Objective

To understand the utilization of mental health education services and its influencing factors among the elderly floating population in China, and to provide reference for the effective development of mental health education for the elderly.

Methods

The data of 5 589 elderly migrants in the 2018 National Floating Population Health and Family Planning Dynamic Monitoring Questionnaire were analyzed, and the Anderson model was used as the theoretical framework, and the main analysis methods included statistical description, χ2 test and binary logistic regression analysis.

Results

The utilization rate of mental health education services for the elderly floating population in China was 16.84%, and the methods which wereused were mainly publicity materials, publicity columns, knowledge lectures and public health consultation activities, and the logistic regression results showed that the education level was college degree or above (OR=1.229, 95%CI: 0.901-1.676), the flow range was inter-city in the province (OR=1.219, 95%CI: 1.047-1.593), and the inflow area was Northeast China (OR=1.419, 95%CI: 1.159-1.737), central region (OR=1.980, 95%CI: 1.582-2.478) were the protective factors for the use of mental health education services in the elderly floating population, the migration duration was 10-20 years (OR=0.689, 95%CI: 0.521-0.913), the health records were not established and unknown (OR=0.473, 95%CI: 0.333-0.672), Unclear health record establishment (OR=0.683, 95%CI: 0.473-0.986), uncontracted and uninformed family contracted doctors (OR=0.537, 95%CI: 0.37-0.767), and unclear family doctor contract (OR=0.597, 95%CI: 0.423-0.844) were risk factors for the utilization of mental health education services in the elderly migrant population.

Conclusion

The utilization rate of mental health education services for the elderly floating population needs to be improved, the awareness of mental health of the elderly floating population should be strengthened, the mental health publicity of the elderly floating population should be increased, and the elderly floating population should be guided to actively receive mental health education.

目的

了解我国老年流动人口对心理健康教育服务利用状况及其影响因素，为老年人心理健康教育工作有效开展提供参考。

方法

对2018年全国流动人口卫生计生动态监测问卷中5 589名老年流动人口相关数据进行分析，以安德森模型为理论框架，主要分析方法有统计描述、χ2检验和二元logistic回归分析等。

结果

我国老年流动人口心理健康教育服务利用率为16.84%，利用方式主要是宣传资料、宣传栏、知识讲座和公众健康咨询活动；logistic回归结果显示，学历为大专及以上（OR=1.229,95％CI:0.901～1.676）、流动范围为省内跨市（OR=1.219,95％CI:1.047～1.593）、流入区域为东北地区（OR=1.419,95％CI:1.159～1.737）、中部地区（OR=1.980,95％CI:1.582～2.478）是老年流动人口心理健康教育服务利用的保护因素；流动时长为10～20年（OR=0.689,95％CI:0.521～0.913）、未建立、不知晓健康档案（OR=0.473,95％CI:0.333～0.672）、不清楚健康档案建立情况（OR=0.683,95％CI:0.473～0.986）、未签约、不知晓家庭签约医生情况（OR=0.537,95％CI:0.376～0.767）、不清楚家庭医生签约情况（OR=0.597,95％CI:0.423～0.844）是老年流动人口心理健康教育服务利用的危险因素。

结论

老年流动人口心理健康教育服务利用率有待提高，应强化老年流动人口心理健康意识，加大老年流动人口心理健康宣传力度，引导老年流动人口积极接受心理健康教育。

目的

了解我国老年流动人口对心理健康教育服务利用状况及其影响因素，为老年人心理健康教育工作有效开展提供参考。

方法

对2018年全国流动人口卫生计生动态监测问卷中5 589名老年流动人口相关数据进行分析，以安德森模型为理论框架，主要分析方法有统计描述、χ2检验和二元logistic回归分析等。

