Objective

To analyze the epidemiological and spatial-temporal aggregation characteristics of other infectious diarrhea (OID) in Hubei Province from 2013 to 2022, to provide scientific basis for disease prevention and control.

Methods

The data were collected from the infectious disease surveillance and reporting system of China Disease Prevention and Control Information system. Descriptive epidemiological methods, seasonal index analysis, and spatial autocorrelation analysis were used.

Results

A total of 393 817 OID cases were reported in Hubei Province from 2013 to 2022. The reported annual incidence rate was 49.2/100 000 to 89.2/100 000. The overall incidence increased at first and then decreased. There were two peaks of incidence in winter and summer every year. The incidence rate showed a high-to-low trend from west to east, and the high incidence area changed dynamically with time, mainly in northwest and southwest regions. The sex ratio of reported cases ranged from 1.20 to 1.31, mainly in children under 5 years old (55.3%) and elderly people aged 60 and above(34.0%). The occupational categories were mainly children living at home (53.1%) and farmers (23.0%). Of the reported cases, 43 020(10.9%) were laboratory confirmed cases, of which viral diarrhea accounted for 95.4% and bacterial diarrhea accounted for 4.4%. The results of seasonal index analysis showed that there was a seasonal backshift in the epidemic month of viral diarrhea. The results of spatial autocorrelation analysis showed that the Moran I index was between 0.307 and 0.401, and there was spatial aggregation in each year. The number of “high-high” aggregation areas fluctuated and decreased, and the distribution was mainly in the northwest.

Conclusion

The incidence of OID in Hubei Province fluctuates, and there are obvious characteristics of temporal and spatial aggregation. Attention should be paid to the high incidence areas and key populations in the northwest, and the surveillance, prevention, and control of OID should be strengthened.

目的

分析2013—2022年湖北省其他感染性腹泻病(other infectious diarrhea diseases,OID)的流行病学及时空聚集特征,为疾病防控提供科学依据。

方法

数据来自中国疾病预防控制信息系统传染病监测报告系统,采用描述性流行病学方法、季节指数分析法、空间自相关分析方法进行分析。

结果

2013—2022年湖北省累积报告OID 393 817例,报告年发病率在49.2/10万~89.2/10万之间,发病总体先上升后下降。每年均有冬季和夏季两个发病高峰。发病率从西向东呈由高到低的趋势,高发地区随时间动态变化,以西北和西南地区为主。报告病例的男女性别比在1.20:1至1.31:1之间,年龄以5岁以下儿童(55.3%)和60岁及以上老年人(34.0%)为主,职业分类以散居儿童(53.1%)和农民(23.0%)为主。报告病例中实验室确诊病例43 020例(10.9%),其中病毒性腹泻占比95.4%,细菌性腹泻占比4.4%。季节指数分析结果显示,病毒性腹泻流行月份存在季节性后移现象。空间自相关分析结果显示,Moran I指数在0.307至0.401之间,各年度发病均存在空间聚集性(P<0.05),“高-高”聚集区数量总体波动下降,分布以西北部为主。

结论

湖北省OID发病呈波动态势,存在明显的时空聚集特征,需关注西北部高发聚集区和重点人群,继续加强对OID的监测和防控。

目的

分析2013—2022年湖北省其他感染性腹泻病(other infectious diarrhea diseases,OID)的流行病学及时空聚集特征,为疾病防控提供科学依据。

方法

数据来自中国疾病预防控制信息系统传染病监测报告系统,采用描述性流行病学方法、季节指数分析法、空间自相关分析方法进行分析。

结果

2013—2022年湖北省累积报告OID 393 817例,报告年发病率在49.2/10万~89.2/10万之间,发病总体先上升后下降。每年均有冬季和夏季两个发病高峰。发病率从西向东呈由高到低的趋势,高发地区随时间动态变化,以西北和西南地区为主。报告病例的男女性别比在1.20:1至1.31:1之间,年龄以5岁以下儿童(55.3%)和60岁及以上老年人(34.0%)为主,职业分类以散居儿童(53.1%)和农民(23.0%)为主。报告病例中实验室确诊病例43 020例(10.9%),其中病毒性腹泻占比95.4%,细菌性腹泻占比4.4%。季节指数分析结果显示,病毒性腹泻流行月份存在季节性后移现象。空间自相关分析结果显示,Moran I指数在0.307至0.401之间,各年度发病均存在空间聚集性(P<0.05),“高-高”聚集区数量总体波动下降,分布以西北部为主。

结论

湖北省OID发病呈波动态势,存在明显的时空聚集特征,需关注西北部高发聚集区和重点人群,继续加强对OID的监测和防控。

Objective

To analyze the epidemiological and spatial-temporal aggregation characteristics of other infectious diarrhea (OID) in Hubei Province from 2013 to 2022, to provide scientific basis for disease prevention and control.

