Objective

To evaluate the applicability of the Job Content Questionnaire (JCQ-22) based on classical test theory (CTT) and item response theory, and to optimize and adjust it.

Methods

A survey of 917 occupational groups who had a physical examination at a hospital from September 1 to September 30, 2021 was conducted using JCQ-22, and the entries were comprehensively analyzed using the Rasch model, two-parameter logistic model (2PL), and grade-response model (GRM) in the IRT, combined with Cronbach’s α coefficient within CTT, and the factor analysis method.

Results

In the IRT analysis, the difficulty coefficients of the entries ranged from -2.49 to 0.36, and the difficulty coefficients of “JOB4”, “JOB7”, “JOB10”,“JOB11” had a differentiation degree lower than 1.34, and the corresponding average information content was significantly lower than the other entries. In the CTT analysis, a total of 18 entries met the statistical criteria, therefore, combining the IRT and CTT, the JCQ-22 was optimized and adjusted, and the results showed that the adjusted scale JCQ-18 had 18 entries, which was more concise and had a more stable structure.

Conclusion

Most of the entries of JCQ-22 performed well, while some of the entries did not meet the acceptable standards and need further optimization.

目的

基于经典测量理论和项目反应理论对工作内容量表(JCQ-22)进行信效度及条目评价,并在此基础上进行优化策略分析。

方法

采用JCQ-22对2021年9月1—30日在某医院进行体检的917名职业人群进行调查,利用IRT中的Rasch模型、双参数logistic模型(2PL)及等级反应模型(GRM),并结合CTT中的Cronbachα系数、因子分析法对条目进行综合分析。

结果

在IRT分析中,各条目难度系数范围为-2.49~0.36,JOB4、JOB7、JOB10、JOB11的区分度均低于1.34,且相应的平均信息量也显著低于其他条目,在CTT分析中,共有18个条目符合统计标准,因此,结合IRT和CTT,对JCQ-22进行优化调整,结果显示调整后的量表JCQ-18拥有18个条目,更简洁且结构更加稳定。

结论

JCQ-22的大部分条目性能良好,部分条目未达到可接受标准,需进一步优化。

目的

基于经典测量理论和项目反应理论对工作内容量表(JCQ-22)进行信效度及条目评价,并在此基础上进行优化策略分析。

方法

采用JCQ-22对2021年9月1—30日在某医院进行体检的917名职业人群进行调查,利用IRT中的Rasch模型、双参数logistic模型(2PL)及等级反应模型(GRM),并结合CTT中的Cronbachα系数、因子分析法对条目进行综合分析。

结果

在IRT分析中,各条目难度系数范围为-2.49~0.36,JOB4、JOB7、JOB10、JOB11的区分度均低于1.34,且相应的平均信息量也显著低于其他条目,在CTT分析中,共有18个条目符合统计标准,因此,结合IRT和CTT,对JCQ-22进行优化调整,结果显示调整后的量表JCQ-18拥有18个条目,更简洁且结构更加稳定。

结论

JCQ-22的大部分条目性能良好,部分条目未达到可接受标准,需进一步优化。

Objective

To evaluate the applicability of the Job Content Questionnaire (JCQ-22) based on classical test theory (CTT) and item response theory, and to optimize and adjust it.

Methods

A survey of 917 occupational groups who had a physical examination at a hospital from September 1 to September 30, 2021 was conducted using JCQ-22, and the entries were comprehensively analyzed using the Rasch model, two-parameter logistic model (2PL), and grade-response model (GRM) in the IRT, combined with Cronbach’s α coefficient within CTT, and the factor analysis method.

Results

In the IRT analysis, the difficulty coefficients of the entries ranged from -2.49 to 0.36, and the difficulty coefficients of “JOB4”, “JOB7”, “JOB10”,“JOB11” had a differentiation degree lower than 1.34, and the corresponding average information content was significantly lower than the other entries. In the CTT analysis, a total of 18 entries met the statistical criteria, therefore, combining the IRT and CTT, the JCQ-22 was optimized and adjusted, and the results showed that the adjusted scale JCQ-18 had 18 entries, which was more concise and had a more stable structure.

Conclusion

Most of the entries of JCQ-22 performed well, while some of the entries did not meet the acceptable standards and need further optimization.

