Objective

To analyze and assess the characteristics of the infected population and the disease burden of clonorchiosis in a city of the Pearl River Delta, providing a scientific basis for future prevention and control strategies.

Methods

Based on population monitoring and medical institution data from 2019 to 2023 regarding clonorchiosis in the city, infection rates and prevalence proportions were calculated for different genders and age groups. The characteristics of patients seeking medical attention were described, and the disease burden was evaluated using Disability-adjusted Life Years (DALYs).

Results

The overall infection rate for clonorchiosis in the surveyed population from 2019 to 2023 was 8.32%, with a male infection rate of 12.50% and a female infection rate of 4.84%, showing a statistically significant difference (χ2=98.54, P<0.001). The weighted infection rate was 7.87% [95% Confidence Interval (CI): 5.67%-10.06%]. The weighted infection rates across various age groups ranged from 0.09% to 16.18%, with higher rates observed in individuals over 40 years of age. The majority of infections were mild (97.00%), while moderate and severe infections were less common. The annual DALYs lost due to clonorchiosis were 14 399.10 for males and 4 606.71 for females, totaling 19 005.81, equating to a loss of 4.76 DALYs per 1 000 people per year.From 2019 to 2023, a total of 3 193 cases of clonorchiosis sought medical attention, primarily through outpatient services (95.49%). The proportions of complications such as cholelithiasis, cholecystitis, cholangitis, liver cysts, gallbladder polyps, cholecystectomy, gallbladder hypertrophy, and cholangiocarcinoma were 1.16%, 0.60%, 0.53%, 0.22%, 0.06%, 0.06%, 0.00%,and 0.00%, respectively. The average outpatient medical cost was 155.62 yuan, while the average inpatient medical cost was 6 545.97 yuan.

Conclusion

The burden of clonorchiosis in the city of the Pearl River Delta is significant, characterized by a high population infection rate, primarily among males, middle-aged and elderly individuals, and mild infections. Future efforts should focus on public education, screening, and treatment for at-risk populations.

目的

分析和评估珠三角某市华支睾吸虫病感染人群特征和疾病负担,为下一步防治策略提供科学依据。

方法

依据2019—2023年珠三角某市华支睾吸虫病的人群监测、医疗机构数据,计算不同性别、年龄组的感染率和感染度占比,描述就诊病例特征和使用伤残调整寿命年(disability-adjusted life years,DALYs)评价疾病负担。

结果

2019—2023年珠三角某市华支睾吸虫病人群调查感染率为8.32%,男性感染率为12.50%,女性感染率为4.84%,男女差异具有统计学意义(χ2=98.54,P<0.001)。经加权感染率为7.87%[95%置信区间(confidence interval, CI):5.67%~10.06%] 。各年龄组加权感染率介于0.09%~16.18%,40岁以上年龄组感染率高。感染者感染度以轻度感染为主,占97.00%,中、重度感染占比低。每年华支睾吸虫病男性损失的DALYs为14 399.10人年,女性为4 606.71人年,合计19 005.81人年,每1 000人口损失DALYs为4.76人年。2019—2023年累积有3 193名华支睾吸虫病病例前往医疗机构就诊,以门诊为主,占95.49%;出现胆石症、胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术、胆囊肥大、肝胆管癌比例分别为1.16%、0.60%、0.53%、0.22%、0.06%、0.06%、0、0;门诊平均医疗费用为155.62元,住院平均医疗费用为6 545.97元。

结论

珠三角某市华支睾吸虫病疾病负担较重,主要表现为人群感染率高,感染人群以男性、中老年人、轻度感染为主,下一步应针对重点人群开展宣传教育和筛查治疗。

目的

分析和评估珠三角某市华支睾吸虫病感染人群特征和疾病负担,为下一步防治策略提供科学依据。

方法

依据2019—2023年珠三角某市华支睾吸虫病的人群监测、医疗机构数据,计算不同性别、年龄组的感染率和感染度占比,描述就诊病例特征和使用伤残调整寿命年(disability-adjusted life years,DALYs)评价疾病负担。

