Objective

To analyze the dynamic trends in incidence and prevalence of periodontal disease in China from 1992-2021, quantify age, period, and cohort effects, and project future trends over the next two decades, thereby informing targeted prevention strategies.

Methods

Data were extracted from the 2021 Global Burden of Disease Study. Joinpoint Regression (JPR5.0.2) was employed to assess temporal trends in periodontal disease incidence and prevalence. Age-Period-Cohort (APC) modeling was applied to disentangle independent age, period, and cohort effects. Bayesian Age-Period-Cohort (BAPC) analysis was utilized to project incidence and prevalence from 2022 to 2041.

Results

Between 1992 and 2021, the incidence and prevalence of periodontal diseases in China exhibited an overall upward trend, with an annual increase of 0.18% in incidence and 0.30% in prevalence, and higher rates observed in males than females. The APC model revealed that net age effects were positive across all age groups, with net changes of 0.61%(95%CI: 0.43-0.80) for incidence and 0.98%(95%CI: 0.81-1.15) for prevalence, showing an initial increase followed by a decline and subsequent rise. The highest incidence occurred in the 50-54 age group, while the highest prevalence was observed in the 60-64 age group. Using 2002-2006 as the reference period (RR=1), the risk of incidence and prevalence began to rise continuously around 2007, peaking at RR=1.04 (95%CI: 0.99-1.09) and RR=1.09 (95%CI: 1.05-1.13), respectively. For the birth cohort, using 1953—1957 as the reference (RR=1), the 1998-2002 birth cohort exhibited the highest risks for both incidence (RR=1.80, 95%CI: 1.14-2.84) and prevalence (RR=1.87, 95%CI: 1.01-3.46). Predictions indicated that the incidence and prevalence of periodontal diseases in China will continue to rise over the next 20 years.

Conclusion

China has experienced escalating periodontal disease burden since 1992, with marked age-dependent heterogeneity. Without effective interventions, incidence and prevalence are expected to rise rapidly through 2041, highlighting the need for heightened attention to its disease burden in public health planning.

目的

分析1992—2021中国牙周病发病率和患病率的动态变化趋势及其年龄、时期和队列效应,并对未来20年进行预测,为防控中国地区的牙周病提供依据。

方法

利用2021年全球疾病负担研究结果,采用JPR 5.0.2计算牙周病发病率和患病率的趋势变化;使用年龄-时期-队列模型估计年龄、时期和队列的净影响;使用BAPC模型预测2022—2041年牙周病的发病率和患病率。

结果

1992—2021年中国牙周病发病率和患病率整体呈上升趋势,发病率年均上升0.18%,患病率年均上升0.30%,且男性高于女性。APC模型结果显示,整个年龄组净变化均大于0,发病率和患病率的净变化分别为0.61%(95%CI:0.43~0.80)和0.98%(95%CI:0.81~1.15),整体呈现先升高后降低再升高的趋势,50~54岁组发病率最高,60~64岁组患病率最高;以2002—2006年为参考时期(RR=1),2007年左右开始发病和患病风险持续上升至最高,分别为(RR=1.04,95%CI:0.99~1.09)和(RR=1.09,95%CI:1.05~1.13);以1953—1957年出生队列为参考(RR=1),1998—2002年出生队列发病和患病风险达到最高,分别为(RR=1.80,95%CI:1.14~2.84)和(RR=1.87,95%CI:1.01~3.46)。预计在未来20年,中国牙周病的发病率和患病率仍会呈现持续上升的趋势。

结论

1992—2021年间中国全人群牙周病的发病率和患病率呈上升趋势,且流行趋势存在年龄差异,预计2022—2041年牙周病的发病率、患病率将持续快速升高,其疾病负担需引起重视。

目的

分析1992—2021中国牙周病发病率和患病率的动态变化趋势及其年龄、时期和队列效应,并对未来20年进行预测,为防控中国地区的牙周病提供依据。

方法

利用2021年全球疾病负担研究结果,采用JPR 5.0.2计算牙周病发病率和患病率的趋势变化;使用年龄-时期-队列模型估计年龄、时期和队列的净影响;使用BAPC模型预测2022—2041年牙周病的发病率和患病率。

