With the rapid development of the social economy and changes in population demographics,public health is facing increasingly complex challenges.Enhancing the public health literacy of non-preventive medicine professionals (including general practitioners, specialists, and nursing staff) has become a crucial task in building a healthy China.The integration of medical care and disease prevention offers a new pathway for the training of non-preventive medicine professionals.This article focuses on healthcare practitioners directly involved in clinical services, exploring how to improve the public health literacy of non-preventive medicine professionals through educational reform,policy support,and practical training under the framework of medical-prevention integration.Proposed strategies include optimizing the curriculum system,strengthening policy evaluation,enhancing continuing education,and innovating postgraduate training mechanisms to promote the deep integration of medical care and disease prevention, therebyenhancing the capacity of public health services.

随着社会经济的快速发展与人口结构变化,公共卫生面临着日益复杂的挑战,提升非预防医学背景的医学人才(含全科医生、专科医师及护理人员)公共卫生素养成为健康中国建设的重要任务。医防融合通过医疗与疾病预防的一体化,为非预防医学人才培养提供了新路径。本文聚焦直接参与诊疗服务的医务人员群体,探讨在医防融合背景下,如何通过教育改革、政策支持及实践培训提升非预防医学人才的公共卫生素养,提出优化课程体系、强化政策考核、完善继续教育及创新研究生培养机制等策略,以推动医防深度融合,提升公共卫生服务能力。

With the rapid development of the social economy and changes in population demographics,public health is facing increasingly complex challenges.Enhancing the public health literacy of non-preventive medicine professionals (including general practitioners, specialists, and nursing staff) has become a crucial task in building a healthy China.The integration of medical care and disease prevention offers a new pathway for the training of non-preventive medicine professionals.This article focuses on healthcare practitioners directly involved