Objective

To explore the risk factors of early neurological deterioration (END) in patients with acute ischemic stroke(AIS) and establish a predictive model.

Methods

In total 622 AIS patients treated in Baiyun Hospital affiliated to Guizhou Medical University from January 2018 to December 2022 were selected and divided into END group (n=127) and non-END group (n=495) according to the diagnostic criteria of outcome events. The clinical baseline data, laboratory data, and imaging data of the patients were retrospectively collected. Based on the Logistic regression algorithm, risk factors for the development of END were investigated. The prediction model was constructed and nomogram was drawn, and the prediction value of the model was evaluated by discrimination, calibration, and clinical applicability.

Results

Multivariate Logistic regression analysis showed that heart rhythm, middle cerebral artery stenosis, carotid artery stenosis, admission NHISS score, fibrinogen, albumin, and brain natriuretic peptide (BNP) were independent risk factors for END. The prediction model was developed using Stata software, and a nomogram was created. Verification set analysis revealed that the predictive model predicted the risk of END with good discrimination, accuracy, and clinical applicability, with an AUC of 0.888 (95%CI: 0.821-0.955), slope of the calibration curve which was close to 1, and net benefit of the clinical decision curve that exceeded the two reference lines.

Conclusion

The model constructed in this study has high degree of discrimination, calibration, and clinical applicability and has good comprehensive prediction efficiency, which has a certain clinical application value for the early prediction of END.

目的

探讨急性缺血性脑卒中(AIS)患者发生早期神经功能恶化(END)的危险因素并建立预测模型。

方法

将2018年1月—2022年12月在贵州医科大学附属白云医院收治的622例AIS患者纳入研究,根据结局事件诊断标准,将病例分为END组127例和非END组495例。回顾性收集患者的临床基线、实验室数据和影像学资料,基于logistic回归算法探讨END发生的危险因素,构建预测模型并绘制列线图,采用区分度、校准度、临床适用度评价模型的预测价值。

结果

多因素二元logistic回归分析显示,心律、大脑中动脉狭窄、颈动脉狭窄、入院NHISS评分、纤维蛋白原、白蛋白、脑利钠肽(BNP)为END的独立危险因素(P<0.05)。通过Stata软件构建预测模型并绘制出列线图,利用验证集分析得到AUC为0.888(95%CI:0.821~0.955),校准曲线斜率接近1,临床决策曲线净收益高于两条参考线,表明该预测模型预测END发生风险具有较好的区分度、准确度和临床适用度。

结论

本研究构建的模型具有较高的区分度、校准度及临床适用度,综合预测效能较好,对END的早期预测具有一定临床应用价值。

目的

探讨急性缺血性脑卒中(AIS)患者发生早期神经功能恶化(END)的危险因素并建立预测模型。

方法

将2018年1月—2022年12月在贵州医科大学附属白云医院收治的622例AIS患者纳入研究,根据结局事件诊断标准,将病例分为END组127例和非END组495例。回顾性收集患者的临床基线、实验室数据和影像学资料,基于logistic回归算法探讨END发生的危险因素,构建预测模型并绘制列线图,采用区分度、校准度、临床适用度评价模型的预测价值。

结果

多因素二元logistic回归分析显示,心律、大脑中动脉狭窄、颈动脉狭窄、入院NHISS评分、纤维蛋白原、白蛋白、脑利钠肽(BNP)为END的独立危险因素(P<0.05)。通过Stata软件构建预测模型并绘制出列线图,利用验证集分析得到AUC为0.888(95%CI:0.821~0.955),校准曲线斜率接近1,临床决策曲线净收益高于两条参考线,表明该预测模型预测END发生风险具有较好的区分度、准确度和临床适用度。

结论

本研究构建的模型具有较高的区分度、校准度及临床适用度,综合预测效能较好,对END的早期预测具有一定临床应用价值。

Objective

To explore the risk factors of early neurological deterioration (END) in patients with acute ischemic stroke(AIS) and establish a predictive model.

Methods

In total 622 AIS patients treated in Baiyun Hospital affiliated to Guizhou Medical University from January 2018 to December 2022 were selected and divided into END group (n=127) and non-END group (n=495) according to the diagnostic criteria of outcome events. The clinical baseline data, laboratory data, and imaging data of the patients were retrospectively collected. Based on the Logistic regression algorithm, risk factors for the development of END were investigated. The prediction model was constructed and nomogram was drawn, and the prediction value of the model was evaluated by discrimination, calibration, and clinical applicability.

