Objective

To apply the case-based problem-based learning (PBL) teaching method in clinical and disease prevention education of obstetric and gynecological nursing and evaluate its effectiveness.

Methods

A total of 50 obstetric and gynecological nursing students were randomly divided into a control group and an experimental group. The control group received traditional teaching methods, while the experimental group received case-based PBL teaching method. After completing all teaching sessions, a comprehensive evaluation of student learning was conducted through performance assessment and survey questionnaires. The collected data were analyzed and visualized using SPSS statistical software.

Results

Students taught using a case-based problem-based learning (PBL) approach demonstrated significantly higher scores in seven assessment indicators compared to students taught using traditional teaching methods (P<0.05). Among these indicators, the differences were most pronounced in clinical thinking ability (t=5.226, P<0.001) and self-directed learning ability (t=3.878, P<0.001). In terms of theoretical and practical scores, the experimental group had an average score of 88.68, which was higher than the control group’s average score of 85.92 (t=3.056, P=0.004).

Conclusion

The application of the case-based PBL teaching method in clinical and disease prevention education of obstetric and gynecological nursing effectively enhances students’ comprehensive abilities, particularly in improving their clinical thinking and self-directed learning abilities.

目的

将病例式以问题为基础的教学模式(Problem-Based Learning,PBL)应用于妇产科护理学临床与疾病预防教学工作并评价其应用效果。

方法

将同批次50名妇产科护理生作为研究对象,并随机分为对照组与试验组,对照组接受传统教学方法,试验组接受病例式PBL教学方法。全部教学环节结束后,通过成果考核以及调查问卷的形式对学生学习情况进行多指标综合评价。运用SPSS统计软件整理并分析考核及调查结果,并将分析结果可视化。

结果

目的

将病例式以问题为基础的教学模式(Problem-Based Learning,PBL)应用于妇产科护理学临床与疾病预防教学工作并评价其应用效果。

方法

将同批次50名妇产科护理生作为研究对象,并随机分为对照组与试验组,对照组接受传统教学方法,试验组接受病例式PBL教学方法。全部教学环节结束后,通过成果考核以及调查问卷的形式对学生学习情况进行多指标综合评价。运用SPSS统计软件整理并分析考核及调查结果,并将分析结果可视化。

结果

采用病例式PBL教学法教授的学生的七项评估指标成绩均高于用传统教学方法教授的学生(P<0.05),其中以临床思维能力(t=5.226, P<0.001)与自主学习能力(t =3.878, P<0.001)差异最为明显。理论实践分数的比较结果来看,试验组的平均得分为88.68,高于对照组的平均得分85.92(t=3.056, P=0.004)。

结论

将病例式PBL教学方法应用于妇产科护理学临床与疾病预防教学可有效提升学生的综合能力,尤其对于学生的临床思维与自主学习能力的提高最为明显。

Objective

To apply the case-based problem-based learning (PBL) teaching method in clinical and disease prevention education of obstetric and gynecological nursing and evaluate its effectiveness.

Methods

A total of 50 obstetric and gynecological nursing students were randomly divided into a control group and an experimental group. The control group received traditional teaching methods, while the experimental group received case-based PBL teaching method. After completing all teaching sessions, a comprehensive evaluation of student learning was conducted through performance assessment and survey questionnaires. The collected data were analyzed and visualized using SPSS statistical software.

Results

Students taught using a case-based problem-based learning (PBL) approach demonstrated significantly higher scores in seven assessment indicators compared to students taught using traditional teaching methods (P<0.05). Among these indicators, the differences were most pronounced in clinical thinking ability (t=5.226, P<0.001) and self-directed learning ability (t=3.878, P<0.001). In terms of theoretical and practical scores, the experimental group had an average score of 88.68, which was higher than the control group’s average score of 85.92 (t=3.056, P=0.004).

Conclusion

The application of the case-based PBL teaching method in clinical and disease prevention education of obstetric and gynecological nursing effectively enhances students’ comprehensive abilities, particularly in improving their clinical thinking and self-directed learning abilities.

