Objective

To study the association between the food environment and the risk of diabetes among residents and the mediating role of dietary intake.

Methods

Based on the data from the surveillance and investigation of chronic diseases and risk factors in Hubei Province from 2018 to 2020, buffers with different radii were constructed centered on the residential areas of the study subjects, and the densities of fast-food restaurants, dessert shops, supermarkets, vegetable markets, and convenience stores in each buffer were calculated to measure the food environment. A multilevel logistic regression model was used to analyze the association between the food environment and the risk of diabetes among residents, as well as the mediating effect of different types of food intake.

Results

A total of 24 467 adults were included in this study, and the prevalence of diabetes was 12.1%. Regression analysis showed that the density of vegetable markets was positively correlated with the risk of diabetes (OR=1.018, 95%CI: 1.004-1.031). The results of the mediating analysis showed that the density of vegetable markets could reduce the risk of diabetes by increasing fruit intake, and the proportion of the mediating effect was 1.32%. Sub-group analysis showed that in the population under 60 years old, the density of supermarkets was negatively correlated with the risk of diabetes (OR=0.962, 95%CI: 0.937-0.988).

Conclusion

The density of vegetable markets is positively correlated with the risk of diabetes among residents, but a higher density of vegetable markets can reduce the risk of diabetes by increasing fruit intake. Optimizing the layout of the food environment and improving the availability of healthy foods can help reduce the risk of diabetes among residents.

目的

研究食品环境与居民糖尿病患病风险的关联及膳食摄入的中介作用。

方法

基于2018—2020年湖北省慢性病及危险因素监测调查数据,以研究对象居住区为中心,构建不同半径的缓冲区,并计算各缓冲区内快餐店、甜品店、超市、菜市场和便利店密度来测度食品环境。采用多水平logistic回归模型,分析食品环境与居民糖尿病患病风险的关联,以及不同类型食物摄入量在其中的中介效应。

结果

本研究共纳入24 467名成年人,糖尿病患病率为12.1%。回归分析表明,菜市场密度与糖尿病患病风险呈正相关(OR=1.018,95%CI:1.004~1.031)。中介分析结果显示,菜市场密度能通过增加水果摄入量降低糖尿病患病风险,其中介效应占比为1.32%。亚组分析表明,在<60岁人群中,超市密度与糖尿病患病风险呈负相关(OR=0.962,95%CI:0.937~0.988)。

结论

菜市场密度与居民糖尿病患病风险呈正相关,但较高的菜市场密度能通过增加水果摄入进而降低糖尿病患病风险。优化食品环境布局、提高健康食品可得性有助于降低居民糖尿病患病风险。

目的

研究食品环境与居民糖尿病患病风险的关联及膳食摄入的中介作用。

方法

基于2018—2020年湖北省慢性病及危险因素监测调查数据,以研究对象居住区为中心,构建不同半径的缓冲区,并计算各缓冲区内快餐店、甜品店、超市、菜市场和便利店密度来测度食品环境。采用多水平logistic回归模型,分析食品环境与居民糖尿病患病风险的关联,以及不同类型食物摄入量在其中的中介效应。

结果

本研究共纳入24 467名成年人,糖尿病患病率为12.1%。回归分析表明,菜市场密度与糖尿病患病风险呈正相关(OR=1.018,95%CI:1.004~1.031)。中介分析结果显示,菜市场密度能通过增加水果摄入量降低糖尿病患病风险,其中介效应占比为1.32%。亚组分析表明,在<60岁人群中,超市密度与糖尿病患病风险呈负相关(OR=0.962,95%CI:0.937~0.988)。

结论

菜市场密度与居民糖尿病患病风险呈正相关,但较高的菜市场密度能通过增加水果摄入进而降低糖尿病患病风险。优化食品环境布局、提高健康食品可得性有助于降低居民糖尿病患病风险。

Objective

To study the association between the food environment and the risk of diabetes among residents and the mediating role of dietary intake.

Methods

Based on the data from the surveillance and investigation of chronic diseases and risk factors in Hubei Province from 2018 to 2020, buffers with different radii were constructed centered on the residential areas of the study subjects, and the densities of fast-food restaurants, dessert shops, supermarkets, vegetable markets, and convenience stores in each buffer were calculated to measure the food environment. A multilevel logistic regression model was used to analyze the association between the food environment and the risk of diabetes among residents, as well as the mediating effect of different types of food intake.

