Objective

To analyze the current situation of sleep quality and its relationship with diabetes among elderly Bai people in rural areas of Dali, Yunnan.

Methods

A multi - stage stratified random sampling method was used to select 1 418 rural Bai elderly people aged ≥60 years in Dali city, Yunnan Province for questionnaire surveys and physical examinations. Binary multi-factor Logistic regression was used to analyze the relationship between sleep quality and the prevalence of diabetes.

Results

The sleep disorder rate in the surveyed population was 48.4%, with 41.1% in men and 54.9% in women. The sleep disorder rate in women was higher than that in men (χ2=26.818, P < 0.001).The prevalence of diabetes was 15.9%, with 15.2% in men and 16.6% in women. The sleep disorder rate increased with age (χ2trend =4.607,P < 0.05). The results of multi-factor Logistic regression showed that the elderly with sleep disorders were more likely to suffer from diabetes than those without sleep disorders (OR=1.425, 95%CI: 1.059-1.916).

Conclusion

Sleep disorder is an important risk factor for diabetes in the elderly. Strengthening sleep management for the elderly is helpful for the prevention of diabetes.

目的

分析云南大理农村白族老年人的睡眠质量现状及与糖尿病的关系。

方法

采用多阶段分层随机抽样，在云南省大理市选取1 418名≥60岁农村白族老年人进行问卷调查和体格检查；采用二分类多因素logistic回归分析睡眠质量与糖尿病患病的关系。

结果

调查人群睡眠障碍率为48.4%，其中男性为41.1%，女性为54.9%，女性的睡眠障碍率高于男性（χ2=26.818，P<0.001）；糖尿病患病率15.9%，其中男性为15.2%，女性为16.6%；睡眠障碍率随年龄增加而升高（χ2趋势=4.607， P<0.05）。多因素logistic回归结果显示，有睡眠障碍的老年人其患糖尿病的可能性要高于无睡眠障碍的老年人（OR=1.425，95%CI:1.059～1.916）。

结论

睡眠障碍是老年人患糖尿病的重要风险因素，加强老年人的睡眠管理，有助于糖尿病的预防工作。

目的

分析云南大理农村白族老年人的睡眠质量现状及与糖尿病的关系。

方法

采用多阶段分层随机抽样，在云南省大理市选取1 418名≥60岁农村白族老年人进行问卷调查和体格检查；采用二分类多因素logistic回归分析睡眠质量与糖尿病患病的关系。

结果

调查人群睡眠障碍率为48.4%，其中男性为41.1%，女性为54.9%，女性的睡眠障碍率高于男性（χ2=26.818，P<0.001）；糖尿病患病率15.9%，其中男性为15.2%，女性为16.6%；睡眠障碍率随年龄增加而升高（χ2趋势=4.607， P<0.05）。多因素logistic回归结果显示，有睡眠障碍的老年人其患糖尿病的可能性要高于无睡眠障碍的老年人（OR=1.425，95%CI:1.059～1.916）。

结论

睡眠障碍是老年人患糖尿病的重要风险因素，加强老年人的睡眠管理，有助于糖尿病的预防工作。

Objective

To analyze the current situation of sleep quality and its relationship with diabetes among elderly Bai people in rural areas of Dali, Yunnan.

Methods

A multi - stage stratified random sampling method was used to select 1 418 rural Bai elderly people aged ≥60 years in Dali city, Yunnan Province for questionnaire surveys and physical examinations. Binary multi-factor Logistic regression was used to analyze the relationship between sleep quality and the prevalence of diabetes.

Results

The sleep disorder rate in the surveyed population was 48.4%, with 41.1% in men and 54.9% in women. The sleep disorder rate in women was higher than that in men (χ2=26.818, P < 0.001).The prevalence of diabetes was 15.9%, with 15.2% in men and 16.6% in women. The sleep disorder rate increased with age (χ2trend =4.607,P < 0.05). The results of multi-factor Logistic regression showed that the elderly with sleep disorders were more likely to suffer from diabetes than those without sleep disorders (OR=1.425, 95%CI: 1.059-1.916).

Conclusion

Sleep disorder is an important risk factor for diabetes in the elderly. Strengthening sleep management for the elderly is helpful for the prevention of diabetes.

在社会飞速发展和人口老龄化加速的时代背景下，老年人的健康状况备受关注。数据显示中国的糖尿病患者已占全球总数的四分之一，并且我国糖尿病人口数正呈快速增长趋势[1]，其中老年人是糖尿病患病的主体[2]。此外，相较于其他年龄段人群，睡眠障碍普遍存在于老年人中，不仅影响他们的生活质量且与多种慢性疾病的发生及不良健康结果有关[3-4]。近年来，随着老年人糖尿病患病率明显升高，且睡眠障碍与糖尿病的共患现象变得愈加普遍，如不及时干预可能会增加糖尿病并发症的风险[5-7]，从而给老年患者的身心健康带来负面影响。中国地域辽阔，不同少数民族地区文化背景独特，在云南的农村边远地区，老年人的健康意识相对较差，对于睡眠问题的重视程度较低，但现有关于睡眠障碍与糖尿病关系的相关研究较少。本研究通过对云南省大理农村白族老年人开展横断面调查，分析白族老年人的睡眠质量现状及与糖尿病的关系，旨在为少数民族地区睡眠健康和糖尿病的综合防控提供科学依据。
1　资料与方法

