Objective

To analyze the epidemiological characteristics of malignancy deaths and the changing patterns of potential life loss in Hengshui City from 2015 to 2024, providing data support for the formulation of malignancy prevention and control strategies.

Methods

We collected data on malignancy deaths among registered residents in Hengshui City, calculated the standardized mortality rates for total, gender, age, and malignancy categories, and analyzed the potential life loss and trends for the top four leading causes of malignancy deaths.

Results

The standardized mortality rates for malignancy among registered residents in Hengshui City from 2015 to 2024 were 81.05/100 000, 72.89/100 000, 92.47/100 000, 97.23/100 000, 105.47/100 000, 97.66/100 000, 115.93/100 000, 116.78/100 000, 125.23/100 000, and 125.87/100 000, showing an upward trend (AAPC=6.20%, P<0.05). The standardized mortality rates for males were higher than those for females (all P<0.05). The top four leading causes of malignancy deaths among registered residents in Hengshui City were lung cancer, stomach cancer, liver cancer, and colorectal cancer, accounting for 26.51%, 11.34%, 11.57%, and 4.64% of deaths, respectively. The standardized mortality rates for all four malignancies showed an upward trend, with AAPC values of 11.01%, 15.17%, 3.35%, and 9.43%, respectively (all P<0.05). The proportions of malignancies, lung cancer, liver cancer, and stomach cancer in the 70-79 age group showed a significant upward trend for both males and females, and the proportion was the highest. The average age at death from malignancies, lung cancer, liver cancer, and stomach cancer showed a slight upward trend (AAPC: 0.46%, 0.47%, 0.54%, 0.46%, respectively; P<0.05), while the trend in the average age at death from colorectal cancer was not significant. Among the top four leading causes of malignancy deaths, lung cancer had the highest mean potential life loss (PYLL: 12 123.00 person-years), liver cancer had the highest average potential life loss (AYLL: 15.97 years), and lung cancer had the highest potential life loss rate (PYLLR: 2.80‰). Based on the average potential life loss, the order of life loss was liver cancer, colorectal cancer, lung cancer, and stomach cancer.

Conclusion

The standardized mortality rates for different types of malignancies showed an upward trend, with significantly higher rates for males than females. The proportion of deaths occurring in the 70-79 age group rose significantly, whereas that in the 40-49 age group declined. AYLL from malignant tumors showed an overall downward trend, while the PYLLR for lung, liver, and colorectal cancer increased, suggesting a growing impact on the life expectancy of residents with Hengshui household registration. These trends should be utilized to adjust malignancy prevention and control measures accordingly.

目的

分析2015—2024年衡水市恶性肿瘤死亡流行特征与潜在寿命损失的变化规律,为制定恶性肿瘤防控策略提供数据支撑。

方法

收集衡水市户籍居民恶性肿瘤死亡资料,计算总计、性别、分年龄和恶性肿瘤类别的标化死亡率,死因前四顺位恶性肿瘤的潜在寿命损失,分析变化趋势。

结果

2015—2024年衡水市户籍居民恶性肿瘤标化死亡率依次为81.05/10万、72.89/10万、92.47/10万、97.23/10万、105.47/10万、97.66/10万、115.93/10万、116.78/10万、125.23/10万、125.87/10万,呈上升趋势(AAPC=6.20%,P<0.05),男性标化死亡率均高于女性(均P<0.05)。衡水市户籍居民恶性肿瘤死因前四顺位为肺癌、胃癌、肝癌、和结直肠癌,占比分别为26.51%、11.34%、11.57%、4.64%,标化死亡率均呈上升趋势,平均年度变化百分比(average annual percentage change,AAPC)分别为11.01%、15.17%、3.35%、9.43%(均P<0.05)。70~79年龄组恶性肿瘤、肺癌、肝癌、胃癌的总计、男性、女性的构成比均呈现显著上升趋势,且构成比最高。恶性肿瘤、肺癌、肝癌、胃癌死亡平均年龄呈小幅上升趋势(AAPC=0.46%、0.47%、0.54%、0.46%,P<0.05),结直肠癌死亡平均年龄变化趋势不显著。恶性肿瘤死亡前四顺位癌症中,潜在寿命损失均值最大人年数为肺癌,潜在寿命损失年数(potential years of life lost,PYLL)为12 123.00人年,平均潜在寿命损失均值最大为肝癌,平均寿命损失年数(average years of life lost,AYLL)为15.97年,潜在寿命损失率均值最大为肺癌,寿命损失率(potential years of life lost rate,PYLLR)为2.80‰。按平均潜在寿命损失计算,减寿次序为肝癌、结直肠癌、肺癌、胃癌。

