目的

分析代际精神支持和抑郁在老年人社会参与和认知功能间的链式中介作用。

方法

本文运用中国健康与养老追踪调查（CHARLS）2020年数据，筛选出符合标准的研究对象3 737人，抑郁和认知功能分别利用流调中心抑郁量表（CES-D 10）和整体认知评分工具进行测量；采用pearson相关性分析对关键变量进行分析；构建多重中介效应模型，研究代际精神支持与抑郁在老年人社会参与活动和老年人认知功能影响的中介效应。

结果

在纳入年龄、性别、婚姻状况、慢性病患病情况、参保情况等控制变量后，回归分析表明，老年人社会参与状况对其认知功能具有显著直接影响（β=0.254，P<0.001），而代际精神支持（β=0.052，P<0.050）及抑郁状况（β= -0.084，P<0.001）对老年人认知功能亦具有显著影响。中介效应分析结果显示，代际精神支持和抑郁在老年人社会参与和老年人认知功能之间的简单中介效应分别为0.006（95% CI:0.001~0.012）和0.020（95% CI:0.006~0.036），链式中介效应为0.002（95% CI:0.001~0.004）。

结论

本研究发现代际精神支持与抑郁在老年人社会参与与认知功能之间存在直接中介效应，同时二者之间还表现出链式中介效应，可以通过提高老年人社会参与度、子女关怀照料、减少抑郁症状来改善其认知功能，延缓或防止老年人认知障碍的发生。

Objective

To analyze the chain-mediated roles of intergenerational spiritual support and depression between social participation and cognitive functioning in older adults.

Methods

In this paper, we used data from the China Health and Aging Tracking Survey (CHARLS) 2020 to screen 3 737 study participants who met the criteria. Depression and cognitive function were measured using the Center for Streaming Depression Scale (CES-D 10) and the Overall Cognitive Scoring Instrument, respectively. Two-column correlation analyses (Pearson’s correlation analyses) of correlation were used, and multiple mediating effect models were constructed to investigate the mediating effects of intergenerational spiritual support and depression in the impact of social participation activities and cognitive functioning in older adults.

Results

After including control variables such as age, gender, marital status, chronic disease prevalence, and insurance participation, regression analysis showed that older adults’ social participation status had a significant direct effect on their cognitive functioning (β=0.254, P<0.001), while intergenerational spiritual support (β=0.052, P<0.050) and depression status (β= -0.084, P<0.001) also had a significant effect on cognitive function in older adults. The results of mediation effect analysis showed that the simple mediation effects of intergenerational psychiatric support and depression status on the relationship between social participation status and cognitive function in older adults were 0.006(95% CI: 0.001-0.012) and 0.020(95% CI: 0.006-0.036), respectively, and the chained mediation effect was 0.001(95% CI: 0.001-0.004).

Conclusion

This study finds a direct mediating effect of depressive status and intergenerational psychiatric support between social participation status and cognitive functioning in older adults, as well as a chain-mediated effect between the two. Social participation of older adults should be improved, while their cognitive function can be improved by children caring more about their parents and reducing depressive symptoms to delay or prevent the occurrence of cognitive impairment in older adults.

随着人口老龄化趋势不断加深，老年失智症患者人数也在不断增加^[1]。截至2023年，全世界超过5 500万人老年人患失智症^[2]，而我国60岁及以上老年失智症患者占全球患者的18%~20%^[3]。同时轻度认知功能障碍作为失智症的前驱阶段，虽然并非所有轻度认知功能障碍患者都会发展成失智症，但有效预防和干预认知功能下降对于减缓或阻止病情进展极为重要^[4]。社会参与被定义为参与社会活动和维持社会关系，已被确定是防止认知能力下降的潜在保护因素^[5-6]，一项随访调查显示与社会参与减少的老年人相比，社会参与增加或保持不变的老年人认知功能得到改善的概率高出近60%^[7]。代际精神支持，包括家庭内不同代际之间的情感和心理援助，可以显著影响老年人的心理健康，与年轻家庭成员的积极互动可以增强老年人的目标感和归属感，从而支持认知健康^[8]。现有研究表明，老年人社会参与活动增多还有效促进了家庭内部的情感交流^[9]，而频繁的代际精神支持可以预防老年人认知功能下降^[10]。此外，多项研究^[11-12]强调代际支持，特别是代际精神支持维度，对老年人的心理健康具有显著的积极影响，进而促使抑郁症状水平得以有效降低。然而，抑郁症作为老年人的常见问题，可能会通过加剧记忆丧失、降低认知速度和损害执行功能来对认知功能产生负面影响^[13-14]。

