Objective

To explore the causal relationship between left and right handgrip strength and the onset of cognitive decline, providing a reference for developing strategies to prevent cognitive decline in the elderly.

Methods

Genome-wide data for left and right handgrip strength were obtained from the UK Biobank, while genome-wide data for cognitive function were sourced from a meta-analysis conducted by the Cognitive Genomics Consortium. The study employed three regression models: Inverse-Variance Weighted (IVW), MR-Egger regression, and Weighted Median, to conduct Mendelian randomization analyses on the causal relationship between left and right handgrip strength and cognitive function.

Results

The IVW model results demonstrated a positive causal relationship between right handgrip strength (β=0.08, 95% CI: 0.03-0.14, P=0.004), left handgrip strength (β=0.08, 95% CI: 0.01-0.14, P=0.017), and cognitive function.

Conclusion

The findings of this study support a positive causal relationship between left-hand grip strength, right-hand grip strength, and cognitive function.

目的

探索左、右手握力与认知功能之间可能的因果关联,为预防老年人认知功能下降策略的制定提供参考依据。

方法

左、右手握力的全基因组数据来源UK Biobank,认知功能的全基因组数据来源于一项基于认知基因组学联盟的meta分析。研究采用随机效应逆方差加权法(Inverse-variance weighted, IVW)、MR-Egger回归、加权中位数法三种回归模型对左、右手握力与认知功能的因果关系进行孟德尔随机化分析。

目的

探索左、右手握力与认知功能之间可能的因果关联,为预防老年人认知功能下降策略的制定提供参考依据。

方法

左、右手握力的全基因组数据来源UK Biobank,认知功能的全基因组数据来源于一项基于认知基因组学联盟的meta分析。研究采用随机效应逆方差加权法(Inverse-variance weighted, IVW)、MR-Egger回归、加权中位数法三种回归模型对左、右手握力与认知功能的因果关系进行孟德尔随机化分析。

结果

IVW模型计算结果显示右手握力(β=0.08, 95% CI: 0.03 ~ 0.14, P=0.004)、左手握力(β=0.08, 95% CI: 0.01 ~ 0.14,P=0.017)与认知能力之间存在正向因果关系。

结论

Objective

To explore the causal relationship between left and right handgrip strength and the onset of cognitive decline, providing a reference for developing strategies to prevent cognitive decline in the elderly.

Methods

Genome-wide data for left and right handgrip strength were obtained from the UK Biobank, while genome-wide data for cognitive function were sourced from a meta-analysis conducted by the Cognitive Genomics Consortium. The study employed three regression models: Inverse-Variance Weighted (IVW), MR-Egger regression, and Weighted Median, to conduct Mendelian randomization analyses on the causal relationship between left and right handgrip strength and cognitive function.

Results

The IVW model results demonstrated a positive causal relationship between right handgrip strength (β=0.08, 95% CI: 0.03-0.14, P=0.004), left handgrip strength (β=0.08, 95% CI: 0.01-0.14, P=0.017), and cognitive function.

Conclusion

The findings of this study support a positive causal relationship between left-hand grip strength, right-hand grip strength, and cognitive function.