结论

分类别恶性肿瘤标化死亡率呈上升趋势,男性显著高于女性,70~79年龄组构成比均呈现显著上升趋势,40~49年龄组构成比呈下降趋势明显,恶性肿瘤的AYLL均呈下降趋势,而肺癌、肝癌、结直肠癌的PYLLR呈上升趋势,说明对衡水市户籍居民寿命影响不断扩大。应加强对这些变化趋势的利用,针对性调整恶性肿瘤的防控措施。

目的

分析2015—2024年衡水市恶性肿瘤死亡流行特征与潜在寿命损失的变化规律,为制定恶性肿瘤防控策略提供数据支撑。

方法

收集衡水市户籍居民恶性肿瘤死亡资料,计算总计、性别、分年龄和恶性肿瘤类别的标化死亡率,死因前四顺位恶性肿瘤的潜在寿命损失,分析变化趋势。

结果

2015—2024年衡水市户籍居民恶性肿瘤标化死亡率依次为81.05/10万、72.89/10万、92.47/10万、97.23/10万、105.47/10万、97.66/10万、115.93/10万、116.78/10万、125.23/10万、125.87/10万,呈上升趋势(AAPC=6.20%,P<0.05),男性标化死亡率均高于女性(均P<0.05)。衡水市户籍居民恶性肿瘤死因前四顺位为肺癌、胃癌、肝癌、和结直肠癌,占比分别为26.51%、11.34%、11.57%、4.64%,标化死亡率均呈上升趋势,平均年度变化百分比(average annual percentage change,AAPC)分别为11.01%、15.17%、3.35%、9.43%(均P<0.05)。70~79年龄组恶性肿瘤、肺癌、肝癌、胃癌的总计、男性、女性的构成比均呈现显著上升趋势,且构成比最高。恶性肿瘤、肺癌、肝癌、胃癌死亡平均年龄呈小幅上升趋势(AAPC=0.46%、0.47%、0.54%、0.46%,P<0.05),结直肠癌死亡平均年龄变化趋势不显著。恶性肿瘤死亡前四顺位癌症中,潜在寿命损失均值最大人年数为肺癌,潜在寿命损失年数(potential years of life lost,PYLL)为12 123.00人年,平均潜在寿命损失均值最大为肝癌,平均寿命损失年数(average years of life lost,AYLL)为15.97年,潜在寿命损失率均值最大为肺癌,寿命损失率(potential years of life lost rate,PYLLR)为2.80‰。按平均潜在寿命损失计算,减寿次序为肝癌、结直肠癌、肺癌、胃癌。

结论

分类别恶性肿瘤标化死亡率呈上升趋势,男性显著高于女性,70~79年龄组构成比均呈现显著上升趋势,40~49年龄组构成比呈下降趋势明显,恶性肿瘤的AYLL均呈下降趋势,而肺癌、肝癌、结直肠癌的PYLLR呈上升趋势,说明对衡水市户籍居民寿命影响不断扩大。应加强对这些变化趋势的利用,针对性调整恶性肿瘤的防控措施。

Objective

To analyze the epidemiological characteristics of malignancy deaths and the changing patterns of potential life loss in Hengshui City from 2015 to 2024, providing data support for the formulation of malignancy prevention and control strategies.