结果

2019—2023年珠三角某市华支睾吸虫病人群调查感染率为8.32%,男性感染率为12.50%,女性感染率为4.84%,男女差异具有统计学意义(χ2=98.54,P<0.001)。经加权感染率为7.87%[95%置信区间(confidence interval, CI):5.67%~10.06%] 。各年龄组加权感染率介于0.09%~16.18%,40岁以上年龄组感染率高。感染者感染度以轻度感染为主,占97.00%,中、重度感染占比低。每年华支睾吸虫病男性损失的DALYs为14 399.10人年,女性为4 606.71人年,合计19 005.81人年,每1 000人口损失DALYs为4.76人年。2019—2023年累积有3 193名华支睾吸虫病病例前往医疗机构就诊,以门诊为主,占95.49%;出现胆石症、胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术、胆囊肥大、肝胆管癌比例分别为1.16%、0.60%、0.53%、0.22%、0.06%、0.06%、0、0;门诊平均医疗费用为155.62元,住院平均医疗费用为6 545.97元。

结论

珠三角某市华支睾吸虫病疾病负担较重,主要表现为人群感染率高,感染人群以男性、中老年人、轻度感染为主,下一步应针对重点人群开展宣传教育和筛查治疗。

Objective

To analyze and assess the characteristics of the infected population and the disease burden of clonorchiosis in a city of the Pearl River Delta, providing a scientific basis for future prevention and control strategies.

Methods

Based on population monitoring and medical institution data from 2019 to 2023 regarding clonorchiosis in the city, infection rates and prevalence proportions were calculated for different genders and age groups. The characteristics of patients seeking medical attention were described, and the disease burden was evaluated using Disability-adjusted Life Years (DALYs).

Results

The overall infection rate for clonorchiosis in the surveyed population from 2019 to 2023 was 8.32%, with a male infection rate of 12.50% and a female infection rate of 4.84%, showing a statistically significant difference (χ2=98.54, P<0.001). The weighted infection rate was 7.87% [95% Confidence Interval (CI): 5.67%-10.06%]. The weighted infection rates across various age groups ranged from 0.09% to 16.18%, with higher rates observed in individuals over 40 years of age. The majority of infections were mild (97.00%), while moderate and severe infections were less common. The annual DALYs lost due to clonorchiosis were 14 399.10 for males and 4 606.71 for females, totaling 19 005.81, equating to a loss of 4.76 DALYs per 1 000 people per year.From 2019 to 2023, a total of 3 193 cases of clonorchiosis sought medical attention, primarily through outpatient services (95.49%). The proportions of complications such as cholelithiasis, cholecystitis, cholangitis, liver cysts, gallbladder polyps, cholecystectomy, gallbladder hypertrophy, and cholangiocarcinoma were 1.16%, 0.60%, 0.53%, 0.22%, 0.06%, 0.06%, 0.00%,and 0.00%, respectively. The average outpatient medical cost was 155.62 yuan, while the average inpatient medical cost was 6 545.97 yuan.

Conclusion

The burden of clonorchiosis in the city of the Pearl River Delta is significant, characterized by a high population infection rate, primarily among males, middle-aged and elderly individuals, and mild infections. Future efforts should focus on public education, screening, and treatment for at-risk populations.

