Objective

To investigate the factors influencing Guangzhou residents’ willingness to use “Internet + pharmaceutical services”, providing references for optimizing related service platforms.

Methods

A questionnaire was designed based on the UTAUT model, and from January to May 2024, residents of Guangzhou were surveyed. Structural equation modeling was utilized to test the path relationships and mediating effects, constructing a model of the influencing factors on residents’ willingness to use“Internet + pharmaceutical services”.

Results

Social influence (β=0.319, P<0.001), effort expectancy (β=0.264, P<0.05), and performance expectancy (β=0.256, P<0.001) positively impacted the willingness of Guangzhou residents to use “Internet +pharmaceutical services”. Social influence had a significant positive effect on performance expectancy (β=0.764, P<0.001), and performance expectancy partially mediated the positive effect of social influence on willingness to use. Facilitating factors (P>0.05) did not have a significant effect on willingness.

Conclusion

Performance expectancy, effort expectancy, and social influence are the main factors affecting Guangzhou residents’willingness to use “Internet + pharmaceutical services”. Related platforms should innovate and improve their design based on residents’ needs, ensuring the practicality and usability of services to enhance user experience. Additionally, promoting and publicizing “Internet + pharmaceutical services” should be strengthened to leverage social influence, thereby increasing public recognition and willingness to use.

目的

研究广州市居民“互联网+药学服务”使用意愿的影响因素，为“互联网+药学服务”相关平台优化服务提供参考。

方法

基于整合型技术接受模型设计调查问卷，于2024年1—5月选取广州市居民进行问卷调查，利用结构方程模型进行路径关系和中介效应检验，构建居民“互联网+药学服务”使用意愿的影响因素模型。

结果

社会影响（β=0.319，P<0.001）、努力期望（β=0.264，P<0.05）、绩效期望（β=0.256，P<0.001）对广州市居民“互联网+药学服务”的使用意愿具有正向作用；社会影响对绩效期望具有显著的正向作用（β=0.764，P<0.001），且绩效期望在社会影响对使用意愿的正向作用之间起部分中介作用；促成因素（P>0.05）对使用意愿的作用不显著。

结论

绩效期望、努力期望、社会影响是影响广州市居民“互联网+药学服务”使用意愿的主要因素；“互联网+药学服务”相关平台应从居民需求出发，不断创新和完善平台设计，确保服务的实用性与易用性，优化居民使用体验；加强对“互联网+药学服务”的推广和宣传，充分发挥社会影响的作用，提升公众的认同感和使用意愿。

目的

研究广州市居民“互联网+药学服务”使用意愿的影响因素，为“互联网+药学服务”相关平台优化服务提供参考。

方法

基于整合型技术接受模型设计调查问卷，于2024年1—5月选取广州市居民进行问卷调查，利用结构方程模型进行路径关系和中介效应检验，构建居民“互联网+药学服务”使用意愿的影响因素模型。

结果

结论

绩效期望、努力期望、社会影响是影响广州市居民“互联网+药学服务”使用意愿的主要因素；“互联网+药学服务”相关平台应从居民需求出发，不断创新和完善平台设计，确保服务的实用性与易用性，优化居民使用体验；加强对“互联网+药学服务”的推广和宣传，充分发挥社会影响的作用，提升公众的认同感和使用意愿。

Objective

To investigate the factors influencing Guangzhou residents’ willingness to use “Internet + pharmaceutical services”, providing references for optimizing related service platforms.

Methods

A questionnaire was designed based on the UTAUT model, and from January to May 2024, residents of Guangzhou were surveyed. Structural equation modeling was utilized to test the path relationships and mediating effects, constructing a model of the influencing factors on residents’ willingness to use“Internet + pharmaceutical services”.

Results

Social influence (β=0.319, P<0.001), effort expectancy (β=0.264, P<0.05), and performance expectancy (β=0.256, P<0.001) positively impacted the willingness of Guangzhou residents to use “Internet +pharmaceutical services”. Social influence had a significant positive effect on performance expectancy (β=0.764, P<0.001), and performance expectancy partially mediated the positive effect of social influence on willingness to use. Facilitating factors (P>0.05) did not have a significant effect on willingness.

