Objective To understand the epidemic characteristics of diabetic nephropathy in China and to provide scientific basis for the prevention and treatment of diabetic nephropathy. Methods Data from the Global Burden of Disease (GBD) database in 2019 was selected to collect data on the incidence of diabetic nephropathy in China from 1990 to 2019. The characteristics and trends of standardized incidence rates and age-specific incidence rates in each year were described, and the Joinpoint regression model was used to analyze the temporal trend of the incidence rate of diabetic nephropathy in China. Results There was no turning point in the trend of age-standardized prevalence of type 1 diabetic nephropathy in the whole population and women in China, and the annual percent change (APC) equaled the average annual percent change (AAPC), showing an upward trend. The average annual growth rate of the whole population was 1.02% (95%CI: 0.87%-1.17%), and that of women as a whole was 1.21% (95%CI: 1.08%-1.35%). The turning point of male trend was in 1994, 2011, and 2017, with an overall average annual growth of 0.56% (95%CI: 0.20%-0.92%). The incidence rates for age groups under 20 years old showed a decreasing trend (AAPC=-0.71% to -0.12%), while those aged 20 and above showed an increasing trend (AAPC=1.06% to 3.99%), all with statistical significance (P<0.05). The standardized incidence rates of type 2 diabetic nephropathy for the whole population, males, and females in China had turning points in 1995, 2000, 2005, 2010, and 2015, with an overall annual decrease of 0.15%(95%CI: -0.17% to -0.12%) for the whole population, 0.12% for males (95%CI: -0.16% to -0.09%), and 0.18% for females(95%CI: -0.22% to -0.15%). Conclusion The epidemic trend of diabetic nephropathy in China from 1990 to 2019 is still grim, and more attention should be paid to diabetes and its complications. In particular, the prevalence rate of women has been higher than that of men, and the prevalence rate of type 2 diabetic nephropathy in the age group of 15-39 years old, 55-59 years old, and ≥ 75 years old, as well as the age group of ≥ 20 years old with the increasing prevalence of type 1 diabetic nephropathy.Strategies and measures should be promptly implemented to address these issues.

目的 了解中国糖尿病肾病患病流行特征,为糖尿病肾病的防治策略和措施提供科学依据。方法 选取全球健康数据交换数据库GBD 2019年的数据,收集1990—2019年中国糖尿病肾病患病数据,描述各年份标准化患病率、年龄别患病率的特征及趋势,利用Joinpoint回归模型分析中国糖尿病肾病患病率的时间变化趋势。结果 中国1型糖尿病肾病全人群和女性年龄标准化患病率趋势变化没有转折点,APC=AAPC,均呈上升趋势,全人群整体年均增长1.02%(95%CI:0.87%~1.17%),女性整体年均增长1.21%(95%CI:1.08%~1.35%);男性趋势变化转折点为1994、2011和2017年,整体年均增长0.56%(95%CI:0.20%~0.92%);年龄别<20岁年龄组患病率呈下降趋势(AAPC=-0.71%~-0.12%),≥20岁年龄组呈上升趋势(AAPC=1.06%~3.99%),差异均具有统计学意义(P<0.05)。中国2型糖尿病肾病全人群、男性和女性年龄标准化患病率趋势变化转折点均为1995、2000、2005、2010和2015年,全人群整体年均下降0.15%(95%CI:-0.17%~-0.12%),男性整体年均下降0.12%(95%CI:-0.16%~-0.09%),女性整体年均下降0.18%(95%CI:-0.22%~-0.15%)。年龄别15~19岁、55~59岁、≥75岁以上年龄组患病率在1990—2019年间呈上升趋势(AAPC=0.05%~0.30%),20~39岁各年龄组呈下降趋势(AAPC=-1.59%~-0.36%),差异均具有统计学意义(P<0.05)。中国2型糖尿病肾病其他年龄组的变化差异无统计学意义(P>0.05)。结论 1990—2019年中国糖尿病肾病流行趋势依然严峻,应对糖尿病及其并发症加大重视。尤其是全面关注女性患病率一直比男性高、2型糖尿病肾病患病率在15~39岁、55~59岁、≥75岁群体持续增长以及1型糖尿病肾病患病率在≥20岁的年龄组逐渐增长的人群,及时调整相应的对策。

