Objective

To explore the mediating effect of activities of daily living (ADL) between arthritis and depression in the elderly, and to provide reference for preventing and intervening the occurrence and development of depression in the elderly.

Methods

Based on the 2018 China Health and Retirement Longitudinal Study (CHARLS) data, stepwise regression analysis and Bootstrap intermediary test were used to analyze the mediating effect of ADL between arthritis and depressive symptoms in the elderly.

Results

The prevalence rate of arthritis was 8.12%, and the detection rate of depressive symptoms was 44.60%. Arthritis was positively correlated with ADL (r=0.025, P<0.05), arthritis was positively correlated with depression symptoms (r=0.036, P<0.01), and ADL was positively correlated with depression symptoms (r=0.348, P<0.001). ADL plays a mediating role in the relationship between arthritis and depressive symptoms. The mediating effect of ADL on depressive symptoms was 0.009,and the mediating effect accounted for 25.71% of the total effect.

Conclusion

Arthritis has a certain influence on the depressive symptoms of the elderly through ADL. Comprehensive intervention measures should be taken to improve the joint health of the elderly, reduce the prevalence of arthritis, and improve ADL of the elderly, to prevent the occurrence of depression in the elderly.

目的

探讨日常生活能力在老年人关节炎和抑郁症状间的中介效应，为预防和干预老年人抑郁症的发生发展提供参考依据。

方法

基于2018年CHARLS数据，采用逐步回归分析和Bootstrap中介检验分析日常活动能力在老年人关节炎与抑郁症状间的中介效应。

结果

关节炎患病率为8.12%，抑郁症状检出率44.60%。关节炎与日常活动能力呈正相关（r=0.025，P<0.05），关节炎与抑郁症状呈正相关（r=0.036，P<0.01），日常活动能力与抑郁症状呈正相关（r=0.348，P<0.001）。日常活动能力在关节炎和抑郁症状之间起部分中介作用，日常生活能力对抑郁症状的中介效应值为0.009，中介效应为总效应的25.71%。

讨论

关节炎可通过日常活动能力对老年人抑郁症状产生一定影响，应采取综合性干预措施改善老年人关节健康，减低关节炎患病情况，提高老年人日常活动能力，进而预防老年人抑郁的发生。

目的

探讨日常生活能力在老年人关节炎和抑郁症状间的中介效应，为预防和干预老年人抑郁症的发生发展提供参考依据。

方法

基于2018年CHARLS数据，采用逐步回归分析和Bootstrap中介检验分析日常活动能力在老年人关节炎与抑郁症状间的中介效应。

结果

关节炎患病率为8.12%，抑郁症状检出率44.60%。关节炎与日常活动能力呈正相关（r=0.025，P<0.05），关节炎与抑郁症状呈正相关（r=0.036，P<0.01），日常活动能力与抑郁症状呈正相关（r=0.348，P<0.001）。日常活动能力在关节炎和抑郁症状之间起部分中介作用，日常生活能力对抑郁症状的中介效应值为0.009，中介效应为总效应的25.71%。

讨论

关节炎可通过日常活动能力对老年人抑郁症状产生一定影响，应采取综合性干预措施改善老年人关节健康，减低关节炎患病情况，提高老年人日常活动能力，进而预防老年人抑郁的发生。

Objective

To explore the mediating effect of activities of daily living (ADL) between arthritis and depression in the elderly, and to provide reference for preventing and intervening the occurrence and development of depression in the elderly.

Methods

Based on the 2018 China Health and Retirement Longitudinal Study (CHARLS) data, stepwise regression analysis and Bootstrap intermediary test were used to analyze the mediating effect of ADL between arthritis and depressive symptoms in the elderly.

Results

The prevalence rate of arthritis was 8.12%, and the detection rate of depressive symptoms was 44.60%. Arthritis was positively correlated with ADL (r=0.025, P<0.05), arthritis was positively correlated with depression symptoms (r=0.036, P<0.01), and ADL was positively correlated with depression symptoms (r=0.348, P<0.001). ADL plays a mediating role in the relationship between arthritis and depressive symptoms. The mediating effect of ADL on depressive symptoms was 0.009,and the mediating effect accounted for 25.71% of the total effect.

Conclusion

Arthritis has a certain influence on the depressive symptoms of the elderly through ADL. Comprehensive intervention measures should be taken to improve the joint health of the elderly, reduce the prevalence of arthritis, and improve ADL of the elderly, to prevent the occurrence of depression in the elderly.

