Objective

To explore the bidirectional association between metabolic syndrome(MS) and depression in middle-aged and elderly adults in China.

Methods

This study used participants aged 45 and above from publicly available longitudinal data from the China Health and Retirement Longitudinal Study(CHARLS) from 2011 to 2018. The baseline was based on 2011 data. Stage I studied the longitudinal association of baseline MS with follow-up depression, while Stage II studied the association of baseline depression with follow-up MS. Stage III further investigated the bidirectional association between MS and depression using a cross-lagged panel model, and the follow-up data for all three stages used 2015 data including blood routine examination and physical examinations.

Results

There was a bidirectional association between the two diseases. The baseline MS and its components showed a positive correlation with the follow-up depression(β=0.052, P<0.01), and vice versa(β=0.305, P<0.01), but there were differences in gender subgroups.

Conclusion

There is a bidirectional correlation between MS and its components and depression in the middle-aged and elderly population in China. We should identify, monitor, and intervene in early MS and depression.

目的

探讨中国中老年人代谢综合征(Metabolic syndrome, MS)和抑郁状态的相互影响。

方法

本研究采用2011—2018年中国健康与退休纵向研究(CHARLS)数据中45岁及以上中老年人,基线采用2011年数据,阶段Ⅰ研究基线MS与新发抑郁状态的关联,阶段Ⅱ研究基线抑郁状态与新发MS的关联,阶段Ⅲ采用交叉滞后面板模型进一步研究两者双向关联,随访均采用包含血检与体检的2015年数据。

结果

两者存在双向关联,基线MS组分与随访抑郁状态(β=0.052,P<0.01)以及基线抑郁状态与随访MS组分(β=0.305,P<0.01)呈正相关,但MS组分在性别亚组上存在差异。

结论

中国中老年人群体MS及其组分与抑郁状态存在双向关联,应对其进行早期MS及抑郁状态的识别、监测及干预策略。

目的

探讨中国中老年人代谢综合征(Metabolic syndrome, MS)和抑郁状态的相互影响。

方法

本研究采用2011—2018年中国健康与退休纵向研究(CHARLS)数据中45岁及以上中老年人,基线采用2011年数据,阶段Ⅰ研究基线MS与新发抑郁状态的关联,阶段Ⅱ研究基线抑郁状态与新发MS的关联,阶段Ⅲ采用交叉滞后面板模型进一步研究两者双向关联,随访均采用包含血检与体检的2015年数据。

结果

两者存在双向关联,基线MS组分与随访抑郁状态(β=0.052,P<0.01)以及基线抑郁状态与随访MS组分(β=0.305,P<0.01)呈正相关,但MS组分在性别亚组上存在差异。

结论

中国中老年人群体MS及其组分与抑郁状态存在双向关联,应对其进行早期MS及抑郁状态的识别、监测及干预策略。

Objective

To explore the bidirectional association between metabolic syndrome(MS) and depression in middle-aged and elderly adults in China.

Methods

This study used participants aged 45 and above from publicly available longitudinal data from the China Health and Retirement Longitudinal Study(CHARLS) from 2011 to 2018. The baseline was based on 2011 data. Stage I studied the longitudinal association of baseline MS with follow-up depression, while Stage II studied the association of baseline depression with follow-up MS. Stage III further investigated the bidirectional association between MS and depression using a cross-lagged panel model, and the follow-up data for all three stages used 2015 data including blood routine examination and physical examinations.

Results

There was a bidirectional association between the two diseases. The baseline MS and its components showed a positive correlation with the follow-up depression(β=0.052, P<0.01), and vice versa(β=0.305, P<0.01), but there were differences in gender subgroups.

Conclusion

There is a bidirectional correlation between MS and its components and depression in the middle-aged and elderly population in China. We should identify, monitor, and intervene in early MS and depression.