1.1　资料来源
数据来源于2022年5—6月在云南省白族居民聚居的大理市≥60岁农村老年人开展的问卷调查和体格检查资料。共发放1 500份调查问卷，有效问卷1 418份，问卷有效回收率为94.5%。本研究已获昆明医科大学伦理审查会审批，项目批准号KMMU2020MEC031，所有参与本研究的被调查者均已知情同意。
1.2　研究方法
1.2.1　抽样方法
采用多阶段分层随机抽样法。第一阶段，将大理市下辖的乡镇按经济水平分为低和高两层，每层随机抽取1个乡镇；第二阶段，按比例概率抽样法（probability proportional to size，PPS），在所抽乡镇中各抽取4个行政村，共计8个样本村；第三阶段，在每个样本村随机抽取≥60岁且在当地居住≥5年的白族农村居民作为调查对象。
1.2.2　问卷调查
本研究采用自编问卷，问卷内容包括：一般人口学信息（性别、年龄、文化程度、家庭人均年收入）、睡眠质量情况及糖尿病患病情况等。问卷采集由调查员进行一对一面访调查，所有调查员均经统一培训并考核。
1.2.3　现场体检
由体检人员统一测量并记录调查对象的健康指标，包括血糖、身高、体重、腰围。其中，用快速血糖仪（德国罗氏公司ACCU-CHEK）采集无名指指尖末梢血测定血糖值，确保测量的精准度和一致性。
1.2.4　诊断标准及相关定义
（1）睡眠障碍：采用匹兹堡睡眠质量指数[8]（Pittsburgh sleep quality index，PSQI）量表进行睡眠评价。量表包含七个维度，每个维度按0～3分的等级计分，总分越高，睡眠质量越差，合计分数≤7分为无睡眠障碍，>7分为睡眠障碍；（2）糖尿病[2]：①既往在乡镇级及以上医疗机构确诊为糖尿病；②在未服用降糖药的情况下，空腹血糖≥7.0 mmol/L，或餐后2 h血糖≥11.1 mmol/L；（3）超重和肥胖[9]：以体质指数（body mass index， BMI）衡量，BMI=体重/身高的二次方（kg/m2），24 kg/m2≤BMI<28 kg/m2为超重，BMI≥28 kg/m2为肥胖；（4）中心性肥胖[9]：男性腰围>90 cm或女性腰围>85 cm；（5）家庭人均年收入：呈偏态分布，以中位数划分，<6 565元为低收入，≥6 565元为高收入。
1.3　统计分析
所有问卷均统一核查并编码，采用Epidata 3.1软件双录入，SPSS 24.0软件进行数据分析。计数资料描述采用频数和构成比，不同组间睡眠障碍率和糖尿病患病率的比较采用χ2检验（等级资料采用趋势χ2检验）；采用二分类多因素logistic回归分析睡眠障碍与糖尿病间的关联；检验水准α=0.05。

2.1　调查人群基本情况
在本次调查的1 418例老年人中，男性为669人（47.2%），女性为749人（52.8%）；年龄在60～64岁有223人（15.7%），65～69岁481人（33.9%），70～74岁335人（23.6%），≥75岁379人（26.7%）；文化程度为文盲有581人（41.0%），小学及以上837人（59.0%）；家庭人均年收入<6 565元者为607人（42.8%），家庭人均年收入≥6 565元者为811人（57.2%）。
2.2　云南大理农村白族≥60岁老年人睡眠质量现状
在本次调查的1 418例老年人中，睡眠质量差者为686人，睡眠障碍率为48.4%；其中女性的睡眠障碍率高于男性（54.9% vs. 41.1%，χ2=26.818，P<0.001）；调查人群的睡眠障碍率随年龄的增加而升高（χ2趋势=4.607，P<0.05）；而在不同文化程度和不同家庭人均年收入的人群中，其睡眠障碍率无统计学差异（均P>0.05）。见表1。
2.3　云南大理农村白族≥60岁老年人糖尿病患病情况分析
1 418名调查对象中检出有糖尿病的226人，糖尿病的患病率为15.9%，其中男性为15.2%，女性为16.6%，超重者的糖尿病患病率高于非超重者（χ2=5.530，P<0.05），肥胖者高于非肥胖者（χ2=9.521，P<0.01），中心性肥胖者高于非中心性肥胖者（χ2=10.238，P<0.01），有糖尿病家族史者高于无家族史者（χ2=9.343，P<0.01），不同性别、文化程度和家庭人均年收入的组间糖尿病患病率无统计学差异（均P>0.05）；不同年龄组与糖尿病患病率间无线性趋势（P>0.05）；有睡眠障碍者的糖尿病患病率高于无睡眠障碍者（χ2=6.197，P<0.05）。见表2。
2.4　云南大理农村白族≥60岁老年人睡眠障碍与糖尿病患病关系的logistic回归分析
在二分类单因素logistic回归分析中（模型1），显示睡眠障碍与糖尿病患病之间存在关联（OR=1.436，95%CI:1.079～1.912）。进一步采用二分类多因素logistic回归分析（模型2），以睡眠障碍为自变量（0=否，1=是），糖尿病为因变量（0=否，1=是），校正性别（0=男，1=女）、年龄（0=60～64岁，1=65～69岁，2=70～74岁，3≥75岁）、文化程度（0=文盲，2=小学及以上）、家庭人均年收入（0=低，1=高）、糖尿病家族史（0=否，1=是）、超重（0=否，1=是）、肥胖（0=否，1=是）、中心性肥胖（0=否，1=是）等8个混杂因素，经检验纳入的混杂变量无多重共线性。结果显示，睡眠障碍与糖尿病患病之间关联程度有所降低，但仍存在相关性（OR= 1.425，95%CI:1.059～1.916），即患有睡眠障碍的白族农村老年人其患糖尿病的可能性是无睡眠障碍者的1.425倍。见表3。