Methods

We collected data on malignancy deaths among registered residents in Hengshui City, calculated the standardized mortality rates for total, gender, age, and malignancy categories, and analyzed the potential life loss and trends for the top four leading causes of malignancy deaths.

Results

The standardized mortality rates for malignancy among registered residents in Hengshui City from 2015 to 2024 were 81.05/100 000, 72.89/100 000, 92.47/100 000, 97.23/100 000, 105.47/100 000, 97.66/100 000, 115.93/100 000, 116.78/100 000, 125.23/100 000, and 125.87/100 000, showing an upward trend (AAPC=6.20%, P<0.05). The standardized mortality rates for males were higher than those for females (all P<0.05). The top four leading causes of malignancy deaths among registered residents in Hengshui City were lung cancer, stomach cancer, liver cancer, and colorectal cancer, accounting for 26.51%, 11.34%, 11.57%, and 4.64% of deaths, respectively. The standardized mortality rates for all four malignancies showed an upward trend, with AAPC values of 11.01%, 15.17%, 3.35%, and 9.43%, respectively (all P<0.05). The proportions of malignancies, lung cancer, liver cancer, and stomach cancer in the 70-79 age group showed a significant upward trend for both males and females, and the proportion was the highest. The average age at death from malignancies, lung cancer, liver cancer, and stomach cancer showed a slight upward trend (AAPC: 0.46%, 0.47%, 0.54%, 0.46%, respectively; P<0.05), while the trend in the average age at death from colorectal cancer was not significant. Among the top four leading causes of malignancy deaths, lung cancer had the highest mean potential life loss (PYLL: 12 123.00 person-years), liver cancer had the highest average potential life loss (AYLL: 15.97 years), and lung cancer had the highest potential life loss rate (PYLLR: 2.80‰). Based on the average potential life loss, the order of life loss was liver cancer, colorectal cancer, lung cancer, and stomach cancer.

Conclusion

The standardized mortality rates for different types of malignancies showed an upward trend, with significantly higher rates for males than females. The proportion of deaths occurring in the 70-79 age group rose significantly, whereas that in the 40-49 age group declined. AYLL from malignant tumors showed an overall downward trend, while the PYLLR for lung, liver, and colorectal cancer increased, suggesting a growing impact on the life expectancy of residents with Hengshui household registration. These trends should be utilized to adjust malignancy prevention and control measures accordingly.