近年来,随着医疗技术的发展以及社会医疗需求的不断增加,如何进一步加强医学生的培养、改善医疗服务质量以及减少各类疾病的发生已成为社会各界关注的重要问题。医学教育作为医疗服务质量的重要保障,其发展的质量与水平需要不断提高。然而,目前国内的医学教育还存在部分问题亟待改进,包括教学形式单一、教材陈旧、教学质量参差不齐等[1-2]
由于妇产科诊疗护理的特殊性,在妇产科护士的培养上,往往会对妇产科护士提出更高的要求,既要求掌握过硬的临床技能和丰富的专业知识储备,也需要具备疾病防控的意识和能力,以减少疾病的发生。在传统的医护培育模式下,妇产科护士的培养过程往往忽视了对于疾病预防思维与能力的培养,也存在理论和实践脱节的问题,导致他们在面对实际工作时面临各种问题和挑战[5]
为了解决这些问题,以问题为基础的教学模式(Problem-Based Learning,PBL)逐渐被引入到医学生培养环节中。传统的PBL教学模式是以问题为导向,通过学生自主发掘问题、深入合作学习、积极提出解决方案等方式,可以激发学生的学习兴趣和主动性,增强其自主学习能力以及团队协作能力,从而显著提高培养学生的实践能力和专业素养[5]
而在病例式的PBL教学中,医学生可通过接触或模拟真实临床场景,探究和解决各种临床问题,有效锻炼应变能力,增强临床意识和判断能力[6]。接触真实或模拟的临床案例能使学生更加深入地了解相关疾病的病因、发病机制以及危险因素。而在模拟课堂或实际场景中,学习与来访人员的沟通方法,了解其生理与心理状态,教授其正确的健康知识,预防疾病的发生。在团队合作的学习环境中,学生可以共同探讨和分析实际案例,提出有效的预防与治疗策略,并从中获得宝贵的实践经验。最终通过培养学生的三级预防观念与综合实践能力,为未来的临床实践与疾病预防工作提供坚实的基础[7]
本研究旨在比较妇产科护理生分别接受不同教学方式后的学习成果,探究病例式PBL教学法在妇产科护理学临床与疾病预防教学中应用效果。
本研究中所纳入的研究对象均为同校同年级同专业的妇产科护理生,均接受相同的高校教育指导。全部研究过程未对参与者进行生物样本采集或检查等医疗或生物学研究行为,研究对象入组前,接受均被告知研究目的与试验方法,所有对象均可自由选择是否参与。然后通过简单的问卷调查筛选,已排除提前接受过额外临床指导以及有过科研经历或参与过2次以上社会实践活动,以及到研究开始时绩点低于2.0或高于4.0的学生。通过Power and Samplesize在线计算工具运用独立样本t检验公式对样本量进行推算,根据前期调研确定关键参数水平,检验水准(α)=0.05,功效(1-β)=0.8,效应值(Mean Difference)=1,标准差(Standard Deviation)=1,计算显示单组所需最小样本量(n)=16。结合考虑实际情况,包括班级人数设置、教学负荷等因素,最终确定两组(对照与实验)总样本量为50。最后纳入50名学生作为研究对象,通过计算机生成随机数的方法将50人随机分为对照组与实验组,每组25人。学习期间,对照组接受传统教学方法,试验组接受病例式PBL教学方法。
全部实验教学阶段持续一个月(四周),每周一至周五为固定教学时间。在教学阶段完成后,通过问卷调查与理论考试形式对学生的学习成果进行考核。
对照组采用传统教学方法,每日教学安排包括查房带教、理论与实践操作学习。包括:在专业知识教学环节中由教师讲授妇产科护理学的基础知识、常见疾病的护理方法、临床护理技能等方面的知识并辅以适当的病例分析和技能演示;在临床教学中在模拟实训室中模拟真实的临床环境,让学生进行各种操作和模拟诊疗,在院区里根据医生的诊断结果和护理计划,根据老师要求对妇产科患者进行护理操作,以提高学生的操作技能和诊疗能力等[3,8]
试验组采用病例式PBL教学法。主要包括五个环节的学习:病例分析、自主学习、小组讨论、实践学习、护理操作、总结和评价。教学安排如下:
