Objective

To measure and evaluate the compatibility between the level of outpatient chronic special disease coverage under the basic health insurance and the level of healthcare resource service utilization in Shanxi Province, so as to provide reference for the coordinated development of the level of outpatient chronic special disease coverage under the basic health insurance and the level of healthcare resource service utilization in Shanxi Province.

Methods

Taking 11 cities in Shanxi Province as the study object, the fitness between the two was measured using the entropy weight-TOPSIS method and gray correlation analysis.

Results

From the perspective of utilization level, the numerical intervals of breadth and depth of healthcare resource service utilization in Shanxi Province were 0.161-0.751 and 0.000-1.000, respectively, with a wide span of the intervals; from the perspective of protection level, the numerical intervals of breadth, width, and sustainability of outpatient chronic and special disease protection of the basic healthcare insurance in Shanxi Province were 0.761-0.994, respectively, 0.518-0.938 and 0.445-1.000; Taiyuan City had a comprehensive score of 0.734, which was the city with the highest degree of suitability.

Conclusion

This can be done by consolidating the stability of basic medical and healthcare human resources, enhancing the balance of medical resource allocation, strengthening the synergy between the breadth and depth of healthcare insurance coverage, and reinforcing the linkage between healthcare insurance policies and healthcare resources, so as to improve the level of coverage for outpatient chronic and special illnesses under the basic healthcare insurance scheme in Shanxi Province, as well as the level of utilization of healthcare resources and the degree of appropriateness of the two.

目的

测算与评估山西省基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平适配度,为山西省基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平的协调发展提供借鉴与参考。

方法

以山西省11个地市为研究对象,利用熵权-TOPSIS法与灰色关联度分析法,测算两者之间的适配度。

结果

从利用水平的角度来看,山西省医疗卫生资源服务利用广度、深度的数值区间分别是0.161~0.751、0.000~1.000,区间跨度较大;从保障水平的角度来看,山西省基本医保门诊慢特病保障广度、宽度、可持续性的数值区间分别是0.761~0.994、0.518~0.938、0.445~1.000;太原市综合得分0.734,为适配度最高的城市。

结论

可通过巩固基层医疗卫生人力资源的稳定性,提升医疗资源配置的均衡性,加强医保保障广度与深度的协同性,强化医保政策与医疗资源的联动性,提高山西省基本医保门诊慢特病保障水平和医疗卫生资源的服务利用水平及二者的适配度。

目的

测算与评估山西省基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平适配度,为山西省基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平的协调发展提供借鉴与参考。

方法

以山西省11个地市为研究对象,利用熵权-TOPSIS法与灰色关联度分析法,测算两者之间的适配度。

结果

从利用水平的角度来看,山西省医疗卫生资源服务利用广度、深度的数值区间分别是0.161~0.751、0.000~1.000,区间跨度较大;从保障水平的角度来看,山西省基本医保门诊慢特病保障广度、宽度、可持续性的数值区间分别是0.761~0.994、0.518~0.938、0.445~1.000;太原市综合得分0.734,为适配度最高的城市。

结论

可通过巩固基层医疗卫生人力资源的稳定性,提升医疗资源配置的均衡性,加强医保保障广度与深度的协同性,强化医保政策与医疗资源的联动性,提高山西省基本医保门诊慢特病保障水平和医疗卫生资源的服务利用水平及二者的适配度。

Objective

To measure and evaluate the compatibility between the level of outpatient chronic special disease coverage under the basic health insurance and the level of healthcare resource service utilization in Shanxi Province, so as to provide reference for the coordinated development of the level of outpatient chronic special disease coverage under the basic health insurance and the level of healthcare resource service utilization in Shanxi Province.

Methods

Taking 11 cities in Shanxi Province as the study object, the fitness between the two was measured using the entropy weight-TOPSIS method and gray correlation analysis.

Results

From the perspective of utilization level, the numerical intervals of breadth and depth of healthcare resource service utilization in Shanxi Province were 0.161-0.751 and 0.000-1.000, respectively, with a wide span of the intervals; from the perspective of protection level, the numerical intervals of breadth, width, and sustainability of outpatient chronic and special disease protection of the basic healthcare insurance in Shanxi Province were 0.761-0.994, respectively, 0.518-0.938 and 0.445-1.000; Taiyuan City had a comprehensive score of 0.734, which was the city with the highest degree of suitability.

Conclusion

This can be done by consolidating the stability of basic medical and healthcare human resources, enhancing the balance of medical resource allocation, strengthening the synergy between the breadth and depth of healthcare insurance coverage, and reinforcing the linkage between healthcare insurance policies and healthcare resources, so as to improve the level of coverage for outpatient chronic and special illnesses under the basic healthcare insurance scheme in Shanxi Province, as well as the level of utilization of healthcare resources and the degree of appropriateness of the two.