Results

A total of 24 467 adults were included in this study, and the prevalence of diabetes was 12.1%. Regression analysis showed that the density of vegetable markets was positively correlated with the risk of diabetes (OR=1.018, 95%CI: 1.004-1.031). The results of the mediating analysis showed that the density of vegetable markets could reduce the risk of diabetes by increasing fruit intake, and the proportion of the mediating effect was 1.32%. Sub-group analysis showed that in the population under 60 years old, the density of supermarkets was negatively correlated with the risk of diabetes (OR=0.962, 95%CI: 0.937-0.988).

Conclusion

The density of vegetable markets is positively correlated with the risk of diabetes among residents, but a higher density of vegetable markets can reduce the risk of diabetes by increasing fruit intake. Optimizing the layout of the food environment and improving the availability of healthy foods can help reduce the risk of diabetes among residents.

糖尿病作为全球性重大慢性非传染性疾病,其患病率持续上升,已成为公共卫生领域的重大挑战[1-2]。1990—2021年,全球糖尿病患病率由3.2%增至6.1%,患病人数高达5.29亿[3]。我国糖尿病患者人数位居全球首位,目前成年人糖尿病患病率已高达12.4%,且预计到2030年将上升至19.8%,这将极大地增加疾病负担[4]。健康膳食是防控糖尿病有效且经济的措施[5],而食品环境作为居民获取食物的主要途径,被发现与个体膳食摄入[6-7]和糖尿病患病风险[8-10]密切相关。然而,当前食品环境与糖尿病患病风险的实证研究多来源于欧美国家,且研究结论尚未取得共识[8-10]。同时,我国食品环境特征和居民饮食习惯均与欧美国家存在较大差异[11],现有研究结果难以直接应用于我国糖尿病防控实践。此外,目前尚缺乏基于我国食品环境与糖尿病患病风险关联的研究证据,且鲜有研究分析其影响机制,亟待开展相关研究为糖尿病防控提供基于我国食品环境的科学依据。因此,本研究基于2018—2020年湖北省慢性病及危险因素监测调查数据,探究食品环境和居民糖尿病患病风险的关联及膳食摄入的中介作用,研究结果将深化对食品环境如何影响居民糖尿病患病风险的理解,并为我国糖尿病防控策略的制定提供食品环境优化视角下的科学证据。
本研究数据来源于2018—2020年湖北省慢性病及危险因素监测调查。该调查采用多阶段整群随机抽样方法,按照城镇化率和人口结构于2018—2020年在湖北省抽取13个地区,随后在每个地区随机抽取1~2个区(县),其次在每个区(县)随机抽取4个街道(乡镇),再次在每个街道(乡镇)随机抽取5个60户及以上的居委会(村),在每个居委会(村)内随机抽取45户居民(村民),最终纳入27 962名18岁及以上的常住居民(村民)作为研究对象。纳入标准:调查前12个月内在监测点居住6个月及以上;非孕妇;未因严重疾病导致行动障碍;无语言、认知、精神障碍等可能影响调查的状况。排除标准:缺失居住地地址数据;缺失空腹血糖检测结果;缺失协变量信息。项目通过了国家慢性和非传染性疾病预防控制中心伦理委员会审查(批号:2018001),所有研究对象均在调查前签署知情同意书。
