Objective

To investigate the association between the expression of mucin-5B(MUC5B) gene and its locus polymorphisms rs2075853, rs4963053, and rs2014486 with coal worker’s pneumoconiosis(CWP).

Methods

The study analyzed age and years of service from 506 subjects (comprising 143 healthy individuals, 132 dust-exposed workers, 121 stage I CWP patients, and 110 stage II CWP patients) collected at the Guiyang City Public Health and Medical Center from February 2020 to October 2023. Gene expression levels were assessed using real-time quantitative polymerase chain reaction(qRT-PCR). Receiver operating characteristic(ROC) curves were plotted, and the area under the ROC curve (AUC) was calculated to evaluate the predictive value of MUC5B for CWP. Genotyping of MUC5B rs2075853, rs4963059, and rs2014486 was performed using a MassARRAY Analyzer, and the association between the three single nucleotide polymorphisms (SNPs) genotypes’ distribution frequency and the risk of CWP was analyzed.

Results

Significant statistical differences were observed among the groups in terms of age (H=242.198, P<0.001), years of service (H=315.822, P<0.001). Expression levels of MUC5B were significantly reduced in stage I and II CWP patients compared to the healthy and dust-exposed groups (H=34.491, P<0.001). ROC curve analysis indicated that MUC5B could serve as an auxiliary diagnostic marker for CWP (AUC=0.755, 95% CI: 0.682-0.828, P<0.001). Furthermore, haplotype analysis revealed that the CG haplotype constructed from rs4963059 and rs2014486 was more frequent in CWP patients, showing a significant association (odds ratio OR=1.719, 95% CI: 1.074-2.751, χ2=5.201, P<0.05), suggesting that the CG haplotype constructed by rs4963059 and rs2014486 loci was associated with an increased risk of coal workers’ pneumoconiosis.

Conclusion

The CG haplotype constructed by MUC5B gene rs4963059 and rs2014486 is closely related to the risk of developing coal workers’ pneumoconiosis.

目的

探讨粘蛋白-5B(MUC5B)基因表达及其位点多态性rs2075853、rs4963059和rs2014486与煤工尘肺相关性。

方法

通过对2020年2月—2023年10月贵阳市公共卫生救治中心506名受检者(包括143名健康对象、132名接尘工人、121名煤工尘肺壹期患者和110名煤工尘肺贰期患者)的研究,分析各组间年龄、工龄,采用实时荧光定量聚合酶链反应(qRT-PCR)评估基因表达水平,绘制受试者工作特征曲线(ROC),计算ROC曲线下面积(AUC)以评估MUC5B对煤工尘肺的预测价值,通过MassARRAY Analyzer质谱仪对MUC5B rs2075853、rs4963059、rs2014486进行基因分型,分析三个单核苷酸多态性(SNPs)基因型分布频率与煤工尘肺发病风险的关联。

结果

研究象对年龄(H=242.198, P<0.001)、工龄(H=315.822, P<0.001)在四组间差异均有统计学意义。煤工尘肺壹期组与煤工尘肺贰期组MUC5B表达量减少,与健康组及接尘组有显著性差异(H=34.491, P<0.001),ROC曲线分析表明MUC5B可作为煤工尘肺辅助诊断指标(AUC=0.755, 95% CI:0.682~0.828,P<0.001)。此外,单体型分析发现rs4963059和rs2014486位点构建的CG单体型在煤工尘肺患者中出现频率较高,显示出了明显的差异(风险比OR=1.719, 95% CI:1.074~2.751,χ2=5.201, P<0.05),暗示了由rs4963059和rs2014486位点构建的CG单体型与煤工尘肺风险增加有关。

结论

MUC5B基因rs4963059、rs2014486构建的CG单体型与煤工尘肺发病风险密切相关。

目的

探讨粘蛋白-5B(MUC5B)基因表达及其位点多态性rs2075853、rs4963059和rs2014486与煤工尘肺相关性。

方法

通过对2020年2月—2023年10月贵阳市公共卫生救治中心506名受检者(包括143名健康对象、132名接尘工人、121名煤工尘肺壹期患者和110名煤工尘肺贰期患者)的研究,分析各组间年龄、工龄,采用实时荧光定量聚合酶链反应(qRT-PCR)评估基因表达水平,绘制受试者工作特征曲线(ROC),计算ROC曲线下面积(AUC)以评估MUC5B对煤工尘肺的预测价值,通过MassARRAY Analyzer质谱仪对MUC5B rs2075853、rs4963059、rs2014486进行基因分型,分析三个单核苷酸多态性(SNPs)基因型分布频率与煤工尘肺发病风险的关联。