代谢综合征(Metabolic syndrome,MS)是多种代谢成分异常聚集的病理状态,至少包含三种代谢异常组分,包括中心性肥胖、高血糖、血压升高、血甘油三酯(Triglyceride,TG)水平高和高密度脂蛋白胆固醇(High density lipoprotein cholesterol,HDL-C)水平低[1]。中国45岁及以上中老年人群MS的患病率高达33.0%[2],显著增加糖尿病、心脑血管疾病及癌症患病风险。抑郁症(Depression)是常见的精神障碍,中国精神卫生调查2021年数据显示我国成人抑郁症终生患病率高达6.8%[3],中老年人抑郁症患病率超过30%,患病情况随年龄增长呈正相关[4]。有研究发现抑郁症的发生与MS有密切联系[5],但国内这两类疾病间双向关联研究较少。因此本文旨在利用中国健康与退休纵向研究(CHARLS)数据进一步探索我国45岁及以上中老年人群体MS和抑郁状态的双向关系,为提高中老年人生命质量提供依据。
本研究采用具有全国代表性的CHARLS公开纵向数据[6],自2011年开展基线调查,随后于2013年、2015年和2018年进行三次随访。目前仅2011年和2015年随访数据包含本研究所需的血检与体检数据。
研究纳入基本人口学特征、健康状况与功能、血检和体检的实验室指标、抑郁自评量表(Center for epidemiologicalsurvey, depression scale,CES-D)的基线与随访数据。本文研究分为三个阶段,阶段Ⅰ研究基线MS是否会增加抑郁状态风险;阶段Ⅱ研究基线抑郁状态是否会增加MS患病风险;阶段Ⅲ进一步分析MS与抑郁状态的双向关联。阶段Ⅰ研究对象纳入标准:(1)基线年龄≥45岁,(2)基线无抑郁状态,(3)成功随访,共4 222例。阶段Ⅱ纳入标准:(1)基线年龄≥45岁,(2)基线无MS,(3)成功随访,共5 203例。阶段Ⅲ纳入标准:(1)基线时年龄≥45岁,(2)成功随访,共6 285例。研究流程如图1所示。
MS采用2009年国际糖尿病联盟(IDF)与美国心脏协会/美国国立心肺血管病学研究所(AHA/NHLBI)联合定义标准,即以下5项组分中包含≥3项者:(1)中心性肥胖:亚洲男性腰围≥90 cm,女性腰围≥85 cm;(2)TG升高:TG≥1.7 mmol/L,或已进行针对此项血脂异常的治疗;(3)HDL-C水平低:男性HDL-C<1.0 mmol/L,女性HDL-C<1.3 mmol/ L,或已进行针对此项血脂异常的治疗;(4)血压升高:收缩压(SBP)≥130 mmHg或舒张压(DBP)≥85 mmHg,或已诊断为高血压;(5)高血糖:空腹血糖(FBG)≥5.6 mmol/L,或已诊断为2型糖尿病。采用酶比色法测定FBG、HDL-C和TG,SBP和DBP经三次测量取平均值。
抑郁状态评估采用简版抑郁自评量表(CESD-10),包含10个条目,每个条目选项包括4个等级,总分0~30分,≥10分为有抑郁症状。
本研究纳入年龄、性别、受教育程度、婚姻状态等基本人口学特征以及吸烟、喝酒、癌症、慢性肺部疾病、心脏病、消化道疾病、情感及精神疾病等健康状况作为协变量。协变量数据来自2011年基线调查。
计数采用例(n)表示,组间比较采用χ2检验;数值采用均数±标准差()表示,组间比较采用t检验。阶段Ⅰ和阶段Ⅱ采用logistic回归估计比值比(OR)和置信区间(CI),均对潜在混杂因素进行调整(模型1根据性别、年龄、受教育程度、婚姻状态等基本人口学特征进行调整;模型2对基本人口学特征以及健康状态等所有协变量进行调整);阶段Ⅲ构建MS和抑郁状态的交叉滞后面板模型,对协变量进行调整;最后分析MS五个组分分别与抑郁状态的双向关系。本研究应用SPSS 25.0及Amos 22.0软件,检验水准α=0.05。