采用中国成人慢性病及危险因素监测调查问卷,由统一培训并考核合格的调查员进行面对面问卷调查。问卷内容包括年龄、性别、文化程度等基本信息,吸烟、饮酒、身体活动、膳食摄入等生活方式信息,以及糖尿病诊断史和控制血糖措施等病史信息。现场调查还包括身高、体重等身体测量和空腹静脉血采集。血液采集要求调查对象空腹8~12 h,于次日上午7~9点在监测点各基层医疗机构采集静脉血10 ml,统一运输至实验室测量空腹血糖。空腹血糖由通过实验室性能验证的监测点实验室负责检测,采用己糖激酶法或葡萄糖氧化酶法现场测定血浆血糖。调查采用自主开发的平板电脑调查系统对问卷、身体测量等信息进行实时录入,经由湖北省疾病预防控制中心校对并复核。
(1)糖尿病诊断标准:空腹血糖(FPG)≥7.0 mmol/L者或既往被社区/乡镇及以上级别医疗机构诊断为糖尿病者或正在服用降糖药物和(或)注射胰岛素治疗者[12],满足以上条件之一即可诊断为糖尿病患者。(2)食品环境:基于2018年高德地图API提取与餐饮服务和食品零售相关的13类兴趣点(point of interest, POI)数据,各类POI均为高德地图二级分类,每类POI相互独立。基于既往文献[610],将13类POI重分类为餐厅、快餐店、甜品店、超市、菜市场、便利店6类食品店,每类食品店定义及其划分标准见表1。以研究对象居住区中心点为圆心,构建250、500、1 000和1 500 m的圆形缓冲区和路网缓冲区,并计算不同尺度缓冲区内各类食品店密度来表征食品环境。(3)协变量:基于既往研究[10],协变量包括人口学特征指标和生活方式指标。人口学特征变量包括性别(男性、女性)、年龄(<60岁、≥60岁)、民族(汉族、少数民族)、文化程度(高中以下、高中及以上)、婚姻状况(已婚/同居、未婚/丧偶/离婚/独居)、职业状况(在职、无业)和居住地(城市、农村)。生活方式变量包含吸烟(是、否)、饮酒(是、否)和身体活动不足(是、否)。有吸烟史者定义为吸烟,包含现在和曾经吸烟者;在过去一年中饮酒频率≥1次定义为饮酒;一周内中等强度活动时间不足150 min或高等强度动时间不足75 min或同等量活动时间不足者定义为身体活动不足[13]。(4)中介变量:使用食物频率问卷收集食物摄入种类、食用频率和食用量,参照《中国居民膳食指南(2022)》,选取蔬菜、水果、粮谷和红肉四类食物,并根据各类食物的食用频率和食用量计算居民每类食物的日均摄入量(g/d)[14]
使用R 4.2.2软件进行统计分析,计数资料以频数和构成比表示,使用χ2检验进行组间差异比较。考虑到同一居委会(村)的不同居民(村民)在食品环境暴露上存在层次聚集性,以居民(村民)个体为水平1,居委会(村)为水平2,使用多水平logistic回归模型探究食品环境与居民糖尿病患病风险的关联。由于快餐店与其他食品店之间存在多重共线性,未纳入后续模型分析。以500 m路网缓冲区内各类食品店密度作为解释变量,模型1调整性别、年龄和民族,并为水平2添加随机截距;模型2在模型1基础上,进一步调整文化程度、婚姻状况、职业状况和居住地;模型3在模型2基础上,进一步调整吸烟、饮酒和身体活动。
为验证模型3结果的稳健性,采用如下敏感性分析:(1)使用250 m路网缓冲区测度食品环境暴露(模型3a);(2)使用1 500 m路网缓冲区测度食品环境暴露(模型3b);(3)使用500 m圆形缓冲区替代路网缓冲区测度食品环境暴露(模型3c);(4)排除患有肥胖(BMI≥28 kg/m2)的研究对象(模型3d)。采用亚组分析探索食品环境和糖尿病患病风险关联在不同人群中的差异。通过中介分析探究蔬菜、水果、粮谷和红肉的日均摄入量在食品环境与糖尿病患病风险关联中的中介作用。双侧检验,检验水准α=0.05。
共纳入24 467名研究对象进行分析,其中<60岁人群16 977人(69.4%),男性9 728人(39.8%),汉族21 314人(87.1%),高中以下文化程度18 929人(77.4%),已婚/同居者22 269人(91.0%),在职18 126人(74.1%),农村居民22 696人(92.8%)。研究样本中糖尿病患病率为12.1%。见表2
