Objective

To explore the relationship between muscle mass and disability in the elderly.

Methods

This study is based on the tracking survey data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) from 2008 to 2018, utilizing Cox regression and constructing restricted cubic spline models to analyze the dose-response relationship between muscle mass and disability in the elderly.

Results

A total of 2 165 elderly patients were followed up 4 times for an average of 10 years. Among them, 603 (27.8%) developed disability. After adjusting for confounders, muscle mass affected disability in the elderly(HR=0.879,95%CI:0.822-0.939). The RCS results showed that the association between muscle mass and disability risk in older adults was non-linear in both older men(Poverall trend<0.05,Pnon-linear<0.05) and older women(Poverall trend<0.05, Pnon-linear<0.05). It is suggested that ASMI level of 7.2 (kg/m2) and above should be maintained in elderly men and 5.2 (kg/m2) and above in elderly women.

Conclusion

Muscle mass is associated with disability in the elderly. Maintenance or improvement of muscle mass can improve the quality of life in the elderly.

目的

探索肌肉质量与老年人失能的关系。

方法

本研究基于中国老年健康影响因素跟踪调查项目2008—2018年四期的追踪调查数据，利用Cox回归，并构建限制性立方样条模型分析肌肉质量与老年人失能的剂量-反应关系。

结果

纳入2 165名老年人，随访4次，平均随访10年。其中603名(27.8%)发展为失能老人；调整混杂因素后肌肉质量对老年人失能产生影响（HR=0.879，95% CI:0.822~0.939）；RCS结果显示，老年人肌肉质量与失能风险的关联在老年男性（P总趋势<0.05，P非线性<0.05）和老年女性（P总趋势<0.05，P非线性<0.05）中均呈非线性增高趋势，提示老年男性ASMI应维持7.2（kg/m2）及以上水平和老年女性ASMI应维持5.2（kg/m2）及以上水平。

结论

老年人肌肉质量与其失能存在关联，通过维持或提高肌肉质量水平，可以改善老年人晚期的生活质量。

目的

探索肌肉质量与老年人失能的关系。

方法

本研究基于中国老年健康影响因素跟踪调查项目2008—2018年四期的追踪调查数据，利用Cox回归，并构建限制性立方样条模型分析肌肉质量与老年人失能的剂量-反应关系。

结果

纳入2 165名老年人，随访4次，平均随访10年。其中603名(27.8%)发展为失能老人；调整混杂因素后肌肉质量对老年人失能产生影响（HR=0.879，95% CI:0.822~0.939）；RCS结果显示，老年人肌肉质量与失能风险的关联在老年男性（P总趋势<0.05，P非线性<0.05）和老年女性（P总趋势<0.05，P非线性<0.05）中均呈非线性增高趋势，提示老年男性ASMI应维持7.2（kg/m2）及以上水平和老年女性ASMI应维持5.2（kg/m2）及以上水平。

结论

老年人肌肉质量与其失能存在关联，通过维持或提高肌肉质量水平，可以改善老年人晚期的生活质量。

Objective

To explore the relationship between muscle mass and disability in the elderly.

Methods

This study is based on the tracking survey data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) from 2008 to 2018, utilizing Cox regression and constructing restricted cubic spline models to analyze the dose-response relationship between muscle mass and disability in the elderly.

Results

A total of 2 165 elderly patients were followed up 4 times for an average of 10 years. Among them, 603 (27.8%) developed disability. After adjusting for confounders, muscle mass affected disability in the elderly(HR=0.879,95%CI:0.822-0.939). The RCS results showed that the association between muscle mass and disability risk in older adults was non-linear in both older men(Poverall trend<0.05,Pnon-linear<0.05) and older women(Poverall trend<0.05, Pnon-linear<0.05). It is suggested that ASMI level of 7.2 (kg/m2) and above should be maintained in elderly men and 5.2 (kg/m2) and above in elderly women.

Conclusion

Muscle mass is associated with disability in the elderly. Maintenance or improvement of muscle mass can improve the quality of life in the elderly.

老年人失能是指因年老、伤残、疾病等因素导致不能独立进行洗澡、穿衣、如厕、室内转移、进食和控制大小便等日常生活基本活动的能力，即日常生活活动能力（Activities of daily living，ADL）受损[1]。第七次人口普查显示我国60岁以上老年人高达2.6亿，其中失能老人约600万左右[2]。有研究预测，我国老年人失能率自2020年至2022年间呈上升态势，在2023年至2041年预计将经历下降趋势后快速上升[3]。多项研究表明[4]，肌肉质量降低可能是老年人失能的危险因素，肌肉质量在30岁左右开始下降[5]，并导致许多不良结局，包括残疾、跌倒、骨折、虚弱、健康相关生活质量受损和死亡率增加[6]。但目前探讨两者关联性的研究较少，且多是横断面分析或将肌肉质量以分类资料进行分析，可能造成偏倚。且在失能过程中，女性的残疾率高于男性，健康行为和社会支持是造成残疾性别差异的最重要因素[7]。为了减少残疾方面的性别不均等，应更多地关注造成失能性别不均等的相关因素。因此本研究拟基于中国老年健康调查数据库（Chinese Longitudinal Healthy Longevity Survey，CLHLS）的数据，通过构建限制向立方样条模型（Restricted cubic spline，RCS）并进行分层讨论，探讨肌肉质量的连续变化与不同性别老年人失能的剂量-反应关系，为老年人失能的预防和健康干预提供参考。
1　研究对象与方法