世界卫生组织国际癌症研究机构(IARC)报告[1],每年全球约有1 800万人被诊断为恶性肿瘤,近1 000万人因恶性肿瘤而死亡,恶性肿瘤不仅给患者及其家庭带来沉重的身心负担,也对社会经济发展和公共卫生体系构成了巨大挑战。在中国,随着人口老龄化进程的加速、城市化水平的提高以及生活方式的改变,恶性肿瘤的死亡率呈现出快速上升的趋势。据国家癌症中心发布的最新癌症报告,恶性肿瘤已成为我国居民死亡的主要原因之一,严重威胁着人民群众的生命健康[2]。然而,由于恶性肿瘤的发病机制复杂多样,不同地区、不同人群之间的流行特征存在显著差异,因此,针对特定地区、特定人群的恶性肿瘤流行特征研究显得尤为重要。衡水市属于典型的平原地区,恶性肿瘤的死亡流行特征具有一定区域代表性,恶性肿瘤死亡率持续上升,给公共卫生体系带来了巨大压力。但随着医疗技术的不断进步和公众健康意识的提高,恶性肿瘤的早期筛查和诊断能力也在逐步增强。目前,目前针对衡水市恶性肿瘤流行特征的系统研究仍相对匮乏,因此,本研究在借鉴前人研究成果的基础上,对衡水市户籍居民恶性肿瘤的死亡流行特征与潜在寿命损失趋势进行分析,为制定更加科学合理的恶性肿瘤防控策略提供有力的数据支撑和科学依据。
本研究对2015—2024年衡水市恶性肿瘤的死亡数据进行整理和分析。通过计算不同恶性肿瘤类型、不同性别、不同年龄段的标化死亡率、构成比等指标,揭示恶性肿瘤的死亡流行特征。同时,采用潜在寿命损失年数(potential years of life lost,PYLL)等指标,评估恶性肿瘤死亡对衡水市居民潜在寿命损失的影响。
研究相关资料来源于国家死因监测系统中2015—2024年衡水市上报的全人群死因监测数据。死因监测[3]的死因编码、分类和统计采用国际疾病分类第十次修订版(international classfication of diseases,ICD-10)。死亡人口数据来源于2015—2024年衡水市公安部门提供的分年龄、分性别户籍人口。
主要计算指标有分恶性肿瘤类型、分性别和分年龄的标化死亡率[4]、死因构成、PYLL、平均寿命损失年数(average year of life lost,AYLL)、寿命损失率(potential years of life lost rate,PYLLR)和平均年度变化百分比(average annual percent vhange,AAPC)。
减寿指标PYLL、AYLL、PYLLR的公式为:PYLL=∑(ai×di);AYLL=PYLL/∑di;PYLLR=(PYLL/N)×1 000%。式中i为死亡年龄,ai为死亡年龄i的寿命损失年数,ai=L衡水-(i+0.5);L衡水为衡水市期望人均寿命,按疾控部门公布计算。di为死亡年龄为i的人数;N为当年衡水市户籍人口。采用年度变化百分比(annual percent vhange,APC)对标化死亡率年度变化率进行计算,计算公式为:APC=(eβ-1)×100%,其中β为每段的回归系数,使用t检验进行验证,在每段内计算APC,年度标化死亡率采用对数线性模型进行拟合,即拟合标化死亡率的自然对数值与年度的关系,以均方误差最小为目标函数,遍历计算显著变化点。AAPC[5]是各段APC的加权和,加权的权重考虑了每段时间的长度和数据点的数量,以确保AAPC能够反映整个时间段的平均变化趋势。
死亡报告数据室通过基层医疗单位、县区、地市疾控中心网络逐级申报,死因数据分类比较明确。每年抽样约5%,对于死因不明和死因不准确的个例,专家重新核查病例信息以明确死因。通过死亡漏报调查,进一步提高数据准确性[6]
采用Excel 2016软件进行标化死亡率、PYLL、AYLL、PYLLR等数据的计算及排序,采用SPSS 25.0软件进行正态性检验及组间差异检验,采用joinpint进行特征流行趋势计算,检验水准α=0.05,均为双侧检验。
