Objective

To study the causal relationship between tea intake and the incidence of colorectal cancer using two-sample Mendelian randomization.

Methods

Using the colorectal cancer genome-wide association analysis data (GWAS ID: ieu-b-4965) and tea intake genome-wide association analysis data (GWAS ID: ukb-b-6066) in the IEU OPEN GWAS public database, we summarized and screened the two data sets for single nucleotide polymorphisms (SNPs) which were closely related to and independent of tea intake as instrumental variables (IVs). By using MR-Egger and inverse variance weighting in two-sample Mendelian randomization, Five methods (IVW), weighted median method (Weighted Median), simple model (Simple Mode) and weighted model (Weighted Mode) were used to determine the causal analysis between tea intake and the incidence of colorectal cancer. OR and 95% CI were used to examine the causal relationship between tea intake and the incidence of colorectal cancer. MR-PRESSO was used for outlier testing. IVW (Cochran Q test) and MR-Egger (Rücker Q test) were used for heterogeneity analysis. Egger-intercept was used for pleiotropy test, sensitivity analysis was performed by leave-one-out method, and funnel plot was used to analyze the presence of potential bias.

Results

39 SNPs were finally included, and MR-PRESSO found no outlier SNPs, P=0.237>0.05; in terms of causality determination: IVW (OR=0.996, 95% CI:0.990-1.002, P=0.293), MR-Egger (OR=0.992, 95% CI:0.977-1.007, P=0.293), Weighted Median (OR=0.997, 95% CI: 0.987-1.007, P=0.479), Simple Mode (OR=0.996, 95% CI: 0.976-1.017, P=0.724), Weighted Mode (OR=0.996, 95% CI: 0.985-1.007, P=0.473). The above results showed that there was no causal relationship between tea intake and the incidence of colorectal cancer. In the heterogeneity test, P=0.181>0.05 in Cochran Q test and P=0.167>0.05 in Rucker Q test. There was no significant heterogeneity. The Egger-intercept test showed that P=0.489>0.05 and was not pleiotropic. The leave-one-out method was removed one by one, and no SNPs significantly affecting the results were found. The funnel plot was basically symmetrical, indicating that the main results were reliable.

Conclusion

There is no causal relationship between tea intake and the incidence of colorectal cancer.

目的

利用两样本孟德尔随机化研究茶摄入量与结直肠癌发病的因果关系。

方法

利用IEU OPEN GWAS公共数据库中的结直肠癌全基因组关联分析数据(GWAS ID:ieu-b-496 5)和茶摄入量全基因组关联分析数据(GWAS ID:ukb-b-606 6),汇总两个数据集,筛选与茶摄入量密切相关且独立的单核苷酸多态性(SNPs)作为工具变量(IVs),通过采用两样本孟德尔随机化中的MR-Egger、逆方差加权(IVW)、加权中位数法(Weighted Median)、简单模型(Simple Mode)和加权模型(Weighted Mode)五种方法进行茶摄入量与结直肠癌发病的因果分析的判断,采用OR以及95% CI判断茶摄入量与结直肠癌发病是否存在因果关系,利用MR-PRESSO进行离群值检验,采用IVW(Cochran Q检验)以及MR-Egger(Rücker Q检验)进行异质性分析,采用Egger-intercept进行多效性检验,通过留一法进行敏感性分析,采用漏斗图分析是否存在潜在偏倚。

结果

最终纳入39个SNPs,MR-PRESSO未发现离群SNPs,P=0.237>0.05;在因果关系的判定上:IVW(OR=0.996,95% CI:0.990~1.002,P=0.293),MR-Egger(OR=0.992,95% CI:0.977~1.007,P=0.293),Weighted Median(OR=0.997,95% CI:0.987~1.007,P=0.479),Simple Mode(OR=0.996,95% CI:0.976~1.017,P=0.724),Weighted Mode(OR=0.996,95% CI:0.985~1.007,P=0.473),上述结果显示:茶摄入量与结直肠癌发病无因果关系,在异质性检测中Cochran Q检验中P=0.181>0.05,Rücker Q检验中P=0.167>0.05,无显著异质性,Egger-intercept进行多效性检验P=0.489>0.05未见多效性,留一法逐个剔除,未见明显影响结果的SNPs,漏斗图基本对称,显示主结果可靠。