1.1　数据来源及研究对象
本研究数据来源于 CLHLS数据库，该数据库涵盖了全国23个省市自治区500多样本点地区各年龄段的老年调查对象，在1998年进行基线调查后进行了七次跟踪调查。本研究已通过北京大学开放研究数据平台申请并获取数据使用权，选取了参与2008年、2011年、2014年和2018年四次调查的老年人（≥60岁）2 440名，基于研究内容需要，剔除基线存在失能状态的老年人275名，最终纳入2 165个研究对象。
1.2　研究方法
1.2.1　日常生活活动能力量表
采用Katz[8]编制的日常生活活动能力量表评估日常生活自理能力，该量表由六个项目组成：从洗澡、穿衣、如厕、室内转移、进食和控制大小便，每一项目有3个选项：“没有困难”、“有困难，需要帮助”和“无法完成”，只要有1项回答为“有困难，需要帮助”或“无法完成”，即判定为失能，否则为非失能。
1.2.2　肌肉质量
利用Wen等学者[9]开发的中国成年人的人体测量方程得到附肢骨骼肌量（Appendicular skeletal muscle mass，ASM），既往研究[10]表明，该公式计算的ASM与双能X射线吸收测定法结果吻合，并使用调整身高后的相对骨骼肌质量指数（Appendicular skeletal muscle index，ASMI）[11]，用以评估肌肉质量。
ASM=0.193×weight（kg）+0.107×height(cm)-4.157×gender（male=1，female=2）-0.037×age(years)-2.631；ASMI=ASM/ height(m)2。
1.2.3　协变量
本研究纳入的协变量包括年龄、性别、婚姻状况、文化程度、吸烟、饮酒、运动、睡眠质量、体重指数（Body mass index，BMI）、慢病共存和认知功能。其中运动根据自我汇报“您现在是否经常锻炼身体？”进行定义；睡眠质量根据自我汇报“过去一周的时间里，最多两周，您睡眠质量如何?”进行定义；对于慢病共存定义为同时存在两种及以上慢性疾病[12]；认知功能采用中文版简易精神状态检查（Mini-Mental State Examination，MMSE）量表评估，该量表由24个问题组成，总分范围为 0~30 分，得分越低，说明认知功能越差[13]。
1.3　统计学方法
采用SPSS 27.0、R 4.3.3软件进行统计分析。连续性资料采用描述，分类资料采用频数、百分比描述。运用独立样本t检验或χ2检验进行差异性分析。利用Cox回归分析，找出老年人失能的影响因素；利用RCS模型进一步判断肌肉质量与老年人失能之间的剂量-反应关系，结合赤池信息准则(akaike information criterion，AIC)最小为准选取最优的RCS模型。检验水准α=0.05。

2.1　一般资料情况
2 164名研究对象中男性1 054名（48.7%），女性1 110名（51.2%），平均年龄（74.28±7.83）岁。在10年的随访中，老年女性新发失能357例(32.16%)多于老年男性(23.33%)。独立样本t检验或χ2检验结果显示，无失能组和失能组在年龄、性别、吸烟、学历、睡眠质量、慢病共存、婚姻状态、BMI、MMSE得分、ASMI间分布差异具有统计学意义。具体见表1。
2.2　老年人肌肉质量与失能的Cox比例风险分析
以是否成为失能老人为因变量，肌肉质量为自变量，年龄、性别、吸烟、学历、睡眠质量、慢病共存、婚姻状态、BMI、MMSE得分为协变量进行多因素Cox比例风险回归分析。结果表明，随着肌肉质量的增高，老年人10年后日常生活能力受损发病风险降低（HR=0.879，95%CI:0.822~0.939）。具体见表2。
2.3　限制向立方样条
应用RCS模型分析生育史与认知功能障碍之间的关系，该模型中节点数选择3~7个为宜。根据样条回归系数和AIC准则，本研究发现在认知功能障碍相关的模型中，AICmale=634.174、AICfemale=609.922时达到最小，节点分别为5个、3个。模型结果显示，老年男性肌肉质量与失能风险之间存在非线性剂量-反应关系（P总趋势<0.05，P非线性<0.05），ASMI<7.2（kg/m2）时，OR>1，此时肌肉质量是老年男性失能的危险因素；老年男性肌肉质量与失能风险之间存在非线性剂量-反应关系（P总趋势<0.05，P非线性<0.05），ASMI<5.2（kg/m2）时，OR>1，此时肌肉质量是老年女性失能的危险因素。结果见图1-图2。
2.4　亚组分析和敏感性分析
采用亚组分析，以研究肌肉质量与老年人失能之间的关系，结果见图3。研究结果表明，在某些亚组中，肌肉质量的减少与老年人失能的出现一致。具体而言，这种相关性在年龄为60~89岁、不存在慢性共病、BMI<27.9 kg/m2均有统计学意义（P<0.05）。交互作用检验显示，年龄、性别、慢性共病、BMI对肌肉质量与老年人失能之间相关性不具有交互作用（Pfor interaction>0.05)。同时，本研究通过剔除三年内出现失能的老年人后进行敏感性分析。结果显示，随着肌肉质量的增高，老年人失能风险降低（HR=0.752，95%CI:0.590~0.914）与本研究结果相似，其他各项敏感性分析结果与主要分析结果基本保持一致，表明研究结果具有稳健性。因篇幅限制，文中对此结果不展示。
3　讨论

