Objective

To investigate the reporting quality of notifiable infectious diseases in Jiangxi Province, analyze the problems in the reporting of infectious diseases, and explore the countermeasures to further standardize the reporting and management of infectious diseases.

Methods

The report card information and quality evaluation data of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 were collected and sorted out. The database was established by Excel2013, and the normal operation rate, timely reporting rate, timely audit rate, and reporting accuracy rate of the institutional network were calculated with the use of SPSS22.0. The quality was evaluated comprehensively by Technique for Order Preference by Similarity to an Ideal Solution (TOPSIS) method.

Results

The timely reporting rate of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 was 99.95%, the timely audit rate was 99.97%, and the accuracy rate was 97.86%. The comprehensive quality of legal infectious disease reports increased year by year during 2016 and 2021.

Conclusion

The information reporting of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 is generally good, but there are still some problems such as untimely reporting, untimely examination, and non-standard reporting. Standardized management should be further strengthened and the informatization construction of medical institutions should be promoted to improve the quality of infectious disease information reporting and provide timely and accurate surveillance information for disease prevention and control.

目的

了解江西省法定传染病报告质量,分析传染病填报工作中存在的问题,探讨针对性措施,为进一步规范传染病疫情报告管理工作。

方法

收集整理2016—2021年江西省法定传染病报告卡信息及质量评价数据,利用Excel 2013建立数据库,计算机构网络正常运行率、及时报告率、及时审核率和填报准确率,应用SPSS22.0完成率的计算。采用TOPSIS法进行质量综合评价。

结果

2016—2021年江西省法定传染病报告的及时报告率为99.95%,及时审核率为99.97%,准确率为97.86%;2016—2021年间法定传染病报告的综合质量逐年上升。

结论

2016—2021年江西省法定传染病信息报告情况总体较好,但仍存报告、审核不及时以及填报不规范等现象,应进一步加强规范管理,推进医疗机构信息化建设,以提升传染病信息报告质量,为疾病防控提供及时、准确的监测信息。

目的

了解江西省法定传染病报告质量,分析传染病填报工作中存在的问题,探讨针对性措施,为进一步规范传染病疫情报告管理工作。

方法

收集整理2016—2021年江西省法定传染病报告卡信息及质量评价数据,利用Excel 2013建立数据库,计算机构网络正常运行率、及时报告率、及时审核率和填报准确率,应用SPSS22.0完成率的计算。采用TOPSIS法进行质量综合评价。

结果

2016—2021年江西省法定传染病报告的及时报告率为99.95%,及时审核率为99.97%,准确率为97.86%;2016—2021年间法定传染病报告的综合质量逐年上升。

结论

2016—2021年江西省法定传染病信息报告情况总体较好,但仍存报告、审核不及时以及填报不规范等现象,应进一步加强规范管理,推进医疗机构信息化建设,以提升传染病信息报告质量,为疾病防控提供及时、准确的监测信息。

Objective

To investigate the reporting quality of notifiable infectious diseases in Jiangxi Province, analyze the problems in the reporting of infectious diseases, and explore the countermeasures to further standardize the reporting and management of infectious diseases.

Methods

The report card information and quality evaluation data of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 were collected and sorted out. The database was established by Excel2013, and the normal operation rate, timely reporting rate, timely audit rate, and reporting accuracy rate of the institutional network were calculated with the use of SPSS22.0. The quality was evaluated comprehensively by Technique for Order Preference by Similarity to an Ideal Solution (TOPSIS) method.

Results

The timely reporting rate of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 was 99.95%, the timely audit rate was 99.97%, and the accuracy rate was 97.86%. The comprehensive quality of legal infectious disease reports increased year by year during 2016 and 2021.

Conclusion

The information reporting of notifiable infectious diseases in Jiangxi Province from 2016 to 2021 is generally good, but there are still some problems such as untimely reporting, untimely examination, and non-standard reporting. Standardized management should be further strengthened and the informatization construction of medical institutions should be promoted to improve the quality of infectious disease information reporting and provide timely and accurate surveillance information for disease prevention and control.

