Objective

To explore the relationship between hormone replacement therapy (HRT) and the time period when postmenopausal women received HRT and accelerated biological aging.

Methods

Based on the biochemical and anthropometric data collected from the UK Biobank baseline survey, the biological age (BA) and BA acceleration index were constructed by Klemera Doubal method (KDM). The subjects were divided into 5 groups according to the status of receiving HRT: no, 10 years before menopause, 0-10 years before menopause, 0-10 years after menopause, and 10 years after menopause. Multivariate linear regression model was used to explore the relationship between HRT and BA acceleration. The characteristic factors of the population were taken as stratified factors for stratified analysis.

Results

A total of 96 889 subjects were included. Compared with people who did not receive HRT, there was a correlation for starting HRT 0-10 years before menopause (β=-0.28, 95%CI:-0.36 to -0.19) and starting HRT postmenopausal 0-10 years (β=-0.39, 95%CI: -0.46 to -0.31) with the delayed accelerated aging. The effect of HRT on delaying and accelerating aging was more obvious in postmenopausal women less than 60 years old.

Conclusion

Receiving HRT slows biological aging and is more effective when used within 10 years of menopause in postmenopausal women under 60 years of age.

目的

探究绝经后妇女接受激素补充治疗(hormone replacement therapy, HRT)及开始治疗的时间段与生物衰老加速之间的关联。

方法

基于英国生物银行(UK Biobank,UKB)基线调查采集的生化检测与人体测量数据,采用Klemera Doubal(KDM)算法构建生物年龄(biological age,BA)和BA加速指标。将研究对象按接受HRT情况分为五组:未曾、绝经前10年以前开始、绝经前0~10年开始、绝经后0~10年开始和绝经后10年以后开始接受HRT。使用多因素线性回归模型探究HRT情况与BA加速之间的关联。并将人群特征因素作为分层因素进行分层分析。

结果

共纳入96 889名研究对象。与未接受HRT的人相比,绝经前0~10年开始(β=-0.28, 95%CI=-0.36~-0.19)和绝经后0~10年开始(β=-0.39, 95%CI=-0.46~-0.31)接受HRT与延缓加速衰老间存在关联。年龄小于60岁的绝经后妇女接受HRT延缓加速衰老的效应更明显。

结论

接受HRT会延缓生物衰老,且60岁以下绝经后妇女在绝经后10年内使用效果更明显。

目的

探究绝经后妇女接受激素补充治疗(hormone replacement therapy, HRT)及开始治疗的时间段与生物衰老加速之间的关联。

方法

基于英国生物银行(UK Biobank,UKB)基线调查采集的生化检测与人体测量数据,采用Klemera Doubal(KDM)算法构建生物年龄(biological age,BA)和BA加速指标。将研究对象按接受HRT情况分为五组:未曾、绝经前10年以前开始、绝经前0~10年开始、绝经后0~10年开始和绝经后10年以后开始接受HRT。使用多因素线性回归模型探究HRT情况与BA加速之间的关联。并将人群特征因素作为分层因素进行分层分析。

结果

共纳入96 889名研究对象。与未接受HRT的人相比,绝经前0~10年开始(β=-0.28, 95%CI=-0.36~-0.19)和绝经后0~10年开始(β=-0.39, 95%CI=-0.46~-0.31)接受HRT与延缓加速衰老间存在关联。年龄小于60岁的绝经后妇女接受HRT延缓加速衰老的效应更明显。

结论

接受HRT会延缓生物衰老,且60岁以下绝经后妇女在绝经后10年内使用效果更明显。

Objective

To explore the relationship between hormone replacement therapy (HRT) and the time period when postmenopausal women received HRT and accelerated biological aging.

Methods

Based on the biochemical and anthropometric data collected from the UK Biobank baseline survey, the biological age (BA) and BA acceleration index were constructed by Klemera Doubal method (KDM). The subjects were divided into 5 groups according to the status of receiving HRT: no, 10 years before menopause, 0-10 years before menopause, 0-10 years after menopause, and 10 years after menopause. Multivariate linear regression model was used to explore the relationship between HRT and BA acceleration. The characteristic factors of the population were taken as stratified factors for stratified analysis.

Results

A total of 96 889 subjects were included. Compared with people who did not receive HRT, there was a correlation for starting HRT 0-10 years before menopause (β=-0.28, 95%CI:-0.36 to -0.19) and starting HRT postmenopausal 0-10 years (β=-0.39, 95%CI: -0.46 to -0.31) with the delayed accelerated aging. The effect of HRT on delaying and accelerating aging was more obvious in postmenopausal women less than 60 years old.