结论

茶摄入量与结直肠癌发病无因果关系。

目的

利用两样本孟德尔随机化研究茶摄入量与结直肠癌发病的因果关系。

方法

利用IEU OPEN GWAS公共数据库中的结直肠癌全基因组关联分析数据(GWAS ID:ieu-b-496 5)和茶摄入量全基因组关联分析数据(GWAS ID:ukb-b-606 6),汇总两个数据集,筛选与茶摄入量密切相关且独立的单核苷酸多态性(SNPs)作为工具变量(IVs),通过采用两样本孟德尔随机化中的MR-Egger、逆方差加权(IVW)、加权中位数法(Weighted Median)、简单模型(Simple Mode)和加权模型(Weighted Mode)五种方法进行茶摄入量与结直肠癌发病的因果分析的判断,采用OR以及95% CI判断茶摄入量与结直肠癌发病是否存在因果关系,利用MR-PRESSO进行离群值检验,采用IVW(Cochran Q检验)以及MR-Egger(Rücker Q检验)进行异质性分析,采用Egger-intercept进行多效性检验,通过留一法进行敏感性分析,采用漏斗图分析是否存在潜在偏倚。

结果

最终纳入39个SNPs,MR-PRESSO未发现离群SNPs,P=0.237>0.05;在因果关系的判定上:IVW(OR=0.996,95% CI:0.990~1.002,P=0.293),MR-Egger(OR=0.992,95% CI:0.977~1.007,P=0.293),Weighted Median(OR=0.997,95% CI:0.987~1.007,P=0.479),Simple Mode(OR=0.996,95% CI:0.976~1.017,P=0.724),Weighted Mode(OR=0.996,95% CI:0.985~1.007,P=0.473),上述结果显示:茶摄入量与结直肠癌发病无因果关系,在异质性检测中Cochran Q检验中P=0.181>0.05,Rücker Q检验中P=0.167>0.05,无显著异质性,Egger-intercept进行多效性检验P=0.489>0.05未见多效性,留一法逐个剔除,未见明显影响结果的SNPs,漏斗图基本对称,显示主结果可靠。

结论

茶摄入量与结直肠癌发病无因果关系。

Objective

To study the causal relationship between tea intake and the incidence of colorectal cancer using two-sample Mendelian randomization.

Methods

Using the colorectal cancer genome-wide association analysis data (GWAS ID: ieu-b-4965) and tea intake genome-wide association analysis data (GWAS ID: ukb-b-6066) in the IEU OPEN GWAS public database, we summarized and screened the two data sets for single nucleotide polymorphisms (SNPs) which were closely related to and independent of tea intake as instrumental variables (IVs). By using MR-Egger and inverse variance weighting in two-sample Mendelian randomization, Five methods (IVW), weighted median method (Weighted Median), simple model (Simple Mode) and weighted model (Weighted Mode) were used to determine the causal analysis between tea intake and the incidence of colorectal cancer. OR and 95% CI were used to examine the causal relationship between tea intake and the incidence of colorectal cancer. MR-PRESSO was used for outlier testing. IVW (Cochran Q test) and MR-Egger (Rücker Q test) were used for heterogeneity analysis. Egger-intercept was used for pleiotropy test, sensitivity analysis was performed by leave-one-out method, and funnel plot was used to analyze the presence of potential bias.

