Objective

To understand the influencing factors of lumbar disc herniation (LDH) among adult residents of different ethnic groups in Gansu Province, providing foundational data for targeted prevention and control measures.

Methods

From May to July 2022,a stratified cluster sampling method was employed based on the altitude of residence (≤1 500 meters, 1 500 to 3 500 meters, >3 500 meters) in Gansu Province. All participants underwent questionnaire surveys, physical examinations, and assessments of blood glucose and lipid levels. A non-conditional binary logistic regression model was used to analyze the influencing factors of LDH prevalence among different ethnic groups.

Results

A total of 4 545 participants were included, with an overall LDH prevalence of 22.8%. The prevalence was highest among the Tibetan population at 30.6%, with significant differences observed among ethnic groups (P<0.05). For the Han ethnic group, a residence duration of less than 36 months, non-drinking, or residing at altitudes of 1 500-3 500 meters were associated with an increased risk of LDH. Conversely, being younger than 30 years, holding a position as a cadre, or residing at altitudes below 1 500 meters were associated with a reduced risk. Among the Tibetan population, low physical activity intensity, an education level of primary school or below, and a residence duration of 24-36 months were linked to an increased risk of LDH, while being male, engaging in low-intensity activities, and being unmarried were associated with a decreased risk. For the Hui ethnic group, a residence duration of 6-23 months or an age range of 30-59 years were related to an increased risk of LDH.

Conclusion

The prevalence of LDH varies among different ethnic groups in Gansu Province, and the influencing factors also differ.

目的

了解甘肃省不同民族成年居民腰椎间盘突出症(lumbar disc herniation,LDH)的影响因素,为针对性制定LDH的防控措施提供基础数据。

方法

于2022年5—7月根据人群居住海拔(<1 500 m、1 500~3 500 m、>3 500 m)在甘肃省进行分层整群抽样。对所有研究对象进行问卷调查、体格检查、血糖血脂水平检测。采用非条件二元logistic回归模型对不同民族LDH患病率的影响因素进行分析。

结果

纳入4 545名研究对象中,LDH总患病率为22.8%,其中藏族患病率最高30.6%,且各民族患病率差异有显著性(P<0.05)。汉族:居住时间小于36月、不饮酒或居住海拔在1 500~3 500 m与增加LDH患病风险有关,年龄<30岁、职业为干部和无业、居住海拔<1 500 m与患LDH的风险降低有关。藏族:低运动强度、文化程度为小学及以下、居住时间24~36月与增加LDH患病风险有关,男性、高强度以下活动、无配偶与患LDH的风险降低有关。回族:居住时间6~23月或年龄30~59岁与增加LDH患病风险有关。

结论

甘肃省各民族居民LDH患病率不同,其影响因素也不同。

目的

了解甘肃省不同民族成年居民腰椎间盘突出症(lumbar disc herniation,LDH)的影响因素,为针对性制定LDH的防控措施提供基础数据。

方法

于2022年5—7月根据人群居住海拔(<1 500 m、1 500~3 500 m、>3 500 m)在甘肃省进行分层整群抽样。对所有研究对象进行问卷调查、体格检查、血糖血脂水平检测。采用非条件二元logistic回归模型对不同民族LDH患病率的影响因素进行分析。

结果

纳入4 545名研究对象中,LDH总患病率为22.8%,其中藏族患病率最高30.6%,且各民族患病率差异有显著性(P<0.05)。汉族:居住时间小于36月、不饮酒或居住海拔在1 500~3 500 m与增加LDH患病风险有关,年龄<30岁、职业为干部和无业、居住海拔<1 500 m与患LDH的风险降低有关。藏族:低运动强度、文化程度为小学及以下、居住时间24~36月与增加LDH患病风险有关,男性、高强度以下活动、无配偶与患LDH的风险降低有关。回族:居住时间6~23月或年龄30~59岁与增加LDH患病风险有关。

结论

甘肃省各民族居民LDH患病率不同,其影响因素也不同。

Objective

To understand the influencing factors of lumbar disc herniation (LDH) among adult residents of different ethnic groups in Gansu Province, providing foundational data for targeted prevention and control measures.

Methods

From May to July 2022,a stratified cluster sampling method was employed based on the altitude of residence (≤1 500 meters, 1 500 to 3 500 meters, >3 500 meters) in Gansu Province. All participants underwent questionnaire surveys, physical examinations, and assessments of blood glucose and lipid levels. A non-conditional binary logistic regression model was used to analyze the influencing factors of LDH prevalence among different ethnic groups.

Results

A total of 4 545 participants were included, with an overall LDH prevalence of 22.8%. The prevalence was highest among the Tibetan population at 30.6%, with significant differences observed among ethnic groups (P<0.05). For the Han ethnic group, a residence duration of less than 36 months, non-drinking, or residing at altitudes of 1 500-3 500 meters were associated with an increased risk of LDH. Conversely, being younger than 30 years, holding a position as a cadre, or residing at altitudes below 1 500 meters were associated with a reduced risk. Among the Tibetan population, low physical activity intensity, an education level of primary school or below, and a residence duration of 24-36 months were linked to an increased risk of LDH, while being male, engaging in low-intensity activities, and being unmarried were associated with a decreased risk. For the Hui ethnic group, a residence duration of 6-23 months or an age range of 30-59 years were related to an increased risk of LDH.

