Objective To explore the research hotspots and development trends of health management (physical examination)centers, providing reference and ideas for further research in health management (physical examination) centers in China. Methods A total of 7 516 articles published by health management (physical examination) centers and included in the China National Knowledge Infrastructure (CNKI) and PubMed databases were analyzed using CiteSpace software to analyze and visualize the knowledge map of keywords and publishing institutions. Results Since 1995, the number of research articles from health management (physical examination) centers had been increasing, with the highest numbers of published Chinese and English articles in the journals “Chinese Journal of Health Management” (164 articles) and “Medicine” (112 articles), respectively. The most frequen keywords in Chinese and English articles were influencing factors (242 times) and risk factors (113 times), and the research hotspots of health management (physical examination) centers also focused on cardiovascular metabolic diseases and common malignant tumors. “Elderly people,” “health management,” and “intestinal flora” were emerging persistent keywords until 2023. The density of the network map of collaboration among Chinese research institutions was relatively low, with few connections between institutions. Conclusion The number of research articles from health management (physical examination) centers in China has been increasing year by year, and research on health management, intestinal flora, and related topics is at the forefront of research in health management (physical examination) centers, with elderly people as the main research subjects.These studies will provide epidemiological theory and practical experience for the prevention and control of chronic diseases, but further multi-center and interdisciplinary cooperation and comprehensive implementation of medical service charges for health management are needed to promote the long-term development of health management.

目的 探究健康管理（体检）中心科研热点及其发展趋势，为我国健康管理（体检）中心开展进一步研究提供参考和思路。方法 检索中国知网（CNKI）和PubMed数据库中收录的健康管理（体检）中心所发表的中文、英文文章，共7 516篇文章被纳入分析，采用CiteSpace软件对关键词、发文机构进行分析并绘制知识图谱。结果 自1995年起健康管理（体检）中心科研文章发表量呈增长趋势，所发表的中文和英文文章刊登量最高的期刊分别为《中华健康管理学杂志》（164篇）和Medicine（112篇）。中文和英文文章关键词出现频次最高的分别为影响因素（242次）和risk factors（113次），此外健康管理（体检）中心研究热点还聚焦于心血管代谢性疾病和常见恶性肿瘤。“老年人”“健康管理”“肠道菌群”等是持续到2023年的突现关键词。中文科研文章机构合作网络图谱密度较低，机构之间联系较少。结论 我国健康管理（体检）中心科研文章发表量逐年增加，健康管理、肠道菌群等相关研究是健康管理（体检）中心的研究前沿，老年人是主要研究对象。这些研究将为慢性病防控工作提供大量流行病学理论和实践经验，但未来仍需通过进一步地多中心和跨学科合作以及全面实现健康管理医学服务收费以促进健康管理学的长足发展。

目的 探究健康管理（体检）中心科研热点及其发展趋势，为我国健康管理（体检）中心开展进一步研究提供参考和思路。方法 检索中国知网（CNKI）和PubMed数据库中收录的健康管理（体检）中心所发表的中文、英文文章，共7 516篇文章被纳入分析，采用CiteSpace软件对关键词、发文机构进行分析并绘制知识图谱。结果 自1995年起健康管理（体检）中心科研文章发表量呈增长趋势，所发表的中文和英文文章刊登量最高的期刊分别为《中华健康管理学杂志》（164篇）和Medicine（112篇）。中文和英文文章关键词出现频次最高的分别为影响因素（242次）和risk factors（113次），此外健康管理（体检）中心研究热点还聚焦于心血管代谢性疾病和常见恶性肿瘤。“老年人”“健康管理”“肠道菌群”等是持续到2023年的突现关键词。中文科研文章机构合作网络图谱密度较低，机构之间联系较少。结论 我国健康管理（体检）中心科研文章发表量逐年增加，健康管理、肠道菌群等相关研究是健康管理（体检）中心的研究前沿，老年人是主要研究对象。这些研究将为慢性病防控工作提供大量流行病学理论和实践经验，但未来仍需通过进一步地多中心和跨学科合作以及全面实现健康管理医学服务收费以促进健康管理学的长足发展。