阶段Ⅰ纳入4 222例研究对象,男性2 059(48.8%)人、女性2 163(51.2%)人,MS组分数≥3项者570(13.5%)例。性别对基线阶段发现MS具有统计学意义(P<0.01)。受教育程度、婚姻状态、吸烟、喝酒对患MS及其组分疾病的影响有统计学意义(均P<0.05)。有慢性病史对研究对象患MS的影响有统计学意义(P<0.01),高血压、血脂异常、糖尿病、心脏病、慢性肺部疾病、消化系统疾病是有明显差异的影响因素(均P<0.05)(表1)。将性别纳入logistic回归并调整相关协变量,发现女性更易发生MS(OR=1.196,95%CI: 1.002~1.428,P=0.048)。
在随访期间,研究对象新发抑郁状态2 407例,发生率为57.0%,新发抑郁状态者中患MS组分数≥1者2 082例,其中370例为随访新增。不同MS组分数产生抑郁状态风险不同,调整基本人口学特征以及健康状态协变量后发现MS组分数≥2时,对抑郁状态的产生有统计学差异(均P<0.01),当组分数≥3(即可确诊为患MS)时风险增加超过2倍(MS组分数为3,模型2 OR=3.184,95%CI: 2.484~4.081,P<0.01;MS组分数为4,模型2 OR=2.749,95%CI: 2.075~3.641,P<0.01;MS组分数为5,模型2 OR=2.594,95%CI: 1.784~3.771,P<0.01),表明MS可以较好地预测抑郁状态发生(表2)。
阶段Ⅱ纳入5 203例研究对象,男性2 474(47.5%)人、女性2 729(52.5%)人,抑郁组1 551(29.8%)例。性别对基线抑郁状态有明显影响(P<0.01)。受教育程度、婚姻状态、吸烟、喝酒的影响有统计学差异(均P<0.05)。抑郁状态者高血压、高血脂、心脏病、慢性肺部疾病、消化系统疾病、情感及精神疾病发病率均高于非抑郁状态者(均P<0.05)(表1)。将性别纳入logistic回归并调整相关协变量,发现抑郁状态更易在女性中产生(OR=1.279,95%CI: 1.135~1.441,P<0.01)。
随访中新发MS 1 490例,发生率为28.6%,新发MS者中抑郁状态者885例,其中198为随访新增。抑郁组患MS风险高于非抑郁组(原模型OR=2.821,95%CI: 2.484~3.204,P<0.01)。当控制协变量时抑郁组MS患病风险均比非抑郁组增加近3倍(模型1 OR=2.805,95%CI: 2.468~3.189,P<0.01;模型2 OR=2.737,95%CI: 2.392~3.132,P<0.01)(表2)。
阶段Ⅲ纳入6 285例研究对象,其中男性2 878(45.8%)人、女性3 407(54.2%)人;随访MS 2 448例,发生率为38.9%;随访抑郁状态4 069例,发生率为64.7%。构建交叉滞后面板模型,模型比较拟合指数(CFI)>0.90,Tucker-Lewis指数(TLI)>0.90,近似误差均方根(RMSEA)<0.08,标准化均方根残差(SRMR)<0.08,P<0.05,模型拟合效果较佳,有统计学意义。
以年龄、性别、受教育程度、婚姻状态、吸烟、喝酒、慢性病史为协变量,由图2所示模型A显示基线MS组分至随访抑郁状态、基线抑郁状态到随访MS组分的路径系数均具有统计学意义(β=0.052、β=0.305,P<0.01);模型B、C在性别层面进行亚组分析均存在双向关联(男性亚组β=0.034、β=0.191,P<0.01;女性亚组β=0.063、β=0.404,P<0.01)。