Conclusion

The prevalence of LDH varies among different ethnic groups in Gansu Province, and the influencing factors also differ.

腰椎间盘突出症(lumbar disc herniation,LDH)是指因外力和腰椎间盘退变共同作用下造成的纤维环破裂继而压迫神经造成的腰腿疼,部分患者可成放射性疼痛,是骨科门诊的常见病多发病,约占门诊下腰痛患者的10%~15%,占骨科腰腿痛住院患者的25%~40%,近年来发病率占椎管疾病的第一位;且病情往往迁延不愈、反复发作,进而影响患者正常的工作和生活,给患者的身心健康造成严重损害[1-3]。20~50岁[1]的壮年体力劳动者是LDH的好发年龄,占发病总人数的90%,国家居民3次主要疾病患病情况的调查发现LDH发病率呈上升趋势,在2003年合计两周患病率达到了2.9‰,疾病患病率上升到第九位[4]。目前LDH的治疗效果不好且多数患者无法完全治愈、存在复发风险,严重影响生活质量[5-6],给家庭带来巨大经济负担,成为重要的公共卫生问题。有研究表明LDH与家族史、糖尿病、劳动活动多、工作负荷增大、久坐、久立等因素有关[7-9]。甘肃是一个多民族聚居的省份,第七次人口普查结果显示,人口排名前三的民族分别是汉族、藏族及回族。不同民族生活方式和行为不同,居住的环境也有所不同,其LDH患病率及影响因素可能不同,所以开展本研究为不同民族的LDH的精准防控提供依据。
采用分层多阶段整群抽样的方法。于2022年5—7月以甘肃省17个市辖区、5个县级市、64个县为单位建立抽样框,为了探究海拔对LDH的影响,根据人群居住海拔(低海拔:<1 500 m、中海拔:1 500 m~3 500 m、高海拔>3 500 m)进行分层随机抽取市县,高海拔抽取2个区县、中海拔抽取4个区县、低海拔抽取2个区县,接着在每个区县随机抽取2~3个乡镇或街道的卫生院或卫生服务中心作为本次的调查现场,在其辖区内随机抽取200~300符合标准的常住居民。排除由于其他原因导致的腰腿疼痛、急性疾病入院治疗人员(LDH引起的除外)共调查4 598人,删除部分缺失数据后最终纳入4 545(98.85%)名研究对象数据进行分析。
本研究通过了甘肃省中医院伦理委员会批准(伦理批号2021-119-01),调查人员阐明调查情况,调查对象签署知情同意书后开展调查。
流行病学调查内容包括:研究对象基本信息(性别、年龄、文化程度、居住时间、婚姻状况、职业类型等)、生活方式(吸烟、饮酒、体力活动、体育锻炼)、疾病史(糖尿病、血脂异常、高血压、冠心病、脑卒中)和LDH家族遗传史、LDH诊断、血压、血脂、血糖、身高、体重、腰围。
突出的椎间盘组织刺激和压迫神经根、马尾神经所导致的临床综合征,表现为腰痛、下肢放射痛、下肢麻木、下肢无力、大小便功能障碍等[10]。电子计算机断层扫描(computed tomography,CT)和核磁共振(magnetic resonance imaging,MRI)平扫表现为间盘膨出、间盘突出、间盘脱出[11]。结合病史和本次检查的症状体征和影像学检查(CT/MRI)可对LDH做出诊断。
(1)吸烟:调查前30 d内吸过烟。(2)饮酒:过去1年内,不论酒类型,只要平均每月饮酒≥1次。(3)糖尿病:糖尿病史;空腹血糖≥7.0 mmol/L或餐后2小时血糖≥11.1 mmol/L[12]。(4)肥胖:体质指数(body mass index,BMI)>28 kg/m2为肥胖[13]。BMI计算公式为体重(kg)÷身高的二次方(m2)。(5)体力活动与体育锻炼强度:根据世界卫生组织(World Health Organization,WHO)标准分为高、中、低三个强度[14]
应用SPSS 26.0对数据进行分析,定性资料用[n(%)]进行描述,组间比较用χ2检验;不同民族的LDH影响因素的分析采用非条件二元logistic回归。检验水准α=0.05。
本次调查共纳入4 545人,其中男性2 026人,女性2 519人。LDH总患病率为22.8%,男性患病率为20.0%,低于女性的25.0%。各民族中藏族患病率最高,为30.6%(519/2 598),汉族患病率为19.9%(430/1 404),回族患病率为16.7%(70/419)。研究对象LDH患病率在有无配偶、文化程度、居住海拔、运动强度、身体活动、饮酒、职业类别、居住时间长短中存在差异,且均有统计学意义。见表1