然而，现有研究^[15-17]仅验证了社会参与、代际精神支持、抑郁与认知功能之间存在的相互影响关系，但这些因素的综合作用以及潜在联系仍较为模糊。基于此，本研究旨在探讨我国老年人社会参与对认知功能的影响，并进一步分析代际精神支持和抑郁状况在此过程中是否存在链式中介效应，以期为预防老年人认知障碍提出切实可行的策略建议。

1　对象与方法

1.1　研究对象

本研究所涉及的所有数据均从CHARLS 2020年全国追踪调查数据集中提取。纳入标准：(1)年≥60岁；(2)随访过程中完成全部认知功能的评估；(3)抑郁量表的信息完整无遗漏。排除标准：(1)关键变量数据存在不完整情况；(2)控制变量的关键信息存在缺失或遗漏。具体纳入流程见图1。

1.2　变量选择

各变量具体解释如下：

1.2.1　社会参与

社会参与作为本文自变量，相关数据来自CHARLS问卷中的“您过去一个月是否进行了下列活动：跟朋友互动、打麻将、打牌”等在内的8项社交活动，若参与任意一项活动，则计1分，否则为0，总分范围为0至8分，分数越高，说明社会参与程度越高^[18]。

1.2.2　认知功能

认知功能作为本文因变量，运用3种综合评估手段对受访者的认知表现进行评估：单词回忆测试、认知状态电话访谈（TICS-10）以及绘画测试。单词回忆测试旨在衡量情景记忆，而TICS-10和绘画测试则用以评估心理完整性。认知功能总分由情景记忆与心理完整性得分叠加而成，满分为21分。分数越高，代表认知功能表现越好。

1.2.3　代际精神支持

代际精神支持作为本文中介变量，主要体现在与子女的见面频率和线上沟通频率两方面。关于见面频率问题：“您和子女不同住时，多久见一次？”频率高则赋分高，如差不多每天赋9分，几乎从来没有赋1分。关于线上沟通频率“您和子女不同住时，多久线上联系一次？”，选项中频率越高赋分越高，如：差不多每天赋分为9分，而几乎从来没有赋分为1分^[19]。

1.2.4　抑郁

抑郁作为文本另一个中介变量，使用流调中心抑郁量表（CES-D 10）来评估过去一周抑郁症状的频率^[20]。CES-D 10量表包含10个条目，答题选项分为四个等级。其中，“我对未来充满希望。”和“我很愉快。”两项进行反向计分，每个条目的得分从0到3，总分范围为0～30分，得分越高，抑郁程度越严重。

1.2.5　控制变量

结合先前研究，本文将其他可能对老年人认知功能产生影响的因素设定为控制变量，包括年龄、性别、婚姻状况、慢性病状况（医生诊断后自我报告的14种慢性病情况）、吸烟状况、饮酒状况、参保状况等。其中，慢性病状况定义为若有一项慢性病，则认为研究对象患有慢性病。

1.3　统计分析

计量资料采用均值±标准差进行描述；计数资料使用频数和百分比进行表述。数据分析采用Pearson相关分析相关性，采用Hayes编制的PROCESS 4.2程序中的模型6进行多重中介效应分析检验，进行更细致的路径分析，通过回归分析估计路径系数，计算各中介路径的间接效应，最后使用Bootstrap方法来估计中介效应的置信区间。所有统计分析使用SPSS 27.0进行，显著性检验均为双侧检验，P<0.05。

2　结果

2.1　基本情况

本研究最终纳入3 721名调查对象，平均年龄67.47±5.55岁，其中男性2 231人，占59.96%，女性1 490人，占40.04%；认知功能在年龄、婚姻状况、饮酒状况、吸烟状况、是否参与医疗保险和养老保险等方面有统计学意义（P<0.050）。见表1。

2.2　现状描述

对3 721名调查对象进行详细分析，按照社会参与状况、代际精神支持状况、抑郁状况以及认知功能四个条目进行了得分统计。具体得分情况详见表2。

2.3　关键变量间相关性分析

对社会参与、代际精神支持、抑郁以及老年人认知功能进行了相关性分析，研究结果表明关键变量之间均存在显著的相关关系。见表3。

2.4　多重中介模型分析

本研究在严格控制性别、年龄、婚姻状况等多个潜在影响因素的基础上，深入探讨社会参与作为自变量对老年人社会认知功能状况的影响，以及代际精神支持和抑郁状况作为中介变量的作用。数据分析结果显示，老年人社会参与状况与代际精神支持（β=0.113，P<0.001）和认知功能（β=0.254，P<0.001）均呈现出正相关关系，与抑郁状况之间存在负相关关系（β= -0.245，P<0.050）；代际精神支持状况与抑郁状况呈现出负相关关系（β=-0.202，P<0.001），与老年人认知功能之间存在正相关关系（β=0.052，P<0.050）。抑郁状况与老年人认知功能之间存在负相关关系（β= -0.084，P<0.001）。结果见表4。

