Objective

To investigate and analyze the situation of hospital acquired infection in surgical ward of Guizhou Province in 2022, and to provide direction and method for improving the prevention and control of hospital acquired infection.

Methods

The prevalence of hospital acquired infection in surgical wards of 186 medical institutions in the province from July to December 2022 was investigated by means of a cross-sectional survey and bedside survey combined with inpatient records.

Results

A total of 27 046 patients were investigated, and 504 cases of hospital acquired infection occurred. The prevalence rate of hospital acquired infection was 1.86%, neurosurgery(4.43%) and cardio-thoracic surgery(3.25%) were the highest, and the difference of infection rate among different surgical systems was statistically significant(χ2=157.980, P<0.001). The main site of hospital acquired infection was respiratory tract(0.99%), surgical site(0.50%), skin and soft tissue(0.24%), and the incidence rate of different infection sites was statistically significant(P<0.05). The prevalence rate of hospital acquired infection in Class I incision was 3.42%. The highest utilization rate of antibiotics was 40.72% in cardio-thoracic surgery(13.01%) and neurosurgery(11.64%). The hospital acquired infection rate of Class I incision in different departments was significantly different(χ2=186.607, P<0.001). The main purpose of medication in surgical ward was treatment(26.10%), and the rate of pre-medication examination in surgical ward was 33.41% for single medication(36.06%).

Conclusion

The incidence rate of hospital acquired infection in surgical ward shows a decreasing trend. Cardio-thoracic surgery and neurosurgery are still the most common departments causing hospital acquired infection of respiratory tract and incision-like surgical site infection.

目的

通过对2022年贵州省外科病区住院患者调查分析知悉医院感染情况,为明确院感防控重点、改进思路提供方向。

方法

采用横断面调查方法,通过床旁调查和住院病历相结合的方式对2022年7—12月省内186家医疗机构外科病区进行医院感染现患率调查。

结果

共调查患者27 046例,发生医院感染504例,医院感染现患率为1.86%,神经外科(4.43%)与心胸外科(3.25%)最高,不同外科系统在感染率间差异存在统计学意义(χ2=157.980,P<0.001)。主要的医院感染部位是呼吸道(0.99%)、手术部位(0.50%)、皮肤软组织类(0.24%),不同感染部位间现患率具有统计学意义(P<0.05)。I类手术医院感染现患率3.42%,心胸外科(13.01%)和神经外科(11.64%)最高,不同科室I类手术医院感染率差异显著(χ2=186.607,P<0.001)。抗菌药物使用率40.72%,外科病区用药目的主要以治疗(26.10%)为主,单一用药(36.06%)最普遍。外科病区用药前送检率33.41%。

结论

外科病区医院感染现患率总体呈下降趋势,心胸外科、神经外科仍是造成医院感染中呼吸道感染及Ⅰ类切口手术部位感染的风险较高科室。此外应继续加强对抗菌药物用药前送检率的监管。

目的

通过对2022年贵州省外科病区住院患者调查分析知悉医院感染情况,为明确院感防控重点、改进思路提供方向。

方法

采用横断面调查方法,通过床旁调查和住院病历相结合的方式对2022年7—12月省内186家医疗机构外科病区进行医院感染现患率调查。

结果

共调查患者27 046例,发生医院感染504例,医院感染现患率为1.86%,神经外科(4.43%)与心胸外科(3.25%)最高,不同外科系统在感染率间差异存在统计学意义(χ2=157.980,P<0.001)。主要的医院感染部位是呼吸道(0.99%)、手术部位(0.50%)、皮肤软组织类(0.24%),不同感染部位间现患率具有统计学意义(P<0.05)。I类手术医院感染现患率3.42%,心胸外科(13.01%)和神经外科(11.64%)最高,不同科室I类手术医院感染率差异显著(χ2=186.607,P<0.001)。抗菌药物使用率40.72%,外科病区用药目的主要以治疗(26.10%)为主,单一用药(36.06%)最普遍。外科病区用药前送检率33.41%。

结论

外科病区医院感染现患率总体呈下降趋势,心胸外科、神经外科仍是造成医院感染中呼吸道感染及Ⅰ类切口手术部位感染的风险较高科室。此外应继续加强对抗菌药物用药前送检率的监管。

Objective

To investigate and analyze the situation of hospital acquired infection in surgical ward of Guizhou Province in 2022, and to provide direction and method for improving the prevention and control of hospital acquired infection.

