Objective

To construct the trajectories of four kinds of scores in Rey Auditory Verbal Learning Test (RAVLT) and to explore the relationship between each trajectory and the progression of mild cognitive impairment (MCI) to Alzheimer’s disease (AD).

Methods

Data were collected from the Alzheimer’s Disease Neuroimaging Program (ADNI). The research subjects were MCI patients who had been followed up for more than 5 years and completed the RAVLT test. Four kinds of trajectories with different scores of RAVLT were constructed based on the group-based trajectory model (GBTM). Cox regression model was used to explore the relationship between four different score trajectories and the progression of MCI patients to AD.

Results

In the analysis of the four types of RAVLT scores, it was found that three trajectories were identified in the verbal learning scores.Compared with the low-stable group, the risk of AD in the moderate-stable group was lower (adjusted HR=0.27, 95%CI: 0.11-0.71). The delayed recall score identified three trajectories. Compared with the low-stable group, the high-rising group had an increased risk of AD (adjusted HR=3.38, 95%CI: 1.41-8.11).

Conclusion

MCI patients with lower verbal learning scores or higher delayed recall scores have a higher risk of developing AD.

目的

结论

较低言语学习得分或较高延迟回忆得分的MCI患者进展为AD的风险更大。

目的

构建雷伊听觉言语学习测验(RAVLT)四类得分的轨迹,探讨各轨迹与轻度认知障碍(MCI)患者进展为阿尔茨海默症(AD)的关联。

方法

数据来源于阿尔茨海默病神经影像学计划(ADNI),研究对象是随访超过五年并完成RAVLT测试的MCI患者。基于组基轨迹模型(GBTM)构建RAVLT四类不同得分的轨迹。采用Cox回归模型探讨四类不同得分轨迹与MCI患者进展为AD的关联。

结果

在RAVLT四类得分轨迹分析发现:言语学习得分识别出三种轨迹,与低-稳定组相比,中-稳定组AD发生风险降低(调整后HR=0.27, 95%CI: 0.11~0.71)。延迟回忆得分识别出三种轨迹,与低-稳定组相比,高-上升组AD发生风险增加(调整后HR=3.38, 95%CI: 1.41~8.11)。

结论

较低言语学习得分或较高延迟回忆得分的MCI患者进展为AD的风险更大。

Objective

To construct the trajectories of four kinds of scores in Rey Auditory Verbal Learning Test (RAVLT) and to explore the relationship between each trajectory and the progression of mild cognitive impairment (MCI) to Alzheimer’s disease (AD).

Methods

Data were collected from the Alzheimer’s Disease Neuroimaging Program (ADNI). The research subjects were MCI patients who had been followed up for more than 5 years and completed the RAVLT test. Four kinds of trajectories with different scores of RAVLT were constructed based on the group-based trajectory model (GBTM). Cox regression model was used to explore the relationship between four different score trajectories and the progression of MCI patients to AD.

Results

In the analysis of the four types of RAVLT scores, it was found that three trajectories were identified in the verbal learning scores.Compared with the low-stable group, the risk of AD in the moderate-stable group was lower (adjusted HR=0.27, 95%CI: 0.11-0.71). The delayed recall score identified three trajectories. Compared with the low-stable group, the high-rising group had an increased risk of AD (adjusted HR=3.38, 95%CI: 1.41-8.11).

Conclusion

MCI patients with lower verbal learning scores or higher delayed recall scores have a higher risk of developing AD.