随着老龄化人口的快速增长,认知障碍已成为一个突出的全球公共卫生问题[1]。因此,早期识别认知能力下降的潜在危险因素对于延缓和预防认知障碍的发生至关重要。
在过去的十年里,人们越来越关注肌肉力量下降的问题。而握力是衡量上肢力量的客观指标,也是整体身体机能的指标[2]。它随年龄的增长而变化,在30岁左右达到平台期,然后在50岁后开始下降[3]。越来越多的证据表明,握力水平和随年龄的变化可能反映了指定和协调运动命令的能力,并且可能对大脑健康的细微变化敏感[4],因此一些人认为握力可能对老龄化人群具有预后价值[5]。握力下降与多种负面结果有关,会严重损害生活质量,从而加重包括住院在内的医疗保健经济负担。从流行病学的角度来看,多项观察性研究和系统综述报道了握力与轻度认知功能障碍(Mild cognitive impairment, MCI)之间的联系[6-9]。然而,由于方法上的挑战,如样本量不足,握力和认知功能之间的关联仍然存在争议,导致研究结果不一致[6-9]。本研究采用孟德尔随机化(Mendelian randomization, MR)研究设计,利用从全基因组关联研究(Genome-wide association studies, GWAS)中获得的独立数据集,在总结水平上阐明左、右手握力与认知功能之间关联的因果关联。据检索,目前还没有MR分析来分别探讨右手握力、左手握力与认知功能之间的潜在因果关系。本研究对老年人认知能力下降的早期预防具有重要意义。
采用双样本MR分析,以调查左、右双手握力与认知功能之间的关系。在MR分析中选择单核苷酸多态性(single nucleotide polymorphisms, SNPs)作为工具变量(instrumental variables, IVs)取决于三个关键假设[10]:首先,选择作为IVs的SNPs应与感兴趣的暴露表现出很强的相关性;其次,IV必须没有混杂因素;最后,IVs对结果的影响应该仅通过暴露而起作用,没有任何直接影响。
右手握力(n=461 089)和左手握力(n=461 026) 的GWAS数据是来源于UK Biobank的用经过验证且广泛使用的Jamar测功机收集的数据[11]
与一般认知能力关联的汇总统计数据是从最近的一项涉及欧洲血统个体(n=257 841)的GWAS中提取的。本数据是由一项基于认知基因组学联盟(COGENT)数据使用样本大小加权meta分析方法进行合并得出的[12]。研究对象的年龄从16岁到102岁不等,没有中风或痴呆的病史。
执行一系列质量控制程序以确定合适的工具变量。首先,选择独立的SNP(连锁不平衡r2<0.001)[13],这些SNP与感兴趣的暴露表现出显著的相关性(P<5×10-8)。随后,从结果GWAS中获得的汇总统计数据中获得了这些SNP的相应效应估计。在结果数据集中没有SNP的情况下,使用与期望SNP具有高相关性(r2 >0.8)的代理SNP。这些精心挑选的SNP然后被用作MR分析的最后一套工具。为了评估潜在的弱工具变量偏差的存在,使用以下公式计算F统计量:F统计量=(β/SE)2。确定作为工具的SNP的平均F统计量,F统计量大于10表示工具可靠[14]。最后,为了确保数据的一致性,我们剔除了所有具有中间等位基因频率的回文SNP[15]
本研究进行了双样本MR分析。使用逆方差加权法(Inverse-variance weighted, IVW)、MR-Egger回归和加权中位数法(Weighted median, WM)进行MR分析,其中用于评估暴露与结果之间因果关系的主要方法是IVW方法。使用β值和95%置信区间量化暴露对结果的因果影响。为了增强的结果的稳健性,使用Bonferroni调整后的P<0.025(0.05/2=0.025)来评估结果的显著性。采用Cochran’s Q检验对遗传工具间的异质性进行量化[16]。采用MR-Egger方法鉴定工具变量的潜在多效性[16]。此外,采用MR-PRESSO统计技术检测和消除潜在的多效性IVs。最后,运用留一法,以评估特定SNP对总体估计值的潜在不成比例的影响。异质性和多效性检验水准为α=0.05。所有数据分析均在R语言统计软件(4.2.2版)中的“TwosampleMR”和“MRPRESSO”包中进行。