Results

39 SNPs were finally included, and MR-PRESSO found no outlier SNPs, P=0.237>0.05; in terms of causality determination: IVW (OR=0.996, 95% CI:0.990-1.002, P=0.293), MR-Egger (OR=0.992, 95% CI:0.977-1.007, P=0.293), Weighted Median (OR=0.997, 95% CI: 0.987-1.007, P=0.479), Simple Mode (OR=0.996, 95% CI: 0.976-1.017, P=0.724), Weighted Mode (OR=0.996, 95% CI: 0.985-1.007, P=0.473). The above results showed that there was no causal relationship between tea intake and the incidence of colorectal cancer. In the heterogeneity test, P=0.181>0.05 in Cochran Q test and P=0.167>0.05 in Rucker Q test. There was no significant heterogeneity. The Egger-intercept test showed that P=0.489>0.05 and was not pleiotropic. The leave-one-out method was removed one by one, and no SNPs significantly affecting the results were found. The funnel plot was basically symmetrical, indicating that the main results were reliable.

Conclusion

There is no causal relationship between tea intake and the incidence of colorectal cancer.

结直肠癌(Colorectal Cancer,CRC)是消化系统最常见的恶性肿瘤之一[1]。最近有报告称,大肠癌是全球第三大常见癌症,但却是第二大常见死因[2]。尽管通过改进外科手术、肿瘤治疗、计划和随访,CRC患者的生存率有了明显提高,但近年来其全球发病率仍在上升[3-4]。茶是全世界都喜爱的饮品,被认为是某些疾病的化学预防剂[5]。部分研究探讨了饮茶与 CRC之间的关系,有证据表明茶叶中可能含有一些预防CRC的成分[6]。研究发现,茶多酚是茶叶中含量最高的成分之一,可通过调节 Ras-MAPK等信号通路,促进肿瘤细胞凋亡、抑制肿瘤细胞增殖和血管生成,从而抑制肿瘤的发展[7];一些研究结果还表明,饮茶与降低CRC风险无关[8]。过去几十年来,许多研究评估了饮茶与CRC发病率和死亡率之间的关系,但结论并不一致,也未达成共识。同时关于饮茶量是否会影响结直肠癌的发病,部分研究显示较大的饮茶量可能会降低结直肠癌的发生率[9],我国学者刘春雨等人的研究显示饮茶量与结直肠癌的发病无关[10],还有部分研究显示较大的饮茶量可能会增加结直肠癌的发生情况[11],关于饮茶量与结直肠癌发生的相关性目前也未达成共识。近几年来,孟德尔随机化(Mendelian Randomization,MR)被广泛应用于临床疾病因果关系检测的流行病研究中,MR研究的基本原理为在遗传当中亲代的等位基因随机分配到子代中,相当于对人群进行了随机分组,同时这种遗传为固定的,人群一出生就伴随,因而将遗传变量做为工具变量(Instrumental Variables,IVs)能够最大限度的减少反向因果以及其他因素对于研究结果的干扰,目前被广泛应用于遗传流行病学的研究中[12-13]。本次研究通过两样本孟德尔随机化研究茶摄入量与结直肠癌发病的因果关系,明确两种之间是否存在因果关系,为将来结直肠癌的饮食方面的预防提供理论依据。
