Objective

To investigate the independent and joint effects of long-term exposure to air pollutants on the metabolic syndrome risk in adults.

Methods

Data were obtained from the surveillance and investigation of chronic diseases and risk factors in Hubei Province from 2018 to 2020. The daily average concentrations of PM2.5, PM10, NO2, SO2, O3, and CO at 1-kilometre spatial resolution were matched according to participants’ residential addresses, and the average levels of these air pollutants over the 1-year period before the survey conducted were calculated to assign individual-level exposure. Multilevel logistic regression and weighted quantile sum regression models were applied to evaluate the independent and joint effects of air pollutant exposure on the risk for metabolic syndrome, respectively.

Results

A total of 24 322 adults were included in the final analysis, with a metabolic syndrome prevalence of 39.8%. Higher exposure to PM2.5(OR=1.011, 95%CI:1.001-1.020) and O3 (OR=1.012, 95%CI: 1.005-1.018) were associated with an increased risk for metabolic syndrome. Subgroup analyses showed that exposure to PM2.5 was positively associated with risk for metabolic syndrome among rural residents (OR=1.018, 95%CI: 1.006-1.030), and the positive association between exposure to O3 and metabolic syndrome were found in adults beyond 60 years (OR=1.015, 95%CI: 1.008-1.023) and those physically active (OR=1.015, 95%CI: 1.008-1.022). The WQS regression revealed a positive association between long-term exposure to air pollutant mixtures and the metabolic syndrome risk (OR=1.188, 95%CI: 1.129-1.251), with O3, CO, and PM2.5 contributing 44.3%, 34.8%, and 20.8% to the joint effect, respectively.

Conclusion

Both long-term individual and joint exposures to air pollutants are positively associated with metabolic syndrome risk in adults, with the joint exposure showing a stronger adverse effect primarily driven by O3, CO, and PM2.5. To mitigate metabolic syndrome risk in adults, comprehensive prevention and control strategies should be implemented to reduce exposure to air pollutants.

目的

研究居住区空气污染物长期暴露与成人代谢综合征患病风险的独立和联合效应。

方法

基于2018—2020年湖北省慢性病及危险因素监测调查数据,根据研究对象居住区地址获取逐日1公里空气污染物数据,并计算研究对象在调查时点前一年的PM2.5、PM10、NO2、SO2、O3、CO年均浓度来估算空气污染物的暴露水平。采用多水平logistic回归和加权分位数和回归模型分别评估各类空气污染物暴露与代谢综合征患病风险关联的独立效应与联合效应。

结果

本研究共纳入24 322名成年人,代谢综合征患病率为39.8%。回归分析表明,较高的PM2.5OR=1.011,95% CI:1.001~1.020)和O3OR=1.012,95% CI:1.005~1.018)暴露与代谢综合征患病风险增加相关。亚组分析表明,在农村居民中(OR=1.018,95% CI:1.006~1.030),PM2.5暴露与代谢综合征患病风险呈正相关;在≥60岁人群(OR=1.015,95% CI:1.008~1.023)和身体活动充足人群中(OR=1.015,95% CI:1.008~1.022),O3暴露与代谢综合征患病风险呈正相关。联合效应分析发现,多种空气污染物长期混合暴露与代谢综合征患病风险呈正相关(OR=1.188,95% CI:1.129~1.251),其中O3、CO和PM2.5贡献权重分别为44.3%、34.8%和20.8%。

结论

空气污染物长期的单一暴露和混合暴露均与居民代谢综合征患病风险呈正相关,且混合暴露的危害效应更强,其中O3、CO和PM2.5的影响尤为显著。应采取综合防治措施对空气污染进行控制,降低居民代谢综合征患病风险。

目的

研究居住区空气污染物长期暴露与成人代谢综合征患病风险的独立和联合效应。

方法

基于2018—2020年湖北省慢性病及危险因素监测调查数据,根据研究对象居住区地址获取逐日1公里空气污染物数据,并计算研究对象在调查时点前一年的PM2.5、PM10、NO2、SO2、O3、CO年均浓度来估算空气污染物的暴露水平。采用多水平logistic回归和加权分位数和回归模型分别评估各类空气污染物暴露与代谢综合征患病风险关联的独立效应与联合效应。

