Objective

To understand the current state of awareness regarding syphilis prevention and control among MSM college students in Guiyang and to analyze related factors.

Methods

From September 2022 to October 2023, an anonymous electronic questionnaire survey was conducted among MSM college students recruited through MSM social organizations in Guiyang. The Chi-squared test was used to compare the awareness rates of syphilis prevention and control knowledge among MSM college students with different characteristics, and a multifactorial logistic regression model was employed to analyze the related factors affecting awareness.

Results

A total of 567 valid questionnaires were collected. The awareness rate of syphilis prevention and control knowledge among the study participants was 78.7%. Multifactorial analysis indicated that awareness of syphilis prevention and control knowledge among MSM college students was positively associated with urban household registration (OR=2.554, 95%CI: 1.662-3.923), medical-related majors (OR=3.025, 95%CI: 1.232-7.432), average monthly expenditure ≥ 2 000 yuan (OR=3.416, 95%CI: 1.565-7.455), and having received peer education in the past year (OR=2.554, 95%CI: 1.662-3.923); it was negatively associated with having engaged in sexual behavior after drinking in the past six months (OR=0.501, 95%CI: 0.289-0.868).

Conclusion

The current awareness rate of syphilis prevention and control knowledge among MSM college students in Guiyang falls short of the target rate of 95%. Future efforts should focus on enhancing educational outreach regarding syphilis prevention and control knowledge among MSM college students, particularly targeting subgroups with lower awareness rates. Interventions could include or combine peer education strategies for this population.

目的

了解贵阳市男男性行为（men who have sex with men，MSM）大学生的梅毒防治知识知晓现状，分析其相关因素。

方法

2022年9月—2023年10月，通过贵阳市MSM社会组织招募MSM大学生进行匿名电子问卷调查。采用χ2检验对不同特征的MSM大学生的梅毒防治知识知晓率进行比较，采用多因素logistic回归模型分析梅毒防治知识知晓情况的相关因素。

结果

本研究共回收有效问卷567份。研究对象的梅毒防治知识知晓率为78.7%。多因素分析结果显示，MSM大学生的梅毒防治知识知晓情况与城镇户籍（OR=2.554, 95%CI:1.662～3.923）、医学类专业（OR=3.025, 95%CI:1.232～7.432）、月均消费≥2 000元（OR=3.416, 95%CI:1.565～7.455）、最近1年接受过同伴教育（OR=2.554, 95%CI:1.662～3.923）呈正向关系；与最近6个月有酒后性行为（OR=0.501, 95%CI:0.289～0.868）呈负向关系。

结论

当前贵阳市MSM大学生梅毒防治知识知晓率与95%的知晓率目标尚存在一定的差距。未来应该加强对MSM大学生梅毒防治知识的宣传教育，尤其是针对其中知晓率较低的亚群，可尝试采取或者结合同伴教育对该人群进行干预。

目的

了解贵阳市男男性行为（men who have sex with men，MSM）大学生的梅毒防治知识知晓现状，分析其相关因素。

方法

2022年9月—2023年10月，通过贵阳市MSM社会组织招募MSM大学生进行匿名电子问卷调查。采用χ2检验对不同特征的MSM大学生的梅毒防治知识知晓率进行比较，采用多因素logistic回归模型分析梅毒防治知识知晓情况的相关因素。

结果

本研究共回收有效问卷567份。研究对象的梅毒防治知识知晓率为78.7%。多因素分析结果显示，MSM大学生的梅毒防治知识知晓情况与城镇户籍（OR=2.554, 95%CI:1.662～3.923）、医学类专业（OR=3.025, 95%CI:1.232～7.432）、月均消费≥2 000元（OR=3.416, 95%CI:1.565～7.455）、最近1年接受过同伴教育（OR=2.554, 95%CI:1.662～3.923）呈正向关系；与最近6个月有酒后性行为（OR=0.501, 95%CI:0.289～0.868）呈负向关系。

结论

当前贵阳市MSM大学生梅毒防治知识知晓率与95%的知晓率目标尚存在一定的差距。未来应该加强对MSM大学生梅毒防治知识的宣传教育，尤其是针对其中知晓率较低的亚群，可尝试采取或者结合同伴教育对该人群进行干预。

Objective

To understand the current state of awareness regarding syphilis prevention and control among MSM college students in Guiyang and to analyze related factors.

Methods

From September 2022 to October 2023, an anonymous electronic questionnaire survey was conducted among MSM college students recruited through MSM social organizations in Guiyang. The Chi-squared test was used to compare the awareness rates of syphilis prevention and control knowledge among MSM college students with different characteristics, and a multifactorial logistic regression model was employed to analyze the related factors affecting awareness.