Methods

The prevalence of hospital acquired infection in surgical wards of 186 medical institutions in the province from July to December 2022 was investigated by means of a cross-sectional survey and bedside survey combined with inpatient records.

Results

A total of 27 046 patients were investigated, and 504 cases of hospital acquired infection occurred. The prevalence rate of hospital acquired infection was 1.86%, neurosurgery(4.43%) and cardio-thoracic surgery(3.25%) were the highest, and the difference of infection rate among different surgical systems was statistically significant(χ2=157.980, P<0.001). The main site of hospital acquired infection was respiratory tract(0.99%), surgical site(0.50%), skin and soft tissue(0.24%), and the incidence rate of different infection sites was statistically significant(P<0.05). The prevalence rate of hospital acquired infection in Class I incision was 3.42%. The highest utilization rate of antibiotics was 40.72% in cardio-thoracic surgery(13.01%) and neurosurgery(11.64%). The hospital acquired infection rate of Class I incision in different departments was significantly different(χ2=186.607, P<0.001). The main purpose of medication in surgical ward was treatment(26.10%), and the rate of pre-medication examination in surgical ward was 33.41% for single medication(36.06%).

Conclusion

The incidence rate of hospital acquired infection in surgical ward shows a decreasing trend. Cardio-thoracic surgery and neurosurgery are still the most common departments causing hospital acquired infection of respiratory tract and incision-like surgical site infection.