在模型1、模型2和模型3中,菜市场密度与糖尿病患病风险呈显著正相关,OR值分别为1.018(95%CI:1.005~1.032)、1.018(95%CI:1.004~1.031)和1.018(95%CI:1.004~1.031)。便利店密度仅在模型1中与糖尿病患病风险呈显著负相关,OR值为0.989(95%CI:0.981~0.998)。见表3
使用250 m路网缓冲区(OR=1.017, 95%CI:1.006~1.029)、1 500 m路网缓冲区(OR= 1.040,95%CI:1.003~1.078)和500 m圆形缓冲区(OR=1.029,95%CI:1.004~1.054)测度食品环境暴露时,菜市场密度与糖尿病患病风险的正向关联均保持稳健。排除肥胖人群后,菜市场密度与糖尿病患病风险的关联与模型3基本一致(OR=1.017,95%CI:1.004~1.031)。见表4
超市密度与<60岁人群的糖尿病患病风险呈负相关(OR=0.962,95%CI:0.937~0.988),但与≥60岁人群的糖尿病患病风险不存在显著关联(OR=1.009,95%CI:0.984~1.036)。未观察到其他食品店密度与糖尿病患病风险关联的人群异质性。见图1
水果摄入量介导了菜市场密度与糖尿病患病风险关联,中介效应占比1.32%。较高的菜市场密度通过增加水果摄入量进而降低糖尿病患病风险,其间接效应量为-0.000 304(P=0.040)。见图2
本研究利用湖北省慢性病及危险因素监测调查数据,分析了食品环境与糖尿病患病风险的关联,并探讨了各类食物摄入量的中介效应。结果显示,菜市场密度与糖尿病患病风险呈正相关,但会通过增加水果摄入量降低糖尿病患病风险。超市密度和<60岁人群的糖尿病患病风险呈负相关。
较高的菜市场密度会显著增加居民糖尿病患病风险,与既往东亚国家的研究结果相似。一项基于1 342名韩国儿童的研究显示,较高的菜市场密度与儿童肥胖风险的增加相关[15]。然而,这一发现与欧美国家的研究结论不同。一项基于美国纽约州680个学区的生态学研究发现,较高的市场密度与较低的儿童肥胖率相关[16]。另一项基于2 808名墨西哥成年人的研究也显示,果蔬市场的减少与较高的糖尿病患病风险相关[17]
菜市场密度的增加能提高水果摄入量,进而降低居民糖尿病患病风险。大量研究表明较高的菜市场密度与水果摄入增加相关[18-20],而水果富含膳食纤维,能通过影响炎症、肠道菌群等调节血糖水平,进而降低糖尿病患病风险[21]。菜市场密度与糖尿病患病风险的正向关联可能源于菜市场提供的加工类食品,其含有较高的盐分和添加剂增加了糖尿病患病风险[22]。这些发现为我国以食品环境优化为核心的干预策略奠定了基础,未来研究需要更加深入地探讨食品环境、膳食摄入和健康结局间的复杂交互影响。
研究发现超市密度和<60岁人群的糖尿病患病风险呈负相关,这与一项基于4 100 650名美国退伍军人的纵向队列研究结果一致[23]。超市通常能够提供多样且新鲜的水果和蔬菜,促进居民均衡饮食[24],进而降低糖尿病患病风险。这些结果表明优化食品环境是降低糖尿病患病风险的潜在途径。
研究首次基于大规模人群队列数据揭示了我国食品环境与居民糖尿病患病风险之间的关联及其中介路径,对我国食品环境优化及糖尿病防控策略的制定具有重要意义。本研究存在局限性。首先,仅考虑了个体在居住区的食品环境暴露,忽视了个体在其他地理空间的食物环境暴露,研究结果可能存在一定偏倚。其次,研究对象来自于湖北省,结果可能无法直接外推到其他地区。此外,慢性病及危险因素监测调查问卷仅纳入了蔬菜、水果、粮谷和红肉的摄入信息,未能考虑其它食物种类的影响,从而限制了本研究评估膳食摄入作为影响路径的全面性。然而,这四类食物在居民日常饮食结构中占据较大比例,研究结果对相关政策的制定仍具有重要的意义。后续随访将优化问卷设计,拓展食物种类来全面评估膳食摄入在关联中的中介作用。最后,横断面研究设计限制了食品环境和糖尿病患病风险的因果关联推断。
综上所述,食品环境对居民糖尿病患病风险具有显著影响且因年龄而异,食物摄入是这一关联的重要中介。研究结果有望促进改善食品环境的公共卫生政策制定,深化推进我国糖尿病防控工作。
