家统计局官网。为衡量地区差异,依据《中国统计年鉴》将31个省级行政单元划分为东、中、西三大区域。
核密度估计(Kernel Density Estimation,KDE)是一种非参数检验方法,用于估计随机变量的概率密度,以连续密度曲线来展现变量的分布形态以及动态演变特征[6]。计算公式为:
式中,n为样本数量,h为带宽,K(*)是核函数,本研究选用选用高斯核函数,xi为独立同分布的样本值,x为样本均值。
全局莫兰指数是一种用于度量空间自相关性的重要工具,它的基本功能是评估空间单元与其周边地区的关联程度,反映的是整个研究区域内空间分布的聚集性或离散性。计算公式为:
式中,n为省份数量;xixj则分别表示空间单元ij上的全科医生数量,Wij是经济距离权重矩阵,基于两个城市之间的经济变量的差异来反映样本城市之间的关系的远近。全局莫兰指数的取值范围为[-1,1],当值为正时,表示全科医生存在空间集聚现象;当值为负时,表示存在空间离散现象;而当值为0时,则表示不存在空间自相关性[7]。Moran I指数可以通过标准化统计量Z值来检验n个省份之间是否存在空间自相关,其公式如下:
式中E(I)为期望值,VAR(I)为方差。当Z值为正且显著时,代表全科医生资源集聚存在空间正自相关;当Z值为负且显著时,代表空间负自相关;当Z等于0时,观测值呈独立随机分布[8]。本研究采用的检验水准为α=0.05,即当P值小于0.05时,认为莫兰指数所表示的空间相关性在统计学上是显著的。
各经济单元并不是孤立存在的而是具有空间上的相关关系,此时若使用传统的OLS(Ordinary Least Squares)模型进行回归分析会造成结果的偏误[9],故本文使用空间计量经济模型进行回归分析。空间计量经济模型包括空间滞后模型(SAR)、空间误差模型(SEM)和空间杜宾模型(SDM)三种。相比另外两个模型,空间杜宾模型假定因变量的取值除了受本地自变量的影响外,还会受到邻近地区的自变量影响,同时考虑到因变量和自变量的空间相关性[10]。计算公式为:
式中,Y为因变量,X为自变量,W为空间矩阵。ρ表示空间自回归系数,βθ为回归系数,ε为误差项。根据Lesage和Pace等人的研究,采用空间杜宾模型系数度量溢出效应会存在系统性偏差[11]。故本研究通过偏微分的方法将各解释变量的效应分解成直接效应、间接效应和总效应。直接效应指的是解释变量在本地区对被解释变量产生的影响,间接效应则是指解释变量通过空间关联对邻近地区被解释变量产生的影响。这两者之和就是总效应,表示解释变量对整个区域被解释变量的平均影响程度。
本研究以全科医生总数为因变量。在影响因素的选择方面,本研究在借鉴相关文献[12]和考虑数据可得性的基础上,从经济、社会、政策、医疗四个层面选取人均GDP、城镇化率、人口密度、老年人口抚养比、平均受教育年限、城镇医疗保险年末参保人数、医疗卫生支出占财政支出的比重、医疗卫生机构数8个指标。
表1可以看出,2012—2021年期间,我国的全科医生总数和每万人口全科医生数持续增加,年均增长率分别为16.53%和28.87%。除2013年外三大区域全科医生总数排序为东部>中部>西部,2013年西部地区全科医生数量略多于中部地区。2016—2021年,每万人口全科医生数排序为东部>中部>西部,2012—2015年间西部地区的每万人口全科医生数略超中部地区。
本研究利用Matlab R 2016a软件绘制全科医生资源的三维核密度估计图,图1描绘了全国31个省的全科医生资源在2012—2021年的动态演进趋势。从分布位置看,Kernel密度曲线的中心逐渐右移,表明全科医生总数在逐年增加。从波峰态势来看,Kernel密度曲线的波峰高度逐渐降低,波峰宽度逐渐变大,表明我国全科医生资源分布趋于分散。从分布延展性来看,Kernel密度曲线存在明显的右拖尾现象且逐年延长,表明我国全科医生数量的空间差距在逐渐扩大。从极化特征来看,全国地区Kernel密度曲线的存在轻微的两级分化特征,表明我国全科医生的分布相对均匀,没有出现极端的集中或分散现象。
图234分别描绘了全科医生数在东、中、西部的动态演进趋势。从分布位置看,Kernel密度曲线的中心逐渐右移,表明东、中、西部的全科医生数量均在逐年增加。从波峰态势来看,东部地区Kernel密度曲线的波峰高度明显低于中西部地区且波峰宽度较宽,全科医生资源分布的趋势趋于扁平化,说明相较于中西部地区,东部地区全科医生资源分布更加分散。从分布延展性来看,三大区域均存在明显的右拖尾现象,表明全科医生的工作范围和职责逐渐扩展,涉及更广泛的医疗领域和疾病类型。从极化特征来看,西部地区的两极化特征较为明显,表明相较于中、东部地区,西部地区全科医生资源的内部差距更加明显。
