Objective

To analyze the epidemiological characteristics and incidence trend of respiratory infectious diseases in children aged 0-14 years in Lanzhou City, and provide reference for scientific prevention and control of children’s infectious diseases.

Methods

The surveillance data of respiratory infectious diseases in children aged 0-14 years in Lanzhou from 2014 to 2023 were collected through the China Disease Control and Prevention Information System. Descriptive epidemiological methods and Joinpoint regression model were used to analyze the epidemiological characteristics and incidence trends of respiratory infectious diseases.

Results

A total of 40 347 cases of 9 kinds of respiratory infectious diseases in children aged 0-14 years were reported in Lanzhou from 2014 to 2023, resulting in an average annual reported incidence rate of 743.60 per 100 000. The overall reported incidence rate showed an upward trend (APC=AAPC=14.576 6%, P<0.05).The incidence rate of the disease among male children was 811.70 /100 000, while the incidence rate among female children was 656.74 /100 000, indicating that the rate in male children was significantly higher (χ2=443.64, P<0.001). Among different populations, the number of reported cases was highest among students, totaling 21 043 cases (accounting for 52.40%). The incidence of respiratory infectious diseases had a seasonal pattern in children aged 0-14 years in Lanzhou. The peak of incidence was from November to December (12 031 cases, accounting for 30.2%), followed by May to June (8 113 cases, accounting for 20.36%) and March (4 556 cases, accounting for 11.46%). Except for COVID-19 infection, the top five reported cases were chickenpox (298.94/100 000), influenza (261.34/100 000), mumps (93.99/100 000), scarlet fever (70.7/100 000) and measles (5.99/100 000). The influenza (11 897 cases, accounting for 38.80%) ranked the first in urban area, and chickenpox (4 964 cases, accounting for 54.90%) ranked the first in county (district), the difference was statistically significant (χ2=1 034.46, P<0.05).

Conclusion

The overall incidence of respiratory infectious diseases showed an upward trend, and the incidence had obvious seasonal characteristics in children aged 0-14 years in Lanzhou from 2014 to 2023. Chickenpox and influenza were the main respiratory infectious diseases. Monitoring of pediatric respiratory infectious diseases was emphasized, with a focus on key populations and regions. It is necessary to strengthen the surveillance of respiratory infectious diseases in children, pay attention to key populations and key areas, and effectively control the spread of respiratory infectious diseases in children.

目的

分析兰州市0~14岁儿童呼吸道传染病流行特征及发病趋势,为科学开展儿童传染病防控工作提供参考。

方法

通过“中国疾病预防控制信息系统”收集兰州市2014—2023年0~14岁儿童呼吸道传染病监测数据,采用描述流行病学方法和Joinpoint回归模型对呼吸道传染病发病情况进行流行特征和发病率趋势变化分析。

结果

2014—2023年兰州市累计报告0~14岁儿童呼吸道传染病9种共40 347例,年均报告发病率743.60/10万,总体报告发病率呈上升趋势(APC=AAPC=14.576 6%,P<0.05)。男童发病率为811.70/10万,女童发病率为656.74/10万,男童发病率高于女童(χ2=443.64,P<0.001)。不同人群中,学生报告病例数占比最高,为21 043例(占52.40%)。兰州市0~14岁儿童呼吸道传染病发病具有季节性,发病高峰在11—12月(12 031例,占30.2%),其次为5—6月(8 113例,占20.36%)、3月(4 556例,占11.46%)。除新冠病毒感染外,累计报告发病居前五位的依次为水痘(298.94/10万)、流行性感冒(261.34/10万)、流行性腮腺炎(93.99/10万)、猩红热(70.7/10万)及麻疹(5.99/10万)。城区流感病例数(11 897例,占38.80%)居首位,县(区)水痘病例数(4 964例,占54.90%)居首位,差异具有统计学意义(χ2=1 034.46,P<0.05)。

结论

2014—2023年兰州市0~14岁儿童呼吸道传染病总体发病率呈上升趋势,发病具有明显季节性特征。水痘、流行性感冒为主要呼吸道传染病。应加强儿童呼吸道多发传染病监测,关注重点人群重点地区,有效控制儿童呼吸道传染病传播流行。

目的

分析兰州市0~14岁儿童呼吸道传染病流行特征及发病趋势,为科学开展儿童传染病防控工作提供参考。

方法

通过“中国疾病预防控制信息系统”收集兰州市2014—2023年0~14岁儿童呼吸道传染病监测数据,采用描述流行病学方法和Joinpoint回归模型对呼吸道传染病发病情况进行流行特征和发病率趋势变化分析。