结果

我国老年流动人口心理健康教育服务利用率为16.84%，利用方式主要是宣传资料、宣传栏、知识讲座和公众健康咨询活动；logistic回归结果显示，学历为大专及以上（OR=1.229,95％CI:0.901～1.676）、流动范围为省内跨市（OR=1.219,95％CI:1.047～1.593）、流入区域为东北地区（OR=1.419,95％CI:1.159～1.737）、中部地区（OR=1.980,95％CI:1.582～2.478）是老年流动人口心理健康教育服务利用的保护因素；流动时长为10～20年（OR=0.689,95％CI:0.521～0.913）、未建立、不知晓健康档案（OR=0.473,95％CI:0.333～0.672）、不清楚健康档案建立情况（OR=0.683,95％CI:0.473～0.986）、未签约、不知晓家庭签约医生情况（OR=0.537,95％CI:0.376～0.767）、不清楚家庭医生签约情况（OR=0.597,95％CI:0.423～0.844）是老年流动人口心理健康教育服务利用的危险因素。

结论

老年流动人口心理健康教育服务利用率有待提高，应强化老年流动人口心理健康意识，加大老年流动人口心理健康宣传力度，引导老年流动人口积极接受心理健康教育。

Objective

To understand the utilization of mental health education services and its influencing factors among the elderly floating population in China, and to provide reference for the effective development of mental health education for the elderly.

Methods

The data of 5 589 elderly migrants in the 2018 National Floating Population Health and Family Planning Dynamic Monitoring Questionnaire were analyzed, and the Anderson model was used as the theoretical framework, and the main analysis methods included statistical description, χ2 test and binary logistic regression analysis.

Results

The utilization rate of mental health education services for the elderly floating population in China was 16.84%, and the methods which wereused were mainly publicity materials, publicity columns, knowledge lectures and public health consultation activities, and the logistic regression results showed that the education level was college degree or above (OR=1.229, 95%CI: 0.901-1.676), the flow range was inter-city in the province (OR=1.219, 95%CI: 1.047-1.593), and the inflow area was Northeast China (OR=1.419, 95%CI: 1.159-1.737), central region (OR=1.980, 95%CI: 1.582-2.478) were the protective factors for the use of mental health education services in the elderly floating population, the migration duration was 10-20 years (OR=0.689, 95%CI: 0.521-0.913), the health records were not established and unknown (OR=0.473, 95%CI: 0.333-0.672), Unclear health record establishment (OR=0.683, 95%CI: 0.473-0.986), uncontracted and uninformed family contracted doctors (OR=0.537, 95%CI: 0.37-0.767), and unclear family doctor contract (OR=0.597, 95%CI: 0.423-0.844) were risk factors for the utilization of mental health education services in the elderly migrant population.

Conclusion

The utilization rate of mental health education services for the elderly floating population needs to be improved, the awareness of mental health of the elderly floating population should be strengthened, the mental health publicity of the elderly floating population should be increased, and the elderly floating population should be guided to actively receive mental health education.

《中国流动人口发展报告（2018）》显示，2018年我国老年流动人口占比由2015年的5.30%上升至8.01%，数量已达1 778.4万[1]，我国老年流动人口规模发展迅速。《“健康中国2030”规划纲要》强调：要加强健康教育，提升心理健康素养，早发现重点人群心理问题，及时加大干预力度，而老龄化、流动性等高危特征使老年流动人口面临更严峻的心理健康风险[2]。因此，针对流动老年人口心理健康教育至关重要。目前，关于老年流动人口的研究主要集中于社会融合[3]、健康状况[4]、慢性病[5]等方面，对其心理健康教育的研究目前相对较少。对此，本文以Anderson模型为理论框架，对我国老年流动人口心理健康教育服务利用现况及其影响因素进行研究，为改善老年流动人口心理健康服务工作和提高老年人口心理健康素养提供参考。
1　资料与方法