本次研究数据库使用IEU OPEN GWAS公共数据库(https://gwas.mrcieu.ac.uk/),其中茶摄入量全基因组关联分析数据(GWAS ID:ukb-b-6066),结直肠癌全基因组关联分析数据(GWAS ID:ieu-b-4965),数据具体信息见表1
工具变量的筛选必须符合孟德尔随机化三个核心假设,具体如下(1)单核苷酸多态性(Single Nucleotide Polymorphisms,SNPs)与茶摄入量强相关,(2)SNPs与混杂因素无关,(3)SNPs只能通过茶摄入量影响结直肠癌发病,于其他混杂因素无关,三个核心假设见图1。为满足上述三个假设,在SNPs的筛选上需要满足全基因组统计阈值<5×10-8,为防止连锁不平衡产生,设置连锁不平衡r2<0.001,距离=10 000 kb,同时为了避免弱工具变量产生,设置F>10,F的计算公式为,其中N为茶摄入量的样本数据,K为SNPs的数量,R2为茶摄入量由SNPs解释的变异占比。R2的计算公式为R2=2(1-EAF)×EAF×β2,其中EAF为SNP的效应基因等位频率,β为等位基因效应值,同时使用PhenoScanner(http://www.phenoscanner.medschl.cam.ac.uk/)搜素最终纳入的SNPs看是否违法核心假设的(2)、(3)假设,其中对于(2)、(3)假设的排除主要根据既往相关的临床以及基础研究发表的文献[14-15]来筛查筛选出的SNPs是否与混杂因素和能通过其他途径影响结直肠癌发病,其中混杂因素包括:年龄、性别、BMI以及咖啡代谢,采用利用MR-PRESSO进行离群值检验,剔除离群值的SNPs。
通过采用两样本孟德尔随机化中的MR-Egger、逆方差加权(Inverse Variance,Weighting,IVW)、加权中位数法(Weighted Median)、简单模型(Simple Mode)和加权模型(Weighted Mode)五种方法进行茶摄入量与结直肠癌发病的因果分析的判断,采用OR以及95% CI判断茶摄入量与结直肠癌发病是否存在因果关系。其中以MR-Egger、逆方差加权(IVW)、加权中位数法(Weighted Median)为主要分析方法,简单模型(Simple Mode)和加权模型(Weighted Mode)这两种方法为次要辅助分析方法。
采用IVW(Cochran Q检验)以及MR-Egger(Rücker Q检验)进行异质性分析,若异质性检测P>0.05,则认为不存在异质性,若异质性检测P<0.05,存在异质性,需进行相关分析,确定异质性来源以及是否对最终结果产生影响;采用Egger-intercept进行多效性检验,其中主要观察MR-Egger截距,当该截距接近0时表示不存在多效性,通过留一法进行敏感性分析,采用漏斗图分析是否存在潜在偏倚。
本次研究采用R 4.3.1软件中的TwoSampleMR包以及MR-PRESSO包进行两样本孟德尔随机化分析,双侧检验水准α=0.05。
本次研究经过去除连锁不平衡以及进行F值检验后(纳入F值>10的工具变量)后初步纳入SNPs为40个,同时将这40个SNPs逐个输入PhenoScanner(http://www.phenoscanner.medschl.cam.ac.uk/)搜素初步纳入的SNPs看是否违反核心假设的(2)、(3)假设,排除rs4410790,因其可以通过咖啡的代谢途径对结直肠癌进行作用,最后纳入39个SNPs(具体见表2),同时,对最终纳入的SNPs利用MR-PRESSO进行离群值检验,最终P=0.237>0.05,显示未有离群的SNPs。
