Objective

To explore the relationship between the romantic status of vocational school students in Guizhou Province and their engagement in non-suicidal self-injury (NSSI), as well as the mediating role of difficulties in emotion regulation, providing a reference for the prevention of adolescent NSSI behaviors.

Methods

A questionnaire survey was conducted from October to December 2023, utilizing a multi-stage stratified cluster random sampling method to select students from nine vocational schools in Guizhou Province. The Chinese version of the Adolescent Non-Suicidal Self-Injury Assessment Questionnaire (ANSAQ) and the Difficulties in Emotion Regulation Scale (DERS) were used to measure NSSI behaviors and emotion regulation difficulties among vocational students. The mediating effect of difficulties in emotion regulation was examined using categorical variable mediation methods.

Results

The prevalence of NSSI among vocational students in Guizhou Province was 27.4%. The standardized relative direct effect of experiencing a breakup on NSSI was 0.575 (95%CI: 0.104 to 0.258), and the relative mediating effect was 0.421 (95%CI: 0.062 to 0.203), with the mediating effect accounting for 26.8% of the total effect. Difficulties in emotion regulation fully mediated the relationship between being in a romantic relationship and NSSI, with the mediating effect constituting 47.3% of the total effect. For students who had previously been in a relationship but were now in the post-breakup phase, the standardized relative direct effect on NSSI was 0.008 (95%CI: -0.021 to 0.037), and the standardized relative mediating effect was 0.011 (95%CI: -0.001 to 0.024), both of which were not significant.

Conclusion

Differences in emotion regulation abilities across various romantic statuses partially explain the occurrence of individual NSSI behaviors. Enhancing adolescents’ emotion regulation skills can help them establish healthy emotional perspectives and positive interpersonal relationships, thereby effectively reducing the incidence of NSSI behaviors and positively impacting adolescent mental health within the public health domain while fostering harmonious social relationships.

目的

探讨贵州省中职学生恋爱状态与非自杀性自伤行为(NSSI)之间的关系，以及情绪调节困难在其中的中介作用，为预防青少年NSSI行为提供参考。

方法

于2023年10—12月通过多阶段分层整群随机抽样方法抽取贵州省共9所中职学校学生进行问卷调查。采用中文版青少年非自杀性自伤评估问卷(ANSAQ)、情绪调节困难量表（DERS）测量中职学生的NSSI行为和情绪调节情况，使用分类变量的中介方法检验情绪调节困难的中介作用。

结果

贵州省中职学生NSSI检出率为27.4%；失恋期对NSSI的标准化相对直接效应为0.575（95%CI：0.104～0.258）、相对中介效应为0.421（95%CI：0.062～0.203），中介效应占总效应比例为26.8%；情绪调节困难在正在恋爱与NSSI间起完全中介作用，中介效应占总效应比例为47.3%；曾经恋爱已过失恋期与NSSI的标准化相对直接效应为0.008（95%CI：-0.021～0.037）、标准化相对中介效应为0.011（95%CI：-0.001～0.024），均不显著。

结论

不同恋爱状态下的情绪调节能力差异部分解释了个体NSSI行为的发生，增强青少年的情绪调节能力，帮助他们建立健康的情感观念和积极的人际关系，从而有效减少NSSI行为的发生，构建和谐社会关系的同时在公共卫生领域实现对青少年心理健康的积极影响。

探讨贵州省中职学生恋爱状态与非自杀性自伤行为(NSSI)之间的关系，以及情绪调节困难在其中的中介作用，为预防青少年NSSI行为提供参考。

方法

于2023年10—12月通过多阶段分层整群随机抽样方法抽取贵州省共9所中职学校学生进行问卷调查。采用中文版青少年非自杀性自伤评估问卷(ANSAQ)、情绪调节困难量表（DERS）测量中职学生的NSSI行为和情绪调节情况，使用分类变量的中介方法检验情绪调节困难的中介作用。

结果

贵州省中职学生NSSI检出率为27.4%；失恋期对NSSI的标准化相对直接效应为0.575（95%CI：0.104～0.258）、相对中介效应为0.421（95%CI：0.062～0.203），中介效应占总效应比例为26.8%；情绪调节困难在正在恋爱与NSSI间起完全中介作用，中介效应占总效应比例为47.3%；曾经恋爱已过失恋期与NSSI的标准化相对直接效应为0.008（95%CI：-0.021～0.037）、标准化相对中介效应为0.011（95%CI：-0.001～0.024），均不显著。

结论

不同恋爱状态下的情绪调节能力差异部分解释了个体NSSI行为的发生，增强青少年的情绪调节能力，帮助他们建立健康的情感观念和积极的人际关系，从而有效减少NSSI行为的发生，构建和谐社会关系的同时在公共卫生领域实现对青少年心理健康的积极影响。

Objective

To explore the relationship between the romantic status of vocational school students in Guizhou Province and their engagement in non-suicidal self-injury (NSSI), as well as the mediating role of difficulties in emotion regulation, providing a reference for the prevention of adolescent NSSI behaviors.