Methods

The data were collected from the infectious disease surveillance and reporting system of China Disease Prevention and Control Information system. Descriptive epidemiological methods, seasonal index analysis, and spatial autocorrelation analysis were used.

Results

A total of 393 817 OID cases were reported in Hubei Province from 2013 to 2022. The reported annual incidence rate was 49.2/100 000 to 89.2/100 000. The overall incidence increased at first and then decreased. There were two peaks of incidence in winter and summer every year. The incidence rate showed a high-to-low trend from west to east, and the high incidence area changed dynamically with time, mainly in northwest and southwest regions. The sex ratio of reported cases ranged from 1.20 to 1.31, mainly in children under 5 years old (55.3%) and elderly people aged 60 and above(34.0%). The occupational categories were mainly children living at home (53.1%) and farmers (23.0%). Of the reported cases, 43 020(10.9%) were laboratory confirmed cases, of which viral diarrhea accounted for 95.4% and bacterial diarrhea accounted for 4.4%. The results of seasonal index analysis showed that there was a seasonal backshift in the epidemic month of viral diarrhea. The results of spatial autocorrelation analysis showed that the Moran I index was between 0.307 and 0.401, and there was spatial aggregation in each year. The number of “high-high” aggregation areas fluctuated and decreased, and the distribution was mainly in the northwest.

Conclusion

The incidence of OID in Hubei Province fluctuates, and there are obvious characteristics of temporal and spatial aggregation. Attention should be paid to the high incidence areas and key populations in the northwest, and the surveillance, prevention, and control of OID should be strengthened.

