Objective

To clarify the current status of diabetes management and services in primary healthcare institutions, explore the factors influencing their diabetes service capacity, and provide theoretical references for enhancing this capacity.

Methods

In-depth interviews were conducted involving 28 participants, including administrators, medical staff, and diabetic patients from four primary healthcare institutions in a district of Chengdu. Grounded theory was employed to summarize and refine the factors affecting diabetes service capacity in these institutions.

Results

Through three-level coding, 24 initial categories, 7 main categories, and 3 core categories were identified, leading to the construction of a theoretical framework for the"Influencing Factors Model of Diabetes Service Capacity in Primary Healthcare Institutions." The current status of the sample institutions was analyzed across three dimensions: resource allocation, policy support, and daily operations.

Conclusion

The diabetes service capacity of primary healthcare institutions in the studied district of Chengdu is influenced by resource allocation, policy support, and daily operations. To enhance this capacity, measures such as strengthening daily operations, improving policy support mechanisms, and optimizing the allocation of human and material resources should be implemented.

目的

明确基层医疗卫生机构糖尿病管理与服务现状，探讨影响基层医疗卫生机构糖尿病服务能力的因素，为提升基层医疗卫生机构糖尿病服务能力提供理论参考。

方法

对成都市某区4家基层医疗卫生机构的管理人员、医务人员及糖尿病患者共计28名访谈对象进行深度访谈，运用扎根理论对影响基层医疗卫生机构糖尿病服务能力的因素进行归纳与提炼。

结果

通过三级编码，得到24个初始范畴，7个主范畴以及3个核心范畴，构建了“基层医疗卫生机构糖尿病服务能力影响因素模型”理论框架，得到样本地区基层医疗卫生机构在资源配置、政策保障和日常运营3个维度的现状。

结论

成都市某区基层医疗卫生机构糖尿病服务能力受资源配置、政策保障、日常运营共同影响，应通过加强基层医疗卫生机构日常运营、健全政策保障机制、优化人力物力资源配置等措施，提高基层医疗卫生机构糖尿病服务能力。

目的

明确基层医疗卫生机构糖尿病管理与服务现状，探讨影响基层医疗卫生机构糖尿病服务能力的因素，为提升基层医疗卫生机构糖尿病服务能力提供理论参考。

方法

对成都市某区4家基层医疗卫生机构的管理人员、医务人员及糖尿病患者共计28名访谈对象进行深度访谈，运用扎根理论对影响基层医疗卫生机构糖尿病服务能力的因素进行归纳与提炼。

结果

通过三级编码，得到24个初始范畴，7个主范畴以及3个核心范畴，构建了“基层医疗卫生机构糖尿病服务能力影响因素模型”理论框架，得到样本地区基层医疗卫生机构在资源配置、政策保障和日常运营3个维度的现状。

结论

成都市某区基层医疗卫生机构糖尿病服务能力受资源配置、政策保障、日常运营共同影响，应通过加强基层医疗卫生机构日常运营、健全政策保障机制、优化人力物力资源配置等措施，提高基层医疗卫生机构糖尿病服务能力。

Objective

To clarify the current status of diabetes management and services in primary healthcare institutions, explore the factors influencing their diabetes service capacity, and provide theoretical references for enhancing this capacity.

Methods

In-depth interviews were conducted involving 28 participants, including administrators, medical staff, and diabetic patients from four primary healthcare institutions in a district of Chengdu. Grounded theory was employed to summarize and refine the factors affecting diabetes service capacity in these institutions.

Results

Through three-level coding, 24 initial categories, 7 main categories, and 3 core categories were identified, leading to the construction of a theoretical framework for the"Influencing Factors Model of Diabetes Service Capacity in Primary Healthcare Institutions." The current status of the sample institutions was analyzed across three dimensions: resource allocation, policy support, and daily operations.

Conclusion

The diabetes service capacity of primary healthcare institutions in the studied district of Chengdu is influenced by resource allocation, policy support, and daily operations. To enhance this capacity, measures such as strengthening daily operations, improving policy support mechanisms, and optimizing the allocation of human and material resources should be implemented.

