Objective

To systematically evaluate the post-market safety of human papilloma virus vaccine in Jiangxi province.

Methods

The doses number of HPV administered from 2017 to 2022 wascollected from all the vaccination institutions, and theadverse events followingimmunization(AEFI)cases were collected through the AEFI Surveillance Module of Chinese Disease Control and Prevention Information System. Using descriptive statistics, the incidences and epidemiological characteristics were estimated.

Results

81 AEFI cases of HPV were reported from 2017 to 2022,and the incidence was 3.38 per 100 000 doses. Moreover, the incidence of HPV2, HPV4 and HPV9 were 3.21, 3.38 and 3.84 per 100 000 doses, respectively. A totalof 71.60% cases were 20-39 years old and 75.31%occurred in 2 days after inoculation. 59.26% were common adverse reactions. Most rare vaccine reactions were allergic rashes, and the reported incidence was 0.50 per 100 000 doses.

Conclusion

The human papilloma virus vaccine has acceptable safety profile in Jiangxi. The sensitivity of monitoring system needs to be further improved.

目的

评价江西省人乳头状瘤病毒(HPV)疫苗接种后的安全性。

方法

收集各接种单位2017—2022年HPV疫苗的接种剂次数,通过中国疾病预防控制信息系统疑似预防接种异常反应(AEFI)监测模块收集AEFI个案数据,采用描述性方法统计HPV疫苗AEFI的发生情况。

结果

江西省2017—2022年共接种HPV疫苗2 394 829剂次,报告81例AEFI,报告发生率为3.38/10万,其中二价(HPV2)、四价(HPV4)、九价(HPV9)分别为3.21/10万、3.38/10万和3.84/10万。AEFI报告例数以20~39岁年龄组为主,共占71.60%。75.31%AEFI发生在接种后2天内。59.26%为一般反应,异常反应主要为过敏性皮疹,报告发生率为0.50/10万。

结论

江西省HPV疫苗接种安全性较好,需进一步提高监测敏感性。

目的

评价江西省人乳头状瘤病毒(HPV)疫苗接种后的安全性。

方法

收集各接种单位2017—2022年HPV疫苗的接种剂次数,通过中国疾病预防控制信息系统疑似预防接种异常反应(AEFI)监测模块收集AEFI个案数据,采用描述性方法统计HPV疫苗AEFI的发生情况。

结果

江西省2017—2022年共接种HPV疫苗2 394 829剂次,报告81例AEFI,报告发生率为3.38/10万,其中二价(HPV2)、四价(HPV4)、九价(HPV9)分别为3.21/10万、3.38/10万和3.84/10万。AEFI报告例数以20~39岁年龄组为主,共占71.60%。75.31%AEFI发生在接种后2天内。59.26%为一般反应,异常反应主要为过敏性皮疹,报告发生率为0.50/10万。

结论

江西省HPV疫苗接种安全性较好,需进一步提高监测敏感性。

Objective

To systematically evaluate the post-market safety of human papilloma virus vaccine in Jiangxi province.

Methods

The doses number of HPV administered from 2017 to 2022 wascollected from all the vaccination institutions, and theadverse events followingimmunization(AEFI)cases were collected through the AEFI Surveillance Module of Chinese Disease Control and Prevention Information System. Using descriptive statistics, the incidences and epidemiological characteristics were estimated.

Results

81 AEFI cases of HPV were reported from 2017 to 2022,and the incidence was 3.38 per 100 000 doses. Moreover, the incidence of HPV2, HPV4 and HPV9 were 3.21, 3.38 and 3.84 per 100 000 doses, respectively. A totalof 71.60% cases were 20-39 years old and 75.31%occurred in 2 days after inoculation. 59.26% were common adverse reactions. Most rare vaccine reactions were allergic rashes, and the reported incidence was 0.50 per 100 000 doses.

Conclusion

The human papilloma virus vaccine has acceptable safety profile in Jiangxi. The sensitivity of monitoring system needs to be further improved.

