Objective

To analyze the burden and trends of acute lymphoblastic leukemia (ALL) in China from 1990 to 2021,providing reference for prevention and treatment strategies.

Methods

Data on incidence, prevalence, mortality, and disability-adjusted life years (DALY) for ALL in China were extracted from the Global Burden of Disease Study database (GBD 2021). The Join point regression model was utilized to fit the trends over the years, identify significant inflection points, and calculate the average annual percentage of change (AAPC) from 1990 to 2021 to reflect changes in disease burden. The distribution and trends of ALL across different time periods, genders, and age groups in China were analyzed.

Results

In 2021, the age-standardized incidence rate (ASIR), age-standardized prevalence rate (ASPR), age-standardized death rate (ASDR), and standardized DALY rate for ALL in China were 3.64 per 100 000, 22.02 per 100 000, 1.36 per 100 000, and 74.06 per 100 000, respectively, with a higher burden in males than females. The ASIR and ASPR showed an upward trend, while the ASDR and DALY rates significantly decreased, with the gender disparity in disease burden becoming increasingly pronounced. After 2019, the incidence and prevalence rates in the 2-4 age group declined significantly, with the highest incidence, prevalence, and DALY rates observed in those under 1 year of age by 2021, while the highest mortality rate shifted from infants to the elderly.

Conclusion

The overall burden of ALL in China from 1990 to 2021 shows a downward trend, particularly notable in children, while the burden in adults has increased. There is a need to strengthen preventive measures for ALL among males and children while enhancing effective management of the high mortality rates in the elderly population.

目的

分析1990—2021年中国急性淋巴细胞白血病(简称急淋)疾病负担情况及变化趋势,为中国急淋的预防和治疗策略提供参考依据。

方法

从全球疾病负担研究数据库(GBD 2021)提取中国急淋的发病、患病、死亡和伤残调整寿命年(DALY)等数据,利用joinpoint回归模型拟合年代变化趋势,找到有意义的转折点,并计算1990—2021年平均年度变化百分比(average annual percentage of change,AAPC)反映疾病负担变化趋势,分析中国急淋不同时间、不同性别、不同年龄段的分布和变化趋势。

结果

2021年中国急淋年龄标化发病率(age-standardized incidence rate,ASIR)、年龄标化患病率(age-standardized prevalence rate,ASPR)、年龄标化死亡率(age-standardized death rate,ASDR)和标化DALY率分别为3.64/10万、22.02/10万、1.36/10万、74.06/10万,男性疾病负担高于女性。ASIR和ASPR呈现上升趋势,而ASDR和DALY率显著下降,且性别差异所导致的疾病负担差异越来越明显。2019年之后,2~4岁年龄段的发病数/率和患病数/率明显下降,到2021年发病率、患病率、DALY率最高年龄段是<1岁,死亡率最高的年龄段从婴幼儿向老年转变。

结论

1990—2021年中国急淋负担总体呈下降趋势,以儿童最为显著,而成人负担有加重趋势。中国仍需加强男性和儿童的急淋预治措施,同时提高对老年人群高死亡率的有效管理。

目的

分析1990—2021年中国急性淋巴细胞白血病(简称急淋)疾病负担情况及变化趋势,为中国急淋的预防和治疗策略提供参考依据。

方法

从全球疾病负担研究数据库(GBD 2021)提取中国急淋的发病、患病、死亡和伤残调整寿命年(DALY)等数据,利用joinpoint回归模型拟合年代变化趋势,找到有意义的转折点,并计算1990—2021年平均年度变化百分比(average annual percentage of change,AAPC)反映疾病负担变化趋势,分析中国急淋不同时间、不同性别、不同年龄段的分布和变化趋势。

结果

2021年中国急淋年龄标化发病率(age-standardized incidence rate,ASIR)、年龄标化患病率(age-standardized prevalence rate,ASPR)、年龄标化死亡率(age-standardized death rate,ASDR)和标化DALY率分别为3.64/10万、22.02/10万、1.36/10万、74.06/10万,男性疾病负担高于女性。ASIR和ASPR呈现上升趋势,而ASDR和DALY率显著下降,且性别差异所导致的疾病负担差异越来越明显。2019年之后,2~4岁年龄段的发病数/率和患病数/率明显下降,到2021年发病率、患病率、DALY率最高年龄段是<1岁,死亡率最高的年龄段从婴幼儿向老年转变。

结论

1990—2021年中国急淋负担总体呈下降趋势,以儿童最为显著,而成人负担有加重趋势。中国仍需加强男性和儿童的急淋预治措施,同时提高对老年人群高死亡率的有效管理。

Objective

To analyze the burden and trends of acute lymphoblastic leukemia (ALL) in China from 1990 to 2021,providing reference for prevention and treatment strategies.