Results

Multivariate Logistic regression analysis showed that heart rhythm, middle cerebral artery stenosis, carotid artery stenosis, admission NHISS score, fibrinogen, albumin, and brain natriuretic peptide (BNP) were independent risk factors for END. The prediction model was developed using Stata software, and a nomogram was created. Verification set analysis revealed that the predictive model predicted the risk of END with good discrimination, accuracy, and clinical applicability, with an AUC of 0.888 (95%CI: 0.821-0.955), slope of the calibration curve which was close to 1, and net benefit of the clinical decision curve that exceeded the two reference lines.

Conclusion

The model constructed in this study has high degree of discrimination, calibration, and clinical applicability and has good comprehensive prediction efficiency, which has a certain clinical application value for the early prediction of END.

急性缺血性脑卒中(acute ischemic stroke,AIS)是一种急性脑血管疾病,根据全球疾病负担研究[1],从1990—2019年其发病率一直呈上升趋势。早期神经功能恶化(early neurological deterioration,END)又称为进展性脑卒中,是影响AIS患者预后效果的重要因素[2],其发病率较高(5%~40%),与3个月临床预后效果和高死亡率相关[3-4]。尽管近年来我国的AIS静脉溶栓和血管内治疗率取得显著提升,对患者临床治疗效果取得显著进展[5],但END仍会导致毁灭性的临床结果,这类患者很难完全治愈或者预后效果较差,患者生活质量将受到严重影响,给个人、家庭及社会带来沉重的疾病负担。目前有关END的发病机制尚不清楚[6-7],标准治疗策略尚未建立,尚无有效的特异性预防和治疗方法,只能依靠医护人员通过早期病情评估,发现高风险患者并重点监测,合理制定治疗方案。但国内对其研究报道较少,在临床实践中没有准确快捷的早期预测指标,因此,建立一种能准确预测END发病风险的临床预测模型对于促进前瞻性、早期介入干预以改善卒中患者临床治疗及预后效果具有重要意义。
为此,基于“十四五”期间贵州深入实施“健康中国”战略背景,本研究旨在调查分析急性缺血性脑卒中患者早期神经功能恶化的发生率和危险因素,基于logistic回归构建风险预测模型并呈现列线图,希望能为临床治疗提供客观的风险评估工具,进行及时、准确、有效的病情评估,为医护人员早期筛选和识别出进展性卒中的高危患者提供线索,为推进提高各级医疗机构对AIS患者临床决策能力提供理论依据。
回顾性收集2018年1月—2022年12月贵州医科大学附属白云医院622例AIS住院患者临床资料。纳入标准:(1)符合2018年《中国急性缺血性脑卒中诊治指南》[8]关于急性缺血性卒中的诊断标准,并经头颅电子计算机断层扫描(computed tomography,CT)或(和)磁共振成像(magnetic resonance imaging,MRI)检查确诊;(2)在24 h内发病入院;(3)患者年龄≥18岁。排除标准:(1)病例临床资料缺失;(2)病例既往存在严重器质性心血管疾病、严重呼吸系统疾病、严重消化系统疾病、严重脏器损伤等类型疾病或者遗有严重功能障碍;(3)病例接受静脉内组织纤溶酶原激活剂溶栓治疗、动脉溶栓或机械取栓。本研究已获得贵州医科大学附属白云医院医学伦理委员会批准,批准号:2022伦审第(15)号。
