Objective

To investigate the association between dietary lycopene intake and cognitive function in adults aged 60 years and older.

Methods

Participants were selected from adults aged 60 years and older in the 2011-2014 NHANES database. A logistic regression model was constructed and adjusted for confounding factors such as age and gender to explore the relationship between dietary lycopene intake and cognitive function in older adults.

Results

A total of 2 524 participants were included in this study, with a median dietary lycopene intake of 1 905.25 mcg. After adjusting for age, gender, and other confounding factors, the logistic regression model showed a significant negative correlation between cognitive impairment and dietary lycopene intake in immediate recall test (OR=0.75; 95%CI: 0.58-0.98), delayed recall test (OR=0.78; 95%CI: 0.60-0.99), and digit-symbol substitution test (OR=0.67; 95%CI: 0.49-0.91). Additionally, a significant negative correlation was observed between cognitive impairment and dietary lycopene intake based on composite cognitive scores (OR=0.67; 95% CI: 0.50-0.90). Trend analysis indicated that the risk of cognitive impairment decreased progressively with increasing dietary lycopene intake.

Conclusion

Dietary lycopene intake is negatively correlated with cognitive impairment in older adults. Increased lycopene intake may reduce the risk of cognitive impairment in the elderly.

探讨60岁以上老年人膳食番茄红素摄入和认知功能之间的关联。

方法

结果

结论

膳食番茄红素摄入与老年认知功能障碍之间存在负相关关系,摄入番茄红素可能会降低老年人患认知功能障碍的风险。

目的

探讨60岁以上老年人膳食番茄红素摄入和认知功能之间的关联。

方法

选择2011—2014年NHANES数据库中60岁以上老年人作为研究对象,通过构建logistic回归模型,对年龄、性别等混杂因素进行调整,探究膳食番茄红素摄入与老年人认知功能之间的关联。

结果

本研究共纳入2 524人,膳食番茄红素摄入量中位数为1 905.25 mcg。在调整了年龄、性别等混杂因素后的logistic回归模型显示出即时回忆测试(OR=0.75;95% CI:0.58~0.98)、延迟回忆测试(OR=0.78;95% CI:0.60~0.99)和数字符号替代测试(OR=0.67;95% CI:0.49~0.91)得分下的认知功能障碍与膳食番茄红素摄入量之间呈显著负相关;在调整了年龄、性别等混杂因素后的logistic回归模型显示出综合化认知评分下的认知功能障碍与膳食番茄红素摄入量之间呈显著负相关(OR=0.67;95% CI:0.50~0.90)。此外,趋势性检验结果表明认知功能障碍的患病风险随着膳食番茄红素摄入量的增加而逐步降低。

结论

膳食番茄红素摄入与老年认知功能障碍之间存在负相关关系,摄入番茄红素可能会降低老年人患认知功能障碍的风险。

Objective

To investigate the association between dietary lycopene intake and cognitive function in adults aged 60 years and older.

Methods

Participants were selected from adults aged 60 years and older in the 2011-2014 NHANES database. A logistic regression model was constructed and adjusted for confounding factors such as age and gender to explore the relationship between dietary lycopene intake and cognitive function in older adults.

Results

A total of 2 524 participants were included in this study, with a median dietary lycopene intake of 1 905.25 mcg. After adjusting for age, gender, and other confounding factors, the logistic regression model showed a significant negative correlation between cognitive impairment and dietary lycopene intake in immediate recall test (OR=0.75; 95%CI: 0.58-0.98), delayed recall test (OR=0.78; 95%CI: 0.60-0.99), and digit-symbol substitution test (OR=0.67; 95%CI: 0.49-0.91). Additionally, a significant negative correlation was observed between cognitive impairment and dietary lycopene intake based on composite cognitive scores (OR=0.67; 95% CI: 0.50-0.90). Trend analysis indicated that the risk of cognitive impairment decreased progressively with increasing dietary lycopene intake.

Conclusion

Dietary lycopene intake is negatively correlated with cognitive impairment in older adults. Increased lycopene intake may reduce the risk of cognitive impairment in the elderly.