Conclusion

Performance expectancy, effort expectancy, and social influence are the main factors affecting Guangzhou residents’willingness to use “Internet + pharmaceutical services”. Related platforms should innovate and improve their design based on residents’ needs, ensuring the practicality and usability of services to enhance user experience. Additionally, promoting and publicizing “Internet + pharmaceutical services” should be strengthened to leverage social influence, thereby increasing public recognition and willingness to use.

随着居民健康意识的提高和医疗需求的不断释放，药学服务需求持续增长，为提供更加高效便捷的药学服务，“互联网+药学服务”应运而生。“互联网+药学服务”是指在开展互联网诊疗或远程医疗服务过程中，以实体医疗机构内的药师为主体，提供在线药学咨询、指导患者合理用药等药学服务[1]。近年来，我国陆续出台多项政策推动“互联网+药学服务”，为响应政策，广东省提出倡导医疗机构打造药学服务一站问答平台，保证药学人员在线提供用药咨询服务，指导患者科学用药，同时探索送药上门、院开店取等方式[2]。目前，广州市已有多地试点开展“互联网+药学服务”，如天河区开展的“线上+线下”家庭药师服务[3]、花都区开展的微信公众号药学服务[4]、番禺区开展的云药房延伸处方用药服务等[5]，具有一定的代表性。
现阶段国内有关“互联网+药学服务”的研究主要聚焦于临床应用效果验证[6-7]、医院实践模式探索[8-9]和文献综述分析[10-11]三个方面，然而关于居民对“互联网+药学服务”使用意愿的定量分析研究仍较为匮乏。整合型技术接受模型（the unified theory of acceptance and use of technology, UTAUT）是Venkatesh等人[12]在理性行为理论、计划行为理论等七种信息技术使用行为理论的基础上提出的整合模型，模型认为绩效期望、努力期望、社会影响和促成因素4个核心变量决定个体的行为意愿。已有学者基于此分析居民对高血压管理平台[13]、移动医疗服务[14]、医生远程会诊[15]等的使用意愿影响因素，结果显示绩效期望、努力期望、社会影响、促成因素均对使用意愿产生正向作用，这些变量广泛用于解释受众群体对新技术的接受度，解释力度高达70%[12]。居民的使用意愿对“互联网+药学服务”的普及具有重要影响，为更好地推广此项服务，本研究基于UTAUT模型分析广州市居民“互联网+药学服务”使用意愿的影响因素，深入探讨居民对服务的需求，为提高服务质量和居民使用意愿提供实证参考依据。
1　资料与方法