结果

本研究共纳入24 322名成年人,代谢综合征患病率为39.8%。回归分析表明,较高的PM2.5OR=1.011,95% CI:1.001~1.020)和O3OR=1.012,95% CI:1.005~1.018)暴露与代谢综合征患病风险增加相关。亚组分析表明,在农村居民中(OR=1.018,95% CI:1.006~1.030),PM2.5暴露与代谢综合征患病风险呈正相关;在≥60岁人群(OR=1.015,95% CI:1.008~1.023)和身体活动充足人群中(OR=1.015,95% CI:1.008~1.022),O3暴露与代谢综合征患病风险呈正相关。联合效应分析发现,多种空气污染物长期混合暴露与代谢综合征患病风险呈正相关(OR=1.188,95% CI:1.129~1.251),其中O3、CO和PM2.5贡献权重分别为44.3%、34.8%和20.8%。

结论

空气污染物长期的单一暴露和混合暴露均与居民代谢综合征患病风险呈正相关,且混合暴露的危害效应更强,其中O3、CO和PM2.5的影响尤为显著。应采取综合防治措施对空气污染进行控制,降低居民代谢综合征患病风险。

Objective

To investigate the independent and joint effects of long-term exposure to air pollutants on the metabolic syndrome risk in adults.

Methods

Data were obtained from the surveillance and investigation of chronic diseases and risk factors in Hubei Province from 2018 to 2020. The daily average concentrations of PM2.5, PM10, NO2, SO2, O3, and CO at 1-kilometre spatial resolution were matched according to participants’ residential addresses, and the average levels of these air pollutants over the 1-year period before the survey conducted were calculated to assign individual-level exposure. Multilevel logistic regression and weighted quantile sum regression models were applied to evaluate the independent and joint effects of air pollutant exposure on the risk for metabolic syndrome, respectively.

Results

A total of 24 322 adults were included in the final analysis, with a metabolic syndrome prevalence of 39.8%. Higher exposure to PM2.5(OR=1.011, 95%CI:1.001-1.020) and O3 (OR=1.012, 95%CI: 1.005-1.018) were associated with an increased risk for metabolic syndrome. Subgroup analyses showed that exposure to PM2.5 was positively associated with risk for metabolic syndrome among rural residents (OR=1.018, 95%CI: 1.006-1.030), and the positive association between exposure to O3 and metabolic syndrome were found in adults beyond 60 years (OR=1.015, 95%CI: 1.008-1.023) and those physically active (OR=1.015, 95%CI: 1.008-1.022). The WQS regression revealed a positive association between long-term exposure to air pollutant mixtures and the metabolic syndrome risk (OR=1.188, 95%CI: 1.129-1.251), with O3, CO, and PM2.5 contributing 44.3%, 34.8%, and 20.8% to the joint effect, respectively.

Conclusion

Both long-term individual and joint exposures to air pollutants are positively associated with metabolic syndrome risk in adults, with the joint exposure showing a stronger adverse effect primarily driven by O3, CO, and PM2.5. To mitigate metabolic syndrome risk in adults, comprehensive prevention and control strategies should be implemented to reduce exposure to air pollutants.