华支睾吸虫病是一种经口摄入华支睾吸虫活囊蚴的淡水鱼虾后,囊蚴在人体发育为成虫并寄生在肝胆管内,引起机体肝胆系统病理变化的食源性寄生虫病。华支睾吸虫病对人体危害严重,可引起胆囊炎、胆石症、肝囊肿等并发症。感染华支睾吸虫的人发生胆管癌的风险是未感染者的7.25倍[1](95%CI:3.97~13.26),2009年国际癌症研究机构将华支睾吸虫列为Ⅰ类致癌物[2]。广东省曾有81个县(市、区)流行华支睾吸虫病,流行区主要分布在珠江和韩江两大水系及其毗邻地区,1988—2015年调查结果显示珠三角地区是广东省华支睾吸虫病重点流行区,其感染率居高不下[3]
本研究调查所属地市,既往人群感染率高,属于珠三角地区华支睾吸虫病流行严重的地区之一。经过多年综合防治,该市建立起政府主导、部门合作、群众参与的防治模式。为了评估当前该市华支睾吸虫病流行特征及其疾病负担,本研究采用2019—2023年该市华支睾吸虫病的人群监测和医疗机构数据进行分析,为下一步优化资源配置、开展综合防治提供科学依据。
按地理方位将该市划分为东、西、南、北、中5个片区,每个片区随机抽取1镇街的1个社区(行政村)开展监测,每个社区(行政村)按整群抽样法抽取不少200人的居民。调查人群要求在本地常住至少1年,年龄不小于3岁,覆盖儿童、青年、中年和老年各个年龄段。
采集调查对象粪便样本(>30 g),按照《肠道蠕虫检测改良加藤厚涂片法》(WS/T 570-2017)制作两份改良加藤厚片,在生物学显微镜下计数华支睾吸虫虫卵数。每克粪便虫卵数(Eggs per gram, EPG)=平均每张改良加藤厚片虫卵数×24。根据EPG判断华支睾吸虫感染度分级,其中<1 000为轻度感染,1 000~9 999为中度感染,≥10 000为重度感染。
加权感染率的计算公式[4]如下:
P′为加权感染率的点估计值,wi为调查个体i的权重,yi为调查个体i的改良加藤厚涂片法检测结果(0为阴性,1为阳性)。
wiwbasewadj的乘积,其中wbase为基础抽样权重,包含调查设计信息;wadj为按人口学特征计算的调整权重,计算公式如下:
Nrc为总体人群在某性别、某年龄段的实际人数,为抽样样本在该性别、该年龄段个体的基础权重之和。
传染病流行病学负担评价方法主要包括流行病学指标、潜在寿命损失年、健康期望寿命、DALYs等。为了充分考虑食源性传染病感染后的症状对人群健康的影响,WHO建议应用DALYs作为衡量疾病负担的单位。本研究使用WHO基于患病率估算全球疾病负担的方法计算DALYs[5],计算公式为:
YLL为因早死所致的寿命损失年(years of life lost, YLL),YLD为疾病所致伤残引起的健康寿命损失年(years lived with disability, YLD);nd为死亡人数,数据来源该市在中国疾病预防控制信息系统报告的华支睾吸虫病的死亡病例数,L为病例死亡时期望寿命的损失值;np为感染人数,使用加权感染率和人口数进行估算;DW为伤残权重(disability weight, DW),是对各种健康状态所致健康损失的量化,反映的是失能程度,取值范围为0(完全健康)~1(死亡)[6]。有研究[7]显示DW与EPG的几何平均值(geometric mean of eggs per gram, GMEPG)成函数关系,本研究采用该研究的函数公式对不同年龄段感染人员的DW进行估算:
考虑华支睾吸虫病感染率在不同性别和年龄组之间存在差异,本研究按照性别、年龄分组计算DALYs后进行合计。每1 000人口损失DALYs=各年龄段、性别分组的DALYs之和×1 000/总人口数(人年/1 000人)。
在该市全市医疗机构门诊和住院记录中抓取2019—2023年主诊断包含“华支睾吸虫”或“肝吸虫”的病例,排除6个月内重复就诊者,收集其年龄、性别、其他诊断、医疗费用信息。筛选主诊断和其他诊断仅包括华支睾吸虫病感染导致症状、体征或诊断(包括胆石症、胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术、胆囊肥大、肝胆管癌),计算其平均医疗费用。