1.1　资料来源
本研究采用2018年全国流动人口卫生计生动态监测调查流动人口问卷（A），问卷具有一定的信度和效度。调查对象为在流入地居住1个月以上、非本区（县、市）户口、年龄在15周岁及以上的152 000人。本研究选取60岁以上老年流动人口共6 230人，由于心理健康教育服务面向6个月以上的流动人口进行，因此剔除在流入地居住6个月以下者280人，去除缺失样本后最终共纳入5 589人。
1.2　理论框架
Anderson模型是研究卫生服务利用行为的重要理论模型，被广泛应用于医疗卫生服务研究领域，最新修正的Anderson模型包含环境因素、个人特征、健康行为和健康结果四个维度[6]。本研究以Anderson模型为理论框架，根据流动老年人口特征，并结合相关文献[7-8]，从倾向、使能和需求三个维度构建我国老年流动人口心理健康教育服务利用行为影响因素分析框架。具体分类情况见图1。
1.3　统计学方法
应用SPSS 27.0进行数据的统计及分析。运用频数和构成比描述老年流动人口基本情况及心理健康教育服务利用状况，使用χ2检验进行单因素分析，以安德森模型为理论框架，采用logistic回归模型进行多因素分析，将倾向因素作为控制变量纳入模型1，模型2、模型3分别在模型1的基础上纳入使能因素和需求因素，模型4同时纳入倾向因素、使能因素和需求因素，检验水准α=0.05，具体如下：

2　结果

2.1　老年流动人口基本情况
5 589名老年流动人口中，年龄集中分布在60~69岁，民族、政治面貌和婚姻状况多为汉族、群众、在婚。其中，男性占比57.3%；受教育程度在小学及以下者占比45.9%；跨省流动者占比44.9%；流动时长<10年者占比65.5%；流入东部地区者占比33.1%；已建立健康档案者占比32.4%；已签约家庭医生者占比17.6%；家庭月均收入在1 000~10 000元者占比83.1%；自评健康为健康者占比46.5%；近一年患病或不适者占比29.4%。
2.2　老年流动人口心理健康教育服务利用状况影响因素分析
2.2.1　老年流动人口心理健康教育服务利用状况单因素分析
据统计，研究对象中仅941人（16.8%）接受过心理健康教育，4 648人（83.2%）未接受过心理健康教育，其接受心理健康教育途径主要包括其他方式、个体化面对面咨询、短信、公众健康咨询活动、宣传栏、宣传资料或知识讲座，占比分别是22.4%、29.9%、37.1%、51.4%、71.6%、75.7%、80.3%。
单因素分析结果显示，不同流动范围、流动时长、流入区域、政治面貌、受教育程度、健康档案建立情况、家庭医生签约情况、家庭月均收入、自评健康状况、近一年患病或不适情况的老年流动人口对心理健康教育服务利用状况不同，差异具有统计学意义（P<0.05）。见表1。
2.2.2　老年流动人口心理健康教育服务利用状况多因素分析
构建基于Anderson模型的二分类logistic回归模型，各变量赋值情况见表2。进一步分析倾向因素、使能因素、需求因素对老年流动人口心理健康教育服务利用的影响，详见表3。
经拟合优度测量，模型4的R2值最大（Cox & Snell R2=0.074，Nagelkerke R2=0.124），表示模型4对于老年流动人口心理健康教育服务利用的解释度最高，故最终选择模型4来解释老年流动人口心理健康教育服务利用的影响因素。
3　讨论