代谢综合征(Metabolic Syndrome,MetS)作为一组以腹型肥胖、高血糖、高血压和血脂异常等集聚发病的代谢紊乱症候群,是心血管疾病和2型糖尿病等慢性非传染性疾病发生发展过程中的关键危险因素[1]。近年来,代谢综合征患病率持续上升,已成为公共卫生领域的重大挑战。国际糖尿病联盟数据显示,2021年全球成年人代谢综合征患病率达25%,患病人数超过10亿[2]。我国形势更为严峻,相关研究表明2017年我国20岁及以上人群患病率已达31.1%,且患病率仍在持续上升,防控压力持续增大[3]。随着我国城市化和工业化的快速推进,空气污染已成为威胁公众健康的重要环境危险因素[4]。现有流行病学研究表明,短期和长期暴露于PM2.5和O3等空气污染物均与代谢综合征及其组分(肥胖、高血压、糖尿病等)的患病风险呈显著正相关[5-7]。然而,我国空气污染物与代谢综合征关联研究在多种污染物联合暴露效应评估上仍存在局限。首先,现有研究多聚焦单一或少量的污染物模型,尚未有研究整合PM2.5、PM10、NO2、SO2、O3及CO六种典型空气污染物的多因素暴露评估体系,从而难以准确解析各类空气污染物长期暴露与代谢综合征患病风险的关联[8]。其次,各类大气污染物排放的同源性特征导致其在扩散中易发生光化学反应和气粒转化等物理化学过程,并通过氧化应激等路径产生毒性协同效应,但现有研究仍缺乏基于复合暴露视角下多种空气污染物混合暴露对代谢综合征患病风险联合效应的流行病学证据[9-11]。因此,本研究基于2018-2020年湖北省慢性病及危险因素监测调查数据,结合高分辨率PM2.5、PM10、NO2、SO2、O3及CO六种典型污染物数据,系统探究居住区空气污染物长期暴露与居民代谢综合征患病风险关联的独立效应与联合效应,识别其关联在不同人群间的差异,为代谢综合征的综合防控策略提供科学依据。
本研究采用湖北省2018—2020年慢性病及危险因素监测调查数据,该调查采用多阶段分层整群随机抽样方法在湖北省各地区纳入常住居民(村民)为研究对象。本研究纳入标准为:调查前12个月内在当地居住时间不少于6个月;非妊娠状态;无重大疾病导致行动障碍;无语言、认知或精神异常等影响调查准确性的情况。排除标准为:居住地址缺失;代谢综合征核心指标缺失;关键协变量缺失。抽样与研究对象筛选流程见图1。项目通过了国家慢性和非传染性疾病预防控制中心伦理委员会审查(审批号:2018001),所有研究对象均在参与调查前签署知情同意书。
采用中国成人慢性病及危险因素监测调查问卷,由统一培训并考核合格的调查员进行面对面问卷调查。问卷内容包括年龄、性别、文化程度等基本信息,吸烟、饮酒、身体活动等生活方式信息,以及慢性病患病状况和血糖、血压、血脂控制措施等病史信息。现场调查还包括身高、体重、腰围、血压等身体测量和空腹静脉血采集。血液采集要求调查对象空腹8~12 h,于次日上午7~9点在监测点各基层医疗机构采集静脉血10 ml,统一运输至实验室测量空腹血糖和空腹血脂。空腹血糖、血脂均由通过实验室性能验证的监测点实验室负责检测。调查使用自主开发的平板电脑调查系统对问卷、身体测量等信息进行实时录入,经由湖北省疾病预防控制中心校对并复核。
(1)代谢综合征诊断标准:采用美国国家胆固醇教育计划成人治疗组第三次指南(The National Cholesterol Education ProgramAdult Treatment PanelIII,NCEP-ATP III)诊断标准,符合以下五项指标中三项及以上者,即可诊断为代谢综合征:①腹型肥胖:腰围≥90 cm(男性)或≥80 cm(女性);②血压升高:收缩压≥130 mmHg或舒张压≥85 mmHg,或已确诊高血压并接受降压治疗者;③空腹血糖升高:FPG≥5.6 mmol/L,或已确诊糖尿病并接受降糖治疗者;④甘油三酯升高:TG≥1.7 mmol/L,或正在接受降脂药物治疗者;⑤高密度脂蛋白胆固醇降低:HDL-C<1.03 mmol/L(男性)或<1.29 mmol/L(女性)[12]。(2)空气污染物暴露:空气污染数据来自研究团队基于地面监测数据、遥感观测数据、气象资料和土地利用信息等数据反演的1公里空间分辨率逐日空气污染物浓度数据集[13]。基于研究对象居住区地理编码,将网格化污染物数据匹配至个体居住地,获取2015—2020年期间PM2.5、PM10、NO2、SO2、O3和CO的日均浓度值,分别计算研究对象在调查时点前一年及前三年各类污染物平均浓度,以此测度居民个体的空气污染物暴露水平。(3)协变量:基于既往研究[14],协变量包括年龄(<60岁、≥60岁)、性别(男性、女性)、民族(汉族、少数民族)、文化程度(高中以下、高中及以上)、婚姻状况(已婚/同居、未婚/丧偶/离婚/独居)、职业状况(在职、无业)和居住地(农村、城市)等社会人口学特征,以及吸烟(是、否)、饮酒(是、否)和身体活动(不足、充足)等生活方式变量。有吸烟史者定义为吸烟,包含当前吸烟者及既往吸烟者;在过去一年内饮酒次数≥1次者定义为饮酒;一周内中等强度活动时间不足150 min、或高等强度活动时间不足75 min、或等量活动时间不足者定义为身体活动不足[15]