1.1　资料来源
本研究于2024年1—5月采用线上线下结合的方法收集问卷。鉴于广州市天河区、番禺区及花都区在“互联网+药学服务”实践的先行探索，本研究随机选取广州市天河区的龙洞街道和五山街道、番禺区的新造镇和小谷围街道、花都区的炭步镇和新华街道作为调查现场。通过线上问卷星和线下实地调研的方法收集问卷，在正式调查之前进行预调查，删除表述不清楚、逻辑有问题的题项。线上通过与各镇街的社工合作，由社工向居民阐明调查详情，获得知情同意后将问卷发放给居民。线上采取以下措施控制样本质量：根据纳入地区标准设置作答限制；限制问卷填写设备的IP地址，同一设备只能填写一次；排除作答时间小于2 min的样本。线下实地调研是通过选取5位调查员，分别前往公园、社区医院等公共场所进行偶遇抽样调查，调查时先向居民说明调查目的和研究意义，获得知情同意后再按照问卷顺序逐个提出问题，确保每个问题都得到回答。数据纳入标准：年龄≥18周岁；广州市常住居民（居住时间≥6个月）；具有良好的沟通能力，正确理解问卷内容并愿意填写问卷。本研究获得了广州医科大学医学伦理委员会的批准（伦理批号：2020120001），被调查者均知情并签署了知情同意书。
根据公式，取α=0.05（Zα/2=1.96）、π=0.50、抽样误差E=4%计算，可得最小样本量为601份，考虑到10%的无效应答，所发放问卷需大于661份。本研究共发放802份问卷，剔除答案前后逻辑不一致、回答不完整的无效问卷，共收集706份有效问卷，问卷有效率为88.03%。
1.2　问卷设计与假设
调查问卷的设计主要分为两个部分，第一部分是调查对象的人口学基本信息，包括性别、年龄、是否患慢性病等；第二部分是对模型中五个潜变量的测量，包括绩效期望、努力期望、社会影响、促成因素和使用意愿。共分为15个题项，采用李克特五级评分，“1～5分”分别代表“非常不同意、比较不同意、中立、比较同意、完全同意”。借鉴Venkatesh、李冰雪等人[13-16]的研究设计问卷量表：（1）绩效期望采用“简化购药的流程”“与药师在线沟通”“提高用药依从性”来测量；（2）努力期望采用“学习操作简单”“容易获取有用信息”“轻松适应变化”来测量；（3）社会影响采用“亲朋好友支持促进使用”“身边人推荐促进使用”“国家政策支持促进使用”来测量；（4）促成因素采用“足够的知识储备”“教学程序简单易懂”“遇到困难能得到帮助”来测量；（5）使用意愿采用“现在愿意使用该服务”“未来愿意使用该服务”“愿意推荐给他人使用”来测量。量表五个维度的Cronbach α系数为0.929，各维度的Cronbach α系数均大于0.7，问卷整体KMO值为0.945，Bartlett检验的显著性小于0.001，累计方差贡献率大于60%，标准化因子载荷量均在0.6以上，通过各维度的标准化因子载荷量计算出的CR组合信度均大于0.7，AVE平均方差提取值均大于0.5，说明问卷具有良好的信效度。基于以往研究[12-17]，本文提出以下假设：（1）H1：绩效期望对“互联网+药学服务”的使用意愿有正向作用；（2）H2：努力期望对“互联网+药学服务”的使用意愿有正向作用；（3）H3：社会影响对“互联网+药学服务”的使用意愿有正向作用；（4）H4：促成因素对“互联网+药学服务”的使用意愿有正向作用；（5）H5：社会影响对“互联网+药学服务”的绩效期望有正向作用。
1.3　统计学方法
研究通过Microsoft Excel 2019建立数据库。使用SPSS 26.0进行描述性统计；采用独立样本t检验或方差分析比较不同特征居民的使用意愿得分情况；采用Pearson法分析各变量的相关性。使用AMOS 26.0进行结构方程模型分析，采用bootstrap法进行中介效应检验，设置bootstrap=5 000。检验水准α=0.05。
2　结果

2.1　调查对象基本情况
706名广州居民中，男性304例（43.06%）；年龄以18～44为主（67.42%）；户籍所在地为城市的421例（59.63%）；学历为高中或中专及以下的255例（36.12%）；月收入情况为2 000元以下的332例（47.02%）；慢性病患者300例（42.49%）。单因素分析结果显示，不同年龄、学历水平、患慢性病情况的广州市居民“互联网+药学服务”使用意愿的得分具有显著差异（P<0.05）；不同性别、户籍所在地、月收入情况的居民使用意愿得分不具有显著差异（P>0.05）。见表1。
2.2　居民“互联网+药学服务”使用意愿模型各变量的相关性分析
研究各变量之间的相关性，可得绩效期望、努力期望、社会影响、促成因素与使用意愿之间均具有显著的相关关系（P<0.01），且相关系数r均大于0.5，说明各变量之间均存在正相关关系。见表2。
2.3　居民“互联网+药学服务”使用意愿影响因素模型的路径分析
将绩效期望、努力期望、社会影响、促成因素、使用意愿作为潜变量，单因素分析结果中显著的年龄、学历水平、是否患慢性病作为控制变量纳入结构方程模型中，结构方程模型见图1。
对模型的适配度进行检验，得到的拟合指标为：χ2/v=2.679<3，RMSEA=0.049<0.06，TLI=0.961>0.9，CFI=0.971>0.9，RMR=0.030<0.08，SRMR=0.074<0.08，各拟合指标均符合参考标准，可进行下一步研究。分析各控制变量的影响，学历水平和是否患慢性病的P值均大于0.05，不具有统计学意义，仅年龄对使用意愿作用的P值小于0.05，标准化路径系数β=0.088<0.1，年龄对使用意愿的影响小。
路径分析结果和假设检验结果见表3。社会影响（β=0.319，P<0.001）、努力期望（β=0.264，P<0.05）、绩效期望（β=0.256，P<0.001）对居民“互联网+药学服务”使用意愿具有正向作用，促成因素（P>0.05）对使用意愿的作用不显著。社会影响对绩效期望具有显著的正向作用（β=0.764，P<0.001）。
检验社会影响→绩效期望→使用意愿路径的中介效应，所得结果见表4。其中中介路径95%的置信区间不包含0，说明中介效应显著。中介路径的效应值为0.185，直接效应系数为0.302，分别占总效应的37.99%、62.01%，说明绩效期望在社会影响对使用意愿的正向作用之间起部分中介作用。见图2。
3　讨论