Conclusion

Receiving HRT slows biological aging and is more effective when used within 10 years of menopause in postmenopausal women under 60 years of age.

绝经是女性卵巢衰竭所引起的月经永久停止,其本质是雌激素缺乏[1]。在短期内,女性绝经会引起包括潮热出汗、乏力虚弱、睡眠障碍等症状的绝经综合征[2];长远看来,它还会增加患骨质疏松、心血管疾病、代谢性疾病等年龄相关疾病的风险[3]。同时,绝经也是人体衰老的重要转折点。有研究提示绝经可能会加速生物的衰老[4]
激素补充治疗(hormone replacement therapy,HRT)是解决绝经综合征的最有效办法,但其对心血管疾病、子宫内膜癌、乳腺癌等风险的影响一度饱受争议[5]。近20年来,研究者对其有了新的认识[6]。一些研究显示绝经后尽早(10年内)或60岁以前开始适当的HRT对治疗血管舒张症状、泌尿生殖系统症状,预防骨质疏松、心血管疾病等有重要作用,并强调了治疗开始的时间节点的重要性[7-9]。还有研究发现绝经后妇女进行长期HRT可以减轻端粒损耗[10]。HRT作为重要的预防和治疗手段,是否能延缓绝经引起的生物衰老加速,目前暂缺乏相关的流行病学研究。生物学年龄(biological age,BA)是一个综合反映身体各个器官系统的衰老状态指标,为年龄相关疾病的早期干预提供了独特的观测角度[11]
本研究基于英国生物银行(UK Biobank, UKB)生化检测和问卷调查的基线数据,在绝经后妇女中探究了使用HRT及开始治疗的时间段与生物衰老加速(biological aging acceleration, BA加速)之间的关系。
UKB是英国一项基于人口的大型前瞻性队列研究,共招募了50多万名40~69岁的参与者[12]。调查内容包括电子通过触摸屏填写的问卷、体格测量、生化测试、表型评估和签署的知情同意书。西北多中心研究伦理委员会(MREC)为UKB研究提供了一般伦理批准。本研究在生化数据完整的基线人群(330 166)中排除了男性(153 323)、未绝经(71 648)、HRT使用情况及开始治疗时年龄数据不完整(8 306)的样本,最终纳入了96 889名研究对象。
HRT是使用雌激素、孕激素或联合使用的对症治疗绝经综合征及预防疾病的方法,根据不同适应症和个体情况改变用药方案。HRT使用情况通过触摸屏电子问卷测量。问卷提问“您是否曾使用过HRT?”,如研究对象回答“是”则继续提问“开始使用HRT的年龄”。UKB并未在调查中详细询问参与者接受治疗的具体药物、剂量、时长。我们用开始接受HRT的年龄减去绝经年龄并据此划分开始接受治疗的时间段。最终将研究对象分为五组:未曾接受过HRT、绝经前10年以前开始接受HRT、绝经前0~10年开始接受HRT、绝经后0~10年开始接受HRT、绝经后10年以后开始接受HRT。
本研究利用通过生化测试和人体测量获得的生物标志物构建了基于Klemera Doubal method (KDM)[13]的生物学年龄(KDM-BA)。该方法在预测死亡率和年龄相关疾病方面表现良好。首先,结合以往研究的指标利用情况,综合考虑生物标志物在衰老过程中的作用和数据的可及性,我们初步筛选出77个生物标志物。在对每个入选的生物标志物进行Box-Cox转换以符合正态分布后,我们对指标与实足年龄(chronological age,CA)进行了Pearson相关分析,仅保留相关系数|r|>0.1且具有统计学意义的生物标志物。然后根据先验知识,我们进一步排除了一些冗余指标。最终纳入了18个生物标志物:收缩压(SBP)、腰臀比(WHR)、体脂率(BFP)、1 s用力呼气容积(FEV1)、平均红细胞体积(MCV)、碱性磷酸酶(ALP)、天门冬氨酸氨基转移酶(AST)、C-反应蛋白(CRP)、胱抑素C(CysC)、γ-谷氨酰转肽酶(GGT)、足跟定量超声指数(QUI)、糖化血红蛋白(HBA1C)、胰岛素样生长因子1(IGF-1)、甘油三酯(TG)、尿酸盐、尿素、白蛋白(ALB)和维生素D(VD)。