Results

A total of 567 valid questionnaires were collected. The awareness rate of syphilis prevention and control knowledge among the study participants was 78.7%. Multifactorial analysis indicated that awareness of syphilis prevention and control knowledge among MSM college students was positively associated with urban household registration (OR=2.554, 95%CI: 1.662-3.923), medical-related majors (OR=3.025, 95%CI: 1.232-7.432), average monthly expenditure ≥ 2 000 yuan (OR=3.416, 95%CI: 1.565-7.455), and having received peer education in the past year (OR=2.554, 95%CI: 1.662-3.923); it was negatively associated with having engaged in sexual behavior after drinking in the past six months (OR=0.501, 95%CI: 0.289-0.868).

Conclusion

The current awareness rate of syphilis prevention and control knowledge among MSM college students in Guiyang falls short of the target rate of 95%. Future efforts should focus on enhancing educational outreach regarding syphilis prevention and control knowledge among MSM college students, particularly targeting subgroups with lower awareness rates. Interventions could include or combine peer education strategies for this population.

梅毒是由梅毒螺旋体感染引起的性传播疾病，2022年我国的梅毒报告发病数在甲乙类传染病中居第三位（441 159例）[1]。近年来，贵州省梅毒流行呈现快速上升的趋势[2]，且有年轻化的倾向[3]。男男性行为人群（men who have sex with men, MSM）是梅毒的易感人群[4-5]；其中大学生处于性活跃期，在性观念和性行为越来越开放的同时，性病和艾滋病的防护意识尚未完全形成[6]，使得MSM大学生成为受到梅毒威胁的高风险人群[7]。提高梅毒防治知识知晓是预防梅毒感染的重要措施[8]。本研究以贵阳市MSM大学生为研究对象，调查其梅毒防治知识知晓情况并探索相关影响因素，为该人群梅毒防治策略和措施的制定提供依据。
1 对象与方法

1.1 研究对象
依托贵阳市MSM社会组织（贵阳市彩艾阳光公益中心）进行研究对象的招募。纳入标准：（1）贵阳市本专科院校在读学生；（2）曾发生过MSM；（3）提供书面知情同意。排除标准：有认知障碍或交流障碍者。本研究经贵州医科大学伦理委员会审查通过（编号：2023伦审第[43]号）。
1.2 方法
2022年9月—至2023年10月，由社会组织工作人员通过现场招募、滚雪球法、网络招募等方式招募研究对象进行现场调查。调查员在调查前经过集中培训，统一调查方法和指导语。符合纳入排除标准的调查对象签署知情同意书后，使用电子设备扫描二维码，自行匿名填写调查问卷。由调查员对问卷进行完整性检查和逻辑审核，及时对漏填误填的条目进行补充纠正。
调查内容包括：（1）社会人口学信息：年龄、生源地、户籍类型、文化程度、专业类型、居住情况、月均消费；（2）性行为特征：性取向、初次性行为年龄、性伴数、最近6个月是否有男男性行为等；（3）梅毒防治知识知晓情况。梅毒防治知识知晓根据中国疾病预防控制中心开发的调查问卷中的8个问题进行评估[9]，答对题数≥6记为知晓。
1.3 统计分析
从问卷星平台导出数据到Excel 2019，采用SPSS 21.0统计软件进行数据整理和统计分析。定量资料的统计描述采用（均数±标准差），定性资料的统计描述采用频数和构成比。采用χ2检验比较不同特征的MSM大学生的梅毒防治知识知晓率有无差异，采用多因素logistic回归分析梅毒防治知识知晓情况的相关因素。检验水准α=0.05（双侧）。
2 结果

2.1 调查对象的社会人口学与性行为特征
本研究共有567名MSM大学生完成问卷，平均年龄为（21.1±2.3）岁。调查对象以年龄为20～23岁（344名，60.7%）、生源地为本省（514名，90.7%）、城镇户籍（380名，67.0%）、汉族（400名，70.5%）、专科/本科文化程度（510名，89.9%）、理工类专业（348名、61.4%）、居住于住校（468名，82.5%）、月均消费<2 000元（472名，83.2%）者为主。见表1。
性行为特征方面，绝大部分调查对象为同性恋、双性恋、性取向不确定人群等性少数人群547名（96.5%），初次性行为年龄≥18岁者368名（64.9%），性伴数<10人者497名（87.7%）。最近6个月研究对象中，发生过MSM的有404名（71.3%）、发生过男男群交行为的有50名（8.8%）、最近一次MSM使用了安全套的有342名（60.3%）、发生过男男商业性行为的有27名（4.8%）、发生过异性性行为的有28名（4.9%）、发生过酒后性行为的有88名（15.5%）。60名（10.6%）MSM大学生在性行为过程中使用过Rush。最近1年，研究对象被诊断过性病者14名（2.5%）、接受过安全套发放/HIV咨询检测者379名（66.8%）、接受过同伴教育者373名（65.8%）。见表1。
2.2 梅毒防治知识知晓情况
调查对象梅毒防治知识总体知晓率为78.7%（446/567）。知晓率最低的三个条目分别是：“与梅毒病人一起吃饭、握手等日常接触会传播梅毒吗？”占64.7%（367/567）、“一个看上去健康的人会是梅毒病人吗？”占69.3%（393/567）、“梅毒会增加艾滋病的传播吗？”占76.7%（435/567）。见表2。
2.3 梅毒防治知识知晓的影响因素分析
2.3.1 单因素分析
结果显示，户籍类型、专业类别、月均消费（元）、初次性行为年龄（岁）、最近6个月是否有MSM、最近6个月是否发生过酒后性行为、最近1年是否接受过安全套发放/HIV咨询检测、最近1年是否接受过同伴教育与MSM大学生梅毒防治知识知晓情况相关（P均<0.05）。见表1。
2.3.2 多因素分析
以调查对象是否知晓梅毒防治知识为因变量（知晓=1，不知晓=0），将单因素分析中P<0.10的变量纳入多因素logistic回归模型。结果显示，相比于户籍为农村、专业为文史类、月均消费<2 000元和最近1年接受过同伴教育的MSM大学生，户籍类型为城镇（OR=2.554, 95%CI:1.662～3.923）、专业为医学类（OR=3.025, 95%CI:1.232～7.432）、月均消费≥2 000元（OR=3.416, 95%CI:1.565～7.455）、最近1年接受过同伴教育（OR=2.554, 95%CI:1.662～3.923）者梅毒防治知识知晓率更高；相比于最近6个月无酒后性行为的MSM大学生，最近6个月发生过酒后性行为（OR=0.501, 95%CI:0.289～0.868）者梅毒防治知识知晓率更低。见表3。
3 讨论

