Objective

To analyze the status of antiretroviral therapy (ART), survival outcomes, and influencing factors among HIV-infected children in Guangxi.

Methods

A retrospective cohort study was conducted, incorporating data from HIV-infected children aged ≤14 years in Guangxi, including treatment status, survival time, and influencing factors. Logistic regression and Cox proportional hazards regression models were employed for statistical analysis.

Results

A total of 472 HIV-infected children were included, with 27 deaths reported. All children received ART. Factors such as age and clinical stage at diagnosis influenced the risk of delayed initiation of treatment. Survival analysis revealed that baseline CD4+ T-cell levels, clinical stage, and treatment regimen were key determinants of survival time. Children with baseline CD4+ T-cell counts >350 cells/μL had longer survival time compared to those with counts <200 cells/μL (aHR=0.31, 95%CI: 0.13-0.74). Children in WHO clinical stage IV had shorter survival times than those in stage I (aHR=3.22, 95%CI: 1.24-12.2). Additionally, children treated with the 3TC+ABC+LPV/r regimen had shorter survival time than those treated with the 3TC+AZT+EFV regimen (aHR=4.26, 95%CI: 1.16-15.61).

Conclusion

The coverage of ART among HIV-infected children in Guangxi is high, with relatively favorable survival rates. However, efforts should be intensified to educate caregivers and initiate treatment early, optimize treatment regimens, and improve quality of life.

目的

分析广西地区HIV感染儿童的抗逆转录病毒治疗情况、生存状况以及影响因素。

方法

本研究采用回顾性队列研究方法,纳入广西地区≤14岁HIV感染儿童的相关数据,包括治疗情况、生存时间和影响因素。采用logistic回归方法和Cox比例风险回归模型统计学分析方法对数据进行处理。

结果

研究共纳入472例HIV感染儿童,27例死亡,100%接受了抗逆转录病毒治疗。确诊后,年龄、临床分期等因素影响了确诊到开始治疗时间延长的风险。生存分析显示,治疗前基线CD4+T细胞水平、临床分期和治疗方案是影响儿童生存时间的关键因素。治疗前基线CD4+T细胞>350个/μl的儿童生存时间高于治疗前基线CD4+T细胞<200个/μl(aHR=0.31,95%CI:0.13~0.74);WHO临床分期为Ⅳ期的儿童生存时间低于Ⅰ期(aHR=3.22,95%CI:1.24~12.2);使用3TC+ABC+克力芝治疗方案的儿童生存时间低于使用3TC+AZT+EFV治疗方案(aHR=4.26,95%CI:1.16~15.61)。

结论

广西地区HIV感染儿童的抗逆转录病毒治疗覆盖率较高,生存率相对较好。然而,需要加强对患儿及其家长的宣传教育,尽早启动治疗,优化治疗方案,提高生存质量。

目的

分析广西地区HIV感染儿童的抗逆转录病毒治疗情况、生存状况以及影响因素。

方法

本研究采用回顾性队列研究方法,纳入广西地区≤14岁HIV感染儿童的相关数据,包括治疗情况、生存时间和影响因素。采用logistic回归方法和Cox比例风险回归模型统计学分析方法对数据进行处理。

结果

研究共纳入472例HIV感染儿童,27例死亡,100%接受了抗逆转录病毒治疗。确诊后,年龄、临床分期等因素影响了确诊到开始治疗时间延长的风险。生存分析显示,治疗前基线CD4+T细胞水平、临床分期和治疗方案是影响儿童生存时间的关键因素。治疗前基线CD4+T细胞>350个/μl的儿童生存时间高于治疗前基线CD4+T细胞<200个/μl(aHR=0.31,95%CI:0.13~0.74);WHO临床分期为Ⅳ期的儿童生存时间低于Ⅰ期(aHR=3.22,95%CI:1.24~12.2);使用3TC+ABC+克力芝治疗方案的儿童生存时间低于使用3TC+AZT+EFV治疗方案(aHR=4.26,95%CI:1.16~15.61)。

结论

广西地区HIV感染儿童的抗逆转录病毒治疗覆盖率较高,生存率相对较好。然而,需要加强对患儿及其家长的宣传教育,尽早启动治疗,优化治疗方案,提高生存质量。

Objective

To analyze the status of antiretroviral therapy (ART), survival outcomes, and influencing factors among HIV-infected children in Guangxi.

Methods

A retrospective cohort study was conducted, incorporating data from HIV-infected children aged ≤14 years in Guangxi, including treatment status, survival time, and influencing factors. Logistic regression and Cox proportional hazards regression models were employed for statistical analysis.