同时研究MS中血压升高、中心性肥胖、高血糖、TG升高、HDL-C水平低五组分分别与抑郁状态的双向关系,模型A调整所有协变量后发现各组分均与抑郁状态有双向关联作用(血压升高β=0.037、β=0.048,P<0.01;中心性肥胖β=0.040、β=0.063,P<0.01;高血糖β=0.087、β=0.069,P<0.01;TG升高β=0.117、β=0.137,P<0.01;HDL-C水平低β=0.053、β=0.082,P<0.01)。模型B、C发现男性中心性肥胖与抑郁状态的双向作用有相关性(β=0.055、β=0.191,P<0.05),女性血压升高(β=0.055、β=0.048,P<0.01)、高血糖(β=0.114、β=0.091,P<0.01)、TG升高(β=0.141、β=0.179,P<0.01)、HDL-C水平低(β=0.072、β=0.111,P<0.01)组分与抑郁状态存在明显双向关联(表3)。
本研究用CHARLS数据库进行疾病影响因素研究及疾病风险预测,得中国中老年群体MS及其组分和抑郁状态存在双向联系,即基线MS可能导致抑郁状态发生,基线抑郁状态也会增加MS或其组分发病风险。本研究发现女性中老年人患MS和抑郁的可能性大于男性,有研究证明MS是女性抑郁的危险因素之一[7],女性生理健康的变化也更易导致抑郁情绪[8]。受教育程度较低是MS和抑郁状态发生原因之一,一项研究表明高等教育对抑郁发生有保护作用[9]。吸烟、喝酒及消化道疾病、慢性肺部疾病等对这两类疾病的产生也有一定影响,这与很多研究结果一致[10-12],不良的生活习惯和慢性病史在心理、激素等各水平上导致抑郁或MS的发生。
Pan等人在一些横断面和前瞻性队列研究中发现抑郁和MS存在显著的双向关联作用[13],这与本研究的结论一致。在阶段Ⅰ建立的logistic模型以及调整潜在混杂因素模型中,均显示基线时期患MS者发生抑郁状态风险比无MS组分者增加2-3倍。在阶段Ⅱ建立的模型中基线抑郁状态者发生MS的风险约为非抑郁者的2.8倍。在阶段Ⅲ建立的交叉滞后面板模型中,两疾病随时间推移保持相对稳定且相互影响,因发现性别对MS和抑郁状态有显著预测作用,所以按性别进行亚组分析,MS与抑郁状态在性别水平上仍存在相互影响。
抑郁症和MS的相互作用可能受多种机制介导。抑郁状态可能使人产生不良生活方式和行为,如吸烟酗酒、拒绝运动或不遵医嘱[14],导致患MS风险增加;MS和抑郁症有相互联系的病理生理机制,抑郁症的神经内分泌效应易致使自主神经系统紊乱和下丘脑-垂体-肾上腺(HPA)轴失调,导致葡萄糖代谢失调、腹部脂肪堆积和血压升高[15],引起中心性肥胖;服用传统的抗抑郁药可能对身体代谢水平有直接副作用[16],会增加高血压和肥胖的风险等。
本文还发现MS各组分和抑郁状态在性别层面上有特异性差异。中老年男性抑郁状态对血压升高、高血糖、TG升高、HDL-C水平低仅存在单向影响,中老年女性抑郁状态对中心性肥胖也仅存在单向影响,反之预测能力不明显。Virtanen等[17]研究认为MS各组分不能独立预测抑郁发生风险,因此还需更多研究来探索性别或其他重要因素的潜在机制。
对此我们应该采取积极的防治措施,对中老年人开展疾病早期识别与有效干预。应在中老年人群体中构建健康信念模型,进行健康促进和健康教育,积极提倡通过吸烟戒断、健康饮食与适当运动改变不良生活方式[18];加强中老年人慢性病管理,通过动员个体、家庭、社区医疗服务机构等方式督促其定期进行健康筛查,督促指标异常者按时服药或治疗[19];此外,鼓励中老年人参与交际活动,增强情绪调节能力,必要时进行心理疏导,从生理和心理两个维度改善晚年生活质量。
本研究仍存在局限性:问卷中慢性病及治疗史虽有医疗信息支撑,但可能有统计缺漏或未检出的疾病,存在信息偏倚;可能存在数据库中未提及的潜在混杂因素,如身体活动、激素或细胞炎症因子分泌等影响研究结果;MS和抑郁可能长期发展,数据库随访期较短,还需进一步跟踪随访数据。
综上所述,中国中老年群体MS及其组分与抑郁状态的发生存在密切双向联系。因此我们需探索中老年人早期MS识别、监控及干预策略,通过普及健康筛查进行早诊断、早治疗;同时也应高度重视中老年人身心健康以及抑郁症人群的MS防治,采取健康促进行动,通过科学干预全面改善中老年人的身心健康和生命质量。