及时和准确的信息报告是传染病监测的灵魂,是传染病防控工作的基础[1]。传染病监测系统是《中国疾病预防控制信息系统》中最重要、最基础的业务系统之一,是实现国家、省、市、县四级疾病预防控制机构对传染病疫情信息实时动态监测,实行监测信息一体化管理和共享的信息系统。持续监测和评价传染报告卡填报情况,发现和解决传染病病例在报告、管理等各环节中存在的问题,是保证报告质量不断提高的重要环节,也是整个传染病疫情报告质量控制的核心工作[2]。本研究对2016—2021年江西省法定传染病报告质量进行系统评价,分析传染病填报工作中存在的问题,为提高传染病信息报告质量,加强传染病防控提供信息。
2016—2021年江西省法定传染病报告卡信息来源中国疾病预防控制信息系统的业务子系统《监测报告管理》,传染病报告质量评价数据来源于业务子系统《传染病监测》。
依据《传染病信息报告管理规范(2015年版)》及《全国传染病信息报告管理工作技术指南(2016年版)》[3-4],对传染病信息报告质量进行评价,主要指标包括机构信息网络正常运行率、及时报告率、及时审核率、填报准确率等。
1.2.1 机构网络正常运行率=网络正常运行机构数/机构总数×100%。在统计期限内,传染病诊疗机构用户能够正常登录网络直报系统,视为机构信息网络正常运行。传染病诊疗机构包括医院(美容医院、按摩医院、精神病医院等专科医院除外)、社区卫生服务中心(站)、卫生院、门诊部、诊所、医务室、村卫生院、急救中心(站)、采供血机构、妇幼保健院(所、站)、专科疾病预防院(所、站)(职业病防治院除外)、疾病预防控制中心(防疫站)等。
1.2.2 及时报告率=及时报告卡片数/报告卡片总数×100%。评价传染病监测系统中所有法定传染病报告卡,包括原始卡、订正卡、已删除卡。报告卡的录入时间与医生诊断时间的间隔在2 h(甲类传染病和乙类传染病中的肺炭疽、传染性非典型肺炎、新型冠状病毒肺炎)或24 h(其他乙丙类传染病)之内为及时报告。原始卡为录入时间与(初次)诊断时间的间隔时间,订正卡为订正报告时间与(修改后)诊断时间的间隔时间。
1.2.3 及时审核率=及时审核卡片数/审核卡片总数×100%。评价传染病监测系统中所有法定传染病报告卡,包括原始卡、订正卡、已删除卡。报告卡终审时间与录入时间的间隔时间在2 h(甲类传染病和乙类传染病中的肺炭疽、传染性非典型肺炎、新型冠状病毒肺炎)或24 h(其他乙丙类传染病)之内为及时审核。原始卡为终审时间与录入时间的间隔时间,订正卡订正终审时间与订正报告时间的间隔时间,未审核状态的删除卡为删除时间与录入/订正报告时间的间隔时间。
1.2.4 填报准确率=填报准确的卡片数/报告卡片总数×100%。评价传染病报告填报准确性,包括原始卡、订正卡。准确率填报不准确报告主要包括逻辑错误和填报缺漏,其中逻辑错误主要包括:诊断日期早于发病日期,死亡日期早于发病日期,医生填卡日期早于发病日期,录入日期早于发病日期,年龄与职业不相符等;填报缺漏主要包括:居民有效证件未填写,民工、教师、医务人员、工人、干部职员未填写发病时所在的工作单位名称,学生、幼托儿童未填写所在学校(托幼机构)名称及班级,14岁及以下儿童未填写家长姓名等。
1.2.5 质量综合评价统计辖区内各级各类医疗机构网络正常运行率、法定传染病及时报告率、及时审核率和填报准确率等指标,采用TOPSIS法进行综合质量评价[5]
整理2016—2021年江西省法定传染病报告卡信息及质量评价数据,利用Excel 2013建立数据库,统计描述机构网络正常运行率、及时报告率、及时审核率和填报准确率,应用SPSS22.0完成统计分析。检验水准α=0.05。
2016—2021年江西省传染病诊疗机构数分别为2 356、2 389、2 400、2 420、2 425和2 451家,各年度机构信息网络正常运行率均为100%。各级各类医疗卫生机构共报告法定传染病报告卡1 374 708张,其中医院、卫生院、妇幼保健机构为主要报告机构,分别占72.57%(997 648/1 374 708)、13.65%(187 613/1 374 708)、9.15%(125 774/1 374 708)。