Results

A total of 472 HIV-infected children were included, with 27 deaths reported. All children received ART. Factors such as age and clinical stage at diagnosis influenced the risk of delayed initiation of treatment. Survival analysis revealed that baseline CD4+ T-cell levels, clinical stage, and treatment regimen were key determinants of survival time. Children with baseline CD4+ T-cell counts >350 cells/μL had longer survival time compared to those with counts <200 cells/μL (aHR=0.31, 95%CI: 0.13-0.74). Children in WHO clinical stage IV had shorter survival times than those in stage I (aHR=3.22, 95%CI: 1.24-12.2). Additionally, children treated with the 3TC+ABC+LPV/r regimen had shorter survival time than those treated with the 3TC+AZT+EFV regimen (aHR=4.26, 95%CI: 1.16-15.61).

Conclusion

The coverage of ART among HIV-infected children in Guangxi is high, with relatively favorable survival rates. However, efforts should be intensified to educate caregivers and initiate treatment early, optimize treatment regimens, and improve quality of life.

2015年,世卫组织推荐所有HIV感染的儿童一旦确诊,无论其临床分期和CD4细胞计数如何,即开始抗逆转录病毒治疗[1]。尽管全球在推动早期抗逆转录病毒治疗方面做出了巨大努力,但HIV感染儿童的抗逆转录病毒治疗覆盖率仍然较低。据2021年估计,在全球范围内有约170万15岁及以下的HIV感染儿童,但只有52.0%接受了抗逆转录病毒治疗[2]。在中国,由于近年来HIV治疗的有效扩大,HIV感染儿童的抗逆转录病毒治疗覆盖率相对较高,例如,截至2020年底,有95.2%的15岁以下HIV感染儿童接受了抗逆转录病毒治疗[3]
抗逆转录病毒治疗的覆盖面不断扩大,这显著降低了儿童艾滋病发病率和死亡率,使得大多数儿童有了生存和成长的机会[4]。然而,在许多资源有限的环境中,长期维持病毒学上的抑制仍然是一项挑战。HIV感染儿童因为年龄小、生理功能未发育完全且自我管理能力较低,疾病进展较快,因此需要及时治疗。据报道,抗逆转录病毒治疗开始后,病毒抑制率从第一年的64%、第二年的62%显著下降到第三年的59%[5]。无法维持病毒学上的抑制通常意味着治疗失败。依从性是与病毒抑制相关的一个主要因素[6]。此外,与成人和幼儿相比,11~19岁的青少年失去随访的风险更高[7]。HIV感染儿童面临着一系列社会挑战,包括贫困、HIV污名、与HIV披露相关的关系问题以及学校系统无法满足其需求[8]。此外,已有研究指出,父母的参与程度、对药物治疗的感知和态度与儿童的依从性密切相关[9]
目前国内对HIV感染儿童全阶段生存情况及影响因素的研究相对较少,因此本研究旨在通过回顾性研究探讨广西地区15岁及以下HIV感染儿童的治疗情况、生存状况及相关影响因素。
本研究数据来源于中国艾滋病综合防治信息系统病例报告和抗病毒治疗数据库,截至2022年12月31日。纳入标准:(1)确诊为HIV感染;(2)现居住地为广西;(3)诊断时年龄≤15岁。剔除基线信息不完整、出生日期缺失以及基线CD4检测结果缺失的病例。
采用回顾性队列研究方法,观察HIV感染儿童的治疗和生存情况,以HIV感染确诊日期为观察起点,2022年12月31日为观察终点。选取中国艾滋病综合防治信息系统中儿童的原始卡片信息作为基线数据,收集儿童的性别、确诊阳性年龄、治疗方案、基线CD4、感染途径、临床症状以及临床随访状态等信息。生存时间定义为:从HIV诊断日期至死亡日期或最后一次随访日期之间的时间跨度,以月为单位。
