目的

分析1990—2019年中国≥70岁老年人阑尾炎疾病负担的相关数据，描述其现状及流行趋势，为探索中国老年人阑尾炎防治策略提供科学依据。

方法

利用2019年全球疾病负担数据，选取1990—2019年中国≥70岁老年人群阑尾炎的发病率、死亡率和伤残调整寿命年（disability～adjusted life years，DALY）进行分析，并采用joinpoint回归模型计算相应的平均年度变化百分比（average annual percentage change，AAPC）。

结果

1990—2019年，我国≥70岁老年人阑尾炎发病率呈逐年上升趋势，2000—2005年上升最为明显，同期死亡率和DALY率则呈下降趋势，2004年开始死亡率和DALY率出现大幅下降，但自2008年以后下降趋势有所减缓。在各指标性别和年龄组构成中，70～74岁年龄段的发病率最高，且女性发病率高于男性；而在85～89岁年龄段死亡率和DALY率最高，且男性的死亡率和DALY率均高于女性。

结论

自1990年以来，我国≥70岁老年人阑尾炎死亡及DALY负担逐渐减轻，但发病率仍处于较高水平，因此，未来中国对老年人群体应采取多种方式防治，以降低≥70岁老年人阑尾炎的疾病负担。

Objective

To analyze relevant data on the burden of appendicitis among individuals aged 70 and above in China from 1990 to 2019, describe its current status and epidemiological trends, and provide scientific evidence for exploring prevention and treatment strategies for appendicitis in the elderly population in China.

Methods

Utilizing the 2019 Global Burden of Disease data, this study analyzed the incidence, mortality, and disability-adjusted life years (DALY) of appendicitis in individuals aged 70 and above in China from 1990 to 2019. The Join point regression model was used to calculate the average annual percentage change (AAPC).

Results

From 1990 to 2019, the incidence of appendicitis among individuals aged 70 and above in China exhibited a year-on-year increasing trend, with the most significant rise occurring between 2000 and 2005. In contrast, both the mortality rate and DALY rate showed a declining trend during the same period, with a substantial decrease starting in 2004. However, the decline rate has slowed down since 2008. Among different gender and age group compositions, the highest incidence was observed in the 70-74 age group, with a higher incidence in females compared to males. Conversely, the highest mortality and DALY rates were found in the 85-89 age group, with both rates being higher in males than in females.

Conclusion

Since 1990, the burden of mortality and DALY due to appendicitis among individuals aged 70 and above in China has gradually decreased, yet the incidence remains at a relatively high level. Therefore, it is essential for China to adopt various prevention and treatment strategies for the elderly population to reduce the disease burden of appendicitis in those aged 70 and above.

随着全球人口年龄分布发生变化，老龄化进程加快已势不可挡^[1]，预期寿命的延长带来更多的社会活动，但是如果健康状况不佳的时间也延长，卫生医疗系统将会面临巨大压力。研究表明从1990—2019年，全球≥70岁人口规模有所增加，其中欧洲、亚洲和美国的≥70岁和≥90岁人群增长速度最快^[2]。中国已进入深度老龄化社会，老年人阑尾炎发病率逐年上升^[3]。急性阑尾炎往往发生在幼儿及青少年时期，老年患者急性阑尾炎症状不典型^[4]，且多合并基础疾病，如高血压、糖尿病等，整体身体状态下降，疼痛敏感性降低，免疫功能减退，穿孔率高，对手术的耐受性和接受度较差，手术风险较高等^[5]。因此，监测老年人的身体健康状况对于缓解社会压力十分重要。本研究利用1990—2019年全球疾病负担研究相关数据，描述中国≥70岁老年人阑尾炎疾病负担现状及流行趋势，为探索中国老年人阑尾炎防治策略提供科学依据。

1 资料与方法

1.1 数据来源

本研究数据来源于2019年全球疾病负担（Global Burden of Disease 2019，GBD 2019）研究数据库，GBD 2019拥有全球层面、区域层面以及204个国家和领土上的疾病负担相关统计数据^[6-7]，其中中国地区的数据主要来源于疾病监测点系统、妇幼监测系统、中国疾病预防控制中心死因报告系统、中国肿瘤登记以及来自香港和澳门的报告^[8]。阑尾炎所对应的国际疾病分类10（ICD-10）编码为K35～K38。