病例分析:每周选取两个典型的妇产科病例,让学生先自行分析病例,讨论可能存在的问题,提出初步的解决方法和护理方法等,并形成小组(5人/组)在每周三与周五晚进行讨论。同时指导学生学习整合临床资料,例如病历、实验室结果、影像学报告等,并学习撰写病例报告,提出疾病控制与预防的可行方案同样在讨论会进行研讨。自主学习:先由教师介入小组讨论,帮助学生理解病例中的关键问题和难点,并引导学生思考问题的多个方面,如病因、病症、诊断、治疗、护理等。然后学生自主学习相关知识,包括文献查找、病例分析、护理方案、疾病预防策略等。小组讨论:再次以小组形式讨论病例,讨论对病人的护理计划,分析病例中的护理问题,并提出可行的解决方法。实践学习环节:通过真实的病例场景来教授学生如何与患者进行有效的沟通。在每日查房带教环节改变以往由老师负责全程讲解的教学方式,改为由学生对病人的相关信息提前学习后,由学生主导问询和交流,带教老师指导补充的带教形式。在孕产期疾病预防方面,学生可以学习如何与孕妇建立信任关系,并向她们传达关于健康生活方式、营养咨询和孕期疾病预防的重要信息。护理操作:学生在教师的指导下进行护理操作,如静脉穿刺、输液等,同加强对妇产科疾病的理解。总结评价:带教老师每周进行总结和评价并针对性地调整教学方法,并在教学阶段结束后评估学生对病例的分析和解决能力,考核学生制定疾病预防策略的能力,同时让学生对整个课程进行评价和反思[9]
全部教学环节结束后,对两组学生进行成果考核,考核方式为教学环节中所教授的所有知识与技能。同时通过调查问卷的形式对学生学习情况进行多指标综合评价。评价指标包括:临床思维能力、文献分析能力、科研思维能力、沟通表达能力、自主学习能力、团队合作能力以及理论实践分数。除理论实践分数外,其余六项指标均采用十分制,0分为最差,10分为最好。理论实践分数采用百分制,0分为最差,100分为最好。
通过查阅文献,参考国内外相关研究的量表工具以及教学评估问卷来设计本次研究使用的调查问卷[10-11]。在这一前提下结合了妇产科护理教学实际编制问卷题目。问卷涵盖了妇产科临床护理学生在不同方面的能力评估,包括临床思维能力、文献分析能力、科研思维能力、沟通表达能力、自主学习能力和团队合作能力这六个主要评价指标。这些指标综合考虑了妇产科护理学生在实践、学术研究和团队合作等方面的能力要求,以全面评价学生的综合素质。每个评价指标下设5个具体的问题,通过对学生在不同情境下的表现进行评估。题目统一采用正向提问方式,问题选项包括符合、一般符合、一般、一般不符合和不符合,赋予分值为2到0分,提供了多个层次的评价选择,以准确地了解学生的能力水平[12]。问卷设计遵循不记名、简洁、有效原则,填写时长约为10分钟,填写前已向学生充分介绍问卷用途和目的,并获得学生支持。问卷收集后,采取双人录入、交换核对方式进行统计。发放问卷50份,回收50份,有效率100%。
本研究采用 SPSS 26.0 软件完成统计学分析,各组性别资料以n(%)表示,通过χ2检验方法分析不同组别之间性别的差异显著性,年龄、问卷得分以及理论实践分数等连续性资料以表示,使用t 检验分析两组间的差异显著性。检验水准α=0.05。
表1所示,是对照组和实验组参与学生基线资料的比较,表中按照性别和年龄两个方面进行了比较。在性别方面,对照组有2名为男性(占总人数的8%),24名为女性(占总人数的92%)。而试验组有1名为男性(占总人数的4%),24名为女性(占总人数的96%)。χ2检验结果显示两组性别差异无统计学意义,P值为1.000。
在年龄方面,对照组的参与学生平均年龄为22.88岁,标准差为0.43。而试验组的参与学生平均年龄为22.96岁,标准差为0.66。通过t检验,计算得到的t值为0.496,P值为0.622,表明两组年龄差异无统计学意义。两组学员在性别与年龄层面无显著差异(P > 0.05)。
通过对两组学生的成果考核结果进行分析整理后发现,如表2所示,采用病例式PBL教学法教授的学生的七项评估指标成绩均显著高于用传统教学方法教授的学生,且差异具有统计学意义(P < 0.05)。其中临床思维能力、科研思维能力、自主学习能力以及理论实践分数四项指标的P值均小于0.01,特别是在临床思维能力(t=5.226, P < 0.001)与自主学习能力(t=3.878, P < 0.001)方面差异有统计学意义,且较为明显。在教学过程中,学生学习了如何收集和整合病人临床资料,通过分析这些临床数据,学生更好地理解病情、诊断疾病,评估不同治疗方案的有效性,并且形成病例报告,为开展疾病预防的控制、减少患者不良预后提供了帮助。
其次,从理论实践分数的比较结果来看,试验组的平均得分为88.68,显著高于对照组的平均得分85.92(t=3.056, P=0.004)。这表明,新的教学方法有助于学生实践能力的提升。同时,在实践学习环节中,学生重点学习了产科保健的相关知识,通过与孕产妇及家属进行实际交流,学习与孕产妇建立信任关系,缓解其紧张情绪,传授正确的孕产期保健知识,以达到减少孕产期疾病发生的目标。我们还发现新的教学方法同样有助于学生科研思维的培养,试验组学生的科研思维能力方面的平均得分为7.36,比对照组均值高出1.04分,且差异有统计学意义(t=2.938, P=0.005)。