结果

1992—2021年中国牙周病发病率和患病率整体呈上升趋势,发病率年均上升0.18%,患病率年均上升0.30%,且男性高于女性。APC模型结果显示,整个年龄组净变化均大于0,发病率和患病率的净变化分别为0.61%(95%CI:0.43~0.80)和0.98%(95%CI:0.81~1.15),整体呈现先升高后降低再升高的趋势,50~54岁组发病率最高,60~64岁组患病率最高;以2002—2006年为参考时期(RR=1),2007年左右开始发病和患病风险持续上升至最高,分别为(RR=1.04,95%CI:0.99~1.09)和(RR=1.09,95%CI:1.05~1.13);以1953—1957年出生队列为参考(RR=1),1998—2002年出生队列发病和患病风险达到最高,分别为(RR=1.80,95%CI:1.14~2.84)和(RR=1.87,95%CI:1.01~3.46)。预计在未来20年,中国牙周病的发病率和患病率仍会呈现持续上升的趋势。

结论

1992—2021年间中国全人群牙周病的发病率和患病率呈上升趋势,且流行趋势存在年龄差异,预计2022—2041年牙周病的发病率、患病率将持续快速升高,其疾病负担需引起重视。

Objective

To analyze the dynamic trends in incidence and prevalence of periodontal disease in China from 1992-2021, quantify age, period, and cohort effects, and project future trends over the next two decades, thereby informing targeted prevention strategies.

Methods

Data were extracted from the 2021 Global Burden of Disease Study. Joinpoint Regression (JPR5.0.2) was employed to assess temporal trends in periodontal disease incidence and prevalence. Age-Period-Cohort (APC) modeling was applied to disentangle independent age, period, and cohort effects. Bayesian Age-Period-Cohort (BAPC) analysis was utilized to project incidence and prevalence from 2022 to 2041.

Results

Between 1992 and 2021, the incidence and prevalence of periodontal diseases in China exhibited an overall upward trend, with an annual increase of 0.18% in incidence and 0.30% in prevalence, and higher rates observed in males than females. The APC model revealed that net age effects were positive across all age groups, with net changes of 0.61%(95%CI: 0.43-0.80) for incidence and 0.98%(95%CI: 0.81-1.15) for prevalence, showing an initial increase followed by a decline and subsequent rise. The highest incidence occurred in the 50-54 age group, while the highest prevalence was observed in the 60-64 age group. Using 2002-2006 as the reference period (RR=1), the risk of incidence and prevalence began to rise continuously around 2007, peaking at RR=1.04 (95%CI: 0.99-1.09) and RR=1.09 (95%CI: 1.05-1.13), respectively. For the birth cohort, using 1953—1957 as the reference (RR=1), the 1998-2002 birth cohort exhibited the highest risks for both incidence (RR=1.80, 95%CI: 1.14-2.84) and prevalence (RR=1.87, 95%CI: 1.01-3.46). Predictions indicated that the incidence and prevalence of periodontal diseases in China will continue to rise over the next 20 years.

Conclusion

China has experienced escalating periodontal disease burden since 1992, with marked age-dependent heterogeneity. Without effective interventions, incidence and prevalence are expected to rise rapidly through 2041, highlighting the need for heightened attention to its disease burden in public health planning.