糖尿病是一种慢性非传染性疾病，其并发症累及眼、肾、心血管等多个器官，给患者本人、家庭和社会带来沉重的负担[1]。随着我国人口老龄化与生活方式的改变，糖尿病患病率逐年上升[2]，糖尿病防控已是我国重要的卫生问题[3]。基层医疗卫生机构是糖尿病防控的主要战场，不少地区已经采取一系列措施提升基层医疗卫生机构糖尿病防控能力，包括增加基层卫生人员数量、加强基层卫生人员业务培训力度、加大上级医院对口支援基层医疗卫生机构等，但是糖尿病患者管理不规范、血糖控制效果差等问题依然存在[4]。既往对基层医疗卫生机构糖尿病服务能力的研究多采用定量分析，鲜有质性研究。与定量研究相比，质性研究更有助于挖掘潜藏在基层医疗卫生机构内部，影响其糖尿病服务能力的核心因素。本研究选择成都市某区的基层医疗卫生机构为研究对象，采用扎根理论分析基层医疗卫生机构糖尿病服务能力现状及影响因素，为促进基层医疗卫生机构糖尿病服务能力提供理论依据。
1　对象与方法

1.1　研究对象
本研究采用目的抽样和方便抽样的方式在成都市某区选择调查对象，并于2023年3—4月开展调研。具体方法为：第一阶段，采取目的抽样的方法，根据地理位置、经济水平、机构规模大小等因素在成都市某区抽取4所乡镇社区医院作为研究单位进行调研；第二阶段，采用方便抽样的方法，在所选取基层医疗卫生机构中各抽取1名机构管理人员、4名医务人员以及所选机构正在管理的2名糖尿病患者作为访谈对象进行半结构化访谈。最终，本研究共选取了28名人员（4名机构管理人员、16名医务人员以及8名在管患者）作为访谈对象。
1.2　调查方法
在访谈前先进行访谈提纲的编制，提纲按照访谈对象包括机构管理人员、医务人员和在管患者分别进行设计。正式访谈时，访谈者按照访谈提纲对受访者进行半结构化访谈，并根据受访者实际回答情况进行补充提问，直至达到信息饱和才停止访谈。每次访谈时间为20～30 min，访谈地点选在被调研机构的独立办公室。在分析中，利用理论饱和度检验评估理论是否达到饱和状态[5]。
1.3　资料分析
本研究运用扎根理论对转录资料进行分析，通过三级编码构建“基层医疗卫生机构糖尿病服务能力影响因素模型”理论框架，见图1。本研究所获资料均采用NVivo 12.0软件进行分析。
2　结果

2.1　开放式编码
对收集到的资料进行概念化，共挖掘出174条初始概念，归纳整合后得到24个范畴。见表1。
2.2　主轴编码
对开放式编码所得的24个初始范畴按逻辑和相互关系整合，得到7个主范畴，包括医疗配备、医师现状、能力提升、医联体情况、疾病费用、患者管理以及诊疗情况。见表2。
2.3　选择性编码
对主轴编码所得的7个主范畴凝练整合后，最终概括出3个核心范畴，包括资源配备、政策保障、日常运营。见表3。
2.4　理论饱和度检验
本研究使用预留的7份访谈资料进行理论饱和度检验，结果显示达到理论饱和状态。
2.5　理论框架构建
结果显示，3个核心范畴对基层医疗卫生机构糖尿病服务能力存在明显影响，均衡资源配备是核心问题，加强政策相关保障是重要手段，完善日常运营是关键环节。在此基础上，本研究构建了“基层医疗卫生机构糖尿病服务能力影响因素模型”的理论框架。见图2。