使用R 4.2.2软件进行统计分析,计量资料以均数和标准差表示,计数资料以频数和构成比表示,使用t检验和χ2检验进行组间差异比较。使用皮尔逊相关系数评估空气污染物的相关性。鉴于同一社区/村内居民空气污染暴露存在层级聚集效应,本研究采用多水平logistic回归模型,分析空气污染物暴露与居民代谢综合征患病风险的关联。由于PM2.5与PM10呈现高度共线性,PM10(方差膨胀因子>10)未纳入后续分析。以调查前一年的五种空气污染物年均浓度作为解释变量,模型1调整性别、年龄、民族、文化程度和婚姻状况,并为社区/村水平添加随机截距;模型2在模型1基础上,进一步调整职业状况和身体活动;模型3在模型2基础上,进一步调整吸烟和饮酒。为验证模型3结果的稳健性,采用如下敏感性分析:①使用调查前三年的平均空气污染物浓度测度个体暴露水平(模型3a);②采用联合声明(Joint Interim Statement,JIS)标准诊断代谢综合征(模型3b);③排除肥胖(身体质量指数[Body Mass Index,BMI]≥28kg/m2)的研究对象(模型3c)。研究进一步通过亚组分析探索空气污染物暴露和代谢综合征患病风险关联在不同人群中的差异,通过Z检验来评估各亚组内的效应差异是否存在统计学意义。采用加权分位数和(Weighted Quantile Sum,WQS)回归模型探索五种空气污染物对居民代谢综合征患病风险的联合效应以及各种污染物的贡献权重。分析均采用双侧检验,检验水准α=0.05。
共纳入24 322名研究对象进行分析,其中<60岁人群16 884人(69.4%),女性14 462人(60.2%),汉族21 179人(87.1%),高中以下文化程度18 818人(77.4%),已婚/同居者22 135人(91.0%),在职人群18 029人(75.2%),农村居民19 905人(81.8%)。研究样本中代谢综合征患病率为39.8%,研究对象基本特征见表1。代谢综合征患者居住区的PM2.5、NO2、SO2、O3和CO暴露水平显著高于非代谢综合征患者,见表2。部分空气污染物间存在较强相关性,见图2
模型1对性别、年龄、民族、文化程度和婚姻状况进行控制,PM2.5和O3暴露均与代谢综合征风险呈显著正相关;模型2进一步调整职业状况和身体活动后,PM2.5和O3暴露仍与代谢综合征患病风险呈显著相关;模型3进一步调整吸烟和饮酒后,PM2.5OR=1.011,95% CI:1.001~1.020)和O3暴露(OR=1.012,95% CI:1.005~1.018)与代谢综合征患病风险的关联保持稳健。见表3
当采用调查前三年平均空气污染物浓度评估个体水平,及采用JIS标准诊断代谢综合征时,PM2.5和O3暴露与代谢综合征患病风险的关联均与模型3一致;排除肥胖人群(BMI≥28 kg/m2)后,CO暴露(OR=2.129,95% CI:1.008~4.497)与代谢综合征患病风险呈显著正相关,PM2.5OR=1.013,95% CI:1.003~1.022)和O3暴露(OR=1.010,95% CI:1.004~1.016)与代谢综合征患病的正向关联与模型3保持一致。见表4
PM2.5暴露(OR=1.018,95% CI:1.008~1.029)与农村居民的代谢综合征患病风险呈显著正相关,与城市居民的患病风险的关联不具有统计学意义。O3暴露与身体活动充足人群(OR=1.015,95% CI:1.008~1.022)及≥60岁人群(OR=1.015,95% CI:1.008~1.023)的代谢综合征患病风险呈显著正相关,但与身体活动不足人群和<60岁人群的代谢综合征患病风险的关联不显著。未观察到其他空气污染物与代谢综合征患病风险关联的人群异质性。见图3
WQS回归结果显示,空气污染物混合暴露与居民代谢综合征患病风险呈显著正相关(OR=1.188,95% CI:1.129~1.251),且联合效应强于单一污染物的独立效应。其中,O3、CO、PM2.5是混合暴露的主要风险因素,其贡献占比分别为44.3%、34.8%和20.8%,三者累计权重约为99.8%,而SO2与NO2的累计权重不足0.2%。见图4