Objective To explore the research hotspots and development trends of health management (physical examination)centers, providing reference and ideas for further research in health management (physical examination) centers in China. Methods A total of 7 516 articles published by health management (physical examination) centers and included in the China National Knowledge Infrastructure (CNKI) and PubMed databases were analyzed using CiteSpace software to analyze and visualize the knowledge map of keywords and publishing institutions. Results Since 1995, the number of research articles from health management (physical examination) centers had been increasing, with the highest numbers of published Chinese and English articles in the journals “Chinese Journal of Health Management” (164 articles) and “Medicine” (112 articles), respectively. The most frequen keywords in Chinese and English articles were influencing factors (242 times) and risk factors (113 times), and the research hotspots of health management (physical examination) centers also focused on cardiovascular metabolic diseases and common malignant tumors. “Elderly people,” “health management,” and “intestinal flora” were emerging persistent keywords until 2023. The density of the network map of collaboration among Chinese research institutions was relatively low, with few connections between institutions. Conclusion The number of research articles from health management (physical examination) centers in China has been increasing year by year, and research on health management, intestinal flora, and related topics is at the forefront of research in health management (physical examination) centers, with elderly people as the main research subjects.These studies will provide epidemiological theory and practical experience for the prevention and control of chronic diseases, but further multi-center and interdisciplinary cooperation and comprehensive implementation of medical service charges for health management are needed to promote the long-term development of health management.

20世纪中期，美国不断增长的医疗费用促使美国保险业率先提出健康管理的概念，他们通过与医疗服务机构合作、医生指导病人自我保健，使医疗费用大幅降低[1-2]。随着我国现代化进程的推进以及老龄化加剧，慢性非传染性疾病成为威胁居民健康的主要疾病，传统以疾病为中心的诊疗模式难以应对持续上涨的医疗卫生需求和费用，因此20世纪末，健康管理理念被引进中国，并促进了一个医学新学科——健康管理学的产生。2018年，健康管理首次进入复旦大学医院管理研究所“医院科室排名”，极大地推进了健康管理学科的发展，同时提升了健康管理科室在医院与行业中的地位[3]。在医院全面建设进程中，健康管理已经成为一门重要学科，健康管理科室建设必须以学科建设为抓手，才能实现健康管理的高质量发展[4]。而科学研究是健康管理（体检）中心学科建设与发展的源动力[5]。因此，本研究运用CiteSpace软件对健康管理（体检）中心发表的文献进行计量分析，探究其所聚焦的研究内容与研究趋势，为健康管理（体检）中心的科研工作提供一定的参考。
1　资料与方法

1.1　数据来源
本研究文献计量数据来源于中国知网（China National Knowledge Infrastructure，CNKI）和PubMed数据库，检索数据库建库至2023年9月6日且作者单位为健康管理（体检）中心的中英文文章。中文文章的检索字段为“健康管理中心、体检中心、健康体检中心、健康管理科、体检科、健康医学科、健康医学部、健康管理研究院”，英文文章的检索字段为中文文章检索字段的英语翻译。在CNKI数据库中获得核心期刊文章3 012篇，以Refworks格式导出，排除重复文章或缺少CiteSpace软件分析所需字段的文章18篇，最终获得2 994篇中文文章。在PubMed数据库中检索获得4 660篇文章，以txt格式导出，排除文章类型为“Letter”或“Editorial”的文章81篇后，获得可纳入分析的英文文章4 522篇。最终合计7 516篇文章被纳入分析。
1.2　数据处理
对最终纳入分析文章的关键词和机构数据进行处理，主要包括关键词或机构的合并、替换、修改等。关键词处理规则：合并同义词，统一使用频次最高的关键词；为了更加全面细致地挖掘健康管理（体检）中心的研究内容，不合并上位词与下位词，如不将“非酒精性脂肪性肝病”与“脂肪肝”合并。机构处理规则：将机构名称保留至二级机构，若导出的题录缺乏二级机构信息，则通过人工查找原文进行补全；将机构的曾用名称替换为现用名称；若同一机构出现多个名称，则保留出现频次最高的名称。
1.3　数据分析
本研究使用由陈超美[6]教授团队开发的CiteSpace Version 6.1 R4绘制知识图谱。在CiteSpace中，时间跨度设置为1995年1月—2023年9月，单个时间分区程度（time slicing）为2年，选择标准设置为Top N=50，节点类型根据研究目的分别设置为关键词和机构，其余设置缺省。模块值（modularity Q，简称Q值）和平均轮廓值（silhouette，简称S值）是评判知识图谱绘制效果的两个指标，Q>0.3意味着聚类结构显著，S>0.7表示聚类结果令人信服。中介中心性（betweenness centrality）描绘了图谱中节点所处位置的重要性，通常与其他节点高度相连的枢纽节点或位于不同聚类之间的节点具有较高的中介中心性，本研究将图谱中具有较大中介中心性（不小于0.1）的节点边缘标注为紫色。利用Burst检验分析节点的突现时间及突现强度，强度越高越能反映某突现时间段内该领域的研究前沿和发展趋势。利用NoteExpress软件的数据分析功能获得年度发文量及期刊分布数据，将其录入Excel软件中进行整理分析。
2　结果