in clinical services, exploring how to improve the public health literacy of non-preventive medicine professionals through educational reform,policy support,and practical training under the framework of medical-prevention integration.Proposed strategies include optimizing the curriculum system,strengthening policy evaluation,enhancing continuing education,and innovating postgraduate training mechanisms to promote the deep integration of medical care and disease prevention, therebyenhancing the capacity of public health services.

随着社会经济的快速发展和人口结构的变化,公共卫生面临日益复杂的挑战。近年来,突发公共卫生事件凸显了公共卫生应对能力的不足,尤其是在疾病预防、健康促进和健康教育领域。健康素养,即个体获取、理解和应用健康信息与服务的能力,已成为提升全民健康水平的重要因素[1]。健康素养不仅影响个人健康决策和生活方式选择,还与公共卫生政策实施效果和社会整体健康水平的提升息息相关。非预防医学背景的医学人才(含全科医生、专科医师及护理人员)不仅需具备扎实的临床知识和技能,还需掌握公共卫生理念和能力,以应对日益复杂的健康问题。然而,目前我国非预防医学人才的公共卫生素养普遍不足,尤其在公共卫生知识、预防控制能力和健康教育能力等领域存在显著挑战[2]
医防融合模式的提出,为提升非预防医学人才的公共卫生素养提供了新路径。该模式将医疗服务与预防性公共卫生服务有机结合,通过一体化运作实现健康服务的全面覆盖和高效整合[3]。这不仅要求非预防医学人才具备临床诊疗能力,还需掌握公共卫生的理论知识和实践能力,实现治病与防病的结合。然而,医防融合在推进过程中仍面临教育体系偏重临床、资源有限及重医轻防观念根深蒂固等挑战[4]。基于此,本文聚焦非预防医学背景的医学人才,旨在探讨在医防融合背景下,如何通过教育与政策创新,构建适应医防融合需求的非预防医学人才培养体系,为推动健康中国建设提供科学依据。
医防融合是指将医疗服务与疾病预防控制有机结合,形成医疗与预防一体化的健康服务模式[3]。该概念首次提出于2018年,标志着中国在优化公共卫生体系方面迈出了重要步伐[5]。2018年3月,国家卫生健康委员会发布了《关于做好2018年家庭医生签约服务工作的通知》,明确提出以家庭医生为基础,为签约居民提供医防融合的综合连续性服务[5]。随后,2019年4月,国家卫生健康委办事厅进一步发布了《关于做好2019年家庭医生签约服务的通知》[6],继续强化基层医疗的融合发展。2022年4月,国务院发布了《关于印发“十四五”国民健康规划的通知》,进一步强调了防治结合和医防融合的重要性[7]
医防融合的核心在于通过制度创新和资源整合,实现医疗服务与公共卫生服务的协同发展。医防融合要求医疗机构不仅具备临床诊疗能力,还需具备疾病预防、健康教育和健康促进等公共卫生职能[8]。通过这种一体化的服务模式,可以提高医疗资源的利用效率,增强公共卫生应急响应能力,促进居民健康水平的全面提升。
在全球范围内,医防融合已成为提升公共卫生体系效率的重要趋势。许多发达国家通过政策支持和制度创新,推动医疗与公共卫生服务的整合。英国的全科医生制度强调预防为主,结合临床治疗与健康促进[9];美国的医疗保健系统则通过社区健康中心实现医疗与预防服务的协调[10]。信息化技术的应用,如电子健康记录和健康信息交换平台,极大地促进了医防融合的发展[11]。这些国家通过系统性政策和实践,不断优化医防融合模式,提升了公共卫生应急响应能力和整体健康水平。在中国,医防融合起步较晚,但近年来取得显著进展。自2018年家庭医生签约服务逐步推广,成为医防融合的重要载体。2022年《“十四五”国民健康规划》进一步推动各地区落实相关政策[7]。各地通过建立综合医疗平台、加强基层医疗建设和推进信息化,实现医疗与公共卫生的结合。同时,医疗机构与公共卫生机构之间协作不足、资源配置不合理、信息共享不畅等诸多挑战仍然存在[12]。因此,必须通过系统性的教育改革、政策支持和资源优化,促进医疗与公共卫生的深度融合,全面提升公共卫生服务能力,保障居民健康。