为研究左、右手握力对认知功能的影响,综合选择过程从GWASs中分别获得了105个与右手握力和92个与左手握力独立且有效的IVs。
IVW结果显示,右手握力(β=0.08, 95% CI: 0.03~0.14, P=0.004)、左手握力(β=0.08, 95% CI: 0.01~0.14, P=0.017)与认知功能间存在因果关联,结果见表1
经计算,所有IVs的F统计值均超过10,表明弱IV偏倚的可能性极小。MR-PRESSO分析中均未发现异常值IVs。表2的MR异质性和多效性检验表明所有分析中均不存在异质性和多效性。散点图描述了单个SNP效应和每种MR方法的联合效应。采用留一法的敏感性分析进一步证实了MR分析结果的可靠性。本MR研究的散点图、漏斗图和留一法结果见图123
左、右手握力与认知功能之间的因果效应一直存在争议,本研究是第一个应用MR方法分别探索左、右手握力与认知功能之间因果关系的研究。本次研究结果确定了右手握力、左手握力对认知功能的因果关联。
现有研究表明,低握力可以预测10年内认知功能的下降[18]。握力与身体其他部位的力量表现出适度的相关性,使其成为整体力量测量的可靠指标[19]。与年龄相关的力量下降也与阿尔茨海默病的风险增加有关[20],在中国人群中,握力被认为是早期识别阿尔茨海默病患者的潜在标志[21]。一项在社区居住的老年人中进行的研究显示,低握力与MCI之间存在显著关联[22]。本研究的结论与这些相关研究的结果一致,进一步支持了左、右手握力与认知功能之间的关系。
握力与认知功能之间的关联可能与生物年龄有关。生物年龄是比实际年龄更准确的定量指标,用于评估人体的衰老[23]。DNA甲基化(DNAm)是一种更可靠的检测生物年龄的方法,称为表观遗传时钟[24]。DNAm年龄可以更好地估计真实生物年龄,握力与DNAm年龄相关,表明以握力为标志的肌肉无力与加速的生物年龄之间存在生理联系,并且在基因水平上握力相对较弱的人比握力较强的人衰老得更快[23]。与实际年龄相比,握力是更好的生存和保持健康状况的预测指标,肌肉力量可以成为有力而准确的生存预测指标[25]。调节大脑生物学并影响认知和行为的表观遗传修饰与阿尔茨海默病有关,显示患者和对照个体之间的DNAm不同,表明DNAm影响了认知功能[23]。认知功能受损不是由实际年龄的增长引起的,而是由于生物年龄的衰老,肌肉力量下降意味着生物年龄增加,从而导致认知功能下降[23]
握力与认知功能间关联的潜在生物学机制可能与神经系统的结构与功能有关。握力和认知功能与左小脑灰质体积(GMV)呈正相关[26]。平均区域GMV在很大程度上介导了握力与认知功能的几种测量之间的关联[27]。海马体与大脑认知功能相关区域之间的连接在老年人中减弱:海马旁回、后扣带回、颞叶前叶、杏仁核和壳核[28]。有氧活动与较大的海马体积、额叶、顶叶和颞叶皮层完整性有关,表明海马体可能在握力与认知功能之间的关联中起中介作[9]。众所周知,运动对认知功能有许多有益影响,肌肉分泌因子使肌肉和大脑功能之间能够直接串扰[29]。运动可以增加肌肉力量,影响中枢神经系统,并导致大脑功能发生变化,尤其是额叶[30]。前额叶皮层中BAZ2B和EHMT1的表达与阿尔茨海默病的进展呈正相关,调节靶基因的H3K9甲基化水平可以改善线粒体功能和认知功能,从而促进健康老龄化,提高老年生活质量[31]
本研究存在一些局限性。首先,本研究中使用的遗传工具变量和GWAS队列主要来自欧洲人群,因此需要验证以确定研究结果对其他种族的普遍性。其次,由于MR分析的工作原理是在遗传水平上推断因果关系,本研究只能识别潜在的因果关系,而这些因果关系背后的特定生物学途径无法明确确定。
MR是利用遗传变异作为工具变量建立因果关系的一种强有力的方法,有效地解决了传统流行病学研究中固有的混杂偏差。本研究选择广泛的GWAS数据集从而减轻了由人口分层引起的偏差,采用严格的MR分析方法确保了研究结果的准确性和有效性。综上所述,本项研究证明了左手握力和右手握力与认知功能之间均存在正向因果关系,这为制定预防老年人认知能力下降政策提供了重要参考,对维持老年人身体健康具有重要意义。