Methods

Data on incidence, prevalence, mortality, and disability-adjusted life years (DALY) for ALL in China were extracted from the Global Burden of Disease Study database (GBD 2021). The Join point regression model was utilized to fit the trends over the years, identify significant inflection points, and calculate the average annual percentage of change (AAPC) from 1990 to 2021 to reflect changes in disease burden. The distribution and trends of ALL across different time periods, genders, and age groups in China were analyzed.

Results

In 2021, the age-standardized incidence rate (ASIR), age-standardized prevalence rate (ASPR), age-standardized death rate (ASDR), and standardized DALY rate for ALL in China were 3.64 per 100 000, 22.02 per 100 000, 1.36 per 100 000, and 74.06 per 100 000, respectively, with a higher burden in males than females. The ASIR and ASPR showed an upward trend, while the ASDR and DALY rates significantly decreased, with the gender disparity in disease burden becoming increasingly pronounced. After 2019, the incidence and prevalence rates in the 2-4 age group declined significantly, with the highest incidence, prevalence, and DALY rates observed in those under 1 year of age by 2021, while the highest mortality rate shifted from infants to the elderly.

Conclusion

The overall burden of ALL in China from 1990 to 2021 shows a downward trend, particularly notable in children, while the burden in adults has increased. There is a need to strengthen preventive measures for ALL among males and children while enhancing effective management of the high mortality rates in the elderly population.