3.1　社会影响、绩效期望、努力期望对使用意愿的正向作用
社会影响的作用与李冰雪等[13]的研究结果一致。目前广州市“互联网+药学服务”实践处于起步阶段，居民因技术焦虑和信任赤字易出现排斥心理，同伴的使用经历帮助其获取信息，增强使用信心并获得社会情感支持，进而提升使用意愿。绩效期望体现服务实用性的影响，与杜涛等[15]的研究结果一致。“互联网+药学服务”提供药物咨询、用药指导等服务，搭建医患沟通桥梁，提高用药安全。努力期望体现服务易用性的影响，与邓仲平[14]等的研究结果一致。易用性直接关系到居民的使用体验，当服务平台的使用流程越简单、信息获取越便捷，居民的使用意愿就越强。
3.2　绩效期望在社会影响和使用意愿之间起部分中介作用
居民倾向于依赖同伴反馈来获取服务具体信息，从而降低使用的不确定性并提升使用意愿。一方面，社会影响有利于提升居民的接受度。广州市尚未公开出台“互联网+药学服务”实施办法或细则，服务的开展缺乏权威的指导，导致居民的接受度偏低。因此同伴的经验反馈与社交媒体的传播，将提高居民对服务的接受度，进而增强使用意愿。另一方面，社会影响有助于居民评估服务质量。目前部分机构“互联网+药学服务”的宣传内容专业化程度较高[10]，居民难以正确理解，参考同伴的反馈建议，可辅助其筛选有效信息、评估服务质量并强化对实用性的认知，提升居民的使用意愿。
3.3　促成因素对使用意愿的作用不显著
能否得到充分的资源和技术支持不是影响居民服务态度和评价的主要因素，与Venkatesh等[16]的研究结果存在差异，这可能与广州市的数字化发展密切相关。广州市已实现5G网络全覆盖，同时人工智能技术广泛应用于医疗健康等领域，为居民获取数字资源和技术援助提供了便利，大幅降低了使用“互联网+药学服务”的技术门槛。所以居民可能将技术支持、资源的可用性等促成因素视为基本保健因素，其存在缺乏增强居民使用意愿的效力。
3.4　总结
本研究创新性地引入了UTAUT模型分析广州市居民“互联网+药学服务”使用意愿的影响因素，证实了绩效期望、努力期望、社会影响对使用意愿的正向作用，以及绩效期望在社会影响与使用意愿之间的中介作用，对于优化“互联网+药学服务”实践有以下启示：
第一，优化宣传策略。通过官方公众号以短视频等通俗易懂的形式替代专业推文宣传，并开展社区药师讲座及居民案例分享会，传播服务的益处并鼓励居民使用。
第二，推广典型案例成果。相关部门应及时推广试点成果，并结合地区实际情况制定针对性政策，不断完善药学服务。
第三，线上平台与线下医疗机构联动，创新药学服务。例如联动家庭医生服务，将线上药学服务与线下随访相结合，构建连续性药学服务体系。
第四，开展个性化精准药学服务。搭建药学专科大数据平台，开发临床用药辅助决策系统，通过分析居民用药历史、过敏反应等信息，提供个性化服务。
本研究具有一定的局限性：第一，样本数据仅来自广州市，其它省市可能由于地区经济或医疗资源差异而呈现不同的结论；第二，本研究聚焦于居民的使用意愿及其影响因素，随着相关政策的不断普及与实施，课题组将进一步考察居民的使用行为及影响因素，为“互联网+药学服务”平台的持续优化提供更多实证参考依据。