基于筛选出的指标,我们利用UKB全人群基线数据分性别对CA与所有生物生物标志物进行回归训练,并根据训练得出的模型参数计算了KDM-BA。最后,生物衰老加速定义为BA与CA之差,正值表明该个体的生理年龄比基于CA的预期年龄要大,衰老程度更严重,反之亦然。
根据HRT使用情况分组描述研究对象的基线特征,其中连续型变量以中位数和四分位数间距描述、分类变量用频数和构成比描述。以未曾接受过HRT为参考组,采用多因素线性回归模型来分析HRT使用情况与BA加速之间的关联。模型1为不调整任何协变量的粗模型;模型2调整了一般社会因素、生活行为和心理因素,包括年龄、种族、受教育水平、职业状况、汤森剥夺指数(Townsend deprivation index, TDI)、吸烟状况、饮酒状况、饮茶状况、饮咖啡状况、健康膳食、失眠状况、抑郁症状、焦虑症状、体力活动、身体质量指数(body mass index,BMI)、是否患有严重疾病(包括心血管疾病、糖尿病、癌症);模型3在模型2的基础上增加调整了生殖史因素,包括初潮年龄、初产年龄、是否曾口服避孕药、自然流产史、人工流产史。为探索人群特征对HRT使用与BA加速之间关系可能的影响,本研究针对年龄、吸烟状况、抑郁症状、BMI、是否曾口服避孕药、自然流产史进行了分层分析。在上述所有分析中,采用了多重填补法(共五次)来填补协变量的缺失数据,然后根据Rubin规则对所有数据集的结果进行了汇总[14]
为验证研究结果的稳健性,本研究进行了一些敏感性分析。首先,排除了基线自报患有严重疾病的参与者进行重复分析。其次,使用未填补的数据再次进行分析。最后,计算了E值以评估可能存在的未测量混杂变量对结果的影响[15]
本研究采用R 4.1.2软件进行统计分析。
本研究共纳入了96 889名研究对象,未接受过HRT的有55 678人,接受过HRT的有41 211人。在接受过HRT的人中,大多数为绝经后0~10年开始,占56%。研究对象的平均年龄为60.83(56.50, 64.67)岁,BA加速为-0.09(-3.97, 4.06)。相比于未接受过HRT和绝经前10年以前开始接受HRT的人,绝经前10年到绝经后10年开始接受HRT的人有更小的BA加速。与未接受过HRT的人相比,绝经前10年到绝经后10年开始接受HRT的人中白人、现在不饮酒、失眠、曾使用过口服避孕药的占比更高。基线特征描述见表1
HRT使用情况与BA加速的关系见图1。3个模型系数的总体趋势保持一致。在调整了所有一般社会因素、生活行为因素与生殖史因素的模型3中,与未接受HRT的人相比,绝经前10年以前开始接受HRT的人有更大的BA加速但结果无统计学意义(β=0.10, 95%CI:-0.15~0.35);绝经前0~10年开始(β=-0.28, 95%CI:-0.36~-0.19)和绝经后0~10年开始(β=-0.39,95%CI:-0.46~-0.31)的人与BA加速呈负关联,且结果具有统计学意义,其中绝经后0~10年开始的人负关联更强。而绝经后10年后与BA加速有较小的但无统计学意义的负关联(β=-0.07, 95%CI:-0.33~-0.19)。
基于人群特征的分层分析的结果见表2。在绝经后0~10年开始接受HRT的人中,小于60岁及有抑郁症状的人呈现出更小的BA加速(<10岁β=-0.61,≥60岁β=-0.34;无抑郁β=-0.37,有抑郁β=-0.64)。其他人群特征未发现明显效应修饰作用。
排除基线自报患有严重疾病的参与者或使用未填补数据进行的重复分析结果与主分析的结果相似,仅系数大小有略微浮动。另外,绝经前0~10年开始接受HRT者的β值和95%CI的E值分别为1.89和1.65,绝经后0~10年开始接受HRT者的β值和95%CI的E值分别为2.16和1.95,说明未测混杂对研究结果的影响较小。
本研究利用UKB的基线数据探究了绝经后妇女HRT的使用情况与生物衰老加速的变化之间的关联,并探索了可能影响这一关联的人群特征因素。研究结果提示,绝经后妇女使用HRT可延缓生物衰老加速,且不同时间段开始接受治疗的效果不同。相较于不接受治疗的人,绝经后0~10年开始接受HRT延缓生物衰老加速的作用最强,绝经前0~10年开始接受HRT的次之,而绝经前过早(10年前)开始治疗可能会加剧BA加速。此外,<60岁和有抑郁症状的绝经后妇女在绝经后0~10年开始接受HRT的延缓加速衰老作用更强。