采用Excel 2016进行数据整理和计算,率的比较采用χ2检验,加权感染率的点估计值和标准误使用R语言(4.2.2)survey包计算。检验水准α=0.05。
本研究所有被调查者(或其家属)均知晓其粪便标本仅用于寄生虫虫卵检测,检测结果由辖区疾控部门工作人员反馈至本人(或其家属),阳性感染者在当地社区卫生服务中心指导下用药驱虫治疗。本研究已通过中山市疾病预防控制中心伦理委员会审查审批,审批号为2024012。
2019—2023年累积调查25个社区共5 219人,华支睾吸虫病调查感染率为8.32%(434/5 219),经加权感染率为7.87%(95%CI:5.67%~10.06%)。
男性感染率为12.50%(296/2 368),女性感染率为4.84%(138/2 851),男女感染率差异具有统计学意义(χ2=98.54,P<0.001)。
各年龄组加权感染率介于0.09%~16.18%。10岁以下年龄组感染率低于1.00%,10~39岁年龄组、60岁以上年龄组感染率介于1.00%~10.00%,40~59岁年龄组感染率大于10.00%。见表1
调查对象中阳性感染者的EPG最小值为24,最大值为13 440,中位数为144。感染度以轻度感染为主,占总感染数的97.00%(421/434),中度感染占2.53%(11/434),重度感染罕见,占0.46%(2/434)。
各社区调查感染率介于0~24.64%。感染率大于10.00%的社区有9个,感染率介于1.01%~10.00%有10个,感染率介于0.01%~1.00%有3个,感染率为0有3个。
中国疾病预防控制信息系统显示,2019—2023年该市未报告华支睾吸虫病死亡病例,YLL计算为0。根据2019—2023年人群调查数据计算得出该市华支睾吸虫病男性一年损失的DALYs为14 399.10人年,女性为4 606.71人年,合计19 005.81人年,见表2;男性每1 000人口损失DALYs为6.75人年,女性为2.48人年,总人群为4.76人年。
2019—2023年累积有3 193名华支睾吸虫病病例前往该市医疗机构就诊,年均就诊人数为639人;3 193名就诊病例中,男性有2 023人,女性有928人,未采集到相关信息的有242人,男女比例为2.18:1;年龄以中老年人群为主,40岁以上病例占68.15%(2176/3193);就诊类型以门诊为主,占95.49%(3 049/3 193);病例出现胆石症、胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术、胆囊肥大、肝胆管癌比例分别为1.16%、0.60%、0.53%、0.22%、0.06%、0.06%、0、0。见表3
筛选主诊断和其他诊断仅包括华支睾吸虫病感染导致症状、体征或诊断(胆石症、胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术、胆囊肥大、肝胆管癌)有1 700例。其中门诊有1 622例,平均医疗费用为155.62元;住院有78例,平均医疗费用为6 545.97元。
2016年起该市实施三年华支睾吸虫病防治规划,在全市镇街医院建立华支睾吸虫病治疗门诊,以开展华支睾吸虫病诊断、治疗和健康咨询。全市社区卫生服务中心在常规医疗门诊服务基础上,开展卫生宣传、健康教育、疾病诊疗服务,提高华支睾吸虫病感染者就诊率和治疗率。市内两个镇街建立华支睾吸虫病综合防治示范区,定期组织各类覆盖社区、学校、餐饮机构的宣传教育活动;对检测发现的阳性病例造册登记,追踪治疗效果。2019—2023年,该市沿用2016—2018年建立的模式防治华支睾吸虫病,并且每年抽取5个镇街社区开展人群感染率监测,5年累积调查25个社区共5 219人,华支睾吸虫感染率为8.32%,较2016年在全市范围内采取分层整群抽样调查的24个社区的感染率19.45%(941/4 837)下降11.13%。尽管有较大幅度的下降,该市感染率在广东省内仍处于较高水平,本次监测结果高于2010—2020年广州市监测结果(3.13%)[8],以及2017—2022年河源市监测结果(5.13%)[9],低于2016—2020年江门市新会区监测结果(14.64%)[10]