结果

研究象对年龄(H=242.198, P<0.001)、工龄(H=315.822, P<0.001)在四组间差异均有统计学意义。煤工尘肺壹期组与煤工尘肺贰期组MUC5B表达量减少,与健康组及接尘组有显著性差异(H=34.491, P<0.001),ROC曲线分析表明MUC5B可作为煤工尘肺辅助诊断指标(AUC=0.755, 95% CI:0.682~0.828,P<0.001)。此外,单体型分析发现rs4963059和rs2014486位点构建的CG单体型在煤工尘肺患者中出现频率较高,显示出了明显的差异(风险比OR=1.719, 95% CI:1.074~2.751,χ2=5.201, P<0.05),暗示了由rs4963059和rs2014486位点构建的CG单体型与煤工尘肺风险增加有关。

结论

MUC5B基因rs4963059、rs2014486构建的CG单体型与煤工尘肺发病风险密切相关。

Objective

To investigate the association between the expression of mucin-5B(MUC5B) gene and its locus polymorphisms rs2075853, rs4963053, and rs2014486 with coal worker’s pneumoconiosis(CWP).

Methods

The study analyzed age and years of service from 506 subjects (comprising 143 healthy individuals, 132 dust-exposed workers, 121 stage I CWP patients, and 110 stage II CWP patients) collected at the Guiyang City Public Health and Medical Center from February 2020 to October 2023. Gene expression levels were assessed using real-time quantitative polymerase chain reaction(qRT-PCR). Receiver operating characteristic(ROC) curves were plotted, and the area under the ROC curve (AUC) was calculated to evaluate the predictive value of MUC5B for CWP. Genotyping of MUC5B rs2075853, rs4963059, and rs2014486 was performed using a MassARRAY Analyzer, and the association between the three single nucleotide polymorphisms (SNPs) genotypes’ distribution frequency and the risk of CWP was analyzed.

Results

Significant statistical differences were observed among the groups in terms of age (H=242.198, P<0.001), years of service (H=315.822, P<0.001). Expression levels of MUC5B were significantly reduced in stage I and II CWP patients compared to the healthy and dust-exposed groups (H=34.491, P<0.001). ROC curve analysis indicated that MUC5B could serve as an auxiliary diagnostic marker for CWP (AUC=0.755, 95% CI: 0.682-0.828, P<0.001). Furthermore, haplotype analysis revealed that the CG haplotype constructed from rs4963059 and rs2014486 was more frequent in CWP patients, showing a significant association (odds ratio OR=1.719, 95% CI: 1.074-2.751, χ2=5.201, P<0.05), suggesting that the CG haplotype constructed by rs4963059 and rs2014486 loci was associated with an increased risk of coal workers’ pneumoconiosis.

Conclusion

The CG haplotype constructed by MUC5B gene rs4963059 and rs2014486 is closely related to the risk of developing coal workers’ pneumoconiosis.