采用SPSS 26.0软件进行统计分析,对研究对象的人口学信息采用频数和百分比进行描述。连续变量使用中位数和四分位数间距表示。采用单因素和多因素逻辑回归模型分析起始治疗时间的影响因素;采用单因素和多因素Cox比例风险回归模型分析影响生存时间的因素。采用双侧检验,检验水准α=0.05。
本研究共纳入了472例艾滋病病毒感染的儿童。其中,2008—2012年诊断的有152例,2014—2017年诊断的有196例,2018—2022年诊断的有124例,所有患者均接受了抗病毒治疗。儿童患者的平均年龄为(3.08±2.90)岁,男童251例(53.18%),女童221例(46.82%)。在感染途径方面,90.47%(427例)为母婴传播。治疗前基线CD4+T细胞以大于350个/μl为主,占44.92%,WHO临床分期以Ⅰ期为主,占65.25%。治疗方案主要包括:拉米夫定(3TC)齐多夫定(AZT)+依非韦仑(EFV),占总数的22.28%;3TC+AZT+奈韦拉平(NVP),占20.97%;3TC+阿巴卡韦(ABC)+克力芝,占17.16%;3TC+AZT+克力芝,占16.31%;3TC+ABC+EFV占10.81%。29.45%的儿童使用了复方新诺明。见表1
所有研究对象均接受了抗病毒治疗,其中接受治疗时间小于6个月的占总数的74.15%(350/472),大于6个月的占总数的25.85%(122/472)。单因素逻辑二元logistic回归模型的分析结果显示,年龄和是否出现临床症状是影响确诊到开始治疗时间延长的风险因素。将单因素分析中具有统计学意义的因素引入多因素二元logistic回归模型,变量的引入标准设定为0.05,剔除标准为0.20。结果显示,年龄和临床分期是影响确诊到开始治疗时间延长的风险因素。见表2
在观察终点时,共有472例儿童被纳入研究,其中27例儿童死亡,中位生存时间为91.5个月。观察对象在HIV感染诊断后1、2、5和10年的累积生存概率分别为97.46%、97.03%、95.34%、94.49%。单因素Cox比例风险回归分析结果显示,治疗前基线CD4+T细胞、临床分期、是否出现临床症状、起始治疗方案可能影响HIV感染儿童的生存时间。经过多因素Cox比例风险回归分析后,发现治疗前基线CD4+T细胞、临床分期、起始治疗方案可能是影响HIV感染儿童生存时间的因素。比较HIV感染儿童的生存时间,治疗前基线CD4+T细胞>350个/μl的儿童生存时间高于治疗前基线CD4+T细胞<200个/μl(aHR=0.31,95%CI:0.13~0.74);WHO临床分期为Ⅳ期的儿童生存时间低于Ⅰ期(aHR=3.22,95%CI:1.24~12.2)。儿童使用3TC+ABC+克力芝治疗方案的儿童生存时间低于使用3TC+AZT+EFV治疗方案(aHR=4.26,95%CI:1.16-15.61)。见表3
广西位于西南部,与越南接壤,是我国艾滋病疫情高发区之一,面临着感染途径复杂、贫困地区、少数民族等多重复杂因素的挑战。自2001年起,我国开展了预防艾滋病母婴传播的工作,母婴传播率从34.8%下降到2021年的3.3%,并计划到2025年将其降至2.0%以实现消除母婴传播的目标。然而,广西地区母婴传播率为2.2%[10],距离目标还有一定差距。
先前的研究表明[11],对母婴传播的HIV感染儿童尽早启动HIV抗病毒治疗有助于改善免疫状况,促进其免疫状态的恢复。本研究发现,HIV感染儿童平均在诊断后1个月内开始治疗,但仍有25.85%的患儿在诊断后6个月以上才开始治疗。这说明我们仍需加强对患儿父母的宣传教育,鼓励他们尽早启动HIV抗病毒治疗。另外,我们也发现,临床分期进展或出现其他临床症状更容易延迟治疗。因此,在处理其他临床症状时,不能忽视HIV抗病毒治疗的重要性。本研究还发现,广西地区HIV感染儿童的1、2和5年生存率均高于全国其他省份的儿童[12-13]。这表明广西地区目前的儿童艾滋病抗病毒治疗效果较好。生存分析结果显示,治疗前基线CD4+T细胞水平、临床分期和起始治疗方案是影响HIV感染儿童生存时间的重要因素。治疗前CD4+T细胞计数越低,死亡风险越高,这也印证了诊断时疾病状态和治疗前CD4+T细胞计数与治疗效果及预后密切相关的观点[14]。因此,越早启动抗病毒治疗,治疗效果越好,对延缓疾病进展和减少相关死亡有重要意义。有研究表明,CD4+T淋巴细胞恢复正常化与初始的ART方案相关[15-16]。因此,对所有的HIV感染儿童进行风险评估,制定个性化的治疗方案也是十分必要的。
然而,本研究仍存在一些不足之处,数据来源于中国艾滋病综合防治信息系统的回顾性队列,无法全面了解HIV感染儿童监护人员的详细情况、儿童抗病毒治疗依从性以及其他可能影响儿童生存的因素。因此,后续研究需要进一步关注这些影响因素。
综上所述,广西地区HIV感染儿童的抗病毒治疗效果和生存率较高,但仍需要尽早启动治疗、优化治疗方案、关注行为和情绪障碍,以提高其生存质量。