本研究利用Stata 16.0计算全科医生数量在2012—2021年的全局莫兰指数,见表2。整体来看,全科医生资源的全局莫兰指数显著为正,表明全科医生资源存在正向自相关性,全科医生资源在空间上并不是随机分布的,而是存在一定的聚集性。此外,全局莫兰指数呈现波动下降趋势,表明全科医生资源的集聚性在逐渐减弱。
本研究运用空间计量经济模型来进行影响因素的分析,首先需要进行模型选择,见表3。LM检验、LR 检验、Wald 检验结果均认为空间杜宾模型无法退化成空间滞后模型、空间误差模型。Huasman检验结果拒绝了随机效应模型的原假设。双固定-个体LR检验结果与双固定-时间LR检验结果都在1%的水平下拒绝原假设,即应当选择双固定效应的空间杜宾模型。综上所述,经过LM(Lagrange Multiplier)、LR(Likelihood Ratio)和Wald等一系列检验,我们选择个体和时间双固定效应的空间杜宾模型。
从直接效应来看,城镇化率与人口密度的系数分别在1%和5%水平下显著为正,老年人口抚养比的系数在10%水平下显著为负。从间接效应来看,城镇化率、人口密度、医疗卫生支出占财政支出的比重、医疗卫生机构数的系数均显著为负。
2012—2021年间我国的全科医生数量逐年增加,这与2011年7月国务院颁布《关于建立全科医生制度指导意见》(以下简称《意见》)有关。2021年我国每万人口全科医生数为3.08,完成了《意见》制定的“到2020年基本实现城乡每万名居民有2-3名合格的全科医生”的总体目标,但与2018年国务院办公厅印发的《关于改革完善全科医生培养与使用激励机制的意见》制定的“到2030年城乡每万名居民拥有5名合格的全科医生”的工作目标还相距甚远。2021年我国的全科医生占所有医生的比例为10.14%,远低于OECD中32个成员国的平均比例(23%),我国的全科医生资源还存在较大缺口。从区域角度来看,三大区域间全科医生资源配置不均衡。全科医生资源拥有量排序为东部>中部>西部,呈现出由东向西递减的阶梯型分布特征。根据Kernel密度曲线的绘制结果,我国的全科医生资源分布趋于分散,空间差距逐渐加大。目前,全科医生短缺现象的关键症结在于全科医生职位的职业吸引力不足以及离职意愿较高[13]。解决这一问题的关键在于提高全科医生的社会地位和待遇,确保全科医生的经济待遇、职业发展机会和社会认可度与其专业贡献相匹配。针对基层和偏远地区的全科医生,应提供额外的薪酬补贴、工作生活配套设施、安家落户等方面的优惠政策,吸引更多的全科医生到基层和偏远地区工作[14],缩小全科医生资源的地区差距。
全局莫兰指数的结果表明全科医生资源在空间上并不是随机分布的,而是存在一定的聚集性。这与政府采取的一系列政策有关,2017年中央印发《关于印发“十三五”全国卫生计生专业技术人员培训规划的通知》,强调要以全科医生为主要关注点,以中西部地区、农村、基层为工作重心,鼓励发达地区发挥引领示范与对口援助作用,促进协调发展[15]。在人口老龄化和慢性病负担加重的背景下,由于全科医生的培养周期相对较长,进入该行业的新兴全科医生的数量无法迅速增加以满足不断增长的医疗需求,所以促进全科医生资源在不同区域间的流动和共享就显得尤为重要。首先要制定有利于跨区域医疗服务和医生流动的政策和法规,以消除行政障碍,简化跨区域行医的行政程序和执照转移流程,促进全科医生自由流动。其次要利用现代信息技术手段,打破地域限制,实现区域内全科医生资源的远程共享和协作。
城镇化率的提升对于本地区全科医生数量的增长具有正面影响,但对邻接地区全科医生数量的增长产生负面影响。城镇化率的提高会带来更多的医疗投入,促进本地区全科医生数量的增加。但是随着城镇化地区的医疗需求增多和待遇改善,邻接地区的全科医生会被吸引到城镇化程度更高的地区工作,造成人才流失。人口密度对本地区全科医生数量的增加具有促进作用,但同时会对邻接地区全科医生数量的增加产生抑制作用。在有限的医疗资源条件下,人口密集的地区吸引了更多的全科医生,这导致邻接地区的全科医生数量相对减少。老年人口抚养比对全科医生数量的影响呈现负相关趋势,但其影响程度微弱。原因是全科医生的培养周期较长,限制了全科医生的供应速度,导致全科医生数量的增长滞后于老年人口的增长。医疗卫生支出占财政支出的比重和医疗卫生机构数对邻接地区全科医生数量的影响为负。原因是高卫生支出地区提供了更好的待遇和培训机会,吸引全科医生前往该地区工作,而邻接地区则会经历全科医生的流失。各地政府在制定全科医生资源配置政策时,必须充分考虑邻接地区的外部环境因素,加强区域协调机制,确保医疗政策的一致性和协调性。邻接地区可以共同制定医疗政策,协商资源的分配和合作机制,以避免因政策差异导致的全科医生资源倾斜问题。