宫颈癌是全世界女性中第四常见的肿瘤[1],也是导致女性死亡的主要疾病之一[2]。随着经济社会发展和工业化、城镇化进程加快,居民生活环境与生活方式快速变化,宫颈癌发病率持续增高并呈现年轻化趋势。研究表明,宫颈癌的主要致病原因是高危型人乳头瘤病毒(HPV)持续感染,而接种HPV疫苗是预防宫颈癌的有效预防手段,很多国家已经将其纳入国家免疫规划[2]。截至目前,在中国上市的HPV疫苗包括进口二价、四价和九价,以及国产二价。
江西省开展进口二价及四价HPV疫苗、进口九价HPV疫苗和国产二价HPV疫苗接种工作分别是2017年、2019年和2020年,均作为非免疫规划疫苗按照“知情、自愿、自费”的原则进行接种。为加速宫颈癌消除进度,2023年江西省政府决定开展全省12~14岁适龄女生免费接种二价HPV疫苗的工作。为提供既往HPV疫苗接种安全性信息,本研究统计分析江西省2017—2022年接种HPV疫苗的疑似预防接种异常反应(AEFI)数据,系统评价各类HPV疫苗的安全性,为政策制定提供参考。
全省接种单位逐级汇总上报各类HPV疫苗的接种数据;AEFI监测个案数据来自中国疾病预防控制信息系统AEFI监测模块。
根据《中华人民共和国疫苗管理法》、《全国疑似预防接种异常反应监测方案(2022年版)》等规定,江西省辖区内接种单位、医疗机构等AEFI责任报告单位发现AEFI后,可通过中国疾病预防控制信息系统AEFI监测模块进行网络直报。
预防接种后发生的,任何一方(受种者、接种单位、生产企业)怀疑与预防接种有关的反应或事件均可作为AEFI进行报告。除单纯发热、接种部位红肿/硬结等明确的一般反应外,其余AEFI均需进行调查、诊断,按AEFI发生原因分类为:不良反应、疫苗质量事故/疫苗质量问题相关反应、接种事故/接种差错相关反应、心因性反应和偶合症。不良反应是由疫苗本身所固有的特性引起的,与预防接种目的无关或意外的反应,按照严重程度可分为一般反应和异常反应。一般反应是对机体造成一过性生理功能障碍的反应,如单纯发热、乏力、接种部位局部红肿/硬结等;异常反应是合格的疫苗在实施规范接种过程中或接种后出现组织器官、功能损害,相关各方均无过错的药品不良反应。
采用描述性流行病学方法统计HPV疫苗的AEFI总报告发生率、AEFI分类构成比及报告发生率、临床诊断分布及报告发生率,以及其它流行病学特征。AEFI报告发生率的计算公式:(AEFI报告例数/疫苗接种剂次数)×10万。
2017—2022年期间,江西省共接种2 394 829剂次HPV疫苗,报告AEFI共81例,报告发生率为3.38/10万。2017年、2018年AEFI报告发生率最高(8.58/10万、8.15/10万),2019年、2020年较高(5.50/10万、6.19/10万),2021年、2022年最低(1.95/10万、2.55/10万)。
其中,二价、四价、九价HPV疫苗分别接种1 308 954、591 083、494 792剂次,AEFI分别报告42、20、19例,报告发生率分别为3.21/10万、3.38/10万和3.84/10万。
81例AEFI中,以20~39岁年龄组为主(58例),共占71.60%;其次是10~19岁、40~49岁年龄组。
81例AEFI中,59.26%为一般反应(48例),报告发生率2.00/10万;22.22%为异常反应(18例),报告发生率0.75/10万;偶合症占13.58%,报告发生率0.46/10万;心因性反应占4.94%,报告发生率0.17/10万(表3)。
48例一般反应病例中,52.08%伴发热症状(25例),47.92%伴接种部位红肿症状(23例),33.33%伴接种部位硬结症状。此外,接种后伴乏力、头晕、肌痛分别占29.16%、27.08%和14.58%(表4)。