医院感染(Hospital acquired infection,HAI)是评价医疗质量与安全的重要指标,也是保障患者健康的关键一环[1-3]。2022年,WHO发布了全球首份感染预防与控制(Infection Prevention and Control,IPC)报告,表明及时的感控措施可减少70%的医院感染[4]。如今,每100名患者中就有7~15名在住院期间至少发生过一次医院感染,每10名院感患者中有1名死亡[5]。2015年,国家卫计委办公厅印发《医院感染管理质量控制指标》,强调了加强指标应用、信息收集和反馈工作的重要性,确保监测数据可在不同区域与不同医疗机构间进行比较[6-7]。近年来相继有许多医院感染的横断面调查数据发表,但多为单个医疗机构且地区代表性不佳[8-10]。外科病区作为医院感染发病率相对较高病区,做到及时发现、有效控制至关重要,但鲜少有关于外科病区的具有地区代表性的Ⅰ类切口感染、抗菌药物使用及抗菌药物病原菌送检率等重点指标的分析。为调查区域医院感染基本情况,分析外科病区感染差异与关键因素,促进地区医院感染精准防控,有效减少不良事件发生,我质控中心于2022年对贵州省186家各级医疗机构全部住院患者进行了横断面调查,报道如下。
对贵州省各级医疗机构住院患者按照自愿原则进行调查,共监测医疗机构186家,通过数据审核患者例数89 409,其中外科病区27 046例。研究包括转科、死亡的患者,不包括调查当日入院患者。
随机选取2022年7月15日—2022年12月31日中任意一天作为调查日,按照每50张床位配备1~2名经培训合格后的调查人员,由院感科专职人员和各病区主治及以上医师,通过全国医院感染监测网下发的调查问卷统一上报结果类数据[7]。由院感科5名专职人员同时审核数据,针对有悖逻辑处给予电话问询并校正,对于缺失部分进行电话或短信咨询并补充,审核完毕后于一周内填报至全国医院感染监测系统,上报时间与全国同步。
1.3.1 至少于调查开始前一周院感科下发调查通知并组织专职人员对临床医生及护士进行培训,具体针对调查问卷内容专项培训,同时完善住院患者的病原学送检等相关检查。
1.3.2 调查当天临床主治及以上医师到患者床旁以询问和体检的方式进行调查,每位患者至少5 min,增做必要的体格检查,同时填写床旁调查表,于当日完成个案调查表与床旁调查表的填写与提交。
1.3.3 院感科人员汇总及审核后录入“全国医院感染横断面调查数据网络处理系统”,统计分析。
按照2001年发布的《医院感染诊断标准(试行)》[11]判定本院医院感染病例,排除医源性感染后不满足医院感染诊断标准的感染判定为社区感染。
审核合格的数据进行汇总导出后建立数据库并归类统计,使用SPSS 22.0对数据进行分析,计数资料采用[n(%)]表示,进行卡方检验,检验水准α=0.05,差异有统计学意义。
在所有住院患者中,医院感染现患率为1.19%,外科病区感染率(1.86%)高于平均水平。外科病区监测人数占所有病区的30.25%,神经外科(4.43%)与心胸外科(3.25%)感染率最高,其他外科均低于外科病区总感染率。不同外科系统在感染率间差异存在统计学意义(χ2=157.980,P<0.001),见表1
外科病区医院感染不同部位现患率见表2,2022年贵州省外科病区共发生医院感染504人次,呼吸道感染现患率居首位,0.99%,其次是手术部位,0.50%,不同部位间外科各系统现患率存在显著差异(P<0.05);感染部位构成比见图1,呼吸道、手术部位、皮肤软组织类构成比位列前三,分别为44.44%、22.22%、10.95%。
共对6 036名I类手术患者进行了监测,发现I类手术医院感染现患率为3.42%。外科病区I类手术监测人数占所有病区的88.15%,以心胸外科和神经外科感染率最高,分别达到了13.01%与11.64%(表3),不同科室I类手术医院感染率差异显著(χ2=186.607,P<0.001)。
在2022年贵州省外科病区27 046例调查人数中,抗菌药物使用率为40.72%。外科病区用药目的主要以治疗(26.10%)为主,单一用药(36.06%)情况最普遍,在外科病区各系统中,泌尿系统抗菌药物使用率最高(69.34%),见表4
在186家医疗机构中,32 241名患者治疗性使用抗菌药物,其中外科病区送检数为3 579,送检率44.91%,用药前送检率33.41%,见表5
WHO表明,有效的感染预防控制是减少医院感染的传播及遏制新出现病原体的基础[5]。2022年全国医院感染监测网开展了第12次全国医院感染横断面调查,贵州省医院感染管理质控中心同期组织全省医疗机构对所有住院患者进行调查,按统一的数据收集、报告方法和审核程序汇总,以分层评价,找出问题,针对性持续改进。外科患者由于手术多、侵入性诊疗操作如气管插管、留置尿管、动静脉置管多,因而机体各部位屏障作用减弱,免疫力下降,为医院感染创造了条件,同时,术后恢复时间长,住院时间相应延长,导致抗菌药物频繁使用,患者菌群失调,容易引发医院感染[12]。据国内外研究,外科不同科室之间医院感染发生率不同[13],发生医院感染的部位也有较大差异。