牙周病(Periodontal Disease)是累及牙龈、牙周膜及牙槽骨等牙齿支持组织的慢性炎症性疾病,不仅损害咀嚼、语言功能和面容美观,还与糖尿病、心血管疾病及类风湿性关节炎等全身性疾病密切相关[1-5]。据《全球疾病负担研究(GBD)2021》统计,中国牙周病患病人数超过2.5亿,其中中重度牙周炎患者占比达34.7%[6],且已成为主要的口腔疾病负担来源之一[7]。现有中国牙周病流行病学研究多局限于横断面调查,对长期流行趋势的驱动机制(如年龄效应、时期变化与出生队列差异)缺乏深度解析,制约精准防控策略的制定。本研究采用GBD 2021研究的数据,分析中国1992—2021年间牙周病发病率和患病率的流行趋势,探讨年龄、时期和队列的净效应,并对2022—2041年牙周病的发病率、患病率趋势进行预测。为更全面地了解牙周病的发病患病趋势,制定有效的预防和干预策略提供科学依据。
本研究提取了2021年全球疾病负担研究数据资源库(GBD 2021)数据库中1992-2021年中国牙周病患病数据,其包括1990-2021年间204个国家和地区的369种疾病和伤害[8]
本研究选取了1992—2021年中国15~79岁人群牙周病发病患病数据,以5年为间距,将人群分为14个年龄组,6个时期组,18个队列组,APC模型默认中心时期和出生队列作为对照组,因此2002—2006期间和1953—1957年的出生队列被定义为对照组。Joinpoint回归分析通过计算年度变化百分比(Annual Percentage Change,APC)和平均年度变化百分比(Average Annual Percentage change,AAPC)及其95%CI判断该时间区段为上升或下降趋势,使用美国国家癌症研究所开发的JPR程序(版本5.0.2)进行分析;年龄-时期-队列研究采用内生因子法,根据Yang等[9]的证明,该方法在不需要设置限制条件下,可以得到收敛且唯一的估计值,分析使用美国国家癌症研究所开发的年龄-时期-队列的网页分析工具进行分析[10];本研究选用由Riebler和Held[11]提出的BAPC集成嵌套拉普拉斯近似(integrated nested Laplace approximations,INLA)算法进行模型拟合,规避了MCMC算法的长耗时和收敛问题并在准确性和覆盖率上得到了验证[12]。BAPC预测模型通过R 4.3.3进行分析。
1992—2021年中国牙周病发病率和患病率均总体呈上升趋势,其变化呈现明显阶段性波动态势,1992至1995年期间明显下降,1995至2000年下降趋缓,2000至2005年快速上升,后2006年开始急速下降,至2010年后开始快速上升,2014年左右开始缓慢下降。
全人群发病率由922.01/10万上升至967.03/10万,AAPC为0.18%(95%CI:0.12~0.26);男性牙周病发病率938.78/10万上升至976.24/10万,女性发病率由903.92/10万上升至956.97/10万。全人群患病率由9 926.79/10万上升至10 799.46/10万,AAPC为0.30%(95%CI:0.20~0.42);男性牙周病患病率由10 309.49/10万上升至11 081.92/10万,女性患病率由9 540.17/10万上升至10 520.13/10万。男女性和全人群均是在2010—2014年段上升最快,2005—2010年段下降最快。男性发病率、患病率在任何观察阶段均高于女性。表1图1
APC模型估计的牙周病发病率、患病率全人群、男性和女性每年的净变化均大于0。且净变化、全时期RR值、全队列RR值均有统计学意义(P<0.001),显示1992—2021年中国全人群、男性和女性的牙周病发病率、患病率的年龄-时期-队列分析整体呈上升趋势。详见表2