基本医保门诊慢特病保障作为医疗卫生体系改革的重要内容,对于提升居民健康水平、优化医疗资源配置具有重要意义。廖臧宜(2021)提出门诊慢特病医保支付的经济杠杆应该倾向基层及医疗卫生机构,“分级”是区域医疗资源有效优化配置的有效抓手[1]。王微(2022)等学者在研究贵州省慢特病门诊保障制度的过程中指出慢特病大多适宜进行分级诊疗,可以配套医保差异支付的经济方式引导患者合理就医[2]。山西省作为全国县域医共体试点省份之一,自2019年以来,不断创新城市三级医院与县级医疗集团的医联体模式,通过成立以特殊病种为主的专科联盟式医联体,始终以分级诊疗病种诊治为中心,对45种门诊慢特病的支付方式采用以病种付费为主的多元复合式医保支付方式,积极探索门诊特殊病按病种付费、慢性病按人头付费。此外,在对县域内医疗卫生资源的整合上,将县域内政府举办的县级医院、乡镇卫生院和社区卫生服务中心,整合组建为县级医疗集团,逐步将村卫生室纳入县级医疗集团管理;对县域内医疗卫生资源的服务供给优化上,由医疗集团根据县域内人群疾病谱,制定统一的常见病、多发病防治指南,并结合功能定位形成集团内县、乡两级疾病诊疗目标,建立慢性病县乡村三级管理制度,提供筛查、诊断、转诊、随访等连续性、整合性医疗卫生服务。因此山西省在探索基本医保门诊慢特病保障发展和医疗卫生资源服务利用适配度方面具有重要的实践意义和借鉴价值,本研究旨在通过分析山西省基本医疗保险门诊慢特病保障发展水平相关指标和医疗卫生资源服务利用水平相关指标,综合测算基本医保门诊慢特病保障发展水平与医疗卫生资源服务利用水平的适配度,从而揭示其差异和影响因素。
本研究的对象为山西省11个地级市,选取其2022年(最新年份)基本医疗保险门诊慢特病保障发展水平相关指标和医疗卫生资源服务利用水平相关指标,综合测算基本医保门诊慢特病保障发展水平和医疗卫生资源服务利用水平的适配度。其中基本医保门诊慢特病保障水平相关指标来自山西省医疗保障局发布,医疗卫生资源服务利用水平相关指标来自山西省11个地市统计局发布的《2023年统计年鉴》和《2022年国民经济和社会发展统计公报》。
(1)对于医疗卫生资源服务利用水平的测算,本研究选取利用广度、利用宽度2个二级指标,主要包含8个三级指标。①医疗卫生机构的数量能够体现该地区医疗卫生资源配置情况与卫生规划的完善程度[3],包含5个三级指标;②医疗卫生人力资源数量关系医疗服务的有效性、可及性和可持续性[4],包含3个三级指标。(见表1)。
(2)对于基本医保门诊慢特病保障水平的测算,本研究选取保障广度、保障深度和保障可持续性3个二级指标,主要包含12个三级指标。①保障广度体现基本医保门诊慢特病保障的发展水平与覆盖范围[5],包含6个三级指标,其中参保率=该地区参保人数/该地区常住人口数,反映该地区基本医疗保险的覆盖范围;保险密度=该地区的基本医疗保险总收入/该地区的常住人口数,反映该地区的人均基本医疗保险额度与人们对保险的认知程度;②保障深度体现实际基本医保门诊慢特病的保障效果[6],包含3个三级指标,其中保险深度=该地区的基本医疗保险总收入/该地区的国内生产总值,反映该地区基本医疗保险的发展速度与在经济发展中所占的地位[7],政策范围内报销比=政策范围内费用/发生费用,反映医保目录可报销范围内的发生的费用;实际报销比=统筹基金支付/发生费用,反映该地区参保人直接体验的报销费用;③保障可持续性体现基本医保门诊慢特病保障基金在当前医保制度下的收支状况及基金结余的合理性[8],包含3个三级指标。(见表1)。
(1)基于熵权的TOPSIS法。本研究采用熵权TOPSIS研究方法测算医疗卫生资源服务利用水平:①基于原始数据的8个三级指标构建矩阵Tij进行标准化处理,得到规范化决策矩阵Zij;②运用熵权法的信息熵原理,计算信息熵ej和信息效用值dj,归一化后得到指标权重系数wj;③以权重系数先对数据进行加权,根据最优距离D+和最劣距离D-计算得分并归一化后计算相对接近度C值,C值表示评价对象与最优方案的接近程度,该值越大说明越接近最优方案利用水平越高;④依据医疗卫生资源服务利用水平广度和深度的C值计算其结果平均值,得到医疗卫生资源服务利用水平的综合测算结果。具体计算公式参考文献[9]