目的 了解中国糖尿病肾病患病流行特征,为糖尿病肾病的防治策略和措施提供科学依据。方法 选取全球健康数据交换数据库GBD 2019年的数据,收集1990—2019年中国糖尿病肾病患病数据,描述各年份标准化患病率、年龄别患病率的特征及趋势,利用Joinpoint回归模型分析中国糖尿病肾病患病率的时间变化趋势。结果 中国1型糖尿病肾病全人群和女性年龄标准化患病率趋势变化没有转折点,APC=AAPC,均呈上升趋势,全人群整体年均增长1.02%(95%CI:0.87%~1.17%),女性整体年均增长1.21%(95%CI:1.08%~1.35%);男性趋势变化转折点为1994、2011和2017年,整体年均增长0.56%(95%CI:0.20%~0.92%);年龄别<20岁年龄组患病率呈下降趋势(AAPC=-0.71%~-0.12%),≥20岁年龄组呈上升趋势(AAPC=1.06%~3.99%),差异均具有统计学意义(P<0.05)。中国2型糖尿病肾病全人群、男性和女性年龄标准化患病率趋势变化转折点均为1995、2000、2005、2010和2015年,全人群整体年均下降0.15%(95%CI:-0.17%~-0.12%),男性整体年均下降0.12%(95%CI:-0.16%~-0.09%),女性整体年均下降0.18%(95%CI:-0.22%~-0.15%)。年龄别15~19岁、55~59岁、≥75岁以上年龄组患病率在1990—2019年间呈上升趋势(AAPC=0.05%~0.30%),20~39岁各年龄组呈下降趋势(AAPC=-1.59%~-0.36%),差异均具有统计学意义(P<0.05)。中国2型糖尿病肾病其他年龄组的变化差异无统计学意义(P>0.05)。结论 1990—2019年中国糖尿病肾病流行趋势依然严峻,应对糖尿病及其并发症加大重视。尤其是全面关注女性患病率一直比男性高、2型糖尿病肾病患病率在15~39岁、55~59岁、≥75岁群体持续增长以及1型糖尿病肾病患病率在≥20岁的年龄组逐渐增长的人群,及时调整相应的对策。

Objective To understand the epidemic characteristics of diabetic nephropathy in China and to provide scientific basis for the prevention and treatment of diabetic nephropathy. Methods Data from the Global Burden of Disease (GBD) database in 2019 was selected to collect data on the incidence of diabetic nephropathy in China from 1990 to 2019. The characteristics and trends of standardized incidence rates and age-specific incidence rates in each year were described, and the Joinpoint regression model was used to analyze the temporal trend of the incidence rate of diabetic nephropathy in China. Results There was no turning point in the trend of age-standardized prevalence of type 1 diabetic nephropathy in the whole population and women in China, and the annual percent change (APC) equaled the average annual percent change (AAPC), showing an upward trend. The average annual growth rate of the whole population was 1.02% (95%CI: 0.87%-1.17%), and that of women as a whole was 1.21% (95%CI: 1.08%-1.35%). The turning point of male trend was in 1994, 2011, and 2017, with an overall average annual growth of 0.56% (95%CI: 0.20%-0.92%). The incidence rates for age groups under 20 years old showed a decreasing trend (AAPC=-0.71% to -0.12%), while those aged 20 and above showed an increasing trend (AAPC=1.06% to 3.99%), all with statistical significance (P<0.05). The standardized incidence rates of type 2 diabetic nephropathy for the whole population, males, and females in China had turning points in 1995, 2000, 2005, 2010, and 2015, with an overall annual decrease of 0.15%(95%CI: -0.17% to -0.12%) for the whole population, 0.12% for males (95%CI: -0.16% to -0.09%), and 0.18% for females(95%CI: -0.22% to -0.15%). Conclusion The epidemic trend of diabetic nephropathy in China from 1990 to 2019 is still grim, and more attention should be paid to diabetes and its complications. In particular, the prevalence rate of women has been higher than that of men, and the prevalence rate of type 2 diabetic nephropathy in the age group of 15-39 years old, 55-59 years old, and ≥ 75 years old, as well as the age group of ≥ 20 years old with the increasing prevalence of type 1 diabetic nephropathy.Strategies and measures should be promptly implemented to address these issues.