图2所示,在调整了时期效应后,同一出生队列中,1992—2021年中国牙周病发病率总体的纵向年龄效应呈现先升高后降低再升高的趋势,50~54岁年龄段人群发病率最高。女性牙周病发病率在15~54岁持续增长,之后发病率保持平稳但略有下降的趋势,70~74岁出现明显下降,75~79岁发病率持续升高并逼近最高点。男性人群变化趋势与女性一致,15~54岁年龄段发病率略高于女性,55~69岁男女置信区间重叠,牙周病发病率无差异,70~74岁出现明显下降后75~79岁发病率略有升高,但此年龄段发病率明显低于女性。
1992—2021年中国牙周病患病率总体的纵向年龄效应均呈现先升高后降低再升高的趋势,60~64岁患病率最高,之后开始下降,且70~74岁下降趋势明显,75~79岁开始略有上升。两性患病率的纵向年龄趋势基本一致,但15~69岁年龄段男性患病率高于女性,70~79岁年龄段女性患病率反超男性且差距逐渐增大。
在控制其他两种效应后,牙周病发病率、患病率时期效应的RR整体呈现上升趋势且女性一直高于男性。相较于参照年份,1992—2014期间中国牙周病发病率、患病率的RR值较低。约2007年开始发病风险持续快速上升至2017年增速放缓且仍保持平稳上升。图3
在总体上1992—2021年中国牙周病发病率、患病率的出生队列RR值均呈现上升趋势且女性高于男性,出生队列发病率、患病率RR值高点均出现在2002年左右分别达到了1.80、1.87。相较于参照队列(RR=1),出生在1957年前的人群牙周病发病、患病风险虽略有波动但整体较低,最高于1942年左右为1.07和0.96,1957年后的人群整体发病和患病较高且持续上升。图4
在调整了年龄、时期效应后,男性和女性发病率和患病率都将出现明显上升趋势,预测男性年龄标准化发病率由2021年的976.24/10万上升至2041年的1 496.11/10万,年龄标准化患病率11 081.92/10万上升至2041年的17 365.19/10万。预测女性年龄标准化发病率,由2021年的956.97/10万上升至2041年的1 543.14/10万,年龄标准化患病率10 520.13/10万上升至2041年为16 668.89/10万(图5)。
牙周病是常见的口腔疾病,长期的反复发作,造成患者生活质量的严重下降。其患病率受个人饮食和生活习惯、年龄、社会经济水平和国家相关卫生政策等多因素影响。本研究分析了1992—2021年中国牙周病的发病率、患病率及其年龄、时期和队列差异。
三十年间,中国牙周病发病率、患病率均呈现上升趋势,可能与中国老龄化程度加剧,口腔医疗体系不断完善以及健康中国等公共卫生政策有关。
JPR回归分析结果显示,男性牙周病发病率、患病率均略高于女性,与既往研究结果相同,可能与男性在吸烟、饮酒、咀嚼槟榔及刷牙频率更低等危险因素中的暴露率更高有关。中国全人群牙周病发病率、患病率变化趋势的阶段性波动特征与中国社会经济发展和公共卫生政策的实施存在显著关联。1992—1995年呈下降趋势可能与1990年启动的《中国2000年口腔健康目标》相关,该政策推动基层口腔健康教育覆盖率提高至38%[13]。1995—2000年下降趋缓可能是由于政策效果的边际递减,农村地区牙刷普及率停滞[14]。2000—2005年快速上升可能与城镇化加速引起含糖食品消费激增[15]有关。2005—2010再次下降可能与新型农村合作医疗(2003年)全面覆盖,使得大量农村居民就医意愿和就医可及性增加。2014年后略有下降可能是因为《健康中国2030》规划推动全国儿童窝沟封闭覆盖率提升至22%(2020年)[16],源头防控初显成效。