1.2 疾病负担评估方法

本研究按照GBD2019数据库现存的年龄分段方法进行分层，将≥70岁的老年人分为：70～74、75～79、80～84、85～89、90～94和≥95岁。采用发病例数、发病率、死亡例数、死亡率、伤残调整寿命年（disability～adjusted life years，DALY）数据来综合评估老年人阑尾炎疾病负担，其中DALY是指从发病到死亡所损失的全部健康寿命年。采用年度变化百分比（annual percent change, APC）、平均年度变化百分比（average annual percentage change，AAPC）分析各指标变化趋势。

1.3 统计学方法

本研究构建joinpoint回归模型计算出1990—2019年中国老年人阑尾炎相应指标的AAPC及95%置信区间（confidence interval，CI），分析其长期变化趋势^[9]。利用Excel 2016软件对相关数据进行筛选整理，RStudio 4.2.3进行绘图。检验水准α=0.05。

2 结果

2.1 1990—2019年中国老年人阑尾炎疾病负担分析

2.1.1 发病负担

1990年中国≥70岁老年人阑尾炎的发病人数为7 481例，发病率为19.55/10万。与1990年相比，2019年中国≥70岁老年人阑尾炎的发病人数增长了548.36%，同期发病率增长了129.75%；老年女性阑尾炎发病率始终高于男性。

2.1.2 死亡负担

1990—2019年我国老年人死亡率均呈下降趋势，由3.85/10万下降至0.94/10万，下降了75.6%；男性患者死亡率超过女性。

2.1.3 DALY

中国的DALY从1990年的21 332.05人年降至2019年的13 570.69人年，下降了36.4%；DALY率由1990的55.76/10万降至12.57/10万，下降了77.5%；男性DALY率高于女性。见表1。

2.2 1990—2019年中国≥70岁老年人阑尾炎疾病负担变化趋势

1990—2019年中国老年人阑尾炎发病率的AAPC值为2.93（95%CI：2.81～3.03），总体上呈上升趋势，尤其在2000—2005年上升最为明显（APC=10.66，95%CI：10.26～11.06）。死亡率的AAPC值为-4.84（95%CI：-4.92～-4.74），总体呈现下降趋势，并在2004—2009年下降最为明显（APC=-8.62，95%CI：-9.28～-8.19）。DALY率的AAPC值为-5.10（95%CI：-5.19～-5.00）。

2.2.1 不同性别老年人阑尾炎疾病负担变化趋势

男女性的发病率趋势与总发病率趋势保持一致，呈先下降后上升趋势，在1995年降至最低点后，持续上升，其中2000—2005年上升最为迅速，男性APC为10.06（95%CI：3.69～10.47），女性APC为11.16（95%CI：10.72～11.58）。见图1A。

1990—2019年男女性阑尾炎死亡率均呈明显下降趋势，其中男性在2004—2008年下降最为迅速，APC为-8.42（95%CI：-9.13～-2.41），女性在2005—2008年下降最为显著，APC为-10.75（95%CI：-11.31～-9.71）。DALY率与死亡率变化趋势完全一致。见图1B、1C。

2.2.2 不同年龄段老年人阑尾炎疾病负担变化趋势

1990—2019年中国70～84岁年龄段老年人阑尾炎的发病率呈上升趋势，其中70～74岁年龄段上升态势最快（AAPC=4.56，95%CI: 4.40～4.70），而≥85岁老年人阑尾炎发病率呈下降趋势，且变化差异具有统计学意义。1990年75～79岁年龄段的发病率略高于70～74岁年龄段的发病率，但随着时间的推移，2019年70～74岁年龄段的发病率近乎为75～79年龄段的两倍。总体看来，1990—2019年中国各年龄段的死亡率及DALY率均呈下降趋势，下降趋势在70～74岁年龄段最为显著（死亡率：AAPC= -5.92，95%CI:-6.01～-5.81；DALY率：AAPC=-5.63，95%CI:-5.72～-5.51）。然而，随着年龄的增长，下降态势有所减缓。性别角度方面，男性与女性的阑尾炎发病率、死亡率和DALY率的整体变化趋势与总体变化趋势大致相同；不同的是，在85～94岁年龄段女性发病率呈上升趋势，且具有统计学意义，而≥95岁呈下降趋势。此外，2019年大部分年龄段的女性发病率、死亡率、DALY率的AAPC均高于同龄男性。见表2、图2。