糖尿病肾病(diabetic kidney discase,DKD)是最常见的糖尿病慢性微血管并发症,是慢性肾脏病(chronic kidney disease,CKD)以及终末期肾病(end-stagerenal disease,ESRD)的最主要原因之一[1]。DKD包括1型DKD和2型DKD。根据国际糖尿病联盟(International Diabetes Federation,IDF)的公布,2021年中国糖尿病患病人数达1.409亿,居全球首位[2]。其中近三分之一的1型糖尿病(T1DM)患者和约二分之一的2型糖尿病(T2DM)患者会发生DKD[3]。而在中国、美国和印度等多个国家和地区,DKD已成为CKD首要病因,给社会带来了沉重的经济负担[4]。传统时间趋势分析对描述局部数据的特定变化趋势存在一定局限,早些年有学者提出的Joinpoint回归模型很好地解决了该问题,近年来逐渐应用于慢性病、传染病等的时间变化趋势[5-7]。目前,缺少对中国DKD患病率的时间趋势分析,因此,本研究采用Joinpoint回归模型对1990—2019中国DKD患病率的时间变化趋势进行分析,为DKD的防治策略和措施提供科学依据。
数据来源主要是全球健康数据交换数据库(http://ghdx.healthdata.org/)获取并下载数据[8]。GBD 2019研究中的世界标准人口构成被用来对DKD的患病率进行标准化[9]。疾病诊断基于《国际疾病分类》第10版(ICD-10)[10]。本研究主要数据包括1990—2019年的中国1型DKD和2型DKD各年份标准化患病率、年龄别患病率。
采用Excel 2016将GBD 2019获取并下载中国DKD数据按照模型和相关软件要求进行整理并建立CSV数据文件,将其中为“0”的值用“0.5”替代。利用率和统计图表等对1990—2019中国DKD不同性别和各年龄组间的标准化患病率、年龄别患病率等指标进行统计描述,年龄分组以5岁为一个年龄段,共选取20个年龄段(0~4岁,…,≥95岁);运用Joinpoint Regression(JPR)5.0.0.0软件,用中国DKD的标准化患病率、年龄别患病率作为因变量,年份作为自变量构建对数线性模型,计算回归系数、95%置信区间、年度变化百分比(annual percent change,APC)、平均年度变化百分比(average annual percent change,AAPC),并进行趋势 t 检验[11]。APC可描述患病水平在各段时间内的变化趋势,AAPC表示整段时间的变化趋势。当APC>0时,表示结局事件发生概率为增长趋势;当APC<0时,代表结局事件发生概率是下降趋势;若无连接点,则APC=AAPC,表示数据总体呈单调下降或上升趋势[12]。检验水准α=0.05。
1990—2019年中国DKD粗患病率呈增长趋势,女性粗患病率比男性高。见图1
经Joinpoint回归分析,1990—2019年中国1型DKD全人群和女性年龄标准化患病率变化没有转折点,APC=AAPC,均呈上升趋势。其中,全人群整体年均增长1.02%(95%CI:0.87%~1.17%),女性整体年均增长1.21%(95%CI:1.08%~1.35%)。男性年龄标准化患病率变化转折点为1994、2011和2017年;1990—1994年呈下降趋势(95%CI:-3.37%~-0.95%),1994—2011年呈上升趋势(95%CI:1.37%~1.67%),2011—2017年呈下降趋势(95%CI:-2.46%~-0.74%),2017—2019年呈上升趋势(95%CI:0.57%~8.77%);整体上年均增长0.56%(AAPC=0.56%)。见图2
<20岁年龄组的1型DKD患病率在1990—2019年以年均-0.71%~-0.12%的速度下降,≥20岁年龄组以年均1.06%~3.99%的速度上升,差异均具有统计学意义(P<0.05)。见表1
1990—2019年中国2型DKD全人群、男性和女性年龄标准化患病率趋势变化转折点均为1995、2000、2005、2010和2015年。