本研究结果表示，贵阳市MSM大学生梅毒防治知识知晓率为78.7%，高于马璐[10]等对贵阳市MSM（73.5%）的研究结果和鲁建瑜[11]等对海南省MSM（65.9%）的研究结果，但低于孙燕鸣[12]等对北京市MSM（84.1%）和李艳芬[13]等对上海市MSM（88.6%）的研究结果，与《中国预防与控制梅毒规划（2010～2020年）》[10]中提出的在2020年底实现MSM人群梅毒防治知晓率达到95%的目标尚存在一定的差距。需要在梅毒防治知识的健康教育和宣传上投入更多的资源。此外，与普通大学生进行比较，本研究的结果高于赵文鹏[14]对某高校大学生（66.6%）的研究结果，可能的原因是MSM大学生人群由于性行为方式较为特殊，是HIV感染的高危人群，也是相关部门进行性健康教育重点关注的人群[15]，因此该人群获取性健康相关知识的渠道更多也更加关注性健康相关的知识。
本研究的结果显示，户籍为城镇的MSM大学生梅毒防治知识知晓率较高，与郭晓嘉[16]等在重庆市的研究结果一致。导致梅毒防治知识知晓存在城乡差异的原因可能是有以下两个方面：一方面城镇人口集中，经济发展水平高，医疗资源配置更为充足，更容易开展梅毒防治知识相关的健康教育[17]。另一方面，在我国农村地区，青少年接受性健康教育的水平较低；人民的性观念普遍比较保守，羞于在公共场合提及与性健康相关的话题，在一定程度上影响了性健康知识的普及与传播[18]。本研究发现专业为医学类的MSM大学生的梅毒防治知识知晓率较高，与赵文鹏[14]对某高校大学生的研究结论一致。原因可能是医学专业的学生在日常学习中长期接触相关专业知识，对医学专业知识的知晓程度更高，往往具备更高的健康素养水平[19]。此外，本研究的结果中月均消费≥2 000元的MSM大学生的梅毒防治知识知晓率更高，与赵碧霞[20]等对住院病人的研究结果相似。可能的原因是月均消费较高者家庭经济条件较好，这类家庭往往对健康更加重视，对健康相关知识也更为关注[21]。
本研究发现最近6个月发生过酒后性行为的MSM大学生的梅毒防治知识知晓率较低。饮酒后酒精作用于大脑会影响行为的决策，因此饮酒被认为是发生高危性行为的危险因素之一[22]。梅毒防治知识知晓率较低的人群由于性健康相关知识掌握不足，风险意识较低，不能很好地识别和避免饮酒后发生性行为的情况。本研究还发现最近1年接受过同伴教育的MSM大学生的梅毒防治知识知晓率较高。这与既往研究的结论相符，既往研究指出个体更容易接受同伴传递的信息，同伴教育可以在短期内提高MSM群体对性病知识的认知，是在MSM的社会网络进行健康教育，促进健康知识传播的有效手段[23]。
综上所述，当前贵阳市MSM大学生梅毒防治知识知晓率与95%的知晓率目标尚存在一定的差距。未来应加强对MSM大学生的梅毒防治知识的宣传教育，尤其是农村户籍、非医学类专业、月均消费<2 000元、发生过酒后性行为的MSM大学生；可尝试采取或者结合同伴教育对该人群进行干预。
本研究存在一定的局限性。本研究采用的设计类型是横断面调查，无法明确研究因素与梅毒防治知识知晓情况之间的因果关系；MSM大学生人群存在较强的隐匿性，因此本研究招募研究对象时采用的方法为非随机抽样，研究对象可能存在选择性偏倚。