然而,需要注意的是,与其他指标相比,试验组学生在沟通表达能力方面的提升较为有限(t=2.271,P=0.028),部分学生仍存在表达障碍,现有的教学方法还需要进一步改善对学生加以引导。这些结果表明,有效的干预措施可以显著提高学生的学习成果,从而促进他们在不同领域的综合能力表现,但仍需要重视和改进对于学生沟通表达能力的培养。
妇产科护理学是护理学的重要组成部分,其教学的主要任务是为妇科患者与孕产妇提供全面的护理服务。传统的妇产科护理学教学方法通常是教师以书本为依据简单地传授知识和技能为主,对学生实践的指导和培训较为有限,同时还忽视了对于学生疾病预防思维的培养,导致学生很难将所学的知识和技能应用到实际工作中[13]。为了解决这一问题,PBL教学法被逐渐引入到妇产科护理学的教学工作中。PBL教学法是一种基于实际问题的教学方法,它鼓励学生通过自主探究、相互讨论以解决实际问题来获取知识和技能[14]。因此,通过在妇产科护理学的教学工作中应用PBL教学法,可以帮助学生更好地理解和应用所学知识和技能,提高其工作能力。
而病例式PBL教学是PBL教学法在医学领域应用后诞生的特殊形式,旨在通过基于真实病例的诊疗来引导学生自主讨论学习提出解决方案。具体来说,是结合真实病例以及具有挑战性的模拟临床病例,如分娩并发症、产后出血、新生儿窒息等,根据患者的病史、症状、检查结果、诊断和治疗等信息,引导学生进行探究和解决。通过实际病例的分析和解决,学生可以更深入地了解相关知识和技能,从而提高他们的实践和解决问题的能力。例如,当学生面对一个分娩并发症的病例时,他们需要了解分娩并发症的病因、症状和治疗方法,从而能够更好地应对这种情况[15]
病例式PBL教学还可以促进学生之间的合作和交流。在小组讨论和合作的过程中,学生需要相互协作,共同解决问题,从而提高他们的团队合作能力和沟通能力,有利于培养学生的协作精神和团队意识。例如,当学生面对一个新生儿窒息的病例时,一个人的能力往往不足以支持其解决,他们需要相互协作配合,共同制定抢救方案,做好工作分配,从而更好地守护患者的生命。此外,病例式PBL教学可以增强学生的批判性思维和自我评价能力。在制定护理方案与疾病预防策略的过程中,学生需要不断地分析和评估自己的解决方案,从而培养自我批判的思维,也有助于培养职业素养和责任心[16]
本研究的结果与上述结论基本一致,从我们的研究结果可以看到在经过系统的病例式PBL教学后,学生的各项能力均得到显著提升,其中临床思维能力与自主学习能力的增强将为学生面对实际病例的考验提供巨大的帮助。除了临床能力的提升外,文献分析能力与科研思维能力的提升也极为重要,医学发展到现阶段,社会对于医学生的要求已不仅仅局限于治病救人的技术培养,更在于如何产出更多的科研成果推动医学领域的进一步发展。然而,从我们的研究中可以观察到,与其他的六项指标相比,学生的沟通表达能力提升较为有限,这可能是由教师指导的仍然较多或缺乏与病人的直接沟通的原因所导致。在未来的教学改革中,可以考虑采取更多的引导学生与病人自主交流,指导老师负责查漏补缺的方法以改变沟通交流能力提升不足的现状。
病例式PBL教学法在妇产科护理学教学中的应用对于妇产科相关疾病预防工作的开展也具有重要的作用和意义。学生通过接受该教学方法,掌握将预防措施融入护理实践的方法和技能,通过亲身体验一级预防阶段的宣传教育,二级预防阶段的早期筛查和治疗,以及三级预防阶段的并发症控制和康复护理环节,全面了解疾病预防的重要性[7,17]。例如,针对乳腺癌的预防,学生可以学习如何进行自我乳房检查与掌握乳房保健知识,并了解乳腺癌筛查的方法和时机。针对妊娠期糖尿病的预防,学生可以学习如何进行合理的孕前评估和干预,以及在妊娠期如何指导孕产妇通过饮食和运动管理控制血糖水平。这些知识和技能的学习对于开展妇产科相关疾病的预防工作具有重要的指导意义。
综上所述,将病例式PBL教学法应用于妇产科护理学教学具有非常重要的意义和广阔的前景。通过真实病例的引导,可以帮助学生快速有效地了解妇产科护理学的相关知识和技能,提高其实践和解决问题的能力,还能够培养其预防意识和预防实践能力,为妇产科相关疾病的预防工作做出积极的贡献。