结果

2014—2023年兰州市累计报告0~14岁儿童呼吸道传染病9种共40 347例,年均报告发病率743.60/10万,总体报告发病率呈上升趋势(APC=AAPC=14.576 6%,P<0.05)。男童发病率为811.70/10万,女童发病率为656.74/10万,男童发病率高于女童(χ2=443.64,P<0.001)。不同人群中,学生报告病例数占比最高,为21 043例(占52.40%)。兰州市0~14岁儿童呼吸道传染病发病具有季节性,发病高峰在11—12月(12 031例,占30.2%),其次为5—6月(8 113例,占20.36%)、3月(4 556例,占11.46%)。除新冠病毒感染外,累计报告发病居前五位的依次为水痘(298.94/10万)、流行性感冒(261.34/10万)、流行性腮腺炎(93.99/10万)、猩红热(70.7/10万)及麻疹(5.99/10万)。城区流感病例数(11 897例,占38.80%)居首位,县(区)水痘病例数(4 964例,占54.90%)居首位,差异具有统计学意义(χ2=1 034.46,P<0.05)。

结论

2014—2023年兰州市0~14岁儿童呼吸道传染病总体发病率呈上升趋势,发病具有明显季节性特征。水痘、流行性感冒为主要呼吸道传染病。应加强儿童呼吸道多发传染病监测,关注重点人群重点地区,有效控制儿童呼吸道传染病传播流行。

Objective

To analyze the epidemiological characteristics and incidence trend of respiratory infectious diseases in children aged 0-14 years in Lanzhou City, and provide reference for scientific prevention and control of children’s infectious diseases.

Methods

The surveillance data of respiratory infectious diseases in children aged 0-14 years in Lanzhou from 2014 to 2023 were collected through the China Disease Control and Prevention Information System. Descriptive epidemiological methods and Joinpoint regression model were used to analyze the epidemiological characteristics and incidence trends of respiratory infectious diseases.

Results

A total of 40 347 cases of 9 kinds of respiratory infectious diseases in children aged 0-14 years were reported in Lanzhou from 2014 to 2023, resulting in an average annual reported incidence rate of 743.60 per 100 000. The overall reported incidence rate showed an upward trend (APC=AAPC=14.576 6%, P<0.05).The incidence rate of the disease among male children was 811.70 /100 000, while the incidence rate among female children was 656.74 /100 000, indicating that the rate in male children was significantly higher (χ2=443.64, P<0.001). Among different populations, the number of reported cases was highest among students, totaling 21 043 cases (accounting for 52.40%). The incidence of respiratory infectious diseases had a seasonal pattern in children aged 0-14 years in Lanzhou. The peak of incidence was from November to December (12 031 cases, accounting for 30.2%), followed by May to June (8 113 cases, accounting for 20.36%) and March (4 556 cases, accounting for 11.46%). Except for COVID-19 infection, the top five reported cases were chickenpox (298.94/100 000), influenza (261.34/100 000), mumps (93.99/100 000), scarlet fever (70.7/100 000) and measles (5.99/100 000). The influenza (11 897 cases, accounting for 38.80%) ranked the first in urban area, and chickenpox (4 964 cases, accounting for 54.90%) ranked the first in county (district), the difference was statistically significant (χ2=1 034.46, P<0.05).

Conclusion

The overall incidence of respiratory infectious diseases showed an upward trend, and the incidence had obvious seasonal characteristics in children aged 0-14 years in Lanzhou from 2014 to 2023. Chickenpox and influenza were the main respiratory infectious diseases. Monitoring of pediatric respiratory infectious diseases was emphasized, with a focus on key populations and regions. It is necessary to strengthen the surveillance of respiratory infectious diseases in children, pay attention to key populations and key areas, and effectively control the spread of respiratory infectious diseases in children.