认知功能障碍是由于脑部加速老化造成的[1],其特征包括记忆力衰退、注意力不集中、信息处理效能减弱、思维清晰度下降或记忆缺失,以及语言意义理解上的偏差。此外,它还可能扰乱正常的逻辑与决策过程[2-3]。认知功能障碍老年人的理解力、记忆力及其他认知功能的衰退不仅加剧了其自身的生存挑战,同时也加重了照护者及家庭其他成员的心理与经济负担。根据世界卫生组织2021年公布的《公共卫生领域应对痴呆症全球状况报告》,随着人口老龄化的加剧,预计到2030年全球患认知功能障碍的人数将达到7 800万,到2050年将达到1.39亿[4]。认知功能障碍的发病机制复杂,目前临床上缺乏特异性的治疗手段[5],故防控措施转向各种风险因素。在公共卫生视角下,认知功能障碍风险防控应集中针对高危群体。对早期认知功能衰退者实施干预管理,能有效延缓其发展[6]。近年来,营养因素对老年人认知功能障碍的影响成为研究热点,其作为可调控的因素对预防老年人认知功能障碍方面具有重要意义。
番茄红素是一种多不饱和烃类植物色素,普遍存在于红色果蔬当中,是四元萜类胡萝卜素家族的重要成员,在番茄中存在最为广泛[7],主要通过食物摄入,具有诱导细胞间隙连接通讯、调控细胞增殖和抗氧化应激等生物活性[8]。有动物研究表明,番茄红素有通过减轻氧化应激、抑制炎症因子的产生等生物学作用,并显示出神经保护作用,从而改善认知功能障碍[9-10]。但番茄红素和认知功能关系的人群研究尚未得到广泛的关注。
因此,本研究拟根据2011—2014年美国国家营养与健康调查(National Health and Nutrition Examination Survey, NHANES)中24小时膳食回顾数据和认知测试问卷,探索60岁及以上年龄老年人番茄红素摄入量和认知功能之间的关系。
本研究选取NHANES 2011—2014的数据集,该数据集包含2011—2012、2012—2014两个周期的调查结果(具有认知功能测试评分最近的两个周期),共有19 931名参与者的信息,因为只有60岁及以上的参与者接受了全面的认知功能评估,故排除年龄小于60岁的参与者(n=16 299),进一步排除没有完整番茄红素摄入信息的参与者(n=825)和认知功能测试数据不全的参与者(n=283),最终本研究共纳入2 524名研究对象,样本筛选过程见图1
本研究中膳食番茄红素的数据来源于对所有参与者进行的两次24小时膳食回忆访谈,两次回忆访谈提供了两个24小时内的详细膳食摄入信息,随后用这些信息来估计能量、营养素和其他食物成分的摄入量,并保存在膳食数据模块中。第一次访谈是在移动检查中心面对面进行,随后,在第一次访谈后的3至10天内,通过电话方式完成了第二次访谈[11]。本研究取两次回忆访谈中番茄红素摄入量的平均值作为参与者番茄红素膳食摄入量[12]
在NHANES 2011—2014年的两个调查周期中,60岁及以上老年人参与者的认知功能评估是通过以下测试完成的:阿尔茨海默病注册中心(CERAD)的即时回忆测试(IRT)和延迟回忆测试(DRT),分别用于评估对新语言信息的即时和延迟学习能力;动物流畅性测试(AFT),用于检查分类语言流利程度,这是执行功能的组成部分,AFT得分较低意味着语言功能的损害;数字符号替代测试(DSST),是韦氏成人智力量表(WAIS Ⅲ)的一个性能模块,它依赖于处理速度、持续注意力和工作记忆。CERAD的即时回忆测试结果为三个单独的试验得分,范围为0到10,三个试验的总分范围为0到30;延迟回忆测试得分范围为0到10;动物流畅性测试的得分范围为3到39;最后,数字符号得分范围在0到105之间。
运用整个队列基线的均值与标准差,将认知测试所取得的分数转化为标准化的z分数。再对这四项z分数进行平均处理以得到一个综合化的认知分数[13]。鉴于目前尚无公认的金标准截断值来界定IRT、DRT、AFT及DSST测试中认知功能障碍的阈值,根据已有的研究,本研究以研究群体中这四项测试分数的25%分位点作为截断值,以此表示不同维度的认知功能障碍[14]