阿尔茨海默症(Alzheimer disease, AD)是一种以渐进性记忆丧失和认知障碍为特征的神经退行性疾病[1],它具有不可逆性,是引起痴呆发生的主要原因,占所有痴呆的60%~70%[2]。阿尔茨海默症国际组织(Alzheimer's Disease International,ADI)调查显示:2019年全球共有5 500万人患有痴呆症,而2050年这一数字将达到1.39亿[2]。AD发生的病程很长,可分为临床前阶段,轻度认知障碍(mild cognitive impairment,MCI)和痴呆,对患者、患者家庭和社会造成了巨大的疾病负担[3]。世界卫生组织(World Health Organization,WHO)估计,2019年全球因痴呆症造成的社会成本为1.3万亿美元[4],采取行之有效的措施减轻AD的疾病负担十分必要。虽然AD不可治愈,但是已有研究证实,在AD早期阶段给予干预会延缓疾病的进展[5],而MCI作为AD的前驱症状得到了越来越多的关注。
MCI患者进展为AD的过程中,情景记忆功能会发生变化[6]。情景记忆功能是指一个人以时间和空间为坐标,对自己亲身经历的、发生在一定时间和地点事件的记忆能力。已有研究证明情景记忆功能可用于识别MCI患者,并能有效预测AD的发生[7-8]。雷伊听觉言语学习测验(Rey auditory verbal learning test,RAVLT)是测量受试者情景记忆功能的方法之一[9],该测验可以反应受试者的及时记忆、延迟记忆和言语学习等功能,可以用于识别早期AD患者[10]。现有研究发现,RAVLT中及时回忆得分和延迟回忆得分有助于识别几年后进展为AD的患有主观记忆疾病的个体[11-13]。以上研究均是基于受试者的单次测量数据。事实上,MCI患者的情景记忆功能会随着时间变化,因此,探讨情景记忆功能随时间变化的轨迹对预测MCI患者是否会进展为AD十分必要。因此,本研究利用阿尔茨海默病神经影像学计划(Alzheimer’s Disease Neuroimaging Initiative,ADNI)中受试者2004—2021年中连续三年的RAVLT测量结果,确定不同受试者的得分轨迹,并探讨不同轨迹亚组与MCI患者进展为AD的关联。
研究对象来自ADNI。ADNI是由首席研究员迈克尔-韦纳(Michael W. Weiner)领导的一项公私合作项目。该项目于2003年启动,是一项多中心纵向研究,旨在利用临床、影像、遗传和生物标志物等信息,检测认知功能状态并追踪AD的病理过程。迄今为止,一共建立四个队列:ADNI-1、ADNI-GO、ADNI-2和ADNI-3。ADNI的开展得到了各研究机构审查委员会的批准,所有受试者都签署了知情同意书。本研究以随访了超过五年的MCI患者作为研究对象。排除基线时年龄、性别和受教育时间缺失的受试者,排除RAVLT中各项得分缺失的受试者,最后排除随访结局为认知正常的受试者,最终纳入117人,见图1
RAVLT根据不同的语言有很多不同的版本,ADNI使用的是Rey等人在1964年提出的版本。在测验中,主考官连续五次(试验1~5)大声朗读15个词语(每个词语之间有1 s的间隔)。每次试验中,词语的朗读顺序保持不变,每次试验后让受试者重复读出听到的词,统计受试者每次试验读出正确词语的数量。在完成试验5的30 min后,让每个受试者回忆之前的15个词语(试验6),统计受试者回忆正确的词语数量。完成试验6后,给受试者铅笔和写有朗读词语的列表,让受试者圈出在试验1~5中出现的词语,统计圈出正确词语数量。
根据RAVLT结果计算每个受试者的得分[14]:(1)即时回忆得分:试验1~5中所有重复正确词语数的总和;(2)言语学习得分:在单词列表中圈出的正确词语数的总和;(3)言语遗忘得分:在试验5和6中正确回忆的词语数量的差异;(4)延迟回忆得分:(言语遗忘得分/及时回忆得分)*100%。