随着我国人口老龄化问题日益凸显，老年抑郁症已成为威胁我国老年人生命健康的公共卫生难题[1]。研究表明，抑郁症不仅会损害老年人的社会功能和生活质量，甚至使得老年人自杀率升高，给我国乃至世界带来沉重的医疗负担[2-3]。因此探讨我国老年人抑郁症状的影响因素及干预措施具有重要现实意义。关节炎是中老年人常见疾病，不仅会造成关节活动受限，损害活动能力[4]，而且会增加抑郁及全因死亡的风险[5-6]。老年人日常活动能力受损，失去对原有生活的控制，易出现抑郁症状[7]。梳理文献多证实关节炎、日常活动能力与抑郁症状之间互为相关，但三者关系研究较为欠缺。因此，本研究旨在探析关节炎与抑郁症状的关系，并探讨日常活动能力的潜在中介作用，为减缓老年人抑郁症的进展，制定老年人心理健康促进措施提供理论依据和干预方向。
1　资料与方法

1.1　资料来源
本研究使用2018年中国健康与养老追踪调查（China Health and Retirement Longitudinal Study，CHARLS）数据。这项调查采用多阶段分层随机抽样策略，覆盖了中国22个省份和自治区，约占中国总人口的85%[8]。因此，这些调查数据高度代表了整个人口。研究纳入年龄≥60岁的10 497名老年人，过滤异常值和缺失值后，最终纳入6 018名老年人。中国健康与养老追踪调查已获得北京大学生物医学伦理委员会的批准（IRB0000105 2-11015），所有调查对象均签署知情同意书[8]。
1.2　研究工具
1.2.1　控制变量
本研究的控制变量主要包括年龄、性别、婚姻、受教育程度、居住地、自评健康、吸烟和饮酒状况等。
1.2.2　解释变量
本研究的解释变量是关节炎患病，通过问卷问题“是否被医生诊断出患有关节炎或风湿病？”确定关节炎患者，回答“是”则判断为患有关节炎，“否”为未患有关节炎。
1.2.3　中介变量
本研究的中介变量是日常活动能力，采用日常活动能力量表[9]进行评定。该量表分为基础性日常活动能力（BADL）和工具性日常活动能力（IADL）。其中BADL包括沐浴、进食、起床、穿衣服、上厕所、控制大小便6项；IADL包括烹饪、致电、购买东西、服用药物、家务处理、财务管理6项。每个条目根据“我完全可以做到”到“我根本做不到”的程度依次计1～4分。总分为12～48分，得分越高表明老年人日常生活能力越差。本研究中，量表Chronbach α系数为0.835。
1.2.4　被解释变量
本研究的被解释变量是抑郁症状，采用流调中心抑郁量表-10（CESD-10）[10]，从抑郁、积极情绪、躯体症状等3个维度10个条目来评价老年人的抑郁症状。抑郁和躯体症状条目正向赋分，积极情绪维度条目反向赋分。总分在0～30分之间，分数越高表明抑郁症状越严重。CESD-10评分≥10分视为存在抑郁症状[11]。本研究中，量表Chronbach α系数为0.752。
1.3　统计方法
采用SPSS 25.0软件对数据进行清洗整理及统计分析。不同特征老年人抑郁症状发生的组间比较进行t检验或χ2检验；控制人口学变量后对关节炎、日常活动能力和抑郁症状进行Spearman相关分析；使用逐步回归分析、偏差校正百分位Bootstrap方法进行日常生活能力的中介效应检验。检验水准α=0.05。
2　结果

2.1　研究对象基本情况
共6 018名老年人被纳入本研究中，其中男性2 749名（45.7%），女性3 269名（54.3%）；年龄：60～69岁3 868人（64.3%），70～79岁1 785人（29.7%），≥80岁365人（6.1%）；文化程度：文盲1 746名（29.0%），小学2 864名（47.6%），初中及以上1 408名（23.4%）；婚姻状态：在婚4 759名（79.1%），非再婚1 259人（20.9%）；居住地：农村4 480名（74.4%），城市1 538人（25.6%）；吸烟状况：否4 546名（75.5%），是1 472名（24.5%）；饮酒状况：否4 270名（71.0%），是1 748名（29.0%）；自评健康：好970名（16.1%），差5 048名（83.9%）。此外，本研究共有489名老年人患有关节炎，总体患病率为8.13%；2 684名老年人具有抑郁症状，占比为44.6%。
2.2　不同特征老年人抑郁症状发生比较
根据CESD-10评分划分为抑郁症状组（CESD-10≥10分）和非抑郁症状组（CESD-10<10分），其中非抑郁症状组3 334名（55.4%），抑郁症状组2 684名（44.6%）。结果显示老年人抑郁症状组与非抑郁症状组之间性别、教育水平等不同特征的构成比差异具有统计学意义（P<0.001）。见表1。
2.3　老年人关节炎、日常活动能力与抑郁症状的相关分析
控制人口学变量后，对关节炎、日常生活能力和抑郁症状进行Spearman相关分析。Spearman相关分析结果显示，老年人关节炎患病和日常活动能力受损均与抑郁症状呈正相关关系（r1=0.025，P<0.05；r2=0.348，P<0.05）；关节炎患病与抑郁症状之间也呈正相关关系（r3= 0.036，P<0.01）。见表2。
2.4　日常活动能力在关节炎与抑郁之间的中介效应分析
标准化处理所有的变量后，采用SPSS宏中的Model 4检验日常活动能力在老年人关节炎和抑郁症状之间的中介作用。表3中模型1和模型2结果显示关节炎对日常活动能力和抑郁症状均有影响（c值，t=2.864，P<0.05；a值，t=2.463，P<0.05）；模型3结果显示在引入日常活动能力得分后，关节炎、日常活动能力得分均对抑郁症状有影响（即c’值，t= 2.251，P<0.05；b值，t=23.220，P<0.001）。建立中介模型后显示，见图1，关节炎对抑郁症状总效应值为0.035，直接效应值为0.026，且ab与c’同号，认为日常活动能力在关节炎与抑郁症状间存在部分中介效应。此外，表4结果显示日常生活能力的中介效应95%CI不包括0，具有统计学意义，且直接效应显著，表明日常活动能力在关节炎与抑郁症状间存在部分中介效应，中介效应0.009，总效应0.035，中介效应占比25.71%。
3　讨论

