Objective

To analyze the epidemiological characteristics of Hepatitis A (HA) and Hepatitis E (HE) in Huai’an City,Jiangsu Province from 2004 to 2023, providing a scientific basis for infectious disease prevention and control in the region.

Methods

Case report data for HA and HE from 2004 to 2023 in Huai’an city were extracted from the Chinese Disease Prevention and Control Information System for descriptive analysis.

Results

From 2004 to 2023, a total of 1 956 cases of HA were reported in Huai’an city, accounting for 4.14% of viral hepatitis cases, with an average annual incidence rate of 1.97 per 100 000 population.Join point regression analysis indicated a significant overall declining trend in HA incidence from 2004 to 2023, with an Average Annual Percentage Change (AAPC) of -12.816% (P=0.016), with inflection points identified in 2006, 2015, and 2018. The peak months for HA cases were March to June and August; the male-to-female incidence ratio was 2.04, with the highest number of cases reported in the age group of 50 to <55 years. Farmers constituted the largest occupational group, accounting for 72.65% (1 421 cases). During the same period, a total of 3 097 cases of HE was reported, accounting for 6.55% of viral hepatitis cases, with an average annual incidence rate of 3.12 per 100 000 population. Join point regression analysis showed a significant overall increasing trend in HE incidence from 2004 to 2023, with an AAPC of 9.266% (P=0.001) and inflection points in 2011 and 2020.The peak months for HE cases were January to May; the male-to-female incidence ratio was 2.93, with the highest number of cases reported in the age group of 55 to <60 years. Farmers also represented the highest occupational group, accounting for 74.91%(2 320 cases).

Conclusion

From 2004 to 2023, HA showed a declining trend while HE exhibited an increasing trend in Huai’an city. Future efforts should focus on implementing vaccination strategies and prioritizing farmers as a key population for comprehensive prevention and control measures to effectively manage the spread of HA and HE.

目的

分析2004—2023年江苏省淮安市甲型病毒性肝炎(简称甲肝)、戊型病毒性肝炎(简称戊肝)的流行病学特征,为本地区传染病防控工作提供科学依据。

方法

提取中国疾病预防控制信息系统中淮安市2004—2023年的甲肝、戊肝病例报告数据进行描述分析。

结果

2004—2023年淮安市累积报告甲肝1 956例,占病毒性肝炎4.14%,年平均报告发病率为1.97/10万。Joinpoint回归分析提示,甲肝在2004—2023年发病率总体呈下降趋势,平均年度变化百分比(AAPC)为-12.816%,差异具有统计学意义(P=0.016),在2006、2015和2018年出现拐点;3—6月、8月为甲肝高发月份;男性与女性的发病数比值为2.04;报告发病最多的为50~<55岁年龄组;职业以农民构成比最高,1 421例,占72.65%。2004—2023年淮安市累积报告戊肝3 097例,占病毒性肝炎6.55%,年平均报告发病率为3.12/10万。Joinpoint回归分析提示,戊肝在2004—2023年发病率总体呈上升趋势,AAPC=9.266%,差异具有统计学意义(P=0.001),在2011和2020年出现拐点。1—5月为戊肝高发月份;男性与女性的发病数比值为2.93;报告发病最多的为55~<60岁年龄组;职业以农民构成比最高,2 320例,占74.91%。

目的

分析2004—2023年江苏省淮安市甲型病毒性肝炎(简称甲肝)、戊型病毒性肝炎(简称戊肝)的流行病学特征,为本地区传染病防控工作提供科学依据。

方法

提取中国疾病预防控制信息系统中淮安市2004—2023年的甲肝、戊肝病例报告数据进行描述分析。

结果

2004—2023年淮安市累积报告甲肝1 956例,占病毒性肝炎4.14%,年平均报告发病率为1.97/10万。Joinpoint回归分析提示,甲肝在2004—2023年发病率总体呈下降趋势,平均年度变化百分比(AAPC)为-12.816%,差异具有统计学意义(P=0.016),在2006、2015和2018年出现拐点;3—6月、8月为甲肝高发月份;男性与女性的发病数比值为2.04;报告发病最多的为50~<55岁年龄组;职业以农民构成比最高,1 421例,占72.65%。2004—2023年淮安市累积报告戊肝3 097例,占病毒性肝炎6.55%,年平均报告发病率为3.12/10万。Joinpoint回归分析提示,戊肝在2004—2023年发病率总体呈上升趋势,AAPC=9.266%,差异具有统计学意义(P=0.001),在2011和2020年出现拐点。1—5月为戊肝高发月份;男性与女性的发病数比值为2.93;报告发病最多的为55~<60岁年龄组;职业以农民构成比最高,2 320例,占74.91%。