呼吸道传染病是全球重大公共卫生挑战,其病原体通过飞沫及气溶胶传播,具有高传播性和人群普遍易感性。我国0~14岁儿童基数庞大[1],其呼吸道黏膜屏障及免疫功能发育不成熟,更易受流行性感冒、麻疹、百日咳等病原体侵袭[2]。近年来,我国北方地区儿童呼吸道疾病发病率明显上升[3],兰州市作为西北地区人口流动枢纽,其显著的大陆性气候特征与频繁的跨区域人口流动,加剧了儿童呼吸道传染病的流行强度。本研究基于2014—2023年兰州市呼吸道传染病监测数据,系统分析0~14岁儿童呼吸道传染病流行特征及发病趋势,为制定更为精准的防控策略提供科学依据,从而有效降低该年龄段儿童的发病水平。
传染病数据来自“中国疾病预防控制信息系统”,选择“发病日期”在 2014年1月1日—2023年12月31日、“审核状态”为“已终审卡”、“现住地址”为兰州市、“病例分类”为“临床诊断病例”和“确诊病例”的传染病监测数据,筛选出兰州市0~14岁儿童呼吸道传染病数据。由于新冠病毒感染为2019年新发传染病,研究未将新冠病毒感染纳入呼吸道传染病顺位分析,仅对报告发病数大于100例的水痘、流行性感冒(简称“流感”)、流行性腮腺炎(简称“流腮”)、猩红热、麻疹进行分析。通过EXCEL 2019建立数据库。人口统计资料来源于甘肃发展年鉴。
对疾病流行强度及季节性采用描述性流行病学分析,指标包括发病率、发病顺位和季节指数。季节指数大于1的月份为呼吸道传染病高发病率月份,接近或小于1表示无明显季节性。使用SPSS 22软件进行分析,率、构成比的比较采用χ2检验。利用Joinpoint Regression Program 4.9.1.0软件对发病率的时间变化趋势进行分析,采用平均年度变化百分比(Average Annual Percent Change,AAPC)和年度变化百分比(Annual Percent Change,APC)及各自对应的95%CI作为评价时间趋势变化的指标。当转折点数量为0时,APC=AAPC,APC>0表示上升趋势,APC<0表示下降趋势。检验水准α=0.05。
2014—2023年兰州市累计报告0~14岁儿童呼吸道传染病9种40 347例,占全人群发病数的52.55%,年均发病率为743.60/10万,高于全人群发病率(192.26/10万),差异具有统计学意义(χ2=1 245.6,P<0.05)。见表1
除新冠病毒感染外,报告发病前五的传染病依次为水痘(16 220例,298.94/10万)、流感(14 180例,261.34/10万)、流腮(5 100例,93.99/10万)、猩红热(3 836例,70.7/10万)、麻疹(325例,5.99/10万),占报告总数的99.2%。
Joinpoint回归分析结果显示,2014—2023年兰州市0~14岁儿童呼吸道传染发病率呈总体上升趋势(AAPC=14.58%,95%CI:5.65~24.24),趋势检验具有统计学意义(t=3.87,P<0.05)。2014—2019年水痘(APC=6.26%,95%CI:-2.59%~15.91%)、流腮(APC=10.23%,95%CI:-8.10%~32.22%)、猩红热(APC=5.96%,95%CI:-1.66%~14.17%)发病率均无显著上升趋势(t值分别为1.79、1.36、1.99,P值分别为0.13、0.23、0.11);2019—2023年水痘(APC=-27.53%,95%CI:-43.29%~-7.39%)、流腮(APC=-43.73%,95%CI:-67.80%~-1.68%)、猩红热(APC=-40.85%,95%CI:-63.92%~-3.03%)发病率均呈显著下降趋势(t值分别为-3.37、-2.64、-2.73,P均<0.05)。2014~2023年流感发病率呈总体上升趋势(AAPC=54.23%,95%CI:31.16~81.38),趋势检验具有统计学意义(t=6.17,P<0.05)。2014—2023年麻疹(APC=-47.35%,95%CI:-72.00%~0.96%)发病率呈总体下降趋势,趋势检验具有统计学意义(t=-2.4,P<0.05)。见图1表2
2014—2023年兰州市0~14岁儿童呼吸道传染病发病呈多峰分布。3月、5—6月、11—12月的季节指数均大于1;11—12月季节指数逐渐上升,提示冬季节呼吸道传染病高发;2月、8月为学校寒暑假期间,季节指数相对较低。水痘、流感、流腮、猩红热发病均呈双峰分布,麻疹发病呈单峰分布,见图2