2015—2024年衡水市户籍居民恶性肿瘤男性标化死亡率均高于女性(男性、女性标化死亡率均值为129.72/10万、76.18/10万,t=14.54,P<0.05)。恶性肿瘤总计标化死亡率依次为81.05/10万、72.89/10万、92.47/10万、97.23/10万、105.47/10万、97.66/10万、115.93/10万、116.78/10万、125.23/10万、125.87/10万,见表1。恶性肿瘤标化死亡率总计、男性、女性均呈现上升趋势(AAPC=6.20%、6.53%、5.91%,均P<0.05),见图1
2015—2024年衡水市户籍居民恶性肿瘤死亡前四顺位依次为肺癌、肝癌、胃癌、结直肠癌。四种恶性肿瘤中,男性占比均显著高于女性,其中肺癌:男性、女性占比均值分别为66.27%、33.73%(t=39.61,P<0.05);肝癌:男性、女性占比均值分别为70.64%、29.36%(t=38.34,P<0.05);胃癌:男性、女性占比均值分别为70.34%、29.66%(t=42.01,P<0.05);结、直肠癌:男性、女性占比均值分别为59.74%、40.26%(t=8.47,P<0.05)。见图2
2015—2024年肺癌、结直肠癌在恶性肿瘤死亡中位列第一和第四,2015—2019年胃癌位列第二、肝癌位列第三,2020—2024年肝癌位列第二、胃癌位列第三。前四顺位癌症分类平均构成占恶性肿瘤的54.06%(43.18%~63.28%),标化死亡率均呈现上升趋势(肺癌、肝癌、胃癌、结直肠癌AAPC分别为11.01%、15.17%、3.35%、9.43%,t值分别为9.25、5.40、2.09、5.36,均P<0.05)。在恶性肿瘤中的构成比,肺癌呈上升趋势(AAPC=6.41%,t=4.27,P<0.05),肝癌、结直肠癌变化趋势不具有显著性,胃癌呈下降趋势(AAPC=-2.69%,t=-2.5,P<0.05)。见表2
表3为各类恶性肿瘤年龄别构成比的AAPC,70~79年龄组中,恶性肿瘤、肺癌、肝癌、胃癌的总计、男性、女性的构成比均呈上升趋势,男性、女性结直肠癌变化趋势不显著。80+年龄组中,恶性肿瘤总计、男性恶性肿瘤、胃癌总计、女性结直肠癌构成比呈上升趋势,其余年龄组各类别恶性肿瘤死亡构成比多为下降趋势,40~49年龄组下降趋势明显,其中肺癌总计、男性肺癌、女性肺癌下降最快(AAPC分别为-14.91%、-15.63%、-14.60%,均P<0.05)。
2015—2024年恶性肿瘤、肺癌、肝癌、胃癌平均死亡年龄呈小幅上升趋势(AAPC分别为0.46%、0.47%、0.54%、0.46%,P<0.05),结直肠癌死亡平均年龄变化趋势不显著。见图3
2015—2024年衡水市户籍居民恶性肿瘤死亡前四顺位癌症中,潜在寿命损失均值最大人年数为肺癌,PYLL为16 348.38人年,平均潜在寿命损失均值最大为肝癌,AYLL为17.61年,潜在寿命损失率均值最大为肺癌,PYLLR为3.78‰。按平均潜在寿命损失计算,减寿次序为肝癌、结直肠癌、肺癌、胃癌。
潜在寿命损失人年数肺癌、肝癌、结直肠癌均呈上升趋势(AAPC分别为8.14%、9.59%、3.55%,t值分别为8.32、4.43、7.08,均P<0.05),胃癌潜在寿命损失变化趋势不显著。平均潜在寿命损失中,肺癌、肝癌、胃癌均呈下降趋势(AAPC分别为-2.99%、-4.10%、-2.99%,t值分别为-7.39、-2.32、-6.45,均P<0.05),结直肠癌平均潜在寿命损失变化趋势不显著。潜在寿命损失率中,肺癌、肝癌、结直肠癌呈上升趋势(AAPC分别为9.20%、10.68%、4.48%,t值分别为8.31、4.87、4.48,均P<0.05),胃癌潜在寿命损失率变化趋势不显著。见表4