18例异常反应病例中,以过敏性皮疹为主(12例),报告发生率为0.50/10万;包括1例过敏性休克,报告发生率为0.04/10万(表5)。
81例AEFI病例均已治愈或好转,无死亡和残疾病例报告。
AEFI主要发生在接种后2天内,共61例,占75.31%(表6)。
HPV疫苗在中国上市使用以来受到了社会公众的高度关注,尤其是九价HPV疫苗[3]。2017年进口HPV疫苗上市,江西省开始开展接种工作。2019年进口九价HPV疫苗上市,接种剂次数显著增加。2020年首个国产二价HPV疫苗上市接种,江西省HPV疫苗接种剂次数继续大幅增加,2022年突破100万剂。接种意愿的调查研究结果表明 [4-6],适龄女性HPV疫苗接种意愿较高。HPV接种剂次数持续增加,持续、严格地开展各类HPV疫苗上市后安全性监测评价显得比较重要。
本研究监测结果表明,2017—2022年随着HPV疫苗接种剂次数的增加,AEFI报告发生率总体呈现逐年下降的趋势,说明评价上市后大规模接种的安全性评价需要较大的样本含量,结果才更为可靠。江西省HPV疫苗AEFI总报告发生率为3.38/10万,远低于上海市(256.35/10 万)[7],低于温州市(30.72/10万)[8]、杭州市(25.15/10万)[9]、福建省(10.11/10万)[10]、广东省(11.07/10万)[11]监测结果,分析原因可能与AEFI监测方式(上海市采用被动监测与主动监测结合)、各省接种剂次数、AEFI监测系统敏感性及地域差异有关。HPV疫苗作为一种非免疫规划疫苗,接种人群为青少年及成人,江西省AEFI报告发生率低于全国HPV疫苗的AEFI监测分析结果[3,12-13],HPV疫苗AEFI报告敏感性需进一步提高。
本研究显示二价IPV、四价HPV与九价HPV的AEFI报告发生率差异不明显,与天津市[14]、广州市[15]的研究结果一致,但与上海市[7]研究结果不一致,且上海市的AEFI报告发生率最高。上海市2017—2019年共接种63.43万剂,其中二价HPV、四价HPV和九价HPV分别为4.73万剂(7.5%)、37.00万剂(58.3%)和21.70万剂(34.2%),三类HPV疫苗的AEFI报告发生率分别为452.38/10万、231.63/10万和255.75/10万。分析原因,可能是接种剂次数的大小影响报告发生率计算结果的代表性。2020年国产HPV疫苗上市前,HPV疫苗供应全部是靠进口,大多情况下供应比较紧张,尤其是九价HPV,通常都是一针难求[12],接种剂次数作为分母,其对AEFI报告发生率的计算影响较大。
HPV疫苗的AEFI以一般反应为主,与国内外研究结果基本一致[7-11,16],异常反应主要为过敏性皮疹[7,9,11]。日本12~16岁女性二价和四价HPV疫苗接种后出现的复杂区域疼痛综合征不良反应并因此暂停HPV接种的情况[17],在江西省未监测到。
美国二价及四价HPV疫苗晕厥病例的报告发生率为8.2 /10万剂[2],本研究结果显示心因性反应的报告发生率为0.17/10万。可能是因为美国已经将HPV疫苗纳入国家免疫规划[2],为9~26岁女性免费接种,而心因性反应的高发群体为青少年;但江西省2017—2022年HPV疫苗作为非免疫规划疫苗,20~39岁中青年女性为HPV疫苗主要受种群体,身体和心智发育基本成熟,因此发生心因性反应的概率相对较低。提示今后开展12~14岁适龄女生免费接种二价HPV疫苗项目需高度关注心因性反应的发生和应急处置。
综上,HPV疫苗上市后接种6年多以来,江西省接种不良反应报告发生率较低,严重不良反应罕见。2023年江西省开展12~14岁适龄女生免费接种二价HPV疫苗后,需进一步加强对HPV疫苗安全性的监测评价。