本研究基于湖北省慢性病及危险因素监测调查数据,分析了居住区空气污染物长期的单一暴露和混合暴露与居民代谢综合征患病风险的关联。结果显示,PM2.5和O3的单一暴露及空气污染物的混合暴露与代谢综合征患病风险呈正相关,O3、CO和PM2.5在混合暴露中占主要权重。亚组分析进一步显示,PM2.5暴露与农村居民代谢综合征患病风险呈正相关,O3暴露与≥60岁及身体活动充足人群的代谢综合征患病风险呈正相关。
PM2.5长期暴露与代谢综合征患病风险的关联与现有研究结果一致,一项基于14 097名中国居民的横断面研究显示,PM2.5暴露与代谢综合征患病风险呈正相关[16]。而O3暴露与代谢综合征患病风险的正向关联和既往研究结果存在区域异质性,一项基于3 499名泰国职业人群的研究表明O3暴露与代谢综合征患病风险不存在显著关联[5],另一项基于广东省6 628名成人研究也未发现O3暴露与代谢综合征患病风险之间的关联[17]。这种效应异质性可能受膳食结构、体力活动模式等地理文化差异影响[18]。相关研究表明,空气污染与代谢综合征的关联可能通过多种潜在机制实现。PM2.5可以通过激活下丘脑垂体甲状腺轴来扰乱甲状腺激素稳态,导致甲状腺激素分泌异常,这种激素失衡可能通过降低基础代谢率、促进脂质蓄积和胰岛素抵抗等途径增加代谢综合征的患病风险[19];而O3可通过激活肺泡巨噬细胞进行一系列反应,诱发系统性胰岛素抵抗,进一步增加代谢综合征的患病风险[20]
研究发现空气污染物暴露与代谢综合征患病风险的关联在不同人群中存在差异。PM2.5暴露与代谢综合征患病风险的正向关联仅在农村居民中显著,PM2.5暴露与代谢综合征患病风险关联的城乡差异可能由于污染来源的不同,农村地区PM2.5主要来源于生物质燃烧,相较于城市地区PM2.5,其含有更高水平的持久性自由基和多环芳烃,这些成分可通过氧化应激和炎症反应等影响代谢,从而增加农村居民代谢综合征患病风险[21]。O3暴露在≥60岁人群和身体活动充足人群中与代谢综合征患病风险呈正相关,O3作用的年龄差异及在不同身体活动强度人群中的差异与Brook等提出的氧化应激级联反应机制结论相一致[20],老年人血清中抗氧化物水平普遍低于年轻人,对O3诱导的氧化损伤更为敏感;运动引起身体氧化应激压力的增加,进而加剧了O3的影响,使身体活动充足人群更易患病。
不同大气污染物的暴露水平具有一定相关性。PM2.5与SO2、NO2、CO等主要来源于工业燃烧、机动车尾气等,排放的同步性导致其浓度趋势趋同[22];臭氧的光化学生成依赖氮氧化物与挥发性有机物反应,同时高浓度NO也会通过化学反应消耗O3,导致其浓度相互关联[23]。空气污染物暴露对居民代谢综合征患病风险存在显著的联合效应,其中O3、CO、PM2.5为主要风险因素,且混合暴露风险高于单一暴露风险。其可能的机制为O3介入PM2.5的二次生成历程,推动其浓度上升并增强毒性[24];同时,O3暴露引发的机体细胞因子分泌增加和线粒体功能障碍可放大PM2.5长期暴露导致的持续性炎症反应和氧化应激,使暴露于PM2.5和O3的危害效应大于它们各自效应的总和[25]。此外,CO可能通过与血红蛋白结合阻碍氧气运输,造成组织缺氧,迫使细胞依赖低效的无氧糖酵解,引发糖代谢紊乱和脂代谢异常,从而增加代谢综合征患病风险[26]
研究基于大规模人群队列数据揭示了空气污染物长期暴露与居民代谢综合征患病风险之间的关联,并且分析了空气污染物对代谢综合征患病风险的独立和联合效应,对我国空气环境优化及代谢综合征防控策略的制定具有重要意义。本研究存在局限性。首先,本研究仅纳入个体居住区污染物暴露,未评估个体在工作场所、通勤路径及室内环境的暴露,可能造成测量偏差,未来研究可结合可穿戴设备获取个体活动轨迹,从而更准确的测量个体空气污染暴露。其次,研究对象局限于湖北省单一区域,由于不同地区空气污染物组分及人群易感性分布存在空间异质性,研究结论可能难以推广至其他地区,未来需开展多中心研究验证结果的普适性。最后,研究仅探讨不同类型空气污染物浓度与代谢综合征患病风险的关联,未涉及PM2.5等颗粒物内部组分的健康效应,未来需开展更细致的研究来探讨颗粒物组分对代谢综合征的影响。
综上所述,空气污染物长期暴露与代谢综合征患病风险显著正相关,且多种污染物混合暴露的联合效应具有更强的健康危害。研究结果有望为多污染物协同防控策略的优化提供科学依据,推动空气质量管理与慢性病防控目标的融合,降低居民代谢综合征的患病风险。
