Objective

To analyze the current situation and influencing factors of post competency of full-time staff in nosocomial infection management in different levels of medical institutions in Guizhou Province.

Methods

Based on the principle of convenience sampling, 69 tertiary hospitals and 151 secondary hospitals in 9 cities (regions) of Guizhou Province were selected, and 766 full-time staff of nosocomial infection management were investigated by questionnaire. Multiple linear regression method was used for multivariate analysis.

Results

The total score of post competence of full-time staff of nosocomial infection management in tertiary medical institutions (5.287 ±1.074) was higher than that in secondary medical institutions (4.835 ±1.194). The full-time staff of the third-level medical institutions outperformed those of the second-level medical institutions in terms of the overall level of competency, the self-evaluation scores of 4 core items, and self-evaluation scores of 11 sub-items (t=5.580, P<0.001). The scores of tertiary medical institutions and secondary medical institutions had the lowest score in comprehensive knowledge items, which were (4.394 ±1.401) and (3.614 ±1.529), respectively. The results of multiple regression analysis showed that the professional titles of the full-time staff of nosocomial infection management (B=-0.541, 95%CI:-1.024 to -0.058), the application level of office software (B=-0.854, 95%CI: -1.108 to -0.599), the importance degree the hospital attached to infection control (B=-0.393, 95%CI: -0.707 to -0.078), and the construction of the nosocomial infection control management system (B=-0.759, 95%CI: -1.314 to -0.204) were the independent factors for the post competency in tertiary medical institutions. The age of the full-time staff in the secondary medical institution (B=0.942, 95%CI: 0.435-1.449), the average monthly income (B=0.535, 95%CI: 0.165-0.905), and the application level of office software (B=-0.657, 95%CI: -0.909 to -0.405) were independent factors for the post competency in secondary medical institutions.

Conclusion

The overall post competency of the full-time staff of nosocomial infection management in Guizhou Province is at the upper-middle level, and the full-time staff of the tertiary medical institutions are obviously better than the secondary medical institutions.

目的

分析贵州省不同级别医疗机构医院感染管理专职人员岗位胜任力现状和影响因素。

方法

基于便利抽样原则,抽取贵州省9个市(州)69所三级医院、151所二级医院,对766名医院感染管理专职人员岗位胜任力水平和影响因素进行问卷调查,利用多元线性回归方法进行多因素分析。

结果

目的

分析贵州省不同级别医疗机构医院感染管理专职人员岗位胜任力现状和影响因素。

方法

基于便利抽样原则,抽取贵州省9个市(州)69所三级医院、151所二级医院,对766名医院感染管理专职人员岗位胜任力水平和影响因素进行问卷调查,利用多元线性回归方法进行多因素分析。

结果

三级医疗机构医院感染管理专职人员岗位胜任力自评总分(5.287±1.074)分高于二级医疗机构(4.835±1.194)分。在胜任力总体水平、4项核心条目和11项子条目自评得分上,三级医疗机构专职人员均优于二级医疗机构(t=5.580,P<0.001)。三级医疗机构和二级医疗机构在综合知识条目得分最低,分别是(4.394±1.401)分和(3.614±1.529)分。多元回归分析结果表明,三级医疗机构医院感染管理专职人员的职称(B=-0.541, 95%CI:-1.024~-0.058)、办公软件应用水平(B=-0.854, 95%CI:-1.108~-0.599)、医院对感控工作重视程度(B=-0.393, 95%CI:-0.707~-0.078)和医院感控管理制度建设情况(B=-0.759, 95%CI:-1.314~-0.204)是影响岗位胜任力高低的独立因素,二级医疗机构专职人员的年龄(B=0.942, 95%CI:0.435~1.449)、平均月收入(B=0.535,95%CI:0.165~0.905)和办公软件应用水平(B=-0.657, 95%CI:-0.909~-0.405)是影响岗位胜任力高低的独立因素。

结论

贵州省医院感染管理专职人员整体岗位胜任力处于中等偏上水平,三级医疗机构专职人员明显优于二级医疗机构。

Objective

To analyze the current situation and influencing factors of post competency of full-time staff in nosocomial infection management in different levels of medical institutions in Guizhou Province.

Methods

Based on the principle of convenience sampling, 69 tertiary hospitals and 151 secondary hospitals in 9 cities (regions) of Guizhou Province were selected, and 766 full-time staff of nosocomial infection management were investigated by questionnaire. Multiple linear regression method was used for multivariate analysis.