2014—2023年兰州市报告0~14岁儿童呼吸道传染病病例中,男童发病率高于女童,差异具有统计学意义(χ2=443.64,P<0.001)。见表3
除新冠病毒感染外,2014—2023年兰州市报告0~14岁儿童呼吸道传染病病例中,学生、幼托儿童、散居儿童发病数占比依次为52.40%、25.90%、21.70%。学生发病数居前5位的依次为水痘、流感、流腮、猩红热、麻疹;幼托儿童发病数居前5位的依次为流感、水痘、猩红热、流腮、麻疹;散居儿童发病数居前5位的依次为流感、水痘、流腮、猩红热、麻疹。学生、幼托儿童、散居儿童病例数前五构成不同,差异具有统计学意义(χ2=6 620.48,P<0.05)。见表4
除新冠病毒感染外,2014—2023年兰州市城区0~14岁儿童呼吸道传染病病例数居前5位的依次为流感、水痘、流腮、猩红热、麻疹;兰州市县(区)居前5位的依次为水痘、流感、流腮、猩红热、麻疹。城区、县(区)病例数前五构成不同,差异具有统计学意义(χ2=1 034.46,P<0.05)。见表5
2014—2023年兰州市0~14岁儿童主要呼吸道传染病共报告9种40 347例,年均发病率为734.22/10万,除新冠病毒感染外,发病居前5的依次为水痘、流感、流腮、猩红热、麻疹。其中,男童发病率高于女童,与田梦圆[4]、范娇[5]等人研究结果一致,可能与男童参与户外活动的频率更高、接触病原体几率更高相关。从发病人群构成来看,学生群体呼吸道传染病发病数占比较高,可能与学生平时在校期间与同学接触密切,个人防护不到位可引起呼吸道传染病多发有关[6]
2014—2023年兰州市0~14岁儿童呼吸道传染病总体发病率呈上升趋势,与湖州市[7]、深圳市[8]等城市研究结果基本一致。2019年、2023年兰州市0~14岁儿童呼吸道传染病发病率明显高于其他年份。2019年兰州市呼吸道传染病主要受水痘、流腮和猩红热高发影响[9-11],可能与2019年2剂次水痘疫苗接种率低、1剂次流腮疫苗的保护效果降低有关[12],2019年兰州市发生多起水痘、流腮聚集性疫情,整体造成全市呼吸道传染病高发。2019年兰州市猩红热发病处于较高水平,与甘肃省、全国形势一致[1113]。2020—2022年为新冠疫情大流行期,社会普遍采取广泛公共卫生措施,如佩戴口罩、保持社交距离、加强卫生消毒等,对呼吸道传染病传播产生抑制作用,遏制了水痘、流腮、猩红热在儿童中的传播,水痘、流腮、猩红热发病明显下降,使得儿童呼吸道传染病发病率整体下降。2023年新冠疫情管控措施放松后,儿童聚集性活动增加,同时疫情期间儿童免疫水平降低、病毒变异株免疫逃逸能力增强等因素叠加[14-15],导致兰州市儿童呼吸道传染病发病数出现明显上升。
2014—2023年兰州市儿童呼吸道传染病在11—12月、5—6月、3月高发,有明显的季节性,11—12月、3月病例增多与气温较低造成儿童呼吸道黏膜抵抗力下降有关[16],5—6月病例增多可能与空气湿度升高有关[17]
2014—2023年兰州市0~14岁儿童水痘发病居首位,可能与国内儿童水痘疫苗接种率普遍偏低有关[18]。水痘疫苗属于二类付费疫苗,需接种2剂次,且兰州市地处西北地区,经济卫生条件差,儿童水痘疫苗接种率较低,无法形成有效免疫屏障。2019—2023年间水痘发病率呈下降趋势,与新冠疫情期间,教育部门落实延后开学、停课政策,以及全民健康防护有关。2014—2023年兰州市0~14岁儿童流感发病率呈总体上升趋势,且在2023年明显升高,与成都市[19]、黄冈市[20]情况基本一致。2023年儿童流感高发可能与新冠疫情后各医疗卫生机构监测敏感度提高、传染病报告逐渐规范、人群就诊意识提升等有关。此外,2020年《流感诊断和治疗指南》更新[21],实验室检测手段增加[22]
2014—2023年兰州市0~14岁儿童流感发病在人口密集的城区居首位,与城区地区经济发展水平较高、人口聚集、人员流动性较大有关[23]。人口较为稀疏的县(区)水痘病例数居首位,可能与县(区)儿童卫生习惯差、水痘疫苗接种率较低有关[24],另外水痘疫苗的保护效果是具有一定的年限[25]。建议各地区根据辖区内儿童高发传染病,有针对性地开展呼吸道传染病监测及健康宣教。
综上所述,兰州市0~14岁儿童呼吸道传染病发病率总体呈上升趋势,发病具有明显季节性特征。学生群体发病水平较高,应予以重视。水痘、流感为主要呼吸道传染病,应重点防控,建议将水痘疫苗纳入常规免疫,进一步提高儿童群体水痘、流感等疫苗可预防性传染病的接种率,同时加强儿童呼吸道传染病监测预警、健康教育以及机构卫生监督等综合防控措施,有效降低0~14岁儿童呼吸道传染病发病水平。