与其他研究结果一致[8-10],该市华支睾吸虫病人群监测男性感染率高于女性,中老年人群感染率更高。本研究对受检者的粪便标本制成的改良加藤厚片在显微镜下计数虫卵数,发现阳性感染者中绝大多数为轻度感染,占97.00%。与之相对应的是,2019—2023年该市95.49%的华支睾吸虫病就诊病例以门诊为主,就诊病例出现肝胆系统相关疾病的比例较低,最高为胆石症,出现比例为1.16%,胆囊炎、胆管炎、肝囊肿、胆囊息肉、胆囊切除术均小于1.00%,未发现胆囊肥大或肝胆管癌。感染人群普遍感染度低,表现或不表现症状,前往医疗机构寻求治疗需求少。同时,低感染度人群在粪便检测中容易出现假阴性,对于华支睾吸虫病人群感染率进一步降低提出了挑战[11]
华支睾吸虫病主要在亚洲国家和地区流行,包括中国、越南、韩国和俄罗斯,其中约85%发生在中国[12]。Fürst[12]等通过收集1980—2008年关于人类食源性吸虫病的报告,在仅考虑重度感染造成的YLD下,评估中国、越南、韩国和俄罗斯华支睾吸虫病的DALYs分别为231 547、26 366、17 403和54人年。赵婷婷[13]等根据2014—2016年全国人体重要寄生虫病现状调查数据,DW采用基于广东省顺德区某华支睾吸虫病高流行村社区调查估计的不同症状加权值,计算得出全国华支睾吸虫病的DALYs为489 174.04人年,每1 000人口损失DALY为0.36人年,其中广东省华支睾吸虫病的DALYs为157 245.48人年,每1 000人口损失DALYs为1.45人年。本研究采用2019—2023年珠三角某市华支睾吸虫病人群监测数据,DW通过感染者的实际感染度进行估算,计算DALYs同时考虑感染率在不同性别和年龄组之间的差异,研究结果能够较真实反映华支睾吸虫病在低感染度人群中的疾病负担水平。
本研究存在一定的不足,一是感染率调查采取分层整群抽样方法,在社区中发动群众以家庭为单位自愿送检粪便,存在选择偏倚。比如在高中或大学住宿的学生参与比例低,15~24岁年龄组人数仅占总调查人员数的4.16%(217/5 219),以及在普及宣传教育下,有感染华支睾吸虫病危险行为的群众更有可能提供粪便检测。二是由于未能获得户籍人口方面的详细数据,本研究在计算DALYs时未对户籍与非户籍感染人群进行分层,未能考虑该因素的影响。三是该市医疗机构监测结果只能在一定程度上反映华支睾吸虫病感染人群出现异常体征或诊断的比例,原因是感染者可能在就诊时无开展华支睾吸虫病相关检测,或者前往市外的其他医疗机构寻找治疗,数据未能纳入统计。
针对存在问题,首先,要提高重点人群的筛查面,以健康咨询、体检为切入点,增大40岁以上男性人群筛查总量,同时还需要进一步提高医疗机构的检测能力。目前该市大部分医疗机构使用大便直接涂片镜检或华支睾吸虫血清抗体检测方法。大便直接涂片镜检检出率低,有研究[14]显示其灵敏度为70.6%,特异度只有66.7%。华支睾吸虫血清抗体检测准确率不高,不能作为肝吸虫诊断的主要方法[15]。近年来,随着技术不断进步,自动粪便分析仪与手工法结果一致性较高[16-17],并且具有检测效率高、生物安全性好,人为误差小的优点[18],可以在医疗机构中推广使用。另外在检测过程中发现的年轻感染者,若判断为新近感染的,应重点开展流行病学调查,以切断传播途径。最后要继续加强宣传教育,普及华支睾吸虫病危害和防治知识,改变饮食习惯,减少新发感染,以及避免对疾病重视不足导致重复感染。