Methods

A questionnaire survey was conducted from October to December 2023, utilizing a multi-stage stratified cluster random sampling method to select students from nine vocational schools in Guizhou Province. The Chinese version of the Adolescent Non-Suicidal Self-Injury Assessment Questionnaire (ANSAQ) and the Difficulties in Emotion Regulation Scale (DERS) were used to measure NSSI behaviors and emotion regulation difficulties among vocational students. The mediating effect of difficulties in emotion regulation was examined using categorical variable mediation methods.

Results

The prevalence of NSSI among vocational students in Guizhou Province was 27.4%. The standardized relative direct effect of experiencing a breakup on NSSI was 0.575 (95%CI: 0.104 to 0.258), and the relative mediating effect was 0.421 (95%CI: 0.062 to 0.203), with the mediating effect accounting for 26.8% of the total effect. Difficulties in emotion regulation fully mediated the relationship between being in a romantic relationship and NSSI, with the mediating effect constituting 47.3% of the total effect. For students who had previously been in a relationship but were now in the post-breakup phase, the standardized relative direct effect on NSSI was 0.008 (95%CI: -0.021 to 0.037), and the standardized relative mediating effect was 0.011 (95%CI: -0.001 to 0.024), both of which were not significant.

Conclusion

Differences in emotion regulation abilities across various romantic statuses partially explain the occurrence of individual NSSI behaviors. Enhancing adolescents’ emotion regulation skills can help them establish healthy emotional perspectives and positive interpersonal relationships, thereby effectively reducing the incidence of NSSI behaviors and positively impacting adolescent mental health within the public health domain while fostering harmonious social relationships.

非自杀性自伤行为(non-suicidal self-injury，NSSI)是指青少年故意伤害或改变身体组织而无自杀意图的行为，如切割、刮擦、灼伤或撞击物体等方式[1]。NSSI在青春期是一个日益严重的心理健康问题，在全球范围内的检出率呈上升趋势[2]。相关研究表明，具有NSSI行为人群的自杀风险是普通人群的66倍[3]，对青少年构成了巨大危害，因此，世界卫生组织将青少年NSSI行为列入了重点关注的公共卫生问题[4]。研究显示，NSSI的影响因素众多，包括生物因素（如年龄、性别、基因等）、心理学因素（如情绪调节、抑郁、焦虑、冲动等）和家庭社会环境因素（如不良亲子关系、父母人格特征、人际关系问题、教育压力）[5]。在NSSI的影响因素中，青春期恋爱被认为是青少年NSSI行为的早期危险因素[6]。恋爱经历可能会导致心理尚未成熟的青少年更易产生抑郁[7]，其过程中产生的负性情绪亦是NSSI行为的危险因素[8]。
已有研究表明，不同恋爱状态的个体在情绪调节能力方面存在差异，正在恋爱或处于失恋期的个体往往情绪调节能力较差[9]，而情绪调节在不同恋爱状态与NSSI行为之间的具体中介作用尚不清楚。研究发现情绪调节困难在依恋与NSSI的关系中起到了中介作用而恋爱关系则是一种浪漫依恋形式[10]，因此，本研究提出假设：情绪调节困难在不同恋爱状态与NSSI行为中起到中介作用。
此外，我国中职学生多数是由于学业成绩不理想而选择职业教育，而社会对中职教育存在一定偏见，导致这些学生长期面临污名化问题[11]。这种社会偏见可能加剧了他们的心理压力和情绪问题，进而影响自我认同、自尊和自我价值感。鉴于中职学生在身心发展和所处环境上的特殊性[12]，本研究以中职学生为研究对象，探讨不同恋爱状态对NSSI行为的影响，以及情绪调节在其中的作用机制，以期为促进中职学生心理健康和NSSI行为的预防和干预提供理论依据。基于本研究建立的假设模型见图1。
1　对象与方法