Results

The total score of post competence of full-time staff of nosocomial infection management in tertiary medical institutions (5.287 ±1.074) was higher than that in secondary medical institutions (4.835 ±1.194). The full-time staff of the third-level medical institutions outperformed those of the second-level medical institutions in terms of the overall level of competency, the self-evaluation scores of 4 core items, and self-evaluation scores of 11 sub-items (t=5.580, P<0.001). The scores of tertiary medical institutions and secondary medical institutions had the lowest score in comprehensive knowledge items, which were (4.394 ±1.401) and (3.614 ±1.529), respectively. The results of multiple regression analysis showed that the professional titles of the full-time staff of nosocomial infection management (B=-0.541, 95%CI:-1.024 to -0.058), the application level of office software (B=-0.854, 95%CI: -1.108 to -0.599), the importance degree the hospital attached to infection control (B=-0.393, 95%CI: -0.707 to -0.078), and the construction of the nosocomial infection control management system (B=-0.759, 95%CI: -1.314 to -0.204) were the independent factors for the post competency in tertiary medical institutions. The age of the full-time staff in the secondary medical institution (B=0.942, 95%CI: 0.435-1.449), the average monthly income (B=0.535, 95%CI: 0.165-0.905), and the application level of office software (B=-0.657, 95%CI: -0.909 to -0.405) were independent factors for the post competency in secondary medical institutions.

Conclusion

The overall post competency of the full-time staff of nosocomial infection management in Guizhou Province is at the upper-middle level, and the full-time staff of the tertiary medical institutions are obviously better than the secondary medical institutions.