其他感染性腹泻病(other infectious diarrhea diseases,OID)是除霍乱、细菌性和阿米巴性痢疾、伤寒和副伤寒以外的感染性腹泻,为《中华人民共和国传染病防治法》中规定的丙类传染病[1]。2021年全国报告OID病例接近133万例,报告年发病率94.3/10万,位居我国法定传染病的第二位[2],报告年发病率排位进一步上升[3],其流行面广,发病率高,严重腹泻可威胁生命,是危害人群健康的重要疾病,且容易在学校等重点场所引发聚集性疫情和暴发疫情,近年来我国OID突发公共卫生事件数已呈上升趋势[4]。江苏[5]、云南[6]、四川[7]等省份和全国[8]的研究表明,OID的流行特征存在地区差异,分析OID的流行特征,对科学指导防控工作具有重要的公共卫生学意义。湖北省OID报告年发病率近年来持续位居全省法定传染病的前五位,OID突发公共卫生事件数和涉及的病例数位居全国前十位[4],已成为需重点关注的公共卫生问题,但缺乏全省OID长期流行特征和疫情形势等分析的研究。本研究分析2013—2022年湖北省OID的流行特征,为防控工作提供依据。
2013—2022年湖北省OID疫情数据来源于中国疾病预防控制信息系统的传染病监测报告系统,同期人口学数据来源于湖北省统计局。
采用描述性流行病学研究方法分析湖北省OID流行病学特征;采用时间序列数据季节指数分析法,季节指数(%) =同月平均数/各年份月平均数×100%,>100%表示该月为流行季节;采用空间自相关分析方法,通过莫兰指数(Moran I)反映OID发病率的总体空间分布,并进行统计学检验。
应用Excel 2017软件进行数据处理,GraphPad Prism 8.0.2进行季节指数结果展示,ArcGIS 10.3软件制作湖北省县区发病率地图,GeoDa-1.16.0.16软件进行空间自相关性分析,检验水准α=0.05。
2013—2022年湖北省报告OID 393 817例,报告年发病率在49.2/10万~89.2/10万之间,报告年发病率2013年为60.8/10万,此后波动上升,至2017年达到最高值89.2/10万,后波动下降至2022年的60.2/10万,发病总体先上升后下降。见表1。OID每年均有两个发病高峰,冬季高峰均高于夏季高峰。夏季高峰在6—8月,冬季高峰有明显季节性后移现象。见图1。季节指数分析结果显示,2013—2016年的流行月份为10月—次年1月,2017—2019年为11月—次年2月,2020—2022年为12月—次年4月。病毒性腹泻发病的流行月份与总体一致,细菌性腹泻发病的流行月份均为5—10月。见图2
2013—2022年湖北省103个区县OID年报告发病率总体呈下降趋势,随着地理分布从西向东发病率呈现出由高到低的趋势,发病率较高的以西北(十堰市、神农架林区)和西南地区(恩施州)为主。见图3
全局空间自相关分析结果显示,2013—2022年湖北省各年度OID发病率Moran I指数在0.307~0.401之间,各年度均存在空间聚集性(P<0.05)。见表2。局部空间自相关分析结果LISA聚类图显示,湖北省近10年OID发病率呈现出“高-高”、“低-低”、“低-高”、“高-低”四种聚集模式,聚集区域的空间分布模式均有统计学意义(P<0.05)。见表3图4。“高-高”聚集区数量呈现波动下降趋势,分布以鄂西北部十堰市为主,至2022年降为郧阳区、丹江口市、房县、竹山县4个,鄂西南部和中南部2020年之后均无“高-高”聚集区,2020年之前有部分“高-高”聚集区,主要集中在恩施土家族苗族自治州和荆州市;“低-低”聚集区呈波动上升趋势,2013—2016年主要分布在鄂东北部,2017—2019年逐步向中部和西北部延伸且分布较为分散,2020—2022年在西北部的襄阳市和荆门市快速聚集,中东部较为分散;“低-高”聚集区范围较小,以“高-高”聚集区邻近县区为主,2022年有所增加,涉及保康县、兴山县、巴东县3地;“高-低”聚集区2013—2022年均不超过2个,均逐步变为不显著区和“低-低”聚集区。
2013—2022年报告病例的男女性别比变化较小,在1.20∶1~1.31∶1之间。报告病例的年龄分布相近,以5岁以下儿童(55.3%)和60岁及以上老年人(34.0%)为主,5岁以下报告病例以2岁以下为主,其中1岁以下占比25.9%,1岁占比19.1%。报告病例职业分类占比前六位依次为散居儿童、农民、学生、幼托儿童、家务及待业、离退人员,分别占53.1%、23.0%、7.1%、4.1%、4.1%、3.0%、。散居儿童占比呈波动下降趋势,学生、幼托儿童、家务及待业人群占比呈上升趋势,其他职业占比变化不大。
各年龄组月报告发病率差异较大,5岁以下儿童发病率明显高于其他各年龄组,60岁及以上年龄组发病明显高于其他成人组。1岁以下组呈双峰型且冬夏季高峰差距较小,总体呈波动下降趋势;1岁组呈冬季单峰型,总体较为平稳;2岁组、3岁组和4岁组基本呈冬季单峰型,总体呈明显上升趋势;5~14岁、15~24岁、25~59岁组发病水平较低,发病高峰较为平缓;60岁及以上组发病呈双峰型,且夏季高峰明显高于冬季高峰。见图5