在中国居民恶性肿瘤中,白血病发病率居第14位,死亡率居第10位,是主要的肿瘤负担之一[1-2]。虽然随着新的治疗策略和靶向药的发展,白血病患者的总生存率和生命质量得到了改善[3-4],但高昂的治疗费用给患者家庭和社会带来巨大经济负担,也对公共卫生构成挑战。
急性淋巴细胞白血病(简称急淋)是源于淋巴细胞在骨髓内异常增生的白血病,在全球儿童各亚型白血病中发病率和患病率位居第二[5]。在中国,急淋作为最常见的儿童恶性血液病,约占儿童急性白血病的65%~70%[6],是导致儿童和青少年疾病死亡的主要原因[7]。最新研究显示,全球急淋发病率按年龄划分呈现明显的双峰,第一个高峰发生在0~4岁,第二个高峰发生在75岁左右。成人白血病亚型分布多样化,在亚洲急淋的比例相对较高[8],占成人急性白血病的20%~30%[9]。且目前儿童急淋的治愈率已高达90%以上,而成人急淋由于更高危的疾病特征以及传统化疗耐受性更差,总体预后较差,治愈率仅约40%[10],成为是成人癌症相关死亡的重要原因[7]
为更全面的评估中国急淋的负担现状,本研究提取了全球疾病负担研究数据库(Global Burden of Disease, GBD)的最新数据,对1990—2021年中国急淋发病、患病、死亡、伤残调整寿命年(DALY)的现状和变化趋势进行了分析,并细化年龄分段的流行趋势和性别差异,为急淋防控政策措施的制定提供更详实的数据支撑。
数据来源于2021年全球疾病负担最新数据库(GBD 2021,http://ghdx.healthdata.org)。根据《国际疾病分类第十一次修订本(ICD-11)》,急淋疾病编码为C91。
根据性别、年龄段分组收集1990—2021年中国的急淋发病人数/率、患病人数/率、死亡人数/率和DALY人数/率。运用joinpoint回归模型分析1990—2021年急淋疾病负担的变化特征,建立分段回归,寻找趋势变化的转折点,计算年度变化百分率(annual percentage rate change,APC)、年均变化百分率(average annual percentage rate change,AAPC)及其95%可信区间(confidence interval,CI)量化变化趋势。当AAPC(或APC)及其95%CI的下限均小于0时,该时间段内呈下降趋势;AAPC(或APC)及其95%CI的下限均大于0时,该时间段内呈上升趋势;若95%CI包含0,则表示趋势变化无统计学意义。此外,AAPC(或APC)绝对值越大,代表趋势变化越明显[11]
采用Excel 2009录入并整理数据,根据年份、性别、年龄段对急淋1990—2021年的发病率、患病率、死亡率、DALY率及其95%不确定区间(uncertainty interval,UI)的分布情况进行统计描述。利用Joinpoint 5.2.0.0软件进行joinpoint回归分析。以年份为自变量,分别以年龄标化发病率(age-standardized incidence rate,ASIR)、年龄标化患病率(age-standardized prevalence rate,ASPR)、年龄标化死亡率(age-standardized death rate,ASDR)、年龄标化DALY率为因变量,在1990—2021年,按性别(总体、女性、男性)对疾病负担变化的趋势进行拟合。采用pairwise comparison进行成对比较分析[12],判断中国女性与男性之前疾病负担变化趋势的差异是否具有统计学意义。检验水准α=0.05。运用Graphpad prism 8和OriginPro 2024对不同性别、年龄段的负担变化趋势进行图像描述性分析。
1990—2021年,中国急淋ASIR和ASPR均呈上升趋势,AAPC分别0.15%和3.87%(P<0.05)。而ASDR和DALY率均显著下降,AAPC分别为-2.61%和-2.77%(P<0.05),见表1。Joinpoint回归模型显示,过去32年中国急淋的变化趋势呈现出2个比较明显的折点,第一个折点在2004年左右,急淋的ASIR、ASPR快速上升,ASDR下降更为缓慢;第二个折点在2019年左右,ASIR、ASPR由上升趋势转为快速下降,DALY率由快速下降转折为快速上升。见图1
1990—2021年,女性的ASIR、ASDR和标化DALY率呈下降趋势,AAPC分别为-0.4%、-3.3%、-3.9%(P<0.05);ASPR呈上升趋势,AAPC为3.2%(P<0.05)。男性的ASIR和ASPR呈上升趋势,AAPC分别为0.6%和4.3%(P<0.05);ASDR和标化DALY率呈下降趋势,AAPC分别为-2.1%和-2.6%(P<0.05)。1990—2021年,相较男性,女性ASIR、ASPR、ASDR和标化DALY率下降更明显,差异有统计学意义。此外,在2019年,男性、女性的ASIR、ASPR同步出现急转向下。见表2图2
1990—2019年,中国急淋发病数、患病数最高的年龄段是2~4岁,呈逐年增加趋势,但在2019年后出现明显下降,见图3A、B。发病率和患病率最高的年龄段集中在4岁以内,尤其是2岁以内,呈逐年上升趋势。但2019年之后,2~4岁年龄段人群发病率、患病率同期出现显著降低,见图4A、B。死亡数/率、DALY数/率在20岁以内年龄段呈逐年下降趋势,中老年人群死亡数、DALY数呈增加趋势,且死亡率较高的年龄段从婴幼儿向老年转变。见图3C、D4C、D