全人群1990—1995年(95%CI:-0.37%~-0.27%)、1995—2000年(95%CI:-1.24%~-1.09%)、2005—2010年(95%CI:-0.42%~-0.28%)、2015—2019年(95%CI:-1.62%~-1.47%)均呈下降趋势;2000—2005年(95%CI:0.85%~1.00%)、2010—2015年(95%CI:1.24%~1.39%)均呈上升趋势;整体年均下降0.15%(AAPC=-0.15%)。
男性1990—1995年(95%CI:-0.24%~-0.09%)、1995—2000年(95%CI:-1.33%~-1.12%)、2005—2010年(95%CI:-0.46%~-0.25%)、2015—2019年(95%CI:-1.28%~-1.08%)均呈下降趋势;2000—2005年(95%CI:0.83%~1.03%)、2010—2015年(95%CI:0.96%~1.17%)均呈上升趋势;整体年均下降0.12%(AAPC=-0.12%)。
女性1990—1995年(95%CI:-0.53%~-0.38%)、1995—2000年(95%CI:-1.22%~-1.01%)、2005—2010年(95%CI:-0.47%~-0.25%)、2015—2019年(95%CI:-1.98%~-1.76%)均呈下降趋势;2000—2005年(95%CI:0.78%~1.00%)、2010—2015年(95%CI:1.40%~1.62%)均呈上升趋势;整体年均下降0.18%(AAPC=-0.18%)。见图3
2型DKD主要发生在≥15岁年龄组人群;20~39岁各年龄组在1990—2019年间以年均-1.59%~-0.36%之间的速度下降,差异均具有统计学意义(P<0.001)。15~19岁、55~59岁、≥75岁以上年龄组以年均0.05%~0.30%之间的速度上升,差异均具有统计学意义(P<0.001)。其余年龄组在1990—2019年间的变化趋势均无统计学意义(P>0.05)。见表1
DKD是常见的糖尿病微血管病变并发症之一。高血糖是DKD发展的主要驱动力,DKD主要防治目标是防止DKD的发生和发展[13]
1990—2019年中国1型DKD女性年龄标准化患病率变化没有转折点,APC=AAPC>0,整体年均增长1.21%。这持续增长在女性中涉及多方面因素。家族中有T1DM或DKD病史,她们患病风险可能会增加。自身免疫可能导致胰岛β细胞受损而引发DKD;自身免疫异常可能在女性中更为常见,导致DKD患病率增长。女性在生育年龄期间,由于雌激素等激素变化,可能会影响代谢和血糖控制;激素波动可能会增加女性患1型DKD风险。这些因素可能相互交织,共同影响1型DKD在女性中的患病率[14]。因此,预防和治疗1型DKD需要综合考虑多方面因素。
1990—2019年中国1型DKD男性年龄标准化患病率变化转折点为1994、2011和2017年,整体年均增长0.56%。其增长呈“波动下降上升”的原因可能涉及多方面。医学知识和诊断技术的更新,医生对其诊断能力增强;这可导致早期发现和诊断的病例增多,使得肾病增长率上升。也可能是生活方式和饮食习惯的变化,男性在应酬、聚餐时摄入高糖、高脂肪、高盐饮食以及缺乏运动等不良生活习惯更容易并发高血压、高血脂等心血管疾病增加糖尿病的风险,这些疾病都可能对肾脏造成损害,导致肾病的发生和发展进而增加DKD发生率[15]。不同地区和医疗机构对1型DKD的治疗和管理水平存在差异。因此,对于1型DKD的预防和治疗需要综合考虑多个因素,采取综合性措施。
目前1型DKD主要好发于儿童或青少年,也发生在各个年龄段[16]。1990—2019年中国1型DKD<20岁年龄组各段均APC<0,AAPC<0,整体呈变化下降;≥20岁以上年龄组各段均APC>0,AAPC>0,整体呈变化增长。目前中国1型DKD在儿童或青少年有下降趋势,可能是人们认识的提高,在世界糖尿病基金会和中华医学会儿科学分会内分泌遗传代谢学组的努力下,我国已建立了多个儿童糖尿病管理中心并进行了规范管理和治疗,从而延缓1型DKD发生[17]。但近年来≥20岁以上各个年龄段呈增长趋势,随着人口老龄化,≥20岁的人群比例增加,而年龄增长本身就是糖尿病和DKD风险因素之一;因此对成人1型DKD加以关注,及时做出相应的对策。