结合相关文献和专家咨询结果,编制统一的病例报告表(eCRF),由经过培训合格的调查员进行资料收集,病例资料录入采用双人双录入方式,分析研究对象的以下因素:(1)一般资料:性别、年龄、血压、心律;(2)既往史:饮酒史、吸烟史、高血压史、糖尿病史、高血脂史、房颤病史、冠心病史;(3)入院NIHSS评分;(4)实验室指标:甘油三脂、总胆固醇、血糖、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、红细胞压积、纤维蛋白原、D-二聚体、同型半胱氨酸、白蛋白、尿素、尿酸、脑利钠肽(BNP)、白细胞计数、中性粒细胞、γ-谷氨酰转移酶、谷草转氨酶、谷丙转氨酶;(5)影像学结果指标:颈动脉狭窄、大动脉粥样硬化、大脑中动脉狭窄、卒中分型(TOAST分型)。
早期神经功能恶化目前国际上尚无统一的指南及共识,不同国家及区域对其定义称呼也有所不同。大部分学者[9-10]认为使用美国国立卫生研究院卒中量表(National Institutes Of Health Stroke Scale,NIHSS)提出的诊断标准为发病7d内NIHSS评分至少增加2分(包括意识、肢体运动各1分)或4分为早期神经功能恶化,本研究选择此诊断标准。
本研究采用SPSS 26.0和Stata两种统计软件对数据进行统计分析和建模。将数据进行预处理、赋值,其中计量资料根据临床指标界值转化为计数资料。按照7:3的比例将研究对象随机分为训练集和验证集,训练集用于模型的学习和训练,验证集用于模型的评价。通过单因素logistic分析方法筛选出差异统计学意义的变量,再纳入多因素logistic回归分析构建预测模型,利用Hosmer-Lemeshow检验对模型拟合度进行评估,采用受试者工作特征曲线(receiver operating characteristic curve,ROC),以曲线下面积(area under curve,AUC)对预测模型效能进行评估,通过临床决策曲线(decision curve analysis,DCA)评价诊断模型临床实用性,所有统计均为双侧检验,检验水准α=0.05。
本研究按照纳排标准进入研究数据库622例,其中男405例,女217例;大动脉粥样硬化型占比最多,为43.6%,其次是小动脉闭塞型和心源性栓塞型,分别为40.4%和9.8%;发生早期神经功能恶化患者127人,发生率为20.4%。
通过单因素logistic分析发现,饮酒史、冠心病史、房颤病史、心律、大脑中动脉狭窄、颈动脉狭窄、大动脉粥样硬化、TOAST分型、入院NHISS评分、纤维蛋白原、白细胞计数、中性粒细胞、白蛋白、γ-谷氨酰转移酶、谷草转氨酶、BNP、尿酸、D-二聚体等18个变量在END组和非END组间差异具有统计学意义。将上述变量纳入多因素logistic逐步回归(向后法)分析,结果显示心律、大脑中动脉狭窄、颈动脉狭窄、入院NHISS评分、纤维蛋白原、白蛋白、BNP等变量可能是与END发生相关的危险因素(P<0.05)。见表1
基于多因素logistic回归,将单因素分析筛选出具有统计学意义的18个变量纳入分析,以是否发生早期神经功能恶化作为因变量(END组=1,非END组=0),通过逐步回归(向后法)并按照赤池信息准则(AIC)最小原则筛选出的变量作为自变量,构建AIS患者发生END的风险预测模型为:Logit(P)=-7.979+0.796×(入院NHISS评分)+1.12×(颈动脉狭窄)+1.185×(大脑中动脉狭窄)+0.565×(D-二聚体)+0.934×(纤维蛋白原)+1.889×(BNP)+0.814×(心律)+1.534×(白蛋白),见表2。分析结果显示模型纳入心律、大脑中动脉狭窄、颈动脉狭窄、入院NHISS评分、纤维蛋白原、白蛋白、脑利钠肽(BNP)、D-二聚体8个指标作为风险预测变量。利用上述8个预测变量构建列线图(Nomogram),图1中各变量对应的线段长度表示了该因素对预测结局事件的贡献值。将各个变量取值后对应的单项分数合者发生END风险的预测概率。
(1)模型区分度模型在训练集中对END预测的AUC为0.865(95%CI:0.823~0.907,P<0.001),约登指数最大值为0.557,灵敏度为66.7%,特异度为88.8%;在验证集中对END预测的AUC为0.888(95%CI:0.821~0.955,P<0.001),约登指数最大值为0.661,灵敏度为79.4%,特异度为86.6%,两者AUC值差异在0.05以内,可以认为模型计即为总分,根据总分对应的预测风险值就是AIS患拟合成功,并且AUC值均大于0.8,提示该风险预测模型效能良好,见图2
(2)模型校准度经过Hosmer-Lemeshow拟合优度检验,结果显示H-L拟合优度检验的P值>0.05(P训练=0.3497,P验证=0.3150),说明当前预测结果和实际状况没有统计学差异,模型拟合效果良好。在图3验证集校准曲线中,回归直线的斜率接近于1(A=1.042),截距接近于0(B=-0.075),表明此预测模型校准度较高。
(3)模型临床适用度图4通过决策曲线分析,END预测模型临床决策曲线显示,横坐标表示概率阈值,纵坐标表示人群被干预后的净收益。在临床决策曲线图中有一条水平的实线,以及一条斜率为负数的实线,模型的决策曲线离这两条实线越远表示获得收益越好。当模型曲线在概率阈值0.8以下时,净收益明显高于两条参考线,表明此预测模型具有较好的临床应用价值。