2.1　年度发文情况与期刊分布
如图1所示，健康管理（体检）中心科研文章发表数量总体呈增长趋势，1995—2006年中文发文量增势缓慢，2007年开始增长迅速，2022年发文量最高为321篇。健康管理（体检）中心自2009年发表英文文章以来，年度发文量快速增长，并在2017年赶超中文文章数量，2022年发文量突破1 000篇。健康管理（体检）中心所发表的中文文章刊登在336种核心期刊上，其中刊登量排前三名的依次为《中华健康管理学杂志》164篇、《中国老年学杂志》142篇、《中国全科医学》119篇；发表的英文文章刊登量排前三名的期刊均为综合性医学期刊，依次为Medicine112篇、Frontiers in public health107篇、Scientific reports90篇。
2.2　关键词共现与聚类分析
节点类型选择关键词，得到关键词共现和聚类图谱，见图2。中文和英文文章分别得到节点数N=703、N=682的关键词共线网络，表1详细列出了健康管理（体检）中心中文和英文文章关键词出现频次分别居前15位或中介中心性≥0.1的关键词，中文文章中出现频次最高的关键词为“影响因素”，其次为“2型糖尿病”“高血压”等，具有较大中介中心性的关键词包括“影响因素”“健康体检”“2型糖尿病”“高血压”。英文文章出现频次排前三位的关键词为“risk factors”“type 2 diabetes”“metabolic syndrome”，共有5个具有较大中介中心性的关键词，分别为“type 2 diabetes”“risk factors”“lung cancer”“cohort study”“body mass index”。对中文文章关键词共现网络聚类分析后得到94个聚类团，Q值为0.599 4，平均S值为0.887 7；英文文章得到139个聚类团，Q值为0.770 4，平均S值为0.927 5。中文和英文文章关键词聚类结构均较为显著、可信度高，并且最大的聚类团均为“代谢综合征”。与此同时，中英文文章的聚类团均包括“老年人”，但从两个聚类团包含的节点来看，中文文章关注老年人的躯体疾病如骨质疏松，而英文文章侧重老年人的心理状况，如抑郁症状、社会支持等。此外，乳腺癌、肺癌等常见恶性肿瘤也是英文文章的研究热点。
2.3　关键词突现分析
健康管理（体检）中心中文科研文章突现关键词的突现持续周期为2~9年，“流行病学”是突现强度最高且持续周期最长的关键词；英文突现关键词的突现持续周期为2~4年，“gut microbiota”是突现强度最高且持续到2023年的关键词。“老年人”（older adults）、“健康管理”（health management）是中文与英文文章共同的持续到2023年的突现关键词。见图3。
2.4　机构分析
选择研究机构为节点，绘制健康管理（体检）中心中文科研文章的机构合作网络图谱，共得到660个节点，513条连线，网络密度为0.002 4，图4标记了出现频次大于20的健康管理（体检）中心，它们之间的连线数量较少，说明各个中心之间的合作交流较少。
3　讨论