2009年至2021年,我国职业病发病人数中尘肺病比例居首。在贵州省,尘肺病是农民工面临的主要职业健康问题,而煤炭产业是该病症影响最为严重的领域之一[1]。尽管实施了防治措施,但每年新诊断病例数量仍然居高不下。煤工尘肺(coal workers’ pneumoconiosis, CWP)是由长期吸入煤尘中无机矿物性粉尘引起,与煤尘暴露时间和浓度密切相关。煤工尘肺危害大且病程不可逆,当前缺乏特定的早期警示和诊断标准,目前其诊断依赖于高千伏胸片,但影像学证实的肺部纤维化症状出现时,患者的肺功能已遭不同程度损害,严重者可能出现呼吸功能丧失甚至呼吸衰竭[2]
煤工尘肺发病机制尚不明确,当前研究认为,煤尘颗粒反复刺激肺组织后产生严重的炎症反应,这种促炎环境会诱发肺间质内成纤维细胞的过度增殖,导致细胞外基质中胶原蛋白过度积累,最终导致弥漫性肺纤维化[3]。粘蛋白-5B(mucin 5B, MUC5B)是哺乳动物气道中的一种主要分泌性粘蛋白[4-5],研究表明,缺乏MUC5B的小鼠模型中,巨噬细胞的吞噬能力大幅下降[6],提示MUC5B在控制炎症和维持免疫平衡中发挥作用,在小鼠的慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)模型中发现,敲除MUC5B的小鼠中相关炎症因子如白细胞介素-6(interleukin-6, IL-6)和肿瘤坏死因子-α(tumor necrosis factor-α, TNF-α)明显增加,这表明MUC5B能通过调节巨噬细胞功能降低肺部炎症[7]。同时,MUC5B的表达量在COPD、囊性纤维化(cystic fibrosis, CF)和特发性肺纤维化(Idiopathic Pulmonary Fibrosis, IPF)等疾病进展中发生变化[8-10]。近年来国际上有众多研究报告不同基因的单核苷酸多态性(single-nucleotide polymorphism, SNP)与尘肺病的关联性,如LPS反应性米色样锚定蛋白(LPS Responsive Beige-Like Anchor Protein,LRBA)rs2290846 AA基因型显著降低了煤工尘肺的风险[11]、高迁移率族蛋白B1(high mobility group box 1 protein, HMGB1)rs1360485 G等位基因增加煤工尘肺风险[12]、维生素D受体基因(Vitamin D Receptor,VDR)ApaI T的ApaI T等位基因及较低的25(OH)2D水平提高了中国汉族人群患煤工尘肺的风险[13]等。除这些候选基因之外,粘蛋白(mucins, MUC)基因的多态性,特别是MUC5B启动子区域的特定位点变异,也被考虑可能增加肺纤维化的风险[14]。现已有研究确认,rs35705950是MUC5B基因启动子区域与肺纤维化发展相关的关键SNP[15]。Hunninghake 等在人群研究中发现,MUC5B启动子多态性位点的携带者影像学证据显示肺纤维化的几率增加了6倍[16]。在IPF中发现MUC5B基因启动子多态性增加了高加索群体及亚裔人群对肺炎的易患性增加[17]。在高加索人、亚洲人及混合人群中,MUC5B基因启动子多态性携带者更易患IPF[18]。尽管多数研究聚焦于IPF,但由于煤工尘肺与IPF具有相似的发病特征,这些发现也为深入探究MUC5B在煤工尘肺病理机制中的作用提供了新的视角。据此推测,MUC5B基因启动子的多态性可能与煤工尘肺紧密相关。本研究将分析MUC5B基因表达水平以及MUC5B 3个SNPs位点(rs2075853、rs4963059和rs2014486)的变异情况,探讨MUC5B基因与煤工尘肺发生发展之间的关联。
选取2020年2月至2023年10月间贵阳市公共卫生救治中心职业健康体检中进行健康体检的健康人143人、在岗体检的煤矿工人132人以及职业病诊断的煤工尘肺患者231人作为研究对象,所有研究对象均为汉族男性;在本研究中,健康人群作为健康对照组,而接触煤尘的煤矿工作人员则定义为接尘组,根据尘肺病的严重程度,将煤工尘肺病患者分为两组,即初期(煤工尘肺壹期)患者组和中期或更严重(煤工尘肺贰期及以上)患者组。本研究中的所有研究对象均接受了贵阳市公共卫生救治中心的职业病诊断专家团队的评估,其诊断依据为GBZ70-2015《职业性尘肺病的诊断》。收集研究对象个人基本资料包括年龄、工龄,研究对象的信息来源包括贵阳市公共卫生救治中心的职业健康体检系统、病历记录系统以及问卷调研。所有参与者在研究开始前均已签订书面的知情同意书。此外,本项研究已获得贵阳市公共卫生救治中心伦理委员会的正式审批(伦理审查批号:201920)。