本研究基于组轨迹模型(group-based trajectory modelling,GBTM)构建RAVLT中四类不同得分随时间变化的轨迹。GBTM是有限混合模型的一种特殊形式,能够探索总体中轨迹亚组的数量并且确定各亚组的发展轨迹[15]。本研究通过比较不同亚组数(1~6组)和不同轨迹形状(线性、二次方和三次方)的模型,根据模型评价标准,选出最优模型。模型评价指标有:(1)贝叶斯信息准则(Bayesian information criterion,BIC)和赤池信息准则(Akaike information criterion,AIC),两者越接近于0说明模型拟合效果越好;(2)各轨迹亚组平均后验概率>0.7表示模型拟合良好;(3)每个轨迹亚组中的受试者数量要大于总样本量的5%[16-17]。秩和检验和χ2检验比较不同轨迹人群的基本特征差异。
使用Cox比例风险回归模型探讨四类得分轨迹亚组与MCI患者进展为AD的关联。根据RAVLT四类得分构建Cox回归模型,针对每种得分轨迹分别构建2个Cox回归模型,其中模型1未调整任何协变量,模型2调整了年龄、性别和受教育时间。数据分析采用Stata 17.0和R 4.3.1,双侧检验,检验水准α=0.05。
本研究共纳入研究对象117人,年龄为(72.84±7.08)岁,其中男性75人,占64.10%。本研究中,及时回忆得分和言语学习得分越高,说明受试者的情景记忆功能越好;言语遗忘得分和延迟回忆得分越高,说明受试者的情景记忆功能越差。基线时各变量的分布见表1
本研究根据RAVLT四类得分拟合了不同的轨迹模型,并根据评价指标从中筛出最优模型。我们发现及时回忆得分轨迹亚组数为4时的BIC最低(BIC=-1 637.88),此时每个亚组均符合平均后验概率>0.7,估计组成员大于总样本量的5%标准。四组分别为:低-稳定组(n=35,29.92%),中-下降组(n=52,44.44%),中-稳定组(n=23,19.66%)和高-稳定组(n=7,5.98%),见图2。言语学习能力得分的轨迹亚组数为3时的BIC最低(BIC=-1 066.33),此时每个亚组均符合平均后验概率>0.7,估计组成员数大于总样本量的5%标准。三组分别为:低-稳定组(n=59,50.43%),中-稳定组(n=46,39.32%)和高-稳定组(n=12,10.25%),见图3。当言语遗忘得分轨迹亚组数为6时BIC最低(BIC=-1 043.27),并且每个亚组均符合平均后验概率>0.7的标准,但是有两组的估计组成员数只占总样本量的0.93%,远小于5%,经过筛选最终确定亚组数为3时模型最优。三组分别为:低-稳定组(n=12,10.26%),中-上升组(n=56,47.86%)和高-稳定组(n=49,41.88%),见图4。延迟回忆得分的轨迹亚组数为3时模型的BIC最低(BIC= -1 028.28),并且每个亚组的平均后验概率>0.7,估计组成员数大于总样本量的5%。三组分别为:低-稳定组(n=44,37.61%),中-上升组(n=47,40.17%)和高-上升组(n=26,22.22%),见图5表2显示了RAVLT不同得轨迹亚组的基线特征比较。四类得分轨迹亚组之间的及时回忆得分、言语学习得分、言语遗忘得分和延迟回忆得分的差异都有统计学意义。老年男性的及时回忆得分更低而言语遗忘得分更高。这两种得分轨迹亚组之间的受教育时间的差异没有统计学意义。言语学习得分和延迟回忆得分轨迹亚组之间的年龄、性别和受教育时间的差异并没有统计学意义。
117名MCI患者中,共有19人从MCI进展为AD。表3展示了RAVLT四类得分的轨迹亚组与AD发生风险的关联。观察受试者的言语学习得分发现,与低-稳定组相比,中-稳定组与AD发生的风险减少有关(HR=0.27,95%CI 0.11~0.71)。观察受试者的延迟回忆得分发现,与低-稳定组相比,高-上升组与AD发生的风险增加有关(HR=3.38,95%CI 1.41~8.81)。受试者的及时回忆得分与言语遗忘得分不同的轨迹亚组和AD发生的风险没有关联。
本研究确定了RAVLT中及时回忆得分(低-稳定组、中-下降组、中-稳定组和高-稳定组)、言语学习得分(低-稳定组、中-稳定组和高-稳定组)、言语遗忘得分(低-稳定组、中-上升组和高-稳定组)和延迟回忆得分(低-稳定组、中-上升组和高-上升组)的轨迹;发现较低的言语学习得分或较高的延迟回忆得分与AD发生的风险增加有关,及时回忆得分与言语遗忘得分的高低与AD发生的风险无关。