汉族居民在不同年龄、有无配偶、居住海拔、饮酒、职业和居住时间的LDH患病率差异有统计学意义(P均<0.05)。藏族居民在不同性别、年龄、有无配偶、运动强度、身体活动、居住时间、和文化程度的LDH患病率差异有统计学意义(P均<0.05)。回族居民在不同年龄、居住海拔、年龄、身体活动和居住时间上的LDH患病情况有差异且均具有统计学意义(P均<0.05)。见表2
以是否患LDH作为因变量(0=否,1=是),以不同民族作为自变量进行哑变量设置,使用二元logistic回归模型。结果显示,民族为汉族和藏族的居民会增加患LDH的风险(P均<0.05,OR均>1),民族为回族的居民会减低患LDH的风险(P<0.05,OR<1)。见表3
以是否患LDH作为因变量(0=否,1=是),根据不同民族进行分层,将单因素分析有统计学意义的变量纳入二元logistic回归模型。结果显示,居住时间小于36月、不饮酒或居住海拔在1 500~3 500 m与汉族居民LDH的患病风险增加有关(P均<0.05,OR均>1);年龄<30岁、职业为干部或无业和居住海拔<1 500 m与汉族居民LDH患病风险降低有关(P均<0.05,OR均<1)。低运动强度、文化程度为小学及以下、居住时间≥24月且<36月与藏族居民LDH的患病风险增加有关(P均<0.05,OR均>1);性别为男、高强度以下活动、无配偶者与藏族居民LDH患病风险降低有关(P均<0.05,OR均<1)。居住时间≥6月且<24月或年龄30~59岁与回族成年居民LDH患病风险增加有关(P均<0.05,OR均>1)。见表4
Kim等[15]调查了韩国居民的LDH患病率为18%,一项关于人群LDH患病率的meta分析显示,我国人群LDH患病率大约为6%[16],均低于本次调查的甘肃省成年居民的患病率(22.8%),本次调查亦发现甘肃省藏族居民患病率最高,为30.6%。甘肃省藏族大多生活在甘南藏族自治州,最高海拔4 920 m,最低海拔1 172 m,相差3 748 m,部分区域海拔在3 500 m以上,以牧区为主,冬季较长,夏季较短,气候寒冷,有研究显示,寒冷更容易导致腰椎间盘突出[17],加之摄入深色蔬菜、奶类、肉类等高钙食物摄入很少,血中钙含量低,影响了骨骼发育[18]。Logistic回归显示居民民族为汉族和藏族患LDH的风险更高,居民民族为回族患LDH的风险更低。
进一步对不同民族LDH患病率的影响因素分析,汉族、回族居民患病风险随着居住时间延长而逐渐减低。这可能是本地居住环境寒冷,居住时间短者还未适应环境变化,腰府失护,邪毒乘虚而入,使机体免疫功能降低,易引起腰椎间盘突出[19],随着居住时间延长,机体逐渐适应环境。汉族人群中不饮酒的汉族居民LDH的发病风险更高,这与适量饮酒可以增加骨密度,而过量饮酒对骨重塑、成骨细胞增殖和活性的机制有负面影响有关[20]。汉族居民中干部患LDH风险更低,这与其劳动性质有关,这类人员以脑力劳动为主,有研究表明脑力劳动会降低LDH的患病风险[21]。居住海拔在1 500 m以下的汉族人群患LDH的风险更高,这可能是由于高原气候寒冷,腰部易受到寒冷刺激,加之移居者为习服高原环境常常引发机体一系列的代谢紊乱导致腰椎功能减退提前,而低海拔不会[22]。藏族人群中小学及以下的藏族居民患LDH的风险更高,这类人往往因文化水平的关系而从事重体力劳动的工作,结合身体活动、运动强度对藏族居民LDH患病率的影响可以看出体力劳动负荷对LDH的影响,过低或过高的体力劳动负荷都会导致藏族居民LDH患病率的升高[23];藏族女性患病可能性高于男性,这与此前的Geng等[24]的研究结果有所不同,这可能与藏族女性承担更多的家务劳动有关。回族人群中:年龄30~59岁居民LDH患病风险更高,这可能与此年龄范围从事较多体力劳动,容易积累伤病加速腰椎间盘退化而更易导致LDH的发生有关。
有研究发现吸烟与LDH存在独立的相关关系且吸烟是危险因素[25],肥胖和糖尿病与LDH患病相关联[919],但本研究未发现这些因素对LDH的患病率有影响。亦有研究显示高原环境中低氧、强紫外线以及经常骑马等因素对LDH患病率产生影响[26-27];但本文未对这些因素展开调查,未来可值得进一步研究,以更全面的分析不同民族LDH的影响因素。本文另一不足在于,因调查受限,未按照民族进行分层抽样调查。
综上所述,甘肃省成年居民不同民族LDH患病率存在差异且藏族和汉族LDH的患病风险更高。居住时间、饮酒、居住海拔、职业和年龄等因素会影响汉族居民的LDH患病风险;运动强度、文化程度、居住时间、性别、身体活动、有无配偶等因素会影响藏族居民的LDH患病风险;而回族居民的LDH患病风险则只与居住时间和年龄有关。