3.1　基层医疗卫生机构卫生资源配备存在不足
尽管国家早在2009年就提出“加强公共服务信息平台的建设”，但在接受本次调研的基层医疗卫生机构中，智慧医疗系统几乎是所有机构应用短板。智慧医疗平台建设属于医疗“新基建”，不仅需要信息化专业技术人员，还需要医疗从业人员和大数据分析相关专业人员，且短期不容易取得显著成效[6]。药品的可及性是患者能否获取规范治疗的最基本保障。本次研究发现各医疗卫生机构都存在不同程度基本药物配备不齐的情况，这对于糖尿病患者是否选择在基层就诊产生很大影响。同时，基层医疗卫生机构是健康中国的“主力军”[7]，相较于综合性医院，基层医疗卫生机构医护人员缺乏，尤其是难以招纳到专科医生。由于缺乏糖尿病等专病持续业务能力提升长效机制，接受调研的4家医疗卫生机构的全科医师在糖尿病规范诊疗和新进展了解等方面有很大的局限性，从而制约区域内糖尿病患者获取到同质化、均等化的糖尿病医疗卫生服务。基层医务人员数量少、编制数不足，且基层工作类别多、工作内容繁琐、工作覆盖辖区大，导致基层医务工作者难以有前往上级医疗单位进行学习或进修的机会。此外，上级医院专科医师人才紧缺、长效激励机制不足等因素限制了上级医疗机构专科医师对基层医疗卫生机构的有效业务指导，进而制约了糖尿病专病服务能力的持续改进与优化。
3.2　基层医疗卫生机构日常运营尚需改善
基层医疗卫生机构对糖尿病患者采取规范化管理能够提高血糖控制率，对糖尿病病情控制和患者的预后都具有积极意义。当前基本公共卫生服务糖尿病管理项目已全面实施，但糖尿病管理的规范性还有待提高。调研结果显示，患者接受最多的随访服务包括健康体检、生活方式指导、随访病情评估、用药指导等，而较少涉及足背动脉搏动检查、心理干预等内容，管理内容不够全面。目前基层医疗卫生机构对于糖尿病患者的诊治和健康管理都凭借医务人员日常操作经验，缺乏制定相应的规范化流程，难以保证管理的系统性和科学性。由于基层工作量大、工作内容繁琐等问题，医务人员均存在不同程度的职业倦怠，对糖尿病患者的健康管理更多是依照国家基本公共卫生服务项目中的内容执行，管理方式缺乏主动性，管理质量和效率有待提高。
3.3　基层医疗卫生机构相关政策保障亟需加强
为了减轻患者的经济负担，近年来各地行政主管部门陆续出台《基本医疗保险门诊特殊疾病管理办法》来改善糖尿病等慢性病的治疗和管理[8]。由于特殊门诊[9]办理的宣传力度不足，使大部分患者对特殊门诊产生办理手续复杂、门槛费用高、作用不大的误解，以致办理门诊特殊疾病患者的比例非常小。此外，区域医联体的创建将是医疗行业发展的基本趋势[10]，而本次所调研地区的基层医疗卫生机构在医联体创建方面进度均较缓。强化医联体工作，推动更多上级医疗机构专业技术人员对区域内医疗机构进行相关惯例、技术培训，是发挥好上级医院辐射带动作用和下级医院做好患者管理，提升区域内糖尿病管理同质化，解决区域内糖尿病患者“看病难”和“看病贵”的重要抓手。
3.4　对策建议
3.4.1　完善基层医疗卫生机构的卫生资源配备
在物资配备方面，基层医疗卫生机构力量薄弱，应向卫生行政主管部门报告智慧医疗的优势与发展趋势，发挥体制机制优势，促进行政区域内智慧医疗体系的构建。在药物可及性提升方面，根据国家诊疗规范、医保政策、经济水平、患者实际需求等多方面因素来进行科学购置，药品购置管理人员应加强与临床医生交流沟通，强化药品进销存管理，做好药品采购计划，降低药品浪费。在人力资源方面，需要科学测算公办基层医疗卫生机构的编制数并满足其实际需求。同时，基层医疗卫生机构要加大专业人员的培养力度，选派业务骨干去上级医院专科进修学习，定期邀请上级专家下基层进行业务培训和工作指导[11]。基层医疗卫生机构应当加强工作激励机制，做好工作专项细化，提高医务人员薪资待遇，改善医护人员工作环境及劳动保障，提升职工工作与学习积极性，力求吸纳更多经验丰富、医术精湛的医务人员，留住医院自身培养的专业人才，确保基层人才队伍稳定。
3.4.2　加强基层医疗卫生机构日常运营
医务人员应当严格遵照要求对患者执行随访管理，随访频次不足和管理内容不全当作为重点着重改善。基层医疗卫生机构尽早制定符合本单位实际情况的糖尿病规范化诊治和管理流程，以便更好指引和规范医务工作者日常工作。医院管理者应当健全单位糖尿病相关工作奖惩制度，着重加大激励机制，激发职工工作积极性，缓解员工职业倦怠情绪，提升医务人员工作效率与管理质量。在沟通方面，需要强化和提升医务工作者的医患沟通技能，以通俗易懂的表达方式给患者进行疾病相关知识的讲解。
3.4.3　健全基层医疗卫生机构相关政策保障
加大糖尿病相关保障政策、特殊门诊政策宣传力度和优化门诊特殊疾病办理程序，适度降低特殊门诊起付标准是降低患者负担、解决看病贵的重要手段。同时，医联体不但能提高医疗服务体系整体效率，而且能提升基层医疗质量[12]。区域内基层医疗卫生机构要积极做好医联体工作，通过上级医疗机构的培训，加快糖尿病管理同质化与均等化的实现。