2013—2022年报告的393 817例病例中,实验室确诊病例43 020例(10.92%)。以病毒感染为主,占41 038例(95.4%),细菌感染1 891例(4.4%),真菌感染56例(0.1%),混合感染35例(0.1%)。
病毒感染以轮状病毒和诺如病毒为主。轮状病毒发病主要在冬季,以3岁及以下儿童为主,占92.5%。诺如病毒占比2017年开始明显上升,发病主要在秋冬季节,以5岁及以下儿童为主,占90.0%。此外,腺病毒感染从2017年开始明显增加,发病主要在夏秋季,以5岁及以下儿童为主,占91.6%。细菌感染以沙门菌为主,发病时间集中在5—10月,以5岁及以下儿童和25岁以上成人为主,分别占82.3%和14.9%。
全国法定传染病监测数据显示,2015—2021年OID报告年发病率从68.8/10万波动上升至94.3/10万,始终位居法定传染病报告的前三位[2,9]。本次研究结果显示,湖北省OID报告年发病率2020年之前呈上升趋势,2020—2022年有所下降,报告年发病率明显低于全国水平且与全国疫情流行趋势有差异。2020—2022年报告发病率下降可能受到新冠病毒疫情影响,全省提高对传染病防控的重视,且公众佩戴口罩和勤洗手等卫生习惯加强、学校等重点场所疾病监测更为敏感、以及人群聚集活动减少等原因,对于控制传染源、切断传播途径和保护易感人群均发挥了重要作用。但年报告发病率仍位居湖北省法定传染病报告发病率的前五位,OID仍为需要重点关注的传染病。
湖北省OID以病毒性腹泻为主,存在季节性后移现象,可能与气候的长期变化趋势有关。病毒感染以轮状病毒为主,主要为3岁及以下儿童,低龄儿童腹泻就诊率高,疾病负担重[10],深圳市2009年5岁以下儿童轮状病毒感染性腹泻经济负担已超3 000元[7],儿童轮状病毒疫苗接种可有效降低疾病负担[11],但轮状病毒疫苗为二类疫苗,目前未在全国范围内免费接种,接种率受经济水平影响,需继续加强轮状病毒疫苗接种的宣传,并在免疫规划方面做进一步探索。诺如病毒发病率2017年开始明显增加,2017年中国疾病预防控制中心修订《诺如病毒感染暴发调查和预防控制技术指南(2015版)》[12],此后各地诺如病毒检出率快速上升,已成为学校和托幼机构聚集性疫情和暴发疫情的主要病原体[13],仍是聚集疫情和暴发疫情的防控重点。腺病毒发病从2017年开始明显增加,与天津[14]、上海[15]的研究结果一直,腺病毒可引起肠胃炎,合并肠系膜淋巴结炎引起更加严重的肠套叠,并可过多种途径逃避宿主免疫应答[16],需提高警惕,加强监测。此外报告病例中实验室确诊比例为10.9%,与全国[8]和江苏省[5]、四川[7]等地相近,但仍处于较低水平,可能与各地经济发展水平、实验室检测能力以及疾病监测方案等有关。提示各地政府需加强对病原学监测的重视,增加经费投入,提高病原检测能力,让病原监测工作能真实反应病原谱的构成,为OID疫情防控提供数据支撑。
湖北省OID的重点人群中,发病率和发病人数均以5岁以下儿童和60岁及以上老年人为主。其中,1岁以下组和1岁组发病率和发病人数最高,2岁组、3岁组和4岁组发病率呈明显上升趋势,需加强5岁以下儿童肠道传染病的防控,关注高发季节,加强轮状病毒疫苗接种、科学喂养、日常消毒和科学看护等;60岁及以上老年人发病明显高于其他成年组,且发病主要集中在夏季,提示其可能以细菌感染为主,需加强食品和饮水安全的健康教育。发病人群以散居儿童、幼托儿童和学生等未成年群体为主,且幼托儿童和学生占比呈上升趋势,提示需做好学校和托幼机构等重点场所的疫情监管和防控,防范聚集性疫情和暴发疫情。此外,各年度发病人群中农民占比基本在20%以上,王婧等人[17]研究发现,湖北省2017—2020年农村家庭两周内腹泻率平均为2. 82%,明显高出全省发病水平,需进一步重视农村地区感染性腹泻病防治,加强医疗救治,避免发生重症和死亡。
湖北省OID发病存在空间聚集性分布,重点地区“高-高”聚集区总体呈逐年缩小趋势,西南和中南方向的“高-高”聚集区已连续2年为不显著区,“低-低”聚集区逐年扩大,“高-低”聚集区和“低-高”聚集区范围均较小,提示疫情形势总体趋缓。但鄂西北部持续为“高-高”聚集区,可能与当地气候和经济发展水平等有关,为防控的重点区域,且在周边连续3年无“低-高”聚集区后,2022年增加保康县、兴山县、巴东县3地,涉及襄阳市、宜昌市、恩施州3个市州,西北部“高-高”聚集区疫情存在蔓延可能。需动态监测聚集区,关注周边地区发病率的变化,加大对重点地区的防控指导和资源支持。
综上所述,湖北省OID发病率近五年呈波动下降趋势,但发病存在明显的时空聚集特征,需加强西北部高发地区的防控工作,重点关注5岁以下儿童和60岁以上老年人等重点人群、学校和托幼机构等重点场所以及农村地区的防控和医疗救治。