(1)健康对照组纳入标准:无职业性粉尘接触史。(2)接尘对照组纳入标准:具有煤尘接触史未达到煤工尘肺诊断标准。(3)煤工尘肺患者组纳入标准:具有煤尘接触史且经贵阳市公共卫生救治中心尘肺诊断专家组依据中华人民共和国卫生部GBZ70-2015《职业性尘肺病的诊断》诊断为职业性煤工尘肺病的患者。
(1)患有肺结核、肺癌、慢性阻塞性肺疾病等与肺部疾病有关的疾病;(2)患有自身免疫性疾病,如:类风湿关节炎、系统性红斑狼疮等;(3)患有冠心病、风湿性心脏瓣膜病、骨折与骨质疏松、严重内分泌疾病或严重代谢疾病以及严重肝、肾疾病等疾病。
参考病例对照非匹配设计公式计算样本量,公式为:
式中:
p0:对照组3个位点中最小等位基因频率,取值为0.322;
RR:预期稀有等位基因型频率携带者患煤工尘肺的相对危险度,值为2;
α=0.05(双侧)、β=0.10,代入上述公式中计算求出样本量n=188例。理论样本量为376例,本研究共纳入研究对象506例。
采用EZ-press Whole Blood RNA Purification Kit试剂盒提取受试者血液总RNA并使用逆转录-聚合酶链反应(Reverse Transcription-Polymerase Chain Reaction, RT-PCR)仪(美国Bio-rad公司)逆转录为10 μL cDNA。选取β-肌动蛋白(β-actin)作为内参,通过定量实时聚合酶链式反应(Quantitative real time polymerase chain reaction, qRT-PCR)检测MUC5B基因表达水平,反应体系为:2×SYBR Green Mix 10 μL,cDNA模板2 μL,上下游引物各0.4 μL,ddH2O 7.2 μL,反应总体积为20 μL;反应条件为:95 ℃ 3 min,95 ℃ 15 s,退火30 s,72 ℃ 60 s,40个循环。MUC5B引物序列上游:5’-TCCCACTATTCCACCTTTGACG-3’,下游:5’-CCAGGTAGAGGCTGAGATTCCC-3’,β-actin引物序列上游:5’-CTAAGTCATAGTCCGCCTAGAAGCA-3’,下游:5’-TGGCACCCAGCACAATGAA-3’,所有引物均由上海生工生物工程有限公司进行合成。
通过查阅PubMed与NCBI数据库,根据SNP位点的MAF筛选出功能性位点(MAF>0.05),最终纳入3个SNPs(rs2075853、rs4963059和rs2014486),研究对象所有SNPs的基因型分布均符合Hardy-Weinberg平衡(P>0.05),见表1。使用血液基因组试剂盒提取基因组DNA,DNA样本置于-80 ℃冰箱保存备用,设计特异性引物进行PCR扩增,分析PCR产物,通过MassARRAY Analyzer质谱仪进行基因分型,使用TYPER 4.0软件分析原始数据及基因分型图。
采用SPSS 23.0软件进行统计学分析,偏态分布的计量资料以MP25P75)表示,多组间比较采用Kruskal-Wallis H秩和检验,组间两两比较采用秩和检验;计数资料率的比较用R×C列联表资料χ2检验;用拟合优度χ2检验对研究样本的群体代表性进行Hardy-Weinberg平衡检验;应用R语言软件包绘制受试者工作特征(Receiver Operating Characteristic curve, ROC curve)曲线,分析MUC5B表达量对煤工尘肺的早期诊断价值。检验水准α=0.05。
本研究共纳入健康组143例、接尘组132例、煤工尘肺壹期组121例、煤工尘肺贰期组110例。研究对象一般人口学资料分析显示,年龄与工龄在四组间具有明显差异(P<0.001),见表2
参考同类研究,每组随机选择40例样本进行表达量分析,结果显示,煤工尘肺壹期组、煤工尘肺贰期组MUC5B表达量较健康组、接尘组均减少,差异均有统计学意义(P<0.05),健康组与接尘组之间差异无统计学意义,详见表3。以后前位高仟伏胸片为金标准,以确诊为煤工尘肺为患病组,健康组与接尘组为未患病组,纳入两组MUC5B表达量进行ROC曲线分析,结果显示,曲线下面积(AUC)为0.755(95% CI:0.682~0.828,P<0.05),灵敏度为78.8%,特异度为57.5%,诊断价值中等,认为MUC5B可作为煤工尘肺的辅助诊断指标,诊断截断值为1.076。见图1
对506例研究对象进行基因分型,MUC5B各SNPs的基因型、等位基因频率见表4。MUC5B基因rs2075853、rs4963059、rs2014486基因型分布及等位基因频率各组间差异无统计学意义(P>0.05)。