在医防融合的背景下,非预防医学人才培养的需求日益迫切,要求医学院校全面提升这类医学人才的综合素质与实践能力,尤其是在临床诊疗与公共卫生协同能力方面。医学教育应紧密结合健康中国战略,将临床医学与预防医学有机融合,培养具备多维度能力的复合型人才。临床医学人才需以岗位胜任力为核心,通过“基础与临床、临床与预防、医学与人文”深度融合,培养“人文素养高、预防观念先、专业基础实、临床技能强”的综合能力,解决人才培养与社会需求、基础与临床、临床与预防、医学与人文的脱节问题[13]。全科医生需围绕防治结合目标,在掌握常见病、多发病和慢性病诊疗能力的基础上,强化公共卫生与预防医学服务能力,落实重点人群红、黄、绿三级健康管理[14];感控专职人员队伍建设关系到医疗机构的感控管理水平及防控成效,通过强化上岗培训与考核,从源头加强管理,能够实现感控专职人员队伍能力的切实提升[15]。培养复合型医学人才需重点加强应急管理和突发公共卫生事件处置能力,提升医务人员的快速反应和防控能力[16]。医学人文教育也是关键环节,医学教育应注重伦理、职业道德和人文关怀的培养,以增强医学生的社会责任感和同理心。中医学人才培养方面,应强化中西医结合的预防理念,提升创新思维与全球视野,以应对现代医学和公共卫生领域的复杂挑战[8]。为实现上述目标,政府、医学教育机构与基层医疗单位需协同合作,共同推进医防融合的人才培养,建立完善的评价机制和支持体系,确保培养质量与社会需求相匹配,提升公共卫生服务能力,保障全民健康。
公共卫生素养是指个体、社区或组织在面对健康问题时,能够有效获取、理解、评估和应用健康信息,从而做出科学决策并采取行动,以促进健康、预防疾病并减少公共卫生风险[17]。在突发公共卫生事件频发的时代背景下,非预防医学人才的公共卫生素养显得尤为关键,直接影响着公共卫生危机的应对能力和效果。公共卫生素养的核心维度包括以下几个方面:
(1)知识素养:指医学人才对公共卫生基本概念、理论、政策和技术的理解与掌握[18]。涵盖疾病预防、健康促进、环境健康等核心知识,以及应对突发公共卫生事件的基本策略和信息处理能力[19]。还包括对健康信息技术(如电子健康记录、健康应用程序)的运用能力,以及公共卫生政策的制定与实施。
(2)行为素养:指医学人才将所学知识转化为实际行动的能力[20]。这不仅体现在突发公共卫生事件中的快速响应与有效处置,还包括在日常工作中规范操作、实施健康教育和疾病预防措施,以及设计和开展健康促进活动[21]。医学人才需在紧急情况下采取必要的安全措施保障公众健康,并能与公众、媒体及其他利益相关者进行有效沟通,清晰传达健康信息并回应公众关切,具备跨部门合作与协调应对复杂公共卫生挑战的能力[19]
(3)态度素养:涉及医学人才对公共卫生问题的认知态度和价值观[22],包括科学态度、社会责任感和文化敏感性。医学人才在公共卫生实践中需遵循伦理规范和职业道德,树立科学的健康观念,重视公共卫生问题,并在决策过程中注重科学性和合理性[23]。同时,关注隐私保护和信息透明度等具体伦理问题。
(4)疾病预防与控制素养:指医学人才在疾病预防和控制方面的专业知识与技能[24]。这包括了解疾病的传播途径、预防措施和控制策略,具备传染病的预防、控制和管理能力,以及在健康促进和环境卫生方面的实践能力[23]。还包括针对新发传染病的预防与控制策略,如快速响应新兴病原体的能力,以有效减少疾病的传播和影响。
2.2.1 公共卫生知识掌握程度不足 公共卫生知识的掌握不足是我国非预防医学人才面临的显著短板。研究显示[25],非预防医学专业的临床医学生对公共卫生核心概念的认知存在明显缺陷,仅58.96%理解三级预防思想,11.94%能准确翻译“公共卫生”英文术语,反映出知识体系的碎片化与基础薄弱。在实践性知识领域,学生的掌握情况同样不容乐观,仅43.97%的学生知晓结核病“治疗知识”,38.90%知晓“诊断方法”[26]。基层医疗机构的相关调研进一步揭示了这一问题的严重性。数据显示[27],53.2%的受访者指出现有公共卫生服务人员的临床知识储备不足;48.9%的受访者认为临床医生普遍缺乏公共卫生知识和技能。这一现象凸显了非预防医学人才在公共卫生知识掌握方面的系统性不足,不仅限制了其职业能力的全面发展,也对公共卫生与临床医疗的深度融合形成了阻碍。