2024年衡水市户籍居民恶性肿瘤标化死亡率为125.87/10万,其中男性恶性肿瘤标化死亡率为158.80/10万,女性恶性肿瘤变化死亡率为93.45/10万,均低于全国水平[7]。2015—2024年衡水市户籍居民恶性肿瘤死亡率变化呈上升趋势,AAPC变化为6.20%,男性恶性肿瘤标化死亡率显著高于女性,这与国内多数研究一致[8]。这可能与人口老龄化、生活方式的改变以及饮食安全等因素有关[9],性别差异可能与多种因素有关,如男性的生活方式(如吸烟、饮酒等)、职业暴露以及遗传因素等。
衡水市2015—2024年恶性肿瘤死亡的前四顺位种类为肺癌、肝癌、胃癌和结直肠癌,研究期间,肺癌和结直肠癌在恶性肿瘤死亡中的顺位保持稳定,肝癌和胃癌的顺位有所变化,肺癌在恶性肿瘤中的构成比呈上升趋势,而胃癌则呈下降趋势。这一变化反映了社会经济发展、医疗技术进步和疾病防控策略实施的综合影响[10]。肝癌的上升可能与乙肝病毒感染、饮酒习惯等因素有关,而胃癌的下降则可能与幽门螺杆菌感染的控制、饮食习惯的改善有关[4]。肺癌、肝癌、胃癌和结直肠癌的标化死亡率均呈上升趋势,其中肺癌和结直肠癌的上升速度较快,这与恶性肿瘤标化死亡率变化是一致的。且男性在这四种癌症中的死亡率均显著高于女性。研究结果提示,在制定防控策略时,应充分考虑性别因素。男性可能因吸烟、职业暴露等生活习惯和环境因素而面临更高的风险[11],因此,针对男性的健康教育和生活方式干预显得尤为重要。
70~79岁年龄组恶性肿瘤、肺癌、肝癌、胃癌的总计、男性及女性的构成比均呈现显著上升趋势,且构成比最高,这与国内多个研究结果一致[12]。结果表明,随着年龄的增加,尤其是进入老年阶段,个体罹患这些恶性肿瘤的风险显著增加。这可能与老年人免疫功能下降、慢性疾病累积以及长期暴露于致癌因素等多种因素有关。80+年龄组与其他年龄组相比,80+年龄组的恶性肿瘤死亡构成比显著较低。这可能与人体免疫力衰老特点、生活饮食、基础疾病服药有关[13]。40~49岁年龄组各类别恶性肿瘤死亡构成比下降趋势明显,其中肺癌总计、男性肺癌和女性肺癌的下降幅度最大。这一趋势可能与近年来公众健康意识的提高、吸烟率的下降以及早期筛查技术的普及有关。这些积极因素共同作用,使得中年人群的癌症死亡率得到有效控制。恶性肿瘤、肺癌、肝癌、胃癌的死亡平均年龄均呈小幅上升趋势,结直肠癌的死亡平均年龄变化趋势不显著。这一趋势可能反映了不同癌症类型的生物学特性和治疗效果的差异。
肝癌AYLL为17.61年,是四种癌症类型中最高的,这反映了肝癌对居民寿命的平均影响最为严重。肺癌、肝癌、胃癌的AYLL均呈下降趋势(AAPC分别为-2.99%、-4.10%、-2.99%,P<0.05),这可能得益于近年来癌症早期筛查、诊断和治疗技术的进步,以及公众健康意识的提高[14]。肺癌PYLLR为3.78‰,是四种癌症类型中最高的,且呈上升趋势,这表明肺癌在衡水市居民中的发病率和死亡率仍然较高,且对居民寿命的影响在不断扩大。肝癌和结直肠癌的PYLLR也呈上升趋势,胃癌的PYLLR变化趋势不显著。这些结果提示,需要特别关注肺癌、肝癌和结直肠癌的防控工作,以降低其对居民寿命的影响。
综上所述,不同年龄段和不同癌症类型的死亡构成比及死亡平均年龄呈现出不同的变化趋势。近年来癌症防控工作取得了一定成效,但这些癌症类型的发病率和死亡率仍然较高,对居民健康构成严重威胁。因此,我们需要继续加强癌症早期筛查、诊断和治疗技术的推广和应用,提高公众健康意识,加强癌症防控工作的力度和效果,以降低癌症对居民寿命的影响。同时,针对不同类型的癌症,我们需要制定更加精准和有效的防控策略,以应对日益严峻的癌症防控形势。
本研究仍存在一定局限性,确诊时间和病程对癌症的分类及发展有一定影响,早期原发性癌症可能无明显相关阳性体征,因此早期诊断较为困难。为具有对比性,选择了总人口中死因前四的癌症进行了对比分析,实际对于女性而言,乳腺癌死亡率高于结直肠癌,对剩余约50%的癌症死因未进行分析。未来研究,参照全国、河北省相关医疗数据进行分析。为衡水市控制恶性肿瘤发病及治疗,制定相关卫生政策提供数据参考。