3.1　老年人抑郁症状处于较高水平
本研究发现44.60%的老年人存在抑郁症状，高于2011、2013和2015年CHARLS截面数据得出的检出率（34.54%、30.02%、33.08%)[12]，一方面可能是随着老年人群年龄增长，老年人的身体机能和认知能力不断下降，导致躯体功能障碍和自理能力下降或丧失，更容易出现抑郁症状[13]。另一方面可能是老年人的社会角色和经济地位的改变等应激事件增加抑郁症状的发病风险。另外，不同的抽样方法、年龄组和样本量等因素也会导致结果差异。
3.2　老年人关节炎、日常活动能力与抑郁症状的相关性分析
本研究发现，老年人关节炎患病与日常活动能力受损呈正相关，这与Rodriguez等[7]研究结果一致，可能是关节炎患病导致软骨损伤、关节囊和支持韧带的松动，进一步导致关节进行性功能丧失，损害老年人日常活动能力[14]。研究结果显示，日常活动能力受损与抑郁症状正相关，与相关研究结果一致[15]。根据活动理论[16]，老年人活动能力的减退和生活角色的丧失阻碍了他们参与社会活动，导致他们自愿退出社会，从而加重了抑郁症状。此外，老年人关节炎患病与抑郁症状呈正相关，与Xue等[17]结论一致。一方面，从生理学上解释，关节炎可能增加主要促炎细胞因子如肿瘤坏死因子-α等的表达，而这些促炎剂可能是与抑郁症状发展机制相关的致病因素[18-19]。另一方面，这可能由于关节炎导致疼痛、疲劳等持续身体症状和衰弱，引发“习得性无助”认知[20]，即当人们遇到普遍、反复出现的负面事件时，他们经常会感到无助和失控，导致缺乏生活动力，从而加剧抑郁症状的发展；而且关节炎作为一种慢性、持续发展的疾病，严重可引起进行性功能障碍，继而降低患者的自尊和自我效能，从而引起更高的抑郁症风险[15]。
3.3　日常生活能力在老年人关节炎与抑郁症状间起中介作用
本研究结果表明，日常活动能力在关节炎患病和抑郁症状之间存在部分中介作用，即减少关节炎的发病率可以直接缓解老年人的抑郁症状，也可以通过提高老年人的日常活动能力以减少关节炎对老年人抑郁症状的影响。其中作用机制可能是当老年人患有关节炎后，易导致关节僵硬、疼痛和功能障碍等症状[21]，直接影响老年人的日常活动能力和社会角色适应能力，继而剥夺其自我价值感，对生活产生无力感，增加了抑郁症状发生的可能性[22]；同时日常活动能力也会影响机体神经系统，从而增加抑郁症状发生的风险[23]。值得注意的是，中介效应分析结果显示与关节炎对抑郁症状的直接效应值相比，日常活动能力对抑郁症状直接效应值更大，说明老年人日常活动能力受损对抑郁症状的进展影响更大。因此，为了促进老年人的心理健康，医护工作者应关注老年人的躯体症状，增强老年人自身预防意识，充分调动老年个体的能动性，加强对关节炎的健康管理和预防工作，以降低老年人关节炎患病情况外；同时，更应重视加强老年人日常活动能力的提升，提供康复训练等保护性支持，减少老年人病、伤、残发生，深入探讨日常活动能力对抑郁症状的中介因素研究，以预防老年人抑郁症的发生。
综上所述，关节炎、日常生活能力影响老年人的抑郁症状，其中日常生活能力起部分中介作用。因此医护工作者在降低老年人关节炎患病水平的同时，应关注老年人的日常生活能力，并进行动态评估和早期干预，充分发挥其在提高老年人心理健康水平中的作用。同时，本研究采用横断面研究的方法来验证关节炎、日常生活活动能力和抑郁症状之间的因果关系存在一定的局限性，且采用自我报告信息测量主要变量，可能导致回忆偏倚，后续需开展多中心、纵向研究进一步探讨其影响机制。