本研究结果提示绝经后妇女使用HRT或许可以延缓BA加速。既往尚未有探索HRT与生物学年龄关联的研究。有研究关注了HRT与端粒长度的关系。一项病例对照研究认为绝经后妇女长期接受HRT可以抑制端粒的损耗[10]。还有纵向队列研究显示,雌激素的使用可能对载脂蛋白-ε4(阿尔滋海默症和早死的遗传危险因素)携带者的端粒缩短有延缓作用[16]。端粒长度决定了细胞的生存寿命,端粒长度越短死亡风险越高[17]。但有研究者认为,端粒长度预测衰老相关健康结局的能力较弱[18]。而作为复合生物标志物的生物学年龄综合考虑多器官系统的生物标志物,能够反映整体的衰老状态[11]。总之,与既往研究相比,本研究从不同的维度和视角为HRT延缓绝经后妇女生物衰老提供了进一步的证据。
既往的机制学研究也支持本研究的结果。比如,在认知功能方面,雌激素在细胞和动物模型中具有神经保护作用,可以促进脑源性神经营养因子和增加海马体中的突触脊柱密度[19]。在卵巢衰竭导致大脑内源性雌激素水平降低的情况下,HRT可以缓解这一情况,预防阿尔兹海默病,进而可能达到延缓机体衰老的效果。另外,在心血管系统方面,绝经前雌激素通过一氧化氮、细胞因子和低密度脂蛋白胆固醇的氧化等机制抑制动脉粥样硬化斑块的形成[8-9]。绝经后不久提供HRT可以保持这一活性,降低心血管疾病发生的风险。而心血管系统疾病正是衰老相关结局的一大重要组成部分。
中国女性绝经的平均年龄为48~52岁[20],本研究的研究对象主要为年龄集中在55~65岁之间的绝经后妇女,结果提示绝经前后10年内开始接受HRT均能延缓BA加速,其中绝经后0~10年效应最强。并且,分层分析的结果提示<60岁的绝经后妇女在绝经后10内开始接受HRT有更明显的延缓衰老作用。这一结果与绝经后尽早(10年内)或60岁以前开始的HRT的指南相互印证。对于<60岁的女性,绝经后短期内开始使用HRT可以提高葡萄糖处置率且有益于冠状动脉,能降低糖尿病与心血管疾病风险[21-22]。对于≥60岁的女性,一方面,随着雌激素持续缺乏时间的增加,雌激素受体功能丧失,接受HRT降低糖尿病风险的效果减弱;另一方面,其血管内可能已经形成了动脉粥样硬化斑块,在此时接受HRT会导致斑块脱落或破裂,导致严重的健康结局[23]。因此绝经后早期开始HRT是发挥雌激素有益作用的关键。此外,许多衰老相关的疾病发生在绝经后10~15年的妇女身上[24],而绝经综合征发生在绝经前后数年内[25],这为我们提供了宝贵的“窗口期”,HRT可以作为除生活行为方式管理之外的有效一级预防干预措施。
HRT在《北美绝经协会2022年激素疗法立场声明》[26]与《中国绝经管理与绝经激素治疗指南(2023)》[20]等指南推荐中得到了再次的强调与重视。目前,我国民众对于激素治疗的了解和使用率逐渐提高[27],但相比于发达国家仍有较大提升空间。本研究的结果作为激素治疗益处的又一证据,或许能为公共卫生指导政策与绝经后女性的预防保健工作提供科学依据。
本研究具有一定的优势。一方面,据笔者所知这是第一个研究HRT与BA加速关联的研究。另一方面,本研究具有较大样本量,且进行了多个敏感性分析以验证研究结果的稳健性。同时,本研究也存在一些局限性。首先,HRT情况来源于自填问卷,可能存在回忆偏倚和暴露错分。其次,HRT的配方、剂量和给药途径并未详细调查,这可能会影响其对生物衰老加速的作用。再次,UKB仅在基线调查时进行了全人群的生化检测,而生物学年龄的计算需要生化指标,受限于数据可及性,本研究为观察性横断面研究,尽管我们已尽量控制已测量混杂变量,仍然可能存在未测量混杂和反向因果的情况。最后,本研究样本人群仅来源于UKB,因此,研究结果在外推到其他人群时需要谨慎。
综上,本研究表明在绝经前后10年内开始接受HRT可以延缓绝经后妇女的生物衰老,并且这一效应在小于60岁的人群中更为明显。鉴于绝经是每一位妇女的必经之路且激素治疗在我国的推行仍任重而道远,本研究能为制定相关政策和指导提供科学依据。