随着医院感染管理成为医疗质量安全管理工作关注的重点,医疗机构逐渐将感染防控人才培养和队伍建设摆在了重要位置[1]。上海市卫生健康委2021年下发的《加强上海市医疗机构院感防控队伍建设实施方案》[2]中提出,以岗位胜任力为导向和核心,改善感控人员的培养模式,加大感控人员培养力度。胜任力包括知识、技能、能力和个人属性,这些能力指标是可观察和可衡量的[3]。2019年,美国感染控制和流行病学专业协会(Association for Professionals in Infection Control and Epidemiology,APIC)在2012年版本基础上重新更新了感控专业的胜任力模型[4],旨在明确感控专业人员需要具备的能力范围和职责,包含领导能力、专业的管理、质量改进、业务实践、信息学和研究。我国《医院感染管理办法》[5]《医院感染管理专业人员岗位培训指南》[6]等法规文件明确提出医院感染管理专职人员的职责和岗位知识培训体系和内容要求,但未体现出针对不同级别医疗机构医院感染管理专职人员的职业发展和培养模式有不同要求。目前国内报道的不同级别医疗机构医院感染管理专职人员岗位胜任力存在差异[7-8]。本研究旨在对贵州省不同级别医疗机构医院感染管理专职人员的岗位胜任力进行现况调查和影响因素分析,获取不同级别医疗机构医院感染管理专职人员岗位胜任力的薄弱环节和差异,为开展以胜任力为导向的专职人员系统性、针对性培养模式提供参考。
来自全省9个市(州)220所医疗机构,共766名医院感染管理专职人员参与问卷调查。纳入标准:专职从事医院感染管理的工作人员。排除标准:兼职人员、借调人员、休假人员及缺勤人员。该研究调查已获批贵州医科大学附属医院医学科学伦理委员会伦理审查(批件号:2023伦审第775号),研究调查对象均提前知晓该研究内容,自愿参与该调查研究。
编制《医院感染管理专职人员岗位胜任力自评问卷》,内容包括三部分:一是医疗机构基本情况,包括医院对院感工作重视程度、感控文化氛围、感控制度等医院感控环境;二是医院感染管理专职人员基本信息;三是《医院感染管理专职人员岗位胜任力自评量表》,基于陈慧斯[9]学者开发的医院感染管理专职人员岗位胜任力评价模型,包括4个核心维度,11个子维度和47个测量条目,调查对象对岗位胜任力现况进行自评,采用Likert 1~7等距尺度,该量表的Cronbachα系数为0.989。自评分数越高说明认为自己在该项工作的岗位胜任力越强。
2022年4月对符合纳入标准的30名医院感染管理专职人员进行预调查,计算Cronbach α系数为0.985。两名工作人员采用EpiData V3.1进行问卷双录入,建立数据库。
统计分析采用SPSS 22.0软件。首先进行数据的方差齐性检验,在单因素分析中,方差齐性数据采用t检验或方差分析,两两比较采用非目的性比较法Scheff法;方差不齐数据采用非参数秩和检验,比较两个独立样本Mann-Whitney U,多个独立样本两两比较采用Kruskal-Wallis单因素ANOVE(k样本)成对比较法。多因素分析采用多元回归分析。检验水准α=0.05。
全省医疗机构医院感染管理专职人员共计766名,其中二级医院390名,三级医院376名,基本情况见表1
调查结果显示,三级医疗机构医院感染管理专职人员岗位胜任力自评总分为(5.287±1.074)分,高于二级医疗机构的(4.835±1.194)分。在胜任力总体水平、4项核心条目和11项子条目自评得分上,三级医疗机构专职人员均高于二级医疗机构(P<0.05),自评结果见表2
结果表明,影响三级医疗机构医院感染管理专职人员岗位胜任力的因素与二级医疗机构存在差异,见表3
结果显示,影响三级医疗机构医院感染管理专职人员胜任力的因素包括职称、办公软件应用水平以及医院对感控工作重视程度和医院感控管理制度建设情况,二级医疗机构专职人员的年龄、平均月收入和办公软件应用水平是影响岗位胜任力高低的独立因素,见表45
以胜任力为导向的培训模式可以激发医院感染管理专职人员的感控能力水平[10]。美国通过胜任力模型形成医院感染管理人员的教育培训发展路径,包括提供临床教育和专业发展课程,根据院感工作年限和能力对人员进行三个等级的划分,有专业认证考试和时效性,成立APIC桥梁计划和研究员计划[11]。目前国内缺乏针对医院感染管理专职人员的系统性标准化培养路径。侯玉娟等[10]报道以胜任力模型为导向开展基层医院感染管理专职人员岗位培训模式和培训效果评价,可提高基层人员岗位胜任力。目前国内相关研究已基于医院感染管理专职人员开发出岗位胜任力评价模型,比如陈慧斯[9]基于胜任力理论、人岗匹配理论和动机理论构建的感控人员岗位胜任力评价模型,崔扬文[12]等构建的以基本认知、技能、专业知识和个人品质为一级指标的核心能力评价体系。国内研究团队基于疫情后大流行时代医院感染防控从业人员开发的胜任力评估模型,调查对象是参与中国抗击疫情一线的院感人员,包括专业技能、规范化管理能力、专业开发能力、组织协作能力和个人特质5维模型,能反映应急响应能力水平[13]。但是这些模型都存在同一个弊端,由于是自评量表,所以结果易出现主观偏差。
本次调研发现,全省医院感染管理专职人员整体岗位胜任力水平处于中等,表明被调查者认为在自己的岗位上基本能够胜任工作,对自己的胜任力表现较为满意。三级医疗机构医院感染管理专职人员岗位胜任力自评总分为(5.287±1.074)分,二级医疗机构为(4.835±1.194)分,均低于四川省[8]、安徽省[7]、河北省[14]、新疆[15]报道的二级和三级医疗机构水平,考虑本次调查在疫情后完成,大量新入职医院感染管理专职人员院感工作经验少有关,同时与本研究调查对象院感工作年限短、职称低、学历低有关。在胜任力总体水平、4项核心条目和11项子条目自评得分上,三级医疗机构专职人员均高于二级医疗机构(P<0.05),与上海市医院感染管理质量控制中心一项研究报道结果一致[15],三级医疗机构感控人员在基本认知、基本技能、专业知识和个人品质方面的胜任力得分均高于二级医疗机构(P<0.05)。考虑是与三级医疗机构医院感染管理专职人员在职称、年龄分布、月收入和办公水平等个人因素以及医院感控环境均优于二级医疗机构有关,三级医疗机构院感队伍建设较完善,资源较集中,提示可通过进修培养、技术帮扶等方式提升二级医疗机构感控水平和胜任力[16]。本研究不同级别医疗机构医院感染管理专职人员职称、学历结构与上海市[12]类似。本研究发现在三级医疗机构中,医院对感控工作重视程度对医院感染管理专职人员岗位胜任力的提高有一定程度的作用,与安徽省546名感控专职人员岗位胜任力调查分析结果一致[17]。尤其是医院领导对感控工作的重视程度[18],可以提升临床医务人员执行院感措施的积极性和主动性。本研究还发现二级医疗机构医院感染管理专职人员月收入对岗位胜任力有极大的影响,说明可通过人才激励政策和薪酬制度,激发出潜在感控水平,进一步提升岗位胜任力[19]
本研究通过胜任力模型分析全省不同级别医疗机构医院感染管理专职人员岗位胜任力现状和影响因素,对进一步开展针对性培养模式有实际指导意义。但该评价量表为自评方式,存在主观偏差,在未来研究中需进一步研制主观、客观相结合的胜任力评价量表。