职业紧张是指个体在某种职业环境下,客观需求与个人适应能力出现失衡,导致的生理心理压力。我国《健康中国行动(2019—2030年)》第九项职业健康保护行动明确指出,由疾病和工作压力引发的生理和心理问题已成为亟待应对的职业健康新挑战[1]。准确评估劳动者的职业紧张状况,对于预防和促进我国职业人群的心理健康具有重要意义。职业紧张量表作为职业紧张研究、监测、评估及干预的基础工具,在国际上仍占据主导地位[2]。其中,基于“工作要求-自主程度-社会支持模型”设计的工作内容量表(job content questionnaire,JCQ)被广泛认为是测量职业紧张的有效工具之一[3]。JCQ已被翻译成多种语言版本,工作内容量表中文版本(JCQ-22)已在不同职业人群中得到广泛应用[4-10]。然而,JCQ-22在对其进行跨文化适用性及跨行业职业群体验证时,存在一定的信效度波动[11-12]。目前对于JCQ-22的适用性评价多是基于经典测量理论进行的,鲜有基于项目反应理论进行,而项目反应理论的优势在于参数估计方面的精确性,对测量维度的估计不受样本量的限制,在一定程度上弥补了经典测试理论的不足[13]。因此,本研究拟同时采用经典测量理论(classical test theory,CTT)和项目反应理论(item response theory,IRT)对JCQ-22进行评价与优化,旨在为提高量表的准确性和稳健性提供科学依据。
本研究将采用工作内容量表(JCQ-22)对不同职业群体开展测试,基于量表测试数据,运用CTT与IRT评价量表的信效度及条目反应特征分析。
2021年9月1—30日至某职业病防治三甲医院体检的全部符合条件的职业体检者为调查对象。纳入标准:年龄≥18岁;目前在岗时间≥1年。排除标准:存在精神疾病或认知缺陷;调查前一周内有精神类药物服用史;已退休人员;拒绝参加研究或配合度较低者。本次研究经四川大学华西公共卫生学院医学伦理委员会审查批准(No. Gwll2024120),研究对象均知情同意且自愿参与调查。
由具有相关研究背景的专业人员组成调查员队伍在2021年9月至某职业病防治三甲医院采用电子问卷进行现场调查,问卷均在调查员指导下,由调查对象自行填写。
主要调查内容及工具:(1)基本情况调查:采用自行设计的调查问卷,内容包括调查对象基本人口学特征(性别、年龄、身高、体重、教育程度、婚姻状况和生活方式等)、职业特征(包括工作类型、职位和职业危害暴露等)。(2)职业紧张状况调查:采用JCQ-22[14],JCQ-22中文版由工作要求、工作自主和社会支持三个维度组成,共22个条目。工作要求(psychological demand)由5个条目组成;工作自主(decision latitude)由技能自主(skill discretion) 6个条目和决策自主(decision authority)3个条目2个模块共9个条目组成;社会支持(support)是由上司支持(supervisor support) 4个条目和同事支持(coworker support)4个条目2个模块共8个条目组成。量表采用Likert 4级评分法进行赋值,以1~4分计,1表示非常不同意,4表示非常同意,各维度总得分为维度的条目得分之和。工作要求和工作自主的比值则为职业紧张评分,评分越高,则职业紧张程度越高。
调查开始前对调查员进行统一培训及考核,确保清楚研究目的,熟悉调查问卷内容。向调查对象详细说明本次调查的的必要细节,包括填写问卷的规范与要求、调查的形式、目的及内容等,以充分获得调查对象的理解与信任,保证问卷的填写质量。问卷提交后由调查员当场审核,剔除无效问卷,保证数据的准确性和有效性。
(1)效度分析:包括结构效度和内容效度。采用探索性因子分析(exploratory factor analysis,EFA)和验证性因子分析(confirmatory factor analysis,CFA)评价量表的结构效度。对量表数据进行KMO检验和Bartlett球形检验,若KMO值>0.7,Bartlett球形检验P<0.05,说明数据适合进行EFA。采用主成分分析法提取因子,结合最大方差法旋转,保留因子载荷≥0.4的条目[15],确保因子结构与理论维度一致,从而验证量表的结构效度。通过计算条目-总分相关系数分析各条目与量表总分的相关性,若相关系数>0.5则表明内容效度良好[16]。