2022年贵州省外科病区住院患者现患率调查结果表明,医院感染现患率为1.86%,低于2016年全国医院感染外科病区现患率3.07%[14]与2017年上海市某区医疗机构2.54%[15]。从2014—2022年,感染率前三从烧伤科5.79%、神经外科5.46%、心胸外科3.78%变为了神经外科4.43%、心胸外科3.25%、骨科1.82%,贵州省外科病区医院感染现患率总体呈下降趋势[16]。烧伤科感染率降低明显,可能与医疗设施设备更新、医疗环境改善有关,烧伤患者创面大,皮肤屏障破损,易感染,良好的医疗环境、先进医疗设备可减少病原微生物的侵入。此外,也可能与住院时长、烧伤部位等有关。
从感染部位现患率来看,心胸外科、神经外科仍是造成呼吸道感染的高发科室,这与2016年全国医院感染监测基本一致,可能由于心胸外科手术部位原因,术后肺炎几率大,且手术时间长、需要全身麻醉等特点,患者发生感染机会更多。神经外科现患率高可能与手术操作复杂、操作时间长,意识障碍导致排痰能力减弱,还与气管切开和留置胃管等相关[14]。从感染部位构成比来看,贵州省外科病区医院感染部位以呼吸道为主,其次为手术部位与皮肤软组织感染,与2014年全国医院感染[17]、2019年云南[18]、2017年内蒙古[19]调查发现主要部位为呼吸道、手术部位和泌尿道感染相比,排名发生了一定变化,提示该省需要加强对皮肤软组织感染医院感染的判定及防控,另一方面,肺炎及导管相关感染的监测、防控与干预仍是今后的工作重点。
在美国,每年要进行2 700万例外科手术,手术部位感染占医院感染的31%,跃居首位,这增加了患者的住院时间、再入院率、医疗保健费用,加重患者负担。据美国疾控中心估计,评价每位患者手术部位感染每年的费用为20 785美元,死亡率为3%,与感染直接相关的死亡风险高出2~11倍[20]。Ⅰ类切口手术部位感染率在我国也是医院感染管理质量评价的重要指标,本次调查发现外科病区Ⅰ类切口手术部位医院感染现患率为3.42%,较2014年有所下降,表明近年来随着诊疗技术的提高、医疗质量管理的加强,该省对于I类手术部位感染控制能力有所提升。调查中,心胸外科与神经外科感染率分别达到了13.01%与11.64%,可能与以上两个科室手术量多、切口大、暴露部位大有关,这也提示该省应重点关注对于心胸外科、神经外科Ⅰ类切口手术部位感染的目标监测及防控。
《全国抗菌药物临床应用专项整治活动方案》中指出,抗菌药物使用率要求≤60% ,WHO建议<20% [21],在2022年贵州省外科病区27 046例调查人数中,发现抗菌药物使用率为40.72%,达到了国家要求但高于WHO建议,与2020年全国情况基本一致(37.40%)[22],但依然高于意大利、西班牙等欧洲国家[23]。在外科病区各系统中,泌尿系统抗菌药物使用率69.34%不符合使用要求,提示贵州地区应加强对泌尿外科抗菌药物使用管理。本调查显示,贵州省外科病区抗菌药物用药目的方面以治疗用药为主(26.10%),低于2012年全国同类数据[24]。单一用药(36.06%)情况最普遍,原因可能是相关知识培训增多,医务人员对抗菌药物的合理应用意识增强;其次是监管制度完善,病原学检验频次提高。
在治疗使用抗菌药物病原菌培养送检率方面,调查发现外科病区用药前送检率33.41%,不符合《“提高住院患者抗菌药物治疗前病原学送检率”专项行动指导意见》中接受抗菌药物治疗的住院患者,抗菌药物使用前病原学送检率≥50%要求[25],其中仅泌尿外科51.56%达到了改进目标的要求,可能由于贵州地区医疗发展滞后,病原学检查的相关硬件设施设备缺乏,医生病原学送检意识不强,微生物室病原学检出率低等因素,导致与发达区域还存在差距,提示本地区仍然存在抗菌药物用药指征掌握不严格、抗菌药物治疗方案不合理、经验用药等问题。
在本次调查中,贵州省外科病区医院感染现患率总体呈下降趋势,心胸外科、神经外科仍是造成医院感染中呼吸道感染及Ⅰ类切口手术部位感染的风险较高科室,在进行院感防控指导时应更重视,此外,皮肤软组织感染也逐渐增多,应提高关注。目标监测方面,抗菌药物使用率逐年下降,但泌尿系统超出改进要求,提示了贵州地区应加强对泌尿外科抗菌药物使用管理。值得一提的是该地区外科病区用药前送检率不符合抗菌药物使用前病原学送检率≥50%要求,应继续加强对抗菌药物用药前送检率的监管,对医务人员进行相关培训,提高临床医生病原学送检的依从性,以有效减少医院感染的发生。