AIS是一种急性脑血管疾病,END是影响其治疗及预后效果的重要因素,导致患者致残率、死亡率升高,严重影响患者的生活质量[11]。END是由多种危险因素、多种病理生理机制共同作用的复杂病理生理过程,在疾病诊治早期快速对END发生风险进行预测和识别,科学辅助临床治疗方案的选择,尽早采取干预措施是避免转归不良的关键。尽管近年来研究证明及时进行取栓、溶栓与降低END发生率相关,一些可控的预测因素,如血管内治疗的取栓次数,可以作为改善急性缺血性卒中管理的目标[12],但是由于时间窗、患者家属依从性及各级医疗机构医疗资源的限制,很大部分接受传统治疗的患者亟需通过容易获取的临床指标,对病情进行快捷准确的评估,是合理分配医疗资源,提高患者治疗效果和生存质量的可行之路。
在本研究中,127例(20.4%)患者患有END,在不同的卒中亚型中,大动脉粥样硬化型是最常见的类型,占缺血性脑卒中的43.6%,其次是小动脉闭塞型,占40.4%、心源性栓塞型占9.8%,这与之前的研究发表相似[13-14],大动脉粥样硬化很多时候会对颅内动脉造成影响,被认为是亚洲国家,引起卒中最常见的原因。一些数据提示,心源性卒中的负担正在逐步增加,尤其在中国与中东地区[15]。研究结果发现心律异常、大脑中动脉狭窄、颈动脉狭窄、入院NHISS评分偏高、纤维蛋白原异常、白蛋白偏低、BNP异常可能是与END的发生相关的危险因素。入院NIHSS评分较高的患者发生END的风险增加,预示患者神经功能损伤程度较重,梗死面积或水肿面积大,这与先前研究的结果一致[16-17]。医护人员应规范评估标准,对入院NIHSS评分高的病人重点观察,提高护理等级。纤维蛋白原与END发生之间有很强的相关性,并且有研究表示不仅限于伴糖尿病脑卒中患者群体中[18-19]。纤维蛋白原负责通过与糖蛋白IIb / IIIa受体结合来聚集血小板,导致纤维蛋白凝块的形成,纤维蛋白原的减少可以降低血液粘度,并有助于凝块溶解恢复脑血流量,针对高风险患者应合理选择抗凝治疗,制定饮食调理方案。研究发现大脑中动脉段狭窄、颈动脉狭窄是END的危险因素,与相关研究[20]结果一致。脑梗发生时若伴有大脑中动脉或者颈动脉狭窄,一般常会出现机械性阻塞,对缺血区域的血流动力学产生影响,可能阻碍有效侧支循环的建立,导致脑梗死部位血供不足,缺血缺氧的情况更为严重,造成缺血半暗带不可逆性损伤,从而致使END的发生,并且狭窄动脉数目和动脉狭窄程度可作为一项新的独立因素对卒中进展情况进行预测[21]。另外,心律过速、BNP值异常升高,可能存在心功能不全,容易诱发心源性栓塞,而栓塞的存在可致缺氧而加重神经功能损害,进而诱发END[7]。在一项关于缺血性卒中的基础性实验中[22],发现通过外源性人血清白蛋白给药能减少脑肿胀、预防缺血后血栓形成、增加抗氧化活性及对缺血组织的再灌注而具有神经保护作用,白蛋白偏低可能与END发生风险相关。根据研究结果提示,AIS患者起病急,发病迅速,整个病程变化较快,对于END的防治,需要从入院NHISS评分、影像学报告结果、实验室指标的动态监测这几个方面重点关注。医护人员要提升诊治和风险评估能力,患者家属应积极配合并尽快完善相关检查,以便对患者进行准确的入院病情评估以及风险预测,这对于患者临床治疗效果及预后生存质量具有重要意义。
本研究基于临床数据并纳入影像学资料进行研究,模型预测因子较易获取,选择合理、应用方便。国内对此类研究较少,与其中一些研究[23-24]相比,本文模型不仅对数据集拆分后在验证集中进行内部验证,而且对模型除AUC指标评价外还进行了临床适用度评价,以此更客观全面的评价模型总体效能。因为AUC指标更侧重于模型的预测准确性,不能以此判断模型在临床上是否可用[25]。本文模型训练、验证集两者AUC值差异在0.05以内,可以认为模型拟合性强,稳定性高,并且AUC值均接近于1,提示该风险预测模型准确性较好,DCA曲线表示模型较好的临床应用价值,可见该模型综合预测性能表现较佳。END风险预测模型可协助医护人员及时评估并重点监测高风险病人,制定科学的治疗方案,采取恰当的应对措施,及时给予有效干预,对其早期防治具有一定的指导意义和临床价值。本研究的局限性:由于数据来源于单中心回顾性研究,因此未来将继续扩大样本量并在不同区域进行前瞻性多中心多时段研究,应用多种算法对模型进行优化和验证。
本研究针对END发生的相关危险因素及预测模型进行探讨,基于logistic回归构建预测模型,模型验证结果表现出较好的区分度、准确度以及临床应用价值,模型具有一定泛化能力。通过这种预测方法建立预警机制,有助于一线临床医生尽早识别和干预进展性卒中的发生,将诊治关口前移,对于提高脑卒中患者临床治疗及预后效果、减轻患者疾病负担具有公共卫生实际意义,同时也可为其他慢性非传染性疾病的防治提供方法学参考。