通过计算平均方差抽取量(average variance extracted,AVE)评估量表的收敛效度,AVE值≥0.5表明量表具有良好的收敛效度。采用CFA对量表结构调整前后的模型进行对比,通过关键拟合指标评估优化效果。具体选取比较拟合指数(comparative fit index,CFI)和Tucker-Lewis指数(tucker-lewis index,TLI)、近似均方根误差(root mean square error of approximation,RMSEA)、标准化均方根残差(standardized root mean square,SRMR)等绝对/相对拟合指标。若调整后模型的CFI、TLI值趋近于0.90、0.95的理想阈值,RMSEA、SRMR值低于0.08、0.06,则表明量表结构得到有效优化。
(2)信度分析:通过计算总量表、各维度的克朗巴赫系数(Cronbach α)、G6(smc)(平均平方多重相关系数)及组合信度(composite reliability,CR)评估JCQ-22的内部一致性[17],Cronbach α系数和G6(smc)通常在0~1之间,数值越高,表明量表的内部一致性和可靠性越高。Cronbach α系数和G6(smc)判断标准为:α≥0.8表示信度优秀,0.6≤α<0.8为良好,α<0.6则提示内部信度不足,需要修订[18-19]CR值≥0.6表示信度良好。
本研究基于IRT框架,采用Rasch模型、双参数logistic模型(2PL)及等级反应模型(GRM)综合分析JCQ-22量表条目性能。首先通过Rasch模型估计条目难度参数(a1)及个体潜在特质水平,并计算内部拟合指数(infit)和外部拟合指数(outfit)评估条目与模型的匹配度(理想范围0.5~1.5),超出范围则提示条目异常反应模式[20-21]。同时,基于2PL和GRM模型计算条目区分度参数(a),依据标准(≤0.64为低,0.65~1.34为可接受,>1.34为优)[22],评估条目对潜在特质的鉴别能力,并结合信息量分析识别测量精度不足的条目。最后,对于区分度过低(如≤0.64)或拟合指数异常(如infit/outfit超限)的条目,进一步结合CTT分析结果,剔除多方法表现不佳的条目,优化量表结构与测量效能。
由双人核查数据后,应用Excel 2016录入整理,使用R 4.3.1软件对数据进行统计描述、因子分析、效度分析和信度分析及IRT框架下的Rasch模型、2PL模型及GRM模型分析。符合正态分布的计量资料采用()描述,不同组间差异比较采用独立样本t检验或单因素方差分析,所有分析均采用双侧检验,检验水准α=0.05。
本研究共纳入917名研究对象,其中男性722人(78.7%),女性195人(21.3%)。年龄介于18~65岁,平均年龄为(32.92±8.56)岁,JCQ-22三个维度得分在不同教育程度、不同薪酬间均存在显著差异(P<0.05),社会支持得分在不同性别、不同年龄组及不同饮酒频率组间也存在显著差异(P<0.05)。见表1
JCQ-22的KMO值为0.94,Bartlett球形检验显示差异有统计学意义(χ2=11 519.87,P<0.001),表明数据适合进行EFA。采用主成分分析经最大方差正交旋转,因子载荷值范围为0.25~0.91,结果显示其提取的5个因子与理论预设维度基本一致。进一步发现,条目JOB4、JOB7、JOB10及JOB11的因子载荷均低于0.4,结构效度不足,需进行针对性修订。条目-维度相关性分析显示,JOB4、JOB7、JOB9、JOB10和JOB11相关系数较低,内容效度较差。另外,AVE值结果提示,工作要求和技能自主维度收敛效度较其他维度低,需进行优化。见表2
各维度及其指标Cronbach α系数分别为工作要求0.74、技能自主0.70、决策自主0.79、上司支持0.91及同事支持0.92,其中上司支持和同事支持维度信度最优,技能自主维度信度相对较低但仍达可接受水平。G6(smc)指标及CR值显示,上司支持与同事支持维度的条目内部一致性较强,技能自主维度较弱,但整体信度良好。见表3