(2)灰色关联度分析法。本研究采用灰色关联度分析法测算基本医保门诊慢特病保障水平:①对山西省11个地市的保障广度、保障深度、保障可持续性构成的矩阵进行正向化和无量纲化处理,得到预处理矩阵A1、A2、A3,构建成矩阵Rij(其中Rij为三级指标对应的第i行,第j列元素,Xij为预处理矩阵A1、A2、A3第i行,第j列元素);②计算预处理矩阵与其各行最大值构成虚拟母序列Y之间的差;③由此得到gij构成的新矩阵G,对矩阵G求各列平均值,得到山西省11个地市保障广度、保障深度、保障可持续性的灰色关联度值。对其进行归一化构建指标权重,与矩阵A1、A2、A3对应元素相乘后归一化得到山西省各地级市基本医保门诊慢特病保障水平测算结果(其中β为分辨系数等于0.5)。具体公式计算参考文献[9]
(3)适配度计算。通过计算将已经获得的山西省医疗卫生资源服务利用水平结果和基本医保门诊慢特病保障水平结果求均值,可得到山西省11个地市基本医保门诊慢特病保障水平和医疗卫生资源服务利用水平的综合测算结果[9]
山西省各地市医疗卫生资源服务利用水平差距较大。太原市在各项指标中表现最佳,综合得分为0.812,排名第一;而阳泉市的综合得分最低,仅为0.108,排名第十一,显示出显著的梯度差异。运城市和临汾市分别排名第二和第三,医疗卫生资源利用水平相对较高。朔州市在利用深度上得分为零,表现出资源利用的严重不足。(见表2图1)。
根据山西省基本医保门诊慢特病保障水平测算结果,阳泉市在保障广度、保障深度和保障可持续性上表现最佳,综合加权得分0.918,排名第一。临汾市保障可持续性得分最高,运城市保障广度得分最高。吕梁市的保障广度和保障可持续性得分最低,综合加权得分排名末位。整体来看,山西省各地市基本医保门诊慢特病保障水平差距较小,差距值为0.339。(见表3图2)。
山西省医疗卫生资源服务利用水平的测算结果区间值位于0.108~0.812,差距值为0.694,11个地市间的医疗卫生资源服务利用水平差距较明显;基本医保门诊慢特病保障水平测算结果区间值0.579~0.918,差距值为0.339,11个地市间的保障水平相对差距较小。太原市综合加权结果得分排名第一,表明太原市基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平的适配度最高,其次为临汾市;而阳泉市综合加权结果排名末位,表明其基本医保门诊慢特病保障水平与医疗卫生资源服务利用水平的适配度较差。整体而言,山西省基本医保门诊慢特病保障水平优于医疗卫生资源服务利用水平。(见表4图3)。
山西省11个地市间的医疗卫生资源服务利用水平存在显著差异。除太原市外的10个地市的医疗卫生资源服务利用广度普遍高于利用深度,11个地市间医疗卫生资源服务利用水平差距显著差距值达0.704,省会城市太原医疗卫生资源服务利用水平最高。具体而言,医疗卫生机构数量是医疗卫生资源服务利用水平的直接体现,医疗卫生人力的数量关系到医疗卫生服务的有效性、可及性和可持续性[4]。而山西省医疗卫生机构数量的配置普遍优于医疗卫生人员数量的配置,这与李静等学者在2021年对山西省卫生资源配置的公平性分析结论一致,人力资源的不足的原因可能是主要受到政策制定、经济条件、服务设施和薪酬待遇等因素的影响[10],尤其是基层医疗卫生机构面临招人难留人难、“缺人”与“缺人才”并存以及高层次医学人才“引不进、留不住”的困境。目前太原市的医疗卫生资源服务利用水平排名第一,与其作为省会城市所具有的经济发展水平[11]、发展战略和由此产生的虹吸效应存在一定的关系,相对而言对优质医疗卫生资源和人才的吸引更大。运城市的医疗卫生资源服务利用水平排名第二,可能与其入选国家城市医联体建设试点城市,全面推开县域医疗卫生一体化改革,形成一体化健康服务模式,从而实现医疗卫生资源的高效利用。从医疗卫生机构功能的角度来看,医院主攻疑难杂症,基层医疗卫生机构解决常见多发病,尽管山西省基层医疗卫生机构的数量远超过医院数量,但部分居民患小病仍选择医院就医偏好的心理暗示,与我国医疗卫生资源主要集中医院,基层医疗卫生机构资源匮乏的现状一致[12],因此医疗机构数量对医疗卫生资源服务利用水平具有一定影响,而阳泉市的医疗卫生机构和卫生人员的资源配置均排在11个地市的末位,反映出其医疗卫生资源服务利用效率低,这与排名前列的城市差距明显,不利于山西省整体医疗卫生资源服务利用水平的均衡与提升。