2016—2021年江西省报告法定传染病卡片分别为227 713、240 769、245 241、229 758、227 217和204 010张,及时报告率从2016年的99.92%逐年上升到2019年99.97%,2020年和2021年出现下降趋势,分别为99.96%和99.94%。2016—2021年共出现未及时报告卡672张,年均及时报告率为99.95%,医学科学研究机构和其它卫生机构及时报告率均为100%,采供血机构和疾病预防控制机构及时报告率分别为99.67%和99.76%,其余类型机构及时报告率在99.83%~99.97%之间。
2016—2021年工作日与休息日平均传染病及时报告率分别为99.95%、99.94%。工作日工作时段与非工作时段平均传染病及时报告率分别为99.95%、99.96%。见表1
2016—2021年江西省法定传染病报告卡应审核卡片数分别为219 596、233 505、251 571、250 912、221 552和196 738张,出现未及时审核卡350张,及时审核率为99.97%。工作日与休息日平均传染病及时审核率分别为99.98%、99.95%;工作日工作时段与非工作时段平均传染病及时审核率均为99.98%。见表2
2016—2021年江西省法定传染病报告信息填报准确率分别为91.57%、97.39%、99.16%、99.28%、99.66%和99.90%。工作日工作时段填报准确率高于非工作时段和周末(χ2=80.032,P<0.05),差异存在统计学意义。见表3
2016—2021年江西省法定传染病填报不准确信息中,有效证件缺报占比由2016年的97.34%下降至2021年6.91%。不准确信息以工作单位/学校缺报为主,其次为日期填报逻辑错误和年龄与职业不符。见表4
采用TOPSIS法综合评价江西省法定报告网络正常运行率、法定传染病及时报告率、及时审核率和填报准确率等指标,2016—2021年江西省法定传染病报告综合质量呈逐年升高趋势,见表5
本研究结果显示,2016—2021年江西省法定传染病报告综合质量逐年上升,与全国整体趋势相似[6-7]。表明在江西省传染病报告管理工作逐步规范,各级各类医疗卫生机构及疾控机构业务人员工作认真负责,成效显著,传染病报告质量稳步上升。
2016—2021年江西省法定传染病及时报告率均保持在99.90%以上,高于天津市、上海市崇明区及连云港市水平[8-10],低于北京市平均水平[11]。出现报告不及时现象主要在于:(1)医疗机构工作繁忙,从病例诊断到纸质传染病报告卡填报,再到系统录入这三个环节均存在时间上的延误;(2)基层医疗机构人员流动频繁,缺乏培训,业务不熟练;(3)日常自查工作流于形式,报告质量监控基本空白。工作日报告及时性高于周末的原因主要为:(1)医疗机构周末期间多为轮休制,临床医师和网络直报人员不足;(2)存在周末传染病报告卡统一集中在周一报送现象。工作日的工作时段报告及时性低于非工作时段,原因主要在于非工作时段传染病报告卡相对较少,工作量少,工作不易被打扰。
2016—2021年江西省法定传染病及时审核率99.97%,高于2016—2019年新疆阿克苏地区质量评价结果[12]。工作日审核及时性高于周末,与周末假日期间人员轮休,节假日值班待遇过低,工作积极性不高等有关。
法定传染病报告信息填报准确率呈持续上升态势,且工作日工作时段高于非工作时段及周末。不准确信息主要为有效证件缺报,以2016—2017年最为严重,随着该项工作的进一步深化和规范及网络直报系统报卡管理逻辑校验功能的完善,填报率明显提高。目前不准确信息以工作单位/学校缺报、日期填报逻辑错误、年龄与职业不符等多见,造成信息填报不准确现象的原因与报告不及时相似,主要源自临床医师填报过程或网报人员录入过程。因此,需加强基层医疗机构和疾控机构传染病报告管理人员的培训,提高业务能力,强化工作责任心,全面提升非工作时间加班、值班待遇。同时加强对医疗机构的技术指导和督导检查亦是提高传染病填报质量的有效手段。
医疗机构信息化的完善程度,对传染病报告质量管理起到至关重要的作用[13-14]。因此,要进一步提升传染病报告质量,应加强医疗机构信息化建设,完善HIS系统中传染病报告管理功能,强化通过信息填报逻辑校验等技术手段,实现传染病报告的自动填报。