1.1　研究对象
于2023年10—12月通过多阶段分层整群随机抽样方法，根据贵州省2022年经济水平按好、中、差分层，抽取3个市；每个市随机抽3所中职学校，每所学校再以3个年级分层随机选取2个班级(50人左右/班)进行整群调查，共抽取6个班级。纳入标准：（1）调查期间属于贵州省在籍的中职学校学生；（2）在调查地区居住半年及以上的中职学生。排除标准：（1）调查当天请假的学生；（2）休学在家的学生；（3）拒绝签署知情同意书的学生。最终，共发放问卷2 500份，回收有效问卷2 372份，有效率为94.9%。平均年龄为（16.6±1.2）岁；男生1 286人，女生1 086人。所有参与者均获得知情同意，并通过贵州医科大学伦理委员会批准(批准号：20230181)。
1.2　调查方法
1.2.1　基本情况调查
采用自填式问卷收集研究对象的基本信息，如性别、年龄、睡眠情况 家庭经济、是否独生子女、是否吸烟、饮酒等情况。
1.2.2　恋爱状态
通过题目“你目前的恋爱状态：（1）从未恋爱；（2）曾经恋爱已过失恋期；（3）失恋期；（4）正在恋爱，”分别计分为1、2、3和4分。收集研究对象的恋爱状态。
1.2.3　NSSI评估
使用万宇辉等人[13]编撰的青少年非自杀性自伤行为问卷（ANSAQ），问卷包括行为和功能部分，评估研究对象过去一年的NSSI情况。本研究采用行为部分，包含两个维度的12个项目(有无明显的组织损伤)，采用Likert 5级计分法将“没有、偶尔、有时、经常、总是”分别按0～4评分；12种自伤行为中任何一种发生次数≥1次则界定为有NSSI。得分越高表示NSSI程度越严重。该量表的Cronbach α系数为0.91。
1.2.4　情绪调节困难评估
使用根据Gratz和Roemer[14]编制的情绪调节困难量表（DERS）翻译和修订的中文版情绪调节问卷。使用Likert 5级计分法将“几乎从不、有时、约半数情况、大多数、几乎总是”按1～5评分，得分越高代表情绪调节困难越严重。本研究中该量表Cronbach α系数为0.93。
1.3　质量控制
调查前：咨询专业人士确保问卷可靠性，进行预调查；对调查员进行严格培训，统一调查方法与标准。调查中：每份问卷完成后核查人员对问卷所有项目进行检查；保证每份问卷内容填写完整。调查后：对问卷逻辑错误检查，问卷进行双人双录入，数据进行一致性检验。
1.4　统计分析方法
使用EpiData 3.1进行数据库的建立与问卷数据的录入，采用SPSS 21.0进行数据分析。符合正态分布的定量数据用（均值±标准差）表示，采用χ2检验进行组间比较；定性数据使用率或构成比表示、用Harman单因子检验法检验数据的共同方法偏差，采用χ2检验对有无NSSI行为两组间进行差异性检验，采用控制变量的偏相关分析探究各变量的相关性，方差分析参考方杰等[15]提出的自变量为类别变量的中介效应分析，使用Process 4.1程序的模型4以从未恋爱为参照水平检验其他水平的中介效应，检验水准α=0.05。
2　结果

2.1　共同方法偏差分析
Harman单因子检验结果显示，有个15公因子特征根大于1，第一个因子解释21.6%的变异量，小于40%的临界值，因此可认为本研究中不存在严重的共同方法偏差。
2.2　基本情况
NSSI行为的总体检出率为27.4%；有NSSI行为的男生336人（26.1%），女生314人(28.9%)；有NSSI行为和无NSSI行为的学生留守经历、吸烟、饮酒、睡眠情况、家庭经济、学习成绩、家庭居住地等方面的差异均具有统计学意义（P<0.05）。见表1。
2.3　恋爱状态
从未恋爱的人数最多，失恋期的最少，但失恋期有NSSI行为的占比最大，占65.7%。见表2。
2.4　方差分析
结果显示，不同恋爱状态对NSSI行为的影响（F=15.89，P<0.05）和对情绪调节困难的影响（F=15.70，P<0.05）有显著差异，不同恋爱状态下NSSI行为能力的均值和情绪调节困难得分均值从高到低排列相同：失恋期>曾经恋爱已过失恋期>正在恋爱>从未恋爱。见表3、4。
2.5　情绪调节困难在恋爱状态与NSSI行为之间的中介效应
结果显示整体总系数效应检验（F=8.32 P<0.001），说明相对总效应不全为0。相对分析结果显示，曾经恋爱已过失恋期状态下，情绪调节困难对NSSI的相对中介效应为0.026（95%CI：-0.021～0.037）、相对直接效应为0.062（95%CI:-0.012～0.051），都不显著；同理，失恋期状态下，情绪调节困难对NSSI的相对中介效应值为0.421（0.062～0.203），95%置信区间不包含0，表明相对中介效应显著（a1=0.376， b1=0.410， a1b1=0.154），失恋期对NSSI的相对总效应c1=0.575（95%CI：0.104～0.258），表明情绪调节困难在其中起到部分中介效应为26.8%（a1b1/c1），见图2；情绪调节困难在正在恋爱对NSSI的相对直接效应c2=0.165（95%CI：0.011～0.092），相对中介效应为0.087（95CI%：-0.009～0.065）（a2= 0.189， b2=0.4103， a2b2=0.078），根据温忠麟等的中介效应模型检验，可知情绪调节困难在正在恋爱与NSSI行为之间起完全中介作用，效果量为47.3%。见表5。