自1995年起健康管理（体检）中心发文量总体呈增长趋势，中文发文量从2007年开始迅速增加，随后英文文章数量也快速增长。这可能与2007年中华医学会健康管理学分会的成立密不可分，学会从学术理论研究与学科建设、产业及行业进步等方面积极引领和大力推动我国健康管理的规范有序发展，极大地促进了我国健康管理领域学术成果的产出。但从健康管理（体检）中心发表文章主要刊登期刊来看，与健康管理学科密切相关的专科期刊只有《中华健康管理学杂志》一本，相较于拥有若干本期刊推广其学科理论及实证研究的其他医学专科，健康管理学的发展任重而道远。另外，本研究结果显示健康管理（体检）中心在科研上彼此之间缺乏协作，同时我国诸多政策呼吁通过跨学科研究以实现科学研究由高度分化转向综合发展，从而促进知识创新和社会进步[7]。因此，如何开展多中心、多学科合作研究，如何构建稳定、紧密的机构合作网络结构是健康管理（体检）中心接续发展所面临的重要议题。
关键词共现与聚类分析结果显示，我国健康管理（体检）中心的科研热点主要聚焦于代谢综合征、糖尿病等心血管代谢性疾病以及肺癌、乳腺癌等常见恶性肿瘤。过去三十年以来，这两大类慢性疾病给我国居民造成了严重的健康负担和经济负担[8]。为响应《“健康中国2030”规划纲要》提出的坚持预防为主的卫生健康方针，健康管理（体检）中心与其他临床专科合作发布了有关这两类疾病的多部早期筛查与风险评估指南，如《中国心血管病风险评估和管理指南》[9]《糖尿病高危人群筛查及干预专家共识》[10]《中国早期结直肠癌筛查流程专家共识意见（2019，上海）》等[11]，一方面为我国慢病防控工作贡献了重要力量，另一方面也促进了自身发展。兼顾关键词中介中心度时发现影响因素/风险因素是健康管理（体检）中心科研领域的关键节点，通过这些研究，有助于早期识别疾病高危人群并进行及时干预。如使用数据挖掘法识别非酒精性脂肪性肝病风险因素，从而筛查高危人群并采取有针对性的措施对其进行预防[12]。
2020年，白书忠教授在《我国健康管理学的发展现状与展望》一文中提出：健康管理（体检）机构建设要努力实现从单纯健康体检服务向健康管理服务转变[3]。虽然关键词突现分析结果提示近几年健康管理（体检）中心发表报道健康管理的文章增多，但解决健康管理从科研层面落地实施困难的关键在于承认健康管理医学行为的专业性和价值，即实现健康管理医学服务收费[13]。然而当前仅四川、河北两省实现了健康管理医学服务收费，仍需继续努力推进其在余下各省的落地。近年来，越来越多研究发现肠道菌群除了参与维持机体正常生理功能，还与心血管疾病、代谢性疾病、恶性肿瘤等多种疾病的发生和转归息息相关[14-16]。一些健康管理（体检）中心也在相关研究中发现了肠道菌群对于疾病风险分层和精准管理以及治疗的潜在作用，可见当前乃至未来一段时期开展有关肠道菌群的研究将为疾病早期筛查、评估及干预提供新的思路[17-18]。全国第七次人口普查数据显示，我国65岁及以上老年人占总人口的比例较20年前增加6.5%，人口老龄化形式日益严峻，由此导致慢性病负担也不断加重。因此未来较长一段时期，老年人始终是健康管理（体检）中心的重要研究对象。
本研究将健康管理科室所发表的所有中英文文章纳入分析，以期全面呈现健康管理（体检）中心科研内容，利用CiteSpace这一文献可视化软件来探究健康管理（体检）中心科研热点与发展趋势，为今后健康管理（体检）中心科研提供思路与方向。由于PubMed数据库导出的英文文章缺乏可用的发文机构信息，导致不能分析健康管理（体检）中心英文文章科研合作情况，这也是本文的局限所在。总而言之，健康管理（体检）中心科研文章发表数量呈增长趋势，并且紧随国家政策与居民健康需求，预计老年人、健康管理、肠道菌群将是健康管理（体检）中心科研的重点报道内容。为了实现健康管理学科的长远发展目标，健康管理（体检）中心还应积极开展多中心和跨学科科研，并争取早日全面实现健康管理医学服务收费。