利用SHEsis在线软件[19],对MUC5B基因rs2075853、rs4963059、rs2014486位点进行连锁不平衡程度(D’r2值)分析,图2为MUC5B基因rs2075853、rs4963059、rs2014486的连锁不平衡D’值与r2值图,结果显示,rs4963059、rs2014486存在连锁不平衡(D’>0.7,r2>0.3),见表5。对MUC5B rs4963059、rs2014486进行单体型分析,结果显示,在rs4963059、rs2014486构建的单体型中,TA单体型在对照组或煤工尘肺组中分布频率低于3%,不做分析;在煤工尘肺组中,CA和TG单体型的出现频率分别是32.5%和55.9%,而在对照组,这两种单体型的频率则为33.5%及59.0%。这一差异在统计学上并不显著(P>0.05)。相反,CG单体型在煤工尘肺组与对照组的频率分别为9.7%和5.9%,显示出了明显的差异(风险比OR=1.719,95% CI:1.074~2.751,P<0.05),见表6
煤工尘肺暴露人数众多,所导致的疾病潜伏期长,进展至后期后通常无法逆转,对患者的健康及生活造成严重影响,因此煤工尘肺的早期预警与识别显得尤为重要。本研究的结果表明,在接尘和煤工尘肺患者群体中,年龄及工作年限均明显高于健康人群,强调了长期接触煤尘和年龄增长作为煤工尘肺进展的影响因素。
本研究发现,MUC5B mRNA的表达水平与煤工尘肺有着密切的联系。与健康人群以及接尘群体相比较,煤工尘肺患者的MUC5B表达水平显著降低,其在煤工尘肺患者、健康人群及接尘但未患病的人群间的对比中,都表现出了统计学上的显著性,提示MUC5B的表达随煤工尘肺发展而降低,这一发现与其他一些关于呼吸道疾病的研究持相似观点。在未暴露于粉尘的健康人群和仅暴露于粉尘的人群之间,MUC5B mRNA的表达水平没有显著差异,这可能是由于初期粉尘接触阶段,机体通过生理调节,利用呼吸道粘液纤毛系统(Mucociliary clearance, MCC)的机械作用有效移除吸入的煤尘颗粒,以维护气道清洁的结果[20],在此过程中,MUC5B的积极分泌参与形成了一个强有力的防护屏障,随着持续的粉尘刺激,机体炎症反应发生和肺纤维化进展,导致了MUC5B的分泌减少。
ROC曲线分析发现MUC5B基因表达水平变化在煤工尘肺早期预警中具有一定的价值,其AUC(0.755)及相对较高的灵敏度(78.8%)表明,MUC5B基因表达水平可以作为煤矿粉尘作业工人中较为敏感的早期预警生物标志物,提示在粉尘接触期间应该密切监测工人血细胞中MUC5B基因表达水平变化情况,使用截断值作为诊断临界值,但需要注意的是,其特异度(57.5%)较低,在实际应用中可能需要与其他诊断方法或指标相结合,以提高煤工尘肺早期预警的准确性。
本研究未发现MUC5B基因rs2075853、rs4963059、rs2014486与煤工尘肺之间的关联。但进一步的连锁不平衡和单体型分析揭示了MUC5B基因内部的遗传结构特点,rs4963059和rs2014486位点之间存在明显的连锁不平衡(D’>0.7,r2>0.3),由这两个SNPs位点组成的CG单体型与煤工尘肺相关,表明MUC5B基因中存在遗传连接区域。煤工尘肺是一种因长期吸入煤尘而导致的肺部疾病,MUC5B 基因与呼吸道黏液分泌及免疫反应密切相关,而rs4963059和rs2014486位于MUC5B基因启动子区域,启动子区发生的变异被预测会破坏转录因子结合位点,这两个位点可能通过连锁遗传影响MUC5B转录水平,降低了MUC5B mRNA的表达,进而影响肺部对煤尘的清除和炎症反应,从而增加煤工尘肺的发病风险。在现有的研究中,MUC5B基因的多态性与多种肺部疾病之间的关联已得到认证[18,21-23],本研究中观察到的MUC5B与煤工尘肺之间的联系揭示了可能存在着类似的分子机制,进一步可深入研究MUC5B基因多态性与煤工尘肺的具体机制,包括对黏液分泌、免疫反应和肺部纤维化的影响。
综上所述,MUC5B基因表达水平可能成为煤工尘肺早期筛查的有效指标,MUC5B基因rs4963059、rs2014486构建的CG单体型与煤工尘肺有关,这两个位点可能通过连锁遗传影响煤工尘肺发生。考虑到地域、民族和环境差异可能对研究结果产生的影响,本次研究结果存在一定局限性。进一步研究需要增大样本量,并纳入更多地理和种族背景不同的煤工尘肺患者与健康及接尘工人,以便对结果进行验证,为煤工尘肺的预防和早期诊断提供坚实依据。