2.2.2 公共卫生应急响应能力不足 我国部分地区的公共卫生应急响应体系仍不完善。虽然许多地区已将公共卫生应急响应纳入政策框架,但在实际执行中,依然面临培训不足、资源配置不均等问题。应急队伍的培训体系不规范,缺乏定期、标准化的培训,且经费和资源投入不足,导致响应能力存在差距[2]。许多地区的应急响应缓慢,医务人员因缺乏应急处置能力和防控经验,未能及时采取有效措施,导致公共卫生事件扩散。在一些地方,公共卫生人员的快速调度和协调能力不足,无法有效配合各项防控措施,影响了疫情防控效果[28]。整体而言,公共卫生应急管理的体系建设仍然面临较大挑战,亟需建立更加高效、灵活的应急响应机制。
2.2.3 课程设置与实践需求脱节,缺乏有效的教育协同机制 医学教育中的公共卫生课程与临床医学课程之间缺乏有效的衔接与协调。大多数医学院校将公共卫生知识视为辅助性课程,课程内容偏重理论,缺少与实际需求对接的实训课程和模拟演练,导致学生应对突发公共卫生事件时应变能力不足[29]。很多医学生和年轻医务人员无法迅速将公共卫生防控理论知识与临床治疗技能相结合,缺乏高效的应急反应能力。公共卫生与临床医学的学科壁垒依然存在,导致非预防医学人才未能在整体教育体系中形成公共卫生与临床医学的综合素养。推动公共卫生与临床医学课程的有机融合,培养复合型医学人才,成为提升应急响应能力的关键。
2.2.4 高层次复合型人才培养体系不足与质量缺口我国公共卫生教育仍偏重单一学科的培养,尤其是流行病学、卫生学等传统学科领域的深耕,使得高层次复合型人才的培养存在较大缺口[2]。复合型人才不仅需要具备扎实的公共卫生专业知识,还应具备一定的临床医学、健康管理、应急响应等跨学科的知识与技能。然而,现有公共卫生教育体系中,跨学科的人才培养机制尚不健全,难以为复杂的公共卫生问题提供综合解决方案。随着公共卫生问题的日益复杂化,传统的公共卫生教育模式已无法满足社会需求,亟需调整培养模式,推动跨学科教育,培养能够灵活应对复杂公共卫生问题的高层次复合型人才。
2.2.5 基层公共卫生人才流动性问题 基层公共卫生人才流动性问题长期存在,且随着公共卫生需求的增加,问题愈加突出。许多基层公共卫生人员因待遇低、职业发展空间有限,导致大量优秀人才流失。王亚楠等指出[30],约59.5%的基层公共卫生人员在五年内离职,尤其在中西部地区,基层卫生人员的流动性更大。基层公共卫生人员不仅面临较低的薪酬水平,还因缺乏系统的继续教育和职业发展路径,导致其对职业前景感到迷茫,进一步加剧了人才流失。同时,基层公共卫生人员的应急处置能力相对较弱,尤其在面对突发公共卫生事件时,由于培训不足、知识储备有限,往往无法有效应对,影响了公共卫生事件的及时处置和防控效果。
为培养具备公共卫生素养的复合型医学人才,医学教育需要全面改革与创新。应在医学教育体系内明确公共卫生学科的重要性,将其视为培养非预防医学人才的基础模块之一。通过开设专门的医防融合课程,不仅涵盖疾病预防与健康管理,还应扩展到更广泛的公共卫生领域,如环境健康、社会医学、卫生政策与法律等内容[31]。这些课程的内容设计应融入现代医学的实际需求,并与国家健康战略目标相对接,如健康中国2030战略中的重点任务,确保所培养的非预防医学人才能够适应未来公共卫生挑战[32]。跨学科的医防融合课程设置应鼓励不同学院与学科之间的合作,特别是临床医学学院与公共卫生学院。
通过联合开发课程,设计医学与公共卫生结合的实践项目,既能增强学生的综合应用能力,又能突破学科间的壁垒,促进非预防医学人才全面发展。以“全科医生”培训为示例[33],可以通过模拟实习和跨学科合作,让学生在面对真实病例时,能同时考虑疾病的临床诊治与公共卫生预防策略,提升其全面应对复杂公共卫生问题的能力。在实践教学体系上,可以通过虚拟仿真技术、案例教学、和社区实习等多元化教学方法,强化学生在实际公共卫生环境中的应变能力。尤其是在基层医疗和公共卫生领域的实践,使学生在面对突发公共卫生事件时,能够迅速做出反应,进行有效处置[34]