本研究发现，贵州省中职学生NSSI行为的检出率为27.4%，这一结果高于武汉市中职学生的调查结果(14.0%)[16]以及贵州省初中生的调查结果（23.0%）[17]，这种差异可能源于教育资源分布不均衡和研究群体不同有关。女生（28.9%）NSSI行为检出率略高于男生（26.1%），虽有研究表明女性青少年的NSSI患病率高于男性，但不同地区存在显著差异[18]，而且可能样本量的限制会导致这一结果无统计学意义。
本研究结果发现，以从未恋爱为参照的情况下，情绪调节困难在正在恋爱和NSSI行为间起到完全中介作用，这表明恋爱关系本身并非直接导致NSSI行为，而是通过情绪调节困难这一中介变量间接影响NSSI行为。其原因可能是：青少年稳定的恋爱关系可能为个体提供满足感和情感支持，有助于情绪的稳定。相反，由于对恋人的依赖、未来的不确定性、双方的冲突与争吵、理性与现实的落差甚至暴力等行为产生的强烈情感波动，这些过于强烈而不稳定的情绪导致他们产生焦虑、抑郁等负面情绪，当个体不能有效调节这些负面情绪，就会采取NSSI行为来转移自己的注意力，以及身体的疼痛使情绪得到释放和压力的减轻。其中，体验回避模型指出， NSSI行为被个体视为一种应对机制来回避或逃离他们不愿面对的体验和行为[19]。
本研究进一步发现，情绪调节困难在失恋期与NSSI行为中起到部分中介作用，表明失恋不仅可以直接导致NSSI行为的发生，还能通过情绪调节困难进一步增强对NSSI的影响。失恋期与恋爱期的情绪变化可能是情绪调节困难在这两种状态与NSSI行为之间中介作用差异的原因。失恋通常伴随极度的悲伤、痛苦、绝望、愤怒等负面情绪，对于心理韧性较差的个体可能更倾向于通过NSSI宣泄自身的情绪。个体无法承受由失恋引发的消极情绪，可能是情绪调节困难在其中发挥部分中介作用的原因之一，如因恋爱减少社交导致失恋之后没有朋友的安慰和帮助、由于感情的破裂导致对自我价值的怀疑和否定、害怕周围同学的议论，这些因素导致个体的痛苦情绪无法释放，加剧了孤独、无助、焦虑情绪，进而增加NSSI行为的风险。因此，认知-情绪模型解释认为，当个体在调节负面情绪时遇到困难，并且意识到NSSI行为是调节这些情绪的有效策略，那么他们更有可能倾向采取NSSI行为[20]。随着情绪调节困难的加剧，NSSI行为的频率和复杂性也可能增加[21]。
无论是恋爱中的情绪波动还是失恋后的负面情绪，都揭示了情绪调节的重要作用。虽然NSSI行为可能在短期内缓解痛苦，但是长期来看往往会形成恶性循环，加重情绪问题和增加自伤行为的频率和严重性。因此，社会及相关教育机构政策人员应予以重视和关注。一方面，学校应该引导学生树立正确的恋爱观和价值观；培训教师和开设情绪教育课程，指导学生认识和理解自身情绪以及如何去表达和调节情绪。另一方面，家长应以尊重和负责任的态度为子女树立榜样；学习情绪辅导技巧，增进家庭成员之间的沟通和理解来提高孩子情绪调节能力。另外，公共宣传应通过提高公众对情绪调节能力重要性的认识来构建一个更加支持和理解的社会环境。青春期恋爱是情感发展的一部分，在无法完全避免的情况下，应通过科学的教育和引导，家校联合共同帮助学生建立健康的情感观，正确处理自己的情绪，减少NSSI行为的发生。
综上，情绪调节困难在NSSI行为的发生中扮演着关键角色，理解其在不同恋爱状态下的作用机制对有效的制定预防和干预措施具有重要意义。由于本研究是横断面研究，且采用了自填式问卷调查方式，极少数个体可能在调查时间点存在恋爱状态的快速转换，单次的测量可能准确度会受限，这也是我们当前研究在控制恋爱状态动态变化方面存在的局限性，未来的研究可以纵向研究设计、动态追踪调查以期在一定程度上减小影响。