在因果关系的判定上:IVW(OR=0.996,95% CI:0.990~1.002),MR-Egger(OR=0.992,95% CI:0.977~1.007),Weighted Median(OR=0.997,95% CI:0.987~1.007),Simple Mode(OR=0.996,95% CI:0.976~1.017),Weighted Mode(OR=0.996,95% CI:0.985~1.007),与之对应的森林图见图2;对应的5种MR分析方法的散点图趋势一致(见图3),上述结果显示:茶摄入量与结直肠癌发病无因果关系。
在异质性检测中Cochran Q检验中P=0.181>0.05,Rücker Q检验中P=0.167>0.05,差异无统计学意义。
Egger-intercept进行多效性检验,其中MR-Egger截距为0.000 137 9接近0,P=0.489>0.05,不支持存在多效性。
留一法逐个剔除,未见明显影响结果的SNPs,具体结果见图4;同时漏斗图(图5)显示代表因果关系的效应点呈对称分布,表示其可能受潜在偏倚的影响可能性较少。
CRC已成为威胁全球个人健康的挑战,对人类健康构成了严重威胁[16]。相应的研究表明,导致CRC的危险因素包括炎症性肠病、一级亲属中有人患有CRC、肥胖、缺乏体育锻炼和吸烟、食用红肉以及水果和蔬菜摄入量少[17]。根据流行病学研究显示饮食因素在CRC癌变中起着关键作用,这已被认为是预防CRC的重要策略[18-19]。此前已有研究发现,茶叶提取物可能具有一定的抗癌作用[20]。然而,关于体内和体外饮茶与CRC之间关系的研究结论仍存在争议 [21]。在饮茶量与结直肠肿瘤发生的相关性分析中,日本的Keiko Wada等人进行了一项绿茶摄入量与结直肠癌相关的前瞻性队列研究-高山研究,高山研究是一项在日本岐阜县进行的基于人群的队列研究,在前8年的随访中,饮用绿茶与通过医院结肠镜检查记录确定的结直肠癌风险之间存在非显著性关联,此后,研究人员从研究地区的癌症登记处积累了结肠直肠癌病例的信息,利用延长的随访期间结直肠癌的最新数据,评估了与饮用绿茶有关的结直肠癌风险,发现饮用绿茶越多,男性结直肠癌发病风险越低[22]。在中国国内学者进行的一项饮茶与癌症风险之间的关系研究显示:一项针对50万中国成年人的前瞻性队列研究中:在既不吸烟也不过量饮酒的日常饮茶者中,饮茶与所有癌症、肺癌、结直肠癌、肝癌、女性乳腺癌和子宫颈癌的风险无关,每天摄入>4.0 克茶叶可能会增加罹患胃癌的风险[23];而在刘春语等人的研究中不支持饮茶与结直肠癌的发病存在因果关系[10]。在茶摄入量与结直肠癌发病的相关性上,目前相应的临床研究均无明确定论。
在本次两样本MR研究中显示:茶摄入量与结直肠癌发病无因果关系,在本次研究中笔者团队采用了5种MR分析方法,5种MR分析方法的散点图趋势一致,5种MR分析法均显示茶摄入量与结直肠癌发病无因果关系。为了验证结果的可靠性,我们进行了异质性检验,在异质性检测中Cochran Q检验中P=0.181>0.05,Rücker Q检验中P=0.167>0.05,差异无统计学意义,同时进行了敏感性分析并且为了更进一步验证本次MR研究是否存在较大潜在的偏倚,还绘制了漏斗图,在敏感性分析中通过留一法逐个剔除,未见明显影响结果的SNPs,同时漏斗图显示代表因果关系的效应点呈对称分布,表示其可能受潜在偏倚的影响可能性较少,上述结果均显示本次研究的MR结果可靠并且稳定,潜在偏倚小。
对于在传统的临床研究中出现饮茶量与结直肠癌发病相关性互相矛盾的研究结果,笔者分析主要原因为:目前虽然病例对照研究是大多数支持茶叶具有防癌作用,但这些研究存在回忆偏差和反向因果关系,因为癌症的诊断前症状可能会改变饮茶习惯[24-25]。对于传统的病例对照研究和队列研究,会受到混杂因素以及反向因果关系的影响,先前报道的观察性研究[20-21]显示饮茶量与结直肠癌发病风险存在相关性可能为观察性研究存在偏倚或者混杂因素的影响所致。
本次研究的创新点在于通过大样本量的GWAS数据库从遗传流行病学的角度对茶摄入量与结直肠癌发病因果关系进行分析,得出了茶摄入量与结直肠癌发病无因果关系的结论。
本研究的局限性在于:本次研究纳入的GWAS数据为欧洲人群,难以外推到其他人群,并且无法进行详细的分层分析。
综上,本研究通过两样本MR方法,对茶摄入量与结直肠癌发病的因果关系进行分析,分析结果表明:茶摄入量与结直肠癌发病无因果关系。