多重中介模型结果分析显示，老年人的社会参与状况对其认知功能产生显著的直接影响（95% CI:0.182～0.326，效应占比：90.07%）。其次，代际精神支持与抑郁状况在老年人社会参与与认知功能之间表现出显著的中介作用（95% CI:0.001～0.012，效应占比：2.13%；95% CI:0.006～0.036，效应占比：7.09%）。此外，代际精神支持和抑郁状况在老年人社会参与与认知功能的关系中表现出边缘性显著的链式中介效应（95% CI:0.001～0.004，效应占比：0.71%）。直接效应值为0.254，总效应值为0.282，而总中介效应值为0.028。结果见表5，图2。

3　讨论

本研究探讨了老年人社会参与、代际精神支持、抑郁状况、认知功能状况四者之间的相互关系和作用机制。研究结果显示，在老年人社会参与与认知功能之间，代际精神支持和抑郁状况起到中介作用，间接效应分别占总效应的7.09%和2.13%。此外，代际精神支持与抑郁状况在老年人社会参与与认知功能之间呈现出链式中介效应，该效应占总效应的0.71%。

3.1　老年人社会参与和认知功能存在显著直接效应

研究结果表明，老年人的社会参与与其认知功能之间存在着显著的正相关关系，即社会参与程度低的老年人认知功能较差。既往研究也表明，参与社会活动可能有助于防止中老年认知能力的衰退及痴呆症的进展^[21-22]。可能的原因是，一方面社会参与活动可能会增加老年人对认知需求环境的暴露，从而对认知下降起到缓冲作用^[23]。另一方面，社会参与可以通过拓宽社交网络，为老年人带来丰富的知识资源以及更为全面的情感和工具支持，进而对其认知功能产生积极影响^[24]。

3.2　代际精神支持在老年人社会参与和认知功能之间存在中介作用

研究结果表明，社会参与活动丰富的老年人与子女的联系可能更为紧密。既往研究表明，随着社会参与程度的提高，老年人接触新事物的机会相应增加，激发其分享经历的欲望^[25]，从而使得老年人与其子女的沟通频率升高。此外，代际精神支持能够有效提升老年人的生活质量，改善其健康状况及认知功能^[26-27]。因此，社会参与可以通过提高子女的代际精神支持，进而影响老年人的认知功能。

3.3　抑郁状况在老年人社会参与和认知功能之间存在中介作用

研究结果显示，社会参与程度高的老年人抑郁程度较低。可能的原因是社会参与活动能增进老年人与他人的互动，强化与社区的联系，增加结识不同年龄层的新朋友的机会，激发生活活力^[28]。此外，抑郁症与老年人认知功能直接相关且具有负面影响^[29]。可能的原因是抑郁症可能损害注意力、记忆力等能力^[30]，降低心理灵活性和记忆巩固能力，从而影响个体的认知功能^[31]。本研究中抑郁的相对中介效应（7.09%）大于代际精神支持的相对中介效应（2.13%），这提示我们，预防老年人抑郁的发生对于健康的认知功能状态极为重要。

3.4　代际精神支持和抑郁状况在老年人社会参与和认知功能之间存在链式中介作用

研究结果显示，积极的社会参与可以有效促进老年人与子女的情感交流，进而减少抑郁症状发生的风险，最终可以减缓老年人认知功能的衰退。可能的原因是老年人社会参与活动的增多，激发了他们对子女的分享欲，从而有效促进了家庭内部的情感交流^[32]。随着与子女交流频率的增加，老年人的存在感与自信心得到提升，抑郁程度得到缓解^[33]，从而延缓认知功能的下降^[34]。

综上所述，为了有效保护老年人的认知功能，应该鼓励他们积极参与社会活动，子女应加强对老年人的情感关怀，同时重视老年人心理健康状况。此外，本研究存在一定局限性。首先，横断面研究仅能分析各变量之间的关联性，而无法证实它们之间的因果关系。其次，在社会参与方面，仅关注了参与程度，未对具体的社会参与类型进行区分，今后研究需进一步细化探讨。最后，由于数据存在一定局限性，剔除部分关键变量缺失值后，可能存在选择性偏倚。