协变量包括年龄(60~69岁、70~79岁、≥80岁)、性别(男、女)、种族(墨西哥裔、其他西班牙裔、非西班牙裔白人、非西班牙裔黑人或其他)、教育程度(高中以下、高中、高中以上)、家庭收入与贫困比率(>1,高于贫困线、≤1,低于贫困线)[15]、体重指数(BMI)(<25 kg/m2、25~30 kg/m2、>30 kg/m2)、吸烟情况(一生中抽过100支烟被定义为“吸烟”)、饮酒情况(在任何一年中至少喝过12杯任何类型的酒被定义为“饮酒”)。另外,糖尿病和高血压的数据来自问卷数据模块。曾被医生或其他健康专业人士告知过患有糖尿病,回答“是”即定义为“患有糖尿病”;曾被医生或其他健康专业人士告知过患有高血压,回答“是”即定义为“患有高血压”。在所有收集到的数据中,协变量的缺失比例均维持在8%以下[16]。因此,本研究采用R语言环境下Mice包提供的多重插补技术来对缺失数据进行填补,以减少缺失数据可能导致的偏倚,从而提高统计效能。
本研究的数据分析使用R 4.1.1进行;在研究人群的基线数据描述中,分类变量用频数(百分比)表示,使用χ2检验或Fisher确切概率法比较组间差异;服从正态分布的连续变量用()表示,使用单因素方差分析或t检验比较组间差异;不服从正态分布的资料以MP25,P75)表示,使用Wilcoxon秩和检验比较组间差异;最后,采用Wilcoxon秩和检验分析等级资料。将IRT、DRT、AFT、DSST及综合化认知评分根据25%分位点作为截断值转化为认知功能障碍和正常认知功能两组纳入分析,使用logistic回归模型分析番茄红素摄入量与认知功能障碍的关系。为了调整混杂因素,本研究构建了三个模型,模型1未调整协变量;模型2调整参与者的年龄、性别、种族、教育程度、家庭收入与贫困比率(经济状况)、BMI、吸烟、饮酒;模型3在模型2的基础上再根据参与者是否患糖尿病和高血压进行调整。
本研究经筛选后共纳入研究对象2 524人,其中男性1 217人(48.2%),女性1 307人(51.8%)。番茄红素摄入量中位数为1 905.25 mcg,所有类型认知功能障碍人群番茄红素摄入量均低于正常认知功能人群,且差异具有统计学意义。参与者平均年龄为69.45岁,60~69岁、70~79和≥80岁人群分别占50.2%、27.8%和22.0%。按认知功能障碍综合化评分、IRT、DRT、AFT、DSST得分分组,分别有631、541、983、730、592人被定义为不同维度的认知功能障碍。具体见表1
本研究使用了三个模型分别对番茄红素摄入量和IRT、DRT、AFT、DSST评分下的认知功能障碍进行了logistic分析,自变量为番茄红素摄入量(Q1、Q2、Q3、Q4),因变量为四种不同维度下的认知功能障碍(正常认知功能、认知功能障碍)。以探索番茄红素摄入量和不同维度下的认知功能障碍之间的潜在关联性。计算结果见表2
在IRT和DRT中,三个模型的Q3和Q4组的OR值显著低于Q1,趋势性检验结果均有统计学意义,表明即时学习能力和延迟学习能力方面的认知功能障碍患病风险随着番茄红素摄入量的增加而降低;在AFT中,Q2、Q3和Q4组的OR值均低于Q1,但模型2和模型3均未显示出统计学意义,趋势性检验也未显示出统计学意义,表明动物流畅性得分下的认知功能障碍患病风险可能与番茄红素摄入量呈负相关,但差异无统计学意义;在DSST中,Q4组的OR值显著低于Q1,趋势性检验结果均有统计学意义,表明数字符号替代测试得分下的认知功能障碍患病风险随着番茄红素摄入量的增加而降低。