1990—2019年中国2型DKD全人群、男性和女性年龄标准化患病率趋势变化转折点均为1995、2000、2005、2010和2015年,呈波动下降上升趋势,结果提示中国2型DKD防控形势依然十分严峻。全人群患病率整体年均增长1.21%,可能是随着社会的发展,健康意识的提升,生活的改善等有助于人们对2型DKD的认知与控制。有研究显示,高血糖可以通过对肾脏生理功能等途径对肾脏造成损伤[18]。若血糖控制效果不佳,并发症可能会提早,甚至会直接影响患者的生存时间。近年来由于人们嗜食厚味、食无节制,熬夜、少运动等的不良习惯,DKD患病率也不断增加[19]。因此对我国糖尿病及其并发症的患者加大重视,开展有效的健康宣教,提高患者的认识,及早诊断和干预,预防更严重的并发症发生。
在性别上,1990—2019年中国2型DKD女性总体年龄标准化患病率高于男性,均AAPC<0。关于DKD是否与性别存在关联,至今国内外的研究还未有明确定论。有研究表明糖尿病患者中,60岁以下男性DKD患病率高于女性,而65岁以上性别没有显著性差异[20]。有的认为基于我国社会特点,男性更容易存在不良饮食习惯和不规律生活作息,更可能出现DKD。可能病程的进展,患者会因社会环境影响,如糖尿病及其并发症的健康教育、饮食控制、药物治疗等,且随着年龄增长,应酬性社交较少,所以性别差异性也会减少[18]。根据本研究性别结果显示,可能与近年鼓励生育政策有关,女性生育期生理功能激素紊乱引起改变,难以在后期逆转高血糖的状态,以致疾病进一步进展;也可能是女性喜欢摄入奶茶、甜食等高糖不良饮食习惯有关,仍需进一步探究。
1990—2019年中国2型DKD<15岁年龄组整体水平趋势基本无变化,主要发生在≥15岁以上年龄组。15~19岁年龄组APC>0,AAPC>0,呈上升趋势;从青少年到成人的转变,可能学业压力大,身体生理变化大,不良生活习惯等导致肥胖症也多见,出现糖耐量异常甚至是糖尿病在该年龄段的可能性变大。20~39岁年龄组APC<0,AAPC<0,呈下降趋势;可能是健康中国行建设让大家的健康意识越来越强,人们逐渐追求健康的生活方式,使得该年龄段的患病有所下降。40~49岁各年龄组APC<0,AAPC<0,呈下降趋势;而50~54岁和60~74岁各年龄组APC>0,AAPC>0,呈上升趋势(P>0.05),可能与不同年龄段糖尿病管理情况有关。40~49岁年龄段的人群可能较早接触糖尿病相关信息,对糖尿病及其并发症的认识更为深入,疾病管理和预防方面做得更好,导致DKD患病率相对较低。相比之下,50~54岁和60~74岁年龄段人群可能由于生活方式、慢性疾病等导致糖尿病管理不佳,从而增加了患DKD风险。其次,这种趋势也可能与不同年龄段人群的身体状况有关。随着年龄增长,人体各项功能会逐渐衰退,包括胰岛功能和肾功能。近30年来中国2型DKD55~59岁和≥75岁以上年龄组APC>0,AAPC>0,呈上升趋势,可能随着人口老龄化加速有关,年龄大、基础疾病多,且衰老是一个自然的、不可避免的、渐进的过程,随着年龄的增长,人体组织和器官功能结构会发生相应的变化,而肾脏是与年龄相关组织损伤的典型靶器官[21]。有些学者研究发现,DKD的发生与患者的年龄、病程具有显著性的关系,目前临床研究表明,随着患者的病程延长,年龄增加,患者出现DKD概率就越高,这与患者病情进展、身体自身机能的退化等因素密不可分[22]。因为患者病程、年龄为不可逆转以及不可控因素,所以患者应尽早采取防治措施,延缓DKD的进展,推迟发病年龄等。国内外多项研究也表明,通过有效的血糖控制可以极大地延缓DKD出现[23-25]。因此,针对不同年龄段人群的特点和需求,制定个性化的糖尿病管理和预防策略,以降低DKD等并发症的风险。在各个年龄段特别是年龄越大的人群,增强对疾病的意识,以期人们能得到早期诊断、有效的行为改变,及早预防措施,从而改善高血糖患者的生活质量,延缓DKD的发生有关。
本研究存在一定的局限性。本研究主要是从GBD2019官方网站获取并下载数据,1990—2019年的所有数据整理、分析在不同时期诊断标准不一,可能数据存在差异性;而且本研究未能获取中国各省份、各地区的详细数据,并未能进一步探讨其之间存在的差异,所以只将数据依据年份和年龄分组做出趋势性分析,未能进行病因学分析和归因分析。