3.1　老年女性失能发生率高于老年男性
本研究每100人年的发病率为2.78%，低于Berlau[14]在美国老年人群中发现发病率为8.3%。这可能与数据的样本构成有关，Berlau研究对象的年龄段是90-94岁显著高于本研究对象的平均年龄。Cox回归显示，老年女性发生失能的风险是老年男性的1.439倍，与杨洋等学者[15]的研究一致。有研究表明女性老年人虽然比男性老年人享有更长的预期寿命，但与男性相比，女性进行体育锻炼和体力活动的时间较少，健康状况较差，以及社会和家庭角色的分配使男女在生活方式、卫生资源利用、卫生服务可及性等方面存在广泛的差别所造成的累积性效应，可能让女性在生命后期经历更糟糕的健康状态[16]。
3.2　肌肉质量是老年人失能的影响因素
本研究显示，肌肉质量是老年人失能的影响因素，随着肌肉质量的降低其对老年人失能的保护作用逐渐减弱当降低到一定程度时发展为危险因素，敏感性分析得到类似结果，这进一步佐证Liu等学者[17]的研究结论。肌肉质量减少是由于蛋白质分解代谢增加或合成代谢抵抗增加而导致的不良肌肉变化的终生积累，这两种情况在老年人中都很常见，它可以增加不良临床结果的发生率，如跌倒、骨折、身体残疾，最终导致老年人失能[18]。此外，肌肉质量减少的发生与机体慢性低度炎症[19]、营养的合成代谢反应受损[20]等方面有关，并引起老年人的多种病理现象有关，包括骨质疏松症、动脉粥样硬化、免疫功能下降和胰岛素抵抗等[21]，使老年人失能风险增加。提示我们可以通过运动和营养提高或维持肌肉质量预防失能。
RCS结果显示，随着肌肉质量的减少，与失能患病风险的关联在老年男性和老年女性中均呈非线性增高趋势，老年男性ASMI应维持在7.2（kg/m2）及以上，老年女性ASMI应维持在5.2（kg/m2）及以上。分析原因可能与男女生在身体成分的组成上存在差异，女性的脂肪普遍高于男性，男性的初始肌肉质量也同样大于女性[22] ，因此在推荐的ASMI具体数值上老年男性高于女性，这与Wen[9]在开发人体测量方程中所考虑的性别差异一致。此外本研究发现，性别并不影响肌肉质量与老年人失能之间的关系。而Liu[17]在纵向研究中发现，低肌肉质量的女性失能风险比男性大，与本研究结论不一致。分析原因可能是该学者在分析时并未涉及到交互效应，因此无法分辨是效应修饰的作用还是随机误差的作用[23]。
Kitamura等学者[24]采用传统的Cox回归模型进行分析，探讨肌肉质量与老年人失能患病风险的关联，发现肌肉质量下降可以预测了老年人失能，与本研究结果一致，但忽略肌肉质量连续性变化与老年人失能患病风险间的动态变化轨迹。而本研究通过限制性立方样条模型将连续性的肌肉质量与老年人失能患病风险结合，呈现二者间的剂量-反应关系，更加直观地表现了关联强度的动态变化轨迹，优于传统的Cox回归模型，更具有公共卫生指导价值。但肌肉质量与老年人失能的潜在机制尚未完全阐明，既往研究发现老年人肌肉衰退最明显的代谢解释是蛋白质合成和分解率之间的不平衡[25]，但其他原因（如神经退行性过程）、合成代谢激素产生或敏感性（如胰岛素、生长和性激素）减少、细胞因子分泌失调、对炎症事件的反应改变、营养摄入不足和久坐不动的生活方式都涉及[26-29]。
本研究结果表明，肌肉质量与老年人失能存在关联。当老年男性ASMI<7.2（kg/m2）和老年女性ASMI<5.2（kg/m2）时，肌肉质量是老年人失能的危险因素。因此，通过合理饮食营养干预、加强体育锻炼等办法提升肌肉质量，减少老年人失能情况的发生，降低老年人失能率。本研究为基于大数据库的纵向研究，缺乏影响肌肉质量的实验室相关指标且依赖于自我报告的数据，可能存在信息偏倚，尚不足以全面揭示肌肉质量与老年人失能之间的生物学机制，但为肌肉质量与老年人失能之间的关系提供了关联线索。