为了进一步探索番茄红素摄入量和认知功能障碍之间的潜在关联性,本研究使用了三个模型对番茄红素摄入量和综合化认知评分下的认知功能障碍进行了logistic分析,自变量为番茄红素摄入量(Q1、Q2、Q3、Q4),因变量为综合化认知评分下的认知功能障碍(正常认知功能、认知功能障碍)。调整的协变量同番茄红素摄入量和不同维度认知功能障碍的logistic分析一致。将番茄红素摄入量按照四分位数分组进行计算,计算结果见表3。三个模型都显示出认知功能障碍与番茄红素摄入量之间呈负相关,且具有统计学意义。此外,趋势性检验结果均具有统计学意义,表明认知功能障碍的患病风险随着番茄红素摄入量的增加而逐步降低。
在对番茄红素摄入量和认知功能障碍进行logistic分析后,本研究进一步根据认知功能综合化评分进行了不同年龄、性别下的亚组分析和交互作用分析,自变量为番茄红素摄入量(Q1、Q2、Q3、Q4),因变量为综合化认知评分下的认知功能障碍(正常认知功能、认知功能障碍)。见表4表5。结果显示,70~79岁人群中与番茄红素摄入量与最低组相比,最高组的认知功能障碍患病风险最低且具有统计学意义上的关联;在男性和女性人群中,番茄红素摄入量均和认知功能障碍患病风险呈负相关性,且具有统计学意义。交互作用分析结果显示,年龄和性别在与番茄红素摄入量影响认知功能障碍方面不具有交互作用。
本研究从NHANES数据库纳入了共2 524名研究对象,探讨了60岁以上老年人群的认知功能障碍和膳食番茄红素摄入之间的关系,结果表明,认知功能障碍患者膳食番茄红素摄入量明显低于正常人群且差异具有统计学意义,较高的膳食番茄红素摄入量与更低的认知功能障碍患病风险相关。亚组分析的结果提示两者的关联性在70~79岁年龄区间更加明显,而在男性和女性人群中没有明显差异。
番茄红素因其抗炎、抗氧化的特点而备受关注[17]。在阿尔茨海默病的体外研究中,番茄红素通过抑制活性氧的产生、减少线粒体功能障碍和降低促凋亡因子的表达,从而抑制小胶质细胞的促炎症反应,抑制Aβ1-42刺激的细胞凋亡,进而降低Aβ暴露诱导的细胞损伤[10]。另外研究表明番茄红素可以穿过血脑屏障,在人脑中可以检测到,这表明番茄红素可能在维持神经元健康和减轻氧化应激引起的神经损伤方面发挥作用[18]
除此之外,动物实验表明番茄红素可以改善大鼠和小鼠的认知功能。例如,用番茄红素干预3个月后的衰老小鼠模型,其大脑衰老和与年龄相关的认知缺陷得到有效控制,其可能通过激活肝脑FGF21信号传导,从而通过增加线粒体ATP水平和增强突触囊泡融合进而促进神经递质释放[19]。另外,番茄红素可以减弱邻苯二甲酸酯诱导的海马组织CA1和CA3区神经元的无序排列,通过调节铁代谢、抑制脂质过氧化、激活半胱氨酸转运蛋白和核因子E2相关因子2/血红素加氧酶-1(Nrf2/HO-1)信号通路来抑制邻苯二甲酸酯诱导的铁死亡的发生从而减轻小鼠的认知功能障碍[20]。此外,番茄红素可通过其抗氧化特性、激活MAPK/ERK通路和抑制神经元凋亡对大鼠海马双酚A中毒发挥神经保护作用,从而改善其学习和认知记忆能力[21]。本研究进一步在老年人群中发现了较高的番茄红素摄入量与更低的认知功能障碍患病风险相关,这些结果为后续认知功能障碍防控措施制定提供了理论依据。
本研究的研究对象从NHANES数据库中获得,该调查的样本通过复杂的分层多阶段抽样获得,所得样本具有代表性;在调整变量时更加全面地调整了种族、教育水平、家庭收入与贫困比率、吸烟、饮酒状况以及糖尿病和高血压两个疾病变量,减少了混杂因素对结果的影响;在分别探讨了番茄红素摄入量和四个不同维度认知功能障碍之间的关联性之后又探讨了番茄红素摄入量和认知功能综合化评分下认知功能障碍之间的关联性;此外,采用了亚组分析分别探讨了年龄和性别对番茄红素摄入量和认知功能关联的影响。同时,本研究还存在一定的局限性。首先,该研究属于横断面研究,无法推断因果关系;其次,膳食数据获取方式为两次24小时膳食回忆访谈,可能存在一些回忆偏倚;最后,该研究主要依赖于几种常用的认知功能测试,可能无法全面评估老年人的认知能力。因此,后续需要继续开展更多的纵向研究以及更加精确的认知功能评估方法以更进一步探索膳食中番茄红素摄入量与老年人认知功能障碍之间的关系。