年龄是牙周病发病、患病的重要的影响因素。年龄效应显示,全人群发病率15~54岁持续增长,55~69岁保持平稳但略有下降,65~69岁进入平台期,这种现象可能与多种危险因素叠加相关。生理方面,中年人群唾液分泌量较青年明显下降[17],自洁能力减弱,同时牙周组织经历多年使用磨损和慢性炎症积累,牙龈退缩导致牙根暴露,促进菌斑堆积。此外糖尿病发病率随年龄上升,60岁左右达峰值[18],高糖状态通过促进促炎细胞因子分泌加速牙槽骨吸收[19]。生活行为方面,2019国民健康行为监测数据显示老年人群日均刷牙次数显著低于青年,且55~64岁男性吸烟率高达 57.3%[20],影响口腔健康。发病率、患病率均在70岁开始出现明显下降,之后在75岁反弹并持续上升,这种现象可能是因为老年阶段牙齿脱落,2019年全国口腔健康调查显示70岁人群存留牙齿数量仅22.5颗,故而发病率等明显下降。而75岁反弹可能与义齿的使用有关,75岁后,中国人群义齿佩戴率较高,其中高达68.3%的使用者存在基牙牙周炎,且老年人口腔自洁能力下降使种植体周围炎发病率显著升高[21]。最后在75岁之后,女性牙周病发病率等反超男性则有可能是绝经后骨质疏松加剧牙槽骨吸收,同时雌激素缺乏导致龈沟液浓度上升[22]所致。
时期效应通常是由一系列复杂的历史事件和环境因素引起的。牙周病的时期效应整体呈现上升趋势,这种变化可能与中国整体经济水平的提高、居民口腔健康意识和医疗可及性的提升有关。2003年新型农村合作医疗和2007年城镇居民医保的全面覆盖以及2005年国家首次开展口腔流行病学普查[23]使得口腔疾病就诊率和检出率显著提升。同时社会发展引起的膳食结构转型也为牙周病的发生带来了风险,一方面精制碳水化合物摄入量增加,含糖饮料消费量增长以及快餐外卖行业的扩张,直接导致口腔微生物组成的改变,使牙周致病菌比例上升,同时2015年全国学生常见病监测中城市儿童乳牙龋患率上升至66%,形成"龋齿-牙周病"恶性循环。另一方面,现代饮食结构导致高血压、糖尿病等慢性病流行,这些全身性疾病通过"炎症-胰岛素抵抗"恶性循环加重牙周损伤[24],研究发现HbA1c每升高1%,牙周探诊深度增加0.24mm[25]。时期效应中女性发病率等均高于男性,这可能与女性就诊依从性高于男性,更易被纳入疾病监测,同时,职业女性比例上升导致个人时间减少从而影响口腔卫生维护。
出生队列效应显示发病率和患病率RR值整体均是持续上升的趋势,尤其是1957年后上升明显,反映出代际风险累积效应。这种现象可能归因于以下几点,首先是氟暴露的效应所致,中国1980年后出生的人群儿童时期普遍接触含氟牙膏以降低龋齿风险,但过量的氟摄入可能诱发牙龈纤维增生,掩盖早期炎症症状[26],影响牙周病的早期预防和发现。其次是抗生素的滥用,1990年代儿童抗生素处方率高达70%~80%,引发口腔菌群耐药性增强,促进牙周致病菌定植。最后是行为习惯模式的转变,新型电子产品导致90后群体日均屏幕时间增加,低头体位减少唾液分泌,流速下降,削弱口腔自洁功能;电子烟的大量使用破坏口腔内环境和唾液组成[27];以及前面提到的外卖行业兴起,90后队列摄入高GI食物和高糖饮料大量增加,促进口腔酸性环境持续延长。此外也可能与中国近年来口腔医疗体系不断完善,晚出生队列更加重视口腔健康,且更易获得口腔疾病治疗有关,与80年代相比,2018年中国口腔医疗规模明显壮大,口腔医生人均占比从1/60 000~1/50 000增长到1/8 000[28],从而导致牙周病的发现率和就诊率更高。
BAPC预测模型显示,2022—2041年间中国牙周病发病率、患病率仍将处于快速上升状态,疾病负担仍然十分严峻,预测女性发病率、患病率均略高于男性。提示中国居民牙周病的预防和监测工作仍有很大改善空间,尤其是女性牙周病的防控。同时在制定公共卫生政策和健康保健方案时要注重牙周病的三早预防,根据牙周病的危险因素有针对性的开展预防教育工作。
研究的局限性在于 GBD 牙周病学定义与标准的牙周病新分类方法有所不同,对本研究可能产生一定的影响,因此结论仍需进一步研究探索。
综上所述,1992—2021年间中国牙周病的患病风险呈现波动上升的状态,中老年群体发病率和患病率最高,越晚出生队列,风险越高,可能与现代人群饮食习惯、对口腔疾病的重视程度和就医意愿的提高有关。未来我们应该持续关注中国人群牙周病的发病风险,尤其要为中老年人群个体提供更多的干预和治疗。同时做好健康教育,提高人群对牙周病危害的认知,开展及时有效且有针对性的预防措施,有效降低中国居民的牙周病疾病负担。