随着老龄化进程的不断加深，老年人的睡眠健康问题已日益凸显，本次调查人群中近二分之一（48.4%）的白族农村居民存在睡眠障碍，高于安徽省农村地区[10]（46.4%）和山东省农村地区[11]（33.8%）的研究结果。这一结果的原因可能在于，本次调查人群中有40%以上的白族老年人未受过正规教育；此外，调查地区经济条件较为落后，大多老年人相对面临着更大的家庭压力和较差的生活环境，可能会忽视健康生活方式和高质量睡眠对健康的重要性。本研究显示，白族农村老年群体的睡眠障碍率，随着年龄增长呈现上升趋势，且女性老年群体的睡眠障碍率高于男性群体，这与既往其它一些农村地区的研究结果[11-13]一致。现有流行病学证据表明高龄会带来机体睡眠调节功能机制退化，而老年女性自身的激素水平、行为习惯以及心理健康状况等因素可能是导致其睡眠质量较男性更差的原因[10，13]。结果提示，应特别关注女性和老年群体的睡眠质量问题，通过实施有针对性的干预措施来改善他们的睡眠问题，以提高其生活质量。
本次横断面调查显示，白族农村≥60岁老年人的糖尿病患病率为15.9%，虽然低于全国老年糖尿病患病总体水平（19.4%）[14]，但高于云南省内其它农村地区的调查结果[15-17]。其原因可能与不同地区的经济发展、民族文化和生活习惯等因素有关。大量研究已表明[18-20]，超重、肥胖、中心性肥胖以及糖尿病家族史均为糖尿病的高危风险因素，这与本次研究结果一致，提示这部分人群应当作为糖尿病管理的重点人群，尽早采取预防措施，定期随访和监测血糖。本次调查的人群中，不同性别、文化程度和家庭人均年收入间糖尿病的患病率无统计学差异，年龄组间无线性趋势，与既往研究结果不一致。在不同研究中的疾病诊断技术、样本量的大小以及调查人群的年龄分布可能是导致各研究结果不一致的部分原因，具体原因有待进一步探讨。
本研究结果表明，有睡眠障碍的白族农村老年群体患糖尿病的风险高于无睡眠障碍者，提示睡眠问题是糖尿病患病的重要风险因素，这与其他学者所开展的研究结论一致[21-22]。目前，关于两种疾病间的生物学机制尚不完全明确，但已有研究提出了一些可能的机制。其潜在原因可能是健康睡眠对调节人体血糖内环境稳定有促进作用，而睡眠障碍会通过氧化应激、神经通路、炎症反应、行为机制等调节机制对糖代谢和体重调节产生不利影响[5-6，21，23]。长期睡眠异常可能诱发并加重对糖尿病，还可能对糖尿病的预后产生负面影响，同时糖尿病的发生及其并发症也可能导致睡眠障碍，形成双向的影响关系，若不积极干预和治疗将形成恶性循环[7，24]。因此，应积极重视当地老年人的睡眠常规评估及老年糖尿病患者的睡眠障碍筛查，加强完善睡眠健康管理体系，有助于推动当地少数民族地区睡眠障碍和糖尿病的综合防控水平。
本研究分析了云南大理农村地区白族老年人的睡眠质量和糖尿病患病现状，并揭示了二者之间存在显著关联，但研究存在若干局限性。一方面，研究采用现场问卷调查，调查对象为老年人，可能存在潜在的回忆偏倚。另一方面，由于经费和现场调查条件的限制，糖尿病的诊断基于指尖血糖的测量，可能导致糖尿病患病率的低估。此外，本研究为横断面设计，限制了推断因果关系的能力，未来可通过前瞻性研究进一步验证睡眠质量与糖尿病的关系。