心理健康教育有助于心理健康知识和技能的传授，是提高心理健康素养的重要手段[9]。研究结果显示，我国老年流动人口心理健康教育利用率较低仅16.8%，与《“十三五”全国流动人口卫生计生服务管理规划》中提到的流动人口健康教育覆盖率要达到95.0%以上相比，仍有较大差距。此外，结果显示：健康教育宣传方式较传统，多为宣传栏/电子显示屏以及宣传资料等，而健康讲座和面对面咨询等更易老年人接受的方式较少[2]，需针对老年流动人口这一人群特征实行心理健康教育宣传工作。
回归分析结果显示，我国老年流动人口心理健康服务利用状况受多种因素影响。学历较高的老年流动人口服务利用率较高，学历水平能够客观反映个人获取健康信息、学习健康知识和掌握健康技能的能力[10]。月收入水平对老年流动人口心理健康服务利用状况具有促进作用，但随着月收入的增加利用率下降，可能是由于月收入较高的老年流动人口因担心公开谈论心理困扰或寻求心理健康服务会影响自身社会形象，从而减少对该服务的利用，与黄倩雯等[11]研究结果一致。此外，自评健康状况较差的老年流动人口心理健康教育服务利用率较高，这是因为该人群面临更多生活压力、情绪困扰和社会孤立等问题[12]，接受服务的意愿更强。老年流动人口心理健康教育服务利用状况与流动特征关系密切。从流动范围来看，跨省流动的老年人心理健康教育服务利用率较低，可能是因为流动范围大影响服务利用稳定性，从而导致流动老年人口对流入地医疗服务资源依赖性弱的现象；从流动时间来看，流动时长与心理健康教育服务利用率呈反比，可能是老年流动人口在流动初期面临陌生环境适应能力较弱，产生的不适感对心理健康造成负面影响[13]。但其对居住环境的熟悉程度随居住时间增加而升高，心理健康教育需求相对降低。从流动地区来看，流入中、西、东北地区的老年人服务利用率较高，这可能与东部地区流动人口人数较多且较分散，社会融合较差有关，也可能与国家加大对经济发展水平较低地区基本公共卫生服务补贴力度有关[14]。
拟合优度检验结果显示，使能因素对老年流动人口心理健康教育服务利用影响最大（Cox & Snell R2=0.070，Nagelkerke R2=0.117），这表明健康档案建立和家庭医生签约是影响老年流动人口服务利用的主要因素。已建立健康档案的流动老年人心理健康意识较强，是由于健康档案能够动态把握老年流动人口心理健康状况，对老年流动人口心理健康教育具有促进作用[15]。此外，家庭医生签约服务对老年流动人口心理健康服务利用也具有正向推动作用，家庭医生定期问诊有助于及时发现和干预心理健康风险因素，提升家庭医生签约率是促进这一人群更好地享受基本公共卫生服务的重要一环[16]。
综上所述，我国老年流动人口心理健康教育服务利用率整体较低，学历水平、流动特征及健康档案建立情况等因素影响利用状况。国外相关研究指出[17-18]，老年流动群体心理健康问题影响因素多为经济收入、社会文化水平等非疾病因素，这与本研究较为一致。为改善国内老年流动人口心理健康状况，应在充分借鉴国内外经验的基础上探索心理健康素养提升策略。首先，国外学者多从文化适应角度对心理健康进行探究，国内未来可多关注地域文化差异，从文化融入新视角进行相关研究。此外，国内学者认可社会支持对解决心理健康问题具有重要意义[19]，与国外社会支持相关研究较为一致，但尚缺乏实践经验，未来应从社区层面试行干预计划，探索可行的社会参与形式，如开展相关心理健康促进活动，通过招募有心理学背景的志愿者为流动老年人口提供心理疏导，打破流动老年人口心理援助壁垒；重点关注社区健康档案、家庭医生签约服务等基本公共卫生服务项目。最后，政府及卫生部门应积极构建老年流动人口社会保障体系，加大健康资源向老年流动群体的倾斜力度；重点关注学历不高、自评健康状况较差、流动范围较广、流动时长较短、流入区域为欠发达地区的老年流动人口心理健康问题；创新健康教育宣传方式、扩展宣传途径，提升老年流动人口心理健康相关知识的认知水平。
本研究使用2018年CMDS横截面数据，数据具有一定滞后性，且影响因素分析过程中纳入变量仅为调查问卷涉及的相关问题，存在一定局限性，以上不足之处今后有待深入研究。