本研究发现,RAVLT及时回忆得分和言语遗忘得分各轨迹亚组中,老年男性的及时回忆得分更低,言语遗忘得分更高,这表明老年男性的情景记忆功能更差,与之前的研究结论是一致的[18-19]。四类得分的轨迹亚组之间受教育时间的差异没有统计学意义,这可能因为受教育时间是一种测量受试者认知储备的手段,但是认知储备并不能直接反应受试者的情景记忆功能,而与受试者的大脑萎缩率有关,有研究表明只有大脑萎缩率较低时,认知储备才能反应受试者的情景记忆功能[20]
此外,较低的言语学习得分或较高的延迟回忆得分的MCI患者进展为AD的风险更大,这与之前的研究结论是一致的[21]。本研究发现,AD发生风险高的MCI患者的言语学习能力和延迟回忆能力的降低在被诊断为AD的前几年就已经开始,这表明言语学习得分和及时回忆得分可以预测AD的发生。已经有研究表明,使用延迟回忆得分预测MCI患者进展为AD有很高的准确性[22]。同时,Estévez等人[11]的研究发现,及时回忆得分同样是预测AD发生的重要指标,但是本研究并没有得到相同的结论。这可能是因为及时回忆功能不会受到时间变化的影响,而是与受试者的认知负荷以及是否受到干扰有关[23-24]。本研究受试者的及时回忆得分轨迹是通过观察受试者不同时间点的测量结果拟合的,所以没有发现及时回忆得分的不同轨迹亚组与MCI患者进展为AD风险的关联。
本研究在纳入协变量时,发现除年龄外,受试者的性别和受教育时间与不同得分轨迹亚组MCI患者进展为AD的关联并没有统计学意义。然而,事实上女性比男性更容易患上AD;受教育时间也与AD的发生有着重要关联[25-26]。因此,本研究最终将性别和受教育时间作为协变量纳入了模型,这样可以减少未知混杂对模型的影响,让结论更加可靠。而出现以上现象可能是因为本研究的样本量较少,扩大样本量能得到更加稳健的结论。
RAVLT言语学习得分的轨迹表明,并不是得分越高的MCI患者进展为AD的风险越低,与低-稳定组相比,中-稳定组的MCI患者进展为AD的风险是降低的,但是高-稳定组与MCI患者是否会进展为AD并不相关;延迟回忆得分也发现了相同的结果。同时我们发现,在及时回忆得分与言语学习得分的轨迹中,随访第二年时,部分亚组的得分有相较于前一年明显上升的趋势,在言语遗忘得分和延迟回忆得分的轨迹中存在随访第三年相较于第二年分数明显下降的情况。出现以上现象与MCI患者的病理过程有关。因为,MCI阶段患者具有可逆性,她/他并不一定会进展为AD,大部分MCI患者会一直处于这一疾病阶段[27];所以MCI患者的情景记忆功能并不一定会随着年龄的增长而下降。同时,由于MCI患者情景记忆功能的变化受到众多因素的影响,例如患者所处的环境,疾病严重程度以及生活方式等,所以MCI患者情景记忆功能的轨迹是上下波动的。在本研究中,虽然四类得分的轨迹并不是单纯上升或下降的趋势,但是轨迹上升或者下降的幅度并不大,四类得分的差异都在1分之内(及时回忆得分是五项测验得分的总和,其波动也没有超过平均每项试验1分)。
本研究对RAVLT中四类得分的三年发展轨迹进行了研究,探讨了四类得分的不同轨迹亚组与MCI患者进展为AD的关联。相比于使用横断面数据,这样的方法可以更加准确区分容易发生AD风险的MCI患者。通过这一方法可以识别高风险的群体,可能有助于高风险人群的有效预防和早期治疗。本研究也有一些局限性。首先,我们使用的是GBTM模型,没有考虑到每个类别中不同个体的异质性,这可能意味着我们低估模型的方差。然而,考虑到四类得分各轨迹亚组的平均后验概率普遍较高,这不太可能影响本研究的结论。其次,我们的样本量较小并且仅基于ADNI数据库,这可能会影响轨迹模型的外推性。将来可以扩大样本量并且在多个队列中进行验证,以确保模型的稳健性。最后,一些相关的特征并没有被纳入到本研究的模型中,例如受试者的一些日常活动能力或者社交的相关情况等,如果纳入这些变量会更好的理解或者解释观察到的轨迹及其与AD发生的关联。
总之,本研究通过多次随访的RAVLT结果确定了四类得分的轨迹,探讨了各轨迹亚组与MCI患者进展为AD的相关性。结果表明较低的言语学习得分或者较高的延迟回忆得分与AD发生风险增加有关。未来的研究需要深入了解这些关联的生物学机制,以及评估使用RAVLT四类得分的轨迹作为筛查的方法识别有AD风险的MCI患者的潜力。