本研究通过GBD 2019数据库分析了我国1990—2019年≥70岁老年人阑尾炎疾病负担及变化趋势，结果显示，2019年全球≥70岁老年人阑尾炎的较1990年增长了39.69%，而我国≥70岁老年群体阑尾炎发病率由1990年的23.81/10万上升至2019年的60.95/10万，增幅为129.75%，整体发病率的增长明显快于全球，可以预见的是我国这一年龄段阑尾炎的发病率将会继续增加。我国整体发病负担的增加可能与人口增长和老龄化进程加剧有关，研究表明世界人口正在老龄化，全球≥70岁人口比例从1990年的3.8%上升至2019年的6%^[10]，其中增长速度最快的为亚洲、欧洲、美国^[2]，而中国作为亚洲人口超级大国，已经步入深度老龄化社会^[11]，老龄化人口的不断增加会给社会带来巨大的医疗卫生需求和压力，而随着社会的发展以及医疗水平的进步，反过来进一步加重了老龄化^[2]，2019年全球几乎所有的国家≥70岁人口的预期寿命比1990年长了2年以上^[12]。2019年中国≥70岁老年人死亡率及DALY率分别下降了75.58%、77.46%，2004—2009年死亡率及DALY率下降速度最快，这可能与我国“十五”“十一五”时期新型农村合作医疗制度建设快速推进有关，大大改善了贫困地区和低收入水平地区居民的健康状况。同一时期，我国城市化进程的快速推进^[13]，农村人口大量迁移至城市，老年人能享受到更好的医疗服务和健康管理，包括定期的健康检查、疾病预防和健康教育，这有助于老年人及时发现并处理健康问题，从而使得死亡风险明显降低。

从年龄段来看，随着年龄的增长，死亡率也随之上升。85～89岁年龄段老年人阑尾炎死亡率最高，这可能与老年人免疫力低下、痛阈较高、炎性反应不易控制等有很大联系等因素密切相关^[14]。既往研究表明，年龄是死亡的重要危险因素，随着年龄的增长，死亡风险不断增加，而与非穿孔的老年人相比，穿孔的老年人死亡风险更高^[15～16]，穿孔相关的危险因素包括男性、发热≥38℃等^[17]，这可能解释了本研究中老年男性患者死亡率高于女性的现象。部分老年患者出现症状时，往往将其归因于消化不良或者便秘等，导致病情被掩盖，直至病情恶化，进而增加了死亡风险^[18]。另外，随着年龄的增长，阑尾结构发生变化，组织黏膜变薄，蠕动能力减弱，动脉血供减少，进而发生阑尾炎时易导致穿孔坏死^[19]。DALY率与死亡率的下降反映了医疗管理和治疗水平的提高，特别是在诊断和手术方面。近年来，随着影像学与评分系统的改进与应用，阑尾炎的诊断准确性得到提高，误诊率降低，从而减少了延误治疗，进而降低了穿孔的发生率^[20]。手术方式的演进与围手术期的全程管理也降低了术后并发症的发生率。据报道^[21-22]，与传统的开放性阑尾切除术相比，腹腔镜下阑尾切除术具有更低的术后感染率、更短的住院时长、更少的术后并发症以及更高的术后生活质量。相比于小儿及年轻患者，老年患者更少见，症状更不典型，合并症更多，死亡率更高，术后并发症发生率更高，住院时间更长等。因此随着人口老龄化进程的加剧，我国老年人阑尾炎的疾病负担也不容乐观。

本研究存在一定的局限性：首先，GBD 2019数据并非全人群数据，与真实世界数据存在一定差异；其次，GBD 2019仅能反映全国范围的疾病负担变化，缺乏我国各省市的具体数据，这导致本研究与实际情况有一定差距，对我国不同经济水平地区老年人患阑尾炎的疾病负担还需进一步深入分析；最后，本研究中≥70岁人群的数据未进行年龄标准化，人口年龄结构的变化可影响阑尾炎负担的变化，其中一个主要因素是人口老龄化，人口老龄化转型的纳入可以反映现实中阑尾炎负担的变化趋势。

综上所述，过去30年我国≥70岁老年人阑尾炎死亡率及DALY率明显下降，而发病率仍处于较高水平，男性的疾病负担高于女性，整体疾病负担仍不容乐观。中国老年人中，女性更容易患阑尾炎，加之中国人口基数大且人口老龄化问题严重，随着患者年龄的增长，老年人阑尾炎的死亡率及DALY率显著升高，导致老年人生活质量下降。因此，在未来我们应积极针对此类人群开展健康教育，提高老年人对阑尾炎症状的认识和及时就医的意识，发挥社区的宣传作用，倡导健康的生活方式，建立完善的防治监测体系，以降低我国老年人阑尾炎的疾病负担。