基于IRT的Rasch模型分析表明,PSD维度条目难度系数(a1)范围为-1.97~0.36,其中JOB4条目的外部拟合指数(outfit=2.06)超出合理范围,内部拟合指数(infit=1.45)接近临界值,提示拟合不佳;技能自主维度条目难度系数范围为-1.94~0.35,JOB11的infit(1.20)与outfit(1.45)高于同维度其他条目,进一步提示需要优化。决策自主、上司支持及同事支持维度的条目难度范围分别为-0.74~-0.45、-1.57~-0.70、-2.49~-2.03,且内外部拟合指数均符合标准。进一步通过2-PL及GRM评估条目区分度,结果显示JOB4、JOB7、JOB9、JOB11的区分度均低于1.34,提示其对潜在特质的区分能力不足。见表4
基于CTT与IRT对量表条目进行综合分析。在CTT框架下,删除条目后Cronbach α系数变化显示:工作要求维度中,条目JOB4删除后该维度Cronbach α系数从0.74升至0.76;技能自主维度中,条目JOB11删除后Cronbach α系数由0.70提升至0.71,其余条目性能良好。
基于因子分析结果,本研究确认条目JOB4、JOB7、JOB9、JOB10及JOB11在结构效度(因子载荷≤0.4)与内容效度(条目-维度相关性r<0.65)表现较差,但JOB10在区分度和量表整体相关性方面表现良好,而JOB9表现较差,此结果与既往研究一致。调整后形成的JCQ-18量表保留18个条目,涵盖工作要求(4个条目)、技能自主(3个条目)、决策自主(3个维度)、上司支持(3个条目)及同事支持(3个条目)5个维度。通过CFA对比调整前后量表结构,结果显示:JCQ-18的模型拟合指数均显著优于原量表,且各条目标准化因子载荷均>0.6,优化后的量表工作自主和技能自主维度的CR值分别提高至0.85和0.83,AVE值分别提高至0.59和0.62,表明优化后量表结构更稳定,信效度也得到了一定的提升,见表5
工作内容量表(JCQ-22)作为评估职业压力最常用的工具之一,已在众多国家得到了广泛应用。然而,在具体执行JCQ-22时,由于语言和文化的差异,其稳定性会受到影响。本研究采用多种方法对JCQ-22的信效度进行了调查,采用因子分析对量表的整体结构进行了评估,并采用CTT和IRT分析对量表的内部一致性和条目质量进行了测试,以期为该量表后期的使用和优化提供依据。结合CTT和IRT分析结果,最终22个条目里,有18个条目的性能良好,将未达到可接受标准的4个条目进行剔除,优化后的JCQ-18模型拟合指数优于原量表,且更加简洁精确。本研究结果表明,JCQ-22在5个维度上均表现出较高的内部一致性。在维度层面,工作要求和技能自主在内部一致性和结构稳定性方面相比其他维度表现较差,这些结论已在相关研究中得到证实[23-25]。在条目层面,JOB10虽因子载荷不符合标准,但区分度和与量表整体的相关性方面表现良好,因此予以保留。而JOB4、JOB7、JOB9和JOB11四个条目均未达到可接受标准,且基于IRT分析所得的infit和outfit值,我们可以筛选出JOB4和JOB11为较差的条目,因此予以剔除。这与以往研究结果一致,JOB4、JOB7、JOB9和JOB11等项目传统上被认为是“较差的项目”,无法有效衡量预期的潜在变量[26-28]。例如,Chien等人[26]使用相同的中文版JCQ-22量表时,量表的Cronbach α系数接近0.8,且JOB4是最简单的项目,而JOB5是最具挑战性的项目。而在本研究中,JOB5同样是最难的项目,而JOB22是最简单的项目。此外,与早期研究相比[28-29],本研究调查结果中社会支持项目的复杂性较低,这可能与本研究调查对象涵盖多种职业群体有关。
CTT和IRT是依据不同的理论假设,计算方法各不相同。CTT是将被试者总体与具体条目属性相结合,体现了宏观层面的信息,IRT将看不见的被试者潜在特质与具体条目属性相结合,体现了微观层面的信息[30]。CTT存在测验结果拓展的有效性、统计量的样本依赖性、误差含糊和信度估计的不精确性、能力与难度量尺的不一致性等局限性。而IRT可以有效弥补CTT的不足,且两者可结合用于量表的条目分析,有效提升量表的应用效果。
本研究使用了传统的CTT方法结合IRT,使我们能够从多个角度全面评估条目的表现,从而增加了研究的可靠性。同时,本研究存在一定的局限性:研究对象均来自于医院进行体检的各种职业群体,存在不可避免的选择偏倚,需在后续研究中扩大样本框架并分类职业群体,此外,针对量表可进行亚人群的探索及分析,继续优化量表结构及内容。
本研究基于CTT和IRT的量表综合分析结果,对量表进行优化,通过CFA对比调整前后量表结构,结果显示JCQ-18的模型拟合指数显著优于原量表,表明优化后的量表结构更加稳定。