山西省11个地市在基本医保门诊慢特病保障水平上存在一定的不平衡。具体体现在保障广度、深度和可持续性三个方面。阳泉市的保障水平最高,得分在三个维度上均位于前列,其基本医保基金收入和保险密度均为全省最高,常住人口较少且经济发展水平较高,显示出其医保发展的领先地位。相对而言,吕梁市的保障水平较低,保险密度最低,且人均医保额度和认知程度较差。从政策实施角度看,尽管全省在病种范围和认定标准上保持统一,各地对门诊慢特病保障的具体政策仍存在较大差异。保障模式、支付限额和支付比例有所不同,晋城市、晋中市和朔州市的支付限额较高,而朔州市、太原市和忻州市的支付比例较高,这些地区保障水平较为领先。此外,在特殊人群的保障方面,各地也存在差异。阳泉市针对两种及以上慢病患者制定了特别保障政策,增加医保支付限额;阳泉市、晋中市和运城市还对贫困人群门诊慢特病患者实施了倾斜政策,确保特殊贫困人群的医疗费用能够得到100%报销。总体而言,山西省各地的医保保障水平存在不平衡,政策和资金投入的差异影响了不同地区的保障效果,尤其是针对特殊人群的差异化保障政策更加凸显了这一特点。
山西省在基本医保门诊慢特病保障水平与医疗卫生资源服务利用方面存在较大差异。表现出复杂的地域特征和挑战,尽管山西省在医疗资源配置和医保保障上取得了一定发展,二者之间的协调性和匹配度仍面临问题。首先,不同地市的医保保障水平与医疗资源使用情况不匹配。例如,阳泉市在医保保障方面表现出色,保障水平在全省领先,但其医疗资源配置相对较低,导致医疗服务的可及性和质量无法充分满足高保障要求。而太原市虽然在医疗资源配置和服务利用方面位居全省前列,但其医保保障政策的覆盖和实施效果仍有提升空间,未能与医疗资源水平相匹配。其次,区域保障政策与资源配置的适配具有明显关联,不同地区的保障政策差异与医疗资源分布相互作用,影响了政策执行效果。例如,阳泉市在特殊人群保障政策上较为宽松,但医疗资源不足,导致医保保障无法转化为有效的医疗服务;吕梁市则缺乏足够的保障政策支持,影响其保障水平的提升。在运城市,尽管医疗资源利用和医保保障水平较高,但政策执行和资源协调仍面临挑战,如何实现保障政策与资源配置的有效对接是当前的关键问题。总体来看,山西省的医保保障与医疗资源的协调性亟需加强,确保政策实施与资源配置的有效结合,提升各地患者的医疗服务体验。
为了提升山西省基本医保门诊慢特病保障水平和医疗卫生资源的服务利用水平及两者之间的适配度,基于研究结果做如下建议。首先,巩固基层医疗卫生人力资源的稳定性,考虑增加基层医务人员的薪酬待遇和职业发展机会,鼓励人才扎根基层,并通过定向培养和合作办学等方式培养符合地方需求的医学人才,以政策支持和激励措施吸引医疗人才到偏远地区工作[13]。其次,提升医疗资源配置的均衡性,政府可通过对部分地区如阳泉市等的财政支持和政策引导,推动县域医共体和远程医疗的融合,优化分级诊疗体系,提升基层医疗机构的服务能力,减轻大医院压力,实现资源的合理流动与优化配置。第三,加强医保保障广度与深度的协同性,针对保障水平较弱的地区如吕梁市,制定倾斜政策以减轻低收入群体负担,以及推广阳泉市的多病种保障政策,确保特殊群体如多病共患、贫困及老年人享有足够保障。最后,强化医保政策与医疗资源的联动性,通过省级及地市级协调机制确保政策落实到位,针对不同地区的需求优化医保政策,对资源匮乏地区提供财政支持,推动医疗设施建设和人员配备,平衡医疗资源和医保保障之间的矛盾,提升整体医疗服务水平[14]