政策支持与绩效考核机制是推动非预防医学人才公共卫生素养提升的关键环节。在政策层面,政府应进一步推动医疗和公共卫生领域的深度融合。国家层面可通过出台相关政策,推动医疗机构不仅仅以治疗为核心,同时加强公共卫生责任。比如,通过医保政策将公共卫生服务纳入医疗服务体系,鼓励医疗机构参与疾病预防、健康促进和健康管理等领域,推动医防融合服务的全面实施[35]。在绩效考核方面,不仅要量化医疗服务的质量,还应将公共卫生工作纳入考核体系,形成与公共卫生服务相关的具体指标。可以参考世界卫生组织对卫生服务绩效评估的标准,考量居民健康改善、疫苗接种率、疾病预防与控制成效、慢性病管理效果等多维度数据[36]。通过综合考核,不仅激励医务人员的工作积极性,还能提升其对公共卫生工作的责任感,推动医防融合的持续优化。同时,为保障医防融合的顺利实施,应建立完善的信息共享平台。通过信息化手段整合公共卫生和医疗资源,建立统一的数据平台,以支持政策决策,提升资源配置效率和公共卫生服务效果。这种共享平台不仅能促进医疗机构与公共卫生机构之间的协作,还能加快疫情监测、预警和资源调配的效率。
非预防医学人才的继续教育和职业培训体系是确保其公共卫生素养持续提升的重要环节。随着公共卫生问题的日益复杂和突发事件的频发,传统的教育模式已经难以满足医务人员日益增长的需求。因此,医学继续教育应结合现代信息技术,采用灵活的学习方式,如在线课程、虚拟实训、远程教学等。通过建立数字化学习平台,不仅能够覆盖到更多地区,特别是偏远地区的医务人员,还能提供灵活的学习方式,使其能够随时更新知识、提高能力[37]。继续教育的课程内容应紧跟公共卫生领域的新发展与新需求。例如,强化传染病防治、慢性病管理、环境健康、健康促进等领域的知识,特别是涉及到公共卫生应急响应和大规模疾病防控的相关知识,确保医务人员在面对疫情等突发公共卫生事件时,能够迅速响应并进行有效处置[38]
同时,继续教育课程还应结合公共卫生的最新研究成果,强调公共卫生数据分析、流行病学研究方法和健康评估等内容,提高医务人员的数据处理与分析能力。继续教育也应将个人学习与绩效考核相结合,采用学分制度和证书体系,激励医务人员自主学习,并将其与职业发展、晋升考核挂钩。建立个性化的继续教育路径,针对不同学科、不同职级的医务人员,提供定制化的培训内容,以促进其专业素质的全面提升。
研究生教育应加强医学、自然科学、人文社会科学与公共卫生学科的交叉融合,培养具备既有扎实医学基础,又具有系统公共卫生知识的高层次人才[39]。通过跨学科培养,使学生能够在面对公共卫生危机时,不仅能够从医学角度进行诊疗,还能从公共卫生角度提出有效的防控措施,做出更为全面的决策。为实现这一目标,建设跨学科教师队伍至关重要。教师应具备多学科背景,结合医学、公共卫生、流行病学、环境健康等领域的专业知识进行授课,并在教学中引导学生进行实际问题的解决。例如,可以通过多学科合作的案例研究,让学生在模拟公共卫生事件中,分析不同领域的相互作用,并寻找最佳解决方案[40]。此外,研究生阶段的教学应注重学生的实际操作能力和创新思维的培养。通过实地调研、公共卫生项目实习、科研论文写作等形式,提高学生在公共卫生领域的实际应用能力和创新能力。学术研究不应仅限于学术论文写作,更应注重如何将研究成果转化为公共卫生实践中的有效干预策略,以推动医学与公共卫生的深度融合。
本文探讨了在医防融合背景下提升非预防医学人才公共卫生素养的路径与策略。通过分析现有非预防医学背景的医学人才培养体系中的不足,明确了当前医学教育在公共卫生领域的短板,包括公共卫生知识的薄弱、应急响应能力的不足、课程设置的脱节以及复合型人才培养体系的缺乏。针对这些问题,提出了包括完善非预防医学人才基础培养体系、强化政策支持、优化绩效考核机制、加强继续教育与职业培训以及创新研究生培养机制等多方面的对策。提升非预防医学人才公共卫生素养,既需要从教育改革入手,也需要通过政策引导与实践支持形成合力。通过跨学科的教育融合和系统的培养机制,非预防医学人才将能够更好地应对未来公共卫生挑战,提升我国整体公共卫生服务能力。政府、教育机构及医疗系统的协同合作,必将为深化医防融合、推动健康中国建设奠定坚实基础。
未来,随着公共卫生领域日益复杂和全球健康挑战的加剧,非预防医学人才的公共卫生素养提升工作仍需持续推进。特别是在培养创新型、复合型人才方面,应加强对新兴公共卫生问题的应对能力和解决方案的研究,并探索更加灵活和高效的教育模式,推动医疗和公共卫生服务的深度融合。这一进程将不仅提升我国医疗体系的综合应对能力,还将为全球公共卫生安全贡献中国智慧与力量。