结论

2004—2023年淮安市甲肝呈现下降、戊肝呈现上升趋势,今后应进一步落实疫苗接种策略,并将农民作为重点防控人群,实施综合防控措施,有效控制甲肝、戊肝的流行。

Objective

To analyze the epidemiological characteristics of Hepatitis A (HA) and Hepatitis E (HE) in Huai’an City,Jiangsu Province from 2004 to 2023, providing a scientific basis for infectious disease prevention and control in the region.

Methods

Case report data for HA and HE from 2004 to 2023 in Huai’an city were extracted from the Chinese Disease Prevention and Control Information System for descriptive analysis.

Results

From 2004 to 2023, a total of 1 956 cases of HA were reported in Huai’an city, accounting for 4.14% of viral hepatitis cases, with an average annual incidence rate of 1.97 per 100 000 population.Join point regression analysis indicated a significant overall declining trend in HA incidence from 2004 to 2023, with an Average Annual Percentage Change (AAPC) of -12.816% (P=0.016), with inflection points identified in 2006, 2015, and 2018. The peak months for HA cases were March to June and August; the male-to-female incidence ratio was 2.04, with the highest number of cases reported in the age group of 50 to <55 years. Farmers constituted the largest occupational group, accounting for 72.65% (1 421 cases). During the same period, a total of 3 097 cases of HE was reported, accounting for 6.55% of viral hepatitis cases, with an average annual incidence rate of 3.12 per 100 000 population. Conclusion

From 2004 to 2023, HA showed a declining trend while HE exhibited an increasing trend in Huai’an city. Future efforts should focus on implementing vaccination strategies and prioritizing farmers as a key population for comprehensive prevention and control measures to effectively manage the spread of HA and HE.