1990—2021年,男性和女性的发病率、患病率、死亡率和DALY率在不同年龄段的分布呈现相同变化趋势,老年男性的死亡率居高不下,见图5。2021年,死亡率呈双峰模式,第一个峰男性和女性都在2岁以内;第二个峰出现在55岁后,女性在80~84岁达峰值,男性在85~89岁达峰值。死亡率的性别差异在60岁后逐渐增大,在85~89岁段最为显著。男性和女性的发病率、患病率和DALY率最高的年龄段都在2岁以内,性别间差异以<1岁年龄段最为显著,其次是12~23月。各年龄段的男性发病率、患病率、死亡率和DALY率均大于女性。见图6
研究结果显示,1990—2021年,中国急淋ASIR和ASPR整体呈现增长的趋势,且与2019年全球数据相比,中国急淋ASIR、ASDR和标化DALY率明显高于全球和各级社会人口学指数(SDI)国家水平[13],表明中国急淋的疾病负担处于较高水平,是中国疾病防控工作的重点。而ASDR和标化DALY率呈现显著的逐年下降,证明了中国在急淋诊疗方面的有效性。在中国,急淋是最常见的儿童恶性肿瘤,儿童急淋诊疗也一直是我国医疗工作的重点,诊疗服务网络建设-患儿不动专家动[14]、加大儿童白血病专项求助力度、新药的审批上市和最新儿童急淋诊疗方案的发布[9]等有效的推动了我国儿童急淋诊疗水平的提升。但值得关注的是1990—2021年中国ASIR和ASPR仍呈上升趋势,表明在急淋的“防”“治”工作中,虽然“治”已经初显成效,但“防”仍是薄弱环节。可喜的是,我们看到2019年中国急淋的ASIR和ASPR由上升趋势转折为快速下降。一方面可能与中国人口出生率明显下降[15],儿童急淋发病数和患病数明显降低有关。另一方面也归功于近年来中国各项政策制度的不断完善和健康教育活动的普及。住房城乡建设部关于完善质量保障体系指导意见的通知,降低了建材材料中有害物质对人体的伤害[16];“健康中国行(2019—2030年)”等主题健康教育活动,有效提升了公众对疾病的认识和预防意识[17]。此外,还可能与全国COVID-19疫情导致的急淋诊断延迟有关[18]
从性别分组来看,中国女性急淋的ASIR、ASPR、ASDR和标化DALY率均低于男性。过去32年,性别差异所导致的疾病负担差异越来越明显。本研究该结果与Wu[5]、Pagliaro L[7]、唐[19]等人的研究结果一致。Pagliaro L等人[7]基于急淋最新进展进行了综述,男性已明确纳入急淋的潜在风险因素,这可能与性别差异的遗传、性别特异性肿瘤抑制基因相关。此外,男性的社会角色和行为习惯,如吸烟、环境污染、不良生活习惯等,可能增加了男性患急淋的风险[20]。因此,男性可作为中国急淋防控工作的重点人群,加强对男性的健康宣教、定期健康体检,以降低疾病负担。
从年龄分组来看,1990—2021年中国急淋发病、患病负担最高的年龄段均集中于儿童阶段,发病人数和患病人数随着年龄增加呈先增加后下降的趋势。而死亡负担较高的年龄段由1990年的婴幼儿向2021年的老年转变。可见,急淋各年龄段疾病的负担存在明显的区别,主要好发于儿童,这可能与儿童更易受遗传、胎儿发育过程中的变异、病毒感染和延迟感染等影响有关[7,21]。可观的是,2019年之后,2~4岁年龄段的发病数/率和患病数/率显著下降,是引起该时间段中国整体ASIR和ASPR疾病负担下降的主要原因。同时儿童急淋疾病的死亡和DALY负担下降,这离不开中国医疗体系的健全完善和服务的提升,如国家出台了系列儿童白血病的救助政策[22];更离不开各种诊疗技术的推进[7]。而老年疾病死亡负担有增加趋势,一方面可能与中国老年化程度加剧[23],另一方面可归因于老年人群随着年龄增长,生理机能下降、慢病增多、治疗耐受性下降等原因。目前,急淋的成年人群体已引起了一定的关注,国家出台了《中国成人急性淋巴细胞白血病诊断与治疗指南(2024年版)》[24]用于指导成人急淋的规范诊疗。总体来看,随着年代的发展,儿童和青少年急淋疾病负担虽有所减轻,但依旧是中国急淋疾病负担的重点人群,同时中老年人群疾病负担并未得到有效的缓解。
综合性别和年龄分组来看,1990—2021年男性和女性的疾病负担在不同年龄段分布呈现相同变化趋势,老年男性的死亡率居高不下。2021年,各年龄段男性各项指标的负担均大于女性。发病率、患病率和DALY率的性别间差异以<1岁年龄段最为显著。而死亡率的性别差异在60岁后逐渐增大,在85~89岁段最为显著。可见,急淋的发病率、患病率和DALY率负担主要在于儿童和男性,而死亡率的负担主要在于老年男性。因此,中国儿童男性人群急淋发病率的增加和老年男性人群急淋死亡负担的加重,是我们今后关注的重中之重。
综上,中国急淋发病和患病负担呈上升趋势,而死亡和DALY呈下降趋势。根据急淋发病年龄、性别特点,中国急淋防控的重点人群仍为儿童和男性。近年来,中国儿童急淋的防控效果是可观的,但在当下人口负增长,老龄化严重等大环境下,儿童的急淋负担继续降低的同时,老年人群的负担可能会有所加重。因此,中国需加强对急淋危险因素、发病机制和新药的研究,在持续加强儿童急淋防治措施的同时,提高对老年人群高死亡率的有效管理。