甲型病毒性肝炎(简称甲肝)和戊型病毒性肝炎(简称戊肝)是由甲肝病毒(hepatitis A virus,HAV)和戊肝病毒(hepatitis E virus,HEV)感染引起的肠道传染病,临床表现以疲乏、食欲减退、肝功能异常为主[1-3],两者均主要经粪口途径传播。随着我国公共卫生状况的明显改善,甲肝病例较以前明显减少,戊肝主要流行株也逐渐从基因型1型转变为基因型4型,即“猪戊肝病毒”,流行于我国、东南亚地区[14-5]。淮安市位于江苏省北部中心地域,境内河湖交织、水网密集,洪涝灾害多发,存在肠道传染病局部暴发的风险。为进一步了解淮安市甲肝、戊肝的流行特征,为地区传染病防控提供科学依据,本研究对2004—2023年淮安市长达20年的甲肝、戊肝监测数据进行分析。
来源于中国疾病预防控制信息系统中淮安市2004年1月—2023年12月按发病日期统计的甲肝、戊肝数据,包括临床诊断与确诊病例,资料包括报告病例数、报告发病率、病例年龄、性别、职业等。
年均发病率(/10万)=年均发病数/年均人口数×10万。季节指数定义为各年同月发病的平均数与各年全部月份平均数的比值,数值≥1提示月份高发[6]。人群分布中,本研究按5岁为一个年龄段进行分组。
采用Joinpoint 4.9.1.0软件分析2004—2023年甲肝、戊肝报告发病率的时间趋势[7-8],计算年度变化百分比(annual percent change, APC)、平均年度变化百分比(average annual percent change,AAPC)和95%置信区间(confidence interval, CI)。APC用来评价模型拟合出最佳拐点数量所划分的各个时间段甲肝、戊肝报告发病率的变化趋势,AAPC用来评价2004—2023年整体报告发病率的变化趋势。APC或AAPC<0表示该时间段甲肝、戊肝报告发病率呈下降趋势;APC或AAPC>0表示该时间段甲肝、戊肝报告发病率呈上升趋势;APC=AAPC则无拐点。检验水准α=0.05。
2004—2023年淮安市共报告甲肝1 956例,占病毒性肝炎4.14%,年平均报告发病率1.97/10万。报告病例数最多为2004年382例。2004—2023年淮安市累积报告戊肝3 097例,占病毒性肝炎6.55%,年平均报告发病率3.12/10万。报告病例数最多为2011年258例。不同年份报告甲、戊肝病例数、发病率见图1
Joinpoint回归分析提示,2004—2023年甲肝发病率总体呈下降趋势,AAPC=-12.816%,发病率平均每年下降12.816%(P=0.016)。在2006、2015和2018年出现拐点,其中,2004—2006年APC=-30.246%(P=0.017),2006—2015年APC=-16.064%(P<0.001),2015—2018年APC=23.312%,上升趋势无统计学意义,2018—2023年APC=-17.104%(P=0.037),见表1图2。2004—2023年1—12月份淮安市甲肝报告季节指数分别为0.77、0.95、1.71、1.42、1.04、1.01、0.81、1.07、0.98、0.72、0.87、0.65。
Joinpoint回归分析提示,2004—2023年戊肝发病率总体呈上升趋势,AAPC=9.266%,发病率平均每年上升9.266%(P=0.001)。在2011和2020年出现拐点,其中,2004—2011年APC=23.984%(P<0.001),2011—2020年APC=-6.289%(P=0.023),2020—2023年APC=28.974%(P=0.028),见表1图2。2004—2023年1—12月淮安市戊肝季节指数分别为1.09、1.53、1.82、1.40、1.01、0.70、0.74、0.74、0.64、0.65、0.86、0.88。
1 956例甲肝病例中,男性1 313例,女性643例,男性、女性的发病数比为2.04∶1。病例数集中在30~74岁,占全部病例数的80.16%(1 568/1 956),以50~<55岁报告发病数最多,占比12.67%,见图3。职业构成居前三位的为农民(1 421例,占72.65%)、工人(100例,占5.11%)、家务及待业(100例,占5.11%)。
3 097例戊肝病例中,男性2 308例,女性789例,男性、女性的发病数比为2.93∶1。病例数集中在30~74岁,占全部病例数的87.63%(2714/3 097),以55~<60岁报告发病数最多,占比14.76%,见图3。职业构成居前三位的为农民(2 320例,占74.91%)、家务及待业(217例,占7.01%)、离退人员(183例,占5.91%)。
本研究首次基于joinpoint回归模型分析了淮安市长达20年的甲肝、戊肝流行特征。结果表明,2004—2023年淮安市甲肝总体呈下降趋势(P= 0.016),与全国[9]及全省[10]报告的趋势基本一致,主要可能与近年来公共卫生状况持续改善,饮用水源中甲肝病毒污染得到逐步控制,以及甲肝疫苗推广接种[11]等因素有关。
2004—2023年淮安市戊肝发病总体呈上升趋势(P=0.001),高于全国水平,可能与医疗机构诊断水平提升、监测敏感性提高、居民就诊意识增强等有关[12-13];但低于江苏省平均水平,可能与经济发展、人口、城市环境发展不平衡等因素有关[14]。2020年报告发病水平下降至低位,可能受新发传染病影响。近年来,因猪源污染导致戊肝病例报告增多,基于模型评估发现,江苏省戊肝发病主要与猪-人传播途径有关,而与环境-人传播途径关系不密切[15]。2015—2016年淮安市[16]戊肝监测数据均表明戊肝病例以基因型4型感染为主,病例与生猪感染的病毒高度同源。基于防控现状,建议后期开展职业暴露人群和宿主动物监测专项调查。
淮安市甲肝、戊肝病例主要集中于30~74岁,以50~60岁报告居多,男性发病高于女性,职业构成中农民占比最高,这些人群分布特征同国内研究基本一致[17-18]。中老年农民是甲肝、戊肝的高发群体,一方面可能与农村地区环境卫生、饮水卫生欠缺导致农民群体易于接触到受病毒污染的水源、猪源等有关,另一方面可能是个人卫生意识较差,包括洗手消毒不勤、加工制作菜肴生熟不分等。研究表明,农村地区老年人预防传染病的知识、态度及行为处于较低水平,年龄、文化程度低等是影响农村传染病防控素养的主要因素[19-20]。因此,应将农民作为甲肝、戊肝的重点防控人群进行健康宣教。
综上所述,2004—2023年淮安市甲肝呈现下降趋势,而戊肝总体呈上升趋势,农民是发病的重点人群,今后应继续落实疫苗接种、健康宣教等综合防控措施,以减少甲肝、戊肝发病。
